Fetal magnetic resonance imaging (MRI) showed atrophy and gliosis in the left side of the cerebrum, with sparing of the anteromedial and posteromedial aspects, consistent with prenatal vascular insult primarily involving the left middle cerebral artery (MCA). A 36-year-old patient, gravida 4 para 3 (three living children), presented to our maternal-fetal medicine clinic at 30 + 4 weeks' gestation for evaluation of a cystic area within the left side of the fetal brain.
