6. Défaillance rénale chez le patient infecté par le VIH

- Abstract
La défaillance rénale est fréquente chez les patients infectés par le virus de l’immunodéficience humaine (VIH) hospitalisés en réanimation. L’amélioration de l’espérance de vie des malades traités par antirétroviraux (ARV) est associée à une prévalence croissante de l’insuffisance rénale chronique (IRC) dans cette population, compte tenu d’une exposition plus fréquente et plus prolongée aux comorbidités à risque rénal (notamment hépatiques et cardiovasculaires) et de la néphrotoxicité tardive de certains ARV. L’incidence de l’HIV-associated nephropathy(HIVAN) et des autres atteintes rénales directement liées au VIH ou au syndrome d’immunodéficience acquise (SIDA) tend parallèlement à diminuer. Par ailleurs, la survenue d’une insuffisance rénale aiguë (IRA) à l’admission ou au cours du séjour concerne un à deux tiers des patients et augmente significativement le risque de décès en réanimation et d’évolution ultérieure vers l’IRC. Le sepsis et l’administration de médicaments néphrotoxiques sont les principales causes d’IRA dans ce contexte. La ponction-biopsie rénale doit être discutée en cas d’IRA organique sans étiologie confirmée par méthodes non invasives, en particulier lorsque le tableau est évocateur de néphropathie glomérulaire. Enfin, en l’absence de données, la gestion des ARV reste problématique chez les patients de réanimation avec fonction rénale instable: cet aspect essentiel de la prise en charge doit faire l’objet de travaux prospectifs spécifiques. Renal failure is common in human immunodeficiency virus (HIV)-infected patients admitted to the intensive care unit (ICU). Improved life expectancy in patients receiving combination antiretroviral therapy (cART) is associated with an increasing prevalence of chronic kidney diseases (CKD) in this population, as a result of extended exposure to comorbidities that may impair renal function (including hepatic and cardiovascular diseases) and late nephrotoxic effects of certain cART regimen. Meanwhile, a decreasing trend is observed in the incidence of HIV-associated nephropathy (HIVAN) and other kidney diseases directly related to HIV infection or the acquired immunodeficiency syndrome. Acute renal failure (ARF) is present upon admission or occurs during the ICU stay in one to two-thirds of patients, and significantly increases the risk of ICU death and subsequent progression to CKD. Sepsis and exposure to nephrotoxic drugs are the main causes of ARF in this context. Renal biopsy should be discussed in each ARF episode when noninvasive methods failed to identify the underlying nephropathy. Lastly, since published data are dramatically lacking, the management of cART remains challenging in critically ill patients with altered renal function. This crucial aspect of intensive care in HIV-infected patients should be the focus of further prospective works.

ImportanceThe longer-term effects of therapies for the treatment of critically ill patients with COVID-19 are unknown.ObjectiveTo determine the effect of multiple interventions for critically ill adults with COVID-19 on longer-term outcomes.Design, Setting, and ParticipantsPrespecified secondary analysis of an ongoing adaptive platform trial (REMAP-CAP) testing interventions within multiple therapeutic domains in which 4869 critically ill adult patients with COVID-19 were enrolled between March 9, 2020, and June 22, 2021, from 197 sites in 14 countries. The final 180-day follow-up was completed on March 2, 2022.InterventionsPatients were randomized to receive 1 or more interventions within 6 treatment domains: immune modulators (n = 2274), convalescent plasma (n = 2011), antiplatelet therapy (n = 1557), anticoagulation (n = 1033), antivirals (n = 726), and corticosteroids (n = 401).Main Outcomes and MeasuresThe main outcome was survival through day 180, analyzed using a bayesian piecewise exponential model. A hazard ratio (HR) less than 1 represented improved survival (superiority), while an HR greater than 1 represented worsened survival (harm); futility was represented by a relative improvement less than 20% in outcome, shown by an HR greater than 0.83.ResultsAmong 4869 randomized patients (mean age, 59.3 years; 1537 [32.1%] women), 4107 (84.3%) had known vital status and 2590 (63.1%) were alive at day 180. IL-6 receptor antagonists had a greater than 99.9% probability of improving 6-month survival (adjusted HR, 0.74 [95% credible interval {CrI}, 0.61-0.90]) and antiplatelet agents had a 95% probability of improving 6-month survival (adjusted HR, 0.85 [95% CrI, 0.71-1.03]) compared with the control, while the probability of trial-defined statistical futility (HR >0.83) was high for therapeutic anticoagulation (99.9%; HR, 1.13 [95% CrI, 0.93-1.42]), convalescent plasma (99.2%; HR, 0.99 [95% CrI, 0.86-1.14]), and lopinavir-ritonavir (96.6%; HR, 1.06 [95% CrI, 0.82-1.38]) and the probabilities of harm from hydroxychloroquine (96.9%; HR, 1.51 [95% CrI, 0.98-2.29]) and the combination of lopinavir-ritonavir and hydroxychloroquine (96.8%; HR, 1.61 [95% CrI, 0.97-2.67]) were high. The corticosteroid domain was stopped early prior to reaching a predefined statistical trigger; there was a 57.1% to 61.6% probability of improving 6-month survival across varying hydrocortisone dosing strategies.Conclusions and RelevanceAmong critically ill patients with COVID-19 randomized to receive 1 or more therapeutic interventions, treatment with an IL-6 receptor antagonist had a greater than 99.9% probability of improved 180-day mortality compared with patients randomized to the control, and treatment with an antiplatelet had a 95.0% probability of improved 180-day mortality compared with patients randomized to the control. Overall, when considered with previously reported short-term results, the findings indicate that initial in-hospital treatment effects were consistent for most therapies through 6 months.

