BACKGROUND Cord blood adiponectin and leptin concentrations are associated with birth weight and adiposity. Birth size and rate of infant weight gain are associated with future obesity risk. However, it is unclear whether biomarkers reflecting the intrauterine environment are predictive of infant prospective body composition change. OBJECTIVES To examine whether cord blood adiponectin and leptin are predictive of neonatal adiposity and fat mass (FM) accrual to 3 months of age. METHODS Participants (n = 36) were healthy African American infants. Leptin and adiponectin concentrations were measured in umbilical cord blood. At 2 weeks and 3 months, infant body composition was assessed via air displacement plethysmography. Weight-for-length z-scores (WLZ) were calculated using World Health Organization standards. Multiple linear regression was used to examine associations of cord blood adiponectin and leptin with birth WLZ; WLZ, FM and fat-free mass at 2 weeks, and the conditional change in these variables from 2 weeks to 3 months (body composition at 3 months adjusted for body composition at 2 weeks). OBJECTIVES To compare the accuracy of 2-dimensional (2D) and 3-dimensional (3D) fetal measurements for prediction of birth weight Z score and neonatal adiposity (percent body fat) in the setting of suspected fetal macrosomia. METHODS We conducted a prospective observational study of term singleton pregnancies with suspected macrosomia. Patients were enrolled on admission to labor and delivery and underwent sonographic examinations. Within 48 hours of delivery, neonatal anthropometric measurements were obtained. RESULTS Thirty-four neonates were included in the analysis. Mothers were very obese (mean body mass index ± SD, 39.1 ± 7.8 kg/m(2)); 56.5% were white; and 39.1% had diabetes. Neonates were 38% female and had a mean birth weight of 3940.0 ± 496.8 g, percent body fat of 18.5% ± 4.0%, and Ponderal index of 2.8 ± 0.3 g/cm(3). Mean 2D estimated fetal weight was 3973 ± 443 g; mean 3D estimated fetal weight was 3803 ± 528 g; and mean thigh volume was 102.5 ± 19.6 cm(3). Both 2D and 3D measurements accounted for about half the variance in predicted birth weight (R(2) for 2D = 0.53, 71% within 10% of birth weight; R(2) for 3D = 0.47, 65% within 10% of birth weight). Thigh volume Z score was the prenatal parameter most highly correlated with both birth weight Z score (R(2) = 0.52; r = 0.72; 95% confidence interval, 0.54-0.84; P < .001) and percent body fat (R(2) = 0.22; r = 0.47; 95% confidence interval, 0.17-0.69; P = .04). CONCLUSIONS In our population of fetuses with suspected macrosomia, fractional thigh volume was the best sonographic estimate of neonatal percent body fat and birth weight Z score. Future research on prediction of neonatal weight and adiposity in macrosomic fetuses should include an estimate of fetal soft tissue given the generalized increase in body fat of these fetuses.

Methods: Fifty-three term infants were evaluated at birth, three times during infancy and in mid-childhood. Logistic regression was used to determine associations between rates of total weight gain, fat mass gain and lean mass gain during infancy and later overweight/obesity (defined as body mass index [BMI] ≥85th percentile), adjusted for birth weight and parent education. Results: At follow-up (age 9.0 ± 1.8 years), 30% were overweight/obese. More rapid total weight gain from 0 to 4 months was associated with twofold odds (odds ratio [OR] 1.98, 95% confidence interval [CI] 1.05–3.74, P = 0.04) of overweight/ obesity in mid-childhood. From 0 to 8 months, more rapid weight gain was associated with nearly fivefold odds (OR 4.76, 95% CI 1.05–21.5, P = 0.04), and more rapid fat mass gain was associated with eightfold odds (OR 8.03, 95% CI 1.11–58.2, P = 0.04) of later overweight/obesity.

