Minimal scan times in rapid fluorine-19 MRI using sulfur hexafluoride (SF6) have been on the order of 10 s. Because of the very short T1 relaxation time of SF6 (T1 = 1.65 ms), high receiver bandwidths are necessary to allow for a high number of excitations. Since high bandwidths cause high levels of electronic noise, SNR per acquisition has been too low to further reduce scan time. The purpose of this study was to investigate whether scan times could be reduced using hexafluoroethane (C2F6), a gas with a longer T1 (T1 = 7.9 ms) at a relatively low bandwidth of 488 Hz/pixel. Gradient-echo images were acquired during and after completion of the wash-in of a 70% C2F6- 30% O2 mixture. Peak SNR values of 16 and 7.9 were observed for coronal projection images acquired within 2 s and 260 ms, respectively. Qualitative and semiquantitative contrast-enhanced (CE) dynamic perfusion MRI techniques are established as noninvasive diagnostic means of assessing coronary artery disease. However, to date quantification of myocardial blood flow (MBF) has not reached the same acceptance as MBF quantification with nuclear techniques. To validate quantification of MBF at rest using the extracellular contrast agent (CA) Gd-DTPA, we performed an animal study in a pig model of acute myocardial ischemia. We quantified MBF from MRI data with a mathematical model (MMID4) of the underlying vasculature. These MBF results were subsequently compared with the results from fluorescent microspheres. The study showed a correlation of r = 0.66 between MBF estimates obtained with MRI and those obtained with fluorescent microspheres. The correlation for ischemic and nonischemic myocardium was r = 0.86 and r = 0.47, respectively. In this work some of the factors that can influence the signal-to-noise ratio (SNR) and spatial resolution in MR images of inhaled hyperpolarized gases are systematically addressed. In particular, the effects of RF depletion of longitudinal polarization and image gradient diffusion dephasing were assessed in terms of their contribution to a k-space filter. By means of theoretical simulations and a novel method of experimental validation using a variable transverse magnetization of the 1H signal, systematic quantitative and qualitative investigations of the effects of k-space filtering intrinsic to imaging of hyperpolarized gas were made. A 2D gradient-echo image is considered for a range of flip angles with centric, sequential, and half-Fourier Cartesian phase-encoding strategies, and the results are assessed in terms of SNR and spatial resolution in the reconstructed images. Centric phase encoding was found to give the best SNR at higher flip angles, with a trade-off in spatial resolution compared to sequential phase encoding. A half-Fourier approach potentially offers increased SNR through the use of higher flip angles without compromising the spatial resolution, which is comparable to that achieved with sequential encoding., (Copyright 2002 Wiley-Liss, Inc.)

Myocardial blood flow (MBF) can be quantified using dynamic T1-weighted MRI of diffusible tracers and a mathematical model of underlying vasculature. Quantification of MBF by means of T1- weighted MRI requires knowledge of the arterial input function (AIF). The AIF can be estimated from the left ventricular (LV) cavity. However, dispersion may occur between the LV and the tissue of interest because of the laminar blood flow profiles, branching of venules, and because of stenosis. To evaluate the influence of dispersion on the results of MBF quantification, a simulation study was performed. The dispersion was described as a convolution of the AIF with an exponential residue function. Synthetic tissue and AIF curves were analyzed and the derived parameters fit to the simulated parameters. The results show that an unaccounted dispersion may result in a systematic underestimation of MBF up to approximately 50%. Underestimation increases with increasing dispersion and with increasing MBF. Assuming equal dispersion at rest and during hyperemia, myocardial perfusion reserve (MPR) estimates are also susceptible to underestimation of approximately 20%. MRI of the lungs using hyperpolarized helium-3 ((3)He) allows the determination of intrapulmonary oxygen partial pressures (p(O2)). The need to separate competing processes of signal loss has hitherto required two different imaging series during two different breathing maneuvers. In this work, a new imaging strategy to measure p(O2) by a single series of consecutive scans is presented. The feasibility of the method is demonstrated in three healthy human volunteers. Maps and histograms of intrapulmonary p(O2) are calculated. Changes in the oxygen concentration of the inhaled gas mixture are well reproduced in the histograms. Monte Carlo (MC) simulations of the temporal evolution of (3)He hyperpolarization within the lungs were performed to evaluate the accuracy of this measurement technique, and its limitations., (Copyright 2002 Wiley-Liss, Inc.)

