Reflection is an essential part of students' critically reflective development within experiential-learning contexts; it is arguably even more important when working cross-culturally. This paper reports from a national, arts-based service-learning project in which students in creative arts, media and journalism, and preservice teachers worked with Aboriginal people in urban and rural areas of Australia. The paper uses Ryan and Ryan's (2010) "4Rs model of reflective thinking" for reflective learning and assessment in higher education to ascertain the effectiveness of the project work toward engendering a reflective mindset. The paper discusses how students learned to engage in critical self-monitoring as they attended to their learning experiences, and it describes how they "wrote" their experiences and shaped their professional identities as they developed and refined the philosophy that related to their developing careers. Examples taken from the narratives of students, community partners and academic team members illustrate the principal finding, which is that through a process of guided reflection, students learned to reflect in three stages: a preliminary drawing out of existing attitudes and expectations; a midway focus on learning from and relating to past experiences; and a final focus on reciprocal learning, change and future practice. The three stages were apparent regardless of program duration. Thus, program phase rather than academic year level emerged as the most important consideration when designing the supports that promote and scaffold reflection.

Successful House Concerts rely on an equal positive interaction between three groups of people: the musicians (or artists), the audience and the host, with the performance being the central activity. A concert facilitator may be engaged by the host to assist with organisation. Each has a participatory role to ensure that the House Concert performance is a rewarding and ongoing experience. House Concerts encompass a variety of music genres and involve musicians performing solo, in duos and trios and larger ensemble including small operatic casts. House Concerts vary from "ad hoc" to formal with levels of organisation ranging from casual to managed events. Musicians who participate in House Concerts may be acknowledged international performers in Australia and overseas, be starting out in a professional protean career, visiting from overseas in an arranged House Concert tour or amateur/semi professional musicians who have another career. Musicians (artistic companies) with a specific project to develop have used the House Concert concept as their choice of performance space to workshop and develop their ideas. House Concerts encourage the development of a musical community in a way that is different from the concert hall venue. This paper takes a focus on the differing perspectives of the host, audience and musician as they come together for a House Concert performance.

This paper presents the design and application of a major individual assessment task created in part to develop students' capacity for lifelong assessment, a key element of lifelong learning. Additionally the task contributes to fostering a sense of community in asynchronous online learning environments. The task is a supra disciplinary report, recently trialled and now adopted for two postgraduate environmental studies courses offered online. The task design's theoretical underpinnings are reviewed with reference to (i) lifelong assessment, (ii) the value of lifelong assessment from the perspective of environmental studies, and (iii) the importance of fostering community to support learning in online contexts. The paper describes the task's disaggregation into three discrete stages, and the opportunities this provides for fostering community and for supporting students to engage in critical assessment of the quality of their own and their peers' written work, through a transparent, structured process of giving and receiving peer feedback. Purpose: A learning community has been developing in a distributed environment amongst the members of the Centre for Enhancing of Learning and Teaching (CELT) located in the Bathurst, Goulburn and Orange campuses of Charles Sturt University. This group is known by the acronym of GDMOB, with the purpose of the community to facilitate the professional development and learning of its members. To facilitate the learning ICT has been employed to enhance, through an improved sense of community and social presence of the more isolated members of the group, the learning of the members of the community. It is the intent of this paper to explore and identify factors that contribute to the successful implementation and use of ICT to enhance learning and the construction of a sense of community. Design/methodology/approach: The methodology used in this study was that of a case study with a questionnaire that used the reflections of members within the GDMOB and personal reflections of the authors, both of whom are active members of the community. The data were interpreted using an insider's perspective. The reflections of the members of the community were gained through the execution of an anonymous survey, through free form discussion as a collective group, and through observations of the interactions of the group. Findings: Three key factors have been identified in this study that contribute to the successful implementation