The eastern subpolar North Atlantic is a source of nutrients to the Northwest European Shelf and Arctic; however, biogeochemical transports in this important region are unknown. We examine variability in nutrients and carbon at the eastern boundary of the subpolar North Atlantic between 2017 and 2020, and calculate their transport by a branch of the North Atlantic Current and the European Slope Current. By combining observations from moorings and ship‐based surveys, we derive novel biogeochemical property transports at high temporal resolution. Data from 63 m provide new evidence of a strong seasonal signal with silicate declining between April and May (− 2.3 μmol kg− 1 ) and a concurrent increase in pH (0.04) and oxygen saturation (3.5%). Additionally, pH and oxygen saturation show a secondary peak in October during the autumn bloom. Biogeochemical transports are northwards and highly variable with volume transport dominating the variability over a multi‐annual timescale. However, historical data suggests that nitrate and phosphate transports were 15% and 19% lower respectively in the late 2000s when the subpolar gyre circulation was weaker and lower nutrient source waters were dominant. These changes may have been amplified by concurrent reductions in volume transport. Changes in carbon and nutrient transports in the eastern subpolar North Atlantic may propagate downstream with potential effects on the Northwest European Shelf and Eurasian Arctic. The Rockall Trough (RT) accommodates the warmest and saltiest branch of the North Atlantic Current, which delivers water from the Gulf Stream into the marine environment around western Europe. In addition, the European Slope Current (ESC) carries warm water northward along the eastern boundary of the RT, and exchange between the ESC and the continental shelf is a dominant factor in determining the oceanographic conditions around the British Isles. However, the picture of the mean circulation and variability in the RT is still emerging, with a continuous observational campaign still in its relative infancy. The ESC, in particular, is poorly constrained by ship‐based, mooring, and satellite observations. In this paper, we examine the RT circulation and volume transport using a temporally extended and spatially expanded observing network. Six years of continuous mooring occupation reveal that a large‐amplitude, basin‐scale freshening event, previously detected south of Iceland around 2015, impacted the RT around 2017. Geostrophic transport was greatly reduced during this period, driven by a concurrent subsurface temperature increase at the western boundary. The circulation regained strength during the latter part of the record. We gathered 110 glider transects over 22 months which capture the ESC velocity field in unprecedented detail. The data are sufficient to characterize both the mean state and the emergent seasonal variability of the ESC, and reveal the year‐round presence of a southward countercurrent at depth. Variability in the strength and structure of this previously unstudied feature modulates net northward transport in the eastern boundary current system. Plain Language Summary: The Rockall Trough (RT) is a key pathway for warm and salty water flowing northward, a process which plays a key role in dictating the western European climate. However, the amount of water flowing northward, and how it changes over time, is still rather uncertain. In particular, the narrow boundary current in the eastern RT has previously proven difficult to measure. In this paper, we use 6 years of continuous oceanographic observations in the RT to monitor the flow. We also used underwater robots, called gliders, to focus on the narrow current in the eastern boundary. The data gathered give the best description of the ocean currents in the region to date. The study period coincided with a period of profound change across the subpolar North Atlantic, with a strong freshwater signal detected in the RT around 2017. The glider observations give a detailed view of the narrow eastern boundary current and how it varies seasonally, and also revealed the presence of a southward‐flowing current at depth. Determining the long‐term nature of the Atlantic Meridional Overturning Circulation (AMOC) presents a major step in understanding ocean temperature forcing, and is crucial both for placing the recently observed AMOC slowdown in the context of climate change and for predicting future climate. We present a time series for AMOC strength over the last 120 years which is derived solely from observations. Application of the Bernoulli inverse to hydrography yields the general geostrophic circulation in the North Atlantic without requiring additional sea surface height information, allowing AMOC evaluation in the pre‐satellite era. AMOC varies on a multidecadal timescale, leading the Atlantic Multidecadal Variability (AMV) in sea surface temperature by 2.5 years. We consider the dynamics of the implied AMOC/AMV coupling, and hypothesize that the low‐frequency variability in both parameters is driven by large‐scale density anomalies circulating in the AMOC. Introduction: The human immunodeficiency virus 1 (HIV‐1) pandemic is characterized by numerous distinct sub‐epidemics (clusters) that continually fuel local transmission. The aims of this study were to identify active growing clusters, to understand which factors most influence the transmission dynamics, how these vary between different subtypes and how this information might contribute to effective public health responses. Methods: We used HIV‐1 genomic sequence data linked to demographic factors that accounted for approximately 70% of all new HIV‐1 notifications in New South Wales (NSW). We assessed differences in transmission