Background To resolve drawbacks of navigator triggering (NT) three-dimensional (3D) magnetic resonance cholangiopancreatography (MRCP), several approaches were proposed to obtain 3D MRCP within a single breath-hold (BH). However, reduced field-of-view technique in the phase-encoding direction combined with two-dimensional spatially selective radiofrequency excitation pulses has not yet been applied to 3D BH MRCP. Purpose To investigate the feasibility and the complementary value of 3D BH zoomed MRCP to conventional 3D NT MRCP in patients with branch duct intraductal papillary mucinous neoplasms (BD-IPMNs) of the pancreas. Study type Retrospective. Population A total of 221 patients (116 male and 105 female, median age 73 years) with BD-IPMNs. Field strength/sequence 3.0 T/3D turbo spin echo ASSESSMENT: MR images were analyzed by three radiologists (R.M., H.O., M.T., with 1, 13, and 17 years of experience) to compare blurring and motion artifacts, background suppression, visualization of main pancreatic duct (MPD), conspicuity of BD-IPMN, and overall image quality. Statistical tests Wilcoxon-signed rank, Mann-Whitney U, chi-squared or Fisher's exact tests (P Results Image quality was significantly higher on 3D NT MRCP images than on 3D BH zoomed MRCP (median (interquartile range); background suppression, 4 (4-4) vs. 3 (3-4); visualization of MPD, 4 (3-4) vs. 4 (3-4), conspicuity of BD-IPMN, 4 (3-4) vs. 3 (3-4); and overall image quality, 3 (3-4) vs. 3 (3-3)). However, in 32 (14%) patients, 3D NT MRCP showed a score of 1 or 2 in overall image quality. Regarding the conspicuity of BD-IPMN, a conspicuity score of 1 or 2 was rendered in 31 (14%) patients in 3D NT MRCP group. Conversely, 3D BH zoomed MRCP showed a score of 3 or 4 in 29 (94%) of these 31 patients. Data conclusion 3D BH zoomed MRCP plays a complementary role to 3D NT MRCP, and may improve the conspicuity of BD-IPMNs in patients with irregular breathing pattern. Aim The impact of sustained virologic response (SVR) on surgical outcomes for patients with hepatitis C virus (HCV)‐related hepatocellular carcinoma (HCC) remains controversial. This study aimed to evaluate the influence of SVR on long‐term surgical outcomes after hepatectomy. Methods This multicenter study included 504 patients who underwent curative resection for HCV‐related HCC. Patients with a history of HCC treatment, HBV infection, poor liver function, and tumor with major vascular invasion were excluded. Long‐term surgical outcomes (overall survival [OS] and recurrence‐free survival [RFS]) among patients who achieved SVR before hepatectomy (Pre‐SVR group: 58 patients), after hepatectomy (Post‐SVR group: 54 patients), and without SVR (Non‐SVR group: 186 patients) were compared after adjusting for 13 confounding factors. Using the surgically resected specimens, comparison of the pathological changes in liver fibrosis between the first and second hepatectomy were analyzed. Results Patients with SVR were younger, had better liver function, and less liver fibrosis compared to patients without SVR. Propensity score‐matched OS and RFS were significantly better in Pre‐SVR group than Non‐SVR group (P = .029 and P = .009, respectively). Inverse probability‐weighted OS and RFS were also significantly better in the Post‐SVR group (P = .001 and P = .021, respectively) than in the Non‐SVR group. Histopathological evaluation revealed that only the patients with SVR had regression of liver fibrosis (P, Although the achievement of sustained virologic response (SVR) is essential for improving survival in patients with hepatitis C virus (HCV) infection, its importance on surgical outcomes for HCV‐related hepatocellular carcinoma (HCC) is still controversial. In this multicenter study, we demonstrated the importance of achieving SVR before or after hepatectomy for improving long‐term surgical outcomes. These results can encourage patients with HCV‐related HCC to undergo antiviral therapy regardless of the timing.

