Aim: Metamizole (dipyrone) is a prodrug not detectable in serum or urine after oral ingestion. The primary metabolite is 4-methylaminoantipyrine (4-MAA), which can be N-demethylated to 4-aminoantipyrine (4-AA) or oxidized to 4-formylaminoantipyrine (4-FAA) by cytochrome P450 (CYP)-dependent reactions. Our aim was to identify the CYPs involved. Methods: We investigated the metabolism of 4-MAA in vitro using CYP expressing supersomes and the pharmacokinetics of metamizole in the presence of CYP inhibitors in healthy volunteers. Results: The experiments in supersomes revealed CYP1A2 as the major CYP for 4-MAA N-demethylation and 4-FAA formation. CYP2C19 and CYP2D6 contributed to N-demethylation but not to FAA formation. In the subsequent clinical study, we investigated the influence of ciprofloxacin (strong CYP1A2 inhibitor), fluconazole (strong CYP2C19 inhibitor) and the combination ciprofloxacin/fluconazole on the pharmacokinetics of a single dose of metamizole in n=12 healthy volunteers in a randomized, placebo-controlled, double-blind study. Both ciprofloxacin and fluconazole inhibited the metabolism of 4-MAA, confirming the in vitro results. Ciprofloxacin, fluconazole and ciprofloxacin/fluconazole increased the AUC0-12h of 4-MAA by 51%, 17% and 92%, respectively. Ciprofloxacin, fluconazole and ciprofloxacin/fluconazole decreased the AUC0-12h of 4-AA by 27%, 12% and 24%, respectively, and of 4-FAA by 33%, 9% and 51%, respectively. Ciprofloxacin, fluconazole and ciprofloxacin/fluconazole increased the half-life of 4-MAA from 3.22 h (placebo) to 3.91, 3.69 and 6.07 h, respectively. Conclusion: CYP1A2 is the major CYP for the conversion of 4-MAA to 4-AA and 4-FAA. CYP1A2 inhibition increases the 4-MAA exposure by a factor of approximately 1.5, which could be relevant for dose-dependent adverse reactions.

The C-X-C chemokine receptor 7 (CXCR7) has evolved as a promising, druggable target mainly in the immunology and oncology fields modulating plasma concentrations of its ligands CXCL11 and CXCL12 through receptor-mediated internalization. This "scavenging" activity creates concentration gradients of these ligands between blood vessels and tissues that drive directional cell migration. This randomized, double-blind, placebo-controlled first-in-human study assessed the safety, tolerability, pharmacokinetics, and pharmacodynamics of ACT-1004-1239, a first-in-class drug candidate small-molecule CXCR7 antagonist. Food effect and absolute bioavailability assessments were also integrated in this multipurpose study. Healthy male subjects received single ascending oral doses of ACT-1004-1239 (n = 36) or placebo (n = 12). At each of six dose levels (1-200 mg), repeated blood sampling was done over 144 hours for pharmacokinetic/pharmacodynamic assessments using CXCL11 and CXCL12 as biomarkers of target engagement. ACT-1004-1239 was safe and well tolerated up to the highest tested dose of 200 mg. CXCL12 plasma concentrations dose-dependently increased and more than doubled compared with baseline, indicating target engagement, whereas CXCL11 concentrations remained unchanged. An indirect-response pharmacokinetic/pharmacodynamic model well described the relationship between ACT-1004-1239 and CXCL12 concentrations across the full dose range, supporting once-daily dosing for future clinical studies. At doses ≥ 10 mg, time to reach maximum plasma concentration ranged from 1.3 to 3.0 hours and terminal elimination half-life from 17.8 to 23.6 hours. The exposure increase across the dose range was essentially dose-proportional and no relevant food effect on pharmacokinetics was determined. The absolute bioavailability was 53.0% based on radioactivity data after oral vs. intravenous 14 C-radiolabeled microtracer administration of ACT-1004-1239. Overall, these comprehensive data support further clinical development of ACT-1004-1239.

9-Br-closo-1,2-C2 B10 H11 has been prepared and its gas-phase structure has been examined by means of gas electron diffraction. The structure of the carbaborane core is similar to the structure of the parent compound, which is of C2v symmetry. A DFT-based search for the corresponding reaction pathway of the bromination of closo-1,2-C2 B10 H12 revealed that the catalytic amount of aluminum reduces the barrier of the initial attack of the bromination agent toward the negatively charged part of the icosahedral carbaborane, i. e., the first transition state, from about 40 to about 27 kcalmol-1 . The Br-Br bond is weakened by an intermediate binding to the large π-hole on the aluminum atom of AlBr3 , which is the driving force for the AlBr3 -catalyzed bromination.