and use of ICT to enhance learning and the construction of a sense of community; communication, culture and purpose. Research limitations/implications: This research is limited by the small size of the community being investigated, though it is argued that the ideas that emerge can be relevant to larger groups. This aspect needs further investigation. Originality/value: The paper reviews an emerging community of practice and provides reflections on the experience of moving from interactions that were purely face-to-face to a distributed and virtual community environment. Borrowing the title of Raymond Williams' famous study, the following reflections--sometimes collective and sometimes individual--are based on a series of "Keywords", specifically: "fear" "community" and "creativity". By reflecting on the meanings these words have for us today, we attempt to capture their dialogical character, posing them as sites of contestation and struggle, and thereby developing a language of both resistance and hope in the face of neoliberal and neoconservative attacks on education. These reflections continue a series of arguments in defence of the profession first presented in the publication ""Only connect": English teaching, schooling and community" in 2006. Background: Since the early 90s, patient navigation programs were introduced in the United States to address inequitable access to cancer care. Programs have since expanded internationally and in scope. The goals of patient navigation programs are to: a) link patients and families to primary care services, specialist care, and community-based health and social services (CBHSS); b) provide more holistic patient-centred care; and, c) identify and resolve patient barriers to care. This paper fills a gap in knowledge to reveal what is known about motivators and factors influencing implementation and maintenance of patient navigation programs in primary care that link patients to CBHSS. It also reports on outcomes from these studies to help identify gaps in research that can inform future studies.Methods: This scoping literature review involved: i) electronic database searches; ii) a web site search; iii) a search of reference lists from literature reviews; and, iv) author follow up. It included papers from Canada, the United States, the United Kingdom, Australia, New Zealand, and/or Western Europe published between January 1990 and June 2013 if they discussed navigators or navigation programs in primary care settings that linked patients to CBHSS.Results: Of 34 papers, most originated in the United States (n = 29) while the remainder were from the United Kingdom, Canada and Australia. Motivators for initiating navigation programs were to: a) improve delivery of health and social care services; b) support and manage specific health needs or specific population needs, and; c) improve quality of life and wellbeing of patients. Eleven factors were found to influence implementation and maintenance of these patient navigation programs. These factors closely aligned with the Diffusion of Innovation in Service Organizations model, thus providing a theoretical foundation to support them. Various positive outcomes were reported for patients, providers and navigators, as well as the health and social care system, although they need to be considered with caution since the majority of studies were descriptive.Conclusions: This study contributes new knowledge that can inform the initiation and maintenance of primary care patient navigation programs that link patients with CBHSS. In higher education (HE), collaborative activities that revolve around a core idea, practice, or knowledge base, such as learning circles, communities of practice or inquiry, peer observation, and peer-assisted teaching, are known to support professional learning. This paper explores the experiences of eight HE scholars from Australia and New Zealand, across four years, as they recognized a new means of collaborative professional development that transcends known approaches and extends beyond an immediate focal point. The group originally formed to support the development of HE fellowship applications, but evolved to what they now consider a community for practice (CfP), where the purpose of collaboration has changed to meet the unique learning needs of each member. During their four years of collaboration, meeting discussions were recorded, and individual and community narrative reflections about participation in the group were created. A thematic analysis of these collective data sources revealed the group's evolution, and the authors draw on their experiences to tell their story with an aim to enhance professional learning in HE. The study highlights that individuals' distinct and varied needs can be developed and supported through scholarly, collegial engagements such as a CfP, which does not necessarily require an immediate point of practice. Australian regional communities are changing. The combined impact of out-migration and ageing populations means that the capacity of regional communities to function as they traditionally have is challenged. In this context, volunteer effort remains a vital part of building community resilience and social capital. Yet, volunteering per se is under threat, and encouraging young people to volunteer an even greater challenge. This paper presents the results of a project that sought to understand the barriers to, and