Abstract Background Cats in Iran are definitive hosts for several zoonotic intestinal helminths, such as Toxocara cati, Dipylidium caninum, Toxascaris leonina, Physaloptera praeputialis and Diplopylidium nolleri. Objective This study aimed to determine the prevalence of intestinal helminth infection in free‐roaming cats in southeast Iran, a region with a high free‐roaming cat population. Methods From January 2018 to December 2021, 153 cadavers of free‐roaming cats from Southeast Iran were necropsied for intestinal helminth infections. The carcasses were dissected, and the digestive systems were removed. The esophagus, stomach, small intestine, caecum and colon were tightly ligated. All adult helminths were collected, preserved and identified. Results The prevalence of gastrointestinal helminth infections was 80.39% (123/153). Of the cats from Kerman, 73% (73/100) were infected with at least one helminth, including D. caninum 70% (70/100), T. leonina 8% (8/100) and P. praeputialis 17% (17/100). Concurrent infection with two helminth species was found in 16% (16/100) and of three species infections was found in 3% (3/100) of the cats. Of the cats from Zabol, 94.33% (50/53) were infected with at least one of the helminths, including D. caninum 69.81% (37/53), T. leonina 11.32% (6/53), P. praeputialis 37.73% (20/53) and T. cati 5.66% (3/53). Concurrent infection with two helminth species was found in 28.3% (15/53), and three species were found in 1.88% (1/53) of the cats. Helminth infections were more prevalent in older cats. There was no association between sex and infection rate. Conclusion Based on the very high prevalence of zoonotic intestinal helminth infections in free‐roaming cats in southeast Iran, the potential public health risk emphasizes the need for intersectoral collaboration, particularly the provision of health and hygiene education to high‐risk populations, such as pre‐school and school‐age children.

Objectives/Purposes. This study compared one-step and two-step impression techniques. Methods. 12 impressions were made of a resin maxillary model (second premolar and second molar) with two prepared abutment teeth using vinyl polysiloxane (VPS); the margin of the second premolar was 0.5 mm subgingivally; and the margin of the second molar tooth was at the level of the gingiva. Introduction. Leishmaniasis is still a neglected tropical disease that can endanger more than 350 million people among 98 countries. Leishmania can survive in fibroblasts as latent inactive forms. This study was conducted to evaluate the role of superparamagnetic iron oxide nanoparticles (SPIONs) in cell culture for tracking the labeled Leishmania major in fibroblasts. Methods. Dextran-coated SPIONs were used for labeling L. major in co-culture of fibroblasts with the parasite. To quantify and trace SPION-labeled Leishmania, Prussian blue staining was undertaken. Fibroblast characterization was undertaken by real time polymerase chain reaction. Transmission electron microscope (TEM) was used for confirming the entry of the labeled L. major to the cytoplasm and the nucleus of the fibroblast. Results. Fibroblasts were spindle-shaped and adherent to culture flasks. Promastigotes were with thin elongated lance-like morphology with an anterior kinetoplast and an emergent free flagellum. Prussian blue staining revealed that internalized SPIONs were localized within cytoplasm and nucleus of the fibroblasts after 24 hours of culture. Prussian blue staining successfully showed the presence of iron (stained blue) in labeled L. major within the fibroblasts. This finding was confirmed by TEM, and labeled L. major was detected in the fibroblast cytoplasm and nucleus too. Conclusion. We can conclude that SPIONs are safe, inexpensive, easy to use, and accurate, and a fast method to label Leishmania parasite in cells that the parasite can be latent, such as fibroblasts. These findings can open a new window in diagnosis, pathogenesis, and treatment of cutaneous leishmaniasis and can be added to the literature.