BACKGROUND Pancreatic exocrine insufficiency is one of the causes of malabsorption syndrome. In many cases of malabsorption syndrome, pancreatic exocrine insufficiency can be treated with pancreatic enzyme replacement therapy. Therefore, it is important to detect pancreatic endocrine insufficiency as early and accurately as possible. Recent studies have shown that cine-dynamic MR cholangiopancreatography (MRCP) may be useful to evaluate pancreatic exocrine function PURPOSE: To identify abdominal symptoms that suggest decreased flow of pancreatic enzyme secretion for which cine-dynamic MRCP should be performed to diagnose pancreatic exocrine insufficiency. STUDY TYPE Prospective. POPULATION In all, 111 patients with various types of abdominal symptoms. FIELD STRENGTH/SEQUENCE 5 T or 3 T, MRCP with spatially selective inversion recovery pulse (cine-dynamic MRCP). ASSESSMENT Cine-dynamic MRCP was performed and an 18-question clinical questionnaire on abdominal symptoms was administered. The secretion grade derived from cine-dynamic MRCP was compared between those answering "yes" and "no" for all 18 items STATISTICAL TESTS: Univariate analysis and further analyzed using multiple regression analysis. The associations between the secretion grade and the items in the clinical questionnaire were analyzed by univariate analysis and further analyzed using multiple regression analysis. RESULTS The following three items showed significantly negative correlations with secretion grade: Q9, Does your rectal gas smell foul? (β = -0.44, P = 0.001); Q13, Is stool quantity large? (β = -0.41, P = 0.001); and Q18, Are your stools soft? (β = -0.53, P < 0.001). No significant correlations with exocrine pancreatic function measured by cine-dynamic MRCP were seen for the remaining 15 abdominal symptom items. DATA CONCLUSION Abdominal symptoms that suggest decreased flow of pancreatic enzyme secretion were foul rectal gas, large stool, and soft stool. Aim To clarify the clinical and magnetic resonance imaging (MRI) features of de novo hypervascular hepatocellular carcinoma (HCC) using serial gadoxetic acid-enhanced MRI. Methods The institutional review board approved this retrospective study. After review of 1007 MRI examinations in 240 patients with chronic liver disease, 17 newly developed hypervascular HCCs in 16 patients detected by follow-up from initial MRI examination without hepatocellular nodules were evaluated. The clinical and MRI findings such as previous treatment history for HCC, period to hypervascular HCC onset, presence or absence of hypovascular hypointense nodules on hepatobiliary phase before hypervascularization, and intralesional fat component were recorded or evaluated. Statistical evaluations included Fisher's exact test, χ2-test, and Mann–Whitney U-test. Results In 17 HCCs, 12 (71%) were de novo hypervascular HCC without showing hypovascular hypointense nodule on hepatobiliary phase before hypervascularization (de novo group) and 5 (29%) were hypervascularized HCC developed during multistep hepatocarcinogenesis (multistep group). The incidence of previous treatment history for HCC in the de novo group (91%) was significantly higher than that in the multistep group (20%) (P = 0.013). The duration to hypervascular HCC onset from initial examination was shorter in the de novo group (mean, 291 days) than in the multistep group (mean, 509 days) (P = 0.035). The incidence of fat-containing lesion in the de novo group (0%) was lower than that in the multistep group (40%) (P = 0.074). Conclusion De novo hypervascular HCC is characterized by rapid growth, patients with previous treatment history for HCC, and lack of intralesional fat, compared to hypervascular HCC with multistep progression.