incentives for, youth volunteering at three regional local government areas in South Australia. First, we find that despite a popular conviction that youth volunteering is on the decline, it has in fact increased; the actual decline is with those volunteers who are within the 35-55-year age groups. Second, we found that two models of volunteering exist in the regions: (1) volunteering as an activity involving participation on committees or doing regular primarily public good group-based work (e.g., emergency services, Rotary, conservation); and (2) event-based, one-off, fun activities (sometimes, but not always, for the broader public good). Volunteering per se, however, was considered by all participants as central to community identity. Culture, sports and youth clubs emerged as important hubs for youth activity and potential volunteer recruitment. We suggest a new model for regional youth volunteering that prioritizes events, partnerships and social media, as well as using existing institutions as bridging organizations. The Nullarbor Plain constitutes one of the main biogeographic barriers of Australia, and it has been suggested to have played a key role in the disjunct distribution of numerous southern Australian species. Although previous research has shown that the origin of this barrier coincides with the timing of the speciation events in some plant lineages, it is not clear whether the uplift of this barrier promoted divergence events in vertebrates. We addressed the role of the Nullarbor barrier and its fringing semiarid habitats as drivers of beta diversity in bird assemblages. Specifically, we determined the effect of distance from the Nullarbor barrier, environmental conditions, and isolation by distance on the composition of local communities on both sides of the plain. We measured beta diversity using taxonomic, phylogenetic, and functional metrics of composition. The influence of precipitation, geographic distance, and distance to the Nullarbor barrier on these metrics was addressed using generalized dissimilarity models and a moving‐window approach. We also tested for differences in local extinction, dispersal and speciation rates, and lineage diversity between two regions, southeastern (SE) and southwestern (SW) Australia. Geological and orogenetic dynamics linked to the appearance of the Nullarbor Plain may have spurred speciation events in SE. However, evidence suggests that subsequent periods in which this region was wetter and forested favored dispersal, mainly from SE to SW. Accordingly, observed dissimilarity in species composition was lower than expected at random, suggesting the existence of considerable turnover between regions. Our results suggest that precipitation deficit (and the xeric vegetation that it promotes) was the most important predictor of beta diversity, whereas the distance to the barrier explained some variation in terms of phylogenetic composition. This study shows that the uplift of the Nullarbor barrier played a minor role in shaping present‐day bird diversity in southern Australia. Most older individuals prefer to age in place during their later years; however, achieving this aspiration presents significant challenges. Naturally Occurring Retirement Communities (NORCs) represent a potential option for promoting healthy aging, both from the perspective of meeting seniors' real needs and cost-effectiveness. This article aims to analyze the distribution of NORCs in Australia and compares census data from 2011 to 2021 to understand the overall distribution patterns and changes across the nation, by providing a localized analysis of the hotspot distribution of NORCs in eight Greater Capital Cities. The study employs methods of geovisualization, Global Moran's I, and Getis-Ord Gi* analysis to examine the spatial correlations and clustering effects of NORCs. The results indicate that NORCs are rapidly growing in Australia, with their distribution primarily influenced by sea change and urbanization. Understanding the trends in NORC distribution can assist the government in developing effective and localized policies and interventions to help older Australians to better age in place. Background: In Australia, dementia is a national health priority. With the rising number of people living with dementia and shortage of formal and informal carers predicted in the near future, developing approaches to coordinating services in quality-focused ways is considered an urgent priority. Key worker support models are one approach that have been used to assist people living with dementia and their caring unit coordinate services and navigate service systems; however, there is limited literature outlining comprehensive frameworks for the implementation of community dementia key worker roles in practice. In this paper an optimised key worker framework for people with dementia, their family and caring unit living in the community is developed and presented.Methods: A number of processes were undertaken to inform the development of a co-designed optimised key worker framework: an expert working and reference group; a systematic review of the literature; and a qualitative evaluation of 14 dementia key worker models operating in Australia involving 14 interviews with organisation managers, 19 with key workers and 15 with people living with dementia and/or their caring unit. Data from the systematic review and evaluation of dementia key worker models