cluster dynamics between subtype B and circulating recombinant form 01_AE (CRF01_AE). Separate phylogenetic trees were estimated using 2919 subtype B and 473 CRF01_AE sequences sampled between 2004 and 2018 in combination with global sequence data and NSW‐specific clades were classified as clusters, pairs or singletons. Significant differences in demographics between subtypes were assessed with Chi‐Square statistics. Results: We identified 104 subtype B and 11 CRF01_AE growing clusters containing a maximum of 29 and 11 sequences for subtype B and CRF01_AE respectively. We observed a > 2‐fold increase in the number of NSW‐specific CRF01_AE clades over time. Subtype B clusters were associated with individuals reporting men who have sex with men (MSM) as their transmission risk factor, being born in Australia, and being diagnosed during the early stage of infection (p < 0.01). CRF01_AE infections clusters were associated with infections among individuals diagnosed during the early stage of infection (p < 0.05) and CRF01_AE singletons were more likely to be from infections among individuals reporting heterosexual transmission (p < 0.05). We found six subtype B clusters with an above‐average growth rate (>1.5 sequences / 6‐months) and which consisted of a majority of infections among MSM. We also found four active growing CRF01_AE clusters containing only infections among MSM. Finally, we found 47 subtype B and seven CRF01_AE clusters that contained a large gap in time (>1 year) between infections and may be indicative of intermediate transmissions via undiagnosed individuals. Conclusions: The large number of active and growing clusters among MSM are the driving force of the ongoing epidemic in NSW for subtype B and CRF01_AE. [ABSTRACT FROM AUTHOR]

A realistic numerical model was constructed to simulate the oceanic conditions and circulation in a large southeast Greenland fjord (Kangerdlugssuaq) and the adjacent shelf sea region during winter 2007–2008. The major outlet glaciers in this region recently destabilized, contributing to sea level rise and ocean freshening, with increased oceanic heating a probable trigger. It is not apparent a priori whether the fjord dynamics will be influenced by rotational effects, as the fjord width is comparable to the internal Rossby radius. The modeled currents, however, describe a highly three‐dimensional system, where rotational effects are of order‐one importance. Along‐shelf wind events drive a rapid baroclinic exchange, mediated by coastally trapped waves, which propagate from the shelf to the glacier terminus along the right‐hand boundary of the fjord. The terminus was regularly exposed to around 0.5 TW of heating over the winter season. Wave energy dissipation provoked vertical mixing, generating a buoyancy flux which strengthened overturning. The coastally trapped waves also acted to strengthen the cyclonic mean flow via Stokes' drift. Although the outgoing wave was less energetic and located at the opposite sidewall, the fjord did exhibit a resonant response, suggesting that fjords of this scale can also exhibit two‐dimensional dynamics. Long periods of moderate wind stress greatly enhanced the cross‐shelf delivery of heat toward the fjord, in comparison to stronger events over short intervals. This suggests that the timescale over which the shelf wind field varies is a key parameter in dictating wintertime heat delivery from the ocean to the ice sheet. Abstract: A high‐resolution numerical hydrodynamic model of Kangerdlugssuaq Fjord and the adjacent southeast Greenland shelf region was constructed in order to investigate the dynamics of fjord‐shelf exchange. Recent studies have suggested that rapid exchange flows, driven by along‐shelf barrier wind events, are the dominant agent of exchange between fjord and shelf. These events are prone to occur during the winter, when freshwater forcing is minimal and observations of the fjord interior are scarce. Subglacial freshwater discharge was held at zero, so that any buoyancy‐driven overturning circulation was driven by melting alone. The model described a geostrophically balanced background flow transporting water masses between the fjord mouth and the glacier terminus, indicating that rotational effects are of order‐one importance. Barrier wind events were found to trigger coastally trapped internal wave activity within fjord, temporarily enhancing exchange and vertical mixing, and causing warm water to oscillate in the along‐fjord direction. These internal waves were also found to enhance the background flow via Stokes' drift. Heat delivery through the fjord mouth was smaller than that recorded in summer observations, however the system is more effective at delivering this heat to the head of the fjord. This article reviews the Mahatma Gandhi National Rural Employment Guarantee Scheme (NREGA), perhaps India's foremost social security scheme, which is in the form of a right for rural households throughout India to work for up to 100 days a year in public works and to be paid the minimum wage for unskilled labourers. It is argued that self-targeting leads to a more pro-poor scheme than a cash transfer programme. However, in many states, especially poor states, work for NREGA is shown to be not a right. This is a major aspect of the corruption which gives the scheme a bad name. Also generally disappointing is the value of the assets created by the public works. But it is argued that NREGA is worth salvaging, especially if the example of the best states could be followed more widely. Attention is particularly drawn to the contribution of NREGA to the empowerment of women and scheduled castes and tribes in much of rural India. [ABSTRACT FROM AUTHOR]

