Objective: To investigate the effect of office‐based blue laser therapy of vocal fold polyps on voice. Methods: The medical records and video recordings of patients who underwent office‐based laser therapy in a tertiary referral center between February 2020 and May 2020, and May 2021 and October 2022 was conducted. Only patients with vocal fold polyps who had undergone office‐based blue laser therapy were included. The voice was evaluated before and after surgery using the Voice Handicap Index‐10 (VHI‐10), GRB perceptual evaluation, acoustic analysis, and maximum phonation time. Results: A total of 18 patients were included. The mean age of the study group was 52.5 ± 11.94 years. The male‐to‐female ratio was 2:1. Ten patients of the total group (53.6%) had hemorrhagic polyps and the most common site was the mid‐third of the vocal fold. All patients who presented for follow‐up (n = 15) had partial or complete regression of the lesion (4 and 11, respectively). There was a significant decrease in the mean score of VHI‐10 (17.6 ± 9.97 vs. 4.27 ± 5.76, p < 0.001) and in the means of grade of dysphonia (2.0 ± 0.73 to 0.5 ± 0.63, p < 0.001), roughness (1.88 ± 0.81 to 0.44 ± 0.51, p < 0.001) and breathiness (0.81 ± 0.75 to 0.13 ± 0.34 p < 0.001). There was also a marked decrease in the perturbation parameters (jitter and shimmer) and a significant increase in the MPT from 10.66 ± 4.22 s to 14.26 ± 6.26 s (p = 0.028). A 34‐year‐old woman with complaints of mandibular swelling that started 4 months earlier was referred to the Oral Diseases Department. Based on the clinical and radiographic appearance, the primary diagnosis was an intraosseous reactive lesion. However, the result of histopathology indicated osteosarcoma. The diagnosis of osteosarcoma is sometimes challenging due to similarity of its clinical manifestation to those of benign lesions. Background: Medical technology is far from reaching its full potential. An area that is currently expanding is that of precision medicine. The aim of this article is to present an application of precision medicine—a deep‐learning approach to computer‐aided diagnosis in the field of dermatology. Materials and Methods: The main dataset was proposed in the edition of the ISIC Challenge that took place in 2019 and included 25 331 dermoscopic images from eight different categories of lesions—three of them were malignant and five benign. The behavior of the model was also tested on a dataset collected from the second Department of Dermatology, of the Colentina Clinical Hospital. Results: The overall accuracy of the model was 78.11%. Of the total 5031 samples included in the test subset, 3958 were correctly classified. The accuracy of the model on the clinical dataset is lower than that obtained in the first instance. Conclusion: The architecture of the model can be considered of general use, being able to be adapted in an optimal way for a wide range of classifications. Background: The introduction of photodynamic therapy (PDT) in various branches of the dental field such as endodontics, implantology, periodontology, and restorative dentistry and oral medicine has become useful in recent decades. This systematic review presents an overview of the literature to evaluate the usefulness of topical PDT for the treatment of benign oral soft tissue lesions and to identify limitations in prior studies to improve PDT applications. Methods: We performed a review of the literature using different search engines (PubMed, ISI Web of Science, and the Cochrane Library) employing MeSH terms such as "Photodynamic therapy" and "PDT" in conjunction with other terms. We utilized the Population, Intervention, Comparison, Outcomes, and Study design (PICOS) method to define our study eligibility criteria. Results: Initial results were 1513. Finally, there were only 21 studies that met our selection criteria. We divided the 21 selected items into two groups: inflammatory diseases and infective diseases. Conclusions: Although topical PDT is an easy to perform and well‐tolerated treatment and appears to be a valid method with promising results in the treatment of benign lesions of the oral cavity's soft tissues, further studies are needed to complete the current knowledge of this technique. Objectives/Hypothesis: A variety of laryngeal pathologies are increasingly