Aims and objectives To examine international studies that specifically focus on transition to practice for graduate registered nurses in rural and remote areas. Background Supportive graduate nursing programmes are essential for enabling nursing graduates’ transition to practice and reducing attrition rates. Literature examining support measures for nursing graduates within metropolitan areas is abundant. However, there is a paucity of evidence on effective graduate programmes for rural and remote-based nursing graduates. Design A systematic approach was used to identify robust research within appropriate electronic databases. Method Eligible articles were critically reviewed using the Mixed Method Appraisal Tool critical appraisal tool. Eligible articles were thematically analysed using the Braun and Clark approach. Results Eight articles met the selection criteria for inclusion. Findings revealed that while most graduate nurses survived the transition process, they often felt overwhelmed and abandoned with intense feelings of frustration. Many suffered transition shock and did not feel ready for the role. Socialisation of graduates to the clinical environment was lacking. Support offered in many graduate programmes was ad hoc and unstructured. Senior staff were inadequately supported in their roles as preceptors to assist with the transition. Critical support measures recommended included both debrief sessions and regular one-on-one support. Conclusions Graduate programmes need to be structured yet flexible to accommodate the needs of rural and remote nurse graduates. Graduates need to be transitioned into practice with decremental support processes for both workloads and education. Preceptors require education on how to mentor before they can provide the appropriate support for graduates. Without these measures in place, a decrease in transition shock may not be possible. Relevance to clinical practice Graduate programmes need to be structured yet flexible, including assistance with both clinical skills and socialisation. Senior staff require education before they can adequately support new graduates.

Obesity is a major public health problem, which, like many forms of addiction, is associated with an elevated tendency to choose smaller immediate rather than larger delayed rewards, a response pattern often referred to as excessive delay discounting. Although some accounts of delay discounting conceptualize this process as impulsivity (placing the emphasis on overvaluing the smaller immediate reward), others have conceptualized delay discounting as an executive function (placing the emphasis on delayed rewards failing to retain their value). The present experiments used a popular animal model of obesity that has been shown to discount delayed rewards at elevated rates (i.e., obese Zucker rats) to test two predictions that conceptualize delay discounting as executive function. In the first experiment, acquisition of lever pressing with delayed rewards was compared in obese versus lean Zucker rats. Contrary to predictions based on delay discounting as executive function, obese Zucker rats learned to press the lever more quickly than controls. In the second experiment, progressive ratio breakpoints (a measure of reward efficacy) with delayed rewards were compared in obese versus lean Zucker rats. Contrary to the notion that obese rats fail to value delayed rewards, the obese Zucker rats’ breakpoints were (at least) as high as those of the lean Zucker rats.

The American Academy of Otolaryngology-Head and Neck Surgery Foundation has published a supplement to this issue of Otolaryngology-Head and Neck Surgery featuring the "Clinical Practice Guideline: Evaluation of the Neck Mass in Adults." To assist in implementing the guideline recommendations, this article summarizes the rationale, purpose, and key action statements. The 12 recommendations developed emphasize reducing delays in diagnosis of head and neck squamous cell carcinoma; promoting appropriate testing, including imaging, pathologic evaluation, and empiric medical therapies; reducing inappropriate testing; and promoting appropriate physical examination when cancer is suspected.

Parkinson's disease is a debilitating neurodegenerative condition for which there is no cure. Converging evidence implicates gangliosides in the pathogenesis of several neurodegenerative diseases, suggesting a potential new class of therapeutic targets. We have shown that interventions that simultaneously increase the neuroprotective GM1 ganglioside and decrease the pro-apoptotic GD3 ganglioside - such as inhibition of GD3 synthase (GD3S) or administration of sialidase - are neuroprotective in vitro and in a number of preclinical models. In this study, we investigated the effects of GD3S deletion on parkinsonism induced by 1-methyl-4phenyl-1,2,3,6-tetrahydropyridine (MPTP). MPTP was administered to GD3S-/- mice or controls using a subchronic regimen consisting of three series of low-dose injections (11 mg/kg/day × 5 days each, 3 weeks apart), and motor function was assessed after each. The typical battery of tests used to assess parkinsonism failed to detect deficits in MPTP-treated mice. More sensitive measures - such as the force-plate actimeter and treadmill gait parameters - detected subtle effects of MPTP, some of which were absent in mice lacking GD3S. In wild-type mice, MPTP destroyed 53% of the tyrosine-hydroxylase (TH)-positive neurons in the substantia nigra pars compacta (SNc) and reduced striatal dopamine 60.7%. In contrast, lesion size was only 22.5% in GD3S-/- mice and striatal dopamine was reduced by 37.2%. Stereological counts of Nissl-positive SNc neurons that did not express TH suggest that neuroprotection was complete but TH expression was suppressed in some cells. These results show that inhibition of GD3S has neuroprotective properties in the MPTP model and may warrant further investigation as a therapeutic target.

