Magnetic resonance imaging (MRI) has been used to evaluate dogs with suspected prostatic neoplasia, however, published studies describing MRI characteristics of canine prostatic neoplasia are currently lacking. The aims of the current retrospective case series study were to describe MRI findings of the pelvic region in dogs with a histopathologic or cytologic diagnosis of prostatic neoplasia. Retrospective analysis of these images was then performed by a board-certified veterinary radiologist for shared imaging characteristics. The most consistent characteristics were heterogeneous hyperintensity of the tumor on T2-weighted images (10/10) and short tau inversion recovery images (10/10), prostatic capsular margin distortion by the tumor (10/10), cavitations (10/10), complete effacement of the prostatic architecture (9/10), neurovascular bundle (NVB) compression or invasion (9/10), heterogeneous isointensity of the tumor on T1-weighted images (9/10), and strong contrast enhancement of the tumor (8/10). Additional features included an overlying pattern of distorted radiating striations (7/10), regional lymphadenomegaly (5/10), mineralization within the mass (5/10), urinary bladder trigone involvement (6/10), and post-prostatic urethral involvement (7/10). These findings supported the use of MRI as an adjunct imaging modality for diagnosis and therapeutic planning of prostatic neoplasia and including prostatic neoplasia as a likely differential diagnosis for dogs with these MRI characteristics.

Tumour stage has been demonstrated to have prognostic significance in canine oral malignant melanoma (OMM). Various evaluation techniques of positron emission tomography/computed tomography (PET/CT) have been reported for staging of head-and-neck tumours in people, but canine-specific data are limited, and reports for CT accuracy have been variable. In this prospective study, the head/neck of client-owned dogs with cytologically or histologically diagnosed OMM were imaged with 18 Fluorine-fluorodeoxyglucose (18 F-FDG) PET/ CT. Bilateral mandibular lymphadenectomy was performed for histopathologic assessment. Two evaluation techniques for CT and PET were applied by four independent observers. CT evaluation utilized both a standardized grading scheme and a subjective clinical interpretation. PET evaluation was first performed solely on 18 F-FDG-uptake in lymph nodes compared to background on a truncated scan excluding the oral cavity. Subsequently, the entire head/neck scan and standardized uptake value (SUV) measurements were available. Receiver operating characteristic analysis was performed with histopathology as gold standard. Twelve dogs completed the study and metastatic OMM was identified in six mandibular lymph nodes from five dogs. Of the CT-interpretation techniques, use of clinical grading performed best (sensitivity = 83% and specificity = 94%). Both PET techniques resulted in 100% sensitivity, but primary tumour site evaluation and use of SUV increased specificity from 78% to 94%. The SUVmax cut-point, 3.3, led to 100% sensitivity and 83% specificity. In this population of dogs, PET appeared to be highly sensitive but at risk of being less specific without use of appropriate parameters and thresholds.

BACKGROUND: This is the largest and only multivariate study evaluating the difference in mortality from coronavirus disease 2019 (COVID-19) between patients with cancer and patients without cancer in the United States. The objective was to assess COVID-19 mortality rates in patients with cancer versus patients without cancer and uncover possible statistically significant characteristics contributing to mortality. METHODS: This retrospective study analyzed patients with cancer and patients without cancer who tested positive for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) from March 1 through April 30, 2020. This was a multicenter study in the state of Louisiana throughout the Ochsner Health System in both tertiary and nontertiary centers. Patients older than 18 years were eligible. Three hundred twelve patients with cancer were compared with 4833 patients without cancer. RESULTS: Mortality was found to be higher in the cancer group. Patients of advanced age with cancer had a significant increase in mortality (odds ratio [OR], 5.96; P < .001). Other significant risk factors for increased mortality were male sex (OR, 2.15), a history of chronic kidney disease (OR, 3.84), and obesity (OR, 1.30). In hospitalized patients with cancer, adverse vital signs on admission, decreased absolute lymphocyte counts, thrombocytopenia, elevated creatinine, lactic acidosis, and elevated procalcitonin all seemed to increase the risk of death. Among patients with cancer, active or progressive disease (P < .001) and recent therapy (OR, 2.34; 95% confidence interval, 1.08-5.08) were shown to increase mortality. CONCLUSIONS: Patients with cancer have increased mortality in the setting of infection with SARS-CoV-2 in comparison with patients without cancer. Patients with cancer who are 65 years of age or older and those with certain comorbidities have the greatest risk of death. Recent cancer-directed therapy and disease status also seem to play roles in mortality. LAY SUMMARY: This is the largest study of patients with cancer versus patients without cancer to date and is the first multivariate analysis study comparing these 2 patient populations. This study confirms the hypothesis that patients with cancer are at increased risk for mortality and that there are multiple characteristics posing the potential to risk-stratify these patients in the setting of a future outbreak.

