Abstract This study investigated the tibiofemoral joint (TFJ) forces and supporting muscle forces displayed by knee osteoarthritis (OA) patients during stair ascent, and if these forces were influenced by the presence of pain. Fifteen knee OA patients partitioned into two groups based on pain experienced during stair ascent trails using a Visual Analogue Scale (VAS) (OA‐pain = 10; OA‐no pain = 5) and 14 healthy aged‐matched controls took part in this study. Kinematic and kinetic data were collected during three stair ascent trials, which provided the inputs for the musculoskeletal model FreeBody. TFJ contact forces and muscles forces were predicted by the model at early, mid‐ and late stance. These variables were compared between groups using a one‐way analysis of variance. Abstract Background The objective of the study is to investigate the load transmission within the pelvic ring under physiological loading during gait and to correlate these results with clinical findings. In a second approach, we analysed how load distribution is altered by fractures of the anterior pelvic ring. Methods Muscle forces and joint reaction forces are calculated by inverse dynamics and implemented in a finite element pelvis model including the joints. Results With the intact configuration and according to the moment of the gait, left and right superior and inferior rami show the highest stresses of the model, corresponding to the typical location of an anterior pelvic ring fracture. A superior ramus fracture induces larger stresses to the lower ramus and a slight increase of stresses on the posterior structures. A total disruption of anterior rami redirects the loads to the back of the pelvis and introduces significantly higher stresses on the posterior structures. Conclusions This investigation enhances the understanding of the biomechanics of the pelvis and highlights the important role of the rami in load carrying and in maintaining integrity of the pelvic ring.

Abstract Background Many different fixation devices are used to maintain the correction angle after medial open wedge high tibial osteotomy (MOWHTO). Each device must provide at least sufficient mechanical stability to avoid loss of correction and unwanted fracture of the contralateral cortex until the bone heals. In the present study, the mechanical stability of following different implants was compared: the TomoFix small stature (sm), the TomoFix standard (std), the Contour Lock, the iBalance and the second generation PEEKPower. Simplified loading, usually consisting of a vertical load applied to the tibia plateau, is used for experimental testing of fixation devices and also in numerical studies. Therefore, this study additionally compared this simplified experimental loading with a more realistic loading that includes the muscle forces. Method Two types of finite element models, according to the considered loading, were created. The first type numerically simulated the static tests of MOWHTO implants performed in a previous experimental biomechanical study, by applying a vertical compressive load perpendicularly to the plateau of the osteotomized tibia. The second type included muscle forces in finite element models of the lower limb with osteotomized tibiae and simulated the stance phase of normal gait. Section forces in the models were determined and compared. Stresses in the implants and contralateral cortex, and micromovements of the osteotomy wedge, were calculated. Results For both loading types, the stresses in the implants were lower than the threshold values defined by the material strength. The stresses in the lateral cortex were smaller than the ultimate tensile strength of the cortical bone. The implants iBalance and Contour Lock allowed the smallest micromovements of the wedge, while the PEEKPower allowed the highest. There was a correlation between the micromovements of the wedge, obtained for the simplified loading of the tibia, and the more realistic loading of the lower limb at 15% of the gait cycle (Pearson’s value r = 0.982). Conclusions An axial compressive load applied perpendicularly to the tibia plateau, with a magnitude equal to the first peak value of the knee joint contact forces, corresponds quite well to a realistic loading of the tibia during the stance phase of normal gait (at 15% of the gait cycle and a knee flexion of about 22 degrees). However, this magnitude of the knee joint contact forces overloads the tibia compared to more realistic calculations, where the muscle forces are considered. The iBalance and Contour Lock implants provide higher rigidity to the bone-implant constructs compared to the TomoFix and the PEEKPower plates.

