Quality early care and education (ECE) presents an unparalleled opportunity to avert disadvantage and promote children's development. Mealtimes are essential daily routines, yet are often overlooked in research on ECE quality. This paper crystallises a composite case study of ECE mealtimes in highly disadvantaged communities by combining Departing Radically in Academic Writing (DRAW) methodology with parent surveys, scorings of educator‐child interactions (inCLASS), and field notes. Poverty is perpetuated in these centres: children do not have enough food, and educator‐dictated feeding practices restrict children's opportunities to exercise their agency and learn. The successful rehabilitation and release of raptor chicks can be challenging, especially when the chicks are still in the post‐fledging dependency period. Here, we report on a recently fledged powerful owl chick that was held in care for 33 days before being successfully reunited with its parents. We document the steps undertaken during the entire process from collection from the wild to post‐release monitoring and recommend clinical procedures for treatment of raptors entering veterinary facilities. Success of this rehabilitation was facilitated by early care and treatment for potential rodenticide poisoning, as well as the integration of citizen scientists monitoring the family unit in the field while the chick was in care and during the post‐release period. Given the emerging evidence of widespread rodenticide poisoning in raptors both in Australia and globally, it is critical to suspect all raptors may have been exposed to anticoagulant rodenticides and commence treatment with vitamin K immediately. Summary: Collaborations between Indigenous and non‐Indigenous scientific researchers are increasingly mandated by global to local conservation policy and research ethics guidelines. Breakdowns occur due to misunderstandings around expected protocols of engagement and cooperation, which are compounded by lack of broader awareness of differences in cultural values, priorities and knowledge systems. Using first‐hand experiences, we outline eight key protocols and guidelines that researchers should consider when undertaking research with Indigenous peoples, or on Indigenous Country, through exploration of biocultural protocols and guidelines within Australian and Indigenous customary laws. We use the onion as a metaphor to highlight the layers of protocols and guidelines that researchers can peel back to guide their research from international to local scales, with ethics around the research question at the core. This paper draws on the perspectives and experiences of an Indigenous researcher (as 'insider'/'outsider') and non‐Indigenous researcher ('outsider'), working on a cross‐cultural and multidisciplinary investigation of past Aboriginal dispersal of rainforest trees on the Australian east coast. Issue addressed: Internationally, universities have recognised the benefits of Pet Support Programs (PSPs). However, existing literature has predominately focused on quantitatively measuring health outcomes associated with a PSP amongst student populations. This research aimed to explore the feasibility of a PSP within an Australian university, as there is a need to explore practical considerations involved in implementation. Methods: This research used a qualitative descriptive approach. Participants were recruited via purposive snowball sampling. The sample included 17 participants across eight administrative divisions within an Australian university. Semi‐structured individual interviews were used to collect data. Data were analysed using thematic analysis. Results: Three main themes emerged from the data: Organisational Change, Program Management and Perceived Outcomes. Findings suggest several considerations from both an organisational and program level that need to be addressed prior to implementation, including the university's policies, the location and animal welfare. The main perceived outcomes were stress relief and social support. Ways to minimise risks to those who may have an aversion towards animals were also discussed. Conclusions: Participants believed implementing a PSP would be feasible in the university setting, provided all aforementioned considerations were adequately addressed. Considerations identified have the potential to act as a barrier or a facilitator depending on how they are managed. Using data from an outcome study of adult individual psychoanalytic psychotherapy, conducted in a low‐cost Australian clinic, a mixed‐methods approach was employed to investigate patient discontinuation. This paper addresses the qualitative component of the discontinuation study, which explored patients' reasons for leaving the clinic service upon assessment or in treatment proper. Of 205 patients commencing clinic contact, 41% discontinued during or shortly after the four‐week assessment period, while 40.5% of patients beginning psychotherapy withdrew before reaching the two‐year treatment limit. Across these two groups, former patients were interviewed about their therapy experience and decision to discontinue. Thematic analysis of 20 interview transcripts generated five descriptive categories of discomfort or dissatisfaction prompting discontinuation: clinic factors, therapist factors, patient factors, therapist–patient relationship factors and therapy factors. Findings suggest that experience of the clinic setting itself, together with negative patient perceptions of therapists and therapist interactional style, weres significant influences and that dissatisfaction with the psychotherapy process and outcome was more relevant than problematic patient factors in treatment withdrawal. A number of patients, mainly late discontinuers, reported positive experiences of psychotherapy and significant treatment gains. Aims: To compare the acceptability, reliability and validity of five contemporary diabetes‐specific quality of life (QoL) scales among adults with