Curcumin has well-established anti-cancer properties in vitro, however its therapeutic potential has been hindered by its poor bioavailability. Lipocurc(™) is a proprietary liposome-encapsulated curcumin formulation that enables intravenous delivery and has been shown to reach its highest concentration within lung tissue. The goal of this study was to characterize the anti-cancer and anti-angiogenic activity of Lipocurc(™) in vitro, in addition to evaluating Lipocurc(™) infusions in dogs with naturally occurring cancer. We therefore evaluated the effect of Lipocurc(™), relative to free curcumin, on the viability of canine osteosarcoma, melanoma and mammary carcinoma cell lines, as well as the ability of Lipocurc(™) to inhibit endothelial cell viability, migration and tube formation. We also undertook a pilot clinical trial consisting of four weekly 8-hour Lipocurc(™) infusions in 10 cancer-bearing dogs. Tumor cell proliferation was inhibited by curcumin at concentrations exceeding those achievable in the lung tissue of dogs. Similarly, equivalent high concentrations of Lipocurc(™) and curcumin also inhibited endothelial cell viability, migration and tube formation. Four out of six dogs completing planned infusions of Lipocurc(™) experienced stable disease, however, no radiographic responses were detected.

The goals of this retrospective study were to determine the patient characteristics of dogs with high-grade primary mediastinal lymphoma and to determine outcome and associated prognostic factors. A total of 42 dogs were identified, in which 36 received treatment and had follow-up information available. The most common clinical signs included lethargy, anorexia and polyuria/polydipsia. Hypercalcemia and pleural effusion were common findings at diagnosis. The phenotype was almost exclusively T-cell, most often in association with lymphoblastic cytomorphology as defined by the World Health Organization (WHO) lymphoma classification scheme. The overall progression-free survival (PFS) and overall survival (OS) were 133 and 183 days, respectively. Treatment with a CHOP (cyclophosphamide, doxorubicin, vincristine, prednisone) protocol was associated with an improved PFS (144 days) and OS (194 days) when compared with dogs that received other medical therapies (P = .005 and P = .002, respectively); the absence of pleural effusion at diagnosis was associated with an increased OS but not PFS. These results suggest that while the prognosis for dogs with mediastinal lymphoma is poor, survival may be improved with treatment using a CHOP-based protocol.

Background Oral chemotherapy agents are frequently compounded in veterinary medicine however, the potency of some formulations have been shown to vary from that of Food and Drug Administration (FDA)-approved products. Aims The objective of this study was to evaluate the potency and stability of three compounded oral chemotherapeutics commonly prescribed to be administered over time. Materials & Methods Compounded chlorambucil 1 mg, cyclophosphamide 5 mg and melphalan 1 mg were obtained and for potency tested upon receipt and 6 weeks later. Recent news stories have explicitly stated that patients with symptoms of COVID‐19 were “turned away” from emergency departments. This commentary addresses these serious allegations, with an attempt to provide the perspective of academic emergency departments (EDs) around the Nation. The overarching point we wish to make is that academic EDs never deny emergency care to any person.

Diffuse large B-cell lymphoma (DLBCL) is the most common haematopoietic malignancy in dogs. Recently, MYC and BCL2 expression levels determined with immunohistochemistry (IHC) were found to be prognostic in people with DLBCL. We hypothesized that canine DLBCL can be similarly subdivided into prognostic subtypes based on expression of MYC and BCL2. Cases of canine DLBCL treated with CHOP chemotherapy were retrospectively collected and 43 dogs had available histologic tissue and complete clinical follow-up. Median values of percent immunoreactive versus immunonegative cells were used to determine positive or negative expression status. Completion of CHOP was significantly associated with a positive outcome. Compared with human patients, our canine DLBCL patients had high IHC expression of both MYC and BCL2, and relative expression levels of one or both markers were not associated with clinical outcome.