Purpose In the lungs of cystic fibrosis patients, Pseudomonas aeruginosa is exposed to a myriad of antibiotics leading to alterations in antibiotic susceptibility. This study identifies mutations resulting in hypersusceptibility in isogenic mutants of a P. aeruginosa clinical isolate, PA34. Methods PA34 was exposed to subinhibitory concentrations of doripenem or meropenem during growth to mid‐log phase. Antibiotic susceptibility of surviving colonies was determined by agar dilution. Two carbapenem‐resistant colonies hypersusceptible to non‐carbapenem antibiotics were selected for further analysis. Antibiotic resistance gene expression was evaluated by RT‐rtPCR and OprD production by SDS‐PAGE. PA34 and isogenic mutants were evaluated with whole genome sequencing. Sequence variants were confirmed by Sanger sequencing, and cognate genes in eight carbapenem‐resistant clinical isolates hypersusceptible to non‐carbapenem antibiotics were sequenced. Lipopolysaccharide preparations of PA34 and hypersusceptible mutants were evaluated with ProQ‐Emerald stain. Results Isogenic mutants showed 4‐ to 8‐fold MIC increase for imipenem, meropenem, and doripenem. However, they were hypersusceptible (≥4‐fold MIC decrease) to aminoglycosides, fluoroquinolones, and non‐carbapenem β‐lactams. Expression of ampC or mex‐opr efflux pumps was unchanged, but OprD production was decreased. Mutations causing Q86H AlgU and G77C LptG amino acid substitutions and nonsense mutations within OprD were observed in both mutants. Lipopolysaccharide modifications were observed between isogenic mutants and PA34. Non‐synonymous mutations in LptF or LptG were observed in 6/8 hypersusceptible clinical isolates resistant to carbapenem antibiotics. Conclusion Evaluation of hypersusceptible mutants identified the association between lptG and a hypersusceptible phenotype. Modifications in lipopolysaccharide profiles suggests LptG modification interferes with lipopolysaccharide transport and contributes to hypersusceptibility., This study identifies mutations resulting in hypersusceptibility in isogenic mutants and clinical isolates of Pseudomonas aeruginosa. Mutations in the gene encoding the LptG subunit of the LptFG heterodimer were found to alter lipopolysaccharide (LPS) profiles in isogenic mutants. Modifications in LPS profiles suggest LptG modification interferes with LPS transport and contributes to hypersusceptibility.

