Abstract: Satellite‐based remote sensing has generally necessitated a trade‐off between spatial resolution and temporal frequency, affecting the capacity to observe fast hydrological processes and rapidly changing land surface conditions. An avenue for overcoming these spatiotemporal restrictions is the concept of using constellations of satellites, as opposed to the mission focus exemplified by the more conventional space‐agency approach to earth observation. Referred to as CubeSats, these platforms offer the potential to provide new insights into a range of earth system variables and processes. Their emergence heralds a paradigm shift from single‐sensor launches to an operational approach that envisions tens to hundreds of small, lightweight, and comparatively inexpensive satellites placed into a range of low earth orbits. Although current systems are largely limited to sensing in the optical portion of the electromagnetic spectrum, we demonstrate the opportunity and potential that CubeSats present the hydrological community via the retrieval of vegetation dynamics and terrestrial evaporation and foreshadow future sensing capabilities. In contrast to Porphyria cutanea tarda (PCT), which is characterized by constant hepatic involvement, the liver seems to be unaltered in Porphyria variegata (PV). However, the authors have found microscopic alterations in the liver of 2 patients with PV (mild inflammation and fibrosis of the portal tracts and the presence of iron deposits in one of the cases). Ulceration is an indicator of unfavourable prognosis in malignant melanoma (MM). But why do melanomas ulcerate?. Possible causes of ulceration were investigated in a group of 69 ulcerated and 69 non-ulcerated malignant melanomas. A significant correlation(P < 0,001) between ulceration and mitotic index was found. In a group of 69 ulcerated malignant melanomas, 49 had over 7, 15 had 4-6, and only 5 had 0-3 mitotic figures per 10 high-power fields. Among 69 non-ulcerated melanomas, 28 had more than 7, 24 had 4 6, and 17 had 0-3 mitotic figures per 10 high-power fields. This study supports the hypothesis that ulceration of melanomas is usually the result of the destruction of the epidermis by the proliferating neoplastic cells or modification of blood supply due to the expansile activity of the tumor. Cocoamidopropylbetaines are surfactants frequently used on cosmetics. We have evaluated the irritant capacity of 3 different qualitities of cocoamidopropylbetaine, using the following method: patch tests have been carried out with 5 different dilutions in 67 patients and the results read at 2 days by noninvasive methods (direct visualization, transepidermal water loss (TFWL), laser Doppler flowmetry (LDF)). The results with the 3 methods were concordant. However, the results with the different concentrations were paradoxical, as irritancy did not increase at higher concentrations.We have tried to explain this by the fact that these substances contain by-products (free amidoamine and sodium monocholoroacetate), the concentrations tested all being above the critical micelle concentration and therefore containing both micelles and monomer. Finally, we believe that noninvasive methods such as TEWL and LDF could be of great use in the evaluation of irritant contact dermatitis. [ABSTRACT FROM AUTHOR]

4 patients with chronic photocontact dermatitis were sensitive to cobalt salts. They presented as cases of contact dermatitis from cement or pig fodder with persistent lesions on exposed areas. Only 2 of them had standard patch test positive reactions to chromate and cobalt, but all showed positive photo-patch tests to cobalt. Photo-oxidation tests proved that cobalt salts are photosensitizing. Exposure to cobalt salts was responsible for the patients' chronic actinic dermatitis. [ABSTRACT FROM AUTHOR]

Systemic candidiasis in intravenous drug abusers (IVDA) is a new syndrome caused by Candida albicans and characterized by the sequential development of skin, eye, and osteoarticular lesions, which together form a typical clinical picture. We studied 30 patients with suggestive skin lesions: papules, nodules, and pustules in hair-bearing areas, particularly the scalp and beard area, associated with hair invasion by candidal hyphae. Ocular and osteoarticular involvement and presence of candidemia in some of the patients suggested blood borne colonization of C. albicans. The infection has been related to ‘brown’ heroin. The origin of C. albicans and the reasons for its exclusive localization in the skin and these organs are discussed. The characteristic clinical picture is widely different from that of classic disseminated candidiasis in immunodeficient patients. Therapy is also discussed. [ABSTRACT FROM AUTHOR]