Abstract This 8‐year (from 2008 to 2016) retrospective study calculated the percentage of carcass and organ (lung and liver) condemnations and estimated the direct financial costs at four slaughterhouses in Sistan‐Baluchestan Province, Southeastern Iran. Each carcass and organ (lung and liver) was thoroughly examined through inspection, palpation and incision following the standard protocol. Identification of the parasites was performed macroscopically. The total direct economic loss due to meat's condemnation was estimated by adding weights of each organ or carcass part and multiplying individual organ totals by their 2016 market unit price. A total of 857,039 cattle were slaughtered during this period, 64,497 livers (7.5%), 31,401 lungs (3.6%) and the carcasses of 1,171 cattle (0.1%) were condemned due to lesions caused by parasites. The main parasitic lesions in the condemned livers were attributed to Echinococcus granulosus (4.2%), Fasciola spp. (3.1%) and Dicrocoelium dendriticum (0.1%). All the condemned lungs were due to E. granulosus (3.6%). Taenia saginata cysticerci were detected in 0.1% of inspected animals. Abstract Head and neck squamous cell carcinoma (HNSCC) represents a heterogeneous group of tumors. While significant progress has been made using multimodal treatment, the 5‐year survival remains at 50%. Developing effective therapies, such as immunotherapy, will likely lead to better treatment of primary and metastatic disease. However, not all HNSCC tumors respond to immune checkpoint blockade therapy. Understanding the complex cellular composition and interactions of the tumor microenvironment is likely to lead to new knowledge for effective therapies and treatment resistance. In this review, we discuss HNSCC characteristics, predictive biomarkers, factors influencing immunotherapy response, with a focus on the tumor microenvironment.

Heteroresiatnce (HR) is the type of resistance toward one or more antibiotics appearing as a population of the bacterial load consisting of one or more subpopulations with lower antibiotic susceptibility levels than others. Due to the lack of appropriate diagnosis of HR isolates and their importance in resistance emergence to antibiotics, investigating the origins, emergence factors, and HR inhibitors is critical in combating antibiotic resistance. Efflux pumps (EPs) are bacterial systems that own an influential role in acquiring resistance toward anti-bacterial compounds. Studies on EPs revealed that they can affect HR emergence mechanisms and are competent to be introduced as a suitable bacterial target for diagnostic and therapeutic development in combating HR isolates. This review will consider the relations between EPs and the emergence of HR isolates and discuss their importance in confronting this type of antibiotic resistance.

In recent decades, green aspects became a key priority for governments worldwide, as sustainable policies are able to promote a more equitable society and a healthier economy from the social, economic, and environmental perspectives, in addition to preserving natural resources for future generations. As an essential context in information technology management, green information technology (GIT) has been developed to cope with the existing environmental problems through organizations. The present study is aimed at identifying the influential factors of decision-making on the adoption of GIT. To collect the required data, interviews were performed along with a structured survey. A total of 112 questionnaires were delivered to chief information officers (CIOs), 99 of which underwent the analysis. The structural equation and partial least square approaches were adopted for data analysis. GIT driver (G-driver) was found to be an intermediary parameter. Findings revealed the GIT readiness (G-readiness) and GIT context (G-context) result in in GIT adoption whenever there was a G-driver indicator (i.e., ethical driver, economic driver, response driver, or regulatory driver). The present study found the significance of all the variables to be above 1.96 except G-context −> green intention to adoption path and G-readiness −> green intention to adoption. Considering that the determination of coefficients and the analysis of relationships between factors directly depends on the opinion of experts and if the opinion of expert’s changes, the results will also change; this can be mentioned as the most important limitation of the research. Therefore, it should be noted that the largest impact was identified to be posed by the economical driver.

In recent years, social networks have made significant progress and the number of people who use them to communicate is increasing day by day. The vast amount of information available on social networks has led to the importance of using friend recommender systems to discover knowledge about future communications. It is challenging to choose the best machine learning approach to address the recommender system issue since there are several strategies with various benefits and drawbacks. In light of this, a solution based on the stacking approach was put out in this study to provide a buddy recommendation system in social networks. Additionally, a decrease in system performance was caused by the large amount of information that was accessible and the inefficiency of some functions. To solve this problem, a particle swarm optimization (PSO) algorithm to select the most efficient features was used in our proposed method. To learn the model in the objective function of the particle swarm algorithm, a hybrid system based on stacking is proposed. In this method, two random forests and Extreme Gradient Boosting (XGBoost) had been used as the base classifiers. The results obtained from these base classifiers were used in the logistic regression algorithm, which has been applied sequentially. The suggested approach was able to effectively address this issue by combining the advantages of the applied strategies. The results of implementation and evaluation of the proposed system show the appropriate efficiency of this method compared with other studied techniques.

