Background This study was designed to prospectively evaluate the efficacy of extracorporeal photopheresis (ECP) to attenuate the rate of decline of FEV1 in lung transplant recipients with refractory bronchiolitis obliterans. Due to an observed higher than expected early mortality, a preliminary analysis was performed. Study Design and Methods Subjects from 10 lung transplant centres were assigned to ECP treatment or to observation based on spirometric criteria, with potential crossover for those under observation. The primary endpoint of this study was to assess response to ECP (i.e., greater than a 50% decrease in the rate of FEV1 decline) before and 6 months after initiation of ECP. Mortality was also evaluated 6 and 12 months after enrolment as a secondary endpoint. Results Of 44 enrolled subjects, 31 were assigned to ECP treatment while 13 were initially assigned to observation on a non‐random basis using specific spirometric inclusion criteria (seven of the observation patients subsequently crossed over to receive ECP). Of evaluable patients, 95% of patients initially assigned to treatment responded to ECP with rates of FEV1 decline that were reduced by 93% in evaluable ECP‐treated patients. Mortality rates (percentages) at 6 and 12 months after enrolment was 32% and 41%, respectively. The most common (92%) primary cause of death was respiratory or graft failure. Significantly (p = 0.002) higher rates of FEV1 decline were observed in the non‐survivors (−212 ± 177 ml/month) when compared to the survivors (−95 ± 117 ml/month) 12 months after enrolment. In addition, 18 patients with bronchiolitis obliterans syndrome (BOS) diagnosis within 6 months of enrolment had lost 38% of their baseline lung function at BOS diagnosis and 50% of their lung function at enrolment. Conclusions These analyses suggest that earlier detection and treatment of BOS should be considered to appreciate improved outcomes with ECP.

Bluff Harbour, in southern New Zealand (Murihiku), is the southernmost location of Polynesian adze production. Here, an industry developed during the colonisation phase based on the exploitation of numerous sources of argillite - a fine-grained, homogeneous rock with conchoidal fracture properties. Argillite from the Bluff and Riverton sources was a major attractor during the earliest period of human occupation and Bluff argillite adzes were distributed widely throughout the lower South Island. The most extensive sources of Bluff argillite are on Colyers Island, where there is evidence of quarrying and preform production along much of the coast. The sites on Colyers Island were surveyed as the first part of an archaeological investigation of quarrying and adze production in Bluff Harbour. The quarried boulders, outcrops and artefact distributions at these sites indicated that the raw material was exploited by highly organised and skilled craft specialists. The initial stages of adze manufacture were undertaken at the quarry, and then preforms were taken to multipurpose camp sites around the harbour for finishing. Adze blanks on Colyers Island were made from large flakes, tabular blocks and cobbles, using imported hammer stones. This paper documents the significance of Colyers Island as an important adze production complex in New Zealand.

Small, remote islands, such as low coral atolls, with nutrient-poor, biogenic soils for food crops and vulnerability to extreme weather, have long been considered marginal environments for human habitation. Yet, four decades of archaeological research in the atoll archipelago of the Marshall Islands, eastern Micronesia have demonstrated sustained human occupation there for over two millennia. Here, we present a fine-grained analysis of mollusc remains from four recently excavated archaeological sites (4476 total MNI / 14843 total NISP) combined with mapping and analysis of extant benthic habitats, on Ebon Atoll, Marshall Islands. We examine spatial and temporal variability in mollusc foraging practices from prehistoric village sites and ephemeral camp sites across the windward–leeward exposure gradient. Our analysis demonstrates that foragers targeted a rich assemblage of taxa from different habitats, reflecting a foraging strategy that was adapted to local environmental conditions. Human foraging over 2000 years documented no observable human impacts to molluscs or nearshore intertidal marine ecosystems, challenging previous notions of atolls as marginal, exceptionally difficult settings for human habitation.

Rapa Nui is famous for its moai (statues) and ahu (platforms), yet research into the island's many basalt quarries, sources and workshops is limited. These geological and archaeological sites provided the raw materials for tools such as toki (adzes and picks), which facilitated the manufacture of Easter Island's iconic stonework. Other basaltic tools such as hoe (knives), ohio (axes) and mangai mā‘ea (stone fishhooks) served for subsistence practices. However, little is known about the sources of these artefacts, the sequence of their manufacture and their geochemical compositions. In this paper, we provide archaeological site descriptions and geochemistry of source material from the Rua Tokitoki quarry and a fine-grain basalt locality on Poike to contribute towards an understanding of prehistoric mining, social interaction and elite oversight on this isolated East Polynesian outpost.

