Although diabetic peripheral neuropathy is the most common diabetic microangiopathic complication, several other neuropathy syndromes can occur in the context of diabetes. We describe a rare case of polyneuropathy associated with diabetic ketoacidosis in a patient with new-onset type 1 diabetes. A 42-year-old man with diabetic ketoacidosis was admitted to our hospital with complications of respiratory and renal failure requiring mechanical ventilation and hemodialysis, respectively. After diabetic ketoacidosis improved from the critical state, he developed upper- and lower-limb paralysis with sensory disturbances and pain, as well as right facial paralysis, left recurrent nerve paralysis, and left hypoglossal nerve paralysis. Autonomic nervous function was also impaired. As the pathophysiology, prevention, and treatment of polyneuropathy associated with diabetic ketoacidosis are unclear, the neurologic function of diabetic ketoacidosis patients should be closely monitored.

Introduction: When an impacted root is observed in an implant placement site in proximity to the sinus, extraction of therootisrequiredbeforeimplantplacement.However,rootextractionpresentsrisksofsinusperforationandrootmigration into the sinus antrum. If the root remnant is close to the sinus floor, a crestal extraction approach may not be indicated because extensive bone resection may be required. The technique presented in this case report, a lateral approach to extraction of an impacted residual root adjacent to a maxillary sinus immediately after sinus floor elevation, avoids these risks. Case Presentation: A 44-year-old female presented for implant treatment. Because vertical bone height in site #15 was not sufficient for implant placement, sinus floor elevation was required. The Schneiderian membrane was elevated after creation of a lateral window, and the root process (site #14) was palpated. The impacted root was exposed using a piezoelectric round insert from the buccal side. Immediately after sinus augmentation and extraction of the root, two implants were placed in the optimal position. Conclusions: This case report demonstrates a novel approach with extraction of an impacted residual root adjacent to a maxillary antrum. Root projection is observed on the sinus cavity, and palpation of the projection after elevation of the membrane can easily identify the positioning of the impacted root. Therefore, the lateral approach after sinus elevation minimized the risks of perforation and vertical bone loss around the root. This approach may be useful in the extraction of an impacted residual root adjacent to a maxillary sinus when the root interferes with implant placement. Clin Adv Periodontics 2014;4:43-48.

Statement of the Problem Although assessment of entire palatal mucosal thickness is important in many dental procedures, available data are mostly limited to the lateral aspect of the palate. Purpose of the Study The objective of this study was to use cone-beam computed tomography (CBCT) to perform a comprehensive analysis of the palatal mucosal thickness from the gingival margin to the mid-palatine suture in a Japanese population. Associations of palatal mucosal thickness with the palatal vault depth were also examined. Methods/Materials Measurements on the coronal plane were obtained from 44 adults with 3-mm interval in the canine (Ca), first premolar (P1), second premolar (P2), midpoint between first and second molars (M1d), first molar (M1), and second molar (M2). Furthermore, the location of greater palatine foramen (GPF) and palatine groove (PG) were also investigated. Results Canine region did not show a significant difference throughout measured points. P1, P2, and all molar regions were thickest at 9, 12, and 12 mm from the gingival margin, respectively. At 3 and 6 mm, Ca, P1, and P2 showed significantly greater thickness than the molar region. At 9 mm, P1 demonstrated a greater thickness than M1d, and P2 was greater than M1 and Mi. At 12 and 15 mm, P1 was thinner than P2, M1, and M2, whereas P2 was thinner than M2. M1 was thinner than M2. The high-vault group showed a significantly greater thickness than the low-vault group. In majority of subjects, GPF and PG were identified in second molar and first premolar to first molar, respectively. Conclusion Palatal mucosa in a Japanese population was the thickest in canine to premolar regions at 9 to 12 mm from the gingival margin. Identification of GPF and PG using CBCT can assist diagnosis of palate seems to minimize surgical complications. Clinical Significance This study evaluated the thickness of palatal mucosa in a Japanese population using cone-beam computed tomography, covering a wide range. Canine to second premolar regions are the most suitable in harvesting palatal mucosa for the purpose of soft tissue grafts.

The higher performance of older people in prospective memory (PM) tasks in a naturalistic setting rather than in a laboratory setting is well known in cognitive aging research and is called the “age prospective memory paradox”. The mechanism of the paradox is poorly understood. To clarify this complex phenomenon, testing of the contribution of candidate variables that may influence PM performance in older adults is needed. To identify possible variables, this study used the Prospective and Retrospective Memory Questionnaire (PRMQ; Smith, Della Sala, Logie, & Maylor, 2000) which was developed to evaluate PM and retrospective memory (RM) failure in daily life. In this large-sample survey study (n = 2610) we verified the reliability and validity of the Japanese version of the PRMQ in young (n = 459), young-old (n = 1291) and old-old (n = 860) people. First, we found high reliability for both the PM and RM dimensions of the PRMQ in all age groups. Second, the validity was confirmed not by the simple score on PM items but with a subtraction score (PM score minus RM score). In addition, our results also provide knowledge of how personality and lifestyle relate to self-rated memory failures.