Heavy deadlift is used as a physical fitness screening tool in the U.S. Army. Despite the relevance of such a screening tool to military tasks performed by Service Members, the biomechanical impact of heavy deadlift and its risk of low-back injury remain unknown. A kinematics-driven musculoskeletal model of spine was implemented to investigate biomechanics of the lower back in a volunteer (23 years old, height of 1.82 m, and body mass of 98.8 kg) during a 68 kg deadlift. In search of protective mechanisms, effects of model personalization and variations in trunk musculature and lumbopelvic rhythm were also investigated. The net moment, compression and shear forces at the L5-S1 reached peaks of 684 Nm, 17.2 and 4.2 kN, respectively. Geometrical personalization and changes in lumbopelvic rhythm had the least effects on predictions while increases in muscle moment arms (40%) had the largest effects that caused, respectively, 32% and 36% decrease in the maximum compressive and shearing forces. Initiating wrapping of back muscles at farther distances from the spine had opposing effects on spinal loads; peak compression at the L5-S1 decreased by 12% whereas shear increased by 19%. Despite mechanisms considered, spinal loads during heavy deadlift exceed the existing evidence concerning the threshold of injury for spinal segments, suggesting the vulnerability to injury. About a third of knee joint disorders originate from the patellofemoral (PF) site that makes stair ascent a difficult activity for patients. A detailed finite element model of the knee joint is coupled to a lower extremity musculoskeletal model to simulate the stance phase of stair ascent. It is driven by the mean of measurements on the hip-knee-ankle moments-angles as well as ground reaction forces reported in healthy individuals. Predicted muscle activities compare well to the recorded electromyography data. Peak forces in quadriceps (3.87 BW, body weight, at 20% instance in our 607 N subject), medial hamstrings (0.77 BW at 20%), and gastrocnemii (1.21 BW at 80%) are estimated. Due to much greater flexion angles-moments in the first half of stance, large PF contact forces (peak of 3.1 BW at 20% stance) and stresses (peak of 4.83 MPa at 20% stance) are estimated that exceed their peaks in level walking by fourfold and twofold, respectively. Compared with level walking, ACL forces diminish in the first half of stance but substantially increase later in the second half (peak of 0.76 BW at 75% stance). Under nearly similar contact forces at 20% of stance, the contact stress on the tibiofemoral (TF) medial plateau reaches a peak (9.68 MPa) twice that on the PF joint suggesting the vulnerability of both joints. Compared with walking, stair ascent increases peak ACL force and both peak TF and PF contact stresses. During development, the embryonic cartilaginous skull in most vertebrates is partially replaced by bones with endochondral and perichondral ossifications. Muscle attachments are thought to influence the patterns of ossification and, hence, the differentiation of the skull. To investigate the association between muscle attachments and early ossifications of reptilian embryos, we conducted digital 3D reconstructions of the cranium, the head, and the neck musculature from a histological section series of a late term embryonic tuatara, Sphenodon punctatus, with a total body length of 52 mm. As the sole living rhynchocephalian species, it is an important outgroup in comparative studies of squamate evolution. We found that head and neck muscles are largely associated with early ossification of the basal plate and the palatoquadrate, and with three other ossifications in an older specimen with a total body length of 72 mm. These results suggest that tensile forces resulting from embryonic muscle contraction are largely, but not exclusively, correlated with the area of endochondral ossification in the chondrocranium and palatoquadrate in tuatara. Beyond little-known genetic factors, the complexity of chondrocranial architecture, the progress of its development, and the effect of multiple muscle transmitting forces in the chondrocranium must be considered to provide a more comprehensive discussion of the mechanical properties of the embryonic skull., (© 2022 The Authors. Axial compressive loads whose direction changes along the spinal curvature (so called compressive follower loads (CFLs)) was postulated as a normal physiological load in the lumbar spine in the literature. Computational analyses were conducted in this study using finite element and optimization models of the spinal system incorporating 244 fascicles of back muscles. It was feasible to find optimum solutions for spinal muscle forces generating CFLs in the lumbar spine in 3-D postures of neutral standing, flexion 40°, extension 10°, axial rotation 10°, or lateral bending 30°. FE analyses demonstrated that the lumbar spine can be in a stable condition not under all CFL generating muscle forces but under those producing CFLs along a curve parallel to the spinal curvature