type 1 diabetes in the United Kingdom and Australia. Methods: Adults with type 1 diabetes (UK = 1139, Australia = 439) completed a cross‐sectional, online survey including ADDQoL‐19, DCP, DIDP, DSQOLS and Diabetes QoL‐Q, presented in randomised order. After completing each scale, participants rated it for clarity, relevance, ease of completion, length and comprehensiveness. We examined scale acceptability (scale completion and user ratings), response patterns, structure (exploratory and confirmatory factor analyses) and validity (convergent, concurrent, divergent and known groups). To assess cross‐country reproducibility, analyses conducted on the UK dataset were replicated in the Australian dataset. Results: Findings were largely consistent between countries. All scales were acceptable to participants: ≥90% completing all items, and ≥80% positive user ratings, except for DSQOLS' length. Scale structure was not supported for the DCP. Overall, in terms of acceptability and psychometric evaluation, the DIDP was the strongest performing scale while the ADDQoL‐19 and Diabetes QoL‐Q scales also performed well. Conclusions: These findings suggest that the recently developed brief (7 items), neutrally worded DIDP scale is acceptable to adults with type 1 diabetes and has the strongest psychometric performance. However, questionnaire selection should always be considered in the context of the research aims, study design and population, as well as the wider published evidence regarding both the development and responsiveness of the scales. Researchers and policy‐makers are increasingly concerned with children's well‐being in Early Childhood Education and Care (ECEC) and Outside School Hours Care (OSHC) settings. Recent research identifies the value of using qualitative methods to understand children's experiences of well‐being in ECEC/OSHC, but there remain few such studies. This article presents findings from a child‐centric qualitative study with Australian children of nurses and police, which sought to understand their experiences of well‐being in ECEC/OSHC. Children's experiences of well‐being were shaped by parental relationships, how informed they were about the rationale for their care arrangements and the degree of age‐appropriate autonomy they experienced. Context Tolerance for ambiguity is essential for optimal learning and professional competence. General practice trainees must be, or must learn to be, adept at managing clinical uncertainty. However, few studies have examined associations of intolerance of uncertainty in this group. Objectives The aim of this study was to establish levels of tolerance of uncertainty in Australian general practice trainees and associations of uncertainty with demographic, educational and training practice factors. Methods A cross-sectional analysis was performed on the Registrar Clinical Encounters in Training (Re CEnT) project, an ongoing multi-site cohort study. Scores on three of the four independent subscales of the Physicians' Reaction to Uncertainty ( PRU) instrument were analysed as outcome variables in linear regression models with trainee and practice factors as independent variables. Results A total of 594 trainees contributed data on a total of 1209 occasions. Trainees in earlier training terms had higher scores for 'Anxiety due to uncertainty', 'Concern about bad outcomes' and 'Reluctance to disclose diagnosis/treatment uncertainty to patients'. Beyond this, findings suggest two distinct sets of associations regarding reaction to uncertainty. Firstly, affective aspects of uncertainty (the 'Anxiety' and 'Concern' subscales) were associated with female gender, less experience in hospital prior to commencing general practice training, and graduation overseas. Secondly, a maladaptive response to uncertainty (the 'Reluctance to disclose' subscale) was associated with urban practice, health qualifications prior to studying medicine, practice in an area of higher socio-economic status, and being Australian-trained. Conclusions This study has established levels of three measures of trainees' responses to uncertainty and associations with these responses. The current findings suggest differing 'phenotypes' of trainees with high 'affective' responses to uncertainty and those reluctant to disclose uncertainty to patients. More research is needed to examine the relationship between clinical uncertainty and clinical outcomes, temporal changes in tolerance for uncertainty, and strategies that might assist physicians in developing adaptive responses to clinical uncertainty. Birthing room practices can either hinder or promote the initiation of breastfeeding. In Australia, midwives usually support mothers and newborns with the first breastfeed. The aim of this paper is to explore midwives reported practice, assisting mothers initiate breastfeeding in the birthing room. A self-report questionnaire that included open-ended questions was mailed to a national sample of Australian midwives. Open-ended responses to questions related to a birthing room practice scenario were analysed using content analysis to facilitate thematic description. Midwives reported that at the immediate time of birth, they offered support and choice to mothers. The midwives also identified that their practice was often impacted on by workplace structures and these were often barriers to their role at this time. Some midwives indicated that they would use a ‘hands-off’ approach at birth, although others indicated ways in which they would intervene including some who reported a ‘hands-on’ approach to assist mothers with the first breastfeed as soon as possible after birth. Lactation education support for midwives is required to change workplace culture and improve practice. More research is needed to establish best practice to support mothers with the first breastfeed. It could be that some level of ‘hands-on’ help provided skillfully may be effective. More research is needed to investigate the effect of ‘hands-on’ help described by skillful experienced midwives, and association with sustained problem free breastfeeding. [ABSTRACT FROM AUTHOR]