Abstract Background Presence of ethanol (EtOH) may alter the relationship between blood lactate concentrations and mortality. This study compares lactate‐associated mortality risk in the presence and absence of EtOH. Methods We performed a retrospective cohort study including all patients, age >17 years, presenting from January 2012–December 2018, to an urban, academic emergency department, with a clinically measured lactate. Data were electronically abstracted from the medical record. The primary outcome was 28‐day in‐hospital mortality. Patients were grouped by EtOH test results as follows: 1) present (any EtOH detected), 2) absent (EtOH concentration measured and not detected), or 3) not ordered. Marginal analysis was used to calculated probability of mortality for fixed values of lactate and model covariates. Results Of 40,956 adult emergency department patients with measured lactate, we excluded 768 (1.89%) for lactate >10.0 mmol/L, leaving 40,240 for analysis: 4,066 (10.1%) EtOH present, 10,819 (26.9%) EtOH absent, 25,355 (63%) EtOH not ordered. Of these, 1790 (4.4%) had 28‐day in‐hospital mortality. Marginal probability of mortality calculated for specific lactate values found less risk for EtOH Present patients versus EtOH absent patients at lactate 0.0 mmol/L (0.8% [95%CI: 0.5–1.2%] vs 3.2% [2.8–3.6%]), 2.0 mmol/L (1.5% [1.1–1.9%] vs 4.0% [3.7–4.3%]), 4.0 mmol/L (2.6% [2.2–3.1%] vs 5.0% [4.6–5.4%]), until 6.0 mmol/L (4.5% [3.7–5.4%] vs 6.2% [5.4–7.0%]). Objective The objective of this study was to assess the performance and reproducibility of the two currently used ocular surface staining scores in the assessment of keratoconjunctivitis sicca in Sjögren syndrome (SS) research classification. Methods In a multidisciplinary clinic for the evaluation of sicca, we performed all tests for the American European Consensus Group (AECG) and the American College of Rheumatology (ACR)/European League Against Rheumatism (EULAR) classification criteria, including the van Bijsterveld score (vBS) and the Ocular Staining Score (OSS), in 994 participants with SS or with non‐SS sicca. We analyzed the concordance between the scores, the diagnostic accuracy and correlation with clinical variables, and interrater and intrasubject reproducibility. Results A total of 308 (31.1%) participants had a discordant vBS and OSS that was due to extra corneal staining points in the OSS. The presence of one or more of the additional points was highly predictive of SS classification (odds ratio = 3.66; P = 1.65 × 10e‐20) and was associated with abnormal results of all measures of autoimmunity and glandular dysfunction. Receiver operating characteristic curves showed optimal cutoff values of four for the vBS (sensitivity = 0.62; specificity = 0.71; Youden's J = 0.33) and five for the OSS (sensitivity = 0.56; specificity = 0.75; Youden's J = 0.31). Notably, there was very poor consistency in interobserver mean scores and distributions (P < 0.0001) and in intrasubject scores after a median of 5.5 years (35% changed status of the ocular criterion). Conclusion Ocular surface staining scores are useful for SS research classification; however, they are subject to significant interrater and intrasubject variability, which could result in changes in classification in 5%‐10% of all subjects. These results highlight the need for objective and reproducible markers of disease that have thus far remained elusive for SS.

