Background: The influence of dental implant length and diameter on primary stability in various bone densities is not well understood. Aim: To in vitro study the effect of length and diameter on resonance frequency analysis (RFA), insertion torque (IT) and displacement (DP) measurements of dental implants in different implant site densities. Materials and methods: Dental implants of four different diameters (Ø 3.5, 4.0, 4.5 and 5.0 mm) and three different lengths (7, 11 and 15 mm) (Neoss Ltd, Harrogate, UK) were placed in polyurethane blocks of three different densities (Sawbones Europe AB, Malmö, Sweden). The primary stability was assessed by RFA (ISQ) (Osstell, Osstell AB, Gothenburg, Sweden) and insertion torque measurements (ITmax in N cm) (iChiropo™, Bien‐Air Dental SA, Bienne, Switzerland). In addition, the blocks were mounted in a rig and a lateral force of 25 N cm was applied to the implants and the DP was measured in μm with a micrometer gauge placed on the opposite side of the load transducer. Statistical analyses using linear and quadratic models were applied. Results: Implant length, diameter and block density were found to be significant independent predictors of RFA, ITmax, and DP measurements. Implant length had a strong effect, while the effect of diameter in general was subtle, particularly in the softest block. Background. Atrophy of the posterior maxilla as a consequence of tooth loss and sinus pneumatization is a frequent condition encountered in the clinical practice. Prosthetic rehabilitation with implants in these patients often requires some kind of bone regeneration procedure to increase the bone volume. Aim. The aim of the present retrospective study is to analyze the survival and success rates of a series of implants placed in the atrophic posterior maxilla with a transcrestal osteotome procedure, without placing a bone grafting material. Materials and Methods. From 2006 to 2014, 36 dental implants (Neoss Ltd., Harrogate, UK) were inserted in 36 patients with at least 4 mm of bone below the maxillary sinus using transcrestal osteotome sinus floor elevation and placement of collagen sponge below the sinus membrane. ISQ measurements were made after implant placement and at abutment surgery after 4 to 6 months. The vertical bone height (VBH) was evaluated in intraoral radiographs taken prior to surgery and in radiographs from annual check-up appointments 5 to 13 years after implant placement. In addition, marginal bone loss (MBL) was evaluated. Results. One implant was lost after four years of prosthetic loading. The remaining 35 implants showed no complications and were loaded with single crowns after 4–6 months of healing. All 35 implants showed clinical success after 8.5 ± 2.8 years of prosthetic loading (from 5 to 13 years). The vertical bone height was 5.9 ± 1.4 mm at surgery, 9.7 ± 1.1 mm at second surgery after 4–6 months, and 8.3 ± 1.8 at the follow-up at 8.5 ± 2.8 years (from 5 to 13 years). The implant stability registered was 73.2 ± 6.2 ISQ at the surgery and 75.8 ± 3.9 at the second surgery after 4–6 months. Conclusions. The present long-term follow-up study showed that the crestal approach for sinus floor bone augmentation without additional bone grafting results in predicable bone formation and high implant survival. Background: Ongoing marginal bone loss is a threat to the longevity of implant‐supported prostheses. Aim: The aim of the present study was to retrospectively evaluate the survival rate and factors affecting marginal bone levels at a hydrophilic implant design after 5 years in function. Materials and Methods: The study group consisted of 51 consecutive patients previously treated with 159 hydrophilic implants (Neoss Straight Proactive implants) and scheduled for annual check‐ups with clinical and radiographic examinations during 5 years. Data were compiled for the entire study population as well as for two subgroups: one where guided bone regeneration (GBR) was performed (91 implants) and the other where no GBR procedures (68 implants) were performed. Marginal bone levels were measured from peri‐apical radiographs taken at placement and annual follow‐ups. Statistical analyses were applied to evaluate the effect of different factors on marginal bone remodeling. Results: Two implant failures, one from each subgroup, occurred during the first year of function resulting in an overall cumulative survival rate (CSR) of 98.7% after 5 years of loading. The mean marginal bone loss amounted to 0.7 ± 0.7 mm after 1 year and 0.8 ± 0.6 mm after 5 years. No implants showed more than 3 mm bone loss after 5 years. Age, gender, implant position, biotype, implant diameter, implant length, indication, surgical/loading protocol, and ISQ at prosthesis delivery were found to affect bone remodeling. No significant differences or correlations were seen for smoking, jaw, bone quantity, bone quality, GBR, sinus lift, and ISQ at implant placement. Conclusions: The present implant design performed well with few failures and minimal marginal bone loss after 5 years of loading. Background: Marginal bone resorption has by some been identified as a "disease" whereas in reality it generally represents a condition. Purpose: The present article is a comparison between oral and orthopedic implants, as previously preferred comparisons between oral implants and teeth seem meaningless. Materials and Methods: The article is a narrative review on reasons for marginal bone loss. Results and Conclusions: The pathology of an oral implant is as little related to a tooth as is pathology of a hip arthroplasty to a normally functioning, pristine hip joint. Oral as well as orthopedic implants are recognized as foreign bodies by the immune system and bone is formed, either in contact or distance osteogenesis, to shield off the foreign materials from remaining tissues. A mild immune reaction coupled to a chronic state of inflammation around the implant serve to protect implants from bacterial attacks. Having said this, an overreaction of the immune system may lead to clinical problems. Marginal bone loss around oral and orthopedic implants is generally not dependent on disease, but represents an immunologically driven rejection mechanism that, if continuous, will threaten implant survival. The immune system may be activated by various combined patient and clinical factors or, if rarely, by microbes. However, the great majority of cases with marginal bone loss represents a temporary immune overreaction only and will not lead to implant failure due to various defense mechanisms. Background. Diagnostic instruments based on resonance frequency analysis (RFA) can be utilised to assess dental implant stability during treatment and follow-up. Aim. The aim of the present study was to investigate the influence of patient- and implant-related factors on implant stability and the 5-year implant survival. In addition, the influence of stability (ISQ value) at placement and abutment connection on implant survival was evaluated. Materials and Methods. RFA measurements from a total of 334 consecutive patients with 745 dental implants (Neoss Ltd., Harrogate, UK) were retrospectively analysed after at least 5 years in function. Statistics were used to evaluate the influence of the different variables on implant stability and implant survival. Odds ratio calculations were performed to compare the risk for implant failure using 60, 65, 70, and 75 ISQ as threshold levels at placement and loading. Results. A total of 20 implant failures in 14 patients were noted during the 5 years of follow-up, giving an overall cumulative survival rate (CSR) of 97.3% at the implant level and 95.8% at the patient level. Gender, jaw, position, bone quality, and implant diameter had an influence on implant stability at placement. Jaw, bone quality, and implant diameter had an influence on stability after 3-4 months of healing. More failures were observed in full than in partial rehabilitations. Age, gender, jaw, position, bone quantity, bone quality, implant diameter, and implant length had no influence on implant survival. Implants with ISQ values below the threshold levels showed lower survival rates compared to implants with values above these levels. Conclusions. The present study showed a significantly higher risk for implant failure, showing an ISQ value below 70 and 75 at placement or after 3-4 months of healing. In the 1960s and 1970s, implant-supported prostheses based on subperiosteal or blade implants had a poor reputation because of questionable clinical outcomes and lack of scientific documentation. The change to a scientifically sound discipline was initiated by the two scientific pioneers of modern implant dentistry, Professor P. I. Brånemark from the University of Gothenburg in Sweden and Professor André Schroeder from the University of Bern in Switzerland. Together with their teams, and independently of each other, they laid the foundation for the most significant development and paradigm shift in dental medicine. The present volume of Periodontology 2000 celebrates 50 years of osseointegration. It reviews the progress of implant therapy over the past 50 years, including the basics of implant surgery required to achieve osseointegration on a predictable basis and evolving innovations. The development of bone-augmentation techniques, such as guided bone regeneration and sinus floor elevation, to correct local bone defects at potential implant sites has increased the indications for implant therapy. The paradigm shift to moderately rough implant surfaces resulted in faster and enhanced bone integration and led to improvements in various treatment protocols, such as immediate and early implant placement in postextraction sites, and made various loading protocols possible, including immediate and early implant loading. In the past 15 years, preoperative analysis and presurgical planning improved as a result of the introduction of three-dimensional imaging techniques. Hereby, cone-beam computed tomography offers better image quality with reduced radiation exposure, when compared with dental computed tomography. This opened the door for digital planning and surgical modifications. Over the last 50 years this evolution has facilitated tremendous progress in esthetic outcomes with implant-supported prostheses and improved patient-centered outcomes. This volume of Periodontology 2000 also discusses the current trends and open questions of implant dentistry, such as the potential of digital implant dentistry in the surgical and prosthetic field, the trend for an increasing average age of implant patients and the related adaptations of treatment protocols, and the second attempt to establish ceramic implants using, this time, zirconia as the implant material. Finally, some of the hottest controversies are discussed, such as recent suggestions on bone integration being a potential foreign-body reaction and the evidence-based appraisal of the peri-implantitis debate. The prevalence, causes and consequences of crestal bone loss at dental implants are a matter of debate. In recent years, a high prevalence of peri-implant soft-tissue inflammation, associated with peri-implant bone loss, has been reported and the need for treatments similar to those offered for natural teeth affected by periodontitis has been proposed. This suggestion is based on the assumption that periodontal indices, such as probing pocket depth and bleeding on probing, are reliable indicators of the peri-implant tissue conditions and good predictors of future bone loss. However, based on a critical review of the literature in the present paper, it is concluded