Aim and objective. This study aimed to design, implement and evaluate strategies to improve the quality and content of hospital-based postnatal care. Background. Following birth, women report physical health problems, difficulties with breastfeeding, a lack of parenting self-efficacy and there is high occurrence of postnatal distress and depression. Despite these significant needs, women are frequently dissatisfied with the advice and support they receive from hospital-based postnatal care. Design. A pre/post test design compared the effect of multifaceted strategies on perceptions of quality and content of postnatal care, knowledge and experience of postnatal problems, parenting self-efficacy and breastfeeding outcomes. The key strategy, 'one-to-one time', focused on providing women an uninterrupted period of time each day when a midwife would be available to discuss women's concerns about their health and that of their baby. Method. A convenience sample of 146 women at baseline and 148 women postintervention completed a postal self-report questionnaire between 2-4 weeks postpartum. Results. There were no significant differences between baseline and postintervention groups in perceived quality of care, breastfeeding outcomes and maternal self-efficacy. Women experiencing health issues, including insufficient milk supply, backache, abnormal bleeding and urinary incontinence, were more likely to report that they received good or excellent care and advice in the postintervention group. Strategies to increase rest appeared effective with women less likely to report excessive tiredness postintervention. The key strategy, 'one-to-one time' was not consistently implemented, 57% of women reported they received 10 minutes or less of uninterrupted time with a midwife and only 11% reported that they were provided with 20 minutes or more. Conclusion. There is the potential for individualised care to impact on outcomes for women but established routines and institutional priorities are difficult to change. Aim: This study aimed to establish current management practice for patent ductus arteriosus (PDA) among individual consultant neonatologists in Australia and New Zealand, to examine the influences that drives practice and highlight the importance of future randomised controlled trials in the region. Methods: Eligible subjects were identified from the Directory of Neonatal Intensive Care Units in Australia and New Zealand, 2007. A questionnaire was sent online to each consultant and was followed up with a letter and telephone call. Seven questions addressed management approach, the drug used and the treatment regimen, threshold for referral for surgical ligation and the literature influencing practice. Data were collected from 22 August 2007 to 22 November 2007. Results: The overall response rate was 95%. For infants ≤28 weeks or ≤1000 g, all consultants treat PDA by one of four distinct management approaches. Expectant management was favoured by 35%, echocardiographic targeted prophylaxis 32%, presymptomatic treatment 16% and prophylaxis by 17%. There were marked regional variations in practice. Within individual units, more than one approach is used in 14 out of 24 units. Long courses of indomethacin are used to treat PDA by 86%. For 22% of consultants, management is not influenced by published literature. Conclusions: Differences of opinion in the literature are reflected by the heterogeneity in clinical practice across regions and within units. Crucial questions undergoing evaluation are whether data extrapolated from a previous area are relevant to PDA in modern neonatology and whether targeting treatment early can translate to improved clinical outcome. Scribbles, the damage caused by mining larvae of the moth Ogmograptis sp. (Bucculatricidae), are a common sight on the trunks of many Eucalyptus species in eastern Australia. They provide a record of larval behaviour that can be easily measured when the trees shed outer bark layers. Measurements were made of scribbles on three species of eucalypt ( Eucalyptus racemosa ssp. rossii, E. pauciflora and E. delegatensis) in the Australian Capital Territory. Scribble density varied between tree species and study sites but was consistently higher on the southern and eastern aspects of tree trunks. Some characteristics of mining behaviour, such as initial mining direction, were found to be random, but others including mine length and number of direction changes follow distinct patterns but differ between eucalypt species. It is likely that there is more than one species of Ogmograptis Meyrick as currently described ( O. scribula Meyrick, from E. pauciflora) and each eucalypt species may be host to a different species of Ogmograptis. Objectives: The purpose of this study was to examine variables impacting on the performance of healthy adults on the Occupational Therapy Adult Perceptual Screening Test (OT-APST), and present the Australian normative data for this standardised assessment tool. Method: The OT-APST was administered to a sample of healthy adults aged 16-97 (n = 356). Results: Neither gender nor highest level of education significantly impacted on OT-APST performance in the healthy normative group. Increasing age significantly correlated with reduced performance on all OT-APST subscales except for body scheme. Australian normative data for the OT-APST are presented in two age intervals: 16-74 years (n = 232) and 75-97 years (n = 124). Conclusion: Presentation of a large, age-stratified Australian normative data set including cut-off scores for intact/impaired perceptual performance for each of the OT-APST subscales will enable occupational therapists to make a norm-referenced interpretation of scores for individual clients when using this screening tool. Objective: Investigate the nutrition and food safety needs of out-of-school hours care (OSHC) services. Design, subjects and setting: Forty-one OSHC services were visited once during their afternoon session. The nutritional quality of food provided for afternoon tea for five consecutive days, and food safety and serving practices were determined using checklist-based qualitative assessments. The availability of written nutrition and food safety policies was determined and concerns of coordinators recorded. Main outcome measures: The proportion of services that met the criteria for nutritional quality of afternoon tea provided over five consecutive days and the median number of days each nutritional quality criteria was met. Statistical analyses: The proportion of services that met the afternoon tea checklist criteria for the whole five days studied was calculated. In addition, the median and interquartile ranges were calculated for the number of days on which the services met the afternoon tea checklist criteria. The proportion of services that met the food safety checklist criteria was also calculated. Results: Seventeen percent of services offered a high calcium food each day for afternoon tea, 20% offered a good or moderate source of iron and 44% offered a fruit or vegetable. Nearly half of the services (46%) served cordial, however no services offered sticky/high-sugar snack foods. Thirty-four percent of services had policies on food provided, 37% had food safety policies, 39% had a written afternoon tea menu and 58% of these displayed this menu for parents. Service coordinators were most concerned about lack of kitchen facilities, low food budgets and attracting and retaining experienced staff. Conclusions: The results of this needs assessment raises concerns regarding the foods provided and food safety and serving practices in OSHC, similar to those demonstrated in child-care settings for younger children. Strategies used in child care are likely to be successful in the OSHC setting, however, differences such as the older age of children, lack of regulations, limited kitchen facilities and lower levels of qualifications of OSHC staff need to be considered. The aim of the research was to investigate the effect of two methods of dissemination on the implementation of a smoking cessation programme and use of smoking cessation interventions in antenatal clinics. A repeated-measures randomized design was used. Hospital antenatal clinics ( n = 23) were randomized to simple or intensive dissemination groups. All clinics in NSW with > 500 births were asked to participate. A survey of all clinical staff ( n = 323) in 23 antenatal clinics was carried out prior to programme dissemination and 18 months after dissemination ( n = 283). The response rate was 63%at baseline and 64% at follow-up. Smoking cessation intervention significantly increased after dissemination. ( F (18,1) = 49.26, p < 0.001). The average number of smoking cessation interventions provided by clinics after programme dissemination increased from 4.5 to 7. 48 (mean difference 2.98, t (19) 7.08, n < 0.001, 95% CI (2.1-3.86). Type of dissemination did not influence the number of programme components used or the number of smoking cessation interventions offered. Also, the estimated proportion of clients offered intervention by clinicians did not vary due to type of dissemination. A simple mail-out of a smoking cessation programme to antenatal clinics for use during pregnancy can increase clinician intervention for smoking. When more intensive methods of dissemination are used, the quality of the interventions implemented by clinicians improves. This study examined students' perceptions of difficult or challenging clinical situations and how facilitators assisted students in relation to these specific situations. Numerous examples exist demonstrating increases in native vegetation and the expansion and recovery of native animal populations at landscape scales following intense rabbit suppression. Ongoing research on methods for supplementing the impact of biological control agents and managing introduced predators are needed to restore Australia's arid‐zone ecosystems. To mitigate the negative impacts of invasive rabbits in Australia, land managers are permitted to release the biocontrol virus, rabbit hemorrhagic disease virus (RHDV), to reduce rabbit numbers. However, it is strongly recommended that RHDV is not released when young rabbits are present in the population as infection in this cohort is sublethal and induces life‐long virus immunity. The recruitment of these rabbits into the breeding population may make the population harder to control in future, potentially leading to increasing rather than decreased population size. To investigate whether the recommended release guidelines are followed, we obtained data on the supply and release of RHDV by land managers. We then used generalized additive models to investigate Australia‐wide and state‐specific annual and long‐term temporal trends in the supply and release of RHDV. Half of all RHDV supply (47%) and three quarters of reported releases (74%) Australia‐wide occurred during the anticipated major rabbit breeding seasons and when the risk of immunizing young rabbits is greatest. We found evidence of both RHDV supply and release during the anticipated major rabbit breeding seasons in almost all states for which data existed. RHDV supply increased with below average annual rainfall. This may indicate a tendency for land managers to notice, and want to control, rabbits and their impacts more following drier years when both rabbits and their impacts are potentially more damaging. Our study raises concerns regarding the inappropriate release of RHDV by land managers and whether its supply should be restricted to ensure ongoing and effective management of invasive rabbits. More broadly, our study serves as a warning to other conservation and pest management activities reliant on land managers or citizens following implementation guidelines. Aim: Tonsillectomy procedures are a core element of