In our Department. the incidence of sensitization to cobalt has more than doubled in the last 5 years. 28% of cobalt-sensitive patients showed no other sensitivity. Of isolated cobalt sensitive patients, 68% were housewives. We determined by atomic absorption spectrophotometry with a graphite camera, the cobalt and nickel content of the 30 most popular Spanish domestic detergents and cleaners sold in 1985. 78.6% of them contain considerable amounts of cobalt. Solitary congenital nodular calcification of the skin is a form of cutaneous calcinosis. it appears as a small, firm nodule on the head and extremities of young infants. The article presents a case study related to myeloma-associated systemic amyloidosis which involves the tongue, heart, gastrointestinal tract, skeletal and smooth muscles, and nerves and skin, whereas in secondary systemic amyloidosis the liver, spleen, kidneys, and adrenal glands are predominantly involved. Amyloidosis develops in only 15% of patients with multiple myeloma. In the case study presented, the purpuric cutaneous lesions and macroglosia were the revealing signs of plasma cell dyscrasia. Macroglosia is present in 12% to 40% of cases and is observed as difuse enlargement of the tongue along with firm consistency and teeth indentation.

Although there are many publications on cutaneous manifestations in AIDS, including adverse drug reactions, and seborrhoeic dermatitis, tinea faciale and atopic dermatitis (22) are all well described in HIV-positive patients, little has been published on the relationship between AIDS and contact dermatitis. A 47-year-old HIV-positive homosexual male, presented 1 year ago with intense outbreaks of seborrhea dermatitis, which improved with topical corticosteroids and ketoconazole but recurred on increasing medication.

The article focuses on a double-blind study of prednicarbate versus fluocortin butyl ester in atopic dermatitis. In this study parallel-group, double-blind trial compared the efficacy and tolerability of topical rednicarbate, ointment, with that of fluocortin butyl ester, ointment. A thin layer of topical prednicarbate or fluocortin butyl ester was applied to the affected area of skin twice daily, without occlusion. Results of the 36 patients receiving prednicarbate and 31 of those receiving fluocortin butyl ester were included in the statistical analysis.

Background: Bullous pemphigoid (BP) is the most common autoimmune subepidermal blistering disease of the skin and mucous membranes. This disease typically affects the elderly and presents with itch and localized or, most frequently, generalized bullous lesions. A subset of patients only develops excoriations, prurigo-like lesions, and eczematous and/or urticarial erythematous lesions. The disease, which is significantly associated with neurological disorders, has high morbidity and severely impacts the quality of life., Objectives and Methodology: The Autoimmune blistering diseases Task Force of the European Academy of Dermatology and Venereology sought to update the guidelines for the management of BP based on new clinical information, and new evidence on diagnostic tools and interventions. The recommendations are either evidence-based or rely on expert opinion. The degree of consent among all task force members was included., Results: Treatment depends on the severity of BP and patients' comorbidities. High-potency topical corticosteroids are recommended as the mainstay of treatment whenever possible. Oral prednisone at a dose of 0.5 mg/kg/day is a recommended alternative. In case of contraindications or resistance to corticosteroids, immunosuppressive therapies, such as methotrexate, azathioprine, mycophenolate mofetil or mycophenolate acid, may be recommended. The use of doxycycline and dapsone is controversial. They may be recommended, in particular, in patients with contraindications to oral corticosteroids. B-cell-depleting therapy and intravenous immunoglobulins may be considered in treatment-resistant cases. Omalizumab and dupilumab have recently shown promising results. Background: The BIOCHIP (Dermatology Mosaic 7; EUROIMMUN, Lubeck, Germany) is a novel multiplex indirect immunofluorescence (IIF) technique used in the serological diagnosis of bullous pemphigoid (BP) and pemphigus., Objective: To validate the accuracy and inter-rater reliability (IRR) of the BIOCHIP in the diagnosis of BP, pemphigus foliaceus (PF) and pemphigus vulgaris (PV)., Methods: Sera from patients with BP (n = 38), PF (n = 8), PV (n = 23), control patients (n = 64) and healthy control volunteers (n = 39) were tested. Sera were collected and analysed during the course of the disease at 1-5 different time points. The BIOCHIP was performed for all patients, digital images were captured of each incubated field, and the images were shared with 10 dermatologists experienced in reading IF from around the world to report. There were 312 BIOCHIP slides consisting of 1872 photos in total. All patients were de-identified. Fleiss Kappa was used to estimate the IRR., Results: Fleiss Kappa was computed for each category (Oesophagus, Oesophagus immunofluorescence pattern, Salt-Split Skin (SSS), SSS immunofluorescence location, BP180, BP230, Dsg 1 and Ds3). The inter-rater agreement between the 10 raters varied between fair and moderate for all categories. Those that demonstrated fair concordance included monkey oesophagus (k = 0.257, P < 0.0001), oesophagus pattern (k = 0.357, P < 0.0001), Dsg1 (k = 0.390, P < 0.0001) and BP230 (k = 0.281, P < 0.0001). Moderate agreement was demonstrated for SSS (k = 0.416, P < 0.0001), SSS immunofluorescence location (k = 0.505, P < 0.0001), Dsg3 (k = 0.437, P < 0.0001) and BP180 (k = 0.559, P < 0.0001)., Conclusion: The BIOCHIP mosaic-based immunofluorescence test is a simple, time and effort saving test that can aid in the diagnosis and screening of BP, PV and PF. However, the level of agreement was relatively low. First described in 1961, photoonycholysis (PO) is a rare nail alteration that may result from drug intake, from topical aminolevulinate photodynamic therapy or from photosensitive conditions such as porphyria or pseudoporphyria. Spontaneous PO is rare. This review updates the numerous causes of PO and highlights some new ways producing this condition. Four different types of PO are clearly recognized without relationship with the responsible drug. An updated list of potential inducing drug is provided. Some practical points on PO have been raised. The inability to reproduce photoonycholysis experimentally should be emphasized, and the pathogenesis of PO still needs to be clarified., (© 2018 European Academy of Dermatology and Venereology.)

