Background: Respiratory dysfunction is an important contributor to morbidity and mortality in advanced Parkinson's disease (PD), but it is unclear what parameters are sensitive to diagnose and monitor respiratory dysfunction across disease phases. Objectives: We aimed to characterize respiratory dysfunction in mild to moderate PD. Methods: In 20 individuals without cardiopulmonary comorbidity, pulmonary and inspiratory muscle function testing were performed ON‐medication. Subsequently, the acute ventilatory response to hypoxia (HVR) was assessed by gradually decreasing FIO2 from 0.209 (room air) to 0.127, which was compared to eight age‐ and sex‐matched healthy controls under arterial blood gas monitoring. Lastly, on different days, the same 20 individuals with PD underwent six blinded exposures to 45‐min normobaric hypoxia at FiO2 0.163 and 0.127 or placebo OFF‐medication to assess breathing responses. Results: At rest, individuals with greatest PD severity had a lower tidal volume (pairwise comparisons: 0.59 vs. 0.74, P = 0.038–0.050) and tended to have a higher breathing frequency (17.7 vs. 14.4, P = 0.076), despite normal pulmonary function. A 45‐min exposure to hypoxia induced a significantly lower acute HVR in individuals with PD compared to controls (−0.0489 vs. 0.133 L.min/%, P = 0.0038). Acute HVR was reduced regardless of disease severity. Subacute HVR in individuals with milder disease tended to be higher compared to those with more advanced disease (P = 0.079). Conclusions: Respiratory dysfunction is present in individuals with PD, including those with relatively mild disease severity, and is characterized by altered breathing patterns at rest, as well as a lower HVR, despite normal pulmonary and inspiratory muscle function testing. Knowledge of muscle contractile properties, physical fitness, and their associations with perceived fatigue may provide insights into mechanisms inducing fatigue and treatment targets. We aimed to identify differences in contractile properties and physical fitness between populations, and examine associations with perceived fatigue. We pooled data on perceived fatigue, physical fitness, and contractile properties from six studies, including a control group (n = 90), cancer survivors (n = 27), patients with chronic obstructive pulmonary disease (COPD; n = 16), chronic myeloid leukemia (CML; n = 20), and statin users (n = 64). We evaluated differences between populations, and associations of contractile properties and physical fitness with perceived fatigue. Compared with the control group, we found differences in contractile properties of patients with COPD (larger muscle force decline: β = −10.5%, 95% CI = −16.7; −4.2, increase in early relaxation time (Rt): β = 84.4%, 95% CI = 51.7; 117.0, increase in half Rt: β = 83.1%, 95% CI = 45.5; 120.7, muscle force rise (MFR): β = 0.2%/ms, 95% CI = 0.1; 0.3, and decrease in MFR: β = −24.3%, 95% CI = −35.7; −13.0) and statin users (early Rt: β = −5.4 ms, 95% CI = −10.0; −0.8, increase in early Rt: β = 19.8%, 95% CI = 2.5; 37.1). Associations between contractile properties and perceived fatigue varied across populations. Longer relaxation times were associated with higher perceived fatigue in hemato‐oncological populations. To conclude, contractile properties were impaired in patients with COPD and statin users. Associations between contractile properties and perceived fatigue varied across populations. The objectives were to determine the interactive effect of particle size of soyabean meal (SBM) and whole wheat, barley and wheat bran (CER) on growth performance of weanling pigs after an enterotoxigenic Escherichia coli F4 challenge (Experiment 1) and on gastrointestinal (GIT) development immediately after weaning (Experiment 2). Experiment 1 consisted of 192 pigs (24 ± 3 days of age; 7.4 ± 1.1 kg weaning bodyweight [BW]) selected for Escherichia coli (E. coli) F4 susceptibility. Pigs were given an oral E. coli inoculum at postweaning day 7, to induce an enteric health challenge. Experiment 2 consisted of 40 pigs (24 ± 3 days of age; 7.2 ± 1.0 kg weaning BW) that were killed on postweaning day 8 or 9, to determine the effects of particle size on GIT development and functionality. Four experimental diets were used in a 2 × 2 factorial design: (1) coarse CER and coarse SBM, (2) coarse CER and fine SBM (CERcSBMf), (3) fine CER and coarse SBM, or (4) fine CER and fine SBM (CERfSBMf). Results showed no interaction between SBM and CER coarseness on growth performance, GIT development and functionality. Diarrhoea incidence was higher (p < 0.05) for CERfSBMf during the 2 weeks following the E. coli challenge compared to the other diets. Daily gain and feed intake during this period were higher (p < 0.05) for pigs fed CERc compared to CERf. Empty stomach weight tended to be greater by 8% (p = 0.09) for CERc compared to CERf. Gastric protein (p = 0.05) and starch (p = 0.04) disappearances were greater for SBMf compared to SBMc. Thus, CERcSBMf resulted in the best growth performance and lowest diarrhoea incidence during the 2 weeks following the E. coli challenge, which may be explained by changes in stomach functionality but not by changes in other parts of the GIT. LS typically involves the anogenital site where it causes itching and soreness; it may lead to sexual and urinary dysfunction in females and males; however, it may be asymptomatic. First signs of LS are usually a whitening of the genital skin, sometimes preceded by redness and oedema; fissuring, scarring, shrinkage and fusion of structures may follow in its course. LS is associated with an increased risk of genital cancer. LS has a huge impact on the quality of life of affected patients, and it is important to raise more awareness of this not uncommon disease in order to diagnose and treat it early. Objectives: The guideline intends to provide guidance on the diagnostic of LS (part 1), highlight important aspects in the care of LS patients, generate recommendations and treatment algorithms (part 2) on topical, interventional and surgical therapy, based on the latest evidence, provide guidance in the management of LS patients during pregnancy, provide guidance for the follow‐up of patients with LS and inform about new developments and potential research aspects. Materials and Methods: The guideline was developed in accordance with the EuroGuiDerm Methods Manual v1.3https://www.edf.one/de/home/Guidelines/EDF‐EuroGuiDerm.html. The wording of the recommendations was standardized (as suggested by the GRADE Working Group). The guideline development group is comprised of 34 experts from 16 countries, including 5 patient representatives. Results: Ultrapotent or potent topical corticosteroids in females and males, adults and children remain gold standard of care for genital LS; co‐treatment with emollients is recommended. If standard treatment fails in males, a surgical intervention is recommended, complete circumcision may cure LS in males. UV light treatment is recommended for extragenital LS; however, there is limited scientific evidence. Topical calcineurin inhibitors are second line treatment. Laser treatment, using various wave lengths, is under investigation, and it can currently not be recommended for the treatment of LS. Treatment with biologics is only reported in single cases. Conclusions: LS has to be diagnosed and treated as early as possible in order to minimize sequelae like scarring and cancer development. Summary: We examined the comparability of children's nocturnal sleep estimates using accelerometry data, processed with and without a sleep log. In a secondary analysis, we evaluated factors associated with disagreement between processing approaches. Children (n = 722, age 5–12 years) wore a wrist‐based accelerometer for 14 days during Autumn 2020, Spring 2021, and/or Summer 2021. Outcomes included sleep period, duration, wake after sleep onset (WASO), and timing (onset, midpoint, waketime). Parents completed surveys including children's nightly bed/wake time. Data were processed with parent‐reported bed/wake time (sleep log), the Heuristic algorithm looking at Distribution of Change in Z‐Angle (HDCZA) algorithm (no log), and an 8 p.m.