Abstract

There have been many improvements in diabetic foot care in recent years. Unfortunately, we have not achieved the 50% reduction in the number of amputations targeted in the St Vincent Declaration. This may be partially due to better documentation and an increased prevalence of diabetes. More effort is required in particular in the management of the poorer members of society, either in the developed or developing world. The organization of services is a recurrent theme: systems must be population-based, not clinic-based because most patients who present for an amputation have not attended a diabetes clinic. Clinical practice must also become more effective, through improved studies and implementation of results. Most ulcers can be healed but far too many patients suffer a recurrence: footwear and education may provide a key but the evidence base is weak and compliance is a problem. 45. What the Practising Physician Should Know about Diabetic Foot Biomechanics.

Abstract

Many diabetic foot ulcers fail to heal because of simple biomechanical issues that are not covered during medical training. When the body is supported by a single limb during walking, the total force under the foot is approximately 110% of body weight. Bony prominences may experience far greater concentrations of load, often not noticed because of peripheral neuropathy. Certain foot structures predispose an individual to higher pressures and callus concentrates pressure. Footwear is the most important extrinsic determinant of elevated pressure: shoes often fit incorrectly and fail to provide adequate cushioning. Prevention of foot pathology starts with identification of patients with neuropathy or ischaemia: procedures for foot examination are described. The gold standard for healing neuropathic ulcers is the total contact cast, in combination with good wound care, but the cast requires specialist staff. The critical requirement is compliance with non-weight-bearing and the patient needs to understand the rationale for this. Abstract

In this review, the evidence for the role of footwear in causing, healing, and preventing foot ulceration in diabetic patients is discussed. The mechanisms of action of therapeutic footwear are elucidated, and an 'ideal' approach to footwear prescription is outlined. Abstract

To examine specific behaviors used by older adults while ascending and descending stairs and to assess the relationships between confidence and stair performance. Cross-sectional. Center for Locomotion Studies, The Pennsylvania State University. Sixteen male (mean age=82.7, range= 77–89) and 16 female (mean age=82.2, range=77–87) community-dwelling adults. A stair self-efficacy (SSE) test was created to assess individuals' confidence in their safety on stairs. Observational stair performance measures, measures of walking speed on stairs, and the total SSE score were examined for differences due to sex, and the relationships between SSE and specific stair behaviors were assessed. There was a significant relationship between SSE and the safety precautions taken during stair negotiation. Those with lower SSE were more likely to ascend and descend the stairs at a slower speed, use the handrail to a greater extent, and position themselves closer to the rail. The women had lower domain-specific SSE and tended to use the handrail to a greater extent than men even though there were no sex differences in self-reported functional ability or general falls and mobility confidence. A small group of subjects exhibited characteristics of instability, particularly during stair descent, yet most of this group had high SSE scores and failed to use the handrail. It appears that confidence related to stair negotiation plays a major role in determining risk-taking propensity during stair use in older adults. [ABSTRACT FROM AUTHOR]