Purpose To examine the value of preoperative multiparametric magnetic resonance imaging (MRI) as a predictor of surgical margin (SM) status in patients with prostate cancer (PC). Materials and Methods The Institutional Review Board approved this retrospective study; the requirement for informed consent was waived. Fifty-six male patients with histologically proven PC underwent preoperative 3T multiparametric MRI including high b value (0, 2000 s/mm2) diffusion-weighted imaging. In each patient, clinical data, such as biopsy Gleason score and D'Amico clinical risk score, and multiparametric MRI findings, such as tumor location, tumor size, tumor extension in the apical or proximal region, tumor apparent diffusion coefficient (ADC), and the presence or absence of MRI findings of extracapsular extension (ECE) were evaluated. Statistical evaluations included the Fisher's exact test, χ2 test, Mann–Whitney U-test, and logistic regression analysis. Results On histopathological evaluation, 15 patients (27%) were SM-positive (SMP group), and 41 (73%) were SM-negative (SMN group). The tumor ADC was significantly lower in the SMP group than in the SMN group (P = 0.001). The frequency of tumor extension in the apex or base and suspected ECE on MRI were significantly higher in the SMP group than in the SMN group (P = 0.037 and 0.011, respectively). On multivariate analysis, tumor ADC was the only predictor of SM status in PC (P = 0.003). Conclusion PC with positive SM was characterized by tumor extension in the apical and proximal regions, lower tumor ADC, and tumors with positive MRI findings of ECE, compared to tumors with negative SM. Purpose To investigate the feasibility of noncontrast-enhanced cine dynamic magnetic resonance cholangiopancreatography (MRCP) with a spatially selective inversion-recovery (IR) pulse for evaluating exocrine pancreatic function in comparison with the N-benzoyl-L-tyrosyl-p-aminobenzoic acid (BT-PABA) test as a pancreatic exocrine function test. Materials and Methods Twenty subjects with or without chronic pancreatitis were included. MRCP with a spatially selective IR pulse was repeated every 15 seconds for 5 minutes to acquire a total of 20 images (cine-dynamic MRCP). The median and mean frequency of the observation (the number of times) and the moving distance (mean secretion grading scores) of pancreatic juice inflow on cine-dynamic MRCP were compared with a BT-PABA test. Results The urinary PABA excretion rate (%) had significant positive correlations with both the mean secretion grade (r = 0.66, P = 0.002) and frequency of secretory inflow (r = 0.62, P = 0.004) in cine dynamic MRCP. Purpose To assess whether noncontrast-enhanced steady-state free precession (SSFP) magnetic resonance imaging (MRI) with a spatially selective inversion recovery (IR) pulse can improve the visibility of renal corticomedullary differentiation in patients showing renal dysfunction, and to investigate the correlation between renal cortical thickness and estimated glomerular filtration rate (eGFR). Materials and Methods Sixty-five patients with and without chronic kidney diseases (CKD) were investigated. Based on eGFR, patients were divided into three groups (Group 1, eGFR 90). All patients underwent noncontrast-enhanced SSFP MRI with spatially selective IR pulses and minimal renal cortical thickness was measured. Results The mean corticomedullary contrast ratio was significantly higher in SSFP images with optimal TI than in in-phase images in all three groups (P = 0.001). Positive correlation was seen between the corticomedullary contrast ratio in SSFP images with optimal TI and eGFR (P = 0.011, r = 0.314). Purpose To evaluate age-related change in renal corticomedullary differentiation and renal cortical thickness by means of noncontrast-enhanced steady-state free precession (SSFP) magnetic resonance imaging (MRI) with spatially selective inversion recovery (IR) pulse. Materials and Methods The Institutional Review Board of our hospital approved this retrospective study and patient informed consent was waived. This study included 48 patients without renal diseases who underwent noncontrast-enhanced SSFP MRI with spatially selective IR pulse using variable inversion times (TIs) (700–1500 msec). The signal intensity of renal cortex and medulla were measured to calculate renal corticomedullary contrast ratio. Additionally, renal cortical