Aim: In this study, we investigated the effect of differences in periodontal status in the masticatory performance of dentate subjects with the same occlusal supporting area., Materials and Methods: The subjects of the analysis were classified into those of Eichner A1-3 (n = 1094) and Eichner B1-4 (n = 529). Subjects' periodontal status was evaluated on the basis of the Community Periodontal Index (CPI). The number of functional teeth and occlusal support were investigated, the latter on the basis of the Eichner Index. Furthermore, masticatory performance was investigated by means of test gummy jelly. For each group, periodontal status was classified in two different ways, either with/without moderate periodontitis (CPI Code ≤2/≥3) or with/without severe periodontitis (CPI Code ≤3/4), and masticatory performance was compared between the various groups., Results: In subjects who were Eichner A1 and B3, masticatory performance was significantly lower in subjects with moderate periodontitis compared with those without, and in subjects with severe periodontitis compared with those without., Conclusion: Periodontal disease affects masticatory performance not only if occlusion is established by natural dentition with no tooth loss but also if occlusal support has decreased., (© 2014 John Wiley & Sons A/S. Purpose: The aim of this cross-sectional study was to investigate the relationship between body fat and masticatory function., Materials and Methods: One hundred dentate and partially edentulous participants (33 male; mean age, 39.7 ± 16.6 years) were selected. Body fat was established through body mass index (BMI). Masticatory function was evaluated by quantifying occlusal pairs and determining masticatory efficiency and swallowing threshold with the sieving method. During the swallowing threshold test, chewing rate was registered. Masticatory ability was also evaluated with a 5-point Likert scale questionnaire. Data were analyzed with Spearman and chi-square tests, as well as binary logistic regression analysis for the presence of increased BMI (α= 0.05)., Results: Age (rho = 0.517), occlusal pairs (chi-square = 26.353), masticatory efficiency (chi-square = 30.935), masticatory ability (chi-square = 25.132; p < 0.001), and swallowing threshold (chi-square = 8.730; p < 0.005) were related to BMI. Age (odds ratio, OR = 1.048, 95% CI = 1.008 to 1.089) and lower masticatory efficiency (OR = 4.792, 95% CI = 1.419 to 16.183) were predictive of increased body fat (p < 0.05). Gender (chi-square = 0.402, p= 0.526) and chewing rate (rho =-0.158, p= 0.117) were not related to BMI., Conclusions: These results suggest that people with lower masticatory efficiency may be at risk for increased body fat., (© 2012 by the American College of Prosthodontists.)

Purpose: Tooth positional changes following posterior tooth loss may alter arch forms and occlusal planes. This may result in the development of occlusal interferences. The purpose of this study was to determine clinical parameters associated with the presence of retruded contact position (RCP) contacts and occlusal interferences associated with posterior tooth loss., Materials and Methods: Diagnostic casts of 100 patients with at least one unopposed posterior tooth and 100 control patients were scanned and analyzed to record clinical parameters described in the earlier publications in this series. Clinical examinations identified the presence of RCP contacts and occlusal interferences. Five generalized linear models were developed to investigate the parameters associated with presence of RCP contacts, protrusive interferences, working side interferences, and nonworking side interferences., Results: RCP contacts were associated with the degree of supraeruption of the unopposed tooth and the presence of other types of interferences. Protrusive interferences were associated with the presence and position of a tooth distal to the extraction site and the presence of other types of interference. Working side interferences were associated with tipping of the tooth mesial to the extraction site and the presence of other interferences. Nonworking side interferences were associated with the presence of other types of interference only., Conclusion: Unopposed posterior teeth are more likely to be involved in RCP contacts or interferences than their matched controls. Many teeth were involved in multiple interferences, and there appears to be a number of interrelationships between these. Initial RCP contacts have associations with the extent of supraeruption of the unopposed teeth. The presence and position of the teeth distal to extraction sites was significant when modeling protrusive interferences. Protrusive interferences are more prevalent where the site of tooth loss was bounded. Mesial tipping of the teeth distal to extraction sites reduced this effect. Working side interferences were associated with tipping of teeth mesial to the site of tooth loss. No associations between patient or tooth factors were found for nonworking side interferences, and the only association found for these interferences were with protrusive interferences at the sites.