Background and purpose The D1CT-7 mouse is one of the best known animal models of Tourette syndrome (TS), featuring spontaneous tic-like behaviours sensitive to standard TS therapies; these characteristics ensure a high face and predictive validity of this model, yet its construct validity remains elusive. To address this issue, we studied the responses of D1CT-7 mice to two critical components of TS pathophysiology: the exacerbation of tic-like behaviours in response to stress and the presence of sensorimotor gating deficits, which are thought to reflect the perceptual alterations causing the tics. Experimental approach D1CT-7 and wild-type (WT) littermates were subjected to a 20 min session of spatial confinement (SC) within an inescapable, 10 cm wide cylindrical enclosure. Changes in plasma corticosterone levels, tic-like behaviours and other spontaneous responses were measured. SC-exposed mice were also tested for the prepulse inhibition (PPI) of the startle response (a sensorimotor gating index) and other TS-related behaviours, including open-field locomotion, novel object exploration and social interaction and compared with non-confined counterparts. Key results SC produced a marked increase in corticosterone concentrations in both D1CT-7 and WT mice. In D1CT-7, but not WT mice, SC exacerbated tic-like and digging behaviours, and triggered PPI deficits and aggressive responses. Conversely, SC did not modify locomotor activity or novel object exploration in D1CT-7 mice. Both tic-like behaviours and PPI impairments in SC-exposed D1CT-7 mice were inhibited by standard TS therapies and D1 dopamine receptor antagonism. Conclusions and implications These findings collectively support the translational and construct validity of D1CT-7 mice with respect to TS. The reproducibility of experimental outcomes depends on consistent control of independent variables. In food-maintained operant performance, it is of utmost importance that the quantity of food delivered is reliable. To that end, some commercial food pellet dispensers have add-on attachments to sense the delivery of pellets. Not all companies, however, offer such add-ons. Aside from availability, cost and temporary reduction in throughput may be a problem for smaller labs. The present paper outlines our recent development of a simple, inexpensive infrared device to detect and confirm the delivery of pellets. The in-line construction of the detector routes the falling pellet through a barrel so that it passes between an infrared emitter and receiver. The circuitry was designed to be compatible with all commercially available behavioral measurement systems, and so may be retrofit to any existing system. Our tests with the detector so far have shown that it is 100% accurate in detecting pellet delivery. The individual unit cost is approximately 25 dollars. The low cost and versatility of the device offer an easy method to ensure the integrity of food delivery in operant settings.

Double-mutant mice (DKO) lacking the two voltage-gated K(+) channels Kv3.1 and Kv3.3 display a series of phenotypic alterations that include ataxia, myoclonus, tremor and alcohol hypersensitivity. The prominent cerebellar expression of mRNAs encoding Kv3.1 and Kv3.3 subunits raised the question as to whether altered electrical activity resulting from the lack of these K(+) channels might be related to the dramatic motor changes. We used the tremorogenic agent harmaline to probe mutant mice lacking different K(+) channel alleles for altered olivocerebellar circuit properties. Harmaline induced the characteristic 13-Hz tremor in wildtype mice (WT); however, no tremor was observed in DKO suggesting that the ensemble properties of the olivocerebellar circuitry are altered in the absence of Kv3.1 and Kv3.3 subunits. Harmaline induced tremor in Kv3.1-single mutants, but it was of smaller amplitude and at a lower frequency indicating the participation of Kv3.1 subunits in normal olivocerebellar system function. In contrast, harmaline tremor was virtually absent in Kv3.3-single mutants indicating an essential role for Kv3.3 subunits in tremor induction by harmaline. Immunohistochemical staining for Kv3.3 showed clear expression in the somata and proximal dendrites of Purkinje cells and in their axonal projections to the deep cerebellar nuclei (DCN). In DCN, both Kv3.1 and Kv3.3 subunits are expressed. Action potential duration is increased by approximately 100% in Purkinje cells from Kv3.3-single mutants compared to WT or Kv3.1-single mutants. We conclude that Kv3.3 channel subunits are essential for the olivocerebellar system to generate and sustain normal harmaline tremor whereas Kv3.1 subunits influence tremor amplitude and frequency.