thickness was measured. Results The renal corticomedullary junction was clearly depicted in all patients. The mean cortical thickness was 3.9 ± 0.83 mm. The mean corticomedullary contrast ratio was 4.7 ± 1.4. There was a negative correlation between optimal TI for the best visualization of renal corticomedullary differentiation and age (r = −0.378; P = 0.001). However, there was no significant correlation between renal corticomedullary contrast ratio and age (r = 0.187; P = 0.20). Similarly, no significant correlation was observed between renal cortical thickness and age (r = 0.054; P = 0.712). Conclusion In the normal kidney, noncontrast-enhanced SSFP MRI with spatially selective IR pulse can be used to assess renal corticomedullary differentiation and cortical thickness without the influence of aging, although optimal TI values for the best visualization of renal corticomedullary junction were shortened with aging. In the last 5 years, the multiparametric approach has been investigated as the method for the MRI of prostate cancer. In multiparametric MRI of the prostate, at least two functional MRI techniques, such as diffusion-weighted MRI (DW-MRI) and dynamic contrast-enhanced MRI, are combined with conventional MRI, such as T2 -weighted imaging. DW-MRI has the ability to qualitatively and quantitatively represent the diffusion of water molecules by the apparent diffusion coefficient, which indirectly reflects tissue cellularity. DW-MRI is characterized by a short acquisition time without the administration of contrast medium. Thus, DW-MRI has the potential to become established as a noninvasive diagnostic method for tumor detection and localization, tumor aggressiveness, local staging and local recurrence after various therapies. Accordingly, radiologists should recognize the principles of DW-MRI, the methods of image acquisition and the pitfalls of image interpretation.

Purpose: To evaluate the incidence and predictive factors of hypervascular transformation during follow-up of “high-risk nodules” detected in the hepatobiliary phase of initial Gd-EOB-DTPA-enhanced MRI in chronic liver disease patients. Materials and Methods: A total of 109 patients with chronic liver disease who underwent Gd-EOB-DTPA-enhanced MRI several times were investigated. Of these, 43 patients had 76 high-risk nodules with both hypointensity in the hepatobiliary phase and hypovascularity in the arterial phase of initial MRI. These nodules were observed until hypervascularity was detected. MRI and clinical findings were compared to assess the incidence and potential predictive factors for hypervascular transformation between the group showing hypervascular transformation and the group not showing hypervascularization. Results: The median observation period was 242.5 ± 203.2 days (range, 47–802 days). Overall, 24 of 76 high-risk nodules (31.6%) showed hypervascular transformation during follow-up (median observation period, 186.0 ± 190.3 days). The growth rate of the nodules (P < 0.001), the presence of fat within nodules (P = 0.037), and hyperintensity on T1-weighted images (P = 0.018) were significantly correlated with hypervascularization. Conclusion: Subsets of high-risk nodules tended to show hypervascular transformation during follow-up, with an increased growth rate, the presence of fat, and hyperintensity on T1-weighted images as predictive factors. Purpose: To assess whether noncontrast-enhanced steady-state free precession (SSFP) magnetic resonance imaging (MRI) with time-spatial labeling inversion pulse (Time-SLIP) can improve the visibility of corticomedullary differentiation of the normal kidney. Materials and Methods: A series of noncontrast-enhanced SSFP MRI with Time-SLIP were performed in 20 patients by using various inversion times (TIs); 500–1800 msec in increments of 100 msec. In-phase (IP) and opposed-phase (OP) MR images were also obtained. The signal intensity (SI) of the renal cortex and medulla was measured to calculate corticomedullary contrast ratio (SI of cortex/medulla). Additionally, the visibility of corticomedullary differentiation was visually categorized using a four-point scale. Results: In SSFP with Time-SLIP, corticomedullary contrast ratio was highest with TI of 1200 msec in eight subjects (40%), followed by 1100 msec in seven (35%) and 1000 msec in three (15%). The corticomedullary contrast ratio in SSFP with optimal Time-SLIP (4.93 ± 1.25) was significantly higher (P < 0.001) than those of IP (1.46 ± 0.12) and OP (1.43 ± 0.14). The visibility of corticomedullary differentiation was significantly better (P < 0.001) in SSFP images with Time-SLIP (averaged grade = 4.0) than in IP images (averaged grade = 2.63) and OP images (averaged grade = 2.05). Conclusion: SSFP MRI with Time-SLIP can improve the visibility of renal corticomedullary differentiation without using contrast agents. J. Magn. Reson. Purpose To evaluate the differences in enhancement of the abdominal solid organ and the major vessel on dynamic contrast-enhanced magnetic resonance imaging (DCE-MRI) obtained with gadolinium ethoxybenzyldiethylenetriamine pentaacetic acid (Gd-EOB-DTPA: EOB) and gadolinium diethylenetriamine pentaacetic acid (Gd-DTPA) in the same patients. Materials and Methods A total of 13 healthy volunteers underwent repeat assessments of abdominal MR examinations with DCE-MRI using either Gd-DTPA at a dose of 0.1 mmol/kg body weight or EOB at a dose of 0.025 mmol/kg body weight. DCE images were obtained at precontrast injection and in the arterial phase (AP: 25 seconds), portal phase (PP: 70 seconds), and equilibrium phase (EP: 3 minutes). The signal intensities (SIs) of liver at AP, PP, and EP; the SIs of spleen, renal cortex, renal medulla, pancreas, adrenal gland, aorta at AP; and the SIs of portal vein and inferior vena cava (IVC) at PP were defined using region-of-interest measurements, and were used for calculation of signal intensity ratio (SIR). Results The mean SIRs of liver (0.195 ± 0.140), spleen (1.35 ± 0.353), renal cortex (1.58 ± 0.517), renal medulla (0.548 ± 0.259), pancreas (0.540 ± 0.183), adrenal gland (1.04 ± 0.405), and aorta (2.44 ± 0.648) at AP as well as the mean SIRs of portal vein (1.85 ± 0.477) and IVC (1.16 ± 0.187) at PP in the EOB images were significantly lower than those (0.337 ± 0.200, 1.99 ± 0.443, 2.01 ± 0.474, 0.742 ± 0.336, 0.771 ± 0.227, 1.26 ± 0.442, 3.22 ± 1.20, 2.73 ± 0.429, and 1.68 ± 0.366, respectively) in the Gd-DTPA images (P < 0.05 each). There was no significant difference in mean SIR of liver at PP between EOB (0.529 ± 0.124) and Gd-DTPA (0.564 ± 0.139). Conversely, the mean SIR of liver at EP was significantly higher with EOB (0.576 ± 0.167) than with Gd-DTPA (0.396 ± 0.093) (P < 0.001). Conclusion Lower arterial vascular and parenchymal enhancement with Gd-EOB, as compared with Gd-DTPA, may require reassessment of its dose, despite the higher late venous phase liver parenchymal enhancement. Purpose To evaluate the diagnostic capability of breath-hold, multisection fluid-attenuated inversion-recovery (FLAIR) imaging using a half-Fourier acquisition single-shot turbo spin-echo (HASTE) sequence in combination with T2-weighted fast spin-echo (FSE) magnetic resonance (MR) sequences for small hepatic lesions found on CT in patients with malignancy. The purpose of this study was to evaluate visibility of the inferior phrenic arteries in normal subjects at thin-section, multiphasic, three-dimensional (3D) contrast-enhanced dynamic magnetic resonance (MR) imaging with fat suppression, and to compare the appearances and frequencies of MR visualization of these vessels between normal and cirrhotic patients. This study included 95 patients (44 normal and 51 cirrhotic patients) who underwent 3D contrast-enhanced dynamic imaging on a high-performance gradient (25 mT/m) system as a part of abdominal MR examinations. The right and left inferior phrenic arteries were visible in 84% and 73% of the normal subjects, respectively. The averaged rating for visibility in the right inferior phrenic artery was significantly greater in the cirrhotic patients than in the normal subjects (2.1 +/- 0.1 vs. 1.7 +/- 0.2; P = 0.040). Mean diameters of the right inferior phrenic artery in the cirrhotic patients (1.7 +/- 0.1 mm) were significantly larger (P = 0.002) than those in the normal subjects (1.3 +/- 0.1 mm). No significant difference was noted in the mean diameters and the visibility of the left inferior phrenic artery between the two groups. The inferior phrenic arteries can frequently be identified on thin-section, 3D contrast-enhanced arterial-phase dynamic MR images with fat-suppression techniques. Dilatation of the right inferior phrenic artery depicted by this technique may be a nonspecific but an additional secondary finding suggestive of cirrhosis.