A method for in vivo measurement of oxygen partial pressure (pO2) in porcine lungs during partial liquid ventilation (PLV) with perflubron (PFOB) was developed. A pulse sequence for high-resolution MRI of the distribution of PFOB in the lung after intratracheal administration was developed as well. Moreover, quantitative measurements of longitudinal relaxation time T(1) of (19)F resonances for assessment of regional pO2 are described. Due to the need to acquire data during a single expiratory breathhold, only low SNRs were achieved in vivo. Therefore, simulations were performed to investigate the influence of background noise on T(1) values calculated from data with low SNR. Based on these simulations, a postprocessing strategy was developed to correct for systematic errors by background noise prior to quantitative analysis. A new method for dynamic imaging of pulmonary wash-in and wash-out kinetics of inhaled sulfur hexafluoride (SF(6)) gas was developed. Measurements at the fluorine-19 Larmor frequency were performed in pigs using a gradient echo pulse sequence with 0.5 ms echo time and a measurement time of 9.1 s per image. Dynamic MRI was performed during wash-in and wash-out of SF(6) gas in mechanically ventilated porcine lungs. A postprocessing strategy was developed for quantitative determination of wash-out time constants in the presence of noise. Mean wash-out constants were 4.78 +/- 0.48 breaths vs. 4.33 +/- 0.76 breaths for left and right lung when ventilation was performed with low tidal volume, and 1.73 +/- 0.16 breaths vs. 1.85 +/- 0.11 breaths with high tidal volume ventilation. In conclusion, breath-hold MRI of SF(6) gas is feasible in large animals. Moreover, regional wash-in and wash-out kinetics of SF(6) can be determined noninvasively with this new method. MRI with hyperpolarized helium-3 ((3)He) provides high-resolution imaging of ventilated airspaces. The first aim of this (3)He-study was to compare observations of localized signal defects in healthy smokers and non-smokers. A second aim was to describe relationships between parameters of lung function, volume of inspired (3)He and signal-to-noise ratio. With Ethics Committee approval and informed consent, 12 healthy volunteers (seven smokers and five non-smokers) were studied. Imaging was performed in a 1.5 T scanner using a two-dimensional FLASH sequence at 30V transmitter amplitude (TR/TE/alpha = 11 ms/4.2 ms/<10 degrees ). Known amounts of (3)He were inhaled from a microprocessor-controlled delivery device and imaged during single breath-holds. Images were evaluated visually, and scored using a prospectively defined 'defect-index'. Signal-to-noise ratio of the images were correlated with localization, (3)He volumes and static lung volumes. Due to poor image quality studies of two smokers were not eligible for the evaluation. Smokers differed from non-smokers in total number and size of defects: the 'defect-index' of smokers ranged between 0.8 and 6.0 (median = 1.1), that of non-smokers between 0.1 and 0.8 (median = 0.4). Intraindividually, an anteroposterior gradient of signal-to-noise ratio was apparent. Signal-to-noise ratio correlated with the estimated amount of hyperpolarization administered (r = 0. 77), but not with static lung volumes. We conclude that (3)He MRI is a sensitive measure to detect regional abnormalities in the distribution of ventilation in clinically healthy persons with normal pulmonary function tests., (Copyright 2000 John Wiley & Sons, Ltd.)

3Helium-MRI ((3)He-MRI) can be used for analysis of lung function, e. g. dynamic imaging of ventilation and gas diffusion within the lung, assessment of intrapulmonary oxygen concentrations and their time course. During imaging, the irreversible signal loss due to depolarizing radio frequency excitations can be described using the flip angle (FA) alpha. This parameter has to be quantified in order to account for it during quantitative assessment of the (3)helium signal intensity and its temporal development. A pulse sequence with magnetization transfer as the main contrast mechanism (MT-FLASH) was developed for improved imaging of breast lesions that requires neither fat suppression nor postprocessing. After optimization of the sequence in phantom and volunteer studies, a clinical pilot study with 14 patients was performed. In carcinomas the relative signal increase after Gd-DTPA administration was on average 34% in MT-FLASH images compared with 169% in conventional T1-weighted (T1W) three-dimensional FLASH images. In MT-FLASH images, all lesions demonstrated a signal intensity higher than that of fat; in T1W images, all lesions have a lower signal intensity. The average postcontrast carcinoma-to-fat contrast-to-noise ratios were +11.6 and -14.2, respectively. The conspicuity of 12 of 13 carcinomas was improved in postcontrast MT-FLASH imaging enables excellent visualization of Gd-DTPA-enhancing breast lesions.