BACKGROUND Serous components within endometrial carcinoma are reportedly poor prognosticators. However, to the authors' knowledge the percentage of tumors which must be comprised of a serous component in order to affect outcome is unknown. The authors compared overall survival (OS) in women with endometrial carcinomas comprised of various percentages of uterine serous carcinoma (USC) with that of women with International Federation of Gynecology and Obstetrics (FIGO) Grade 3 endometrioid carcinoma (G3EC) to determine whether outcomes varied between these two poorly differentiated histologies. METHODS Data concerning women with either G3EC or USC who were diagnosed between January 1990 and November 2000 were collected retrospectively. Cases were reviewed to confirm diagnosis and estimate the fraction of tumor comprised of USC. Variables assessed included patient age and race, tumor stage, and lymphovascular space invasion. Associations between variables were tested using the Fisher exact test. The Kaplan–Meier method was used to evaluate OS with comparisons performed using the log-rank test. RESULTS Fifty-two women with G3EC and 87 women with USC were identified. The OS of women with tumors comprised of > 50% USC was found to be significantly worse compared with women with G3EC (hazard ratio [HR] of 2.4; 95% confidence interval [95% CI], 1.2–5.2). Women with USC were more likely to present with extrauterine disease (odds ratio of 2.2; 95% CI, 1.1–4.5). The 5-year survival rate for women with G3EC was 75% compared with 41% for women with tumors that were > 50% USC (P = 0.01). There was a significant trend toward a worse OS in women with even 10% USC compared with women with G3EC. CONCLUSIONS USC involving > 50% of an endometrial carcinoma was found to be predictive of worse OS compared with the OS of women with G3EC. In patients with early-stage disease, a trend toward a worse prognosis was found to exist when USC comprised even 10% of a tumor. Investigation into the treatment of endometrial carcinoma should include and document tumors with any percentage comprised of USC. Cancer 2004. © 2004 American Cancer Society.

The relationship between step reductions in inspired oxygen and the amplitude of evoked field excitatory postsynaptic potentials (fEPSPs) recorded from hippocampal CA1 neurons was examined in anaesthetized rats with a unilateral common carotid artery occlusion. The amplitudes of fEPSPs recorded from the hippocampus ipsilateral to the occlusion were significantly more depressed with hypoxia than were the fEPSPs recorded from the contralateral hippocampus. The adenosine A1-selective antagonist, 8-cyclopentyl-1,3-dimethylxanthine (8-CPT), blunted the hypoxic depression of the fEPSP. Tissue partial pressure of oxygen (Ptiss,O2) was measured in the ipsilateral and contralateral hippocampus using glass Clark-style microelectrodes. Ptiss,O2 fell to similar levels as a function of inspired oxygen in the ipsilateral and contralateral hippocampus, and in the ipsilateral hippocampus after administration of 8-CPT. Hippocampal blood flow (HBF) was measured using laser Doppler flowmetry. A decline in HBF was associated with systemic hypoxia in both hippocampi. HBF, as a function of inspired oxygen, fell significantly more in the ipsilateral than in the contralateral hippocampus. We conclude that endogenous adenosine acting at the neuronal A1 receptor plays a major role in the depression of synaptic transmission during hypoxic ischaemia. The greater susceptibility of the fEPSP in the ipsilateral hippocampus to systemic hypoxia cannot be explained entirely by differences in Ptiss,O2 or HBF between the two hemispheres.

Transgenic mice overexpressing neurotrophin-3 (NT-3) in skeletal muscle (mlc/NT-3 mice) develop abnormal muscle spindles in skeletal muscle and display abnormal motor function in the form of gait and locomotive disturbances. The purpose of this work was to characterize the functional consequences of NT-3 overexpression in skeletal muscle with further behavioral assessments that permitted inferences about muscle weakness in the tongue or forelimbs as well as potential central nervous system (CNS) abnormalities compared to wild-type controls. Wild-type (n=12) and mlc/NT-3 (n=12) male mice were tested in five procedures (in chronological order): lick dynamics, locomotor activity, grid ataxia, go-no-go discrimination procedure, and grip strength. Relative to wild-type mice, the mlc/NT-3 mice exhibited lower tongue force, hyperactivity, slowed limb retrieval in the grid ataxia test, similar discrimination performance, and lower grip strength. Overall, the data suggest that chronically elevated levels of NT-3 in mouse skeletal muscle cause muscle weakness in the mlc/NT-3 mice. Surprisingly, mlc/NT-3 mice also exhibited significant hyperactivity, suggesting that NT-3 overexpression in the periphery may have caused abnormalities in the CNS that are related to the cortical processing of proprioceptive afferent information.

