The article presents a case-study of 60-year-old male patient who underwent electrophysiologic study followed by ablation for recurrent supraventricular tachycardia with manifest left posterior accessory pathway. During the ablation attempt using the retrograde approach, a significant discrepancy was observed between the anatomic location and the local electrogram obtained from the coronary sinus (CS) and the ablation catheter. CS angiography (following left main coronary artery injection with coronary catheter) revealed that the CS catheter was not in place.