being managed with in‐office KTP laser (IOKTP) endoscopic procedures. The primary goal of this study was to identify patient characteristics and disease‐related features that correlated with tolerance for IOKTP. Study Design: Retrospective chart review. Methods: The study was a retrospective review of adult patients undergoing office‐based laryngeal laser procedures between November 2016 and December 2019 at a tertiary care center. Two blinded otolaryngologist reviewers scored videotaped recordings of IOKTP procedures and assessed severity and distribution of disease using a modified Derkay score, and evaluated procedure tolerance using a visual analog scale. Results: A total of 56 patients who received IOKTP procedures for laryngeal pathology were reviewed, 42 male and 14 female, with a mean age of 61 years. Gender, age, and BMI were not correlated with tolerance. There was a moderate, negative correlation between tolerance and total number of pathological anatomic laryngeal subsites (rs(56) = −0.35, P =.01). There was a weak, negative correlation between tolerance and total modified Derkay score (rs(56) = −0.29, P =.03). The median tolerance score was lower for patients with posterior lesions (Mdn = 6.4) compared with patients with non‐posterior lesions (Mdn = 7.4), P =.04, and lower for current or former smokers (Mdn = 6.5) compared with never smokers (Mdn = 7.3), P =.04. Conclusion: Patients with large disease burden or posterior lesions and patients with smoking history may exhibit poorer tolerance of IOKTP procedures, factors which can help guide pre‐procedural counseling and management decisions. Background: The purpose of this study was to determine the pathological and ultrasound (US) features of benign nonpalpable breast lesions (NPBLs) classified as Breast Imaging Reporting and Data System (BI‐RADS) category 4C or 5. Methods: Between 2003 and 2007, 849 consecutive NPBLs detected at US and classified as BI‐RADS category 4C (505) or 5 (344) initially underwent US‐guided fine needle aspiration (FNA) at our institution. Benign diagnoses were established according to surgical excision findings or during a minimal 6‐month imaging follow‐up (mean, 3.7 years [SD, 2.6 years]). US BI‐RADS features were reviewed and compared retrospectively using a chi‐square test for the following pathological categories: epithelial and fibrous proliferation (EFP), cystic and papillary lesion (C&P), inflammatory lesion (IL), benign tumor (BT), intramammary lymph node (ILN), intraepithelial proliferative lesion (IPL), and nonspecific morphological alteration (NMA). The performance of FNA in the diagnosis of benignity was assessed. Results: Of 849 NPBLs, 110 (12.9%) NPBLs were benign: 88 (17.4%) were BI‐RADS category 4C, and 22 (6.4%) were BI‐RADS category 5. Forty‐four (40%) were EFPs, 21 (19%) were C&Ps, 13 (12%) were NMAs, 11 (10%) were ILs, 11 (10%) were BTs, 8 (7%) were IPLs, and 2 (2%) were ILNs. Lesion shape, US pattern distribution, and posterior features showed statistically significant differences between these categories (P <.05): 33 (75%) EFPs exhibited posterior shadowing, 18 (86%) C&Ps were homogenous, 9 (82%) ILs were heterogeneous, 11 (100%) BTs were homogeneous, 9 (82%) BTs were oval, and 6 (75%) IPLs were irregularly shaped. Of the 110 benign NPBLs, FNA diagnosis was falsely positive in 7 (6%), suspicious in 10 (9%), and benign in 90 (82%), and 3 (3%) were inadequate for diagnosis. Conclusion: A diverse array of benign NPBLs can be classified as BI‐RADS category 4C or 5 on US, each showing specific imaging presentations. Background: Cryotherapy is the deliberate destruction of tissue by the application of extreme cold, and has been indicated as a treatment modality for the management of commonly occurring benign oral pathologies. The aim of this study was to investigate treatment protocol parameters and effectiveness of a closed cryotherapy system for the management of oral mucoceles and vascular malformations.Methods: This retrospective study analyzed data collected from a specialist oral medicine clinic referred for treatment of oral mucoceles or vascular malformations. Data was analyzed to determine lesion characteristics, treatment parameters, and clinical effectiveness of a closed cryotherapy system.Results: The closed cryotherapy system achieved 