that periodontal indices are not reliable either for identifying peri-implant disease or for predicting future risk for peri-implant crestal bone loss and implant failure. The long-term experiences with dental implants, presented in the literature, indicate that the presence of bleeding on probing, probing pocket depths much larger than 4 mm and some bone loss seem to reflect, in most instances, normal conditions of well-functioning dental implants, bearing in mind that healing of dental implants is the result of a foreign body reaction with the formation of scar tissue. Therefore, the use of probing pocket depth and bleeding on probing assessments may lead to over-diagnosis and possibly to over-treatment of assumed biofilm-mediated peri-implantitis lesions. It is the opinion of the authors of this review that a treatment should only be initiated when a clinical problem is present based on patient's symptoms (discomfort, pain), the presence of swelling, redness and pus, and significant crestal bone loss over time (as verified with radiographs). Implant treatment in an atrophied edentulous posterior maxilla constitutes a challenge for the therapeutic team. The authors of the present study acknowledge that modern micro-rough surface implants in lengths of about 8-10 mm or longer and of different brands are similarly successful. Consequently, the authors propose that the use of different sinus floor elevation techniques should be considered when < 8 mm of bone is available below the maxillary sinus. The type of sinus floor elevation technique selected is mainly based on residual vertical bone height, marginal bone width, local intrasinus anatomy and the number of teeth to be replaced, although other factors (such as surgical training and surgical experience) may have an impact. It is proposed that a transcrestal sinus floor elevation approach can be considered as a first-choice method for single tooth gaps in situations with sufficient width for implant placement and a residual bone height of 5-8 mm, while lateral sinus floor elevation, with or without grafting materials, is indicated when < 5 mm of bone is available and when several teeth are to be replaced. Successful osseointegration is the result of a controlled foreign body reaction to dental implants. Osseointegrated implants have demonstrated excellent long-term survivability, although they may be subject to limited marginal bone loss. Marginal bone loss during the first few years after implant placement seldom represents disease, but is instead the result of an adaptive bone response to surgical trauma and implant loading. It is not uncommon for implants with early marginal bone loss to enter a long-lasting state of bone stability. Extensive bone resorption after the first year is generally due to an exacerbation of adverse body reactions caused by non-optimal implant components, adverse surgery or prosthodontics and/or compromised patient factors. Disease in the form of peri-implantitis is a late complication that affects some implants with suppuration and rapid loss of crestal bone, and is probably caused by bacterial pathogens and immunological reactions. Unfortunately, the literature is not consistent with respect to the type or magnitude of clinical implant problems, including how they are defined and diagnosed. If the peri-implantitis diagnosis is confined to cases with infection, suppuration and significant bone loss, the frequency of the disease is relatively low, which is in sharp contrast to the frequencies reported with unrealistic definitions of peri-implantitis. We suggest that when modern implants are placed by properly trained individuals, only 1-2% of implants show true peri-implantitis during follow-up periods of 10 years or more. Purpose The aim of the present 1-year clinical and radiographic study was to evaluate a hydrophilic dental implant when used in everyday cases in one clinic. The purpose was also to compare augmented with nonaugmented sites. Materials and Methods The study group consisted of 50 consecutive patients treated with 159 dental implants ( Proactive, Neoss Ltd, Harrogate, UK) in both mandibles and maxillae. Ninety-two implants were placed with adjunct bone augmentation procedures, whereas 40 implants were placed in augmented maxillary sinus sites. A two-stage procedure was used for 84 implants and a one-stage procedure for 47 implants. Twenty-eight implants were immediately loaded. Implant stability was measured with resonance frequency analysis ( RFA) at placement and at prosthesis delivery. The patients were scheduled for checkups after 6 and 12 months in function with clinical and radiographic examinations. Marginal bone level measurements were performed in baseline and follow-up intraoral radiographs. Results Two implant failures in two patients were experienced after 1 year of loading, giving a survival rate of 98.7%. One implant in nonaugmented sites and one implant in conjunction with an augmentation procedure (sinus lift), giving a survival rate of 98.5% and 98.9% for healed and augmented sites, respectively ( NS). RFA measurements showed 70.2 ± 9.5 ISQ at placement and 76.5 ± 5.9 ISQ ( p ≤ 0.001) after a mean healing time of 5.6 ± 1.6 months. The marginal bone loss amounted to 0.7 ± 0.7 mm after 1 year of loading. Frequency distribution showed bone gain or less than 1 mm bone loss for the majority of implants (69.3%), whereas 25.7% showed 1.1 to 2 mm, 5.0% more than 2 mm and no implants more than 3 mm of bone loss. Conclusions The use of Neoss Proactive implants for prosthetic rehabilitation of consecutive patients resulted in predictable clinical and radiographic outcomes after 1 year of loading with no differences between augmented and nonaugmented sites. Background. Hydrophilic and moderately rough implant surfaces have been proposed to enhance the osseointegration response. Aim. The aim of this study was to compare early changes of stability for two implants with identical macrodesign but with different surface topographies. Materials and Methods. In 11 patients, a total of 22 implants (11 bimodal (minimally rough, control) and 11 proactive (moderately rough and hydrophilic, test), Neoss Ltd., Harrogate, UK) were immediately placed into fresh extraction sockets and immediately loaded. The peak insertion torque (IT) was measured in Ncm at placement. Resonance Frequency Analysis (RFA) measurements were made at baseline and 2, 4, 6, and 12 weeks after surgery. Results. The two implant types showed similar IT and RFA values at placement (NS). A dip of RFA values after 2 weeks followed by an increase was observed, where the test implant showed a less pronounced decrease and a more rapid recovery than the control implant. The test implants were significantly more stable than the control ones after 12 weeks. Conclusions. The results from the present study indicated that the hydrophilic and rougher test implant was more resistant to immediate loading and showed a significantly higher stability than the smoother control implant after 12 weeks. Background Cell interactions, adherence, and osseointegration at the bone-implant interface can be directly influenced by the surface properties of the titanium implant. Purpose To characterize osseointegration of Neoss® implants with conventional (control group) and hydrophilic (test group) surface treatments. Materials and Methods Six Labrador dogs received Neoss implants with conventional and hydrophilic surfaces. The bone-implant interfaces were evaluated 1 and 4 weeks after implantation, and osseointegration was evaluated using histological, histomorphometric, fluorescence, and resonance frequency analyses. The surfaces were also subjected to topographic and hydrophilicity analyses. Results The topographic analyses revealed increased surface roughness in the test group compared with the control group (surface area roughness 0.42 and 0.78 μm, respectively, for control and test group surfaces; p ≤ .05). The wettability values were higher in the test group (contact angles 67.2° and 27.2° for the control and test group surfaces, respectively; p ≤ .05). Implants in the test group also exhibited better stability, more bone-implant contact, and increased bone area compared with implants in the control group. Background Previous studies have shown predictable bone formation in the maxillary sinus after membrane elevation. However, how and where the bone is formed is not well understood. Purpose The aim of the study was to histologically and immunohistochemically study the early bone formation events in primates after membrane elevation in the maxillary sinus. Materials and Methods Nine adult male tufted capuchin primates ( Cebus apella) were included in the study. Eight animals were subjected to bilateral maxillary sinus membrane elevation using a lateral replaceable bone window technique. One oxidized dental implant was placed into the maxillary sinus cavity on both sides. In four animals, one sinus was left without any additional treatment, whereas the contralateral sinus was filled with autologous bone grafts from the tibia. In two animals, the implants were inserted under the elevated sinus membrane on both sides. In two animals, the sinus membrane was totally removed. The animals were euthanized after 10 or 45 days. One nonoperated animal representing pristine tissue conditions served as control. The maxillary sinuses with implants were retrieved and further processed for light microscopic ground sections or decalcified sections for immune-histochemical analyses. Results Bone formation started from the bottom of the sinus floor, sprouting into the granulation tissue along the implant surface under the elevated membrane irrespective of time and surgical technique. Bone formation was not seen in direct conjunction with the sinus membrane. A distinct expression of osteopontin was observed in the serous glands of the lamina propria close to the implant within all groups. Conclusion Bone formation after sinus membrane elevation with or without additional bone grafts starts at the sinus floor and sprouts into the elevated space along the implant surface. Background Bone density examination ( BDE) using preoperative cone beam computed tomography ( CBCT) might be used to predict primary implant stability in implant patients. Purpose The aim of the study was to validate a novel CBCT scanner in vitro with regard to BDE in preoperative scans and to analyze the in vivo correlation of CBCT scan results with primary implant stability measurements. Materials and Methods A CBCT scanner was validated in vitro with regard to spatial uniformity and linearity of CT numbers ( Hounsfield units, HU) by using a series of phantoms and plastic and hydroxyapatite specimens of various densities. Forty-nine patients (27 female and 22 male, mean age 55.6 ± 9.8) were scanned prior to and 1 to 6 months after the placement of 155 dental implants of different lengths and diameters. Mean and peak insertion torque ( IT) in Ncm were registered during implant placement. Resonance frequency analysis ( RFA) measurements in ISQ units were performed after placement. The second scan was used to export and superimpose the exact positions of bone and implants into the first scan. Virtual probes with the same length as the actual implant were automatically placed at the implant sites, and mean HU values were measured in a 1 mm-wide circular corridor from the tip of the threads and out. Results The in vitro validation showed high uniformity and linearity of CT numbers ( HU). The clinical study showed significant correlations between bone density and ISQ, mean IT, and peak IT, respectively. Conclusions The White Fox CBCT scanner measures bone density with high accuracy. There is a correlation between bone density and primary implant stability as assessed with IT and RFA measurements. The findings suggest that BDE may be used as an additional feature in treatment-planning software to estimate primary stability at predetermined implant sites. [ABSTRACT FROM AUTHOR]

