Abstract

Background: Type 2 diabetes (T2D) is associated with an increased risk of dementia and early features may become evident even in mid-life. Characterizing these early features comprehensively requires multiple measurement modalities and careful selection of participants with and without T2D. Objective: We conducted a cross-sectional multimodal imaging study of T2D-discordant twins in late mid-life to provide insights into underlying mechanisms. Methods: Measurements included computerized cognitive battery, brain MRI (including arterial spin labelling, diffusion tensor, resting state functional), fluorodeoxyglucose (FDG)-PET, and retinal optical coherence tomography. Results: There were 23 pairs, mean age 63.7 (±6.1) years. In global analyses, T2D was associated with poorer attention (β= –0.45, p <0.001) and with reduced FDG uptake (β= –5.04, p = 0.02), but not with cortical thickness (p = 0.71), total brain volume (p = 0.51), fractional anisotropy (p = 0.15), mean diffusivity (p = 0.34), or resting state activity (p = 0.4). Higher FDG uptake was associated with better attention (β= 3.19, p = 0.01) but not with other cognitive domains. In regional analyses, T2D was associated with lower accumbens volume (β= –44, p = 0.0004) which was in turn associated with poorer attention. Conclusion: T2D-related brain dysfunction in mid-life manifests as attentional loss accompanied by evidence of subtle neurodegeneration and global reduction in cerebral metabolism, in the absence of overt cerebrovascular disease. Abstract

This Article argues that the Delaware rule that equity “dilution claims are classically derivative” is not consistent with the Tooley test, which sets forth that whether a claim is direct or derivative “turn[s] solely on” who “suffered the harm” and who “recover[s].” Examining the caselaw behind the “classical[]” rule reveals that the underlying reasoning is a relic from the “special injury” framework that Tooley overruled. It does not comport with Tooley as the company suffers no harm when the company issues stock. That is because stock that the company itself holds is not a company asset, has no economic value, and cannot be voted. Indeed, it is well established in the literature that treasury stock is valueless. Put another way, compare the company and its assets to a pie, of which all stockholders own a slice. When an entity issues new shares for cash, pre-existing stockholders hold a smaller percentage of the pie, but the total size of the pie increases. Stockholders hold the same-sized slice as before so long as equity purchasers paid a fair price. When share purchasers do not pay enough, however, existing stockholders find that their slices of the pie have shrunk, even though the pie itself has not. In fact, the pie is now bigger due to the cash payment. An equity issuance does not transfer value out of the company; it only redistributes existing stockholders’ economic and voting rights in the company to the equity purchasers. The entity is uninjured and only its ownership structure is affected. Rather, it is existing stockholders who pay the bill when the company issues equity, and it is therefore stockholders who are harmed—not the company—when a company dilutes stockholders by overpaying in stock. Abstract

Background. Hospitalised older patients with coronavirus disease 2019 (COVID-19) have an increased risk of poor health outcomes. We aimed to determine the associations of length of stay (LOS) with frailty status and disease severity among older patients admitted to a community treatment facility (CTF) in Singapore. Methods. Medical records of consecutive patients admitted to the CTF were retrospectively reviewed. Data retrieved included demographics, frailty, LOS, 30-day readmission, disease severity, and functional statuses on admission and at discharge. Results. In total, 96 older patients (mean age, 80.3 years) were included. According to the Clinical Frailty Scale (CFS), 26.0% were robust (CFS 1-3), 27.1% were mildly frail (CFS 4-5), and 46.9% were moderately/ severely frail (CFS 6-9). Compared with the robust and mildly frail groups, the moderately/severely frail group had more severe COVID-19 cases (p=0.033), longer LOS at the CTF (p=0.005), and fewer patients discharged home (p=0.01). Greater disease severity was associated with poorer frailty status (p=0.033), worsening of functional status at discharge (p=0.02), higher International Severe Acute Respiratory and emerging Infection Consortium score (p=0.031), and receipt of <2 doses of COVID-19 vaccine (p=0.005). Risk factors for longer LOS at the CTF and overall LOS were basic ADL–assisted status on admission (p=0.09 and p=0.017, respectively), fully dependent status on admission (p=0.043 and p=0.002, respectively), presence of infective changes on chest X-ray (p=0.027 and p=0.003, respectively), and receipt of <2 doses of COVID-19 vaccine (p=0.007 and p=0.005, respectively). Conclusions. Risk factors for longer LOS in older patients with COVID-19 were basic ADL–assisted status and fully dependent status on admission, presence of infective changes on chest X-ray, and receipt of <2 doses of COVID-19 vaccine. High-risk patients should be identified through assessments of these characteristics. Abstract

Microaggressions between medical team members may require difficult conversations to restore inclusivity, diversity, and a healthy work culture. This article outlines important considerations to assist with intentional preparation and modulation of responses for conversation initiators, observers of the incident, and conversation receivers. Microaggressions between members of a team occur often in medicine, even despite good intentions. Such situations call for difficult conversations that restore inclusivity, diversity, and a healthy work culture. These conversations are often hard because of the unique background, experiences, and biases of each person. In medicine, skillful navigation of these interactions is paramount as it influences patient care and the workplace culture. Although much has been published about difficult interactions between providers and patients, significantly less information is available to help navigate provider-to-provider interactions, despite their critical role in improving multidisciplinary patient care teams and organizational environments. This article is intended to serve as a guide for medical professionals who are interested in taking personal responsibility for promoting a safe and inclusive culture by engaging in and modeling difficult conversations with colleagues. The article outlines important considerations to assist with intentional preparation and modulation of responses for all parties involved: conversation initiators, observers of the incident, and conversation receivers. Although these interactions are challenging, together as medical professionals we can approach each other with humility and compassion to achieve our ultimate goal of promoting humanity, not only for our patients but for ourselves and one another.