Clinical trials have been performed mainly in adults and accordingly the necessary information is lacking for pediatric patients, especially regarding dosage recommendation for approved drugs. This gap in information could be filled with results from pharmacokinetic (PK) modeling, based on data collected in daily clinical routine. In order to make this data accessible and usable for research, the Swiss Pharmacokinetics Clinical Data Warehouse (SwissPKcdw) project has been set up, including a clinical data warehouse (CDW) and the regulatory framework for data transfer and use within. Embedded into the secure BioMedIT network, the CDW can connect to various data providers and researchers in order to collaborate on the data securely. Due to its modularity, partially containerized deployment and open‐source software, each of the components can be extended, modified, and re‐used for similar projects that require integrated data management, data analysis, and web tools in a secure scientific data and information technology (IT) environment. Here, we describe a collaborative and interprofessional effort to implement the aforementioned infrastructure between several partners from medical health care and academia. Furthermore, we describe a real‐world use case where blood samples from pediatric patients were analyzed for the presence of genetic polymorphisms and the results were aggregated and further analyzed together with the health‐related patient data in the SwissPKcdw.

The first clinically relevant reports of preparations of St. John's wort (SJW), a herbal medicine with anti-depressant effects, interacting with other drugs, altering their bioavailability and efficacy, were published about 20 years ago. In 2000, a pharmacokinetic interaction between SJW and cyclosporine caused acute rejection in two heart transplant patients. Since then, subsequent research has shown that SJW altered the pharmacokinetics of drugs such as digoxin, tacrolimus, indinavir, warfarin, alprazolam, simvastatin, or oral contraceptives. These interactions were caused by pregnane-X-receptor (PXR) activation. Preparations of SJW are potent activators of PXR and hence inducers of cytochrome P450 enzymes (most importantly CYP3A4) and P-glycoprotein. The degree of CYP3A4 induction correlates significantly with the hyperforin content in the preparation. Twenty years after the first occurrence of clinically relevant pharmacokinetic drug interactions with SJW, this review revisits the current knowledge of the mechanisms of action and on how pharmacokinetic drug interactions with SJW could be avoided. An improved synthesis of the simplest nitric acid ester, methyl nitrate, and a new synthesis of fluoromethyl nitrate use the metathesis of the corresponding iodomethanes with silver nitrate. Both compounds were identified by spectroscopy and the structures determined for in situ grown crystals by X‐ray diffraction as well as in the gas phase by electron diffraction. Fluorination leads to structures with shorter C−O and N−O bonds, has an energetically destabilizing effect and increases friction sensitivity, but decreases detonation performance., Six centuries after the discovery of the simplest nitric acid ester, methyl nitrate (H3CONO2), experimental solid‐state and gas‐phase structures as well as energetic properties are compared with those of the new monofluorine‐substituted analogue, fluoromethyl nitrate (FH2CONO2).

Small‐ to medium‐sized clusters occur in various areas of chemistry, for example, as active species of heterogeneous catalysis or as transient intermediates during chemical vapor deposition. The manipulation of stable representatives is mostly limited to the stabilizing ligand periphery, virtually excluding the systematic variation of the property‐determining cluster scaffold. We now report the deliberate expansion of a stable unsaturated silicon cluster from six to seven and finally eight vertices. The consecutive application of lithium/naphthalene as the reducing agent and decamethylsilicocene as the electrophilic source of silicon results in the expansion of the core by precisely one atom with the potential of infinite repetition.

AIMS: The anthelminthic ivermectin is receiving new attention as it is being repurposed for new indications such as mass drug administrations for the treatment of scabies or in malaria vector control. As its pharmacokinetics are still poorly understood, we aimed to characterize the population pharmacokinetics of ivermectin in plasma and dried blood spots (DBS), a sampling method better suited to field trials, with special focus on the influence of body composition and enterohepatic circulation. METHODS: We performed a clinical trial in 12 healthy volunteers who each received a single oral dose of 12 mg ivermectin, and collected peripheral venous and capillary DBS samples. We determined ivermectin concentrations in plasma and DBS by liquid chromatography tandem mass spectrometry using a fully automated and scalable extraction system for DBS sample processing. Pharmacokinetic data were analysed using non‐linear mixed effects modelling. RESULTS: A two‐compartment model with a transit absorption model, first‐order elimination, and weight as an influential covariate on central volume of distribution and clearance best described the data. The model estimates (inter‐individual variability) for a 70 kg subject were: apparent population clearance 7.7 (25%) l h(−1), and central and peripheral volumes of distribution 89 (10%) l and 234 (20%) l, respectively. Concentrations obtained from DBS samples were strongly linearly correlated (R (2) = 0.97) with plasma concentrations, and on average 30% lower. CONCLUSION: The model accurately depicts population pharmacokinetics of plasma and DBS concentrations over time for oral ivermectin. The proposed analytical workflow is scalable and applicable to the requirements of mass drug administrations.