were analysed by the researchers and the expert working and reference group using a constant comparative approach to define the essential components of the optimised framework.Results: The developed framework consisted of four main components: overarching philosophies; organisational context; role definition; and key worker competencies. A number of more clearly defined sub-themes sat under each component. Reflected in the framework is the complexity of the dementia journey and the difficulty in trying to develop a 'one size fits all' approach.Conclusions: This co-designed study led to the development of an evidence based framework which outlines a comprehensive synthesis of components viewed as being essential to the implementation of a dementia key worker model of care in the community. The framework was informed and endorsed by people living with dementia and their caring unit, key workers, managers, Australian industry experts, policy makers and researchers. Racism in various forms exists worldwide. In Australia, racism is inextricably linked to the history of Australian immigrants and early setters. Although the Australian education system has adopted inclusive education, evidence shows several incidents of racial exclusion. With the public education system experiencing an increased cultural diversity in student population, schools are required to develop inclusive education policies. While policies related to disability inclusion have been in practice for many years, only recently has there been an increasing awareness of racial inclusion. This research paper explores the importance of racial inclusion in education by examining the causes and effects of racial exclusion in the Australian education context. This paper considers existing practices at the national level and in schools to explore racial discrimination. It identifies the factors contributing towards racism and proposes a framework employing key strategies at the macro, meso and micro levels to achieve racial inclusion in education. Introduction: There have been few controlled evaluations of Social Prescribing (SP), in which link workers support lonely individuals to engage with communitybased social activities. This study reports early outcomes of a trial comparing General Practitioner treatment-as-usual (TAU) with TAU combined with Social Prescribing (SP) in adults experiencing loneliness in Queensland. Methods: Participants were 114 individuals who were non-randomly assigned to one of two conditions (SP, n = 63; TAU, n = 51) and assessed at baseline and 8 weeks, on primary outcomes (loneliness, well-being, health service use in past 2 months) and secondary outcomes (social anxiety, psychological distress, social trust). Results: Retention was high (79.4%) in the SP condition. Time × condition interaction effects were found for loneliness and social trust, with improvement observed only in SP participants over the 8-week period. SP participants reported significant improvement on all other outcomes with small-to-moderate effect sizes (ULS-8 loneliness, wellbeing, psychological distress, social anxiety). However, interaction effects did not reach significance. Discussion: Social prescribing effects were small to moderate at the 8-week follow up. Group-based activities are available in communities across Australia, however, further research using well-matched control samples and longer-term follow ups are required to provide robust evidence to support a wider roll out. Muslims living in non-Muslim countries may experience marginality, which has associations with exclusion, poor socio-emotional health, higher rates of family violence, and poor quality of life. Faith-based strategies have the potential to bridge the gaps and improve the outcomes for these communities. We undertook a reflective evaluation of the individual and group interventions of a Muslim start-up NGO, Community Development, Education and Social Support Inc. (CDESSA) (Adelaide, SA, Australia). Qualitative data were generated via dialogue, storytelling, and making connections with meaning based on observations of the lived experiences of the narrators. The analysis involved revisiting, reordering, refining, and redefining the dialogue, and conscious framing around a theoretical model of community cultural wealth. The results showed the growth of family and community engagement in CDESSA's support and intervention activities, commencing with a small religious following in 2021 and growing to more than 300 Muslims regularly joining together for faith, health, welfare, and social wellbeing activities. Reflections on the dimensions of aspirational, navigational, social, familial, ethnoreligious, and resilient forms of community cultural wealth showed that the range of individual and group interventions, involving religious leaders, contributed to improving health and wellbeing, thereby growing community capital as a mechanism for strengthening families in this community. Technology, new demands from consumers and changing social needs – all the uncontrollable, incredibly fast forces of modern Australia – mean that public libraries need to keep up to avoid going the way of the movie rental store, travel agents, catalogues or even the computer. Can ‘The Library’ be saved or do we need a reinvention that truly reflects their full value in 2015? This paper considers the role of the public library as a community hub, engagement space, and