The Early chroNic kiDney disease pOint of caRe Screening (ENDORSE) project aimed to evaluate the clinical and economic implications of a targeted training intervention for general practitioners (GPs) to enhance CKD awareness and early diagnosis.Methods: Data on estimated Glomerular Filtration Rate (eGFR) and Urinary Albumin-Creatinine Ratio (uACR) were collected by 53 Italian GPs from 112,178 patients at baseline and after six months. The intervention involved six months of hybrid training provided by 11 nephrologists, which included formal lectures, instant messaging support, and joint visits for complex cases.Results: The results demonstrated a substantial increase in the use of eGFR (+44.7%) and uACR (+95.2%) tests. This led to a 128.9% rise in the number of individuals screened for CKD using the KDIGO classification, resulting in a 62% increase in CKD diagnoses. The intervention’s impact was particularly notable in high-risk groups, including patients with type 2 diabetes, hypertension, and heart failure.Discussion: A budget impact analysis projected cumulative five-year savings of € 1.7 million for the study cohort. When these findings were extrapolated to the entire Italian CKD population, potential savings were estimated at € 106.6 million, highlighting significant cost savings for the national health service. The clinical simulation assumed that early diagnosed CKD patients would be treated according to current indications for dapagliflozin, which slows disease progression.Conclusion: The ENDORSE model demonstrated that targeted training for GPs can significantly improve early CKD detection, leading to better patient outcomes and considerable economic benefits. This approach shows promise for broader implementation to address the underdiagnosis of CKD on a national and potentially international scale. The present study proposes a fiducial marker for location systems that uses computer vision. The marker employs a set of tape-shaped markers that facilitate their positioning in the environment, allowing continuous reading to cover the entire perimeter of the environment and making it possible to minimize interruptions in the location service. Because the marker is present throughout the perimeter of the environment, it presents hierarchical coding patterns that allow it to be robust against multiple detection scales. We implemented an application to help the user generate the markers with a floor plan image. We conducted two types of tests, one in a 3D simulation environment and one in a real-life environment with a smartphone. The tests made it possible to measure the performance of the tape-shaped marker with readings at multiple distances compared to ArUco, QRCode, and STag with detections at distances of 10 to 0.5 m. The localization tests in the 3D environment analyzed the time of marker detection during the journey from one room to another in positioning conditions (A) with the markers positioned at the baseboard of the wall, (B) with the markers positioned at camera height, and (C) with the marker positioned on the floor. The localization tests in real conditions allowed us to measure the time of detections in favorable conditions of detections, demonstrating that the tape-shaped-marker-detection algorithm is not yet robust against blurring but is robust against lighting variations, difficult angle displays, and partial occlusions. In both test environments, the marker allowed for detection at multiple scales, confirming its functionality.

The importance of continuous cover forestry (CCF) is increasing, yet there is lack of data and understanding about many aspects of this management, including the operational costs. Our objectives were to retrieve available harvesting cost models from published studies on selection cutting in Norway-spruce-dominated stands in Scandinavian countries and to evaluate them against real case studies. First, we retrieved three recently published harvesting cost models which provided explicit cost functions. Models 1 and 2, based on rotation forestry (RF) data and adapted for CCF, had separate sub-models for cutting and hauling costs. Model 3 was based on CCF data and produced total harvesting costs, including the cutting and hauling costs combined. Second, we measured cutting costs for 29 harvesting operations on stands with different stages of CCF structure. We then compared the observations with the simulations of Models 1 and 2 cutting cost sub-models for those cases. Third, we expanded the dataset, including a further 34 harvesting operations in stands with more advanced CCF structures (without measured costs). We then simulated the total harvesting costs for all three models in this dataset to investigate their general behaviour. On average, Models 1 and 2 cutting cost sub-models had relatively good and consistent predictions compared with the observed values. However, they differed in total costs due to different estimates for the hauling cost sub-models. Model 3 had predictions comparable to Models 1 and 2 in the more advanced stages of CCF, but much higher in the less advanced. This study provides important data regarding cutting costs in CCF and demonstrates the feasibility of using existing harvesting cost models.