Background Full-arch clearances of compromised teeth and placement of implant-supported prostheses is one solution for the prosthetic rehabilitation of partially dentate patients. Purpose To retrospectively evaluate treatment outcomes after full clearance, immediate placement, and early loading of full-arch fixed bridges. Materials and Methods Fifty-five patients subjected to full clearance and placement of 284 Neoss implants ( Bimodal™ and Proactive™, Neoss Ltd, Harrogate, UK) in 29 edentulous maxillae and 26 mandibles for early loading (1 to 3 days) of a provisional full-arch bridge were retrospectively evaluated after 1 to 6 years of loading. Osstell™ measurements ( Osstell AB, Göteborg, Sweden) were taken at placement and after 3 to 9 months when the provisional bridge was replaced with a permanent one. Marginal bone levels were measured in intraoral radiographs. Results All patients (100%) wore a fixed bridge at the time of finalizing the study. A total of 18 failures (6.3%) were encountered during the follow-up, giving an overall cumulative survival rate of 93.7%. All failures occurred in the maxilla (10.6%), and no implants were lost in the mandible. More Bimodal™ (9.0%) than Proactive™ (4.1%) implants failed. Failing implants showed a significantly lower mean primary stability than successful ones ( p = .015). Failed cases showed a significantly lower average ISQ for all implants ( p = .015) and a marked decrease to the second registration, while successful cases showed and maintained high ISQs. The average bone loss after 1 year was 0.8 ± 0.5 mm. Conclusions Full-arch clearance of severely diseased teeth followed by immediate placement of Neoss implants, early loading with provisional full-arch bridges, and subsequent permanent bridges is a possible treatment modality for partially dentate patients. Background Long-term data regarding survival and crestal bone loss for Replace Select Tapered implants (Nobel Biocare AB, Gothenburg, Sweden) are lacking. Purpose The study aims to present the 5-year outcomes from a retrospective analysis of Replace Select Tapered implants placed and restored in consecutive patients. Materials and Methods A total of 88 consecutive patients (32 male, 56 female, mean age 65 ± 12 years) treated by one clinician ( PP) were clinically and radiographically evaluated during at least 5 years of function. A total of 271 dental implants ( Replace Select Tapered, Nobel Biocare AB) with an oxidized surface ( Ti Unite, Nobel Biocare AB) had been placed in both jaws (228 in the maxilla, 43 in the mandible). The majority of implants were placed in healed sites ( n = 244), while 27 implants were immediately placed in extraction sockets. The majority of implants ( n = 262) healed for 3 to 4 months prior to loading, and nine implants were immediately loaded. A total of 121 implant-supported restorations were delivered; 42 single tooth replacements, 61 fixed partial bridges, 14 fixed full bridges, and 4 fixed partial implant-tooth connected bridges. The marginal bone level was measured in intraoral radiographs taken after surgery (baseline), and after 1, 2, 3, 4, and 5 years. Results Fifty-one patients with 160 implants were followed throughout the study. One implant failed at healing abutment connection 4 months after insertion, resulting in a cumulative survival rate of 99.6%. The average crestal bone loss was 0.9 ± 1.6 mm after 1 year and 0.1 mm ± 2.4 after 5 years. There were 14.8% of measured implants that showed more than 2 mm and 5.2% more than 3 mm bone loss after 5 years, with no progression since the 1-year examination. One patient (2.0%) treated with six implants presented with significant crestal bone loss and recurrent peri-implant purulent infections at all implants. Background Clinical studies on immediate loading of implants in the posterior atrophic maxilla are rare. Purpose The study aims to evaluate immediate loading of implants placed with sinus membrane elevation without additional grafting material for bone augmentation of the maxillary sinus floor. Materials and Methods The study group comprised of 10 patients in whom a total of 10 maxillary sinus floor augmentations were performed. A total of 21 dental implants (1 to 4) were inserted through the residual bone to protrude into the maxillary sinus under the elevated sinus membrane. The implant site was underprepared to improve primary stability. All the implants were inserted with a torque insertion no less than 20 Ncm. Implants were loaded immediately after surgery with a screw-retained temporary acrylic restoration. Intraoral X-rays were taken at implant insertion, after 6 months loading, and after 1st and 2nd year of loading. Resonance frequency analysis ( RFA) was performed at the time of initial placement and after 6 months of functional loading. Results RFA after implant insertion gave an implant stability quotient ( ISQ) level with a range from 62 to 72. All implants remained clinically stable during the follow-up period of 2 years. Radiography demonstrated on average 5.7 ± 3.4 mm of intrasinus new bone formation after 6 months of implant loading. RFA measurements showed ISQ mean values of 67 (range: 62-72) and 68 (range: 62-71) at placement and after 6 months of loading, respectively. Conclusion Within the limits of this case series report, it is concluded that maxillary sinus membrane elevation with simultaneous placement and immediate loading of implants without the use of any additional grafting material shows predictable results after 2 years of functional loading. Moreover, evidence of intrasinus bone formation around the implants was found in all patients. Background New implant designs are continuously introduced to the market. It is important to evaluate and report on their clinical performance when used in everyday practice. Purpose The aim of the present study was to evaluate the clinical performance of a novel hydrophilic dental implant for 1 year. Materials and Methods A total of 49 patients previously treated with 102 hydrophilic dental implants ( Neoss Proactive, Neoss Ltd, Harrogate, UK) were retrospectively evaluated with regard to survival rate and marginal bone loss. Fifty-four implants were installed in maxillae and 48 in mandibles to replace single teeth ( n = 21), to support partial bridges ( n = 26), total maxillary bridges ( n = 2), or mandibular overdentures ( n = 2). The majority of patients ( n = 37) had implants placed in healed sites without any adjunctive procedures. In 12 patients, implants were immediately placed in extraction sockets or in conjunction with maxillary sinus floor augmentation. All implant sites had been classified according to the Lekholm and Zarb index. Baseline and 1-year intraoral radiographs were used to calculate marginal bone levels and bone loss. Implant stability quotient ( ISQ) measurements had been taken at placement and after 3 to 4 months of healing Results The implants became rapidly covered with blood at the first contact. One implant was lost, giving a cumulative survival rate ( CSR) of 99.0% after 1 year. The marginal bone loss amounted to 0.7 ± 0.6 mm with 3.5% of the implants showing more than 2 mm of bone loss and no implant more than 3 mm bone loss after 1 year. The primary stability was found to be 72.7 ± 7.5 ISQ, which slightly increased to 73.6 ± 7.2 ISQ ( NS) after 3 to 4 months of healing. The stability was significantly higher in the mandible than in the maxilla at placement and after healing. Background When a foreign body is placed in bone or soft tissue, an inflammatory reaction inevitably develops. Hence, osseointegration is but a foreign body response to the implant, which according to classic pathology is a chronic inflammatory response and characterized by bone embedding/separation of the implant from the body. Purpose The aim of this paper is to suggest an alternative way of looking at the reason for marginal bone loss as a complication to treatment rather than a disease process. Materials and Methods The present paper is authored as a narrative review contribution. Results The implant-enveloping bone has sparse blood circulation and is lacking proper innervation in clear contrast to natural teeth that are anchored in bone by a periodontal ligament rich in blood vessels and nerves. Fortunately, a balanced, steady state situation of the inevitable foreign body response will be established for the great majority of implants, seen as maintained osseointegration with no or only very little marginal bone loss. Marginal bone resorption around the implant is the result of different tissue reactions coupled to the foreign body response and is not primarily related to biofilm-mediated infectious processes as in the pathogenesis of periodontitis around teeth. This means that initial marginal bone resorption around implants represents a reaction to treatment and is not at all a disease process. There is clear evidence that the initial foreign body response to the implant can be sustained and aggravated by various factors related to implant hardware, patient characteristics, surgical and/or prosthodontic mishaps, which may lead to significant marginal bone loss and possibly to implant failure. Admittedly, once severe marginal bone loss has developed, a secondary biofilm-mediated infection may follow as a complication to the already established bone loss. Conclusions The present authors regard researchers seeing marginal bone loss as a periodontitis-like disease to be on the wrong track; the onset of marginal bone loss around oral implants depends in reality on a dis-balanced foreign body response. Background: Calcium phosphate-based materials have been widely used as bone substitutes and more recently are being exploited together with growth factors as bone tissue engineering scaffolds regulating cell behavior. The aim of this study is to evaluate the in vitro and in vivo response to a newly developed calcium metaphosphate (CMP) bone graft, with and without bone-stimulating growth factor. Methods: Porous scaffolds of CMP were developed and extensively tested in vitro. Subsequently, CMP grafts with osteogenic protein-1 (OP-1) (test) and without OP-1 (control) were implanted into experimental rabbit maxillary bone defects. Animals were sacrificed at 2, 4, and 8 weeks, and samples were examined with microcomputed tomography (micro-CT) and processed for histomorphometric analysis. Results: At 8 weeks, the scaffolds containing OP-1 induced greater bone formation (P = 0.018) than CMP alone, based on histomorphometric evaluation (percentage bone area: test: 57.1 ± 5.6; control: 49.4 ± 7.7) and micro-CT analysis (percentage bone volume density: test: 63.46 ± 5.61; control: 51.20 ± 6.71). Thus, these data indicated that both test and control CMP grafts showed a good degree of bone formation. Furthermore, the CMP materials showed signs of resorption from 4 weeks, and no graft materials were observed at 8 weeks. Conclusions: In vitro, the OP-1 loaded graft demonstrated a release profile and bioactivity over a 28-day period. Background: Platelet concentrate/platelet-rich plasma (PRP) has been studied extensively in various experimental models and there is some agreement among workers to its early effect in bone regeneration and healing. We have earlier showed in vitro that titanium