Polynesian adze sourcing studies that rely on geochemical analyses to assign distant artefacts to a source or quarry have been undertaken for more than three decades. Understanding intra-source geochemical variability is essential for robust artefact source assignments, yet these data are not available for most quarries. We provide a seven-step protocol for sampling source rocks and associated artefacts at quarries and, using the large Eiao quarry (Marquesas Islands) as a case study, demonstrate the efficacy of the protocol. A "sampling to redundancy" statistical procedure documents that ∼7-11 samples are required to capture the variability of 10 oxides, 49 trace elements and seven isotope ratios. We advocate using the broadest array of fully quantitative geochemical values to characterise quarries and sources that will facilitate current and future sourcing studies. In the biological sense, this is analogous to the formal description of a holotype. Quarry samples should also be collected and curated to enable research when new protocols are established and more precise and accurate geochemical techniques develop.

We report our attempts to experimentally replicate 25 fine-grained basalt Hawaiian quadrangular adzes using freehand percussion. We document the techniques, stages and distinctive debitage resulting from each stage of the reduction process. While the later experimental adzes in our series reproduced the Hawaiian quadrangular adze with accuracy, we were unable to achieve the level of finesse seen in some finished archaeological specimens. Our experiments, and the gap between our own results and the finest examples seen in some archaeological collections, indicates the very high levels of skill needed to make the most refined Hawaiian adzes, and supports the notion that some adze makers were undoubtedly specialists. Our experimental work complements another study of archaeological adzes that examines the technological strategies and the cumulative addition of morphological elements to adzes during manufacture. We hope this study will serve as a useful benchmark for objectively and systematically comparing quadrangular adze technology across Hawai‘i and throughout Polynesia, leading to a better understanding of regional and temporal variation in adze technology.

Three fish bone identification protocols used for determining taxa composition for Pacific island archaeofaunal assemblages are evaluated. The protocols include using the following: (1) the most commonly identified five paired cranial bones and 'specials' or unique elements; (2) an expanded number of cranial bones; and (3) the less common inclusion of all vertebrae. Explicit identification and quantification protocols are outlined for systematically incorporating all vertebrae which, predictably, increases the number of identified specimens for an assemblage, thus providing more bones useful for reconstructing live fish biomass (weight and length). Significantly, a range of unique archaeological vertebrae are useful for calculating minimum number of individuals. Using a well-preserved assemblage from Henderson Island, Pitcairn Group, southeast Polynesia, numbering 6480 fish bones (concentration index=21580m), we demonstrate differences in rank-order abundance from three taxon identification protocols. For example, when using all vertebrae grouper (Serranidae) and surgeonfishes (Acanthuridae) are more numerically equivalent than when relying mostly on cranial bones for identification for minimum number of individuals and number of identified specimens. This has important implications for making comparisons between sites or across regions where different identification protocols were used. This pilot study demonstrates that using all vertebrae for taxon identification and quantification, not just unique hypurals (terminal vertebrae) or those from sharks and rays (Elasmobranchii), should be standard practice for identifying a greater number of bones to taxon and thereby providing better reconstructions of prehistoric fishing and subsistence practices in the Pacific.

Island archipelagos of the tropical coast of central Queensland include the most distant offshore islands used by Aboriginal Australians. Excavations on Collins, Otterbourne and High Peak Islands, located up to 40 km from the mainland, reveal evidence of offshore voyaging and marine specialisation in the Shoalwater Bay region for at least 5200 years. A time lag of up to 3000 years between island formation and systematic island use may reflect delayed development of key marine resources. Expansion of island use commencing around 3000–3500 years ago is linked to population increases sustained by synchronous increases in marine resources. Occupational hiatuses variously between 1000 and 3000 years ago are associated with increased ENSO activity. Intensified island use within the past 1000 years is primarily a social phenomenon associated with continuing demographic pressures and the development of more coastally and marine-focused mainland groups, with settlement patterns increasingly encompassing adjacent islands. The viability of risky offshore canoe voyaging was underwritten by two key high-return subsistence pursuits – hunting green turtles and collecting turtle eggs. In addition to subsistence and quartz quarrying, a key motivation for island visitation may have been socially restricted (e.g. ceremonial) practices.