Renewable bioenergy has the potential to contribute sustainably to the energy sector. Forestry is the main source of biomass for energy in Europe, and poplar (genus Populus) is widely used for short rotation coppice (SRC). Many studies have assessed poplar clones’ productivity but there is a lack of regional studies and links with the climate. We investigated the biomass productivity of twenty hybrid poplar clones for SRC. Clones were planted in sixteen locations across nine countries in Europe, although not all clones were replicated in all locations. In each location, clones were planted in three replicated plots. All plots were harvested after four years, and the aboveground dry biomass estimated. We fitted clone-specific linear mixed models of total aboveground dry biomass production at plot level as function of climatic variables. For many clones (eight) only annual heat moisture deficit negatively affected productivity, in few cases (3) together with a quadratic term for a smoother relationship. In some other clones (five) only the mean summer precipitation positively and linearly affected productivity. On average, the variance explained by the fixed effects in those models was 56%. For the remaining clones (seven), no climate variables resulted significant. Our study explicitly investigated the quantitative link between water availability and poplar SRC productivity, one of the most important known factors but not often studied with a modelling approach. Further, we show the most productive clones in dried conditions. We also highlight the need to larger scale regional experiments to produce models that can be used in climate change scenarios.

The auto-HSCT procedure enables hematological recovery after high-dose chemotherapy, otherwise not tolerable, by the infusion of autologous hematopoietic stem cells. Unlike allogeneic transplant (allo-HSCT), auto-HSCT has the advantage of lacking acute-graft-versus-host disease (GVHD) and prolonged immunosuppression, however, these advantages are counterbalanced by the absence of graft-versus-leukemia. Moreover, in hematological malignancies, the autologous hematopoietic stem cell source may be contaminated by neoplastic cells, leading to disease reappearance. In recent years, allogeneic transplant-related mortality (TRM) has progressively decreased, almost approaching auto-TRM, and many alternative donor sources are available for the majority of patients eligible for transplant procedures. In adults, the role of auto-HSCT compared to conventional chemotherapy (CT) in hematological malignancies has been well defined in many extended randomized trials; however, such trials are lacking in pediatric cohorts. Therefore, the role of auto-HSCT in pediatric oncohematology is limited, in both first- and second-line therapies and still remains to be defined. Nowadays, the accurate stratification in risk groups, according to the biological characteristics of the tumors and therapy response, and the introduction of new biological therapies, have to be taken into account in order to assign auto-HSCT a precise role in the therapeutic strategies, also considering that in the developmental age, auto-HSCT has a clear advantage over allo-HSCT, in terms of late sequelae, such as organ damage and second neoplasms. A novel methodology is introduced to dynamically analyze the complex scaling behavior of financial data across various investment horizons. This approach comprises two steps: (a) the application of a distribution-based method for the estimation of time-varying self-similarity matrices. These matrices consist of entries that represent the scaling parameters relating pairs of distributions of price changes constructed for different temporal scales (or investment horizons); (b) the utilization of information theory, specifically the Normalized Compression Distance, to quantify the relative complexity and ascertain the similarities between pairs of self-similarity matrices. Through this methodology, distinct patterns can be identified and they may delineate the levels and the composition of market liquidity. The soft excess in active galactic nuclei (AGNs) may arise through a combination of relativistic reflection and the effects of a warm corona at the surface of the accretion disc. Detailed examination of the soft excess can therefore constrain models of the transport and dissipation of accretion energy. Here, we analyse 34 XMM–Newton observations from 14 type 1 AGNs with the reXcor spectral model that self-consistently combines emission from a warm corona with relativistic reflection assuming a lamppost corona. The model divides accretion energy between the disc, the warm corona, and the lamppost. The