Objective Increased subcutaneous adipose tissue is a well known characteristic of diabetic fetopathy. Prenatal estimation of adipose tissue can be performed by ultrasound, while postnatally skinfold measurements are performed using a Holtain caliper. The aim of this study was to compare these methods in the same patients. Methods This was a prospective study of 172 pregnant patients (142 controls and 30 with gestational diabetes) at ≥ 37 gestational weeks. In addition to fetal weight estimation, fetal subcutaneous tissue was measured at the anterior abdomen lateral to the umbilicus (SonoSfAbd) and at the middle of the femur (SonoSfFem). Within 72 h after delivery, a Holtain caliper was used to measure neonatal skinfold thickness at the left anterior iliac spine (SfAbd), at the lower angle of the left scapula (SfSca), at the middle of the femur, above the left quadriceps femoris (SfFem) and at the middle of the left triceps (SfHum). Ultrasound and mechanical measurements were correlated. Results The sonographic and mechanical methods showed good correlation with each other. Linear regression analysis gave the following equations: SfAbd (mm) = SonoSfAbd (mm) × 0.489 + 1.988 (r2 = 0.34, P < 0.001); SfSca (mm) = SonoSfAbd (mm) 0.457 + 2.043 (r2 = 0.40, P < 0.001); SfFem (mm) = SonoSfFem (mm) × 0.714 + 1.763 (r2 = 0.41, P < 0.001); SfHum (mm) = SonoSfFem (mm) 0.564 + 2.09 (r2 = 0.39, P < 0.001). Conclusions Ultrasound examination is a reliable method for non-invasive intrauterine measurement of fetal subcutaneous tissue and can be used to predict mechanical neonatal skinfold thickness measurements. Obesity's increasing prevalence has reached epidemic proportions in the USA, with close to one-third of the adult population affected in 2000. Additionally, there is increasing prevalence of obesity in other industrialised areas of the world such as Europe. Of potentially more concern is the potential risks associated with obesity and related metabolic complications in the developing world. The maternal, fetal, peripartum and neonatal complications of obesity in pregnancy have far-reaching implications for both mother and offspring. Of alarming interest is the increasing rate of obesity among adolescents and the cycle of obesity in future generations it portends. The purpose in this review is to briefly review the maternal perinatal morbidities associated with maternal pregravid obesity. Additionally, we will review evidence of both short- and long-term effect of maternal obesity on the in utero environment as it relates to fetal growth, neonatal body composition and adolescent obesity.

Objective To evaluate levels of 24-hour urine insulin excretion before the onset of pre-eclampsia and gestational hypertension. Design Nested case–control study within the Calcium for Preeclampsia Prevention (CPEP) study cohort. Setting Five university medical centres in the United States. Sample Cases had developed pre-eclampsia (n= 70) or gestational hypertension (n= 142) in the absence of gestational diabetes. Controls (n= 429) had remained normotensive without gestational diabetes. Methods Subjects were required to have had an adequate baseline 24-hour urine collection prior to CPEP enrolment at 13–21 weeks. Controls were matched to cases by enrolment site and specimen storage time, without regard to gestational age or CPEP treatment. Adjusted mean 24-hour urine insulin excretion was, however, calculated using analysis of covariance, with adjustment models for pre-eclampsia considering body mass index, race and smoking status; and for gestational hypertension, gestational age at specimen collection, height, body mass index and smoking. Urine insulin was measured by radio-immunoassay. Main outcome measures Twenty-four-hour urine insulin excretion. Results Adjusted 24-hour urine insulin excretion at baseline (mean 17 weeks of gestation) was greater in women who developed pre-eclampsia than in normotensive controls (mean [SE]: 15.6 [1.5] vs 13.1 [1.2] × 103μIU/24 hour, P= 0.06), but not in women who developed gestational hypertension (14.7 [0.9] vs 15.0 [0.6] × 103μIU/24 hour, P= 0.79, in cases vs controls). Among women who developed pre-eclampsia, adjusted urine insulin excretion was greater than controls only in women with mild pre-eclampsia and not in severe pre-eclampsia (mild pre-eclampsia vs controls: 17.3 [2.0] vs 13.7 [1.6] × 103μIU/24 hour, P= 0.04; severe pre-eclampsia vs controls: 12.3 [2.2] vs 11.5 [1.2], P= 0.69). Conclusion The data suggest that early hyperinsulinaemia, a marker of insulin resistance, may predispose to mild pre-eclampsia.