in whole blood activates the thrombogenic response to a higher degree than PRP and that a fluoridated test surface augmented the effect compared with control. Purpose: We designed this study to evaluate the effect of PRP and whole blood on bone regeneration in a dog implant defect model and, in addition, the effect of a test surface modified in hydrofluoric acid. A correlation attempt between platelet count, release of growth factors, and bone regeneration was made. Materials and Methods: Six dogs were used and simultaneously with the experimental surgery and implant installation, autologous PRP was prepared. Defects were prepared (6 mm in diameter and 5 mm deep), and implants were installed (TiO2 gritblasted and hydrofluoric acid treated [test] or TiO2 gritblasted [control], 5 mm in diameter and 9 mm long) in defects filled with either PRP or whole blood. Randomization of sides between PRP and whole blood, and sites for test and control implants were made. Blood samples were collected from PRP and whole blood. The dogs were killed after 5 weeks of healing, and samples with implants and surrounding bone were collected and processed for analysis. Enzyme linked immunosorbent assays were used for detection of growth factors in PRP. Results: The mean increase of platelet count was 424% in PRP. A correlation for platelet counts and transforming growth factor β was found in each dog ( r2 = 0.857). Approximately 50% of the region of interest (ROI) in the defects was filled with new bone after 5 weeks. No difference could be observed in ROI by using PRP or whole blood in the defects regarding new bone formation, bone in contact with implant, or distance to first bone contact. However, the fluoridated implants exhibited more new bone formation ( p = .03) compared with control, regardless of comparing PRP or whole blood, and also displayed a shorter distance from first bone contact to the margin of the bone envelope ( p = .05). Conclusions: Platelet concentrate/PRP failed to show more new bone regeneration in a peri-implant defect model compared with whole blood. ABSTRACT Purpose: Evaluate correlations between volume change for iliac crest bone grafts in maxillary reconstruction (graft volume change [GVC]) and bone mineral density (BMD), bone volume fraction (BVF), hematologic bone metabolic factors (I), and identify indicators of implant failure (II). Material and Methods: Forty-six consecutive patients had their edentulous atrophic maxilla reconstructed with free autogenous bone grafts from anterior iliac crest. Endosteal implants were placed 6 months after graft healing. Computer tomography was performed after 3 weeks and 6 months after grafting. Bone biopsies were taken from the internal table of donor site for calculation (BVF), and blood samples were collected. Implant stability was measured at placement with resonance frequency analysis and expressed as implant stability quotient (ISQ). Implant failure was registered. Results: GVC in onlay bone graft was 37%. The BVF in iliac crest biopsies was 32%. Serum-IGFBP3 differed with 79% of the samples over normal range. Fifteen patients had one or more implant failures prior to loading (early failures). Forty-two patients were followed for a minimum of 3 years after implant loading and, in addition, 6/42 patients had one or more implants removed during the follow-up (late failures). GVC correlated to decreased BMD of lumbar vertebrae L2-L4 (Kruskal-Wallis test, p = .017). No correlation was found between GVC and hematologic factors (Pearson correlation test) or between GVC and BVF (Kruskal-Wallis test). No correlation was found between ISQ and GVC (Pearson correlation test, p = .865). The association between implant failures and the described factors were evaluated, and no significant correlations were found (unconditional logistic regression). Conclusion: Onlay bone grafts decrease 37% during initial healing period, which correlate to BMD of lumbar vertebrae L2-L4. No other evaluated parameters could explain GVC. Implant stability and maintenance of stable crestal bone level are prerequisites for the successful long-term function of oral implants, and continuous crestal bone loss constitutes a threat to the longevity of implant-supported prosthetic constructions. The prevalence/incidence and reasons for crestal bone loss are under debate. Some authors regard infection (i.e. peri-implantitis) as the cause for virtually all bone loss, while others see crestal bone loss as an unavoidable phenomenon following surgery and implant loading. Irrespective of the cause of continuous crestal bone loss, correct usage and scientifically sound interpretation of radiographs are of utmost importance for evaluation of oral implants. The periapical radiographic technique is currently the preferred method for evaluating implant health based on bone loss, and digital radiographs allow easy standardization of the image contrast. It is suggested that baseline radiographs should be taken at the time the transmucosal part pierces the mucosal tissues and annually thereafter. The number of unreadable radiographs should be presented in scientific publications to give insights into the quality of the radiographic examination. It is suggested that not only mean values, but also the range of bone levels, should be presented to describe the proportion of implants that show continuous crestal bone loss. In the absence of other clinical symptoms, bleeding on probing around implants seems to be a weak indicator of ongoing or future loss of crestal bone. According to recent longitudinal studies on modern implant surfaces peri-implantitis defined as 'infection with suppuration associated with clinically significant progressing crestal bone loss' occurs with a prevalence of less than 5 % in implants with 10 years in function. ABSTRACT Background: Primary stability can be improved by using a tapered implant in a slightly underprepared implant site. This may lead to high compression forces and elevated insertion torques. It has been postulated that disturbance of the local microcirculation may occur, leading to necrosis of the osteocytes and bone resorption. Purpose: Report on the clinical outcome of 42 implants placed with an insertion torque equal or greater than 70 Ncm and evaluate bone levels around these implants. Materials and Methods: This prospective study included 48 patients treated with 66 4.5 mm diameter Tapered Screw-Vent implants (Zimmer Dental®, Carlsbad, CA, USA). Maximum insertion torque (MIT) was recorded with an electronic torque measuring device (Tohnichi® STC200CN, Hitachi, Tokyo, Japan). Nine implants (control group) presented MIT between 30 and 50 Ncm (mean = 37.1 Ncm) and 42 implants (experimental group) MIT greater than 70 Ncm (mean = 110.6 Ncm, range: 70.8-176 Ncm). Marginal bone levels were recorded at the time of loading and 1 year later for the two groups. Results: After 2-3 months of non-sumerged healing, all implants were clinically stable. Mean marginal bone resorption was 1.03 mm (SD = 0.44) for the control group (low torque) and 0.72 mm (SD = 0.56) for the experimental group (high torque) at time of loading, and 1.09 (SD = 0.62) and 1.24 mm (SD = 0.75), respectively, after 1 year. There were no significant differences between the two groups for bone stability and implant success rate. Conclusions: The use of high insertion torques (up to 176 Ncm) did not prevent osseointegration. ABSTRACT Background: Insufficient bone volume often hamper placement of dental implants in the posterior maxilla. Purpose: The aim of the present clinical study was to evaluate retrospectively the clinical outcome of implant placement in the resorbed posterior maxilla using an osteotome technique without adding any grafting material. Materials and Methods: Twenty patients with 5 to 9 mm of residual alveolar bone height in the posterior maxilla received twenty-nine implants (Neoss Ltd., Harrogate, UK) using an osteotomy technique without bone grafts. Intraoral radiographs were taken before and after implant placement, at the time of loading and after 11 to 32 months of loading (mean 16.4 months), to evaluate bone formation below the sinus membrane and marginal bone loss. Implant stability measurements (OsstellTM, Gothenburg, Sweden) were performed after implant installation and at abutment connection 5 months later. All implants were installed with the prosthetic platform level with the bone crest. Results: No implant was lost giving a survival rate of 100% after a mean follow-up time of 16.4 months. The average vertical bone height was 7.2 ± 1.5 mm at placement and 10.0 ± 1.0 mm after 11 to 32 months. The average increase of 2.8 ± 1.1 mm was statistically significant. There was a statistically significant improvement in implant stability from 70.7 ± 9.2 implant stability quotient (ISQ) at placement to 76.7 ± 5.7 ISQ at abutment connection, 5 months later. The mean marginal bone loss amounted to 0.7 ± 0.3 mm after 11 to 32 months of loading. Conclusion: It is concluded that the osteotome technique evaluated resulted in predictable intrasinus bone formation, firm implant stability, and good clinical outcomes as no implants were lost and minimal marginal bone loss was observed. ABSTRACT Background: Extensive atrophy of the alveolar process may require a bone-grafting procedure prior to implant treatment. Autogenous bone grafts from the iliac crest, used as onlay block and particulate bone, have been used together with sinus-lift procedure in order to rehabilitate patients with extremely resorbed maxillae. However, there are to our knowledge no 5-year follow-up studies evaluating the extent of bone-level change in patients treated with respectively block and particulate autogenous bone grafts. Purpose: The purpose of this prospective clinical study was to conduct a 5-year follow-up analysis with focus on bone-level alteration in block versus particulate onlay bone grafts. Material and Methods: Fifteen out of originally 19 patients who were treated with iliac bone grafts and oral implants in the maxilla have been followed through the first 5 postoperative years. In a first study conducted on 19 patients, the role of platelet-rich plasma in conjunction with autogenous bone was evaluated. In this 5-year follow-up study, the marginal bone alterations have been documented at base line, 1 year and 5 years of loading to the nearest 0,1 mm at mesial and distal surfaces of the implants. Two implants were installed on each side of the midline in either block or particulate bone grafts giving test and control sides in each patient. Additionally, two implants on each side were installed in residual bone/grafted sinus floor. Result: Marginal bone alteration in the anterior maxilla appeared larger at the side augmented by block bone at baseline, and after 1 and 5 years of loading, but the change was not statistically significant. Moreover, there was a significantly higher degree of marginal alteration during the first year of loading, compared with the examinations after 5 years. Conclusion: The present follow-up study showed that there is no significant difference in the extension of resorption between block- and particulate autogenous bone grafts over a 5-year period. ABSTRACT Background: Concerns have been raised that use of surface-modified implants may result in peri-implant infection and marked marginal bone loss over time. Purpose: The aim of this prospective study was to evaluate the survival rate, marginal bone, and soft tissue conditions at surface-modified titanium dental implants after 10 years of function. Material and Methods: Forty-six totally and partially edentulous patients