paediatrics; however, perioperative management differs. This study aimed to describe tonsillectomy management, including the burden of pain, nausea and delayed recovery.Methods: A prospective cohort study was undertaken through an audit of tonsillectomy perioperative practice and recovery and survey interviews with family members 7-14 days post-surgery. The study was undertaken at an Australian tertiary referral paediatric hospital between June and September 2016.Results: The audit included 255 children undergoing tonsillectomy, with 127 family members interviewed. Most participants underwent adenotonsillectomy (n = 216; 85%), with a primary diagnosis of obstructive sleep apnoea (n = 205; 80%) and a mean age of 7 years (standard deviation; 3.9). A variety of intra-operative pain relief and antiemetics was administered. Pain was present in 29% (n = 26) of participants at ward return, increasing to 32-45% at 4-20 h and decreasing to 21% (n = 15) at discharge. A third of the children (32%; n = 41) had moderate to severe pain at post-discharge interview, and 30% (n = 38) experienced nausea at home. Most parents (82%; n = 104) were still giving regular paracetamol at 7 days post-operatively, and 31% (n = 39) had finished their oxycodone. Of the participants, 14% (n = 26) presented to the emergency department within 7 days of discharge; 8% (n = 20) of the total cohort were re-admitted.Conclusions: There was variety in perioperative and post-discharge care. Pain scores were infrequently documented post-tonsillectomy, and parents are generally dissatisfied with the management of post-operative pain and nausea. In its invasive range in Australia, the European rabbit threatens the persistence of native flora and fauna and damages agricultural production. Understanding its distribution and ecological niche is critical for developing management plans to reduce populations and avoid further biodiversity and economic losses.We developed an ensemble of species distribution models (SDMs) to determine the geographic range limits and habitat suitability of the rabbit in Australia. We examined the advantage of incorporating data collected by citizens (separately and jointly with expert data) and explored issues of spatial biases in occurrence data by implementing different approaches to generate pseudo‐absences. We evaluated the skill of our model using three approaches: cross‐validation, out‐of‐region validation, and evaluation of the covariate response curves according to expert knowledge of rabbit ecology.Combining citizen and expert occurrence data improved model skill based on cross‐validation, spatially reproduced important aspects of rabbit ecology, and reduced the need to extrapolate results beyond the studied areas.Our ensemble model projects that rabbits are distributed across approximately two thirds of Australia. Annual maximum temperatures >25°C and annual minimum temperatures >10°C define, respectively, the southern and northern most range limits of its distribution. In the arid and central regions, close access to permanent water (≤~ 0.4 km) and reduced clay soil composition (~20%–50%) were the major factors influencing the probability of occurrence of rabbits.Synthesis and applications. Our results show that citizen science data can play an important role in managing invasive species by providing missing information on occurrences in regions not surveyed by experts because of logistics or financial constraints. The additional sampling effort provided by citizens can improve the capacity of SDMs to capture important elements of a species ecological niche, improving the capacity of statistical models to accurately predict the geographic range of invasive species. Farm advisors are recognised as playing an increasingly central role in facilitating interactions between scientists and farmers to improve local implementation of sustainable soil management practices and agricultural innovations more broadly. However, there has been limited scrutiny of what farm advisors do when faced with conflicting interpretations among actors over techniques or approaches for facilitating agricultural innovation. This article advances knowledge in this area by investigating the role of farm advisors in aligning different frames on agricultural soil research and extension across seven Australian mixed farming regions. Drawing upon theoretical work on frame alignment, we argue that farm advisors use three types of strategies to align conflicting frames—frame bridging, frame amplification and frame transformation. These strategies seek to frame local soil research and extension priorities in ways that are assumed to resonate more closely with the frames of multiple constituents, such as farmers and soil scientists. Through our analysis, we argue that the application of a frame alignment approach enables greater precision in identifying which (a) interactive and social learning processes, (b) key local influencers and communities of practice and (c) resourcing and governance arrangements are most likely to be effective in facilitating soil research and extension that is locally useful and useable. Aim: As climate change intensifies and wildfire frequency and scale increase, it is critical we develop a robust understanding of how species recover from these major disturbances. Here, we aim to determine whether source populations for recovery following large‐scale intense wildfires are derived from either in situ survival, or immigration from surrounding unburnt areas (ex situ). Secondly, we sought to determine whether habitat elements (e.g., logs) within the landscape facilitate in situ survival of small mammals during fires. Location: Grampians National Park, south‐eastern Australia. Methods: We used long‐term post‐fire small mammal monitoring to investigate sources of recovery for small mammals, and camera trapping and habitat surveys immediately following large intense wildfires to assess evidence for and drivers of post‐fire survival. Results: We found