Objective Determine whether prolonged consumption of high- or low-protein diets modifies the thermogenic response to a standard meal. Methods Twenty-four healthy individuals were randomized to overfeeding diets containing low (5%, n = 8), normal (15%, n = 9), or high (25%, n = 7) protein for 56 days while inpatients. The thermic effect of food (TEF) was measured over 4 h by indirect calorimetry following a standard meal (20% of energy, 20% protein) or a meal that matched the study diet (“study meal”). Results As expected, the TEF following the study meal (i.e., either low, normal, or high protein content) was significantly associated with dietary protein (P = 0.007), and the TEF was significantly increased in the high-protein diet (15.4%) versus the normal-protein (5.6%) and low-protein diets (6.4%) (P = 0.05 and P = 0.03, respectively). However, returning to a standard meal (20% protein) after 42 days of overfeeding the study diets, the TEF response did not differ from baseline between groups or within subjects regardless of the prolonged intake of the habitual study diet (high protein: P = 0.32, low protein: P = 0.11, normal protein: P = 0.79). Conclusions TEF is related to acute dietary protein intake but not altered by prolonged intake of high-energy diets with high or low protein content.

Growing demand for palm oil is driving its expansion into the African tropics, potentially leading to significant carbon emissions if tropical forest is converted to palm monoculture. In this first study of a Central African oil palm concession (31,800 ha), we predict that the conversion of 11,500 ha of logged forest to a palm plantation in Gabon will release 1.50 Tg C (95% CI = [1.29, 1.76]). These emissions could be completely offset over 25 years through sequestration in planned forest set-asides given a 2.6:1 ratio of logged to converted forest. Using an agricultural suitability model, we find that careful national land-use planning could largely avoid high carbon emissions while meeting goals for palm oil production. We recommend that Gabon adopts a national carbon threshold for land conversion and requires concession-level set-aside ratios that meet no-net emissions criteria as mechanisms for steering plantations away from high carbon forests.

Intermittent administration of tyrosine kinase inhibitors (TKI) combined with cytotoxic chemotherapy may effectively chemosensitize canine MCT while decreasing cost and adverse effects associated with either agent administered as monotherapy.Hypothesis/objectivesThe primary study objectives were to (1) identify the maximally tolerated dose (MTD), (2) determine the objective response rate (ORR) and (3) describe the adverse event profile of pulse-administered toceranib phosphate (TOC) combined with lomustine.AnimalsForty-seven client-owned dogs with measurable MCT.MethodsToceranib phosphate was given PO on days 1, 3 and 5 of a 21-day cycle at a target dosage of 2.75 mg/kg. Lomustine was given PO on day 3 of each cycle at a starting dosage of 50 mg/m(2) . All dogs were concurrently treated with diphenhydramine, omeprazole, and prednisone.ResultsThe MTD of lomustine was established at 50 mg/m(2) when combined with pulse-administered TOC; the dose-limiting toxicity was neutropenia. Forty-one dogs treated at the MTD were evaluable for outcome assessment. The ORR was 46% (4 complete response, 15 partial response) and the overall median progression-free survival (PFS) was 53 days (1 to g752 days). On multivariate analysis, variables significantly associated with improved PFS included response to treatment, absence of metastasis, and no previous chemotherapy.Conclusions and clinical importanceCombined treatment with pulse-administered TOC and lomustine generally is well tolerated and may be a reasonable treatment option for dogs with unresectable or metastatic MCT.

Objectives To determine whether doxorubicin–piroxicam combination is safe and has activity against transitional cell carcinoma in dogs. Methods Data was collected retrospectively from 34 dogs from two institutions over a 6-year period. Signalment, clinical presentation, treatment specifics, adverse events, response, progression-free survival and overall survival time were evaluated. Results Dogs received doxorubicin every 3 weeks and daily piroxicam; 17 dogs (50%) had surgery. Clinical presentations were those typically reported for transitional cell carcinoma. Mean number of doses administered was 3·5. Of the 23 dogs with measurable disease, 14 (60·5%) had stable disease, 7 (30·5%) had progressive disease and 2 (9%) a partial response. Adverse events were generally manageable, and gastrointestinal in origin; one dog died of treatment-related complications. Overall median progression-free survival and overall survival were 103 and 168 days, respectively. Cytoreductive surgery did not result in prolongation of progression-free survival, but significantly prolonged overall survival. All dogs but one died as a result of disease progression. Clinical Significance Doxorubicin–piroxicam combination therapy is well-tolerated in dogs with transitional cell carcinoma although progression-free survival, overall survival and biological response rates appear modest. Combination with surgery appears to offer a survival advantage; however, this may reflect tumour location and volume. Prospective studies are necessary to compare activity of combination doxorubicin–piroxicam to currently applied therapies.