–8 a.m. window (generic log) using the R‐package 'GGIR' (version 2.6‐4). Mean/absolute bias and limits of agreement were calculated and visualised with Bland–Altman plots. Associations between child, home, and survey characteristics and disagreement were examined with tobit regression. Just over half of nights demonstrated no difference in sleep period between sleep log and no log approaches. Among all nights, the sleep log approach produced longer sleep periods (9.3 min; absolute mean bias [AMB] = 28.0 min), shorter duration (1.4 min; AMB = 14.0 min), greater WASO (11.0 min; AMB = 15.4 min), and earlier onset (13.4 min; AMB = 17.4 min), midpoint (8.8 min; AMB = 15.3 min), and waketime (3.9 min; AMB = 14.8 min) than no log. Factors associated with discrepancies included smartphone ownership, bedroom screens, nontraditional parent work schedule, and completion on weekend/summer nights (range = 0.4–10.2 min). The generic log resulted in greater AMB among sleep outcomes. Small mean differences were observed between nights with and without a sleep log. Although platform trials have many benefits, the complexity of these designs may result not only in increased methodological but also regulatory and ethical challenges. These aspects were addressed as part of the IMI project EU Patient‐Centric Clinical Trial Platforms (EU‐PEARL). We reviewed the available guidelines on platform trials in the European Union and the United States. This is supported and complemented by feedback received from regulatory interactions with the European Medicines Agency and the US Food and Drug Administration. Throughout the project we collected the needs of all relevant stakeholders including ethics committees, regulators, and health technology assessment bodies through active dialog and dedicated stakeholder workshops. Furthermore, we focused on methodological aspects and where applicable identified the corresponding guidance. Learnings from the guideline review, regulatory interactions, and workshops are provided. Based on these, a master protocol template was developed. Issues that still need harmonization or clarification in guidelines or where further methodological research is needed are also presented. These include questions around clinical trial submissions in Europe, the need for multiplicity control across the whole master protocol, the use of non‐concurrent controls, and the impact of different randomization schemes. Master protocols are an efficient and patient‐centered clinical trial design that can expedite drug development. However, they can also introduce additional operational and regulatory complexities. It is important to understand the different requirements of stakeholders upfront and address them in the trial. This study presents the results of a set of large-scale fire tests performed on horizontal polyurethane foam slabs. Slab thickness, ignition location, and environment (free burn or corner wall) were varied over 23 individual tests. In a continuation of a previous study by the authors, four different slab thicknesses, five different ignition locations, and three environmental modification combinations were tested to examine the potential effects these parameters may have on the test outcomes. Wall configurations were composed of either gypsum plaster board or particle board wall linings and enclosed two sides of the foam slabs. The results obtained were heat release rate, peak HRR, total heat release, and effective heat of combustion, smoke production rate, total smoke production, specific extinction area and soot yield along with flame spread rate calculations obtained using a specifically designed sample tray. These were then analysed to quantify the effects of the test input parameter changes. Results highlight the influence of test input parameter changes on almost all the chosen fire metrics analysed. Slab thickness showed relatively straight forward linear changes to global metrics such as total heat release and total smoke production, while effective heat of combustion showed no change. The smoke production parameters of specific extinction area and soot yield showed minor trends towards higher values with increased slab thickness. Flame spread rates also showed an increased velocity with thicker slabs. Changes to the environment surrounding the foam slabs, from a free-burning scenario to a corner wall configuration that obstructed two sides of the foam slab generally resulted in faster growth of HRR and higher peak values when compared to the free-burn tests. This outcome was most obvious for the wall tests using combustible materials, where growth rates were comparative to an ultra-fast t^2 fire, and peak HRR value rose to over triple that of the free-burn test results. Flame spread rates were highly impacted by the addition of a corner wall, spread rates increased, and the overall pattern of spread rates across the face of the slabs changed entirely. The influence on smoke production values was minor, and for other smoke metrics, the scenario changes showed little influence. However, the values obtained for soot yields were approximately an order of magnitude lower than those given in standard reference books, such as the SFPE handbook for flexible polyurethane foam. This was an interesting additional finding as soot yield values are often directly used in CFD models to simulate smoke spread and determine life safety criteria in fire safety engineering (FSE). Background: The specific nutrient combination Fortasyn Connect (available in Souvenaid®), has been shown to bring benefits to subjects with various stages of Alzheimer's disease (AD). The objective of this meta‐analysis was to investigate the effect of Souvenaid on cognition and memory using individual patient data (IPD) from all published RCTs in the early phases of AD ranging from mild cognitive impairment (MCI) due to AD (prodromal AD) to mild AD. Method: The study population comprised all drug‐naïve subjects with a diagnosis of prodromal or mild AD included in double‐blind, randomised controlled clinical trials (RCTs) testing the efficacy of orally administered Souvenaid. Electronic databases and Google Scholar were searched to find all relevant RCTs. This meta‐analysis compared differences between Souvenaid and control groups using the IPD of analogous scales measuring overall cognition and episodic memory domains that were specified as either primary or secondary endpoints. Result: In total, 779 subjects from three separate RCTs were included (Souvenir I, Souvenir II, and LipiDiDiet). The IPD meta‐analysis showed a significant favorable effect of Fortasyn Connect