located in the vicinity of the base spinal curve constructed by connecting the geometrical centers of the vertebral bodies. It was also possible to estimate the stable range of the relative location of such CFL curve to the base spinal curve. These results suggest that the lumbar spine in various 3-D postures can be stabilized by spinal muscles that generate CFLs in the spine, which at least in part supports the hypothesis of CFLs as a physiological load in the lumbar spine. © 2018 Orthopaedic Research Society. The incidence of congenital spine deformities, including congenital scoliosis, kyphosis, and lordosis, may be influenced by the in utero mechanical environment, and particularly by fetal movements at critical time-points. There is a limited understanding of the influence of fetal movements on spinal development, despite the fact that mechanical forces have been shown to play an essential role in skeletal development of the limb. This study investigates the effects of muscle forces on spinal curvature, vertebral segmentation, and vertebral shape by inducing rigid or flaccid paralysis in the embryonic chick. The critical time-points for the influence of fetal movements on spinal development were identified by varying the time of onset of paralysis. Prolonged rigid paralysis induced severe defects in the spine, including curvature abnormalities, posterior and anterior vertebral fusions, and altered vertebral shape, while flaccid paralysis did not affect spinal curvature or vertebral segmentation. Early rigid paralysis resulted in more severe abnormalities in the spine than later rigid paralysis. The findings of this study support the hypothesis that the timing and nature of fetal muscle activity are critical influences on the normal development of the spine, with implications for the understanding of congenital spine deformities. © 2017 Orthopaedic Research Society. A change in hamstring strength and activation is typically seen after injuries or invasive surgeries such as anterior cruciate reconstruction or total knee replacement. While many studies have investigated the influence of isometric increases in hamstring load on knee joint kinematics, few have quantified the change in kinematics due to a variation in medial to lateral hamstring force ratio. This study examined the changes in knee joint kinematics on eight cadaveric knees during an open-chain deep knee bend for six different loading configurations: five loaded hamstring configurations that varied the ratio of a total load of 175 N between the semimembranosus and biceps femoris and one with no loads on the hamstring. The anterior-posterior translation of the medial and lateral femoral condyles' lowest points along proximal-distal axis of the tibia, the axial rotation of the tibia, and the quadriceps load were measured at each flexion angle. Unloading the hamstring shifted the medial and lateral lowest points posteriorly and increased tibial internal rotation. The influence of unloading hamstrings on quadriceps load was small in early flexion and increased with knee flexion. The loading configuration with the highest lateral hamstrings force resulted in the most posterior translation of the medial lowest point, most anterior translation of the lateral lowest point, and the highest tibial external rotation of the five loading configurations. As the medial hamstring force ratio increased, the medial lowest point shifted anteriorly, the lateral lowest point shifted posteriorly, and the tibia rotated more internally. The results of this study, demonstrate that variation in medial-lateral hamstrings force and force ratio influence tibiofemoral transverse kinematics and quadriceps loads required to extend the knee. © 2016 Orthopaedic Research Society. Osteoarthritis (OA) is the leading cause of pain and disability in the elderly with the knee being the most affected weight bearing joint. We used a musculoskeletal biomechanical model of the lower extremity including a detailed validated knee joint finite element model to compute lower extremity muscle forces and knee joint stresses-strains during the stance phase of gait. The model was driven by gait data on OA patients, and results were compared with those of the same model driven by data on normal controls. Additional analyses were performed with altered cartilage-menisci properties to evaluate the effects of deterioration during OA. In OA patients compared to normal subjects, muscle forces dropped at nearly all stance periods except mid-stance. Force in the anterior cruciate ligament remained overall the same. Total contact forces-stresses deceased by about 25%. Alterations in properties due to OA had negligible effects on muscle forces, but increased contact areas and cartilage strains and reduced contact pressures. Reductions in contact stresses and increases in tissue strains and transfer of load via menisci are partly due to the altered kinetics-kinematics of gait and partly due to deterioration in cartilage-menisci properties in OA patients., (© 2013 Orthopaedic Research Society.