were provided with 121 Brånemark oxidized implants (TiUnite™, Nobel Biocare AB, Gothenburg, Sweden). Twenty-four (20%) implants were immediate loaded and 97 (80%) were placed using a two-stage procedure. A total of 22 single, 23 partial, and 7 total restorations were delivered. Clinical and radiographic checkups were carried out after 3, 6, 12 months, and thereafter annually up to 10 years. At these occasions, oral hygiene was evaluated and peri-implant mucosa examined by probing. If needed, patients were enrolled in an individual program for hygiene controls and professional cleaning. Marginal bone loss was evaluated in intraoral radiographs taken at baseline and after 1, 5, and 10 years of function. Results: One (0.8%) implant failed after 8 years giving a Survival Rate (SR) of 99.2% after 10 years. A total of 11 sites (9.2%) showed bleeding on probing (BP) at the 10th annual checkup. The mean marginal bone loss was 0.7 ± 1.35 mm based on 106 readable pairs of radiographs from baseline and from the 10th annual examination. Twelve (11.3%) implants showed more than 2 mm bone loss, and five (4.7%) showed more than 3 mm of bone loss after 10 years. For the latter, all patients were smokers and had poor or acceptable oral hygiene. All five implants with >3 mm bone loss showed BP and two (1.9%) showed suppuration from the pocket. For the remaining seven implants with more than 2 mm bone loss, no correlation to smoking, oral hygiene, bleeding, or pus could be seen. Time/marginal bone level plots of the 12 implants with more than 2 mm bone loss after 10 years, showed minor changes from the first annual checkup except for the two infected implants. Conclusions: It is concluded that good long-term clinical outcomes can be obtained with oxidized titanium dental implants. ABSTRACT Background: The bone tissue responses to Cyanoacrylate have been described in the literature, but none used N-butyl-2-cyanoacrilate (NB-Cn) for bone graft fixation. Purpose: The aims of the study were: (a) to analyze the bone grafts volume maintenance fixed either with NB-Cn or titanium screw; (b) to assess the incorporation of onlay grafts on perforated recipient bed; and (c) the differences of expression level of tartrate-resistant acid phosphatase (TRAP) protein involved in bone resorption. Materials and Methods: Eighteen New Zealand White rabbits were submitted to calvaria onlay grafting on both sides of the mandible. On one side, the graft was fixed with NB-Cn, while on the other hand the bone graft was secured with an osteosynthesis screw. The computed tomography (CT) was performed just after surgery and at animals sacrifice, after 1 ( n = 9) and 6 weeks ( n = 9), in order to estimate the bone grafts volume along the experiments. Histological sections of the grafted areas were prepared to evaluate the healing of bone grafts and to assess the expression of TRAP protein. Results: The CT scan showed better volume maintenance of bone grafts fixed with NB-Cn ( p ≤ 0.05) compared with those fixed with screws, in both experimental times (analysis of variance). The immunohistochemical evaluation showed that the TRAP expression in a 6-week period was significantly higher compared with the 1-week period, without showing significant difference between the groups (Wilcoxon and Mann-Whitney). Histological analysis revealed that the NB-Cn caused periosteum damage, but provided bone graft stabilization and incorporation similar to the control group. Conclusion: The perforation provided by screw insertion into the graft during fixation may have triggered early revascularization and remodeling to render increased volume loss compared with the experimental group. Background A consensus meeting was arranged to critically analyze whether the high figures of peri-implantitis at machined implants that recently have been reported in the literature are valid also for modern implants. Purpose The aims of this paper were to present the outcomes from the consensus meeting and to evaluate recent long-term clinical studies on modern implants with regard to frequency of peri-implant infection. Materials and Methods Ten different studies of three modern implant brands of moderately rough surfaces with 10-year or longer follow-up times were found through a PubMed and manual search. Results It was concluded that bleeding on probing or probing depths are weak indicators of crestal bone loss ( CBL); that CBL occurs for many other reasons than infection; that implant-, clinician-, and patient-related factors contribute to CBL; and that modern oral implants outperform older devices. Based on a literature search, the frequency of implants with reported peri-implant infection and significant bone loss leading to implant removal or other surgical intervention was on average 2.7% during 7 to 16 years of function. Conclusion The summed frequency of peri-implantitis and implant failure is commonly less than 5% over 10 years of follow-up for modern implants when using established protocols. ABSTRACT Background: The presence of localized defects and/or small amounts of bone below the maxillary sinus is a common finding, which may compromise implant placement. There is therefore a need for predictable techniques for bone augmentation in such situations. Purpose: The study aims to clinically and histologically evaluate a porcine bone (PB) substitute used for augmentation of the alveolar crest or the maxillary sinus floor prior to or in conjunction with implant placement. Materials and Methods: Nineteen patients were treated with a porcine bone substitute and barrier membranes (OsteoBiol, Tecnoss Dental, Turin, Italy) for lateral bone augmentation (Group 1a) and healing of bone defects (Group 1b) or for augmentation of the maxillary sinus floor using either a replaceable (Group 2a) or an infractured bone window (Group 2b). A total of 34 implants (Neoss Ltd., Harrogate, UK) were placed in conjunction or 5 to 7 months after the procedure. Implants were followed with implant stability measurements at placement and abutment connection, and with intraoral radiographs at abutment connection and after at least 1 year of loading. A biopsy for histology and morphometry was taken at the first reentry operation. Results: All but one of the procedures was successful (94.7%) as one maxillary sinus procedure (Group 2a) resulted in insufficient bone for implant placement. One of the 34 implants failed, giving an implant survival rate of 97.1% after 1 year. Implant stability measurements showed a mean stability of 71.9 ± 7.7 implant stability quotient (ISQ) at placement, which significantly increased to 75.3 ± 6.8 ISQ at abutment connection ( p = .03). The average bone loss was 0.5 ± 0.7 mm during 1 year. Histology revealed new bone formation at the PB surface, which formed bridges between particles and between particles and preexisting bone. The presence of scalloped resorption lacunae and new osteons inside the particles indicated ongoing resorption/remodeling of the particles. The histomorphometric analyses showed that the total specimen area consisted of, in average, 56.5 ± 15.7% mineralized tissue of which 24.8 ± 13.9% of the total area was PB particles. Conclusion: This study showed good clinical results when using a PB substitute and barrier membranes for augmentation of the alveolar crest and maxillary sinus. ABSTRACT Background: The mere lifting of the maxillary sinus membrane by implants protruding into the sinus cavity allows the establishment of a void space for blood clot and new bone formation. Purpose: To evaluate bone formation by using a spherical, hollow, and perforated hydroxyapatite space-maintaining device (HSMD) in a two-stage sinus lift procedure where residual alveolar bone height was ≤2 mm. Material and Methods: Spherical, hollow, and perforated HSMDs with a diameter of 12 mm were manufactured for this pilot study. Three patients with a residual bone height of 1-2 mm, as verified clinically and radiographically, and in need of a sinus augmentation procedure prior to implant installation were selected for the study. The HSMD and bone formation was evaluated by cone beam computerized tomography (CBCT) 6 months after augmentation procedure. Implants were installed 6 to 9 months after augmentation. The implant sites were prepared by a trephine drill to obtain a specimen of HSMD and bone for histological evaluation. After implant installation, the condition of the sinus membrane adjacent to the HSMD was evaluated endoscopically. After an additional 8 weeks, fixed partial prostheses were fabricated. Results: Bone formation verified by CBCT was found around and inside the device in all three patients after 6 months. Despite the fact that residual bone before augmentation was ≤2 mm, 12-mm-long implants with diameter of 4.8 mm could be inserted with preservation of an intact and healthy sinus membrane verified endoscopically. Bone formation inside HSMDs was noted histologically in two out of three HSMDs. Implants were stable and without any marginal bone loss after 1 year of prosthetic loading. Conclusion: A spherical, hollow, and perforated HSMD used in sinus lift procedures can produce a void space for blood clot and new bone formation and subsequent implant installation. ABSTRACT Background: New dental implant systems are continuously introduced to the market. It is important that clinicians report their experiences with these implants when used in different situations. Aim: The study aims to report the outcomes from a retrospective study on Neoss implants when used with or without guided bone regeneration (GBR) procedures. Materials and Methods: The study group comprised of 50 consecutive patients previously treated with 183 Neoss implants (Neoss Ltd., Harrogate, UK) in 53 sites because of single, partial, or total tooth loss. Implants were placed in healed bone in 23 sites, while a GBR procedure was used in 30 sites in conjunction with implant placement. A healing period of 3 to 6 months was utilized in 45 sites and in 8 sites a crown/bridge was fitted within a few days for immediate/early function. The number of failures, withdrawn and dropout implants was analyzed in a life-table. All available intraoral radiographs from baseline and annual check-ups were analyzed with regard to marginal bone level and bone loss. Results: A cumulative survival rate (CSR) of 98.2% was found for the non-GBR group and 93.5% for the GBR group with an overall CSR of 95.0% after up to 5 years of loading. In spite of the failures, all patients received and maintained their prostheses. Based on all available radiographs, the bone level was situated 1.3 ± 0.8 mm ( n = 159) below the top of the collar at baseline and 1.7 ± 0.8 mm ( n = 60) after 5 years of follow-up. Based on paired baseline and 1-year ( n = 70) and 5-year radiographs ( n = 59), the bone loss was found to be 0.4 ± 0.9 and 0.4 ± 0.9 mm, respectively. There were no statistically significant differences between GBR and non-GBR sites with regard to implant survival or bone loss. ABSTRACT Background: Titanium zirconium alloy with 13-17% zirconium (TiZr1317) shows significantly better mechanical attributes than pure Ti with respect to elongation and fatigue strength. This material may be suitable for thin implants and implant components exposed to high mechanical constraints. Purpose: The aim of this study was to test the hypothesis that TiZr1317 and Ti implants show comparable osseointegration and stability. Materials and Methods: The mandibular premolars (P1, P2, P3) and the first molar (M1) in 12 adult miniature pigs were extracted 3 months prior to the study. Six specially designed implants made from Ti (commercially pure, Grade 