We found up to 10 ppm wet weight of selenium in blood samples collected from emperor geese (Chen canagica) on their breeding grounds on the Yukon-Kuskokwim Delta in western Alaska, USA. Incubating adult females captured in late May through mid-June 1997 had significantly higher concentrations of selenium in their blood (mean = 5.60 ppm) than adult females captured during wing molt in late July 1996 (mean = 2.78 ppm). Females that nested early or were in good body condition had higher concentrations of selenium in their blood than did other nesting females. Blood samples from 4 of 29 goslings had detectable levels of selenium (mean = 0.14 ppm). Our findings suggest that emperor geese are exposed to more selenium in the marine environment of their wintering and staging areas on the Alaska Peninsula than on the breeding grounds. The highest concentration of mercury found in the blood of emperor geese was 0.24 ppm. One bird had a blood lead concentration of 0.67 ppm, but 82% had no detectable lead in their blood, suggesting that lead exposure from the ingestion of lead shot poses little threat for emperor geese in western Alaska, contrary to findings reported for sympatric spectacled eiders (Somateria fischeri).

Despite having been first marketed in the 1950s, new designs of progressive addition spectacle lens continue to appear. Some of the recent patent literature is reviewed on the design of such lenses. As well as a number of improvements to general purpose designs, specifically to include aspheric surfaces for the distance portion of progressive lenses, the literature includes a recent patent on an improved version of the Alvarez lateral translation lens system. The optimisation of single vision lens forms in order to extend the depth of field for early presbyopes is also discussed.

Previous research suggests that brain oxidative stress and altered rodent locomotor behavior are linked. We observed bio-behavioral changes in methionine sulfoxide reductase A knockout mice associated with abnormal dopamine signaling. Compromised ability of these knockout mice to reduce methionine sulfoxide enhances accumulation of sulfoxides in proteins. We examined the dopamine D(2)-receptor function and expression, which has an atypical arrangement and quantity of methionine residues. Indeed, protein expression levels of dopamine D(2)-receptor were higher in knockout mice compared with wild-type. However, the binding of dopamine D(2)-receptor agonist was compromised in the same fractions of knockout mice. Coupling efficiency of dopamine D(2)-receptors to G-proteins was also significantly reduced in knockout mice, supporting the compromised agonist binding. Furthermore, pre-synaptic dopamine release in knockout striatal sections was less responsive than control sections to dopamine D(2)-receptor ligands. Behaviorally, the locomotor activity of knockout mice was less responsive to the inhibitory effect of quinpirole than wild-type mice. Involvement of specific methionine residue oxidation in the dopamine D(2)-receptor third intracellular loop is suggested by in vitro studies. We conclude that ablation of methionine sulfoxide reductase can affect dopamine signaling through altering dopamine D(2)-receptor physiology and may be related to symptoms associated with neurological disorders and diseases.

We studied the effect of a severe drought on the population dynamics and community structure of grassland birds in western North Dakota. During the spring and summer of 1988 the northern Great Plains suffered one of the warmest, driest periods in its recorded history. We compared the changes in bird populations and nesting productivity over a 3-yr period before, during, and after the drought. Total grassland bird density declined 61% (P < .05) between June 1987 and June 1988. Densities of six of eight common species declined significantly during the drought. Populations of all but two species recovered in 1989 and total bird density in June 1989 did not differ significantly from June 1987. Species richness and species diversity both declined significantly during the drought and recovered to predrought levels in 1989. Species richness declined more on fair condition than on good condition range during the drought. Vesper Sparrow (Pooecetes gramineus) hatching success, number of young fledged per successful nest, and nesting success were significantly lower in 1988 than either 1987 or 1989. Clutch size did not differ among the three years. The decline in nesting success in 1988 was primarily due to nest abandonment during incubation. Nesting of Vesper Spar- rows, Horned Larks (Eremophila alpestris), and Western Meadowlarks (Sturnella neglecta) ended abruptly in mid-June 1988 during a period of extremely hot weather. In 1987 and 1989, nesting continued into July. Despite substantial reductions in bird density and productivity during the drought, many species recovered to predrought levels 1 yr following the drought. This suggests that year-to-year fluctuations in densities of some of these species may not be tightly linked to short-term changes in local productivity. However, sequential years of low productivity may have more substantial effects on these short-lived species. Thus, if drought conditions in North American grasslands become more frequent, as some climate models predict, there could be related changes in the avifauna of the region.