no relationship between distance to unburnt vegetation and the occurrence of any native species, suggesting that in situ survival is the probable mechanism for recovery of post‐fire mammal populations, compared with immigration from external unburnt areas. We also show that key habitat elements such as rocks and large trees were associated with the occurrence of several species immediately post‐fire, suggesting a role for these features in facilitating the survival of species during and following fire. Main conclusions: We present evidence for post‐fire recovery being driven by in situ survival. In situ survival is facilitated by small unburnt patches and habitat elements in burnt areas. These surviving individuals become the founders for subsequent post‐fire population recovery. Given that globally we are seeing increasingly frequent large‐scale wildfires driven by climate change, the capacity for in situ survival will help mitigate some of the fire‐related impacts of climate change. 1. Leaf trait relationships of native and exotic invasive species from a range of habitats were compared to assess consistency across habitats and the role of disturbance. 2. One hundred and twenty-two native and exotic species were sampled in five habitats in eastern Australia. Specific leaf area, foliar nitrogen ( Nmass), assimilation rate ( Amass) and dark respiration ( Rmass) were measured for each species. Plants were classified into four types: native undisturbed, native disturbed, exotic invasive undisturbed and exotic invasive disturbed. 3. All traits were positively correlated and slopes were homogeneous within habitats. Significant differences between plant types in slope elevation were found in only two of 18 cases. There were significant shifts in group means along a common slope between plant types within habitats. These shifts were associated with disturbed vs. undisturbed areas, with plant types from disturbed areas having higher trait values. 4. Synthesis. Exotic invasive and native species do not have fundamentally different carbon capture strategies. The carbon capture strategy of a species is strongly associated with disturbance, with species from disturbed sites having traits that confer capacity for fast growth. Thus, differences between exotic invasives and natives may reflect differences in the environmental conditions of the sites where they occur rather than differences between exotic invasives and natives per se. Background: To date, no population-level data have been published examining the obstetric and neonatal outcomes for women with an alcohol-related hospital admission during pregnancy compared with the general obstetric population. This information is critical to planning and implementing appropriate services. Methods: Antenatal and delivery admissions to New South Wales (NSW) hospitals from the NSW Inpatient Statistics Collection were linked to birth information from the NSW Midwives Data Collection over a 5-year period (1998–2002). Birth admissions were flagged as positive for maternal alcohol use where a birth admission or any pregnancy admission for that birth involved an alcohol-related International Statistical Classification of Diseases and Related Health Problems, 10th Revision, Australian Modification (ICD-10-AM) code. Key demographic, obstetric, and neonatal variables were compared for births to mothers in the alcohol group with births where no alcohol-related ICD10-AM was recorded. Results: A total of 416,834 birth records were analyzed over a 5-year period (1998–2002). In this time, 342 of these were coded as positive for at least 1 alcohol-related ICD-10-AM diagnosis. Mothers in the alcohol group had a higher number of previous pregnancies, smoked more heavily, were not privately insured, and were more often indigenous. They also presented later on in their pregnancy to antenatal services and were more likely to arrive at hospital unbooked for delivery. Deliveries involved less epidural and local and more general anesthesia. Cesarean sections were more common to women in the alcohol group and were performed more often for intrauterine growth retardation. Neonates born to women in the alcohol group were smaller for gestational age, had lower Apgar scores at 5 minutes, and were admitted to special care nursery more often. Conclusions: This study shows that linked population-level administrative data provide a powerful new source of information for examining the maternal and neonatal outcomes associated with alcohol use in pregnancy. Recreational fisheries contribute substantially to the sociocultural and economic well‐being of coastal and riparian regions worldwide, but climate change threatens their sustainability. Fishery managers require information on how climate change will impact key recreational species; however, the absence of a global assessment hinders both directed and widespread conservation efforts. In this study, we present the first global climate change vulnerability assessment of recreationally targeted fish species from marine and freshwater environments (including diadromous fishes). We use climate change projections and data on species' physiological and ecological traits to quantify and map global climate vulnerability and analyze these patterns alongside the indices of socioeconomic value and conservation effort to determine where efforts are sufficient and where they might fall short. We found that over 20% of recreationally targeted fishes are vulnerable to climate change under a high emission scenario. Overall, marine fishes had the highest number of vulnerable species, concentrated in regions with sensitive habitat types (e.g., coral reefs). However, freshwater fishes had higher proportions of species at risk from climate change, with concentrations in northern Europe, Australia, and southern Africa. Mismatches in conservation effort and vulnerability were found within all regions and life‐history groups. A key pattern was that current conservation effort focused primarily