entrepreneurial incubator in the context of the city, city governance, and local government planning. It considers this role from the perspective of library experts and their future visions for libraries in a networked knowledge economy. Public libraries (often operated by or on behalf of local governments) potentially play a pivotal role for local governments in positioning communities within the global digital network. Fourteen qualitative interviews with library experts informed the study which investigates how the relationship between digital technology and the physical library space can potentially support the community to develop innovative, collaborative environments for transitioning to a digital future. The study found that libraries can capitalise on their position as community hubs for two purposes: first, to build vibrant community networks and forge economic links across urban localities; and second, to cross the digital divide and act as places of innovation and lifelong learning. Libraries provide a specific combination of community and technology spaces and have significant tangible connection points in the digital age. The paper further discusses the potential benefits for libraries in using ICT networks and infrastructure, such as the National Broadband Network in Australia. Background: Childhood obesity is a serious public health concern worldwide. Community-based obesity prevention interventions offer promise due to their focus on the broader social, cultural and environmental contexts rather than individual behaviour change and their potential for sustainability and scalability. This paper aims to determine the effectiveness of a South Australian community-based, multi-setting, multi-strategy intervention, OPAL (Obesity Prevention and Lifestyle), in increasing healthy weight prevalence in 9 to 11-year-olds.Methods: A quasi-experimental repeated cross-sectional design was employed. This paper reports on the anthropometric, health-related quality of life (HRQoL) and behaviour outcomes of primary school children (9-11 years) after 2-3 years of intervention delivery. Consenting children from primary schools (20 intervention communities, INT; 20 matched comparison communities, COMP) completed self-report questionnaires on diet, activity and screen time behaviours. HRQoL was measured using the Child Health Utility 9D. Body Mass Index (BMI) z-score and weight status were determined from children's measured height and weight. A multilevel mixed-effects model, accounting for clustering in schools, was implemented to determine intervention effect. Sequential Bonferroni adjustment was used to allow for multiple comparisons of the secondary outcomes.Results: At baseline and final, respectively, 2611 and 1873 children completed questionnaires and 2353 and 1760 had anthropometric measures taken. The prevalence of children with healthy weight did not significantly change over time in INT (OR 1.11, 95%CI 0.92-1.35, p = 0.27) or COMP (OR 0.85, 95%CI 0.68-1.06, p = 0.14). Although changes in the likelihood of obesity, BMI z-score and HRQoL favoured the INT group, the differences were not significant after Bonferroni adjustment. There were also no significant differences between groups at final for behavioural outcomes.Conclusions: OPAL did not have a significant impact on the proportion of 9 to 11-year-olds in the healthy weight range, nor children's BMI z-score, HRQoL and behaviours. Background: The aim of this year-long mixed methods research was to examine the intersection between health, health literacy and local government to identify ways to better connect people to place-based primary health care (PHC). Methods: Four local government areas located within the Perth metropolitan geographic area provided the setting for the current research. Researchers were co-located into the four local governments over a 10-month period to engage with community stakeholders and services. Two methodologies were used to achieve the objective: eight group model building (GMB) workshops were conducted with N = 148 participants to create causal loop diagrams of the barriers and enablers to people being healthy and well in each of the LGAs and develop potential action ideas from these. Surveys were used to collect health service use and health literacy, as measured using a validated Health Literacy Questionnaire (HLQ), across the four LGAs (N = 409, approximately 100 respondents/area). Results: The causal loop diagram themes common across LGAs included: (1) mental health; (2) access to services; (3) health system capacity; (4) economics; and (5) physical wellbeing. Health literacy was relatively high for all nine domains of the HLQ. In the five domains rated from one to four the lowest score was 2.8 for 'appraisal of information' and the highest was 3.2 for 'feeling understood and supported by healthcare providers'. In the four domains rated from one to five; the lowest score was 3.7 for 'navigating the healthcare system' and the highest was 4.1 for 'understand health information well enough to know what to do'. Prioritised action ideas recommended increases in practitioners to meet local needs and training General Practitioners and other health staff in culturally sensitive and trauma informed health care. The survey findings and field notes from the GMB were used to construct personas embodied in vignettes highlighting general themes identified in the workshops including those relevant to local areas. Conclusions: There are many possibilities for health care and local governments to work together to bring services to community members disengaged from the health system. Bringing together people from diverse backgrounds and organisations created synergies that resulted in novel and feasible potential strategies to improve community health. [ABSTRACT FROM AUTHOR]