on episodic memory; the mean difference (MD) was 0.211 (95% confidence interval: 0.05, 0.37); p = 0.01, which was confirmed when corrected for possible confounders and effect modifiers (MD: 0.24 [0.07, 0.41]; p = 0.006) (Figure; Table). The IPD meta‐analysis for overall cognition suggested participants receiving Fortasyn Connect performed better than controls in the combined dataset (MD: 0.16 [−0.00, 0.32]; p = 0.055), which was confirmed when corrected for possible confounding effects and effect modifiers (MD: 0.23 [0.06, 0.41]; p = 0.01) (Figure; Table). Conclusion: The total body of evidence available from RCTs suggests that Fortasyn Connect has a favorable effect on cognition and memory in early AD without any safety concerns, and consequently that this intervention is a viable option in the management of MCI due to AD (prodromal AD) and mild AD. Evaluation of the diet of the pig (Sus scrofa) in natural settings may provide new views on diet optimization for growth and development of commercially raised piglets under farm conditions. A field study was conducted to gain insight in the diet and stomach characteristics of feral piglets. Forty animals (body weight: 4.6 ± 1.37 kg) were collected from the Bahía Samborombón (Buenos Aires, Argentina). Stomachs were weighed after storage in formalin and the particle size distribution of their contents was determined by wet sieving. Diet items present in their stomachs were classified and their proportional weight and relative abundance was calculated. Based on their dentition, 5, 16 and 19 piglets were approximately 1, 3–6 and 6–16 weeks of age respectively. Vegetable matter (mainly 'leaves and stems') was predominantly present in 39 animals. It represented on average 83 ± 36.4% of total stomach contents by weight. The stomachs of 12 piglets contained curd and represented on average 16 ± 35.1% by weight. Other diet items were less abundant or absent. The proportion of stomach particles retained were 24%, 13%, 22%, 13% and 28% for sieves with mesh sizes of 2000, 1000, 420, 210 and <210 µm respectively. For comparison, we used data of farmed piglets of similar age and fed a nutrient‐dense, finely ground diet. Feral piglets' relative empty stomach weights increased with age (p < 0.050), whereas this was not the case for farmed piglets. Relative stomach contents weight increased significantly with age only for farmed piglets (p < 0.050). We infer from our data that feral suckling piglets consumed a variety of non‐milk items, mainly consisting of vegetable material with a coarse particle size from their first week in life onwards. Summary: Background and Objectives: Infant feeding affects child growth and later obesity risk. We examined whether protein supply in infancy affects the adiposity rebound, body mass index (BMI) and overweight and obesity up to 11 years of age. Methods: We enrolled healthy term infants from five European countries in a double blind randomized trial, with anticipated 16 examinations within 11 years follow‐up. Formula‐fed infants (n = 1090) were randomized to isoenergetic formula with higher or lower protein content within the range stipulated by EU legislation in 2001. A breastfed reference group (n = 588) was included. Adiposity rebound and BMI trajectories were estimated by generalized additive mixed models in 917 children, with 712 participating in the 11 year follow‐up. Results: BMI trajectories were elevated in the higher compared to the lower protein group, with significantly different BMI at adiposity rebound (0.24 kg/m2, 0.01–0.47, p = 0.040), and an increased risk for overweight at 11 years (adjusted Odds Ratio 1.70; 1.06–2.73; p = 0.027) but no significant difference for obesity (adjusted Odds Ratio 1.47; 0.66–3.27). The two formula groups did not differ in the timing of adiposity rebound, but all children with obesity at 11 years had an early adiposity rebound before four years. Conclusions: Compared to conventional high protein formula, feeding lower protein formula in infancy lowers BMI trajectories up to 11 years and achieves similar BMI values at adiposity rebound as observed in breastfed infants. The large majority of germline alterations identified in the DNA mismatch repair (MMR) gene PMS2, a low‐penetrance gene for the cancer predisposition Lynch syndrome, represent variants of uncertain significance (VUS). The inability to classify most VUS interferes with personalized healthcare. The complete in vitro MMR activity (CIMRA) assay, that only requires sequence information on the VUS, provides a functional analysis‐based quantitative tool to improve the classification of VUS in MMR proteins. To derive a formula that translates CIMRA assay results into the odds of pathogenicity (OddsPath) for VUS in PMS2 we used a set of clinically classified PMS2 variants supplemented by inactivating variants that were generated by an in cellulo genetic screen, as proxies for cancer‐predisposing variants. Validation of this OddsPath revealed high predictive values for benign and predisposing PMS2 VUS. We conclude that the OddsPath provides an integral metric that, following the other, higher penetrance, MMR proteins MSH2, MSH6 and MLH1 can be incorporated as strong evidence type into the upcoming criteria for MMR gene VUS classification of the American College of Medical Genetics and Genomics and the Association for Molecular Pathology (ACMG/AMP). Concomitant use of nonsteroidal anti‐inflammatory drugs (NSAIDs) and antithrombotic agents is associated with increased risks of both bleeding and thromboembolism. In this prospective intervention study, community pharmacists screened for NSAID‐antithrombotic interactions and contacted the prescribing physician to discuss interaction management. We included 782 interactions; these were found in an older, polymedicated patient population (mean age: 68 y, median of 5 other drugs). Ibuprofen (in 43.0% of cases) and low‐dose aspirin (78.8%) were the most frequently involved NSAID and antithrombotic, respectively. Anticoagulants were involved in 16.1% of interaction cases. For 61% of cases, the interacting drugs were prescribed by the same physician. The pharmacist–physician discussion about how to manage the interaction mostly resulted in no change of pharmacotherapy (60.7%); the most frequent reason given by physicians was that the NSAID was for short‐term use only. In 39.3% of cases the discussion resulted in a pharmacotherapy change; replacing the NSAID by paracetamol was the most common change. [ABSTRACT FROM AUTHOR]

Summary: Past research on the effect of oxygen concentration on the heat release rate and carbon monoxide yield has focused on materials in isolation and there is a lack of research that studies end‐use products. This paper investigates the effect of oxygen concentration on the heat release rate per unit area (HRRPUA) and carbon monoxide yield for two different cables and a mineral oil used as an operating fluid for vacuum pumps. A standard cone calorimeter was used to conduct the tests at an ambient oxygen concentration of 21% and an open controlled atmosphere cone calorimeter (OCACC) was used for the tests conducted below 21%. The OCACC allows cone calorimeter tests to be run at different oxygen concentrations by attaching a chamber to the standard cone calorimeter. The effects of oxygen concentration on the HRRPUA and carbon monoxide yield for the cables were limited when there was flaming over the entire exposed surface area. For the mineral oil, there was a large decrease in the HRPPUA at a reduced oxygen concentration, but the carbon monoxide yield appeared to be unaffected. [ABSTRACT FROM AUTHOR]