We experimentally studied the effects of avian predation on grasshopper abundance in western North Dakota during the summers of 1988 and 1989. Grasshopper densities in 15 x 15 m plots from which birds were excluded (NO BIRDS) were compared with similar-sized plots where birds were allowed to forage (BIRDS). Plots were established in early June at 16 sites (8 per year), and grasshopper densities were estimated from hoop counts in NO BIRDS and BIRDS plots at 2-wk intervals until the end of July. There were significantly more grasshoppers in NO BIRDS plots than in BIRDS plots in 1989 (P .388). After the final hoop count each year, sweep-net sampling was also used to estimate grasshopper densities. By this method, average grasshopper density in late July was 26% and 37% lower in BIRDS plots than in NO BIRDS plots in 1988 and 1989, respectively. Average length, total biomass of grasshoppers, species richness, and species diversity, however, did not differ between the treatments. In 1988, 2 of 15 grasshopper species were significantly more abundant in the NO BIRDS plots. There was no difference between the treatments among 16 species identified in the 1989 samples, but the power of our tests to detect differences for individual species was low. Our results support the hypothesis that avian predation reduces insect populations at low and moderate densities.

Between 1977 and 1985, the Gynecologic Oncology Group (GOG) conducted three clinical trials in locally advanced carcinoma of the cervix, clinical Stages I to IVA as classified by the International Federation of Gynecology and Obstetrics (FIGO). All 626 patients had primary carcinoma of the cervix and underwent operative assessment of the para-aortic (PA) lymph nodes. Patients received standardized external radiation therapy to the pelvis or to the pelvis and PA lymph nodes followed by one or two brachytherapy applications. To date, no statistically significant differences in progression-free interval (PFI) or survival time have been identified between the randomization treatment arms on any of these studies. Basic similarities among these studies led us to pool these data to identify patient characterisitcs and tumor characteristics associated with an increased risk of treatment failure. Multi-variate analysis showed patient age, performance status (PS), PA lymph node status, tumor size, and pelvic node status to be significantly associated with PFI. When modeling for survival, all these factors and clinical stage and bilateral extension were significant.

The connection between low-dimensional chaos in ordinary differential equations, and turbulence in fluids and other systems governed by partial differential equations, is one that is in many circumstances not clear. We discuss some examples of turbulent fluid flow, and consider ways in which they may be related to much simpler sets of ordinary differential equations, whose behavior can be reasonably well understood. (We are not advocating drastic Fourier truncation.) The generation of aperiodic solutions through the occurrence of homoclinic orbits is briefly analysed for ordinary differential equations, and the same kind of heuristic analysis is sketched for partial differential equations (in one space dimension). It is suggested that such an analysis can explain certain features of chaos, which have been observed in real fluids.

Five multifactor models, in both orthogonal and oblique versions, and a single-factor model of the WAIS-R's factor pattern were examined by confirmatory maximum likelihood factor analyses of a data matrix constructed from the results for 90 neuropsychiatric patients. None of the models fits the data matrix in an absolute sense, even though all of the models represented an improvement over a null statistical model. For the multifactor models, the best results were obtained by oblique solutions, in which the degree of correlation between the factors varied from .71 to .93. The single-factor model fit nearly as well as, and in some instances better than, many of the multifactor models. The best-fitting model, viz., a three-factor oblique one, was only marginally better in its fit than two of the three two-factor models or the competing three-factor model.

Acute neuroleptic administration results in within-session decrements for conditioned active avoidance, discriminated lever release (reaction time), fixed-ratio responding, lever pressing for intracranial electrical stimulation, licking, runway running, and treadmill running. Similarly, catalepsy has been shown to be increased by repeated testing. This paper briefly reviews these findings and reports new results on the effects of haloperidol on the micro-characteristics of rats' operant behavior maintained by fixed-ratio 20 food reinforcement. With these methods, within-session decrements were seen to be produced by abrupt cessation of responding and by response slowing that was evident from the first few responses in the session. It is argued that within-session decrements can be viewed as manifestations of neuroleptic-induced pseudoparkinsonism in the rat and that dynamic neurochemical changes in the nigrostriatal dopamine system mediate the behavioral changes.

