Objective: To study the prevalence and pattern of tuboperitoneal pathologies among infertile women in Nigeria, using laparoscopy.Methods: A prospective study was undertaken of infertile women who underwent diagnostic laparoscopy in two fertility clinics in Nigeria between November 2015 and April 2017. The rates of identified tuboperitoneal diseases were examined.Results: The age of the 230 women ranged from 21 to 46 years, and most women had a parity group of 0-1 (87.8%; n=202). Secondary infertility accounted for 124 (53.9%) cases, and the mean duration of infertility was 4.6 ± 2.7 years. Tuboperitoneal pathologies were seen in 171 (74.4%) women and mainly comprised tubal occlusion (56.5%; n=130), hydrosalpinx (41.7%; n=96), pelvic adhesions (39.6%, n=91), and endometriosis (8.8%; n=19). Bilateral tubal occlusion was seen in 46 (20.0%) women, whereas proximal tubal occlusion accounted for 73 (56.2%) of all cases of tubal occlusion.Conclusion: There was a high rate of tuboperitoneal abnormalities in the studied population, which mainly comprised tubal occlusion, hydrosalpinx, pelvic adhesions, and endometriosis. Aim The aim of our study was to assess the levels of human epididymis protein 4 (HE4) with the common tumor marker carbohydrate antigen 125 (CA125) in the diagnosis and monitoring of therapy for primary fallopian tube carcinoma (PFTC). Methods Serum HE4 and CA125 levels from 82 PFTC patients and 154 patients with benign pelvic masses as the control were measured by Roche electrochemiluminescent immunoassay. HE4 determinations for surgery response and recurrence monitoring were assessed in PFTC patients. Results Serum HE4 and CA125 concentrations were significantly higher in PFTC patients compared with those seen in patients with benign pelvic masses ( P < 0.001). Compared with CA125, HE4 had higher specificity, but lower sensitivity whether at early or advanced stage, and the combination of HE4 + CA125 led to higher sensitivity and specificity. HE4 + CA125 performed significantly better than CA125 or HE4 alone in early stage patients. In early stage the sensitivity was 35.7% for HE4 and 64.3% for CA125, while sensitivity for the combination of HE4 and CA125 could reach 71.4%. Furthermore, the two markers were associated with the progression and histology of PFTC. Serum HE4 level was closely correlated with surgical therapy. PFTC patients displayed a greater decline in the level of HE4 compared with CA125 (76.4% vs 55.7%). Combined with CA125, HE4 elevation better predicted recurrence in PFTC patients. Conclusions This study indicated that serum HE4 levels are closely associated with PFTC and the outcome of surgical therapy and recurrence in Chinese patients. [ABSTRACT FROM AUTHOR]