XMM–Newton observations span a factor of 188 in Eddington ratio (λobs) and 350 in black hole mass, and we find that a warm corona is a significant contributor to the soft excess for 13 of the 14 AGNs with a mean warm corona heating fraction of 0.51. The reXcor fits reveal that the fraction of accretion energy dissipated in the lamppost is anticorrelated with λobs. In contrast, the relationship between λobs and both the optical depth and the heating fraction of the warm corona appears to transition from an anticorrelation to a correlation at λobs,t ≈ 0.15. Therefore, at least one other physical process in addition to the accretion rate is needed to explain the evolution of the warm corona. Overall, we find that a warm corona appears to be a crucial depository of accretion energy in AGNs across a broad range of λobs and black hole mass. [ABSTRACT FROM AUTHOR]

Constraining the primary growth channel of supermassive black holes (SMBHs) remains one the most actively debated questions in the context of cosmological structure formation. Owing to the expected connection between SMBH spin parameter evolution and the accretion and merger history of individual black holes, population spin measurements offer a rare observational window into the cosmic growth of SMBHs. As of today, the most common method for estimating SMBH spin relies on modeling the relativistically broaden atomic profiles in the reflection spectrum observed in X-rays. In this paper, we study the observational requirements needed to confidently distinguish between the primary SMBH growth channels based on their distinct spin-mass distributions predicted by the Horizon-AGN cosmological simulation. Indoing so, we characterize outstanding limitations associated with the existing measurements and discuss the landscape of future observational campaigns which could be planned and executed with future X-ray observatories. We focus our attention on the High-Energy X-ray Probe (HEX-P), a proposed probe-class mission designed to serve the high- energy community in the 2030s. [ABSTRACT FROM AUTHOR]

INTRODUCCION: La hiperglucemia constituye un factor pronóstico de morbi-mortalidad. Los pacientes con hiperglucemia tienen mayor riesgo de complicaciones intrahospitalarias tales como neumonía, infección de vías urinarias, enfermedad renal aguda, sepsis, muerte durante la internación o en cortos períodos post alta. Durante la internación se recomienda valores de glucemia entre 140-180 mg/dL. OBJETIVO: Establecer las causas de hiperglucemia en pacientes adultos internados en la Clínica Universitaria Reina Fabiola. MATERIALES Y METODOS: Estudio observacional, retrospectivo y descriptivo. La población incluyó pacientes mayores de 18 años de ambos sexos que ingresaron al internado del Servicio de Clínica Médica de la Clínica Universitaria Reina Fabiola. Se evaluó la glucemia plasmática al ingreso y a las 72 horas de internación, así como la hemoglobina glicosilada (HbA1c) a los pacientes hiperglucémicos que no tenían diagnóstico de diabetes previo al ingreso hospitalario. Se consideró hiperglucemia a valores de glucemia ≥140 mg/dl, se infirió diagnóstico de diabetes mellitus al momento de la internación a la presencia de una HbA1c ≥6.5 % y pacientes con hiperglucemia de estrés a los que tenían HbA1c menor a 6.5 % y alguna causa que lo justifique. Variables analizadas: valor de glucemia, hemoglobina glicosilada, edad en años, sexo, antecedente personales patológicos, incluido diabetes mellitus, motivo de internación, situaciones relacionadas a la internación. Análisis estadístico: los datos se presentaron con estadística descriptiva. RESULTADOS: La muestra final estuvo conformada por un total de n=900 pacientes, el 54% fueron de sexo masculino. La edad media fue de 57,3 años. De los pacientes ingresados, 192 (21.3%) tenían un valor de glucemia mayor o igual a 140 mg/dl, y a las 72 horas este indicador se elevó a 486 (54%) pacientes. Con respecto al sexo, no se hallaron diferencias en las mediciones de la glucosa. La hiperglucemia se presentó en mayor proporción en aquellos pacientes que tuvieron una internación no programada: 156 (81%) vs. 42 (19%). Del total de pacientes, hubo 126 (14%) con antecedente previo de diabetes. Se les midió la hemoglobina glicosilada a 426 (47%) pacientes que habían presentado hiperglucemia al ingreso o a las 72 hs, 72 (16.9%) presentaron una hemoglobina glicosilada >6.5%, a los cuales se los podría definir como pacientes con diabetes mellitus de reciente diagnóstico. Natural regeneration is crucial for silvicultural approaches based on the continuous presence of a forest cover, or Continuous Cover Forestry (CCF). Sitka spruce (Picea sitchensis) is the main commercial species in