Epstein-Barr virus ( EBV) is associated with several malignancies, including post-transplant lymphoproliferative disorder ( PTLD). Conventional treatments for PTLD are often successful, but risk organ rejection and cause significant side effects. EBV-specific cytotoxic T lymphocytes ( CTLs) generated in vitro from peripheral blood lymphocytes provide an alternative treatment modality with few side effects, but autologous CTLs are difficult to use in clinical practice. Here we report the establishment and operation of a bank of EBV-specific CTLs derived from 25 blood donors with human leucocyte antigen ( HLA) types found at high frequency in European populations. Since licensure, there have been enquiries about 37 patients, who shared a median of three class I and two class II HLA types with these donors. Cells have been infused into ten patients with lymphoproliferative disease, eight of whom achieved complete remission. Neither patient with refractory disease was matched for HLA class II. Both cases of EBV-associated non-haematopoietic sarcoma receiving cells failed to achieve complete remission. Thirteen patients died before any cells could be issued, emphasizing that the bank should be contacted before patients become pre-terminal. Thus, this third party donor-derived EBV-specific CTL cell bank can supply most patients with appropriately matched cells and most recipients have good outcomes. [ABSTRACT FROM AUTHOR]

BACKGROUND rhIGF-I has been used effectively to promote growth in growth hormone insensitivity syndrome (GHIS) in doses ranging from 40 μg/kg twice daily to 150–200 μg/kg once daily. It appears that the dose of 80 μg/kg twice daily s.c. may induce an equivalent response to higher doses with less side-effects. OBJECTIVE To study the efficacy and safety of rhIGF-I, 80 μg/kg twice daily s.c., in children with GHIS and to analyse the relationship of growth response to severity of phenotype. PATIENTS AND DESIGN: Eleven prepubertal children (3 females, 8 males) with GHIS; basal GH > 2.5 μg/l, IGF-I < 50 μg/l, IGFBP-3 < - 2SD; were treated with IGF-I 80 μg/kg twice daily in a multi-centre study. The baseline characteristics of these patients were as follows (mean ± SD): age, 7.5 ± 2.5 years (range, 2.5–11.7 years), bone age (Tanner–Whitehouse - 2 RUS), 5.2 ± 2.4 years (range, 2.3–9.1 years), mean height SDS, - 5.6 ± 1.6 (range, - 3.1 to - 8.1), height velocity (HV), 3.1 ± 1.1 cm/year (range, 1.9–4.9 cm/year). Height, HV, weight, skinfold thickness, puberty stage and bone age were measured at baseline and 6 monthly for 2 years. RESULTS During the first 12 months of IGF-I therapy, the mean ± SD HV was 7.7 ± 1.6 cm/year (range, 6.1–11.2 cm/year), the mean ± SD increase in HV was 4.7 ± 2.1 cm/year (range, 1.7–8.8 cm/year) and the mean ± SD progression of bone age was 1.9 ± 1.0 years (range, 0.8–3.8 years). Pre-treatment height SDS at the start of IGF-I therapy correlated positively with pretreatment serum IGFBP-3 SDS levels (r = 0.85; P < 0.01). There was a significant inverse correlation between gain in height SDS and pre-treatment height SDS (r = - 0.76; P < 0.01). During the 2nd 12 months of therapy, mean HV was 7.0 ± 3.4 cm/year (range 3.8–12.4) change in height SDS from 12 to 24 months was not significantly correlated with pre-treatment height SDS. Subscapular skinfold SDS decreased significantly (P < 0.05) during the study period, whereas there was no significant change in body mass index and triceps skinfold thickness SDS. Adverse events reported in the patient group included headache (2 patients), hypoglycaemia (2 patients), papilloedema (transient, 1 patient), lipohypertrophy (5 patients) and tonsillectomy/adenoidectomy (2 patients). CONCLUSION This study reveals that IGF-I treatment at a dose of 80 μg/kg twice daily is effective in patients with growth hormone insensitivity syndrome. During the first 12 months of therapy, there was a significant inverse relationship between growth response to IGF-I therapy and the severity of the phenotype of growth hormone insensitivity syndrome, as measured by height SDS, at the start of therapy. Patients with a more severe clinical phenotype of growth hormone insensitivity syndrome, who also had most severe IGFBP-3 deficiency, responded better than those who were more mildly affected. In response to R. A. B. Leaper's challenge in the last issue of this journal we present a report which we have just prepared Jot the BBC North-East. We believe that a better understanding of the costs of high unemployment to the country is needed to inform decisions on how much we can afford to pay to tackle the problem. In the final section of the paper we show, how this evidence can be of value in costing policy options. The last detailed costings of unemployment were published in 1982,for the financial year 1981/82. In February 1982 it appears that apolitical decision was taken not to publish the latest Treasury estimate updating their 1981 calculations. Since then numerous parliamentary questions on the subject have received very limited answers disputing the validity of the whole exercise. [ABSTRACT FROM AUTHOR]

Aim: To report the case of a 14-y-old boy with coeliac disease and obesity. Methods and results: A 14-y-old boy presented with episodic diarrhoea associated with eating spaghetti. His body mass index (BMI) at presentation was 37.2 kg/m 2 (>99.9th centile). Both antigliadin and anti-endomysial antibodies were positive, and coeliac disease was diagnosed by jejunal biopsy. His diarrhoea ceased and the gliadin and endomysial antibodies disappeared after starting gluten-free diet. At 17 y, his BMI increased to 42.7 kg/m 2 despite dietary support. Conclusion: Obesity in a child does not exclude the diagnosis of coeliac disease, especially if presenting with suggestive symptoms. [ABSTRACT FROM AUTHOR]