The progressive democratisation of Australian higher education has numerous causes including the increase in the number of universities and therefore university places across the later 20th and into the 21st century, as well as initiatives by governments of different ideological hues to increase the total percentage of the population with a university degree. This latter impulse, in particular, has increased significantly the number of students entering university via pathway programs, meaning programs which provide the opportunity to undertake study to inculcate university-standard skills and enable matriculation into university for people who are otherwise ineligible. The students in pathway programs are sometimes disengaged from learning, coming from so-called non-traditional pathways that may signify a disconnect between them and their institutions of learning. This practice report, therefore, examines efforts to address "connectedness" in pathway programs at a regional university in Australia. The pathway programs at UniSQ College are underpinned by an inclusive and holistic teaching philosophy that supports students' access to higher education. Through the lenses of social, emotional, cognitive, behavioural, and collaborative engagement, practitioners examined and reflected on ways in which this philosophy was embedded in the day-to-day work of UniSQ College through deliberate efforts to promote and maintain connectedness. By reflecting on and articulating these connections, we have provided a base for developing a future evaluative framework that will critically examine the extent to which our practices, through student engagement, foster connectedness. Background: Significant efforts by governments at a global and national level have not resulted in a significant increase in the duration of breastfeeding to six months. The views of family and social networks, and community attitudes particularly around breastfeeding in public, influence infant feeding decisions. Yet many interventions designed to increase breastfeeding focus on the individual woman and have not been developed from the 'ground up' in consultation with women and communities. This study aimed to identify the key components of Mother Infant Caring Communities that promote and support breastfeeding and early parenting. Methods: Appreciative Inquiry was used to facilitate a 'Community Conversation' workshop in two local councils in Australia. Thirty-five participants attended the community conversation workshops including new parents, grandparents, children's services, local government, and representatives from maternity and child health services. In addition, one focus group discussion was conducted with six retail business owners or managers. Qualitative content analysis was used to analyse data. This paper presents the findings of the first phase (the Discovery phase) of the study. Results: Four major themes emerged: "PLACE – A community for everyone"; "A PLACE for children and families"; "Sometimes a PLACE to breastfeed" and "The parent room: a hidden and unsafe PLACE to breastfeed". Participants described the characteristics of communities that provided a sanctuary and fostered well-being for parents and infants including, open green spaces, safe playgrounds, walking tracks and community hubs. Shopping centres were described as having the potential to be the 'village'. Community-based services to support breastfeeding and parenting were highly valued. Yet in both sites, participants stated that breastfeeding was rarely observed in public and bottle feeding was more evident. Conclusion: Breastfeeding and parenting are embedded in the places where women and families live. Community spaces including shopping centres, should be designed to include infants and young children and offer appropriate facilities such as safe and clean parenting rooms. Health services must work with local government, businesses, and diverse community members to identify what parents' value about their community and design and implement innovative local strategies to support breastfeeding. Museums are judged not solely on the basis of their exhibition quality and collection care but, within a corporate model, they are also judged on quantitative measures such as audience numbers and, in turn, their financial viability. Programming has, therefore, become a major focus of forward planning and the basis for funding development. Blockbuster exhibitions, strategically placed throughout annual programs, have been a common way to increase audience numbers and sustain support. In more recent times, the blockbuster model has developed to address more complex measures of success beyond their quantifiable benefits. In addition to the aim of increasing visitor numbers, the blockbuster exhibition and its associated public and education programs, have been effectively utilized as a means of broadening and diversifying audiences. Such efforts help museums to meet expectations, often set by governments, to address and reflect the diverse demographic communities