With the exception of solar-induced dermal hemangiosarcoma (HSA), the biologic behaviour of canine HSA is characterised by rapid tumour growth, a high metastatic rate and short survival times. Outcome of dogs with HSA of the tongue has not been previously reported. The purpose of this study was to assess outcome and prognostic factors in dogs with lingual HSA. Clinical data was collected retrospectively and histopathology was reviewed for 20 dogs. Median progression free survival was 524 days and the median overall survival time was 553 days. All dogs had low or intermediate grade tumours; most tumours were small and located on the ventral surface of the tongue. Prognostic factors significantly associated with increased survival included small tumour size and absence of clinical signs of an oral mass at the time of diagnosis. Dogs with HSA confined to the tongue may have a better prognosis compared with HSA in other organs.

Dose intense CHOP protocols have been shown to improve outcome for people with non-Hodgkin's lymphoma, but evaluation of dose intense CHOP protocols for canine lymphoma is currently limited. The hypothesis of this retrospective study was that a 15-week dose intense CHOP protocol would have shorter treatment duration with similar efficacy to other doxorubicin-based multidrug protocols. Thirty-one client owned dogs with multicentric lymphoma were treated with a 15-week CHOP chemotherapy protocol with an overall response rate of 100% and a median progression-free interval (PFI) of 140 days [95% confidence interval (CI) 91-335 days]. Dogs that had two or more treatment delays had significantly prolonged PFI and overall survival in multivariate analysis. Dose intensity did not correlate with patient outcome. Dogs experiencing multiple treatment delays secondary to adverse events may receive their individual maximally tolerated dose while dogs with no adverse events may be underdosed. Future studies should focus on individual patient dose optimization.

Background: Low-dose, continuous (metronomic) chemotherapy improves tumor control by inhibiting tumor angiogenesis and suppressing regulatory T cells (Treg) in mice and humans. The effects of metronomic chemotherapy on Treg and tumor angiogenesis in dogs has not been investigated previously. Objective: To determine whether metronomic cyclophosphamide (CYC) therapy decreases Treg or exhibits antiangiogenic activity or both in dogs with soft tissue sarcoma (STS). We hypothesized that Treg numbers would be increased in dogs with STS and that continuous dosing of CYC would decrease Treg in a dose-dependent manner, as well as exhibit antiangiogenic activity. Animals: Eleven client-owned dogs with grade I or II STS. Twenty-one healthy dogs were used as controls. Methods: Prospective, open, clinical trial. Dogs with STS were enrolled in 2 dose cohorts and administered CYC at 12.5 or 15 mg/m2 PO once daily for 28 days. Whole blood and tumor biopsy specimens were obtained on days 0, 14, and 28 to assess changes in T lymphocyte subsets by flow cytometry and tumor microvessel density (MVD), respectively. Results: Administration of CYC at 12.5 mg/m2/d significantly decreased the number of Treg from days 0 to 28, but there was no change in the percentage of Treg or tumor MVD. In dogs that received CYC at 15.0 mg/m2/d, both the number and percent of Treg as well as tumor MVD were significantly decreased over 28 days. Conclusions: CYC administered at 15 mg/m2/d should be used in further studies examining the antitumor properties of low-dose CYC in dogs.

Background: Increased numbers of regulatory T cells (Treg) and decreased ratios of CD8+ T cells to Treg have been shown to correlate with decreased survival times (ST) in humans with certain malignancies. A possible connection between Treg and ST in dogs with cancer has not been investigated previously. Hypothesis: The purpose of this study was to compare numbers of Treg and T lymphocyte subsets in dogs with osteosarcoma (OSA) to those of healthy dogs and to determine whether pretreatment values were associated with disease-free interval or with ST. We hypothesized that Treg numbers would be increased in dogs with cancer and that dogs with a high percentage of Treg would have a poorer prognosis. Animals: Twelve client-owned dogs with appendicular OSA were entered into a prospective clinical trial. Twenty-two healthy dogs were used as controls. Methods: The percentages and numbers of Treg and CD4+ and CD8+ T cells in blood, lymph nodes, and tumors were determined with flow cytometry and compared between dogs with OSA and control dogs. Results: Dogs with OSA had significantly fewer circulating CD8+ T cells and significantly more Treg compared with healthy dogs. The CD8/Treg ratio also was significantly lower in dogs with OSA compared with control dogs. In dogs with OSA, a decreased CD8/Treg ratio was associated with significantly shorter STs. Conclusions: These data support a role for Treg in the immune control of canine OSA and suggest that determination of the CD8/Treg ratio may be useful for assessing outcomes.