Maternal obesity affects 1 in 5 pregnant women in the United States1. Maternal obesity is associated with increased circulating total, but not fetal, cf DNA2. This may be a result of increased production or decreased clearance of cf DNA in obese women. It is more likely that this is due to increased production of total cfDNA, because decreased clearance would also likely lead to an increase in cell-free fetal cfDNA. In a prior study performed on obese pregnant women, we showed that active remodeling of adipose tissue via adipocyte necrosis and/or apoptosis of the stromal vascular fraction results in an increased release of cfDNA of maternal origin into the circulation3. The focus of the prior study was on the mechanisms underlying the release of the cfDNA. In this study we more closely examined the correlation between maternal weight and total DNA levels. This study was approved by the Institutional Review Boards at Tufts Medical Center and Metrohealth Medical Center. The samples are from the same cohort reported previously3, but the analysis is different. Briefly, sixteen obese (mean=39.2; pre-gravid BMI range 31–51) and 14 lean (mean 21.8; pre-gravid BMI range 17–24) women carrying male fetuses and 10 women carrying female fetuses (negative controls) were recruited at term (37–40 weeks) prior to an elective cesarean section. Written informed consent was obtained prior to obtaining the samples. Women with a multiple gestations, placenta previa or invasive placentation, labor, infection, fetal anomalies or aneuploidy, intrauterine growth restriction, or preeclampsia were excluded. Maternal peripheral venous blood was collected at MetroHealth Medical Center on admission to labor and delivery, prior to placement of an intravenous line for hydration. All subjects had the same instructions prior to admission and had nothing to eat or drink for 6–8 hours prior to the blood draw. The blood was collected in an EDTA tube and plasma was separated by centrifugation and kept frozen at −20°C prior to being shipped to Tufts Medical Center for further analysis. DNA was extracted from 400 uL of plasma using the QIAamp DNA Blood Mini Kit (Qiagen, Valencia, CA) according to the blood and body fluid protocol. DNA was eluted in 50 μL of the elution buffer. Real time quantitative PCR amplification was performed as previously described4 to amplify glyceraldehyde-3-phosphate dehydrogenase (GAPDH) and the Y chromosome sequence DYS14 as markers of total and fetal DNA, respectively. All samples were analyzed in triplicate. Analysis was blinded, and a female processed and handled all samples so that there was no risk of contaminating samples with male DNA. Conversion of raw PCR data to genome equivalents per mL of plasma was performed using the methods of Lo et al.5 The levels of total and fetal cf DNA in plasma from lean and obese women were compared using the t test. In addition, the maternal plasma volume from both obese and lean subjects was adjusted for blood volume6 and compared using the t test, as prior experiments in our laboratory suggested an increased blood volume as a function of weight7. Finally, we performed a regression analysis between total and fetal cf DNA and maternal BMI. Analysis of the raw data from all subjects showed that there was a 1.7% increase in total cell-free DNA per BMI unit (kg/m2). Following adjustment for blood volume, there was a 3.2% increase per BMI unit (Table 1). When the subjects were categorized into lean and obese groups, however, there was a decrease in total cf DNA per BMI unit in lean women and an increase in obese women (Table 1 and Figure 1). Neither of these changes were statistically significant. However, this lack of significance may be a function of the small sample size. Further studies with more subjects are warranted to further characterize this relationship between cf DNA and BMI. Table 1 Change in maternal cell-free DNA by body mass index In this study, we found a correlation between BMI and total DNA levels. In lean women, the decrease in total cf DNA likely reflects a dilutional effect seen in all pregnant women due to increasing plasma volume. Conversely, in obese women, the increased levels of total cf DNA may reflect the increased adipocyte necrosis3 and stromal vascular apoptosis that is significant enough to overcome the dilutional effect that occurs in pregnant women. Our results are similar to a previous study by Lapaire et al.2, in which a correlation was found between maternal BMI and total, but not fetal, second trimester cf DNA levels. There are several differences between our study and the Lapaire study, however. Firstly, the gestational ages of the study subjects were different. In the present study, maternal blood samples were drawn at term, in contrast to 20 to 21 weeks. Secondly, in the Lapaire et al. study there was no adjustment for maternal weight. We adjusted for the effect of maternal weight on blood volume using the method described by Lemmens et al.6 The present study and that of Wataganara et al.7 suggest that a correction factor for maternal weight is needed when cf DNA analyses are performed. It is standard practice to correct for maternal weight when performing serum screening for Down syndrome. Diagnostic laboratories typically derive their own regression curves for the relationship between serum analytes and maternal weight because of several factors, including differences in instrument sensitivity in the measurement of serum analytes, as well as differences in maternal weight distribution among centers8. Review of the literature, however, indicates that there is no consensus as to the best method to adjust serum markers, including cf nucleic acids, for maternal weight in cases of extreme obesity. We selected the Lemmens et al.6 formula because it correlated total blood volume with BMI, although that formula was used for quantification of blood volume in non-pregnant surgical patients. In summary, we show here that maternal BMI affects total cf DNA levels in both lean and obese pregnant women. In the future, raw DNA values may need to be adjusted for maternal BMI for adequate interpretation of clinical tests that involve assessment of the fetal fraction9. In addition, the presence of increased total cf DNA levels in obese pregnant women suggests that this analyte may be a biomarker for systemic problems during gestation that may impact both maternal and fetal health.

Lipoatrophic diabetes mellitus is a rare syndrome characterized by lipoatrophy and insulin-resistant diabetes mellitus. Partial lipodystrophy without clinical diabetes mellitus has been associated with intrauterine growth retardation and fetal death. We report successful pregnancy outcomes in two women with lipoatrophic diabetes mellitus.