within which they are situated and to whom they serve. The Art Gallery of New South Wales (AGNSW) in Australia provides one such example of a museum that is working creatively within the blockbuster model in order to present exhibitions that build on their collection strengths and existing programs, attract large audiences and engage diverse audiences by focusing on community building. This paper uses the AGNSW blockbuster exhibition The First Emperor: China's Entombed Warriors, to examine the role of this format in contemporary museums more broadly. We use this exhibition as a frame by which to analyse how the Gallery has modified the blockbuster model, and indeed built on it, in order to target geographically and culturally diverse audiences. We argue that this has been effectively achieved as a result of the Gallery building blockbusters around their curatorial and collection strengths, by working with external organizations and community groups and by offering a range of activities and opportunities for engagement both within the museum and outside of its boundaries. This exhibition proves that when blockbusters are used creatively to support museum wide efforts to engage culturally and linguistically diverse audiences they can achieve success that is both quantitative and qualitative. Age-friendly cities are crucial to achieve the WHO goal of healthy aging. Such cities promote opportunities for health, participation, and security, thus enhancing quality of life as people age. Older people commonly experience psychosocial challenges such as anxiety, depression, substance abuse, loss of autonomy, grief, fear, and loneliness. Australian and Canadian cities continue to seek innovation to improve healthy urban aging and create more age-friendly environments for older adults. There is increasing evidence on the effectiveness and feasibility of mobile technology in health promotion and closing psychological treatment gaps. Older adults have been demonstrated to engage frequently with mobile devices, particularly text messaging. Background: Little is known about the extent, perceptions or experiences of consumers involved in clinical trials across Australia. The purpose of this National study was to better understand the activity and perceptions of clinical trial networks (CTNs), research co-ordinating centres and their consumers, around consumer involvement in clinical trials. Methods: CTNs and research co-ordinating centres who were members of the Australian Clinical Trials Alliance (ACTA), and consumers involved in the activities of these organisations, were invited to participate in online surveys. Surveys were completed between April and September 2018. Results: 80 respondents completed the surveys in full: 25 of 34 CTNs, 5 from 15 research co-ordinating centres, and included research investigators of 13 active trials, 10 completed trials. There were 27 consumer respondents. Consumers were involved in clinical trial activities across 19/25 (76%) of CTNs and 3/5 (60%) of research co-ordinating centres. Consumers were involved at all stages of the trial cycle. Despite this, only 8/30 (27%) of research organisations provided specific training to their employees or members on consumer involvement in research, and most did not have a specific policy or process relating to conducting consumer involvement in clinical trials. At the organisation level, barriers to consumer involvement in clinical trials included being unsure how to involve consumers effectively and systematically, difficulty in accessing consumers or lack of infrastructure and resources. At the consumer level, barriers included limited understanding of their roles, a lack of resources and training. Enablers included education, training and funding for both the research sector and for consumers. Almost all consumer respondents (25/27; 92%) would recommend the consumer role to other potential consumers, stating it was a valuable experience that led to knowledge exchange and learning about the research process. Conclusions: Over the last few years, consumer involvement in clinical trials has increased in Australia, but the scope of involvement varies across different research organisations, and therapeutic areas. Consumer involvement in clinical trials is valued by most that do it, however, there are opportunities to further foster and strengthen ongoing partnerships, for example by providing practical advice for researchers on how to best engage and involve consumers. Plain English summary: Consumer involvement in clinical research (including clinical trials) is considered best practice in Australia, yet it is not yet standard practice. Before doing this research, we did not know the involvement, perceptions and experiences of researchers and consumers within research networks or organisations. Surveys were used to ask Australian Clinical Trials Alliance (ACTA) member organisations including their consumers, about how consumers are being involved in clinical trials. The surveys found that consumers are involved across many clinical trials conducted in Australia, and that their input is valued by researchers and research organisations. Sometimes researchers were uncertain on how to best involve consumers in the different aspects of clinical trials so their contribution was effective and added value. Consumers also expressed at times they were uncertain as to what was expected of them, and in some instances