Sand shoals provide both a potentially unique habitat resource for marine organisms and a source of sand for the replenishment of eroded beaches. Sand removal may negatively influence marine communities, so understanding how marine fauna utilize habitats at and around shoals would provide much-needed guidance in selecting sites for sand harvest. A 2-year study was conducted on the inner continental shelf of the Middle Atlantic Bight, U.S.A., to compare finfish and invertebrate assemblages at sand shoal and nearby flat-bottom habitats. Multiple sampling modalities were used to sample organisms across a range of sizes, living habits, and life history stages. There was a trend toward greater abundance, species richness, and species diversity in flat-bottom habitats than in shoal habitats, and all of these community measures were generally lower during winter than in spring, summer, or fall. The Scottish Audit of Surgical Mortality is a voluntary, peer reviewed, critical event analysis of patients who die under the care of consultant surgeons in acute hospitals in Scotland. The anaesthetic contribution to surgical mortality over a 10-year period from 1996 was reviewed. The total number of deaths was 44 230 or 1.5% of all admissions. Forty thousand, eight hundred and ninety-six deaths (92%) were audited. Deaths after elective surgery declined over 10 years. Over 80% of deaths followed emergency admission. The number of deaths where an anaesthetist was present was 16 981 or 0.6% of all admissions. Anaesthetic areas of concern were identified in 8% of deaths. Of these, 43% were related to pre-operative assessment. Anaesthesia also played a part in a further 18% of deaths where decision making was shared with the surgical team. Of these, 41% were related to access to critical care. A further 24% related to communication failures, principally when the operation should not have been done or was unnecessary.

Summary Thermal death data were obtained for B. stearothermophilus spores heated in (a) a laboratory scale, direct heating UHT plant and (b) a larger scale direct heating UHT plant. These were compared with the corrected data from experiments in which spores of the same batch were heated in capillary tubes. Temperature coefficients (Q10 values) for inactivation of spores in UHT plants were higher than those for spores in capillary tubes but agreement was closer when spores were suspended in water than in milk. For aqueous spore suspensions, the discrepancy between plants and capillaries was least for the laboratory scale plant. the divergence always became greater at lower temperatures and the D values were coincident at 145°C. Possible reasons for these various effects are discussed.

Summary A study was initiated into the inconsistencies reported to occur between (a) estimations of sterilizing efficiency (of UHT plants) based on experiments in which bacterial spores are heated in capillary tubes at sub-UHT temperatures and (b) determinations of sterilizing efficiency made by heating spores at UHT temperatures in the plants themselves. Capillary tube experiments were extended to 160°C with the aim of obtaining data suitable for direct comparison, i.e. obviating any need for extrapolation, with those obtained by plant inoculation. As heat-treatment temperatures were increased from 120 to 160°C the shape of the survivor curves became increasingly sigmoidal, possibly due to the greater significance of heat transfer times at high temperatures. Temperature coefficients for spores suspended in water were constant (Q10= 23.5) up to 132.5°C and for spores suspended in milk were constant (Q10= 13.2) up to 142.5°C; beyond these temperatures, Q10 values gradually diminished. Such an effect could not be explained by Arrhenius kinetics and again may reflect the physical limitations of the capillary tube system. Its importance is discussed in relation to the value of capillary tube experiments for predicting the sterilizing efficiency of UHT plants.

