Abstract

Although DCL is an extremely rare event and only 5% of AML patients carry a I DDX41 i germline variant, the number of patients identified with a I DDX41 i variant in the context of DCLs is increasing due to comprehensive pathogenetic analyses. Hematopoietic stem cell transplantation (HSCT) represents the standard treatment for patients with high or intermediate-2 risk (IPSS classification) myelodysplastic syndrome (MDS) and acute myeloid leukemia (AML) with intermediate or unfavorable risk (ELN classification) [[1]]. Interestingly, in 1690 screened AML patients, 5% carried I DDX41 i germline variants, and 82% of them showed further I DDX41 i variants as a second somatic aberration supporting the assumption that I DDX41 i loss of function is a main driver of AML [[4]]. Abstract

Objective: ABO mismatch between donor and recipient occurs in 40% of allogeneic hematopoietic stem cell transplantations (HCT). Different strategies have been described to reduce isohemagglutinins (IHA) before HCT. We describe the effect of selective ABO immunoadsorption (ABO IA) on erythrocyte transfusion rate and the development of post‐transplant pure red cell aplasia (ptPRCA). Methods: 63 patients with major ABO incompatibility were retrospectively analyzed. Nine patients with major ABO incompatibility and high‐IHA titer were treated by ABO IA before HCT. We analyzed the need for transfusion and the occurrence of ptPRCA. We compared the outcome with patients treated by other methods to reduce IHA. Results: In all nine patients treated by ABO IA, IHA decreased in a median four times. PtPRCA occurred in one patient. The median number of transfusions was 8 (range: 0‐36) between d0 and d100. In 25 patients with high‐IHA titer without treatment or treated by other methods to reduce IHA, the need for transfusions was comparable. No difference in the incidence of ptPRCA was observed. Conclusions: Selective ABO IA is a feasible, safe, and effective method to reduce IHA before HCT in major ABO incompatibility. Abstract

Background: Nowadays, the collection of PBPCs by apheresis from healthy donors is a routine method. The mobilization with rHu G-CSF and the apheresis procedures are usually well tolerated without severe side effects.Study Design and Methods: We report a severe complication in a 41-year-old unrelated female donor who was allowed to donate PBPCs and was mobilized with 10 microg of G-CSF per kg per day. During PBPC apheresis, she experienced a circulatory arrest after 132 minutes and processing of 7078 mL of blood (twice the donor's blood volume).Results: Immediate cardiopulmonary resuscitation restored sinus rhythm and regulatory respiration without sequelae. Subsequent cardiologic examinations (heart catheterization, electrophysiologic testing, tilting table test) resulted in the diagnosis of a neurocardiogenic syncope. Other cardiac or circulatory disorders could be excluded. The implantation of a cardiac pacemaker was recommended to the donor. The 4-year-old recipient was successfully transplanted with the partial product collected until the arrest occurred. The patient received a total of 2.54 x 106 CD34+ cells per kg of body weight.Conclusion: After exclusion of other cardiac diseases, the diagnosed neurocardiogenic syncope probably induced the circulatory arrest during apheresis rather than the administration of G-CSF. Abstract

Purpose: An escalation in standard irradiation dose ensuring improved local tumor control is estimated, but this strategy would require the exclusion of the most sensitive individuals from treatment. Therefore, fast and reliable assays for prediction of the individual radiosensitivity are urgently required. Methods and Materials: Seven parameters in lymphocytes of 40 patients with leukemia were analyzed before, during, and after total body irradiation (TBI) and in vitro X-ray irradiation. These were: cell proliferation, nuclear damage, activation of cytokines, and numbers of total leukocytes of CD34+ hematopoietic blood stem cells and of CD4+ and CD8+ lymphocytes. Additionally, antioxidative capacity of blood plasma, uric acid, and hemoglobin levels were measured. Blood samples of 67 healthy donors were used as controls. Results: In vivo and in vitro irradiations showed comparable results. A dose–response relationship was found for most parameters. Three parameters were associated with severe acute oral mucositis (Grade 3 or 4 vs. Grade 0 to 2): leukocytes fewer than 6200/μL after 4 Gy TBI, a rate of >19% lymphocytes with reduced DNA and protein content (“necroses”) after 4 Gy in vitro irradiation, and a small antioxidative capacity in blood plasma (<0.68 mMol) after 8 Gy TBI. Conclusion: Three simple blood assays were associated with oral mucositis that are posed here hypothetically as an early symptom of enhanced radiosensitivity in leukemic patients: leukocyte count, damaged lymphocyte score, and the antioxidative capacity after exposure.