We examined the relative contribution of genetic, shared environmental and non-shared environmental factors to the covariance between parental sensitivity and limit-setting observed twice in a longitudinal study using a child-based twin design. Parental sensitivity and parental limit-setting were observed in 236 parents with each of their same-sex toddler twin children (Mage  = 3.8 years; 58% monozygotic). Bivariate behavioral genetic models indicated substantial effects of similar shared environmental factors on parental sensitivity and limit-setting and on the overlap within sensitivity and limit-setting across 1 year. Moderate child-driven genetic effects were found for parental limit-setting in year 1 and across 1 year. Genetic child factors contributing to explaining the variance in limit-setting over time were the same, whereas shared environmental factors showed some overlap. [ABSTRACT FROM AUTHOR]

Background: Studies concerning pediatric lichen sclerosus are limited, and, to date, there have been no studies comparing the course of lichen sclerosus in boys and girls. We sought to examine all publications on boys and girls with lichen sclerosus and assess and compare epidemiology, symptoms and signs, genetic background, risk factors, treatment, and prognosis. Methods: A systematic search was performed in the Embase, Medline, Cochrane, and Web of Science databases. Inclusion criteria were information on children ages 0–18 years and a clinical or histologic diagnosis of lichen sclerosus. Literature from 1985 to 2021 was reviewed. Results: A total of 1780 articles were retrieved from the search, of which 90 articles were eligible for inclusion. Boys and girls present similarly on many aspects; nonetheless, treatment and follow‐up are approached differently. Conclusions: Though the clinical approach is often different, lichen sclerosus in boys and girls demonstrates many similarities. More research is needed, especially on follow‐up, to gain a better understanding of the course of lichen sclerosus and establish an advanced management plan for children. [ABSTRACT FROM AUTHOR]

Adolescence is a developmental period characterized by significant changes in brain architecture and behaviour. The immaturity of the adolescent brain is associated with heightened vulnerability to exogenous agents, including alcohol. Alcohol is the most consumed drug among teenagers, and binge‐drinking during adolescence is a major public health concern. Studies have suggested that adolescent alcohol exposure may interfere with the maturation of frontal brain regions and lead to long‐lasting behavioural consequences. In this study, by using a slightly modified version of the Drinking in the Dark paradigm, adolescent C57Bl6 mice reach high blood alcohol concentration after voluntary binge‐drinking. In order to assess short‐ and long‐term consequences of adolescent alcohol exposure (AAE), a battery of behavioural tests was performed during late adolescence and during adulthood. We showed that AAE had no short‐term effect on young mice behaviour but rather increased anxiety‐ and depressive‐like behaviours, as well as alcohol consumption during adulthood. Moreover, alcohol binge‐drinking during adolescence dramatically decreased recognition memory performances and behavioural flexibility in both adult males and females. Furthermore, we showed that voluntary consumption of alcohol during adolescence did not trigger any major activation of the innate immune system in the prefrontal cortex. Summary: Cone calorimeter and controlled atmosphere cone calorimeter experiments were conducted on various samples. The intent of the tests was to examine the behavior of uniform and composite samples in a range of thicknesses, irradiances, and oxygen concentrations. Single, uniform layers of acrylonitrile butadiene styrene (ABS) were compared to a composite mix, comprising of ABS with a surface layer of cardboard and a secondary layer of polyethylene bubble wrap (intended to represent a potential storage arrangement). The horizontal samples have been tested at irradiances of 25 and 50 kW/m2 and oxygen concentrations of 20.95%, 17%, and 15% to examine a range of significant variables. Results for the uniform arrangement indicated various correlations, previously observed in the works of others, such as the relationships typically described between applied heat flux, ignitability, heat release rate and the effect of the introduction of hypoxic conditions. However, results were shown to change significantly when samples were arranged to feature composite layers. A hypothesized cause of the behavioral change, namely the soot and char residual introduced from the incomplete combustion of the cardboard layer, highlights further important variables that require consideration in material testing under hypoxic conditions. Such variables, namely specific material behaviors and sample orientation, must be sufficiently captured in the design methodologies of systems reliant upon the introduction of hypoxic conditions. It is concluded that sufficiently capturing a wider range of variables in burning materials under hypoxic conditions will introduce further design resilience and help optimize fire protection/prevention methods. Physical activity (PA) is a complex human behavior, which implies that multiple dimensions need to be taken into account in order to reveal a complete picture of the PA behavior profile of an individual. This scoping review aimed to map advanced analytical methods and their summary variables, hereinafter referred to as wearable‐specific indicators of PA behavior (WIPAB), used to assess PA behavior. The strengths and limitations of those indicators as well as potential associations with certain health‐related factors were also investigated. Three databases (MEDLINE, Embase, and Web of Science) were screened for articles published in English between January 2010 and April 2020. Articles, which assessed the PA behavior, gathered objective measures of PA using tri‐axial accelerometers, and investigated WIPAB, were selected. All studies reporting WIPAB in the context of PA monitoring were synthesized and presented in four summary tables: study characteristics, details of the WIPAB, strengths, and limitations, and measures of association between those indicators and health‐related factors. In total, 7247 records were identified, of which 24 articles were included after assessing titles, abstracts, and full texts. Thirteen WIPAB were identified, which can be classified into three different categories specifically focusing on (1) the activity intensity distribution, (2) activity accumulation, and (3) the temporal correlation and regularity of the acceleration signal. Only five of the thirteen WIPAB identified in this review have been used in the literature so far to investigate the relationship between PA behavior and health, while they may provide useful additional information to the conventional PA variables. Summary: CERN operates one of the most complex particle accelerator facilities in the world. Several different hazards, including fires, are present and need to be investigated and reduced to a tolerable