Introduction: Universal screening for preterm delivery by adding transvaginal ultrasound measurement of cervical length to routine second trimester ultrasound has been proposed. The aim is to estimate inter- and intraobserver agreement and reliability of second trimester transvaginal ultrasound measurements of cervical length performed by specially trained midwife sonographers.Material and Methods: This is a prospective reliability and agreement study performed in seven Swedish ultrasound centers. In total, 18 midwife sonographers specially trained to perform ultrasound measurements of cervical length and 286 women in the second trimester were included. In each center, two midwife sonographers measured cervical length a few minutes apart in the same woman, the number of women examined per examiner pair varying between 24 and 30 (LIVE study). Sixteen midwife sonographers measured cervical length twice ≥2 months apart on 93 video clips (CLIPS study). The main outcome measures were mean difference, limits of agreement, intraclass correlation coefficient, intra-individual standard deviation, repeatability, Cohen's kappa and Fleiss kappa.Results: The limits of agreement and intraclass correlation coefficient of the best examiner pair in the LIVE study were -4.06 to 4.72 mm and 0.91, and those of the poorest were -11.11 to 11.39 mm and 0.31. In the CLIPS study, median (range) intra-individual standard deviation was 2.14 mm (1.40-3.46), repeatability 5.93 mm (3.88-9.58), intraclass correlation coefficient 0.84 (0.66-0.94). Median (range) interobserver agreement for cervical length ≤25 mm in the CLIPS study was 94.6% (84.9%-98.9%) and Cohen's kappa 0.56 (0.12-0.92), median (range) intraobserver agreement was 95.2% (87.1%-98.9%) and Cohen's kappa 0.68 (0.27-0.93).Conclusions: Agreement and reliability of cervical length measurements differed substantially between examiner pairs and examiners. If cervical length measurements are used to guide management there is potential for both over- and under-treatment. Uniform training and rigorous supervision and quality control are advised. Purpose: There remain uncertainties due to inter‐ and intraobserver variability in soft‐tissue‐based patient positioning even with the use of image‐guided radiation therapy (IGRT). This study aimed to reveal observer uncertainties of soft‐tissue‐based patient positioning on cone‐beam computed tomography (CBCT) images for prostate cancer IGRT. Methods: Twenty‐six patients (7–8 fractions/patient, total number of 204 fractions) who underwent IGRT for prostate cancer were selected. Six radiation therapists retrospectively measured prostate cancer location errors (PCLEs) of soft‐tissue‐based patient positioning between planning CT (pCT) and pretreatment CBCT (pre‐CBCT) images after automatic bone‐based registration. Observer uncertainties were evaluated based on residual errors, which denoted the differences between soft‐tissue and reference positioning errors. Reference positioning errors were obtained as PCLEs of contour‐based patient positioning between pCT and pre‐CBCT images. Intraobserver variations were obtained from the difference between the first and second soft‐tissue‐based patient positioning repeated by the same observer for each fraction. Systematic and random errors of inter‐ and intraobserver variations were calculated in anterior–posterior (AP), superior–inferior (SI), and left–right (LR) directions. Finally, clinical target volume (CTV)‐to‐planning target volume (PTV) margins were obtained from systematic and random errors of inter‐ and intraobserver variations in AP, SI, and LR directions. Results: Interobserver variations in AP, SI, and LR directions were 0.9, 0.9, and 0.5 mm, respectively, for the systematic error, and 1.8, 2.2, and 1.1 mm, respectively, for random error. Intraobserver variations were <0.2 mm in all directions. CTV‐to‐PTV margins in AP, SI, and LR directions were 3.5, 3.8, and 2.1 mm, respectively. Conclusion: Intraobserver variability was sufficiently small and would be negligible. However, uncertainties due to interobserver variability for soft‐tissue‐based patient positioning using CBCT images should be considered in CTV‐to‐PTV margins. [ABSTRACT FROM AUTHOR]

The article presents a study which compares the inter and intraobserver variation of fetal biometric measurements by using the two-dimensional (2D) and three-dimensional (3D) ultrasound imaging. It says that optimal standard image plane is important for sonographers to make a precise biometric measurement. The study suggests that both 2D and 3D ultrasounds are reproducible and 3D ultrasound significantly reduces the variation for head circumference, abdominal circumference, and femur length.

The article investigates the reproducibility of fetal liver-length measurement in pregnant women with diabetes. Results reveal that measurement of fetal liver length in the diabetic pregnancy is reproducible. The intraobserver SD was 3.06 mm, the interobserver SD was 2.17 mm, the intraobserver correlation was 0.77, and the interobserver correlation was 0.84.

The purpose of this study was to assess the intraobserver variation of various echocardiographic measures in standardbred trotters. Serial echocardiographic examinations were carried out on eight standardbred mares by one ultrasonographer for 5 separate days. During each examination, five nonconsecutive cardiac cycles (frames) were recorded and an average obtained for each individual measure. Various echocardiographic measures were obtained by use of two-dimensional (2-D), M-mode, color flow Doppler and pulsed wave Doppler echocardiography. The total variation in the echocardiographic measurements was split into three levels: the variation between horses, the day-to-day variation within individual horses, and finally the variation within horse on the same day of examination (intercardiac cycle variation). The intraclass correlation coefficient (ICC) was calculated for each measure. The ICC represents the variability of the measurements because of differences between the horses. In general the 2-D, M-mode and color flow Doppler measures had higher ICC values (ICC from 0.63 to 0.95) than the pulsed wave Doppler measures (ICC from 0.24 to 0.46), and the former measures were more repeatable than the pulsed wave measures. Exceptions to that were left ventricular free wall in diastole, the pulmonary artery in systole and the left ventricular mass, which all had low repeatability (ICC from 0.22 to 0.49). The results were used to calculate the relative differences that must be detected to diagnose a statistically significant change between two measurements in an individual horse. Differences from 4.2% to 21.8% must be achieved to document significant changes between serial measurements. A general tendency is that the color flow and pulsed wave Doppler measures require a larger relative difference (11.4–21.8%) between the measures to point out statistically significant cardiac changes than the 2-D and M-mode measures (4.2–13.9%).