4) or TiZr1317 (Roxolid®, Institut Straumann AG, Basel, Switzerland) with a hydrophilic sandblasted and acid-etched (SLActive, Institut Straumann AG, Basel, Switzerland) surface were placed in each mandible; three standard implants modified for evaluation of removal torque (RT) in one side and three bone-chamber implants for histologic observations in the contralateral side. RT tests were performed after 4 weeks when also the bone chamber implants and surrounding tissue were biopsied for histologic analyses in ground sections. Results: The RT results indicated significantly higher stability ( p = 0.013) for TiZr1317 (230.9 ± 22.4 Ncm) than for Ti implants (204.7 ± 24.0 Ncm). The histology showed similar osteoconductive properties for both implant types. Histomorphometric measurements showed a statistically significant higher ( p = 0.023) bone area within the chamber for the TiZr1317 implants (45.5 ± 13.2%) than did the Ti implants (40.2 ± 15.2%). No difference was observed concerning the bone to implant contact between the groups with 72.3 ± 20.5% for Ti and 70.2 ± 17.3% for TiZr1317 implants. ABSTRACT Background: Computerized tomographs (CTs) are commonly used for presurgical planning of dental implant placement. It is possible that implant stability can be predicted based on quantitative measurements of bone density at planned implants sites with the use of diagnostic software. Purpose: The aim was to evaluate if there is a correlation between bone density measurements in specific implant positions in preoperative CTs and insertion torque (IT) and implant stability measurements when placing the implants. Materials and Methods: The study comprised of four patients in whom presurgical CTs had been used to plan implant treatment. A total of 26 implants (Neoss, Harrogate, UK) were placed in the totally edentulous maxilla ( n = 3) or mandible ( n = 1). IT was measured during implant insertion and the torque/time curves examined for mean IT (newton-centimeters) over the total curve. The stability of implants was measured with resonance frequency analysis. The positions of the implants were extracted from a postoperative CT to the preoperative one. Bone density was measured with a dedicated software (3Diagnosys™ 3.0, 3Diemme, Cantù, Italy) in virtual hollow probes, indicating the bone volume within 1 mm from each implant surface. The Spearman Rank correlation test was used to find possible correlations. Results: Statistically significant correlations were found between mean bone density, mean IT, and implant stability measurements. Conclusions: This pilot study showed a correlation between bone density, as measured in Hounsfield units in preoperative CTs, and IT and implant stability measurements at the surgical placement of the implants. The findings support the idea that integration of bone density measurements in implant probes in preoperative CTs using treatment-planning software may be a useful feature to predict implant stability and to avoid failures. ABSTRACT Background: The Neoss implant system has been available since 2004. Few studies documenting the clinical performance of this implant are available. Purpose: To study the stability and clinical/radiographic outcomes of Neoss implants 1 year of loading when using a two-stage protocol. Materials and Methods: Ninety (90) consecutive patients scheduled for implant treatment using a two-stage procedure were enrolled in a prospective follow-up study. A total of 218 implants (Neoss System, Bimodal surface, Neoss Ltd, Harrogate, UK) in diameters of 3.5, 4.0, and 4.5 mm and in lengths from 7 to 15 mm were placed to support 29 single tooth replacements, 53 partial bridges, 5 full bridges, and 10 overdentures in both jaws. Abutment connection was made after a healing period of 3-4 months. The patients were followed during 1 year of loading with clinical, radiographic, and resonance frequency analysis (Osstell Mentor™, Osstell AB, Gothenburg, Sweden) examinations. Prostheses were removed at the annual check-up for individual testing of implant stability. Results: Three implant failures were experienced, giving a survival rate of 98.6% after 1 year. A mean bone loss of 0.6 mm (SD 0.8) was observed after 1 year. There was a significant inverse correlation between implant diameter and marginal bone loss ( p < .003). The mean implant stability quotient levels were 73.7 (SD 7.6), 74.4 (SD 6.4), and 76.7 (SD 5.2) at placement, abutment connection, and first annual check-up, respectively. The stability had increased significantly from placement to 1 year ( p < .001) and from abutment to 1 year ( p < .0001). Implant stability was higher in the mandible than in the maxilla at all time points. There was a significant correlation between bone quality and stability at placement ( p < .0001) and abutment connection ( p < .001) but not after 1 year. Conclusions: The use of Neoss implants for prosthetic rehabilitation of consecutive edentate patients with different needs resulted in predictable clinical and radiographic outcomes after 1 year of loading. ABSTRACT Background: Various designs of dental implants representing different geometries and surface technologies are commercially available and clinically used in patients. However, data with regard to bone tissue responses and stability for comparison of their biologic performances are rare. Purpose: The aim of the present experimental investigation was to compare the bone tissue responses and implant stability between two commonly used dental implants representing different geometries and surface characteristics. Materials and Methods: A total of 90 dental implants (4.3 mm in diameter, 10 mm long) with an oxidized surface (Replace Select Tapered, TiUnite, Nobel Biocare AB, Gothenburg, Sweden) (OX) and 90 implants (4.1 mm in diameter, 10 mm total length) with a hydrophilic sand-blasted and acid etched surface (Standard Plus, SLActive, Institut Straumann AG, Basel, Switzerland) (HSBA) were placed in the distal femur ( n = 1) and tibia ( n = 2) of 30 rabbits. The implants were analyzed with implant stability quotient (ISQ) measurements, removal torque (RTQ) and histomorphometry (bone-implant contact, BIC) after 10 days, 3, and 6 weeks. Moreover, RTQ values were corrected for differences in surface area by calculating the shear strength for each implant. Results: RTQ and ISQ measurements showed an increase with time for both implant types. A significantly higher RTQ value was observed for the HSBA implant at 3 weeks ( p = .05). A lower ISQ value was seen for HSBA than for OX implants at placement in the tibia ( p < 0.001). HSBA implants showed higher shear strength values than OX implants after 3 weeks ( p < .001), and 6 weeks ( p < .01). The morphometric measurements showed significantly higher BIC for HSBA implants after 10 days ( p < .01), similar values after 3 weeks and significantly higher BIC for OX implants after 6 weeks ( p < .001). Conclusions: Both HSBA and OX implants were well integrated in bone and showed firm and increased stability from placement to after 6 weeks of healing. The HSBA implant showed more BIC after 10 days and the OX implant more BIC after 6 weeks of healing. The HSBA implant showed significantly higher shear strength after 3 and 6 weeks and higher RTQ values after 3 weeks than the OX implant. ABSTRACT Background: Maxillary sinus floor augmentation is a procedure that is indicated in cases when the volume of the posterior maxillary bone is inadequate. The goal of this treatment is to obtain sufficient amount of bone tissue in order to gain osseointegration of endosseous implants. Purpose: The purpose of this study was to conduct a clinical and histological analysis of calcium sulfate (CaS) as bone graft substitute in sinus floor augmentation. Material and Methods: Ten patients with edentulous maxillas were included in this study. They had moderate to severe atrophy of the posterior maxilla. Surgiplaster (Classimplant®, Rome, Italy) was used as graft material in the maxillary sinus and was covered by BioGide® (Geistlish Pharmaceutical, Wolhusen, Switzerland). After 4 months of graft healing, 40 dental implants were placed and a biopsy for histomorphometry was taken at these occasions. The specimens were viewed by light microscope, and the extent of bone regeneration and remaining graft material was evaluated. Radiographs were taken at the time of sinus augmentation and after 4 months of graft healing. Results: At the time of abutment surgery, one implant was considered as a failure and was consequently removed, giving a survival rate of 97.5% after 1 year of loading. Radiographs showed a mean of 26.5% shrinkage of the augmented area. A significant resorption of CaS was noted with a mean value of 8.8% of remaining graft material after 4 months of healing. The biopsies also revealed new bone formation with a mean value of 21.2% of the total biopsy area. Histology showed signs of an acellular substitution of CaS with bone-like tissue. Conclusion: The results of this study show that new bone regeneration occurs in the maxillary sinus after augmentation with CaS. This enabled successful placement, integration, and loading of dental implants in the posterior maxilla, as only 1 of 40 implants was lost during 1 year of follow-up. [ABSTRACT FROM AUTHOR]

ABSTRACT Background: Clinical studies have shown a higher degree of implant failures in grafted bone compared with normal nongrafted maxillary bone. Additionally, a prolonged time for integration of titanium implants in grafted block bone has been shown by means of resonance frequency analysis (RFA). Purpose: The aim of this prospective study was to compare the stability of implants placed in particulate bone, onlay block bone, interpositional bone, and nongrafted maxillary bone during the early phase of osseointegration using RFA and implant failure. Material and Methods: Thirty-five patients with edentulism in the maxilla were included in the study. In all, 260 Astra Tech TiOblast™ implants (Astra Tech AB, Mölndal, Sweden) were installed. Twenty-five of these patients had severe maxillary atrophy and were treated with iliac bone grafts 5 to 6 months prior to implant placement, 19 with lateral onlay block grafts on one side (group A, 38 implants) and particulate bone for lateral augmentation on the other (group B, 38 implants). These 19 patients also got bilateral sinus floor augmentation with particulate bone (group C, 76 implants). Six patients had an unfavorable sagittal relation between the jaws and underwent a LeFort I operation with interpositional bone blocks grafted to the nasal and sinus floors (group D, 48 implants). The remaining 10 patients could be treated with implants without bone augmentation and served as control (group E, 60 implants). RFA was performed at implant placement and abutment connection 6 months later and an implant stability quotient (ISQ) value was given for each implant. Results: Four implants (1.5%) were found mobile at abutment connection and removed (two in group A and two in group D). RFA showed a slight increase in stability from installation to abutment connection but the differences were not statistically significant in any of the groups (Wilcoxon signed rank test for comparison of paired data). Implants installed in group D had a significantly lower ISQ value at both measurements compared with the other groups (Wilcoxon Rank Sum test for comparisons of independent samples, p = .05). Conclusion: It is concluded that TiO2-blasted implants placed in nongrafted and grafted maxillary bone using a two-staged protocol show similar stability during the early phase of osseointegration. Background: Survival