Abstract

Simple Summary: An inhibitor of BUB1 increases the cytotoxic ability of different classes of chemotherapy, targeted agents, and radiotherapy in triple-negative breast cancer, which is the most difficult subtype of breast cancer to treat due to its lack of expression of targetable genes. The data presented here demonstrate that the cell cycle checkpoint kinase BUB1 can serve as a novel molecular target for the treatment of triple-negative breast cancer and form the basis for the development of rational combinatorial treatment approaches. BUB1 is overexpressed in most human solid cancers, including breast cancer. Higher BUB1 levels are associated with a poor prognosis, especially in patients with triple-negative breast cancer (TNBC). Women with TNBC often develop resistance to chemotherapy and radiotherapy, which are still the mainstay of treatment for TNBC. Our previous studies demonstrated that a BUB1 kinase inhibitor (BAY1816032) reduced tumor cell proliferation and significantly enhanced radiotherapy efficacy in TNBC. In this study, we evaluated the effectiveness of BAY1816032 with a PARP inhibitor (olaparib), platinum agent (cisplatin), and microtubule poison (paclitaxel) alone or in combination with radiotherapy using cytotoxicity and clonogenic survival assays. BUB1 inhibitors sensitized BRCA1/2 wild-type SUM159 and MDA-MB-231 cells to olaparib, cisplatin, and paclitaxel synergistically (combination index; CI < 1). BAY1816032 significantly increased the radiation sensitization of SUM159 and MDA-MB-231 by olaparib, cisplatin, or paclitaxel at non-toxic concentrations (doses well below the IC50 concentrations). Importantly, the small molecular inhibitor of BUB1 synergistically (CI < 1) sensitized the BRCA mutant TNBC cell line HCC1937 to olaparib. Furthermore, the BUB1 inhibitor significantly increased the radiation enhancement ratio (rER) in HCC1937 cells (rER 1.34) compared to either agent alone (BUB1i rER 1.19; PARPi rER 1.04). The data presented here are significant as they provide proof that inhibition of BUB1 kinase activity sensitizes TNBC cell lines to a PARP inhibitor and radiation, irrespective of BRCA1/2 mutation status. Due to the ability of the BUB1 inhibitor to sensitize TNBC to different classes of drugs (platinum, PARPi, microtubule depolarization inhibitors), this work strongly supports the role of BUB1 as a novel molecular target to improve chemoradiation efficacy in TNBC and provides a rationale for the clinical evaluation of BAY1816032 as a chemosensitizer and chemoradiosensitizer in TNBC. Abstract

Simple Summary: Patient-reported outcome (PRO) scores can help to reduce barriers between patient expectations and caregiver perspectives. The relationship between PRO scores and social determinants has not been studied widely. At a single large urban institution, all patients with a new cancer diagnosis who completed a survey over a two-year period were included. The PRO survey recorded perceived outcomes for depression, fatigue, pain interference and physical function. Over the two-year period, 1090 patients were included. Married patients had overall better PRO scores for each domain. Patients who had the ability to use the online portal also had overall better PRO scores. Male patients and minority patients had worse pain scores. PRO scores varied by cancer site and in association with specific social determinants. Future interventions can address patient support by utilizing these findings. Patient-reported outcome (PRO) scores have been utilized more frequently, but the relationship of PRO scores to determinants of health and social inequities has not been widely studied. Our goal was to determine the association of PRO scores with social determinants. All patients with a new cancer diagnosis who completed a PRO survey from 2020 to 2022 were included. The PRO survey recorded scores for depression, fatigue, pain interference and physical function. Higher depression, fatigue and pain scores indicated more distress. Higher physical condition scores indicated improved functionality. A total of 1090 patients were included. Married patients had significantly better individual PRO scores for each domain. Patients who were able to use the online portal to complete their survey also had better individual scores. Male patients and non-White patients had worse pain scores than female and White patients, respectively. Patients with prostate cancer had the best scores while patients with head and neck and lung cancer had the worst scores. PRO scores varied by cancer disease site and stage. Social support may act in combination with specific patient/tumor factors to influence PRO scores. These findings present opportunities to address patient support at institutional levels. Abstract

Objectives: To investigate the impact of reducing Clinical Target Volume (CTV) to Planning Target Volume (PTV) margins on delivered radiation therapy (RT) dose and patient reported quality-of -life (QOL) for patients with localized prostate cancer. Methods: Twenty patients were included in a single institution IRB-approved prospective study. Nine were planned with reduced margins (4 mm at prostate/rectum interface, 5 mm elsewhere), and 11 with standard margins (6/10 mm). Cumulative delivered dose was calculated using deformable dose accumulation. Each daily CBCT dataset was deformed to the planning CT (pCT), dose was computed, and accumulated on the resampled pCT using a parameter-optimized, B-spline algorithm (Elastix, ITK/VTK). EPIC-26 patient reported QOL was prospectively collected pre-treatment, post-treatment, and at 2-, 6-, 12-, 18-, 24-, 36-, 48-, and 60-month follow-ups. Post -RT QOL scores were baseline corrected and standardized to a [0--100] scale using EPIC-26 methodology. Correlations between QOL scores and dosimetric parameters were investigated, and the overall QOL differences between the two groups (QOLMargin-reduced-QOLcontrol) were calculated. Results: The median QOL follow-up length for the 20 patients was 48 months. Difference between delivered dose and planned dose did not reach statistical significance (p > 0.1) for both targets and organs at risk between the two groups.At 4 years post-RT, standardized mean QOLMargin-reduced-QOLcontrol were improved for Urinary Incontinence, Urinary Irritative/Obstructive, Bowel, and Sexual EPIC domains by 3.5, 14.8, 10.2, and 16.1, respectively (higher values better). The control group showed larger PTV/rectum and PTV/bladder intersection volumes (7.2 ± 5.8, 18.2 ± 8.1 cc) than the margin-reduced group (2.6 ± 1.8, 12.5 ± 8.3 cc), though the dose to these intersection volumes did not reach statistical significance (p > 0.1) between the groups. PTV/rectum intersection volume showed a moderate correlation (r = -0.56, p < 0.05) to Bowel EPIC domain. Conclusions: Results of this prospective study showed that margin-reduced group exhibited clinically meaningful improvement of QOL without compromising the target dose coverage. Abstract

