Abstract

Introduction The global burden of maternal mortality remains high and inequitably distributed between countries. Antenatal care (ANC) was identified as critical component in achieving the Millennium Development Goal of improving maternal health. This scoping review aimed to summarize trends and critically explore research about ANC attendance for East African women conducted during the Millennium Development Goals initiative, with a specific focus on barriers to ANC access. Methods Using a scoping review methodology, aggregator databases were searched for relevant articles. Articles were screened by independent reviewers using a priori inclusion criteria. Eligible articles were retained for data charting and analysis. Results Following screening, 211 articles were analyzed. The number of relevant articles increased over time; utilized primarily quantitative methods; and involved authors with affiliations from various African countries. Many interrelated physical, social, and cultural factors influenced women's seeking, reaching, and receiving of quality ANC. The extent of studies identified suggest that ANC is a priority research area, yet key gaps in the literature exist. Limited qualitative research, and few articles examining ANC experiences of women from vulnerable groups (e.g. adolescents, women with a disability, and Indigenous women) were identified. Discussion These context-specific findings are important considering the Sustainable Development Goals aim to nearly triple the maternal mortality reductions by 2030. In order to achieve this goal, interventions should focus on improving the quality of ANC care and patient-provider interactions. Abstract

We assess whether the household is an apt scale of analysis to examine food insecurity of Indigenous Batwa of Kanungu District, Uganda. Our objectives were: 1) estimate the prevalence and range of food insecurity, 2) estimate the variation in food insecurity associated with household and community factors. The survey was administered 6 times at 3-month intervals (Jan 2013-April 2014). Multilevel modeling was used to determine household and community associations with food insecurity. The Batwa were highly food insecure (97%). Variation in food insecurity that is explained by household and community factors was low. Food insecurity analyses should be considered scale-dependent. Abstract

Climate change is projected to negatively affect human health and nutrition. There is a growing literature on the effects of climatic changes on food availability, quantity, and agricultural production, but impacts on the nutritional content of foods has not been widely studied. The aim of this paper is to systematically characterise empirical literature exploring the effects of climatic drivers on macronutrients and micronutrients in foods causing malnutrition globally. 69 peer-reviewed empirical articles (excluding experimental and modelling studies) analysing the effect of climatic drivers on nutrients in foods were retrieved from Web of Science ^TM , Scopus ^® and PubMed ^® databases (2013–2019). Publication frequency and trends, and existing evidence of the extent of nutrient change associated with variation in climate-related conditions were assessed. There is relatively limited literature on associations between climate and nutrients in foods. Where it exists, only crude proxies of climate (e.g. wet/dry season) are used, with limited interrogation of the potential causal mechanisms linking climate to nutrient content. 98% of the articles showed a change in nutrient content in relation to a seasonal or meteorological variable. Most analysed the association of nutrient changes between seasons over 1–2 years, rarely over longer periods of time. Preliminary descriptive estimates point to variation in nutrient content by meteorological variability, particularly in ocean and freshwater food sources. Robust assessment of potential climate impacts on nutrient content of foods would benefit from more precise estimation of specific causal pathways and variables that mediate climate impacts on food, going beyond seasonal or crude proxies. There is need for clear articulation of how climate change might impact nutrient content given mechanisms linking meteorological and seasonal variation with nutrients. This research highlights emerging evidence that climate change may have impacts beyond agricultural productivity by affecting food nutrient content, an understudied but potentially important pathway for climate impact on global food and nutrition security.

39. Identifying Predictors for Minimum Dietary Diversity and Minimum Meal Frequency in Children Aged 6–23 Months in Uganda.

Abstract

Adequate complementary foods contribute to good health and growth in young children. However, many countries are still off-track in achieving critical complementary feeding indicators, such as minimum meal frequency (MMF), minimum dietary diversity (MDD) and minimum acceptable diet (MAD). In this study, we used the 2016 Ugandan Demographic Health Survey (UDHS) data to assess child feeding practices in young children aged 6–23 months. We assess and describe complementary feeding indicators (MMF, MDD and MAD) for Uganda, considering geographic variation. We construct multivariable logistic regression models—stratified by age—to evaluate four theorized predictors of MMF and MDD: health status, vaccination status, household wealth and female empowerment. Our findings show an improvement of complementary feeding practice indicators in Uganda compared to the past, although the MAD threshold was reached by only 22% of children. Children who did not achieve 1 or more complementary feeding indicators are primarily based in the northern regions of Uganda. Cereals and roots were the foods most consumed daily by young children (80%), while eggs were rarely eaten. Consistent with our hypotheses, we found that health status, vaccination status and wealth were significantly positively associated with MDD and MMF, while female empowerment was not. Improving nutrition in infant and young children is a priority. Urgent nutritional policies and acceptable interventions are needed to guarantee nutritious and age-appropriate complementary foods to each Ugandan child in the first years of life. 40. Indigenous Peoples' Perceptions of Their Food System in the Context of Climate Change: A Case Study of Shawi Men in the Peruvian Amazon.

Abstract

Biodiversity and ecosystem conservation in the Amazon play a critical role in climate-change mitigation. However, institutional responses have had conflicted and complex relations with Indigenous peoples. There is a growing need for meaningful engagement with—and recognition of—the centrality of Indigenous peoples' perceptions and understanding of the changes they are experiencing to inform successful and effective place-based adaptation strategies. To fill this gap, this study focuses on the value-based perspectives and pragmatic decision-making of Shawi Indigenous men in the Peruvian Amazon. We are specifically interested in their perceptions of how their food system is changing, why it is changing, its consequences, and how/whether they are coping with and responding to this change. Our results highlight that Shawi men's agency and conscious envisioning of their future food system intersect with the effects of government policy. Shawi men perceive that the main driver of their food-system changes, i.e., less forest food, is self-driven population growth, leading to emotions of guilt and shame. During our study, they articulated a conscious belief that future generations must transition from forest-based to agricultural foods, emphasising education as central to this transition. Additionally, results suggest that the Peruvian government is indirectly promoting Shawi population growth through policies linking population size to improved service delivery, particularly education. Despite intentional Shawi moves to transition to agriculture, this results in a loss of men's cultural identity and has mental-health implications, creating new vulnerabilities due to increasing climatic extremes, such as flooding and higher temperatures. Abstract

Background: Hospital stays that are prolonged due to non-clinical factors are costly to health care systems and are likely suboptimal for patient well-being. We assessed the influence of psychosocial factors on hospital length of stay (LOS) for older Canadians in a retrospective cohort study. Data and Methods: Data from the Canadian Community Health Survey were linked with the Discharge Abstract Database. Analyses were stratified by age, 55–64 (n = 1,060) and 65 and older (n = 2,718). Main predictor variables of interest included four measures of social support, sense of belonging, and living alone. Multivariate models of LOS adjusted for age, sex, income, smoking, and frailty. Results: Among the younger respondents, low positive social interactions, low emotional/informational support, and living alone were associated with a longer LOS. Among respondents 65 and older, low affection, low positive social interactions, low emotional/informational support, and a weak sense of belonging were associated with a longer LOS. Discussion: Having low social support is associated with longer hospital stays in this Canadian cohort. Social support may influence LOS as risk factors for poor health and precarious care in the community.
