Abstract

OBJECTIVES: To provide evidence-based recommendations for vaccination against influenza virus and S. pneumoniae in patients with autoimmune rheumatic diseases (ARDs). METHODS: A Consensus Committee including physicians with expertise in rheumatic and infectious diseases was established by two Italian scientific societies, Società Italiana di Reumatologia (SIR) and Società Italiana di Malattie Infettive e Tropicali (SIMIT). The experts were invited to develop evidence-based recommendations concerning vaccinations in ARDs patients, based on their clinical status before and after undergoing immunosuppressive treatments. Key clinical questions were formulated for the systematic literature reviews, based on the clinical pathway. A search was made in Medline (via PubMed) according to the original MeSH strategy from October 2009 and a keyword strategy from January 2016 up to December 2017, updating existing EULAR recommendations. Specific recommendations were separately voted and scored from 0 (no agreement with) to 100 (maximal agreement) and supporting evidence graded. The mean and standard deviation of the scores were calculated to determine the level of agreement among the experts' panel for each recommendation. Total cumulative agreement ≥70 defined consensus for each statement. RESULTS: Nine recommendations, based on 6 key clinical questions addressed by the expert committee, were proposed. The aim of this work is to integrate the 2011 EULAR recommendations on vaccination against influenza and S. pneumoniae in ARDs patients. An implementation plan was proposed to improve the vaccination status of these patients and their safety during immunosuppressive treatments. CONCLUSIONS: Influenza and pneumococcus vaccinations are effective and safe in patients with ARDs. More efforts should be made to translate the accumulated evidence into practice.

Abstract

In psoriatic arthritis (PsA), ultrasonography (US) plays a growing role in the differential diagnosis and in monitoring treatment response.1 PsA is a heterogeneous disease with different domains and peculiar sites involved.2 Therefore, a dedicated US composite score is needed to monitor disease activity and to identify structural damage progression. Abstract

Shift-work disrupts the sleep-wake cycle and could bring about sleep disorders and excessive daytime sleepiness. We studied two samples of shift-workers, a group of 178 nurses and one of 174 police officers, all working in the town of Palermo (Italy); their answers to a sleep disorder questionnaire were scored and added in order to create a Sleep Disorder Score (SDS). The SDS cut-off value, discrimining pathological values from physiological ones, was settled a-priori. In both groups SDS did not depend on sex, age, weight, height nor on working seniority, but it increased non linearly (cubic form) with shift-work seniority. In nurses this mathematical description of SDS exceeded the cut-off value after 15 years of shift-work seniority; in police officers it settled asymptotically under the critical value. This could be ascribable both to the different composition in sex of the two samples (nurses: 49% F-51% M vs. police officers: 6% F-94% M) both to the self-selection process that seems to undergo police officers (nurses do not leave shift-work because of salary incentives).

Abstract

Background. This study aims to evaluate the effects of HYADD 4-G, a not chemically cross-linked HA (Hyaluronic Acid) derivative, and hylan G-F 20, a cross-linked HA product, on synovial matrix metalloproteinases and cytokines in patients suffering from knee osteoarthritis. Methods. 31 patients were randomized to receive HYADD 4-G or hylan G-F 20. Synovial fluid was collected before and one week after the first injection. Activity of MMP-3, MMP-2, MMP-13, IL-6 and other cytokines were measured. Changes in synovial fluid neutrophils and lymphocytes were analyzed. The VAS, the WOMAC questionnaire, and a Physician Global Assessment (PhGA VAS) were recorded. Results. A trend towards a greater decrease in MMP-3 activity was observed in the Hymovis® group. Active MMP-2 and MMP-13 decreased in both groups. IL-6 levels also decreased significantly in both groups. Median change in neutrophils from baseline was significantly different between the treatment groups. No differences between the treatment groups were observed for the WOMAC, VAS and PhGA VAS scores. No serious adverse events were reported. Conclusions. These findings demonstrate that intra-articular HA injections in patients with knee osteoarthritis tend to protect cartilage structural integrity reducing the activity of key proteolytic enzymes implicated in cartilage degradation as well as inflammatory mediators. Abstract

Objectives: This study aimed to determine the prevalence of ultrasound-detected tendon abnormalities in healthy subjects (HS) across the age range., Methods: Adult HS (age 18-80 years) were recruited in 23 international Outcome Measures in Rheumatology ultrasound centres and were clinically assessed to exclude inflammatory diseases or overt osteoarthritis before undergoing a bilateral ultrasound examination of digit flexors (DFs) 1-5 and extensor carpi ulnaris (ECU) tendons to detect the presence of tenosynovial hypertrophy (TSH), tenosynovial power Doppler (TPD) and tenosynovial effusion (TEF), usually considered ultrasound signs of inflammatory diseases. A comparison cohort of patients with rheumatoid arthritis (RA) was taken from the Birmingham Early Arthritis early arthritis inception cohort., Results: 939 HS and 144 patients with RA were included. The majority of HS (85%) had grade 0 for TSH, TPD and TEF in all DF and ECU tendons examined. There was a statistically significant difference in the proportion of TSH and TPD involvement between HS and subjects with RA (HS vs RA p<0.001). 49. [Incidence of occupational accident in Sicilian University Hospital].

Abstract

The evolution of the disease accident has been analysed in a University Hospital in Sicily (approx. 2500 employees/year) for the time period between 2002 and 2006. The industrial injuries index has shown a swing in course (from a value of 3.80% in 2002 to 5.17% in 2003 to 4.49% in 2004), affecting the categories of nurses and health social workers most. The phenomenon shows a constant course within the same period for medical staff, technical assistants and administrative staff. Biological risk industrial injuries have the highest frequency compared to all others (62.5% in 2002 down to 42.3% in 2006). The most interesting datum is the relative scarce entity of the phenomenon when compared to other similar hospital situations and the progressive reduction of biological risk incidents.

Abstract

The AA describe an unusual case of pilonidal sinus in a barber. The clinical analysis showed the presence, in all digital spaces, of erythemateous areas irregularly infiltrated by small fistular tracts and a keratinization area. In the first interdigital space of the left hand, the discharge of a bristle from the apex of a papule was also visibly evident. The histological examination showed the presence of a foreign body granuloma, with giant cells, lymphocyte and plasma cells, centered on a bristle fragment immersed in a collagenous matrix with abundant keratine, skin thickening and fibrous dermis. Using a probe, it was possible to follow the fistular path and obtain the discharge of several hair fragments, conglutinated by corneous scales, but it was not possible to achieve clinical cure of the lesion. The solution to the problem therefore required surgical therapy.