Planar tridentate Lewis acids were synthesised by hydrosilylation reactions of triethynylbenzene with chlorosilanes. Hydrogenation of the rigid triethynylbenzene using hydrogen in the presence of Lindlar's catalyst resulted in the formation of trivinylbenzene. The hydrosilylation of this compound enabled the synthesis of flexible tridentate Lewis acids with SiClMe2 and SiCl2Me, SiCl3 groups. In order to increase the acidity, the chlorine substituents of the tridentate Lewis acids were substituted by fluorine and compounds with SiF2Me and SiF3 groups were obtained. The Lewis acidity of the isolated compounds was determined with the Gutmann-Beckett method. The results show the presence of cooperative effects of the silyl groups.

The reactivity of the frustrated Lewis pair (FLP) (F5C2)3SnCH2P(tBu)2 (1) was investigated with respect to the activation of elemental hydrogen. The reaction of 1 at elevated hydrogen pressure afforded the intramolecular phosphonium stannate(II) (F5C2)2SnCH2PH(tBu)2 (3). It was characterized by means of multinuclear NMR spectroscopy and single crystal X‐ray diffraction. NMR experiments with the two isotopologues H2 and D2 showed it to be formed via an H2 adduct (F5C2)3HSnCH2PH(tBu)2 (2) and the subsequent formal reductive elimination of pentafluoroethane; this is supported by DFT calculations. Parahydrogen‐induced polarization experiments revealed the formation of a second product of the reaction of 1 with H2, [HP(tBu)2Me][Sn(C2F5)3] (4), in 1H NMR spectra, whereas 2 was not detected due to its transient nature., Hydrogen splitting and two pathways of reductive elimination were found by reacting H2 with an intramolecular Sn/P frustrated Lewis pair; quantum‐chemical calculations and high‐pressure NMR experiments indicate an neutral and zwitterionic phosphonium stannate(II) as the energetically favored product of reaction.

Three 1,2diaryltetramethyldisilanes X5C6(SiMe2)2C6X5 with two C6H5, C6F5, or C6Cl5 groups were studied concerning the importance of London dispersion driven interactions between their aryl groups. They were prepared from 1,2dichlorotetramethyldisilane by salt elimination. Their structures were determined in the solid state by Xray diffraction and for free molecules by gas electrondiffraction. The solidstate structures of the fluorinated and chlorinated derivatives are dominated by arylaryl interactions. Unexpectedly, Cl5C6(SiMe2)2C6Cl5 exists exclusively as an eclipsed synconformer in the gas phase with strongly distorted SiC6Cl5 units due to strong intramolecular interactions. In contrast, F5C6(SiMe2)2C6F5 reveals weaker interactions. The contributions to the total interaction energy were analyzed by SAPT calculations. (VLID)4867022

The structures of the three para-substituted halotetrafluoropyridines with chlorine, bromine and iodine have been determined in the solid state (X-ray diffraction). The structures of these compounds and that of pentafluoropyridine were also determined in the gas phase (electron diffraction). Structures in the solid state of the bromine and iodine derivative exhibit halogen bonding as a structure deter-mining motif. On the way to an investigation on the formation of the halogen bond of halotetrafluoropyridines in the solid state with the stronger Lewis base, e.g. a better halogen-bond acceptor, pyridine, co-crystals of benzene adducts were investigated. These co-crystals show halogen bonding only for the two heavier halotetrafluoropyridi-nes. In the pyridine co-crystals halogen bonding was observed for all three para-halotetrafluoropyridines. The formation of homodimers and heterodimers with pyridine is also supported by quantum-chemical calculations of electron-density topologies and natural-bond orbitals. © 2019 WILEY-VCH Verlag GmbH & Co. OBJECTIVE: The FKBP5 gene codes for a co-chaperone that regulates glucocorticoid receptor sensitivity and thereby impacts the reactivity of the hypothalamic-pituitary-adrenal (HPA)-axis. Evidence suggested that subjects exposed to childhood abuse and carrying the TT genotype of the FKBP5 gene single nucleotide polymorphism (SNP) rs1360780 have an increased susceptibility to stress-related disorders. METHOD: The hypothesis that abused TT genotype carriers show changes in gray matter (GM) volumes in affect-processing brain areas was investigated. About 1,826 Caucasian subjects (age ≤ 65 years) from the general population [Study of Health in Pomerania (SHIP)] in Germany were investigated. The interaction between rs1360780 and child abuse (Childhood Trauma Questionnaire) and its effect on GM were analyzed. RESULTS: Voxel-based whole-brain interaction analysis revealed three large clusters (FWE-corrected) of reduced GM volumes comprising the bilateral insula, the superior and middle temporal gyrus, the bilateral hippocampus, the right amygdala, and the bilateral anterior cingulate cortex in abused TT carriers. These results were not confounded by major depressive disorders. In region of interest analyses, highly significant volume reductions in the right hippocampus/parahippocampus, the bilateral anterior and middle cingulate cortex, the insula, and the amygdala were confirmed in abused TT carriers compared with abused CT/CC carriers. CONCLUSION: The results supported the hypothesis that the FKBP5 rs1360780 TT genotype predisposes subjects who have experienced childhood abuse to widespread structural brain changes in the subcortical and cortical emotion-processing brain areas.