on marine fishes of high socioeconomic value rather than on the freshwater and diadromous fishes that were predicted to be proportionately more vulnerable. While several marine regions were notably lacking in protection (e.g., Caribbean Sea, Banda Sea), only 19% of vulnerable marine species were without conservation effort. By contrast, 72% of freshwater fishes and 33% of diadromous fishes had no measures in place, despite their high vulnerability and cultural value. Aim: Central venous access devices (CVADs) are vital medical devices to support the treatment of paediatric cancer; however, device occlusion is common, which disrupts treatment. This study aimed to improve the identification and management of CVAD occlusions in children with cancer, as well as to identify the demographic, clinical and device characteristics associated with increased risk for CVAD occlusion. Methods: A pre–post‐implementation study was conducted at a metropolitan paediatric oncology facility in Australia, using the Theoretical Domains Framework. Patients with a CVAD for anti‐cancer therapy were prospectively followed for occlusive events pre‐ and post‐ the implementation of clinical resources to support the identification and management of CVAD occlusive events. CVAD occlusion and management data were collected and compared pre‐ and post‐implementation. Risk factors for CVAD occlusion were described by mixed‐effects Poisson regression and incident rate ratios (IRR). Results: A total of 133 CVADs were inserted into 131 patients for a total of 6784 catheter days. The incidence of CVAD‐related occlusion pre‐implementation was 59.7 (95% confidence interval (CI) 51.4–69.0, per 1000 catheter days); compared to 31.6 (95% CI 26.4–37.6); P < 0.01) post‐implementation of clinical resources. In multivariate models, other than post‐implementation phases (IRR 0.51 (95% CI 0.32–0.81)), only neutropaenia significantly increased the risk of CVAD occlusion (IRR 2.14 (95% CI 1.15–3.97)). Conclusion: CVAD occlusions in paediatric oncology are common. The development and implementation of CVAD occlusion resources to guide the identification and management of occlusive episodes led to a significant decrease in occlusive events. Introduction: Robot‐assisted therapy for the upper limb (RT‐UL) is an emerging form of intervention for stroke survivors with upper limb deficits. However, there is limited knowledge regarding therapists' perceptions of RT‐UL and the factors influencing the implementation of RT‐UL into the clinical setting. This is important when considering that therapists in Australia are primarily responsible for the prescription of RT‐UL in daily practice. This study aimed to explore occupational therapists' and physiotherapists' perceptions of RT‐UL and the perceived barriers and enablers influencing implementation. Methods: Two discipline‐specific focus groups were conducted involving occupational therapists (n = 6) and physiotherapists (n = 6). Participants were members of the same multidisciplinary team working in an Australian public health rehabilitation facility where RT‐UL (i.e. InMotion2) was being introduced for the first time. Focus groups explored therapist perceptions of the new RT‐UL as well as perceived barriers and enablers to implementation. Focus groups were recorded, transcribed and deductively analysed using the Theoretical Domains Framework (TDF). Results: Out of the 14 domains of the TDF, 7 were raised by participants during the focus groups: environmental context and resources, beliefs about consequences, optimism, knowledge, skills, social influences, and social and professional role and identity. Therapists' expressed their optimism towards the introduction of RT‐UL but believed successful implementation would be primarily dependent on the availability of clinical leadership, training and a suitable client mix. Conclusion: Therapists perceived that RT‐UL would provide opportunity for increased upper limb practice particularly for patients with severe upper limb impairment. To facilitate implementation, support of RT‐UL should come from both management and clinical leaders and training include RT‐UL efficacy, device functionality and patient suitability. Leaf photosynthetic CO2 responses can provide insight into how major nutrients, such as phosphorus ( P), constrain leaf CO2 assimilation rates ( Anet). However, triose-phosphate limitations are rarely employed in the classic photosynthesis model and it is uncertain as to what extent these limitations occur in field situations. In contrast to predictions from biochemical theory of photosynthesis, we found consistent evidence in the field of lower Anet in high [ CO2] and low [ O2] than at ambient [ O2]. For 10 species of trees and shrubs across a range of soil P availability in Australia, none of them showed a positive response of Anet at saturating [ CO2] (i.e. Amax) to 2 kPa O2. Three species showed >20% reductions in Amax in low [ O2], a phenomenon potentially explained by orthophosphate ( Pi) savings during photorespiration. These species, with largest photosynthetic capacity and Pi > 2 mmol P m−2, rely the most on additional Pi made available from photorespiration rather than species growing in P-impoverished soils. The results suggest that rarely used adjustments to a biochemical photosynthesis model are useful for predicting Amax and give insight into the biochemical limitations of photosynthesis rates at a range of leaf P concentrations. Humans have introduced plants and animals into new continents and islands with negative effects on local species. This has been the case of the olive that was introduced in Australia, New Zealand and Pacific islands where it became invasive. Two subspecies were introduced in Australia, and each successfully invaded a specific area: the African olive in New South Wales (NSW) and the Mediterranean olive in South Australia. Here, we examine their origins and spread and analyse a large sample of native and invasive accessions with