The objective of this study was to establish normal reference ranges for porphyrins in healthy neonates. There is little information about urinary porphyrin excretion in this age group. This knowledge may provide an early diagnostic tool for detecting subtle alterations or latent forms in disorders of heme biosynthesis. Fifty healthy neonates were selected from the Department of Obstetrics. Total urinary porphyrins in random specimens were analysed by a spectrofluorometry method. The measurement of porphyrin fractions was made by fluorometric high-performance liquid chromatography (HPLC). The results were adjusted to urinary creatinine excretion to correct any imprecision and interindividual variation in body mass. The urinary total porphyrin had a median value of 331.50 (nmol/L). A statistically significant relationship between total porphyrin (nmol/L) and creatinine (mmol/L) was found (p < 0.01). The porphyrin/creatinine ratio showed a median value of 56.30 nmol/mmol creatinine. The study of individual porphyrins revealed that coproporphyrin and uroporphyrin were the major porphyrins excreted in neonates (coproporphyrin represents 81.98% and uroporphyrin 16.64% of total porphyrin); in both cases, isomer I was predominant with median values of 22.36 and 6.25 nmol/mmol creatinine, respectively. No significant relationships were found between porphyrin excretion and sex, gestational age, weight, or height. Our data provide the reference limits for porphyrins in neonates as a diagnostic guideline for evaluation of subtle alterations in heme biosynthesis.

The porphyrias are uncommon diseases caused by enzymatic deficiencies in the heme pathway. In the 25 year period 1969-1994, the Department of Dermatology of the Hospital Clinic of Barcelona has been able to study 793 cases of porphyria (724 cases of PCT, 27 of EPP, 26 of PV, 5 of CEP, 5 of HEP, 5 of AIP, 1 of HCP). Homozygous expression of an enzymatic deficiency in the heme pathway produces severe disease. Commonly, clinical manifestations appear in the homozygous state (the autosomal recessive porphyrias). However, homozygous forms of autosomal dominant porphyrias may occur exceptionally. Moreover, there are cutaneous porphyrias whose clinical manifestations do not permit dermatologists to classify them clearly into one of the well-defined syndromes. These uncommon and atypical forms are difficult to recognize without biochemical and enzyme studies. The porphyrias have a wide clinico-biochemical spectrum, including a large proportion of well defined diseases. Nevertheless, atypical forms occur and may be difficult to evaluate. It is important to note the genetic heterogeneicity of porphyrias, which accounts for the varying phenotypic expression.