Findings suggest that ingestion of a single oral dose of caffeine at typical consumptive levels results in measurable changes in physiological functioning and that whole-arm physiological tremor is a sensitive measure of physiological change due to drug effects. 4. However, the results suggest that whole-arm physiological tremor may not detect reliably TOD changes in functioning.

Purpose: To evaluate a variety of materials that might serve as fiducial markers for abdominal MRI‐ultrasound fusion applications. Methods: Two experiments were performed: (1) in a phantom, a broad set of candidate materials were evaluated based on visibility in spin echo T1 and T2, and gradient echo T1 and T2* MRI pulse sequences; (2) the leading candidates were evaluated using standard clinical abdominal pulse sequences, both in a phantom and volunteer. Experiment 1 evaluated: two commercial fiducial MRI markers (IZI Medical Products and Beekley Medical); vitamin E and fish oil capsules; water; and copper sulfate solution. Experiment 2 evaluated fish oil capsules and copper sulfate solution. In experiment 2, ultrasound coupling gel was also evaluated. Liquids were poured in wells drilled in a plexiglass base. All scanning was performed with a clinical 1.5T GE Signa Excite scanner. For each pulse sequence, maximum intensity projection images (MIPs) were formed, after removing the phantom signals. Visibility was evaluated by rank ordering signal magnitude in the MIPs. Results: In experiment 1, copper sulfate solution and fish oil were superior to the other materials. In experiment 2, ultrasound gel and copper sulfate solution were clearly superior to the other materials, and were easily seen in the all of the phantom and volunteer images. Conclusion: Based on these preliminary experiments, ultrasound gel and copper sulfate solution appear to be the most promising fiducial marker materials for MRI‐ultrasound fusion applications in the abdomen.

Purpose: Peritoneal seeding remains a prominent failure pattern in patients with invasive epithelial ovarian cancer, even following a pathologically negative second-look laparotomy (2LL). In an attempt to decrease the risk of peritoneal recurrence, we have treated patients with no clinical or histopathologic evidence of disease at 2LL with 15 mCi of chromic phosphate suspension intraperitoneally ( 32 P). Methods: Between 1973-1987, 69 patients with stages I-III invasive epithelial ovarian cancer in complete clinical remission were found to have no evidence of disease at 2LL. Fifty-one patients received intraperitoneal 32 P. Thirty-five patients, otherwise eligible for 32 P, did not receive it primarily due to other treatment protocols, peritoneal adhesions, or the recommendation that no further therapy be given following a negative 2LL. Patients in both groups were comparable with regard to stage, histology, grade, median age, residual disease following initial surgery, and chemotherapeutic regimen. The median follow-up for uncensored patients is 58 months (minimum, 18 months). Results: The 5-year actuarial disease-free survival rate from the date of 2LL was 86% for those receiving 32 P and 67% for those not receiving 32 P ( P = 0.05). The corresponding 5-year overall survival rates were 90 and 78%, again favoring patients treated with 32 P. Complications: There were minimal acute side effects from 32 P. Late adverse effects were similar in the two groups. Bowel complications were seen in 3 of 51 patients receiving 32 P and in 1 of 18 patients not receiving 32 P. Conclusions: We have found intraperitoneal 32 P administration immediately after 2LL to be a safe and well-tolerated therapy. Our data suggest that 32 P confers a survival advantage to patients with a pathologically negative 2LL. These findings suggest a continued role for second-look laparotomy with the use of 32 P in selected patients.