The origin of the nanoscale emission inhomogeneities of red emitting InGaN/InGaN quantum wells (QWs) grown directly on a GaN template and on an InGaN on sapphire (InGaNOS) substrate is investigated. InGaNOS is a partly relaxed InGaN pseudo-substrate fabricated by Soitec. As the latter approach provides an interesting optical internal quantum efficiency of 6.5% at 624 nm at 290 K, a deeper study, at the microstructure level, was conducted. The emission inhomogeneities on InGaNOS were highlighted by cathodoluminescence wavelength mappings where three areas were chosen: one emitting at a shorter wavelength, i.e., 588 nm, and two at a longer wavelength, i.e., 607 and 611 nm. Specimens from these zones were extracted by focused ion beam milling to perform cross-sectional characterization techniques. High-angle annular dark field scanning transmission electron microscopy images demonstrated that, while red emitting areas present homogeneous QWs, shorter wavelength areas exhibit non-uniform QWs, in terms of thickness and In composition. Complementary deformation mappings in the growth direction obtained by geometrical phase analysis show that longer emission wavelengths are originating from homogeneous QWs with an InN mole fraction evaluated at 39.0 ± 1.5%. This result demonstrates the possibility of achieving red emission with a coherent (In,Ga)N alloy when using an adapted substrate. A comparison of identical QWs grown on a GaN template is also given. The full InGaN structure is used to achieve red light emitting diodes (LEDs). This LED structure is composed of a partly relaxed InGaN pseudo-substrate fabricated by Soitec, namely, InGaNOS, a n-doped buffer layer formed by a set of InxGa1−xN/GaN superlattices, thin InyGa1−yN/InxGa1−xN multiple quantum wells, and a p doped InxGa1−xN area. p-doped InGaN layers are first studied to determine the optimal Mg concentration. In the case of an In content of 2%, an acceptor concentration of 1 × 1019/cm3 was measured for a Mg concentration of 2 × 1019/cm3. Red electroluminescence was then demonstrated for two generations of LEDs, including chip sizes of 300 × 300 μm2 and 50 × 50 μm2. The differences between these two LED generations are detailed. For both devices, red emission with a peak wavelength at 620 nm was observed for a pumping current density of 12 A/cm2. Red light-emission is maintained over the entire tested current range. From the first to the second LED generation, the maximum external quantum efficiency, obtained in the range of 17–40 A/cm2, was increased by almost one order of magnitude (a factor of 9), thanks to the different optimizations. Background: Different field tests are used to evaluate muscle capacity, in particular maximal voluntary isometric endurance. However, although there are some normative values for a few muscle endurance tests, these do not consider the weight, height, gender, or age of individuals, which are well-known factors that influence muscle performance. Hypothesis/Purpose: The purpose of this study was to investigate the test–retest reproducibility of eight field tests and establish muscle endurance norms, in a healthy population, based on their anthropometric characteristics, which could allow the optimal evaluation of the entire muscle function in a quick manner. Design: Case series. Methods: This study was conducted in two phases. The first phase was to check the reproducibility inter- and intra-assessor for eight isometric muscle field tests on 20 volunteer subjects aged 40.9 ± 11.6 years old (age range, 21–58 years). The second part was to establish muscle maximal voluntary isometric endurance norms according to these tests on a total of 400 healthy participants grouped by age (50 males and females in each of the age brackets, 20–29; 30–39; 40–49; 50–59 years old, for a total of 200 males and 200 females). Results: The intra- and inter-assessor reproducibility tests are good for all muscle measurements (the intraclass correlation coefficients varied between 0.915 and 0.996 and the coefficient of variation between 3.6 and 11.8%). The area under the receiver operating characteristic curves demonstrates a good sensibility with values greater than 0.7 for each test. Each muscle belt presents same ratio regardless of the age and gender group. The simultaneous multiple regression analyses highlight that the anthropometric characteristics of subjects influence significantly the performance of isometric tests. Conclusion: This study has permitted establishing prediction equations in a healthy population according to their anthropometric characteristics as well as agonist/antagonist ratios for eight muscle isometric field tests after demonstrating a good reproducibility of all tests. 7;47