level. Toward this goal, CERN aims at developing a catalog containing detailed fire dynamics descriptions of combustible items present in its facilities. This paper contributes to this catalog in two ways. First, through the development of a design fire calculator for electrical cabinets that allows the determination of potential design fire curves for any number of electrical cabinets/racks. The second contribution was to experimentally characterize the smoke production rates and smoke particle properties of the most common cables and insulating oils used at CERN by coupling a fast particle mobility analyzer to a cone calorimeter. The two particle size modes (accumulation and nucleation mode) could be linked to the fire properties and heat release rate. Accumulation mode particles (~200 nm) were associated with high heat release rates and high soot emissions from the flame. This study identifies a necessity to consider ultrafine particle emissions with low mass emissions but high number emissions in relation to risk assessments pertaining to nuclear facilities and dispersion of radioactive aerosols to the surrounding environment. Those using intermittent catheterization (IC) have many catheter types and styles to select from; including what manufacturers refer to as discreet catheters. Discreet catheters are often manufactured to be smaller in length, coiled in design, or foldable, thus allowing for easier concealment or discretion when carried in public and offering a degree of personal privacy. This study explores discreet catheter use and its alignment with the lifestyle experience of those who use such catheters. This study assesses the user and user experience with discreet catheters. User experience is defined as catheter choice, associated activities, and urinary tract health. A cross‐sectional survey investigated life and health status of individuals in the Netherlands requiring IC. The study enrolled 1892 individuals of which 784 use a discreet catheter as their primary catheter. Catheter choice: 82% of study respondents state they have the ability to choose the type of catheter they use. Of those indicating not having the ability to choose their catheter, they state their health care professional did not inform them of different catheters, brands, or options. Catheter associated activities: Those employed full or part time are satisfied with their discreet catheter (95.1%) claiming it is discreet, and gives them confidence to live their life the way they choose; that the catheter did not interference with social activities (72.6%), and had little to no interference with work outside of the home (76.1%). Urinary tract infections (UTI): 39.4% report a clinician diagnosed UTI within 6 months prior to the survey. The data indicates that 18.3% of the sample are deficient in UTI knowledge stating they do not know what a UTI is, or what associated symptoms are; 41% followed a clinician suggested regimen for catheterizing; 25.5% altered catheterization schedules to fit daily activities; 33.3% stated they did not follow a regimen, but catheterized when convenient The data supports positive experiences for individuals who use discreet catheters as their primary method of bladder voiding, particularly socially active individuals requiring an element of personal privacy in their life. SUMMARY Fire safety provisions in building regulation are about managing risk. In order to appropriately characterize and incorporate risk measures into building regulation, it is helpful to view building regulatory systems (BRS) as complex socio‐technical systems (STS), wherein there are interactions between institutions, technology and people, which ideally work together to mitigate risk to a societally tolerable level. A description of BRS as STS and how to assess the efficacy of the BRS in managing fire risk is presented. To illustrate how STS concepts can be used to evaluate and restructure a functional‐ or performance‐based BRS, STS concepts are applied to the evaluation of the building regulatory system in England. Dietary fibre (DF) is implicated in gastrointestinal health of weaned piglets, either through its physiochemical properties, through modulation of gut microbiota and (or) improved gut integrity. We aimed to study the effect of DF enriched supplemental diets fed to suckling piglets ('creep feed') on health and performance after weaning when challenged with an enterotoxigenic E. coli (ETEC). Seventy‐two piglets originating from 28 litters had been fed four creep diets, that is a low‐fibre control (CON); a diet containing 2% long‐chain arabinoxylans from wheat (lc‐AXOS) or 5% purified cellulose (CELL) or a diet containing the high fermentable and the low‐fermentable fibre source (i.e. 2% lc‐AXOS and 5% CELL). Upon weaning, piglets were individually housed and all fed the same diet. On days 7, 8 and 9, animals received an oral dose of ETEC (5 ml containing 107 to 108 CFU/ml). Besides growth performance, faecal and skin scores were recorded daily. Gut permeability was assessed by urinary excretion of Co‐EDTA prior and post‐ETEC challenge. Repeated measures in time were statistically evaluated with generalized linear mixed models. We used a binominal distribution for evaluating the faecal and skin scores. Feed intake and body weight gain did not differ between treatments (p >.05). Piglets on CELL decreased gain:feed ratio in week 2 + 3 week compared to CON (p =.035). Prior to ETEC challenge, gut permeability tended to increase for lc‐AXOS (p =.092). Moreover, lc‐AXOS as main effect increased intestinal permeability before ETEC challenge (p =.013), whereas the low‐fermentable fibre lead to elevated intestinal permeability after ETEC challenge (p =.014). The incidence of diarrhoea was higher for lc‐AXOS + CELL compared with lc‐AXOS (p =.036), while skin condition was unaffected. Summary This study experimentally investigates the radiation heat flux distribution received on the floor due to fire plume impinging upon an inclined ceiling, which has not been quantified previously. The radiation heat fluxes were measured on the floor for 160 experimental conditions, involving various fire source heat release rates, source‐ceiling heights, angles of ceiling inclination and dimensions (aspect ratios) of the rectangular sources. The main findings include that the declining rate of the radiation heat flux along with distance received by the downstream floor decreases, while that received by the upstream floor increases, with the increasing of ceiling inclination angle. The radiation heat flux received by the floor is higher as the ceiling inclination angle is smaller for the downstream side, while it is lower as the ceiling inclination angle is smaller for the upstream side. Both of these variations can be explained by change of the flow distribution as well as flame length due to combustion and heat released in the two directions beneath the inclined ceiling. Further, a model with various fire source heat release rates, source‐ceiling height, and ceiling inclination angles is proposed, to globally describe the radiation heat flux received by both the upstream and downstream floors. Summary: This study presents the results from a set of 11 large‐scale open fire tests performed on flexible polyurethane foam slabs/mattresses. The purpose of the study was to investigate the influence of the ignition location on the fire behaviour of the foam slabs and to generate data on a highly characterised material that could be used for modelling work in the future. A method for obtaining spatially resolved flame spread data for this type of material was presented using a gridded array of 5 × 10 thermocouples placed on the underside the foam slab and from this, flame spread was examined using three different approaches. The heat release rate (HRR) results showed clear shapes forming that were dependent on the ignition location, with two distinct behaviours being observed between the various different ignition locations, this was also observed in the calculated flame spread rate (FSR) data. Results within an individual test, showed the calculated range of FSRs over the geometry of the slab varied between approximately 1 and 8 mm/s depending on the ignition location. The average FSR values between tests varied between 3 and 7 mm/s and the maximum and minimum values were calculated to be approximately 11 and 2 mm/s respectively. [ABSTRACT FROM AUTHOR]

