In Brazil, research on access to care in the public health system reveals a number of inequalities in care faced by certain patients - particularly women, the working classes and racialized minorities, during medical consultations. The aim here is to highlight a specific form of prejudicial treatment known as epistemic injustice. Based on observations and consultation’s reporting in health posts, emergency departments and hospitalization services in Franca (Brazil), we will see how these injustices are often co-produced by the existence of negative social prejudices against patients, a lack of awareness of social inequalities in health, a more or less authoritarian exercise of power by the doctor and a rigid application of certain medical procedures. Three forms of 'epistemic silencing' (Dotson, 2011) have been identified: 'not believing', 'not saying' and 'silencing'. [ABSTRACT FROM AUTHOR]

This study followed 173 newborn infants in the PREmedication Trial for Tracheal Intubation of the NEOnate multicenter, double-blind, randomized controlled trial of atropine-propofol vs atropine-atracurium-sufentanil for premedication before nonemergency intubation. At 2 years of corrected age, there was no significant difference between the groups in death or risk of neurodevelopmental delay assessed with the Ages and Stages Questionnaire. At present, it is still difficult to distinguish a priori different phenotypes of early-onset asthma in infants, especially to be able to differentiate infants with asthma who are at risk of persistence and severity from those with transient wheezing. It is thus essential to improve the prediction of the prognosis of early-onset asthma because of its high frequency, about 10%, in the pediatric general population. The pulmonary epithelium secretes into the serum a variety of proteins, including some known as pneumoproteins, and these proteins can now be assayed in the serum. Some of them might represent an interesting way in the search of new biomarkers associated with particular asthma phenotypes., Il reste difficile actuellement de distinguer a priori les différents phénotypes d’asthme du nourrisson, et notamment de distinguer l’asthme à risque de persistance et de sévérité de l’asthme transitoire. Il est essentiel d’améliorer la prédiction du pronostic de l’asthme au cours de l’enfance, car l’asthme à début précoce concerne une proportion non négligeable, environ 10 %, de la population générale pédiatrique. L’épithélium pulmonaire sécrète différentes protéines, dont certaines, appelées pneumoprotéines, sont dorénavant dosables dans le sérum. Certaines d’entre elles pourraient constituer une piste intéressante dans la recherche de biomarqueurs associés aux phénotypes de l’asthme.

Objective: 10% of newborns require positive pressure ventilation (PPV) at birth. There are few data on prenatal or early postnatal factors that are predictive of the need for a paediatrician in the delivery room. The study analysed prenatal obstetric and early postnatal factors associated with the requirement for paediatrician assistance in this setting.Methods: Over a three-month period, all consecutive births in a tertiary hospital's maternity unit were prospectively evaluated with regard to the need for paediatrician assistance (requested either before or after the delivery), the requirement for resuscitation, and transfer to a neonatal intensive care unit (NICU).Results: For a total of 584 consecutive births, paediatrician assistance was requested before delivery in 170 cases (30.5%) and after in 78 cases (13.3%). 78% of the newborns requiring PPV, 95.8% of those requiring endotracheal intubation and 86.3% of those requiring transfer to the NICU matched recently published prenatal criteria for paediatrician assistance. Along with a low Apgar score and a cord blood pH <7.20, these criteria covered 95% of the prenatal and early postnatal requests for paediatrician assistance.Conclusions: These criteria for neonatal resuscitation in the delivery room would enable medical staff to anticipate the need for paediatrician assistance. Co-funded policies are proliferating in the public sector, thus creating the need for evaluations in partnership. In this context, evaluation faces specific difficulties: objectives are multiple and ambiguous, indicators may not be perceived as relevant, data collection is complex, and partners' views legitimately differ. The article describes the evaluation of an urban development policy co-decided and co-funded by the French government and the Poitou-Charentes Region. It demonstrates that it was possible to overcome the difficulties of joint evaluations through a series of technical innovations: a concept map structured the evaluation; scoring sheets were used to construct qualitative impact indicators, data about impacts was collected via 20 case studies, and, finally, a multicriteria-multijudge analysis was used to conclude the evaluation while respecting different points of view. A wide-ranging reform of French defence has been in hand since 2007. The White Paper on defence and national security adopted in June 2008 included a redefinition of France's defence and security strategy in line with the new global risks and the creation of armed forces that meet the country's security needs at affordable cost. The Military Programme Bill adopted by the Council of Ministers on 29 October 2008 and due to come before Parliament shortly is a major step towards implementing the White Paper's guidelines. In parallel, reform of the organization of the MOD is currently in hand. Taken together, these measure will consolidate and modernise our defence for the coming decades. [ABSTRACT FROM AUTHOR]