Incontinentia pigmenti (IP) is an X-linked dominant neurocutaneous syndrome with ophthalmologic, neurologic, cutaneous, and dental manifestations and in most cases antenatally lethal in boys. Occasionally, typical IP may occur in boys due to Klinefelter syndrome or a genomic mosaicism. Skin lesions are observed in 4 stages: blistering, verrucous linear plaques, swirling macular hyperpigmentation, followed by linear hypopigmentation that develop during adolescence and early adulthood. Neonatal herpes simplex virus (HSV) infection can be manifested in 3 forms: localized, disseminated, and central nervous system (CNS) involvement. Timely diagnosis and treatment of neonatal HSV infection is critical. In this case report, we present a 12-day female newborn with a history of maternal genital HSV in second trimester and vesicular lesions on the upper and lower limbs that was appeared at first hours of life. She was admitted in the maternity hospital that was born and was treated by antibiotic and acyclovir for 11 days. Then, she readmitted for her distributed vesicular lesions. The results of blood and CSF for HSV PCR were negative. Eventually the diagnosis for incontinentia pigmenti was made by consultation with a dermatologist, and skin biopsy confirmed the diagnosis.

Aim. Sonography has been brought in percutaneous nephrolithotripsy (PCNL) as an adjunct to or substitute for X-ray to restrict radiation exposure. This study was designed to investigate the possible predictors for the success of the solo sonographically guided PCNL. Methods. 148 consecutive cases were prospectively enrolled. All steps of PCNL were performed solely with sonography guidance under spinal anesthesia. Residual stones were evaluated the day after surgery using sonography and plain radiography. Results. The mean age was 46±15 years; 40% of kidneys had hydronephrosis. The mean stone burden was 504±350 mm2. The mean duration of surgery was 43±21 minutes. The early stone-free rate was 92% in inferior or middle calyceal stones, 89.5% in single pelvic stones, 81.5% in partial staghorn stones, and 61.9% in staghorn stones. The mean residual stone size was 13±8 mm. Logistic regression showed that a lower age and a larger stone burden significantly predicted positive residual stones. Fifteen percent of patients presented with grade I or II and six percent showed grade III complication based on Clavien classification. There was no cases of organ injury or death. Conclusion. Solo ultrasonographically guided PCNL under spinal anesthesia is feasible with an acceptable stone-free rate and complication rate.

Considering the variety of studies that have been reported in low-power designing era, the subthreshold design trend in Very Large Scale Integrated (VLSI) circuits has experienced a significant development in recent years. Growing need for the lowest power consumption has been the primary motivation for increase in research in this area although other goals, such as lowest energy delay production, have also been achieved through sub-threshold design. There are, however, few extensive studies that provide a comprehensive design insight to catch up with the rapid pace and large-scale implementations of sub-threshold digital design methodology. This paper presents a complete review of recent studies in this field and explores all aspects of sub-threshold design methodology. Moreover, near-threshold design and low-power pipelining are also considered to provide a general review of sub-threshold applications. At the end, a discussion about future directions in ultralow-power design is also included.

Significant controversy exists regarding vesicoureteral reflux (VUR) management, due to lack of sufficient prospective studies. The rationale for surgical management is that VUR can cause recurrent episodes of pyelonephritis and long-term renal damage. Several surgical techniques have been introduced during the past decades. Open anti-reflux operations have high success rate, exceeding 95%, and long durability. The goal of this article is to review the Gil-Vernet trigonoplasty technique, which is a simple and highly successful technique but has not gained the attention it deserves. The mainstay of this technique is approximation of medial aspects of ureteral orifices to midline by one mattress suture. A unique advantage of Gil-Vernet trigonoplasty is its bilateral nature, which results in prevention from contralateral new reflux. Regarding not altering the normal course of the ureter in Gil-Vernet procedure, later catheterization of and retrograde access to the ureter can be performed normally. There is no report of ureterovesical junction obstruction following Gil-Vernet procedure. Gil-Vernet trigonoplasty can be performed without inserting a bladder catheter and drain on an outpatient setting. Several exclusive advantages of Gil-Vernet trigonoplasty make it necessary to reconsider the technique role in VUR management.