Objective: To determine the prevalence of Chlamydia trachomatis infection among Nigerian women undergoing hysterosalpingography (HSG) and to identify any correlation between chlamydial antibody levels and a diagnosis of tubal disease.Methods: A prospective cross-sectional study was conducted from January 1 to June 30, 2013, among women scheduled to undergo HSG in the radiology department of Lagos University Teaching Hospital, Nigeria. Endocervical swabs and serum samples were collected to assess the levels of chlamydial antigen and antibody, respectively.Results: Among 150 participants, 83 (55.3%) had bilateral tubal patency and 67 (44.7%) had tubal disease. Overall, 53 (35.3%) women had positive test results for chlamydial antibodies; however, none of the participants tested positive for chlamydial antigen. Women with tubal disease were more likely to test positive for chlamydial antibodies (n=44 [65.7%]) than were those whose test results were negative (n=9 [10.8%]; P<0.001). The sensitivity and specificity of chlamydial antibody testing to predict tubal disease diagnosed by HSG were 66% and 89%, respectively.Conclusion: The presence of chlamydial antibodies was quantitatively related to the likelihood of HSG-diagnosed tubal disease. The fallopian tube is the least common site for origin of malignancy in the female genital tract. Most primary fallopian tube malignancies are adenocarcinomas. Primary transitional cell carcinoma (TCC) of the fallopian tube is an extremely rare tumor with a small number of cases reported in the literature. We present a 67-year-old woman who was found incidentally to have a left adnexal mass on a screening pelvic ultrasound. Subsequently the patient underwent laparoscopic left salpingo-oophorectomy and the specimen was submitted for intraoperative frozen section, which revealed a high-grade carcinoma; therefore, she underwnt a laparotomy and total abdominal hysterectomy, right salpingo-oophorectomy and omentectomy. Histopathology revealed high-grade transitional cell carcinoma in the left fallopian tube. Post-surgery she was treated with four cycles of adjuvant chemotherapy with carboplatin and paclitaxel with no complications. Our patient had a family history of malignancy, so genetic testing for BRCA1 and BRCA2 mutations was undertaken and did not reveal any mutation or unclassified variants. Objective: To assess the usefulness of magnetic resonance imaging (MRI) in women with tubercular tubo-ovarian masses.Methods: Twenty-four women with a confirmed diagnosis of tubercular tubo-ovarian masses underwent MRI. The findings were compared with laparotomy/laparoscopy results wherever possible.Results: The mean age was 30.7 years and the mean parity was 1.5. The main symptom in 37.5% of patients was infertility. The MRI findings included unilateral definitive tubo-ovarian masses (n=4 [16.7%]); bilateral definitive tubo-ovarian masses (n=4 [16.7%]); unilateral hydrosalpinx (n=8 [33.3%]); bilateral hydrosalpinx (n=4 [16.7%]); unilateral adnexal cyst (n=4 [16.7%]), tuberculous deposits on the liver (n=1 [4.2%]); and cervical growth (n=1 [4.2%]). Other associated findings were endometriosis (n=2 [8.3%]), inclusion cyst (n=1 [4.2%]), subserous fibroid (n=1 [4.2%]), sacroiliac joint arthritis (n=1 [4.2%]), and enteritis (n=1 [4.2%]).Conclusion: MRI seems to be a useful modality for the diagnosis of tubercular tubo-ovarian masses. Objective: To determine the clinical characteristics that indicate the presence of tubo-ovarian abscess (TOA) among patients with severe pelvic inflammatory disease (PID).Methods: An observational cohort study was performed from October 2011 to March 2013. The study included all patients with a diagnosis of TOA and PID admitted to a university hospital in Mexico. A complete medical history and physical examination were performed, and laboratory studies were reviewed. A logistic regression analysis was performed on variables with statistical significance.Results: Overall, 26 patients with PID and TOA (TOA group) and 26 with PID without TOA (PID group) were included in the study. Significant differences between patients with TOA and PID were found with regard to the patients' age (39.3years vs 33.1years; P=0.04), educational level (only elementary, 13 [50%] vs 5 [19%]; P=0.14), presentation with fever (23 [88%] vs 16 [62%]; P=0.025), white blood cell count (21.8×10(9)/L vs 14.9×10(9)/L; P<0.001), number of deliveries (2.2 vs 1.1; P=0.01), and presence of diarrhea (16 [62%] vs 5 [19%]; P<0.001). The triad of fever, leukocytosis, and diarrhea was positively related to the presence of TOA.Conclusion: The triad of fever, leukocytosis, and diarrhea should alert clinicians to the possibility of TOA formation in patients with PID. Objectives: To compare the diagnostic accuracy of hysterosalpingography and sonohysterosalpingography in detecting tubal and uterine abnormalities.Methods: In a prospective study for the evaluation of uterine and tubal pathologies, 186 patients with primary and secondary infertility rates (51.6% and 44%, respectively), as well as recurrent abortion rates (3.8%), underwent sonohysterosalpingography. The tubal pathologies, as well as intracavitary and/or structural uterine abnormalities, detected with this procedure were compared with preoperative hysterosalpingography and operative procedures.Results: With surgical findings as the gold standard, sonohysterosalpingography had a sensitivity of 78.2%, a specificity of 93.1%, a positive predictive value of 82.7%, and a negative predictive value of 91%. For total tubal and uterine pathologies, the findings for the same parameters using HSG were 76.3%, 81.8%, 90.9%, and 59.2%, respectively. Sonohysterosalpingography was more accurate than hysterosalpingography for detecting intrauterine adhesions and various forms of uterine anomalies.Conclusions: Sonohysterosalpingography is a safe, easy, accurate, and promising procedure for the detection of female upper genital tract pathologies, especially because it can differentiate specific uterine anomalies. A letter to the editor is presented which focuses on the use of cervicovaginal smears (CVS) in diagnosing primary fallopian tube carcinoma.

The article discusses the unusual case of a 51-year-old with torsion hydrosalpinx after hysterectomy. The woman was admitted because of lower abdominal discomfort and difficulty in micturition. She has a history of vaginal hysterectomy for prolapse with no tubal interruption. The diagnosis found that hydrosalpinx was the cause of her fallopian tube torsion. A case that usually present in the reproductive age group.

Enterosalpingeal fistula is a rare complication of Crohn's disease. And it is rarely diagnosed pre-operatively. We describe two cases of enterosalpingeal fistula. Both cases were diagnosed during exploration and required en bloc resection of the small bowel and fallopian tube. Literature review yielded only one specific case report of ileosalpingeal fistula and two other cases described in a larger series. Management of ileosalpingeal fistula should include resection of the diseased bowel as well as removal of the affected fallopian tube. Crohn's disease has an extremely wide spectrum of clinical manifestations. The hallmark of Crohn's disease is bowel inflammation with fistula or stricture formation. Organs commonly involved in fistula formation include the skin, small bowel, colon, and bladder. Rare fistula sites include the duodenum, stomach and gynecological structures such as the ovaries, fallopian tubes and vagina. We present two cases of Crohn's disease of the terminal ileum fistulizing to the fallopian tube. There is only one specific report of an ileosalpingeal fistula, although another author has described two additional cases in a large series of Crohn's fistulas. [ABSTRACT FROM AUTHOR]