Purpose Clostridium difficile infection (CDI) rates have been rising in recent years. We aimed to characterize CDI in lung transplant recipients in the modern era and hypothesized that CDI would increase the mortality risk. Methods We performed a retrospective chart review of patients undergoing transplantation at our center from 1/2006 to 7/2011. Attributes of CDI+ and CDI− groups were compared using Student's t- and chi-square tests (α = 0.05). Multivariate Cox proportional hazard models were used to control for confounding factors. Results Overall CDI incidence was 22.5%. Seven of 151 patients (4.6%) developed CDI during the initial hospitalization after transplantation (mean time 10.6 ± 6 d) while 27 patients (19.7%) developed CDI after discharge (mean time 467 ± 471 d). Incidence rate was 224.6 cases/100 000 patient-days compared to 110 cases/100 000 patient-days (rate for entire hospital). CDI was not predictive of mortality (HR 2.06, 95% CI 0.94–4.52). Conclusion CDI rates in lung transplant recipients are high in the modern era. No risk factors for CDI were identified. Although not statistically significant, CDI+ patients had a higher risk of death. The economic burden of CDI and trend toward worse outcomes for CDI patients have important implications for post-operative surveillance of CDI-related complications and need for CDI prophylaxis.

Abstract: Introduction: Pulmonary insufficiency following bone marrow transplant (BMT) is common and has significant associated mortality. Lung transplantation (LTX) is the only viable treatment for patients with end-stage pulmonary disease, but LTX after BMT is an uncommon event given the medical candidacy of the potential recipients. We sought to evaluate the short- and long-term outcomes of LTX in BMT recipients. Methods: We performed a retrospective evaluation of our institution’s longitudinal LTX and BMT databases. Demographic and outcomes variables were collected. Results: We identified 639 LTX from January 1, 1988, through December 31, 2009, and 5525 BMT from program inception, March 21, 1974, through December 31, 2009. From the cross-referenced cohort, we identified four patients who had BMT followed by LTX. Our series was composed of two men and two women, with a mean age of 32.3 yr (range, 20–59 yr). Single LTX were performed in two recipients (50%). All patients had significant and expected morbidities related to their transplant immunosuppression. Three patients (75%) required cardiopulmonary bypass at the time of LTX. The two recipients who underwent bilateral LTX required open chest management and subsequent tracheostomy. All patients are still alive at follow-up (range, 19–119 months, median 39.5). Conclusion: Our study demonstrates that LTX in the setting of BMT is a high-risk operation with the potential for a tumultuous perioperative course. Despite this, good outcomes and survival are obtainable in carefully selected patients. Selection factors include clinically stable patients without active sepsis and preoperative optimization of nutrition in anticipation of a prolonged recovery. An experienced multidisciplinary team approach and a protocol-driven management plan are paramount for successful outcomes in this challenging population.

Recent observations of fishing, the ethnohistoric literature, the archaeological fishing tool kit, marine environments adjacent to the site, and the fish bone assemblage were considered to understand fishing strategies on the makatea island of Rurutu, Austral Islands, French Polynesia. Excavations totalling 53.5 m(2) at the Peva dune site (ONI) were conducted in 2003. The sandy, calcareous deposits from Area 2 (33 m(2)) were dry sieved through 3.2 mm mesh and 5,011 fish bones weighing 2,229.7 g were retained for analysis. Two distinct cultural layers were identified. Archaic period layer D had 20 fish families inventoried from a total of 141 minimum numbers of individuals (MNI) and 1,081 numbers of identified specimens (NISP). Average bone weight was 0.42 g and median vertebra width between 5-6 mm (n=747). The Classic period layer A, associated with a marae complex, contained only seven fish families, a MNI of 24 and NISP of 403. Average bone weight was 0.63 g and median vertebra width between 10-11 mm (n=107). While a broad spectrum fish capture strategy is inferred for the Archaic, selective larger fish, including an order of magnitude increase in shark, were likely prestige items used in ritual offerings during the Classic period. Comparisons of the archaeological assemblages from five makatea islands show that in all but one case, sites are dominated by groupers, unlike many other Pacific island sites where parrotfish are most frequent. This, alone, might be the unique signature of makatea assemblages.