The open abdomen requires intensive and specific treatment efforts. Long hospital admissions, treatment duration, high mortality rates, deferred and delayed wound closures with alloplastic materials or elaborate closure techniques, and the need for subsequent surgical procedures justify and call for implementation of new therapy options. The case presented here demonstrates the use of a new product (Fasciotens Abdomen) to prevent fascial retraction in the open abdomen of an extubated, conscious patient with four-quadrant peritonitis after perforated appendicitis. Controlled, anteriorly directed fascial traction of 50-60 Newtons prevented fascial retraction during open treatment of the abdomen. Once edema was reduced, abdominal closure was completed without difficulty. This new form of therapy was well tolerated by the patient and led to a markedly more rapid abdominal closure without mesh or abdominal wall reconstruction. [ABSTRACT FROM AUTHOR]

Connectivity of populations through the transfer of individuals is one of the key processes for maintaining ecosystem stability and resilience of coastal ecosystems. During reproduction, dislodged seaweeds of the genus Sargassum form large pelagic surface rafts that can persist for several weeks, and potentially act as a dispersal vector. In surface rafts, seaweeds are rapidly exposed to increased light, elevated temperatures, and altered hydrodynamic conditions. Acclimation to surface conditions is necessary for survival during the rafting phase, but there is limited knowledge of seaweed physiology in rafts. To understand the mechanisms for acclimation, we created rafts of floating Sargassum spinuligerum for 2 weeks and compared these experimental fronds to those attached to the seafloor. We measured nutrients, photosynthetic and nonphotosynthetic pigments, and phlorotannins to investigate their role in the persistence of mature Sargassum at the ocean surface as indicated by photosynthetic rates and reproductive status. We also studied potential surface movement of the rafted seaweed over 3‐weeks of particle tracking using an existing oceanographic model. Photosynthesis and reproductive status were similar between benthic and rafted seaweeds, indicating no change to overall metabolic processes during rafting. While phlorotannin concentrations and photosynthetic pigments were unchanged, photoprotective xanthophyll pigments were more abundant in rafted individuals, suggesting acclimation to surface light conditions. Our results suggest that, in the short term, S. spinuligerum employ chemical strategies to acclimate and maintain physiological processes in the rafting environment and potentially fix more carbon, allowing these rafts to act as dispersal vectors among populations over tens of kilometers apart. Summary: This work aims to explore possible ways of improving the precision of ignition measurements in the cone calorimeter. Both inherent repeatability of parts of the testing equipment and operator‐dependent variations are considered. Inherent repeatability is indicated to be slightly improved if the test samples used are circular rather than square. Operator‐dependent variation is discussed in terms of the method used for determining ignition. Four procedures are compared, namely, visual observation, usage of a light sensor, and looking at the peak of the second and first derivatives of the mass loss and heat release curves, respectively. Results indicate that the preferable operator‐independent method depends on the test conditions; the derivative of the heat release rate is an alternative to the mass loss rate derivative when the scale is of standardised quality. Summary: Background: While non‐eosinophilic asthmatics are usually considered poorly responsive to inhaled corticosteroids (ICSs), studies assessing a step‐down of ICS in this specific population are currently lacking. Objectives: To assess the proportion of non‐eosinophilic asthmatics in whom ICS may be withdrawn without any clinical degradation and to determine the predictive markers of a failure to stop treatment with ICS. Methods: This prospective study was completed by 36 non‐eosinophilic asthmatics, defined by sputum eosinophils <3% and blood eosinophils <400/μL. In these patients, whichever the baseline asthma control level, the dose of ICS was gradually reduced every 3 months until they met the failure criteria or successfully discontinued ICS for 6 months. The failure criteria were an ACQ score ≥1.5 with an increase from baseline >0.5 or a number of severe exacerbations during the study which was greater than the number during the year prior to the baseline visit. Receiver‐operating characteristic (ROC) curves were constructed to assess predictors of a failure to stop ICS. This study is registered with ClinicalTrials.gov, number NCT02169323. Results: In 14 patients (39%), ICSs were completely withdrawn, and in 10 further patients (28%), ICS were stepped‐down to a reduced ICS dose without any deterioration of asthma control and exacerbation rate. Baseline predictors of a failure to stop ICS were a greater age (area under ROC curve [ROC AUC] and [95% CI]: 0.77 [0.62‐0.93]) and elevated blood eosinophils (ROC AUC [95% CI]: 0.77 [0.61‐0.93]). After the first step‐down of ICS, the best predictor was an elevated blood eosinophil count (ROC AUC [95% CI]: 0.85 [0.72‐0.99]). Conclusions & Clinical Relevance: Withdrawing or reducing the dose of ICS is feasible in two‐thirds of non‐eosinophilic asthmatics irrespective of baseline asthma control. Summary: Background: Stopping nucleos(t)ide analogues (NA) after hepatitis B e antigen (HBeAg) seroconversion is associated with high relapse rates in Asian patients, but data in Caucasian cohorts are scarce. Clinical course, outcomes and immunological aspects of chronic hepatitis B infections differ substantially between distinct ethnicities. Aim: The aim of this study was to determine relapse rates, factors predicting relapse and clinical outcomes after nucleos(t)ide analogue cessation in a large, predominantly Caucasian cohort of chronic hepatitis B patients with nucleos(t)ide analogue‐induced HBeAg seroconversion. Methods: This is a nationwide observational cohort study including HBeAg positive, mono‐infected chronic hepatitis B patients with nucleos(t)ide analogue‐induced HBeAg seroconversion from 18 centres in Belgium. Results: A total of 98 