Summary Objective Elucidation of abnormal connections throughout the whole brain is necessary to understand temporal lobe epilepsy (TLE). We examined abnormalities in whole‐brain white matter integrity and their relationship with duration of illness in patients with TLE. Methods The subjects were 15 patients with TLE and 17 healthy controls. Mean duration of illness in the TLE group was 21.6 years. Tract‐based spatial statistics (TBSS) were used for diffusion tensor imaging (DTI) analysis. Four diffusion tensor metrics, that is, fractional anisotropy (FA), mean diffusivity (MD), axial diffusivity (AD), and radial diffusivity (RD) were calculated and then examined for differences between the TLE and healthy control groups. We also examined for correlations between DTI parameters and duration of illness in the TLE group. Results In the TLE group, compared with the healthy control group, FA was reduced, and MD and RD were increased, not only in the limbic and temporal lobe regions and their directly connecting regions in both hemispheres, but also in remote white matter regions. Duration of illness showed a significant negative correlation with mean whole‐brain FA and a significant positive correlation with both mean whole‐brain MD and RD. Brain regions showing correlation between disease duration and DTI metrics also extended to the limbic area and its connecting regions, and to remote white matter regions. Significance The results of this study suggest that widespread abnormalities in white matter integrity in patients with TLE are associated with long‐term disease.

We report an 83-year-old woman with atypical senile dementia with Fahr-type calcification. Brain computed tomography demonstrated asymmetrical calcification predominant in the basal ganglia on the right side and pronounced diffuse cortical atrophy in the frontotemporal areas. The patient was clinically diagnosed with diffuse neurofibrillary tangles with calcification. Brain single photon emission computed tomography findings revealed that cerebral blood flow was reduced on the right side, as compared with the left side, in widespread areas. Hemispheric asymmetry in both calcification and cerebral blood flow suggests a relationship between calcification and vascular changes.

Aim: While the underlying therapeutic mechanisms of repetitive transcranial magnetic stimulation (rTMS) treatment for depression remain unclear, recent animal studies have suggested that hippocampal neurogenesis might be required for the effects of antidepressant treatments including antidepressant drugs and electroconvulsive therapy. The aim of this study was to examine chronic rTMS effects on hippocampal neurogenesis in rats. Methods: Using a 70-mm figure-of-eight coil, the stimulating parameters were set to 25 Hz and 70% of the rTMS device's maximum power. For 14 consecutive days, bromodeoxyuridine (BrdU) and 1000 pulses of rTMS were administered daily. Cell proliferation in the dentate gyrus was examined with immunohistochemistry. Results: In the rTMS-treated group, BrdU-positive cells were significantly increased in the dentate gyrus. Conclusion: Our results suggest that hippocampal neurogenesis might be involved in the antidepressant effects of chronic rTMS.

Background: Progression of the core and accessory symptoms of dementia can be slowed if drug therapies and psychosocial interventions are administered at an early stage. The aim of this study was to develop and standardize a neuropsychological test for the elderly that can detect dementia at an early stage with high sensitivity and can evaluate a wide range of severities of dementia based on assessments of various cognitive functions. Methods: A preliminary test consisting of 23 items and the Nishimura Mental State Scale for the Elderly (NM Scale), which evaluates the mental functions of elderly individuals by observing their actual behaviors in daily life, were administered to 448 elderly subjects. After applying Hayashi’s quantification theory type I to the results, we revised the preliminary test to construct a neuropsychological test for the elderly, which we named the Nishimura Dementia Test (ND Test), and standardized it. Then, we examined its validity and test–retest reliability. Results: Among the 448 subjects, there was a strong correlation between the ND Test scores and NM Scale scores. The ND Test showed a good general agreement rate for the discrimination of the severity of dementia, and good sensitivity and specificity of discrimination of dementia when compared with the actual NM Scale. Using different groups of elderly subjects, the ND Test showed validity and test–retest reliability, and the ND Test scores showed strong correlations with the Revised Hasegawa Dementia Scale scores and the Mini-Mental State Examination scores. Conclusions: The ND Test is based on assessment of a variety of cognitive functions and can evaluate a wide range of severities of dementia with good validity and reliability.

Working memory performance has been inconsistently reported in autism spectrum disorders (ASD). Several studies in ASD have found normal performance in digit span and poor performance in digit symbol task although these are closely related with working memory. It is assumed that poor performance in digit symbol could be explained by confirmatory behavior, which is induced due to the vague memory representation of number-symbol association. Therefore it was hypothesized that the performance of working memory task, in which vagueness did not cause confirmatory behavior, would be normal in ASD. For this purpose, the Advanced Trail Making Test (ATMT) was used. The performance of digit span, digit symbol and ATMT was compared between ASD and normal control. The digit span, digit symbol and ATMT was given to 16 ASD subjects and 28 IQ-, age- and sex-matched control subjects. The scores of these tasks were compared. A significantly lower score for ASD was found only in digit symbol compared with control subjects. There were no significant difference in digit span and working memory estimated by ATMT. Discrepancy of scores among working memory-related tasks was demonstrated in ASD. Poor digit symbol performance, normal digit span and normal working memory in ATMT implied that ASD subjects would be intact in working memory itself, and that superficial working memory dysfunction might be observed due to confirmatory behavior in digit symbol. Therefore, to evaluate working memory in ASD, tasks that could stimulate psychopathology specific to ASD should be avoided.

Desynchronization in the right auditory cortices, including the transverse gyrus of Heschl, planum temporale and supramarginal cortex, occurred during musical hallucinations in a 78-year-old woman with hearing impairment and depression. This phenomenon was assessed using a novel, spatially filtered magnetoencephalography (MEG) analysis, termed synthetic aperture magnetometry (SAM). In general, the affected areas are consistent with neuroimaging studies of normal musical perception and imagery, suggesting that musical hallucinations involve abnormal spontaneous activity in the neural substrate dedicated to musical perception and imagery in which false imagery occurs.

Background: Cognitive decline associated with normal aging, mild cognitive impairment (MCI) and Alzheimer’s disease (AD) are, in theory, reflected in the disturbance of selective neural networks. Methods: The effect of aging on the functional connectivity of multiple areas of the brain was investigated by using relative power contribution (RPC) analysis of alpha activity. Electroencephalograms were recorded from eight channels in 38 healthy elderly subjects aged 50‐89 years and in 22 control subjects aged 20‐29 years. Results: Entropy of the RPC of alpha activity, indicating the amount of dependence in each area, showed a significant decrease in the left temporooccipital area in 50‐80-year-old subjects and in the right inferior frontal area in 70‐80-year-old subjects. Conclusions: A metabolic decrease in these areas is also reported in patients with AD or MCI. The results from this study suggest that the normal aging process and pathological processes may exhibit a partially common pattern of neural degeneration. However, further studies are required using a larger sample size and strict discrimination between normal aging and MCI.

We treated an 80-year-old man with vascular dementia who developed increased libido induced by fluvoxamine. Sexual stimulation associated with fluvoxamine administration has been rarely reported and the present findings suggest that organic brain dysfunction could be a risk factor for the development of the condition.