Abstract: Purpose: Severe primary graft dysfunction (PGD) is the major early problem following lung transplantation. Aprotinin, a serine protease inhibitor, has many anti-inflammatory properties that might reduce or prevent lung injury. Our hypothesis was that the incidence of PGD could be reduced by a combination of donor lung perfusion and systemic administration of aprotinin to recipients. Methods and Materials: The study was randomized and placebo controlled. Donor lungs were perfused during procurement with 4 L Perfadex containing aprotinin (280 mg load + 70 mg/hL) or placebo. Aprotinin or placebo was also administered peri-operatively to the recipients. The study was powered to detect a 10% improvement in the primary endpoint of developing ISHLT grade III PGD anytime within 48 hr following the transplant procedure. Results: There were 48 patients randomized. Diagnosis and the use of bypass were different between groups. The study was stopped prematurely at the planned interim analysis point because of published concerns about renal toxicity of aprotinin. There was no difference in the occurrence of the primary endpoint between groups of patients. The median change from the baseline creatinine level at 24, 48, 72 hr; 7 and 30 d following the transplant was not associated with the administration of aprotinin. Conclusions: There was no statistically significant difference in the incidence of the primary endpoint between groups in the study. Excess renal failure related to aprotinin administration in a patient population at high risk for the event was not observed.

Archaeologists rely on the spatial and temporal distribution of artifacts and other site-based materials to understand the stratigraphic integrity of the matrix in which remains are embedded. Although they are aware of taphonomic and site formation processes that can cause postdepositional movement of objects, misinterpretation can occur. We used high-precision 230 Th dating of branch corals found throughout cultural layers of a coastal Hawaiian midden to identify the effects of post-depositional disturbances to the archaeological record. Fifteen corals distributed in three cultural layers of a Mo’omomi bay site on west Moloka’i, Hawaiian Islands, were 230 Th dated between A.D. 1513 and A.D. 1623. Even though the cultural layers appeared visually intact, the positions of the dated coral samples indicate stratigraphic mixing as there is no positive age–depth correlation. Consequently, all cultural layers should be considered one analytical unit for analysis of contents. This study is applicable to other Pacific archaeological sites, especially throughout Hawaii and East Polynesia generally, that have well-preserved branch coral for 230 Th dating. © 2010 Wiley Periodicals, Inc.

Excavations at Mask Cave on the sacred islet of Pulu off Mabuyag in the central west of Zenadh Kes (Torres Strait) reveal four occupational phases: Phase 1 (2900-3800 years ago), Phase 2 (2100-2600 years ago), Phase 3 (1500-1700 years ago) and Phase 4 (last 1500 years). Faunal remains indicate marine specialization (turtle and fish) during all phases. Petrographic analysis of sherds of finely made red-slipped pottery dating back 2400-2600 years reveals a unique fabric in terms of current understandings of Oceanic ceramic technologies. Mineral inclusions are consistent with local geology suggesting local manufacture and the existence of Indigenous Australia's first pottery tradition. Pre-ceramic Phase 1 is associated with demographic expansions across the western islands of Zenadh Kes by local populations of marine-based hunter-gatherers who were primarily Aboriginal language speakers. Phase 2 is associated with the immigration of Papuan maritime, horticultural and pottery-making peoples to the eastern and western islands of Zenadh Kes commencing 2600 years ago. Australian then Papuan settlement expansions across the western islands of Zenadh Kes explain why the local Western-Central Language has an Aboriginal base with a Papuan overlay. First colonization of the eastern islands by Papuans explains why the local Meriam Mir language is Papuan. Early red-slipped pottery in Zenadh Kes is linked to southern coastal Papuan pottery traditions that are reassessed to have a comparable 2600 year antiquity. Papuan settlement of the southern Papuan coast and Zenadh Kes was an extension of the post-Lapita settlement of the Pacific, an event memorialized in part by Torres Strait Islander oral tradition.