the United Kingdom (UK), and its potential for CCF has been demonstrated in various studies. However, there are no quantitative models available to predict its natural regeneration in the country. We describe models for Sitka spruce seedlings presence and density under canopy cover in the UK forests, to be used as a substitution of a regeneration survey. Using a natural regeneration dataset comprised of 340 plots, a Generalized Linear Mixed Model (GLMM) was calibrated to estimate the likelihood of regeneration presence at plot level. Seedling density was simulated in a subsequent step using only the subset of data with regeneration presence (138 plots): we compared methods based on GLMMs calibrated to the observed seedling density, and the simple generation of random numbers similar in distribution to the observed values. We validated the models with a cross-validation method using the calibration dataset and with an independent dataset of 78 plots collected in forests already in the process of transformation to CCF. The best GLMM for regeneration presence included age of the plantation, time after last thinning, favourable ground cover and basal area. After the cross-validation, 73% of the plots were correctly estimated (76% for presence of regeneration and 71% for the absence). After the independent validation process, 82% of the plots were correctly estimated, although 100% for presence of regeneration and only 12% for the absence. Both methods for estimating seedling density had a poor performance, both with the cross-validation and independent validation. The results showed that the tools here described are appropriate for estimating regeneration presence in traditional Sitka spruce plantations. However, alternative methods are required for forests already in an advanced stage of transformation to CCF systems.

Tannins have shown antifungal effects and have been considered a potential natural compound for wood preservation. Extracts produced from softwood bark contain both tannins and non-tannin compounds, which may reduce the effectiveness of tannin used as a wood preservative. The purpose of this research is to study the environmental impact of hot water extraction, identify the hot spots within the tannin cradle-to-gate life cycle and give suggestions to optimize its environmental profile. Different extraction and post-extraction scenarios of tannin production are compared using the life-cycle assessment method. Experiments were designed to study the tannin yield under different extraction scenarios; the post-extraction scenario analysis was based on literature review. The results show that the extract drying process is the primary contributor to the environmental impact of tannin production. Both preliminary cold water extraction and ultrafiltration after extraction are beneficial as they have fewer non-tannin compounds in the final products; however, preliminary cold water extraction had a considerably lower environmental performance. Successive extractions using fresh water at each cycle increased the total tannin yield, but increased the environmental burden. Using only evaporation to obtain a desired tannin concentration is not environmentally efficient. This paper provides a quantified environmental analysis for the development of tannin-treated wood products and discusses the different tannin extraction scenarios from an environmental point of view.

We report on an X-ray polarimetric observation of the high-mass X-ray binary LMC X-1 in the high/soft state, obtained by the Imaging X-ray Polarimetry Explorer (IXPE) in 2022 October. The measured polarization is below the minimum detectable polarization of 1.1 per cent (at the 99 per cent confidence level). Simultaneously, the source was observed with the Neutron Star Interior Composition Explorer (NICER), Nuclear Spectroscopic Telescope Array (NuSTAR), and Spectrum-Rontgen-Gamma (SRG)/Astronomical Roentgen Telescope – X-ray Concentrator (ART-XC) instruments, which enabled spectral decomposition into a dominant thermal component and a Comptonized one. The low 2–8 keV polarization of the source did not allow for strong constraints on the black hole spin and inclination of the accretion disc. However, if the orbital inclination of about 36° is assumed, then the upper limit is consistent with predictions for pure thermal emission from geometrically thin and optically thick discs. Assuming the polarization degree of the Comptonization component to be 0, 4, or 10 per cent, and oriented perpendicular to the polarization of the disc emission (in turn assumed to be perpendicular to the large-scale ionization cone orientation detected in the optical band), an upper limit to the polarization