patients with nucleo(s)tide analogue‐induced HBeAg seroconversion were included in the study. Of the 62 patients who stopped treatment after a median consolidation treatment of 8 months, 30 relapsed. Higher gamma‐glutamyl transferase levels at both treatment initiation (HR 1.004; P = 0.001 per unit increment) and HBeAg seroconversion (HR 1.006; P = 0.013 per unit increment) were associated with an increased risk of clinically significant relapse in a multivariate Cox regression model. Treatment cessation led to liver‐related death in 2 patients, of whom one showed a severe flare. Of the patients who continued treatment after HBeAg seroconversion, none relapsed or developed severe hepatic outcomes. Conclusion: Treatment withdrawal in Caucasian chronic hepatitis B patients after nucleos(t)ide analogue‐induced HBeAg seroconversion results in viral relapses in more than half of patients with potential fatal outcomes. Background and purpose: Intravenous thrombolysis (IVT) is the only approved pharmacological treatment for acute ischemic stroke. Off‐label IVT for ischemic stroke is common. We aimed to analyse its safety in a large database. Methods: This was a retrospective analysis of the safe implementation of treatments in stroke (SITS) thrombolysis registry with regard to 11 off‐label criteria according to the European licence for alteplase. Symptomatic intracranial haemorrhage (SICH) according to SITS was defined as primary safety endpoint and SICH according to the European Cooperative Acute Stroke Study (ECASS II) definition and the National Institute of Neurological Disorders and Stroke definition as secondary safety endpoints. Multivariable logistic regression analyses after replacing missing values using multiple imputations were performed. Results: Patients from 793 centres in 44 countries were included, mainly (95%) in Europe. A total of 56 258 patients who were treated with intravenous alteplase were included. Median age was 71 (IQR 61–78) years and median National Institutes of Health Stroke Scale score was 12 (IQR 7–17). A total of 16 740 (30%) patients received off‐label IVT and 1037 (1.8%) patients suffered from SICH according to the SITS definition (SICH SITS). Median percentage of missing values per variable was 0.4%. The only two off‐label criteria constituting independent positive and negative predictors for SICH SITS were high blood pressure (odds ratio, 1.39; 95% confidence interval, 1.08–1.80; P = 0.012) and minor stroke (odds ratio, 0.51; 95% confidence interval, 0.33–0.78; P = 0.002). Very severe stroke, previous stroke and diabetes, age and high glucose levels were additional independent predictors of SICH according to the ECASS II and National Institute of Neurological Disorders and Stroke definitions. Conclusions: Thrombolysis appears to be safe with regard to SICH for most of the off‐label criteria, especially for minor stroke, but is risky in patients with high blood pressure. The aim was to develop sedentary (sitting/lying) thresholds from hip and wrist worn raw tri-axial acceleration data from the ActiGraph and GENEActiv, and to examine the agreement between free-living time spent below these thresholds with sedentary time estimated by the activPAL. Sixty children and adults wore an ActiGraph and GENEActiv on the hip and wrist while performing six structured activities, before wearing the monitors, in addition to an activPAL, for 24 h. Receiver operating characteristic (ROC) curves were used to determine sedentary thresholds based on activities in the laboratory. Agreement between developed sedentary thresholds during free-living and activPAL were assessed by Bland-Altman plots and by calculating sensitivity and specificity. Using laboratory data and ROC-curves showed similar classification accuracy for wrist and hip thresholds (Area under the curve = 0.84-0.92). Greatest sensitivity (97-98%) and specificity (74-78%) were observed for the wrist thresholds, with no large differences between brands. During free-living, Bland-Altman plots showed large mean individual biases and 95% limits of agreement compared with activPAL, with smallest difference for the ActiGraph wrist threshold in children (+30 min, P = 0.3). Sensitivity and specificity for the developed thresholds during free-living were low for both age groups and for wrist (Sensitivity, 68-88%, Specificity, 46-59%) and hip placements (Sensitivity, 89-97%, Specificity, 26-34%). Laboratory derived sedentary thresholds generally overestimate free-living sedentary time compared with activPAL. Wrist thresholds appear to perform better than hip thresholds for estimating free-living sedentary time in children and adults relative to activPAL, however, specificity for all the developed thresholds are low. Modelling fire performance of building fire barriers would allow optimising the design solutions before performing costly fire resistance tests and promote performance-based fire safety engineering. Numerical heat conduction analysis is widely used for predicting the insulation capability of fire barriers. Heat conduction analysis uses material properties and boundary condition parameters as the input. The uncertainties in these input parameters result in a wide range of possible model outcomes. In this study, the output sensitivity of a heat conduction model to the uncertainties in the input parameters was investigated. The methodology was applied to stone wool core sandwich panels subjected to the ISO 834 standard fire resistance temperature/time curve. Realistic input parameter value distributions were applied based on material property measurements at site and data available in literature. A Monte Carlo approach and a functional analysis were used to analyse the results. Overall, the model is more sensitive to the boundary conditions than to the material thermal properties. Fires in enclosures equipped with mechanical ventilation remain one of the key issues for fire safety assessment in multifamily homes and industries. Therefore, a wide variation of methods for preventing smoke spread through the ventilation system exist and are applied, in performance-based designs. Through the use of the heating, ventilation and air conditioning (HVAC) model in the fire dynamics simulator, several different common and less common methods for preventing smoke spread in the ventilation system were tested. The effects on smoke spread with changing building leakage and fire growth rates were also investigated. The results were evaluated by determining the total soot spread from the fire room to other compartments connected to the ventilation system, as well as soot/thermal load on the fans and system in general. The maximum and average heat release rate was also of interest and hence compared between systems. It was found that, while many methods perform similar, a few proven methods, such as fire and smoke dampers, performed very well with very little smoke spread to the rest of the system. The study should be considered as an introduction to implementing a similar methodology in specific cases because different ventilations systems will present very different challenges and weaknesses. Flooded (paddy) rice ( Oryza sativa) can take up ions from the irrigation water by foliar uptake via the exposed stem base. We hypothesised that the stem base uptake of radiocaesium (RCs) is a pathway for rice grown in RCs-contaminated environments., We developed