Despite images of paradise, eastern Polynesian islands were devoid of most economically-useful animals and cultigens when colonised by humans about 1500 years ago; other views on chronology are more conservative. We analyse the faunal material from the 1959 excavations by Green in Mangareva, southeast Polynesia where 13,598 bones (NISP) were identified to nearest taxon. We discuss the significance of the purposely introduced chicken or Pacific Jungle Fowl (Gallus gallus), dog (Canis familiaris) and pig (Sus scrofa) not known from the historic records of that island group as well as the Pacific rat (Rattus exulans) and human dental and skeletal material. The dog remains now represent the most eastern limit of this species known prehistorically for Oceania. Many of the pig remains were associated with a marae (religious edifice) confirming the importance of this animal in its ceremonies. The majority of fractured human bones and teeth were recovered from midden contexts, thus alluding to the possibility of cannibalism as reported in late prehistoric oral traditions. We suggest that rats, and not human predation, were responsible for the early local extinction of the chicken in the prehistoric sequence for Mangareva.

The prehistoric settlement pattern and economy of Utrōk Atoll, situated near the northern limit of permanently-inhabited atolls in the Marshall Islands, is described and interpreted from a complete intensive survey and excavations totalling 64 m2 (70.45 m3). Sites range from the most expansive, 25 hectare habitation and horticultural system on the largest islet, to smaller, temporarily-occupied habitations situated on diminutive islets. A suite of 16 radiocarbon age determinations suggests human occupation of the atoll by the first few centuries AD (Beta-103903). Four additional dating samples, collected from emerged beachrock, determined when the islets adjusted shape in response to changing sea levels and coastal processes. Some 173 prehistoric tools and ornaments included 39 shell adzes, pumice abraders, pearl shell scrapers, pounders and hammerstones, a trolling lure point, a single piece fishhook point and ornaments such as Conus shell rings, Spondylus beads and disks, pendants and a small drilled whale tooth. Of the 13,545 bones, 88.6% were of fish, mostly inshore taxa, with dog (Canis familiaris), Pacific rat (Rattus exulans), sea turtle (cf. Chelonia mydas), lizard and bird (Puffinus, Phaethon, Sula, Fregata, Egretta and Heteroscelus), as well as three human burials. Some 45.7 kg of shellfish representing 34 families was dominated by bivalves (67%) of mostly Tridacnidae. The Bullmouth helmet, Cypraecassis rufa, was extirpated as a result of human predation, although the archaeological evidence suggests that low human populations on this relatively dry atoll had little noticeable impact on the marine ecosystem, while the terrestrial landscape was markedly altered.

Respiratory infections remain a significant cause of morbidity and mortality after lung transplantation. In addition to cytomegalovirus, the community respiratory viruses such as respiratory syncytial virus (RSV), parainfluenza virus (PIV), influenza virus, and adenovirus, are important causes of infection in transplant recipients, often involve the lower respiratory tract, and may be associated with significant morbidity and mortality. In this review, we summarize the current state of knowledge regarding the epidemiology, clinical manifestations, diagnosis, treatment and outcomes associated with RSV, PIV, influenza virus, and adenovirus infections in lung transplant recipients.

The cultivation of aroids (herbaceous plants with starchy corms) is the foundation of Oceanic societies, yet the study of prehistoric atoll agriculture (utilizing Cyrtosperma chamissonis) has been almost totally neglected. Aroid pit agricultural features, some measuring up to 100 m long and 20 m wide, excavated in the Marshall Islands (center ca. 8°N latitude, 170°E longitude) provide the first chronometric dates (1910 ± 70 B.P., Beta-79576) for this type of cultivation practice associated with coral atolls found throughout the Pacific. Excavations through an aroid pit cultivation pit rim identified a stratigraphic sequence beginning with the sterile subsoil, an A horizon deeply buried under pit spoil dirt, and a prehistoric midden deposit beginning below the surface A horizon. Granulometric analysis of sediments and identification of foraminifers documented the nearshore lagoon as the source for all non-cultural sediments. Anthropophilic land snails (Gastrocopta pediculus and Lamellidea pusilla) in the dated, buried A horizon is a firm basis for confirming the presence of humans near initial colonization (ca. 2000 B.P.) and anchors the culture-historical sequence for the long-term study of human impacts to low coral islands. Consequently, on-going analyses of plant opal phytoliths, starch grains, and charcoal from the buried A horizon, should document the nature of early atoll ecology prior to significant human modification. As aroid pit construction is associated, in many examples, with traditional property boundaries, detailed mapping and dating of these cultivation systems should relate to changes in land tenure and prehistoric social organization.

