Objective To describe and elucidate the time trends of the academic productivity of NHLBI's obesity-related research funding via bibliometric analysis of 30 years of NHLBI-supported obesity-related publications. Methods In total, 3,545 NHLBI-funded obesity-related publications were identified in the Thomson Reuters InCites™ database. Shared references in a community detection algorithm were used to identify publication topics. Characteristics of publications and topical communities were analyzed based on citation count and percentile rank. A percentile rank >90 was considered “highly cited.” Results Obesity-related publications increased more than 10-fold over 30 years, whereas NHLBI-funded publications only increased twofold NHLBI-funded obesity publications were cited a median of 23 times (IQR 8-55, range 0-2,047, mean 52). Thirty percent of these publications were highly cited compared to the expected ten percent. Six topical communities were present in 1983 compared to 16 in 2013. The most highly cited topical areas were sleep (n = 199 publications, 38% highly cited), cardiovascular morbidity and mortality (n = 277, 36%), obesity correlates and consequences (n = 588, 35%), and asthma and inflammation (n = 283, 35%). Conclusions NHLBI-funded obesity publications have contributed substantially to the obesity literature, with many highly cited. Publications grew in number and topical diversity over 30 years and grew at a faster rate than total NHLBI publications.

Comparative effectiveness research (CER) is the study of existing treatments or ways to deliver health care to determine what intervention works best under specific circumstances. CER evaluates evidence from existing studies or generates new evidence, in different populations and under specific conditions in which the treatments are actually used. CER does not embrace one research design over another but compares treatments and variations in practice using methods that are most likely to yield widely generalizable results that are directly relevant to clinical practice. Treatments used in transfusion medicine (TM) are among the most widely used in clinical practice, but are among the least well studied. High-quality evidence is lacking for most transfusion practices, with research efforts hampered by regulatory restrictions and ethical barriers. To begin addressing these issues, the National Heart, Lung, and Blood Institute convened a workshop in June 2011 to address the potential role of CER in the generation of high-quality evidence for TM decision making. Workshop goals were to: 1) evaluate the current landscape of clinical research, 2) review the potential application of CER methods to clinical research, 3) assess potential barriers to the use of CER methodology, 4) determine whether pilot or vanguard studies can be used to facilitate planning of future CER research, and 5) consider the need for and delivery of training in CER methods for researchers.

OBJECTIVES: To evaluate the prognostic value in older adults of two predictors of mortality: impaired functional capacity and an attenuated heart rate recovery. SETTING: Academic medical center. DESIGN: Prospective study with mean 3.7 years follow-up. PARTICIPANTS: Seven thousand three hundred fifty-four adults aged 65 and older consecutively referred for exercise testing between 1990 and 1999. Patients with heart failure, valvular disease, atrial fibrillation, and pacemakers were excluded. MEASUREMENTS: The primary endpoint was all-cause mortality. Impaired functional capacity was defined as the peak exercise workload in the lowest quintile of metabolic equivalents achieved according to prespecified strata of age and sex. Heart rate recovery was defined as the fall in heart rate during the first minute after exercise and was abnormal if 12 or fewer beats per minute, except for patients undergoing stress echocardiography, in which case 18 or fewer beats per minute was abnormal. RESULTS: There were 842 deaths. Patients with impaired functional capacity were at increased risk for death (23% vs 9%, hazard ratio (HR) = 2.7, 95% confidence interval (CI) = 2.2–3.1, P < .0001) as were patients with an abnormal heart rate recovery (17% vs 9%, HR = 2.0, 95% CI = 1.8–2.3, P < .0001). After adjusting for age, sex, coronary history, and other confounders, impaired functional capacity (adjusted HR = 2.1, 95% CI = 1.8–2.4) and an abnormal heart rate recovery (adjusted HR = 1.5, 95% CI = 1.3–1.7) independently predicted death. No interactions between these two variables with age were noted. CONCLUSIONS: In older patients, impaired functional capacity and heart rate recovery were independent predictors of death.

Objective To describe mortality rates, causes of death, and potential mortality risk factors in pediatric rheumatic diseases in the US. Methods We used the Indianapolis Pediatric Rheumatology Disease Registry, which includes 49,023 patients from 62 centers who were newly diagnosed between 1992 and 2001. Identifiers were matched with the Social Security Death Index censored for March 2005. Deaths were confirmed by death certificates, referring physicians, and medical records. Causes of death were derived by chart review or from the death certificate. Standardized mortality ratios (SMRs) and 95% confidence intervals (95% CIs) were determined. Results After excluding patients with malignancy, 110 deaths among 48,885 patients (0.23%) were confirmed. Patients had been followed up for a mean ± SD of 7.9 ± 2.7 years. The SMR of the entire cohort was significantly decreased (0.65 [95% CI 0.53–0.78]), with differences in patients followed up for ≥9 years. The SMR was significantly greater for systemic lupus erythematosus (3.06 [95% CI 1.78–4.90]) and dermatomyositis (2.64 [95% CI 0.86–6.17]) but not for systemic juvenile rheumatoid arthritis (1.8 [95% CI 0.66–3.92]). The SMR was significantly decreased in pain syndromes (0.41 [95% CI 0.21–0.72]). Causes of death were related to the rheumatic diagnosis (including complications) in 39 patients (35%), treatment complications in 11 (10%), non-natural causes in 25 (23%), background disease in 23 (21%), and were unknown in 12 patients (11%). Rheumatic diagnoses, age at diagnosis, sex, and early use of systemic steroids and methotrexate were significantly associated with the risk of death. Conclusion Our findings indicate that the overall mortality rate for pediatric rheumatic diseases was not increased. Even for the diseases and conditions associated with increased mortality, mortality rates were significantly lower than those reported in previous studies.