a bi-compartmental device which discriminates the stem base from root RCs uptake from solutions, thereby using RCs isotopes (137Cs and 134Cs) with < 2% solution leak between the compartments., Radiocaesium uptake was linear over time (0-24 h). Radiocaesium uptake to the entire plant, expressed per dry weight of the exposed parts, was sixfold higher for the roots than for the exposed stem base. At equal RCs concentrations in both compartments, the exposed stem base and root uptake contributed almost equally to the total shoot RCs concentrations. Reducing potassium supply to the roots not only increased the root RCs uptake but also increased RCs uptake by the stem base., This study was the first to experimentally demonstrate active and internally regulated RCs uptake by the stem base of rice. This study aims to give an overview over different reaction-to-fire prediction models developed over the last decades by finding similarities and differences between models, as well as identifying their robustness in scaling. The models have been divided into four categories - empirical, thermal, polynomial and comprehensive - depending on how pyrolysis is modelled. Empirical models extrapolate bench-scale test results to larger scales. These models are pertinent to applications that they have been validated for, but surfacic parameters used may not be scalable. In thermal models, pyrolysis is represented by heat transfer rates. The models are feasible for materials with high activation energies and where little pyrolysis occur before ignition. Polynomial models are empirical models that also take the environment into account. The validity of scaling is yet to be established. The comprehensive methodology includes chemical kinetics in the condensed phase. It has the potential to be used for any application; however, many parameters are needed. This increases the degrees of freedom versus data available for the description of the problem. Consequently, possible errors are introduced, and uncertainty is increased. Aerogels of nanocellulose (NC) prepared by freeze-drying of cellulose nanofibrils (CNF) hydrogels and followed by impregnation with methylaluminoxane serve as nanoporous organic supports for immobilizing single site iron catalysts such as bisiminopyridine iron(II) complexes. The resulting catalyst systems, exploiting renewable biomaterials as organic supports, are highly active in low pressure ethylene polymerization. They afford simultaneous control of high density polyethylene (HDPE) particle morphology and facile NC dispersion within the HDPE matrix. In the early stage of ethylene polymerization, mesoscopic shape replication and NC-mediated templating yield platelets containing an NC core and a HDPE shell, as confirmed by scanning electron microscopy (SEM) of virgin polyethylene powders. Opposite to conventionally dried CNF hydrogels, forming large agglomerates, this facile NC aerogel-mediated in situ NC/HDPE nanocomposite formation is vastly superior to melt compounding of HDPE with NC, failing to produce such fine NC dispersions. On increasing NC content to 3.0 wt%, both Young's modulus (+50%) and tensile strength (+40%) increase at the expense of elongation at break (−80%). According to the SEM analysis of NC/HDPE morphology, the dispersion of NC nanosheets together with the in situ formation of 'shish-kebab' polyethylene fiber-like structures accounted for HDPE matrix reinforcement. Fire hazards analyses and probabilistic fire safety analyses have demonstrated that fire can be an important contributor to core damage frequency and other major plant damage states of nuclear power plants.[1] In this issue, fire modelling has been undertaken by a diverse set of contributors, including, nuclear licensees, technical safety organizations, and, in some countries, regulators, to assess the consequences of fires. A number of members of the Organization for Economic Co-operation and Development (OECD) Nuclear Energy Agency (NEA) expressed their interest in participating in a joint international research project on the topic of fire events to be carried out under the auspices of the NEA. Background and Purpose: Diaphragm muscle weakness occurs in patients with heart failure (HF) and is associated with exercise intolerance and increased mortality. Reduced sensitivity of diaphragm fibres to calcium contributes to diaphragm weakness in HF. Here we have investigated the ability of the calcium sensitizer levosimendan to restore the reduced calcium sensitivity of diaphragm fibres from rats with HF.Experimental Approach: Coronary artery ligation in rats was used as an animal model for HF. Sham-operated rats served as controls. Fifteen weeks after induction of HF or sham operations animals were killed and muscle fibres were isolated from the diaphragm. Diaphragm fibres were skinned and activated with solutions containing incremental calcium concentrations and 10 µM levosimendan or vehicle (0.02% DMSO). Developed force was measured at each calcium concentration, and force-calcium concentration relationships were plotted.Key Results: Calcium sensitivity of force generation was reduced in diaphragm muscle fibres from HF rats, compared with fibres from control rats (P < 0.01). Maximal force generation was ∼25% lower in HF diaphragm fibres than in control fibres (P < 0.05). Levosimendan significantly increased calcium sensitivity of force generation in diaphragm fibres from HF and control rats, without affecting maximal force generation.Conclusions and Implications: Levosimendan enhanced the force generating capacity of diaphragm fibres from HF rats by increasing the sensitivity of force generation to calcium concentration. In the province-wide colorectal cancer (CRC) screening program in Ontario, Canada, individuals with a family history of CRC are offered colonoscopy screening and those without are offered guaiac fecal occult blood testing (gFOBT, Hemoccult II). We used microsimulation modeling to estimate the cumulative number of CRC deaths prevented and colonoscopies performed between 2008 and 2038 with this family history-based screening program, compared to a regular gFOBT program. In both programs, we assumed screening uptake increased from 30% (participation level in 2008 before the program was launched) to 60%. We assumed that 11% of the population had a family history, defined as having at least one first-degree relative diagnosed with CRC. The programs offered screening between age 50 and 74 years, every two years for gFOBT, and every ten years for colonoscopy. Compared to opportunistic screening (2008 participation level kept constant at 30%), the gFOBT program cumulatively prevented 6,700 more CRC deaths and required 570,000 additional colonoscopies by 2038. The family history-based screening program increased these numbers to 9,300 and 1,100,000, a 40% and 93% increase, respectively. If biennial gFOBT was replaced with biennial fecal immunochemical test (FIT), annual Hemoccult Sensa or five-yearly sigmoidoscopy screening, both the added benefits and colonoscopies required would decrease. A biennial gFOBT screening program that identifies individuals with a family history of CRC and recommends them to undergo colonoscopy screening would prevent 40% (range in sensitivity analyses: 20-51%) additional deaths while requiring 93% (range: 43-116%) additional colonoscopies, compared to a regular gFOBT screening program.