Electronic spirometry units were used to monitor lung transplantation recipients upon their return home. The data from 77 participants were used to develop methods to verify that the pulmonary function measurements, forced vital capacity (FVC) and forced expiratory volume in 1 s (FEV1), were reliable and valid. The standard deviation was calculated for the best daily effort on consecutive days of home spirometry. An acceptable upper limit for the standard deviation, as the measure of day-to-day reliability, was 0.20 for FVC and 0.15 for FEV1. Validity was determined by examining the mean difference (bias) between the spirometry done in the pulmonary function laboratory and the home monitoring results. The clinic values were slightly higher, with an average difference of 0.15 for FVC and 0.12 for FEV1. Therefore, the home spirometry measurements have a high degree of reliability and validity and can now be used for early detection of serious complications.

Recent discovery of a prehistoric archaeological site at Emily Bay, Norfolk Island, enables the question of settlement origins to be revisited. Analysis of a sample of basalt flakes by non-destructive, energy-dispersive XRF analysis indicates that there was local adze production, not merely refurbishment, suggesting that quarry and reduction sites might yet be discovered. Analysis of an obsidian blade by major elements and PIXE shows that it originated in a Raoul Island (Kermadecs) source. It is hypothesized that the colonisation history of the Norfolk group belongs with those of other temperate archipelagoes (Kermadecs, Chathams, Snares) which appear to have originated in New Zealand rather than tropical Polynesia.

The Tangatatau Rockshelter (site MAN-44, Mangaia, Cook Islands) has produced one of Eastern Polynesia's most comprehensive chrono-stratigraphic sequences of artifacts, vertebrate and invertebrate fauna, and botanical materials. Two seasons of excavation exposed 29 m2 out of an estimated total floor area of 225 m2. A suite of 30 radiocarbon age determinations indicates that human use of the shelter spanned the period from ca. 1000 to 1700 cal AD. This paper outlines the major temporal trends in the artifact, faunal, and paleoethnobotanical assemblages recovered from the site, and discusses these in terms of the development of classic Mangaian society, an exemplar of Polynesian ‘Open’ chiefdoms.

We propose a strategy for evaluating the contribution of human predation to the depletion of endemic birds on Pacific islands. Humans reduced avian populations indirectly through habitat alteration, for which there is much archaeological evidence. However, direct evidence of human predation and its role in avian extinctions has seldom been critically evaluated. Because many archaeological sites containing bird bones occur in nesting locales such as coastal dunes and rockshelters, distinguishing naturally from culturally deposited material is crucial. We address this problem by analyzing bird bones from nine archaeological sites on Moloka'i, Hawaiian Islands. Species composition, weathering, non-human predation, burning, element frequencies, and modification are considered. Similarity between archaeological and replicated breakage suggests human consumption of some extirpated bird species. We couple these results with converging, independent lines of evidence to identify human predation in avian assemblages from Moloka'i. We stress that determining the chronology of avian extinctions is best accomplished by directly dating bird bones with butchering marks and other evidence of human consumption.

The role of exchange in Hawaiian prehistory has received only minimal attention despite Hawai'i being one of the most highly stratified societies in Oceania. This paper demonstrates the utility of provenance studies of volcanic glass as a means of providing an empirical grounding to addressing exchange and interaction in Hawaiian prehistory. Volcanic glass studies in Hawaiian archaeology are reviewed, geologic sources of the material described. and a geologically-based model formulated to guide the discovery of volcanic glass sources on West Moloka'i, Hawaiian Islands. The results of a 19.000 hectare survey are reported and the first two sources of volcanic glass on Moloka'i are described. Using an electron microprobe, the geochemical similarity of source material and volcanic glass artifacts from distant habitation sites is demonstrated. It is concluded that sources of volcanic glass can be discovered in a systematic way; microprobe analysis is a good technique for geochemical characterization; and, with additional work, hypotheses of exchange and interaction in Hawaiian prehistory can be tested with a systematically collected, empirical data base.