of the disc emission of 1.0, 0.9, or 0.9 per cent, respectively, is found (at the 99 per cent confidence level). We present a novel approach to address dust production by low- and intermediate-mass stars. We study the asymptotic giant branch (AGB) phase, during which the formation of dust takes place, from the perspective of post-AGB and planetary nebula (PN) evolutionary stage. Using results from stellar evolution and dust formation modelling, we interpret the spectral energy distribution of carbon-dust-rich sources currently evolving through different evolutionary phases, believed to descend from progenitors of similar mass and chemical composition. Comparing the results of different stages along the AGB to PNe transition, we can provide distinct insights on the amount of dust and gas released during the very late AGB phases. While the post-AGB traces the history of dust production back to the tip of the AGB phase, investigating the PNe is important to reconstruct the mass-loss process experienced after the last thermal pulse. The dust surrounding the post-AGB was formed soon after the tip of the AGB. The PNe dust-to-gas ratio is ∼10−3, 2.5 times smaller than what expected for the same initial mass star during the last AGB interpulse, possibly suggesting that dust might be destroyed during the PN phase. Measuring the amount of dust present in the nebula can constrain the capacity of the dust to survive the central star heating. [ABSTRACT FROM AUTHOR]

We present an X-ray spectro-polarimetric analysis of the bright Seyfert galaxy IC 4329A. The Imaging X-ray Polarimetry Explorer (IXPE) observed the source for ∼500 ks, supported by XMM–Newton (∼60 ks) and NuSTAR (∼80 ks) exposures. We detect polarization in the 2–8 keV band with 2.97σ confidence. We report a polarization degree of 3.3 ± 1.1 per cent and a polarization angle of 78° ± 10° (errors are 1σ confidence). The X-ray polarization is consistent with being aligned with the radio jet, albeit partially due to large uncertainties on the radio position angle. We jointly fit the spectra from the three observatories to constrain the presence of a relativistic reflection component. From this, we obtain constraints on the inclination angle to the inner disc (<39° at 99 per cent confidence) and the disc inner radius (<11 gravitational radii at 99 per cent confidence), although we note that modelling systematics in practice add to the quoted statistical error. Our spectropolarimetric modelling indicates that the 2–8 keV polarization is consistent with being dominated by emission directly observed from the X-ray corona, but the polarization of the reflection component is completely unconstrained. Our constraints on viewer inclination and polarization degree tentatively favour more asymmetric, possibly out-flowing, coronal geometries that produce more highly polarized emission, but the coronal geometry is unconstrained at the 3σ level. We report on the second observation of the radio-quiet active galactic nucleus MCG-05-23-16 performed with the Imaging X-ray Polarimetry Explorer (IXPE). The observation started on 2022 November 6 for a net observing time of 640 ks, and was partly simultaneous with NuSTAR (86 ks). After combining these data with those obtained in the first IXPE pointing on 2022 May (simultaneous with XMM–Newton and NuSTAR) we find a 2–8 keV polarization degree Π = 1.6 ± 0.7 (at 68 per cent confidence level), which corresponds to an upper limit Π = 3.2 per cent (at 99 per cent confidence level). We then compare the polarization results with Monte Carlo simulations obtained with the monk code, with which different coronal geometries have been explored (spherical lamppost, conical, slab, and wedge). Furthermore, the allowed range of inclination angles is found for each geometry. If the best-fitting inclination value from a spectroscopic analysis is considered, a cone-shaped corona along the disc axis is disfavoured. [ABSTRACT FROM AUTHOR]

We present an X-ray spectropolarimetric analysis of the bright Seyfert galaxy NGC 4151. The source has been observed with the Imaging X-ray Polarimetry Explorer (IXPE) for 700 ks, complemented with simultaneous XMM–Newton (50 ks) and NuSTAR (100 ks) pointings. A polarization degree Π = 4.9 ± 1.1 per cent and angle Ψ = 86° ± 7° east of north (68 per cent confidence level) are measured in the 2–8 keV energy range. The spectropolarimetric analysis shows that the polarization could be entirely due to reflection. Given the low reflection flux in the IXPE band, this requires, however, a reflection with a very large (>38 per cent) polarization degree. Assuming more reasonable values, a polarization degree of the hot corona ranging from ∼4 to ∼8 per cent is