100% success rate in lesion resolution. For both pathologies, increased lesion diameter was related to increased number of cryotherapy appointments, increased number of freeze cycles, and increased cumulative freeze time. 95% of vascular malformations and 83.34% of mucoceles resolved with no signs of post-operative complication, and patients did not report postoperative pain.Conclusions: Cryotherapy is an excellent treatment option for benign lesion management. The Milan System for Reporting Salivary Gland Cytopathology (MSRSGC) represents a standardized, evidence‐based reporting system for salivary gland fine‐needle aspiration (FNA). The role of FNA is well established for the preoperative evaluation of patients with salivary gland lesions; however, the lack of a uniform system for salivary gland FNA has limited its effectiveness. To address this, an international panel of experienced cytopathologists proposed a uniform reporting system in 2015 under the sponsorship of the American Society of Cytopathology and the International Academy of Cytology. The MSRSGC consists of 6 diagnostic tiers: 1) nondiagnostic, 2) non‐neoplastic, 3) atypia of undetermined significance, 4) neoplasm (subdivided into benign and salivary gland neoplasm of uncertain malignant potential), 5) suspicious for malignancy, and 6) malignant. On the basis of evidence from the literature, each category has a suggested risk of malignancy that ranges from 5% for the neoplasm‐benign category to >90% for the malignant category. The overall goal of the MSRSGC is to improve the effectiveness of salivary FNA by providing a uniform system with the ultimate result of better communication and improved patient care. Background: An international panel of experts in the field of salivary gland cytology (SGC), supported by the American Society of Cytopathology (ASC) and the International Academy of Cytology, conducted a survey to seek evidence and practice patterns regarding SGC. Results were used to provide focus for the proposed Milan System for Reporting Salivary Gland Cytopathology.Methods: The study group, formed during the 2015 European Congress of Cytology held in Milan, Italy, generated a survey that included 49 specific questions related to the taxonomies, practices, and diagnostic entities of salivary cytology. Qualtrics software was used as the study platform. Software and server support were provided by the division of information technology at the University of Wisconsin. The survey was available online from November 2015 until February 2016. Participants were invited through the Web sites of the ASC, the International Academy of Cytology, and the Papanicolaou Society of Cytopathology as well as by the ASC e-mail "ListServe"; responses were evaluated by the Milan System editors.Results: Responses from a total of 515 participants were collected and reviewed. A total of 347 participants provided demographic data information. Responses revealed variations in diagnostic practice and subsequent management. Participants believed that the acceptable rate for nondiagnostic samples should not be higher than 10%. There were varied opinions regarding the approach to neoplastic lesions of uncertain malignant potential, those that may or may have not local invasion and distant spread.Conclusions: Results of the survey demonstrated strong support for the development of a unified system for reporting SGC. Objectives/ Hypothesis: The use of molecular testing is becoming more significant for the diagnosis and classification of disease. The application of fine-needle aspiration (FNA) biopsy as the means of sampling lesions in union with molecular testing could be a powerful combination in laryngology. The objectives of this study were to investigate 1) if FNA was feasible to sample benign vocal fold lesions; 2) if FNA samples provided sufficient RNA quality for molecular analysis; and 3) if gene expression of FNA samples matched paired surgical excised specimens. Study Design: Prospective cross-sectional. Methods: Fifteen vocal fold specimens were obtained from adult patients undergoing routine surgical removal for benign vocal fold lesions using FNA and surgical excision. Comparisons were made between FNA and excision biopsies for RNA quality. Correlative analysis was completed for RNA expression of nine genes, including