chloroplast and nuclear microsatellites. African olive populations from the invaded range exhibit two South African chlorotypes hence supporting an introduction from South Africa, while populations from South Australia exhibit chlorotypes of Mediterranean cultivars. Congruently, nuclear markers support the occurrence of two lineages in Australia but demonstrate that admixture took place, attesting that they hybridized early after introduction. Furthermore, using an approximate Bayesian computation framework, we found strong support for the serial introduction of the African olive from South Africa to NSW and then from NSW to Hawaii. The taxon experienced successive bottlenecks that did not preclude invasion, meaning that rapid decisions need to be taken to avoid naturalization where it has not established a large population yet. Wild rabbits ( Oryctolagus cuniculus) in Australia are the descendents of 24 animals from England released in 1859. We surveyed rabbits and rabbit fleas ( Spilopsyllus cuniculi) in Australia for the presence of trypanosomes using parasitological and PCR-based methods. Trypanosomes were detected in blood from the European rabbits by microscopy, and PCR using trypanosome-specific small subunit ribosomal RNA (SSU rRNA) gene primers and those in rabbit fleas by PCR. This is the first record of trypanosomes from rabbits in Australia. We identified these Australian rabbit trypanosomes as Trypanosoma nabiasi, the trypanosome of the European rabbit, by comparison of morphology and SSU rRNA gene sequences of Australian and European rabbit trypanosomes. Phylogenetic analysis places T. nabiasi in a clade with rodent trypanosomes in the subgenus Herpetosoma and their common link appears to be transmission by fleas. Despite the strict host specificity of trypanosomes in this clade, phylogenies presented here suggest that they have not strictly cospeciated with their vertebrate hosts. We suggest that T. nabiasi was inadvertently introduced into Australia in the 1960s in its flea vector Spilopsyllus cuniculi, which was deliberately introduced as a potential vector of the myxoma virus. In view of the environmental and economic damage caused by rabbits in Australia and other islands, the development of a virulent or genetically modified T. nabiasi should be considered to control rabbits. The aim of this paper is to identify certain important population trends among heroin users in Australia for the period 1971 - 97, such as: population growth, initiation, i.e. the number who were initiated to heroin in a given year, and quitting, i.e. the number that quit using heroin. For this purpose, we summarize and extract relevant characteristics from data from National Drug Strategy Household Survey (NDSHS 1998) conducted in Australia in 1998. We devise a systematic procedure to estimate historical trends from questions concerning past events. It is observed from our findings that the size of the heroin user population in Australia is in a sharp increase, especially from the early 1980s onwards. The general trend obtained for the period 1971 - 97 is strikingly similar to that obtained by Hall et al. (2000) for the dependent heroin user population in Australia, even though their study was based on different datasets and a different methodology. In our reconstruction of the time history we also detect a levelling-off prior to 1990. Initiation is also observed to be on a sharp increase. The latter trend is accompanied by a similar trend of quitting, perhaps indicating a relatively short heroin use career. A sharp decrease in both initiation and quitting is observed after 1990. In conclusion, in the case of the trend in the population of heroin users a high rate of growth has been identified that is consistent with the existing literature. In the process, we demonstrated that even a static survey such as NDSHS 1998 can, sometimes, be used to extract historical (dynamic) trends of certain important variables. With ongoing introductions into Australia since the 1700s, the European rabbit (Oryctolagus cuniculus) has become one of the most widely distributed and abundant vertebrate pests, adversely impacting Australia's biodiversity and agroeconomy. To understand the population and range dynamics of the species and its impacts better, occurrence and abundance data have been collected by researchers and citizens from sites covering a broad spectrum of climatic and environmental conditions in Australia. The lack of a common and accessible repository for these data has, however, limited their use in determining important spatiotemporal drivers of the structure and dynamics of the geographical range of rabbits in Australia. To meet this need, we created the Australian National Rabbit Database, which combines more than 50 yr of historical and contemporary survey data collected from throughout the range of the species in Australia. The survey data, obtained from a suite of complementary monitoring methods, were combined with high‐resolution weather, climate, and environmental information, and an assessment of data quality. The database provides records of rabbit occurrence (689,265 records) and abundance (51,241 records, >120 distinct sites) suitable for identifying the spatiotemporal drivers of the rabbit's distribution and for determining spatial patterns of variation in its key life‐history traits, including maximum rates of population growth. Because all data are georeferenced and date stamped, they can be coupled with information from other databases and spatial layers to explore the potential effects of rabbit occurrence and abundance on Australia's native wildlife and agricultural production. The Australian National Rabbit Database is an important tool for understanding and managing the European rabbit in its invasive range and its effects on native biodiversity and agricultural production. It also provides a valuable resource for addressing questions related to the biology, success, and impacts of invasive species more generally.