Purpose: Much anecdotal evidence is available on tooth positional changes following loss of an adjacent tooth, but only a few studies are available. In Part 1 of this series, supraeruption was assessed and Generalized Linear Models were made to determine the clinical parameters associated with the supraeruptive process. The models demonstrated that clinical parameters were not only associated with the extent of supraeruption, but also with the type of eruption present. This investigation of tooth positional changes adjacent to sites of posterior tooth loss attempts to provide increased understanding of the magnitude, direction, and associated features that may be helpful in decision making and treatment planning in the clinical setting., Materials and Methods: One hundred patients with an unopposed posterior tooth, with 100 age, sex, and bone level matched controls, were drawn from patients undergoing routine restorative care at Leeds Dental Institute. Study models were scanned, and the extent of eruption, type of eruption of the unopposed tooth, the overbite, overjet, buccal occlusion, and degree of crowding in the dentition, tipping, rotation, and buccal movement of the teeth associated with the edentulous site were recorded. Generalized Linear Models were developed to examine associations between each tooth movement and patient or dental factors., Results: A statistical significance in the degree of tipping of teeth both mesial and distal to the extraction site was detected between the subject and control groups. There was also a significant difference in rotation of the tooth mesial to the site. Four Generalized Linear Models were produced of the types of non-vertical movements found in teeth associated with sites of tooth loss., Conclusions: Teeth adjacent to the site of tooth loss may undergo non-vertical movements. Teeth mesial to the extraction site had a tendency to tip distally. The degree of tipping was increased in upper teeth and in subjects with a cusp-to-cusp buccal occlusion. Rotation of teeth mesial to the extraction site was more prevalent in the lower arch. Tipping of the tooth distal to the extraction site could be extreme and was found to be more prevalent in subjects with a reduced (Code 1) overbite and in the lower arch. Rotation of teeth distal to the extraction site was greater in the upper arch and was also associated with a reduced (Code 1) overbite. It also had an association with rotation of the tooth mesial to the extraction site. Models of non-vertical movement are likely to be of limited value due to overdispersion, indicating a high degree of variability within the model.

Purpose: Masticatory performance of denture patients is an outcome of the intricate inter-relationship between patient characteristics and denture construction. This study aimed to identify both patient- and denture-related factors associated with masticatory performance in unilateral distal extension removable partial denture patients (RPDs)., Materials and Methods: Seventy-two patients (25 male and 47 female, mean age 63.4 +/- 6.7 years) with unilateral distal extension RPDs were included in the study. The Mixing Ability Index (MAI) obtained from a chewing test using standard two-colored wax cubes, determined masticatory performance. The effect of each evaluated factor on masticatory performance was first determined by an independent t-test or one-way analysis of variance. The statistically significant factors were entered in a model of multivariate linear regression analysis to estimate the independent effect on MAI., Results: The MAI ranged from -3.06 to 0.80 (Mean -0.71 +/- 0.86). Univariate analyses identified male gender, high maximum bite force, more functional tooth units, and natural dentition opposing RPDs as patient factors increasing the MAI. Multivariate analysis (adjusted R(2)= 0.346) revealed that gender (p < 0.001), maximum bite force (p= 0.02), and number of functional tooth units (p= 0.03) were independent predictors for masticatory performance. None of the denture-related factors had a significant influence on MAI., Conclusion: Gender, maximum bite force, and number of functional tooth units were the main factors influencing masticatory performance of unilateral distal extension RPD patients., (Copyright (c) 2006 by The American College of Prosthodontists.)

It is generally assumed that dentition has ceased to be a predominant selection factor in the process of human evolution. With differentiation and reintegration of the human species, gross differences in the morphology and physiology of the stomatognathic system have become apparent. Variability and adaptation of this system have proven that deviations from what may be considered ideal are not pathologic but may, in fact, prove to be a range of normality in this general pattern of evolution. The treatment of patients according to inflexible postulates may be detrimental to the individual patient and the concept of a variable and patient-specific optimal dentition should be considered, especially in the context of the shortened dental arch.

Objective: The purpose of this study was to evaluate trabeculae changes in partially edentulous bone in ovariectomized rabbits., Background: Numerous clinical studies have suggested that the greater risk for oral bone loss in females may be correlated with osteoporosis after menopause. Knowledge of trabecular changes in partially edentulous bone in animals with loss of ovarian function may be beneficial in the diagnosis and treatment of partially edentulous patients of postmenopausal women., Methods: Twelve adult female Japanese white rabbits were examined. The mandibular incisors were initially extracted to simulate the partially edentulous bone. Six animals were bilaterally ovariectomized and the other six sham-ovariectomized 12 weeks after tooth extraction. The partially edentulous parts of distal mandibular bodies were processed undecalcified 12 weeks after ovariectomized or sham-ovariectomized surgeries and examined by quantitative trabecular bone histomorphometry., Results: In ovariectomized rabbits, there were significant increases in trabecular separation, osteoid volume, osteoid thickness, osteoid width, eroded surface, and mineral apposition rate, and a significant decrease in trabecular number., Conclusion: The results of sparser trabecular structure, more trabecular osteoid, and increased trabecular bone turnover demonstrate mineralized bone loss in partially edentulous trabeculae of ovariectomized rabbit mandibles and suggest that the same loss may occur in postmenopausal women.