decorin (DCN), connective tissue growth factor (CTGF), vascular endothelial growth factor (VEGF), collagen type VI alpha 3 (COL6A3), superoxide dismutase 1 (SOD1), glutathione S-transferase (GST2), collagen type I alpha 2 (COL1A2), ATP binding cassette (ABC), and procollagen I alpha 1 (COL1A1). Results: FNA and excision samples demonstrated similar RNA quality ( P > 0.05). Per gene expression, four out of nine genes were moderately correlated between the paired samples ( P < 0.05). Conclusions: FNA of the vocal fold lamina propria is technically feasible to perform. The association of estrogen receptor, progesterone receptor, and human epidermal growth factor receptor 2 (HER2) status of breast cancer patients with total choline (tCho) concentration and tumor volume was investigated using in vivo proton magnetic resonance spectroscopy and MRI at 1.5 T. Values for tCho concentration were determined in 120 locally advanced breast cancer patients (stages IIB, IIIA, IIIB, and IIIC), 31 early breast cancer patients (stage IIA), 38 patients with benign lesions, and 37 controls. Significantly higher tCho concentration and lower tumor volume were observed in early breast cancer patients compared to locally advanced breast cancer patients ( P < 0.05). tCho concentration and tumor volume did not correlate with age and menstruation. tCho cutoff values were obtained for the differentiation of malignant from benign breast tissues (2.54 mmol/kg); malignant versus normal (1.45 mmol/kg) and benign versus normal tissues (0.82 mmol/kg). Estrogen receptor negative patients showed significantly larger tumor volumes, indicating higher angiogenesis with aggressive tumor behavior. Nontriple negative and triple positive patients had a significantly higher tCho concentration compared to triple negative patients ( P < 0.05), indicating complex molecular mechanism of cell proliferation and the molecular heterogeneity of breast lesions. Objective: To assess the incidence of benign renal lesions in our Japanese clinical experience with surgical resection. Methods: A total of 411 renal masses harvested by radical or partial nephrectomy between January 1991 and April 2011 at our institution were retrospectively assessed. The incidence of benign lesions in 1-cm increments in diameter was determined, and a logistic regression model was used to assess relationships between the incidence of benign lesions and other factors. Results: Histological examination confirmed a total of 18 (4.4%) benign lesions. The incidence of benign lesions was 42.8% for nodules <1 cm and 10.0% for nodules 1 to <2 cm. In contrast, the incidence of benign lesions in each 1-cm increment between 2 and 6 cm was 4.1-4.9%. The incidence of benign lesions 2 to <4 cm was 4.8% and of benign nodules ≥6 cm was just 0-1.0%. The incidence of benign lesions ≥2 cm (3.5%) was significantly lower than that of masses <2 cm (16.2%; P < 0.001). Multivariate analysis showed that female gender (odds ratio 3.68) and smaller mass size (<2 cm; odds ratio 4.84) were significant predictors for benign lesions. Conclusions: The incidence of benign lesions among renal masses ≥2 cm in diameter was found to be much lower than previously reported. Objective/Hypothesis: To demonstrate a correlation between the duration and specific pattern of trauma of benign lesions of the vocal folds and their histopathologic appearance. Benign lesions of the vocal folds have various macroscopic appearances. Investigations demonstrate characteristic histopathologic features for three clinically well-defined lesions: 1) vocal fold polyps, 2) Reinke edema, and 3) vocal fold nodules. It is expected that additional histological stainings can contribute to additional insight into the pathophysiology of these lesions. Study Design: Retrospective. Methods: Histological stainings were used to study the constitution of the lamina propria of the mucosa: Verhoeff-van Gieson, Masson trichrome, alcian blue and alcian blue after pretreatment with hyaluronidase. Results: Estimation of the age of a lesion was not possible. Specific observations: 1) accumulation of hyaluronic acid around vessels occurred uniquely in polyps and 2) transverse orientation of elastic fibers was more often seen in vocal fold nodules. Combinations of histopathologic findings were specific to the lesion.