rates of implants placed in transalveolar sinus floor augmentation sites are comparable with those placed in non-augmented sites. Flapless implant surgery can minimize postoperative morbidity, alveolar bone resorption and crestal bone loss. The use of cone beam computerized tomography (CBCT) provides 3D presentations with reduced dose exposure. Objectives: To evaluate a flapless, CBCT-guided transalveolar sinus floor elevation technique with simultaneous implant installation. Material and methods: Fourteen consecutive patients in need of maxillary sinus floor augmentation were enrolled in this study. Preoperative CBCT with a titanium screwpost as an indicator at the intended implant position was used to visually guide the flapless surgical procedure. Twenty one implants all with a length of 10 mm and a diameter of 4.1 and 4.8 mm were inserted and followed clinically and with CBCT for 3, 6 and 12 months postoperatively. Intraoral radiographs were taken for comparison. All patients were provided with permanent prosthetic constructions 8-12 weeks after implant surgery. Results: Ten (47.6%) implants were inserted in residual bone of 2.6-4.9 mm and 11 (52.3%) implants were inserted in residual bone of 5-8.9 mm. No implants were lost after surgery and follow-up. There was no marginal bone loss during the follow-up verified by CBCT. The implants penetrated on average 4.4 mm (SD 2.1 mm) into the sinus cavity and the mean bone gain was 3 mm (SD 2.1 mm). Conclusion: Flapless transalveolar sinus lift procedures visually guided by preoperative CBCT can successfully be used to enable placement, successful healing and loading of one to three implants in residual bone height of 2.6-8.9 mm. Background: Immediate/early prosthetic rehabilitation of the edentulous mandible with four or more implants for support of a fixed bridge is a well-documented procedure. Only a few studies have reported on the outcome of using three implants for this purpose. Aim: To retrospectively analyze the clinical outcome when using three implants for support of an immediately loaded fixed bridge in the totally edentulous mandible. Materials and methods: In total, 132 patients (67 females and 65 males, mean age 62.6 years, range 35-85 years) treated with 396 implants (Brånemark System) over an 11-year period in one clinic were retrospectively analyzed with regard to implant survival. Implants with both machined and oxidized surface had been used. Eighty percent of the implants were 13 mm in length or longer. The permanent bridge with acrylic teeth was delivered to the patient the same day. The patients were re-called annually for clinical examinations. Results: All patients had been followed for at least 1 year, with the longest follow-up of 10 years and a mean follow-up of 5 years. Thirteen implant failures in 10 patients were experienced, all occurring during the first 12 months of loading. The overall survival rate was 96.7% for implants and 92.4% for original bridges. More machined than oxidized implants failed, 7% vs. 1.2%. Fourteen additional implants were placed at the time of removal of the failed implants when also the fixed bridge was adjusted on the same day. Thus, all patients received and maintained a fixed bridge, except for a few hours in the 10 failure cases when the bridges were adjusted. Conclusions: This retrospective survival analysis shows that three implants can be used to support a fixed bridge for immediate loading in the edentulous mandible. Objectives: To investigate the long-term clinical and radiographic results of the maxillary sinus membrane elevation technique where implants were inserted in a void space created by the elevation of the sinus membrane without adding any graft material. Materials and methods: A total of 84 patients were subjected to 96 membrane elevation procedures and simultaneous placement of 239 implants. Changes of intra-sinus and marginal bone height in relation to the implants were measured in intraoral radiographs taken at insertion, after 6 months of healing, after 6 months of loading and then annually. Computerized tomography was performed pre-surgically and 6 months post-surgically. Resonance Frequency Analyses measurements were performed at the time of implants placement, at abutment connection and after 6 months of loading. The implant follow-up period ranged from a minimum of one to a maximum of 6 years after implants loading. Results: All implants were stable after 6 months of healing. A total of three implants were lost during the follow-up period giving a survival rate of 98.7%. Radiography demonstrated on average 5.3±2.1 mm of intra-sinus new bone formation after 6 months of healing. RFA measurements showed adequate primary stability (implant stability quotient 67.4±6.1) and small changes over time. Conclusion: Maxillary sinus membrane elevation and simultaneous placement of implants without the use of bone grafts or bone substitutes result in predictable bone formation with a high implant survival rate of 98.7% during a follow-up period of up to 6 years. The intra-sinus bone formation remained stable in the long-term follow-up. It is suggested that the secluded compartment allowed for bone formation according to the principle of guided tissue regeneration. The high implant survival rate of 98.7% indicated that the implants sufficiently supported the fixed bridges throughout the study period. ABSTRACT Background: Previous studies have pointed out that the mere elevation of the maxillary sinus membrane promotes bone formation without the use of augmentation materials. Purpose: This experimental study aimed at evaluating if the two-stage procedure for sinus floor augmentation could benefit from the use of a space-making device in order to increase the bone volume to enable later implant installation with good primary stability. Materials and Methods: Six male tufted capuchin primates ( Cebus apella) were subjected to extraction of the three premolars and the first molar on both sides of the maxilla to create an edentulous area. The sinuses were opened using the lateral bone-wall window technique, and the membrane was elevated. One resorbable space-making device was inserted in each maxillary sinus, and the bone window was returned in place. The animals were euthanatized after 6 months, and biopsy blocks containing the whole maxillary sinus and surrounding soft tissues were prepared for ground sections. Results: The histological examination of the specimens showed bone formation in contact with both the schneiderian membrane and the device in most cases even when the device was displaced. The process of bone formation indicates that this technique is potentially useful for two-stage sinus floor augmentation. The lack of stabilization of the device within the sinus demands further improvement of space-makers for predictable bone augmentation. Conclusions: It is concluded that (1) the device used in this study did not trigger any important inflammatory reaction; (2) when the sinus membrane was elevated, bone formation was a constant finding; and (3) an ideal space-making device should be stable and elevate the membrane to ensure a maintained connection between the membrane and the secluded space. Objectives: To compare resorption of a synthetic biphasic calcium phosphate (BCP) bone–graft substitute with deproteinized bovine bone (DBB) used for human maxillary sinus augmentation. Materials and methods: Eleven patients underwent bilateral maxillary sinus floor augmentation with DBB in one side and a BCP (40%β-tricalcium phosphate (β-TCP) and 60% hydroxyapatite) in the contralateral side. Simultaneously, with the augmentation on each side a microimplant was placed vertically from the top of the alveolar crest penetrating the residual bone and the grafting material. Eight months after initial surgery the microimplants were retrieved with a surrounding bone core. The composition of residual graft material and surrounding bone was analysed by scanning electron microscopy and energy dispersive X-ray spectroscopy. Results: Residual graft material of both types was present as 10–500 μm particles in direct contact with, or completely surrounded by, newly formed bone; smaller particles were also present in non-mineralized tissue. In the case of BCP the bone–graft substitute interface showed evidence of superficial disintegration of particles into individual grains. Median Ca/P ratios (at.%), determined from >200 discreet sites within residual graft particles and adjacent bone, were: DBB: 1.61 (confidence interval [CI] 1.59–1.64); BCP: 1.5 (CI 1.45–1.52); DBB-augmented bone: 1.62 (CI 1.59–1.66); BCP-augmented bone: 1.52 (CI 1.47–1.55); P=0.028 for DBB vs. BCP and DBB- vs. BCP-augmented bone. The reduction in Ca/P ratio for BCP over the healing period is consistent with the dissolution of β-TCP and reprecipitation on the surface of calcium-deficient hydroxyapatite. Conclusion: The β-TCP component of BCP may be gradually substituted by calcium-deficient hydroxyapatite over the healing period. Background: Implant design and surface may have an influence on the marginal bone response during immediate functional loading. Aim: The purpose of this study was to radiographically study the effect of implant design on marginal bone preservation at immediately loaded implants used for prosthetic rehabilitation of the completely edentulous mandible. Materials and Methods: A total of 39 patients, previously treated with five implants for support of a full-arch fixed bridge in the mandible, were included in the study. Either machined Brånemark® implants (Ma) (Nobel Biocare AB, Gothenburg, Sweden) or surface modified Astra Tech® implants with (Mi) or without a microthreaded neck (Ti) (TiOblast, AstraTech AB, Mölndal, Sweden) were used. All fixtures were loaded with a provisional glass fiber or metal-reinforced screw-retained restoration within 24 hours. The provisional restorations were replaced by a 12-unit screw-retained metal-ceramic or metal-resin cantilever bridge after 3 months. Bone loss from baseline to 1 year of loading was measured by means of intraoral radiographs. Only patients with baseline and 1-year radiographs of all implants were selected for comparison. Statistical analysis was carried out on both patient and implant levels. Results: The survival rates after 1 year in function were 98.6, 100, and 100% for the Ma, Ti, and Mi implants, respectively. The overall mean bone loss after 1 year was 1.03 mm (SD 0.87; range −0.77 to 2.5). The mean bone loss was calculated to 1.52 (SD 0.66) for the Ma group, 0.79 (SD 0.79) for the Ti group, and 0.70 (SD 1.01) for the Mi group. There was a significant difference between Ma and Ti ( p = .023) and between Ma and Mi ( p = .046) groups but not within Ti and Mi implants ( p = .70). These conclusions were also valid when the statistical analysis was performed on implant level. Conclusions: There is no impact of design and surface on implant survival in the completely edentulous mandible. Bone preservation in immediately loaded implants in the mandible is influenced by implant design and significantly better on surface-modified AstraTech implants compared with machined Brånemark implants. Background: An increasing number of studies show that immediate/early function of dental implants can be as successful as two-stage procedures. However, the results may not be universal for all implant types and it is important that new implants are tested for this treatment modality. Purpose: The aim was to evaluate an immediate/early function protocol in the maxilla and in the posterior mandible using Neoss implants (Neoss Ltd., Harrogate, UK). Materials and Methods: A total of 21 patients were provided with 69 Neoss implants (4 mm in diameter and 9–15 mm in length) and a provisional bridge within 7 days (mean 4.6 days). Sixteen implants were placed in immediate extraction sites where seven were treated with autologous bone grafts ( n = 6) or bone grafts + resorbable membrane ( n = 1). A final fixed prosthesis was made 3 to 6 months later. The patients were followed-up with clinical examinations for 18 months. In addition, the implants were monitored with resonance frequency analysis (RFA) measurements at surgery and after 1, 2, and 6 months. Intraoral radiographs were taken after surgery and after 1, 6, and 18 months. Results: One implant in an extraction site in the maxilla failed after 1 month, giving a survival rate of 98.5% after 18 months. The mean marginal bone loss was 0.7 mm (SD 0.7) after 18 months. RFA showed a mean implant stability quotient (ISQ) value of 68.1 (SD 8.8) at surgery, which increased to 73.7 (SD 5.7) after 6 months. The primary stability for maxillary and mandibular implants was similar, although mandibular implants showed slightly higher values with time. Implants in extraction sockets showed a lower initial stability than in healed sites, ISQ 65.8 (SD 7.5), which increased to ISQ 67.5 (SD 6.9) after 6 months. The failed implant showed an ISQ of 74 at placement, which decreased to 42 1 month after surgery. Conclusion: Within the limitations of the present study, it is concluded that immediate/early function with Neoss implants is a reliable method with an implant survival rate comparable to that of the traditional two-stage protocol. Background: Titanium (Ti) is widely proven to enhance bone contact and growth on its surface. It is expected that bone defects could benefit from Ti to promote healing and to increase strength of the implanted area. Purpose: The present study aimed at comparing the potential of porous Ti sponge rods with synthetic hydroxyapatite (HA) for the healing of bone defects in a canine model. Material and Methods: Six mongrel dogs were submitted to three trephined osteotomies of 6.0 × 4.0 mm in one humerus and after 2 months another three osteotomies were performed in the contralateral humerus. A total of 36 defects were randomly filled either with Ti foam, particulate HA, or coagulum (control). The six animals were killed 4 months after the first surgery for histological and histometrical analysis. Results: The Ti-foam surface was frequently found in intimate contact with new bone especially at the defect walls. Control sites showed higher amounts of newly formed bone at 2 months – Ti ( p = 0.000) and HA ( p = 0.009) – and 4 months when compared with Ti ( p = 0.001). Differently from HA, the Ti foam was densely distributed across the defect area which rendered less space for bone growth in the latter's sites. The use of Ti foams or HA resulted in similar amounts of bone formation in both time intervals. Nevertheless, the presence of a Ti-foam rod preserved defect's marginal bone height as compared with control groups. Also, the Ti-foam group showed a more mature bone pattern at 4 months than HA sites. Conclusion: The Ti foam exhibited good biocompatibility, and its application resulted in improved maintenance of bone height compared with control sites. Background: Previous studies have shown that membrane elevation results in predictable bone formation in the maxillary sinus provided that implants can be placed as tent poles. In situations with an extremely thin residual crest which impairs implant placement, it is possible that a space-making device can be used under the sinus membrane to promote bone formation prior to placement of implants. Purpose: The present study was conducted to test the hypothesis that the use of a space-making device for elevation of the sinus membrane will result in predictable bone formation at the maxillary sinus floor to allow placement of dental implants. Materials and Methods: Eight tufted capuchin primates underwent bilateral sinus membrane elevation surgery, and a bioresorbable space-making device, about 6 mm wide and 6 mm in height, was placed below the elevated membrane on the sinus floor. An oxidized implant (Nobel Biocare AB, Gothenburg, Sweden) was installed in the residual bone protruding into the created space at one side while the other side was left without an implant. Four animals were sacrificed after 6 months of healing. The remaining four animals received a second implant in the side with a space-making device only and followed for another 3 months before sacrifice. Implant stability was assessed through resonance frequency analysis (RFA) using the Osstell™ (Osstell AB, Gothenburg, Sweden) at installation, 6 months and 9 months after the first surgery. The bone-implant contact (BIC) and bone area inside the threads (BA) were histometrically evaluated in ground sections. Results: Histologically there were only minor or no signs of bone formation in the sites with a space-making device only. Sites with simultaneous implant placement showed bone formation along the implant surface. Sites with delayed implant placement showed minor or no bone formation and/or formation of a dense fibrous tissue along the apical part of the implant surface. In the latter group the apical part of the implant was not covered with the membrane but protruded into the sinus cavity. Conclusions: The use of a space-making device, with the design used in the present study, does not result in bone formation at the sinus floor. However, membrane elevation and simultaneous placement of the device and an implant does result in bone formation at the implant surface while sites with implants placed 6 months after membrane elevation show only small amounts of bone formation. Background: The quality of bone is an important factor in the successful implant treatment, and it is evident that higher implant failure is more likely in poor quality of bone. The primary stability of oral implants related to resistance to micromotion during healing is influenced by bone quality, surgical technique, and implant design. Purposes: The aims of this biomechanical study were to explore the effect of bone quality on initial intraosseous stability of implants, and to determine the correlations between the bone quality and implant stability parameters. Materials and Methods: Twenty-four implants (Neoss Ltd., Mölnlycke, Sweden) were placed into anterior and posterior regions of three human cadaver mandibles. The bone densities of implant recipient sites were preoperatively determined using computerized tomography (CT) in Hounsfield unit (HU). The maximum insertion torque values were recorded, and primary implant stability measurements were noninvasively performed by means of resonance frequency analysis (RFA). Results: The bone density values ranged from −267 HU to 553 HU. It was found that mean bone density, insertion torque, and RFA values were 113 ± 270 HU, 41.9 ± 5 Ncm, and 70 ± 7 implant stability quotient (ISQ), respectively. Statistically significant correlations were found between bone density and insertion torque values ( r = 0.690, p < .001); bone density and ISQ values ( r = 0.557, p < .05); and insertion torque and ISQ values ( r = 0.853, p < .001). Conclusion: CT is a useful tool to assess bone quantity and quality in implant recipient sites, and bone density has a prevailing effect on implant stability at placement. [ABSTRACT FROM AUTHOR]

Background: The use of immediate/early implant loading protocols offers obvious advantages for the patient. Although well documented in the totally edentulous mandible, information about clinical outcomes from such protocols in the partially edentate maxilla is lacking. Purpose: The present study was conducted to clinically and radiographically evaluate a tapered implant design with an oxidized surface for immediate/early loading in the partially edentulous maxilla. The aim was also to correlate implant stability measurements using resonance frequency analysis (RFA) with implant diameter and length, bone quality and quantity, and marginal bone levels and marginal bone loss. Materials and Methods: A total of 32 patients with a need of implant treatment in their partially edentulous maxilla were included in the study. A total of 53 Replace Select TiUnite™ implants (Nobel Biocare AB, Göteborg, Sweden) were used in the study; 16 for single tooth replacements in 16 patients and 37 implants for partial bridges in another 16 patients. The single tooth replacements were loaded the same day with a temporary crown, while permanent partial bridges were delivered within 16 days. Intraoral radiographs were taken at surgery and after 1 year for marginal bone measurements. RFA measurements were performed at baseline and after 3, 6, and 12 months. Results: One implant used for a single tooth replacement failed, giving an overall survival rate of 98.1% after 1 year. On average, 1.1 mm (SD 1.0) bone was lost during 1 year; 1.5 mm (SD 1.0) in single tooth and 0.9 mm (SD 1.0) in partial cases. The implant stability increased with time from 63.3 implant stability quotient (ISQ) (SD 6.1) at baseline to 64.3 (SD 5.3), 65.0 (SD 4.6), and 66.8 (SD 5.6) after 3, 6, and 12 months, respectively. The average change from baseline to 1 year was 3.3 ISQ (SD 5.0) and was statistically significant ( p < .05). There was no difference between single and partial cases. Implant stability correlated with bone quantity and quality at implant sites, but not with marginal bone level measurements. Conclusions: It is concluded that immediate/early loading can be used in the partially edentulous maxilla with good clinical and radiographic short-term outcomes. Implant stability at placement correlated with bone quantity and quality, and increased with time as measured with RFA, indicating a favorable bone tissue response to the loaded implants. Background: Clinical studies show promising outcomes with implants inserted at the time of extraction. However, this often results in an initial bone defect at the marginal region which preferably should heal for an optimal function. Therefore, monitoring of these implants is vital. Purposes: The aims of this study were to determine the initial stability of implants placed into fresh extraction sockets, and to explore the correlations between the peri-implant bone levels and implant stability parameters. Materials and Methods: Six human cadaver mandibles including all natural teeth were selected for this study. All natural teeth were gently extracted, and 84 implants were immediately placed into fresh extraction sockets with five different implant depths. The maximum insertion torque values were recorded, and primary implant stability measurements were performed by means of resonance frequency analysis (RFA). The vertical distance between implant/abutment junction and the first bone–implant contact was recorded using a periodontal probe. Results: It was found that the insertion torque and RFA were 28.9 ± 7 Ncm and 65.6 ± 9 implant stability quotient (ISQ), respectively, for 420 measurements from all 84 implants. Statistically significant correlation was found between insertion torque and ISQ values ( r = 0.86; p < .001) for all implants. Both insertion torque and ISQ values dramatically decreased when the amount of peri-implant vertical bone defect increased. Conclusion: The results of this study demonstrated a linear relationship between peri-implant vertical bone defect depth and RFA value. It is proposed that the RFA method is sensitive to detect changes of the marginal bone level and may be used to monitor healing of peri-implant bone defects. [ABSTRACT FROM AUTHOR]