The purpose of this study was to determine whether the finding of an enlarged hilar periportal space is a sign for early cirrhosis at magnetic resonance (MR) imaging. Forty-one pathologically proved cirrhotic patients in the early stage of disease who did not show conventional imaging findings of cirrhosis (early cirrhosis group) and 47 patients without history of chronic liver diseases (control group) were included in this study. MR images were qualitatively and quantitatively evaluated for the presence of enlargement of the periportal space. Enlargement of the periportal space was seen in 98% of patients in the early cirrhosis group, while this finding was seen in 11% of patients in the control group (P < 0.0001). The mean value of the hilar periportal fat thickness was significantly greater (P < 0.0001) in the early cirrhosis group (15.5 ± 6.2 mm) than in the control group (5.3 ± 3.1 mm). The sensitivity, specificity, accuracy, and positive predictive value of this finding for the MR diagnosis of cirrhosis with a cutoff value of 10 mm were 93%, 92%, 92%, and 91%, respectively. Enlargement of the hilar periportal space is a helpful sign at MR imaging in the discrimination between normal and early cirrhotic livers. J. Magn. Reson. The purpose of our study was to compare T2-weighted breath-hold fast spin-echo sequence (BHFSE) and high-resolution dynamic MR imaging (HR-DMRI) in the detection of hepatocellular carcinoma (HCC). Short and long T2-weighted BHFSE sequences and biphasic HR-DMRI including arterial-dominant and delayed phase images with a phased-array body coil were performed in 30 consecutive patients with 37 HCCs. The lesion-to-liver contrast-to-noise ratio (CNR) was quantitatively measured. The lesion conspicuity and delineation was qualitatively rated according to a four-point scale. The lesion-to-liver CNR was highest with the arterial-dominant phase HR-DMRI and was significantly higher than those obtained with both short and long T2-weighted BHFSE and those obtained with unenhanced and delayed HR-DMRI. The CNR obtained with short T2-weighted BHFSE was significantly higher than those obtained with long T2-weighted BHFSE and with unenhanced and delayed HR-DMRI. The sensitivity for the sequences was 78.4% (29/37) for short T2-weighted BHFSE, 67.6% (25/37) for long T2-weighted BHFSE, 37.8% (14/37) for unenhanced HR-DMRI, 97.3% (36/37) for arterial-dominant phase HR-DMRI, and 43.2% (16/37) for delayed HR-DMRI. The sensitivity of serial dynamic MR imaging combined with unenhanced, arterial-dominant phase imaging and delayed phase imaging was 100% (37/37). The score in the qualitative analysis of the lesion conspicuity and delineation was highest for the arterial-dominant phase HR-DMRI and was significantly higher than that for the short T2-weighted BHFSE. The score for the short T2-weighted BHFSE was significantly higher than that for the long T2-weighted BHFSE and that for the unenhanced HR-DMRI. Arterial-dominant phase HR-DMRI is superior to the T2-weighted BHFSE technique, and also HR-DMRI combined with unenhanced, arterial-dominant and delayed phases is the most sensitive technique in the detection of HCC.