Construction activities around Campeche's central park led to the discovery of an early colonial church and an associated burial ground, in use from the mid-16th century AD to the late 17th century. Remains of some individuals revealed dental mutilations characteristic of West Africa. Analyses of strontium isotopes of dental enamel from these individuals yielded unusually high 87Sr/86Sr ratios, inconsistent with an origin in Mesoamerica, but consistent with an origin in West Africa in terrain underlain by the West Africa Craton, perhaps near the port of Elmina, a principal source of slaves for the New World during the 16th century. OBJECTIVE: To determine the rate of unrecognized endotracheal tube misplacement when performed by emergency medical services (EMS) personnel in a mixed urban and rural setting. METHODS: The authors conducted a prospective, observational analysis of out-of-hospital endotracheal intubations (EIs) performed by EMS personnel serving a mixed urban, suburban, and rural population. From July 1, 1998, to August 30, 1999, emergency physicians assessed and recorded the position of out-of-hospital EIs using auscultation, direct laryngoscopy, infrared CO(2) detectors, esophageal detector devices, and chest x-ray. The state EMS database also was reviewed to determine the number of EIs involving patients transported to the authors' medical center and paramedic assessment of success for these encounters. RESULTS: A total of 167 out-of-hospital EIs were recorded, of which 136 (81%) were deemed successful by EMS personnel. Observational forms were completed for 109 of the 136 patients who arrived intubated to the emergency department. Of the studied patients, 12% (13 of 109) were found to have misplaced endotracheal tubes. For the patients with unrecognized improperly placed tubes, 9% (10 of 109) were in the esophagus, 2% (2 of 109) were in the right main stem, and 1% (1 of 109) were above the cords. Paramedics serving urban and suburban areas did not perform significantly better (p < 0.05) than intermediate-level providers serving areas that are more rural. CONCLUSIONS: The incidence of unrecognized misplacement of endotracheal tubes by EMS personnel may be higher than most previous studies, making regular EMS evaluation and the out-of-hospital use of devices to confirm placement imperative. The authors were unable to show a difference in misplacement rates based on provider experience or level of training.

Decades of research show that barium and strontium enter skeletal tissues in proportion to their dietary abundance and, hence, to local environmental levels. Among regions that contrast geologically or climatologically, the environmental abundance of these elements can vary substantially and exceed variation due solely to local dietary differences. Because dental enamel develops during childhood and retains the original levels of these elements, it can provide a chemical signature of the geographic origins of individuals. Strontium isotope analysis of bone and tooth enamel from prehistoric human skeletons is an important new technique used to address questions regarding migration. Two problems arise in such investigations: (1) levels of strontium isotope ratios in local bedrock, soil, water, plants and animals are variable; and (2) a range of values in human bone and enamel data make it difficult to distinguish some migrants from locals. Analysis of the bones of small animals provides a robust measure of local strontium isotope ratios and a reliable, if conservative, means for determining confidence limits for distinguishing migrants. Data from various geographical areas are presented here in a discussion of variability in strontium isotope values. Examples are provided using modern and prehistoric materials. Two thirds of the US population is overweight or obese and childhood obesity is increasing. Growing evidence supports a connection between obesity, diabetes, metabolic syndrome and environmental chemical exposure. Effects of imidacloprid, a neonicotinoid insecticide, on intestinal fat deposition and lifespan were tested in wild type (N2) and daf-16 deficient (daf-16(mgDf50)I) Caenorhabditis elegans (C. elegans) in liquid culture supplied with 5-fluoro-2’-deoxyuridine (0.24mM). Control group received E. coli (OP50). Experimental group was treated with additional imidacloprid (0.1-1,000nM). Half of the nematodes were fixed in 4% paraformaldehyde, washed with PBS, stained with Nile red and fluorescent spectrometry determined intensity. Objective To assess adrenocortical function following intravenous etomidate use in emergency department (ED) patients requiring intubation. Methods This was a prospective, randomized, controlled trial of consecutive patients presenting to the ED requiring intubation. Patients were randomized to receive a single bolus induction dose of either 0.05-0.1 mg/kg midazolam (control group) or 0.3 mg/kg etomidate (etomidate group) during a standardized rapid-sequence intubation (RSI) with succinylcholine. The primary outcome variable was adrenocortical function at 4, 12, and 24 hours post-induction as assessed by measured serum cortisol response to exogenous cosyntropin (cosyntropin stimulation test, CST). Fisher's exact test was used to compare CST results between groups. Results Thirty-one patients were enrolled: 8 control, 10 etomidate, and 13 excluded from analysis for either incomplete data or steroid use during the study period. The 4-hour CST results were significantly different between study groups, with a normal response in 100% of control patients vs 30% of etomidate patients (p = 0.004). The 12- and 24-hour CSTs did not differ significantly between groups: normal CST in 100% of control patients at 12 and 24 hours vs 100% and 90% among etomidate patients at 12 and 24 hours, respectively (p = 1.0 at 12 and 24 hours). Measured cortisol levels of patients with abnormal CSTs remained within normal laboratory reference ranges. Conclusion Use of etomidate in ED patients requiring RSI results in adrenocortical dysfunction. However, cortisol levels remain within normal laboratory levels during this period of dysfunction. Adrenocortical dysfunction appears to resolve within 12 hours of a single bolus dose of 0.3 mg/kg etomidate.