found. The observed polarization degree excludes a 'spherical' lamppost geometry for the corona, suggesting instead a slab -like geometry, possibly a wedge , as determined via Monte Carlo simulations. This is further confirmed by the X-ray polarization angle, which coincides with the direction of the extended radio emission in this source, supposed to match the disc axis. In this study, we analyzed the value of topoisomerase II alpha (TOP2A) and transducin-like enhancer of split 3 (TLE3) as predictive markers of response to therapy with anthracyclines and taxanes. Materials and methods: TOP2A and TLE3 protein expressions were evaluated using immunohistochemistry on 28 samples, obtained by core needle biopsy in patients with locally advanced breast carcinoma, subsequently subjected to epirubicin- and paclitaxel-based neoadjuvant chemotherapy. The immunohistochemical staining was correlated with the clinical response measured by the tumor size reduction evaluated by breast magnetic resonance imaging, prior and after chemotherapy, and by pathologic evaluation of the surgical specimen. Results: Neoadjuvant chemotherapy achieved a size reduction in 26/28 tumors (92.9%), with an average percentage decrease of 45.6%. A downstaging was achieved in 71.4% of the cases of locally advanced carcinoma. TOP2A positivity was correlated with a greater reduction in tumor diameter (P=0.06); negative staining for TLE3 was predictive of a better response to neoadjuvant chemotherapy (P=0.07). A higher reduction in tumor diameter (P=0.03) was also found for tumors that were concurrently TLE3-negative and TOP2A-positive. Conclusion: TOP2A and TLE3 showed a correlation with response to neoadjuvant chemotherapy. While TOP2A is a well-known marker of response to anthracyclines-based chemotherapy, TLE3 is a new putative predictor of response to taxanes. Data from the current study suggest that TOP2A and TLE3 warrant further investigation in a larger series as predictors of response to neoadjuvant chemotherapy for locally advanced breast carcinoma. The Type 1 active galactic nucleus (AGN) ESO 511-G030, a formerly bright and soft excess dominated source, was observed in 2019 in the context of a multi-wavelength monitoring campaign. In the new exposures, the source was found to be in a flux state approximately ten times lower than archival exposures and without any trace of the soft excess. Interestingly, the X-ray weakening observed in the 2019 data corresponds to a comparable fading of the UV flux, suggesting a strong link between these two components. The UV-X-ray spectral energy distribution (SED) of ESO 511-G030 shows remarkable variability. We tested both phenomenological and physically motivated models on the data, finding that the overall emission spectrum of ESO 511-G030 in this extremely low flux state is due to the superposition of a power-law-like continuum (1:7) and two reflection components emerging from hot and cold matter. Both the primary X-ray continuum and relativistic reflection are produced in the inner regions close to the supermassive black hole. The prominent variability of ESO 511-G030 and the lack of a soft excess can be explained by the dramatic change in the observed accretion rate, which dropped from an L=LEdd of 2% in 2007 to one of 0.2% in 2019. The X-ray photon index also became harder during the low flux observations from 2019, perhaps as a result of a photon starved X-ray corona. [ABSTRACT FROM AUTHOR]

We report spectro-polarimetric results of an observational campaign of the bright neutron star low-mass X-ray binary Cyg X-2 simultaneously observed by IXPE, NICER , and INTEGRAL. Consistently with previous results, the broad-band spectrum is characterized by a lower-energy component, attributed to the accretion disc with kT in ≈ 1 keV, plus unsaturated Comptonization in thermal plasma with temperature kT e = 3 keV and optical depth τ ≈ 4, assuming a slab geometry. We measure the polarization degree in the 2–8 keV band P  = 1.8 ± 0.3 per cent and polarization angle ϕ = 140° ± 4°, consistent with the previous X-ray polarimetric measurements by OSO-8 as well as with the direction of the radio jet which was earlier observed from the source. While polarization of the disc spectral component is poorly constrained with the IXPE data, the Comptonized emission has a polarization degree P  = 4.0 ± 0.7 per cent and a polarization angle aligned with the radio jet. Our results strongly favour a spreading layer at the neutron star surface as the main source of the polarization signal. However, we cannot exclude a significant contribution from reflection off the accretion disc, as indicated by the presence of the iron fluorescence line. [ABSTRACT FROM AUTHOR]