Sulcular temperatures were measured on 35 clinically stable and restored osseointegrated dental implants and compared to 50 remaining natural teeth in 9 partially-edentulous adults using the PerioTemp temperature probe (ABIO-DENT, Inc., Danvers, MA USA). Replicate peri-implant sulcular measurements showed a mean difference of 0.1 +/- 0.15 (SD) degrees C. Implant temperatures varied from 0.41 to 3.9 degrees C below sublingual temperatures measured on each patient, and maxillary implants were significantly cooler than mandibular implants (p < 0.001, t-test). A posterior-to-anterior temperature gradient was found in both the maxilla and mandible, with anterior implants significantly cooler than posterior fixtures. No significant differences were found in sulcular temperatures of osseointegrated dental implants and natural teeth located in anatomically-equivalent oral sites. Similarly to natural teeth, sulcular temperature may serve as a valuable diagnostic tool in evaluating dental implant status.

To test the hypothesis that localized and generalized juvenile periodontitis is one disease entity, the localization of bone loss was measured from orthopantomograms of 88 patients with untreated juvenile periodontitis. Linear regression analysis showed a significant correlation between the age of the patient and the number of teeth with bone loss. Similarly, a correlation was found between the Gingival Bleeding Index scores, the age of the patient, and the number of affected teeth. It is concluded that juvenile periodontitis, known to be genetically controlled, is a disease entity which starts as a localized form with affected first molars and/or incisors and develops, if not treated, gradually to the generalized form of the same disease, which resembles the adult periodontitis.

The purpose of this study was to record the rate of periodontal disease progression over a 10-year period with respect to individual subject, age and tooth type. 283 subjects, who had undergone a full-mouth radiographic examination in 1974-76 and at that time were 25-70 years old, were randomly selected from a larger patient sample for a new radiographic examination in 1985-86. 201 subjects (71%) agreed to participate. For each respondent, tooth loss over the 10-year period was calculated. The radiographic bone height at the mesial and distal aspect of all teeth was assessed by measuring the distance between the cemento-enamel junction and the bone crest. Difference in periodontal bone height between the 2 examinations was calculated for each tooth site. The results revealed that the mean number of teeth lost over the 10-year period was 3.8 (SD 4.6). Tooth mortality, expressed as a % of the no. of teeth present at the initial examination, increased with age and varied between 2.9%-28.5%. In all age groups, molars had the highest and canines the lowest rate of tooth mortality. The frequency of subjects with loss of greater than 10 teeth was highest for the initially 45-year old individuals (20%). 7 individuals (3%) had become edentulous. The mean annual reduction of alveolar bone height varied between 0.07-0.14 mm in the age groups 25-65 years. The initially 70-year old subjects showed a statistically significant higher annual rate of bone loss (0.28 mm).(ABSTRACT TRUNCATED AT 250 WORDS)

This paper describes the initiation, rate of progress of periodontal disease and consequent tooth loss in a population never exposed to any programs or incidents relative to prevention and treatment of dental diseases. The group consisted of 480 male laborers at two tea plantations in Sri Lanka. The study design and baseline data have been published. At the initial examination in 1970, the age of the participants ranged between 14 and 31 years. Subsequent examinations occurred in 1971, 1973, 1977, 1982 and 1985. Thus, the study covers the age range 14-46 years. Throughout the study, the clinical indices were scored by the same two examiners, both well-trained and experienced periodontitis. Intra-examiner reproducibility for each index was tested at baseline and repeated periodically during the study. The data for each examination were computerized and updated on an ongoing basis. At the last examination in 1985, there were 161 individuals who had participated in the first survey. This population did not perform any conventional oral hygiene measures and consequently displayed quite uniformly large aggregates of plaque, calculus and stain on their teeth. Virtually all gingival units exhibited inflammation. Based on interproximal loss of attachment and tooth mortality rates, three subpopulations were identified: (1) individuals (approximately 8%) with rapid progression of periodontal disease (RP), those (approximately 81%) with moderate progression (MP), and a group (approximately 11%) who exhibited no progression (NP) of periodontal disease beyond gingivitis. At 35 years of age, the mean loss of attachment in the RP group was approximately 9 mm, the MP group had approximately 4 mm and the NP group had less than 1 mm loss of attachment. At the age of 45 years, the mean loss of attachment in the RP group was approximately 13 mm and the MP group approximately 7 mm. The annual rate of destruction in the RP group varied between 0.1 and 1.0 mm, in the MP group between 0.05 and 0.5 mm, and in the NP group between 0.05 and 0.09 mm. Since this population was virtually caries free, essentially all missing teeth were lost due to periodontal disease. In the RP group, tooth loss already occurred at 20 years of age and increased throughout the next 25 years. At 35 years of age, 12 teeth had been lost, at 40 years of age 20 teeth were missing and at 45 all teeth were lost. In the MP groups, tooth mortality started after 30 years of age and increased throughout the decade.(ABSTRACT TRUNCATED AT 400 WORDS)