Strontium in archaeological human bones is widely, almost paradigmatically, used as a measure of the relative dietary abundances of plants and meat. Quantitative modeling reveals, however, that there is not a simple proportional relationship between bone strontium and the dietary plant/meat ratio. While knowledge of specific foods and their compositions may permit accurate calculation of average bone strontium levels, knowledge of bone strontium does not inversely allow accurate calculation of specific foods. Although bone strontium quantitatively reflects the average dietary Sr/Ca ratio, it is disproportionately sensitive to high-calcium foods and can be easily affected by minor dietary constituents and culinary practices. Bone strontium, and by analogy, barium, should be seen as a reflection of the high-mineral dietary components rather than a quantitative index of trophic position. © 1995 Wiley-Liss, Inc.

A mark-recapture study was conducted in the Delaware River during the fall of 1990 to estimate population size of age-0 striped bass Morone saxatilis. Approximately 95,000 hatchery-reared fish were individually marked with coded wire tags and released in 15 separate batches at 10 different locations within the nursery area. Handling mortality and tag loss were estimated through the use of in situ enclosures. Recapture efforts yielded 53 tagged fish from approximately 300 seine and trawl samples. A total population of 972,937 age-0 striped bass was estimated by means of the Petersen method and an area-weighted volumetric expansion for upriver areas. Analysis of the assumptions of the Petersen method indicated that releasing tagged fish over a broad area and randomizing recapture methods are the most important steps to take in estimating population size ofage-0 striped bass. A summer minimum flow requirement was instituted in 1982 for the Conowingo Hydroelectric Dam, Maryland, where no such requirement existed previously. The response of fish to this minimum flow was assessed by monitoring food consumption and condition factor of three abundant, downstream species (white perch Morone americana, yellow perch Perca flavescens, and channel catfish Ictalurus punctatus), and by comparing the values from before and after the minimum flow was established. We also compared growth rates of white perch before and after the minimum flow was established by back-calculating length from otoliths. We found that fish condition (weight at length) was significantly greater for all species in years when the minimum flow was maintained. Similarly, growth rate (length at age) was significantly higher after the minimum flow requirement was instituted. Warming Arctic temperatures can drive changes in vegetation structure and function directly by stimulating plant growth or indirectly by stimulating microbial decomposition of organic matter and releasing more nutrients for plant uptake and growth. The arctic biome is currently increasing in deciduous shrub cover and this increase is expected to continue with climate warming. However, little is known how current deciduous shrub communities will respond to future climate induced warming and nutrient increase. We examined the plant and ecosystem response to a long-term (18 years) nutrient addition and warming experiment in an Alaskan arctic tall deciduous shrub tundra ecosystem to understand controls over plant productivity and carbon (C) and nitrogen (N) storage in shrub tundra ecosystems. In addition, we used a meta-analysis approach to compare the treatment effect size for aboveground biomass among seven long-term studies conducted across multiple plant community types within the Arctic. We found that biomass, productivity, and aboveground N pools increased with nutrient additions and warming, while species diversity decreased. Both nutrient additions and warming caused the dominant functional group, deciduous shrubs, to increase biomass and proportional C and N allocation to aboveground stems but decreased allocation to belowground stems. For all response variables except soil C and N pools, effects of nutrients plus warming were largest. Soil C and N pools were highly variable and we could not detect any response to the treatments. The biomass response to warming and fertilization in tall deciduous shrub tundra was greater than moist acidic and moist non-acidic tundra and more similar to the biomass response of wet sedge tundra. Our data suggest that in a warmer and more nutrient-rich Arctic, tall deciduous shrub tundra will have greater total deciduous shrub biomass and a higher proportion of woody tissue that has a longer residence time, with a lower proportion of C and N allocated to belowground stems.