Renal excretion of urea was assessed in adult and larval Ambystoma tigrinum by comparing simultaneous clearances of urea and inulin. The mean adult urea clearance ratio (Cu/CIn) was 0.70 ± 0.08 indicating net urea absorption. Larvae yielded a mean CU/CIn) of 0.92 ± 0.04. Individual larvae were quite variable however as approximately 30% displayed CU/CIn>1. There was a significant negative correlation between CU/CIn and fractional water reabsorption. Comparisons of larval kidney urea concentration (KU) and plasma urea concentration (PU) revealed a mean KU/PU of 1.43 ± 0.15. Treatment with 2,4-dinitrophenol decreased larval CU/CIn to 0.80 ± 0.05 and KU/PU to 0.91 ± 0.08. Phloridzin also decreased CU/CIn (0.67 ± 0.06) however para-aminohippuric acid had no effect. Hypophysectomy combined with cautery of the pituitary stalk resulted in CU/CIn of 1.71 ± 0.33 as compared to 0.84 ± 0.08 in shams. These results are consistent with the possibility of net urea secretion at some point in the A. tigrinum nephron, followed by passive reabsorption in the distal segments under the influence of antidiuretic hormone. The current study examines the factor pattern of the Wechsler Adult Intelligence Scale for a sample of 151 neuropsychiatric inpatients (97 men, 54 women). As expected, these patients performed less well on all 11 of the subtests and ranged from .60 to 1.54 SD below normative standards. These patients were neither more variable nor were the intercorrelations among the subtests lower than those of the normative group. As in previous research, maximum likelihood factor analyses revealed the presence of a Verbal Comprehension dimension and a Perceptual Organization dimension, factors that were moderately correlated (.60). The stability of a two-factor solution for the WAIS in this population and its implications for the inclusion of the WAIS as a component of a neuropsychological assessment paradigm are discussed.

A rapid reduction of the dissolved oxygen content of the water occurred during the first few days of treatment. The activity of 12 herbicides was tested against various freshwater algae found in Britain. Three of the most troublesome species, Cladophora glomerata L., Rhizoclonium hieroglyphicum Katz., and Vaucheria dichotoma Ag. were all susceptible to methylthio-1,3,5-triazines. Terbutryne and cyanatryn were the most active and both killed C. glomerata and R. hieroglyphicum at 0.05 mg/litre. The concentration of terbutryne had to be increased to 0.1 mg/litre to control V. dichotoma. Asulam and WSCP (2.0 mg/litre), metflurazone and HZ5914 (1.0 mg/litre), copper sulphate (1.0 mg copper ion/litre) and cutrine (2.0 mg copper ion/litre) had no visible effect on the algae. Diquat (1.0 mg cation/litre) did, however, control C. glomerata and R. hieroglyphicum although, in natural conditions, the algae quickly recovered after treatment. Success with diquat (2.0 mg/litre) against V. dichotoma in laboratory experiments was not achieved in a field experiment.

This paper presents a new linear model methodology for clustering judges with homogeneous decision policies and differentiating dimensions which distinguish judgment policies. This linear policy capturing model based on canonical correlation analysis is compared to the standard model based on regression analysis and hierarchical agglomerative clustering. Potential advantages of the new methodology include simultaneous instead of sequential consideration of information in the dependent and independent variable sets, decreased interpretational difficulty in the presence of multicollinearity and/or suppressor/moderator variables, and a more clearly defined solution structure allowing assessment of a judge's relationship to all of the derived, ideal policy types. An application to capturing policies of information systems recruiters responsible for hiring entry-level personnel is used to compare and contrast the two techniques.

Between 1973 and 1985, 118 patients in clinical remission after initial surgery and postoperative chemotherapy for epithelial ovarian carcinoma underwent second-look laparotomy at the University of North Carolina. No evidence of disease (NED) was found in 57 of these patients; 43 patients received 15 mCi of radioactive chromic phosphate (32P) suspension given intraperitoneally in the immediate postoperative period. In 29 other patients, only microscopic or minimal residual disease (nodules less than 2 cm in size) was found, seven received 32P alone, ten received 32P and further chemotherapy, and 12 received chemotherapy alone. The 4-year postsecond-look survival of the patients with NED at second-look was 89% for those receiving 32P and 67% for those who had not. The respective figures for patients with minimal residual disease at second-look are 59% versus 22%. Irrespective of treatment, a group at high risk for failure after negative second-look laparotomy has been identified; those with an initial International Federation of Gynecology and Obstetrics (FIGO) stage greater than I and histologic grade greater than 1. A comparison of our data with 18 previously published series, indicates that use of postsecond-look intraperitoneal 32P can improve the progression-free interval, and possibly overall survival, of patients with NED or minimal residual disease without adding significant complications.