Purpose: Deep learning‐based networks have become increasingly popular in the field of medical image segmentation. The purpose of this research was to develop and optimize a new architecture for automatic segmentation of the prostate gland and normal organs in the pelvic, thoracic, and upper gastro‐intestinal (GI) regions. Methods: We developed an architecture which combines a shifted‐window (Swin) transformer with a convolutional U‐Net. The network includes a parallel encoder, a cross‐fusion block, and a CNN‐based decoder to extract local and global information and merge related features on the same scale. A skip connection is applied between the cross‐fusion block and decoder to integrate low‐level semantic features. Attention gates (AGs) are integrated within the CNN to suppress features in image background regions. Our network is termed "SwinAttUNet." We optimized the architecture for automatic image segmentation. Training datasets consisted of planning‐CT datasets from 300 prostate cancer patients from an institutional database and 100 CT datasets from a publicly available dataset (CT‐ORG). Images were linearly interpolated and resampled to a spatial resolution of (1.0 × 1.0× 1.5) mm3. A volume patch (192 × 192 × 96) was used for training and inference, and the dataset was split into training (75%), validation (10%), and test (15%) cohorts. Data augmentation transforms were applied consisting of random flip, rotation, and intensity scaling. The loss function comprised Dice and cross‐entropy equally weighted and summed. We evaluated Dice coefficients (DSC), 95th percentile Hausdorff Distances (HD95), and Average Surface Distances (ASD) between results of our network and ground truth data. Results: SwinAttUNet, DSC values were 86.54 ± 1.21, 94.15 ± 1.17, and 87.15 ± 1.68% and HD95 values were 5.06 ± 1.42, 3.16 ± 0.93, and 5.54 ± 1.63 mm for the prostate, bladder, and rectum, respectively. Respective ASD values were 1.45 ± 0.57, 0.82 ± 0.12, and 1.42 ± 0.38 mm. For the lung, liver, kidneys and pelvic bones, respective DSC values were: 97.90 ± 0.80, 96.16 ± 0.76, 93.74 ± 2.25, and 89.31 ± 3.87%. Respective HD95 values were: 5.13 ± 4.11, 2.73 ± 1.19, 2.29 ± 1.47, and 5.31 ± 1.25 mm. Respective ASD values were: 1.88 ± 1.45, 1.78 ± 1.21, 0.71 ± 0.43, and 1.21 ± 1.11 mm. Our network outperformed several existing deep learning approaches using only attention‐based convolutional or Transformer‐based feature strategies, as detailed in the results section. Conclusions: We have demonstrated that our new architecture combining Transformer‐ and convolution‐based features is able to better learn the local and global context for automatic segmentation of multi‐organ, CT‐based anatomy. Abstract

In a phase I dose escalation and safety study (NCT02555397), a replication-competent oncolytic adenovirus expressing yCD, TK and hIL-12 (Ad5-yCD/mutTKSR39rep-hIL-12) was administered in 15 subjects with localized recurrent prostate cancer (T1c-T2) at increasing doses (1 × 1010, to 1 × 1012 viral particles) followed by 7-day treatment of 5-fluorocytosine (5-FC) and valganciclovir (vGCV). The primary endpoint was toxicity through day 30 while the secondary and exploratory endpoints were quantitation of IL-12, IFNγ, CXCL10 and peripheral blood mononuclear cells (PBMC). The study maximum tolerated dose (MTD) was not reached indicating 1012 viral particles was safe. Total 115 adverse events were observed, most of which (92%) were grade 1/2 that did not require any treatment. Adenoviral DNA was detected only in two patients. Increase in IL-12, IFNγ, and CXCL10 was observed in 57%, 93%, and 79% patients, respectively. Serum cytokines demonstrated viral dose dependency, especially apparent in the highest-dose cohorts. PBMC analysis revealed immune system activation after gene therapy in cohort 5. The PSA doubling time (PSADT) pre and post treatment has a median of 1.55 years vs 1.18 years. This trial confirmed that replication-competent Ad5-IL-12 adenovirus (Ad5-yCD/mutTKSR39rep-hIL-12) was well tolerated when administered locally to prostate tumors. [ABSTRACT FROM AUTHOR]