Background: Delayed intubation is associated with high mortality. There is a lack of objective criteria to decide the time of intubation. We assessed a recently described combined oxygenation index (ROX index) to predict intubation in immunocompromised patients. The study is a secondary analysis of randomized trials in immunocompromised patients, including all patients who received high-flow nasal cannula (HFNC). The first objective was to evaluate the accuracy of the ROX index to predict intubation for patients with acute respiratory failure. Results: In the study, 302 patients received HFNC. Acute respiratory failure was mostly related to pneumonia (n = 150, 49.7%). Within 2 (1–3) days, 115 (38.1%) patients were intubated. The ICU mortality rate was 27.4% (n = 83). At 6 h, the ROX index was lower for patients who needed intubation compared with those who did not [4.79 (3.69–7.01) vs. 6.10 (4.48–8.68), p < 0.001]. The accuracy of the ROX index to predict intubation was poor [AUC = 0.623 (0.557–0.689)], with low performance using the threshold previously found (4.88). In multivariate analysis, a higher ROX index was still independently associated with a lower intubation rate (OR = 0.89 [0.82–0.96], p = 0.04). Conclusion: A ROX index greater than 4.88 appears to have a poor ability to predict intubation in immunocompromised patients with acute respiratory failure, although it remains highly associated with the risk of intubation and may be useful to stratify such risk in future studies., SCOPUS: ar.j, info:eu-repo/semantics/published

OBJECTIVE: We wished to explore the use, diagnostic capability and outcomes of bronchoscopy added to noninvasive testing in immunocompromised patients. In this setting, an inability to identify the cause of acute hypoxaemic respiratory failure is associated with worse outcome. Every effort should be made to obtain a diagnosis, either with noninvasive testing alone or combined with bronchoscopy. However, our understanding of the risks and benefits of bronchoscopy remains uncertain. PATIENTS AND METHODS: This was a pre-planned secondary analysis of Efraim, a prospective, multinational, observational study of 1611 immunocompromised patients with acute respiratory failure admitted to the intensive care unit (ICU). We compared patients with noninvasive testing only to those who had also received bronchoscopy by bivariate analysis and after propensity score matching. RESULTS: Bronchoscopy was performed in 618 (39%) patients who were more likely to have haematological malignancy and a higher severity of illness score. Bronchoscopy alone achieved a diagnosis in 165 patients (27% adjusted diagnostic yield). Bronchoscopy resulted in a management change in 236 patients (38% therapeutic yield). 49. Rationalizing antimicrobial therapy in the ICU: a narrative review

- Abstract
The massive consumption of antibiotics in the ICU is responsible for substantial ecological side effects that promote the dissemination of multidrug-resistant bacteria (MDRB) in this environment. Strikingly, up to half of ICU patients receiving empirical antibiotic therapy have no definitively confirmed infection, while de-escalation and shortened treatment duration are insufficiently considered in those with documented sepsis, highlighting the potential benefit of implementing antibiotic stewardship programs (ASP) and other quality improvement initiatives. The objective of this narrative review is to summarize the available evidence, emerging options, and unsolved controversies for the optimization of antibiotic therapy in the ICU. Published data notably support the need for better identification of patients at risk of MDRB infection, more accurate diagnostic tools enabling a rule-in/rule-out approach for bacterial sepsis, an individualized reasoning for the selection of single-drug or combination empirical regimen, the use of adequate dosing and administration schemes to ensure the attainment of pharmacokinetics/pharmacodynamics targets, concomitant source control when appropriate, and a systematic reappraisal of initial therapy in an attempt to minimize collateral damage on commensal ecosystems through de-escalation and treatment-shortening whenever conceivable. This narrative review also aims at compiling arguments for the elaboration of actionable ASP in the ICU, including improved patient outcomes and a reduction in antibiotic-related selection pressure that may help to control the dissemination of MDRB in this healthcare setting. © 2019, Springer-Verlag GmbH Germany, part of Springer Nature.