training would have been useful to help them engage more effectively. Support and training also needs to be more appropriate for people from different populations, ethnic backgrounds and cultures, including First Nations peoples. Community and catchment-based approaches to salinity management continue to attract interest in Australia. In one such approach, Catchment Demonstration Initiative (CDI) projects were established by the Western Australian (WA) Government in 2000 for targeted investment in large-scale catchment-based demonstrations of integrated salinity management practices. The aim was to promote a process for technically-informed salinity management by landholders. This paper offers an evaluation of the effectiveness of one CDI project in the central wheatbelt of WA, covering issues including: its role in fostering adoption of salinity management options, the role of research and the technical requirements for design and implementation of on-ground works, the role of monitoring and evaluation, the identification and measurement of public and private benefits, comparison and identification of the place and value of plant-based and engineering-based options, reliance on social processes and impacts of constraints on capacity, management of governance and administration requirements and an appreciation of the value of group-based approaches. A number of factors may reduce the effectiveness of CDI-type approaches in facilitating landholder action to address salinity, many of these are socially-based. Such approaches can create considerable demands on landholders, can be expensive (because of the planning and accountability required) on the basis of dollars per hectare impacted, and can be difficult to garner ownership from all involved. An additional problem could be that few community groups would have the capacity to run such programs and disseminate the new knowledge so that the CDI-type projects can impact outside the focus catchment. In common with many publicly-funded approaches to salinity, we found that direct benefits on public assets are smaller than planned and that results from science-based requirements of monitoring and evaluation have long lead times, causing farmers to either wait for the information or act sooner and take risks based on initial results. We also found that often it is a clear outline of the process that is of most importance in decision making as opposed to the actual results. We identified limitations in regulatory processes and the capacity for local government to engage in the CDI. The opportunities that CDI-type approaches provide centre around the value of its group-based approach. We conclude that they can overcome knowledge constraints in managing salinity by fostering group-based learning, offer a structured process of trialling options so that the costs and benefits can be clearly and transparently quantified, and avoid the costly mistakes and “learning failures” of the past. Wetlands in Australia provide considerable ecological, economic, environmental and social benefits. However, the use of wetlands has been indiscriminate and significant damage to many Australian wetlands has occurred. During the last 150 years one third of the wetlands in Victoria have been lost. A conspicuous problem in wetland management is the paucity of involvement by stakeholders. This paper uses the Analytic Hierarchy Process (AHP) to incorporate stakeholder objectives in the ‘Wonga Wetlands’ on the Murray River. The study shows that the AHP can explicitly incorporate stakeholder preferences and multiple objectives to evaluate management options. The AHP also provides several approaches for policy makers to arrive at policy decisions. [Copyright &y& Elsevier]

Mine closure is an integral part of the lifecycle of a mining project. The closure of a mine has social impacts on the surrounding community and employees who have gradually become dependent on the mine financially, culturally and emotionally. By recognising the consequences of mine closure on local communities, companies respond to the assessment and management of the social impacts. While there are applications of social impact assessments (SIAs) for areas available in different sectors, there are limited practices of SIA for mine closure planning, and there is a lack of information on the role of SIA in mine closure planning. Importantly, there is a dearth of information on how SIA can play an important role to make a mine closure plan by bringing all stakeholders together. This empirical study investigated the contribution of SIA on the development of a coal mine closure plan in regional Queensland in Australia. By integrating social issues and community concerns into the closure planning process, through SIA of the closure planning, the mining company, its employee and the local community were collectively able to formulate the mine closure. This study shows how the SIA can be used to bring relevant stakeholders together to formulate the plan for mine closure and make it acceptable to the stakeholders including company, local communities and employees. Practical policy implications include community engagement through SIA and an assurance of the socioeconomic security of the local community and employees of the mine. It is crucial to undertake SIA at the beginning of the closure planning process and involve the relevant stakeholders to formulate closure plan acceptable to all relevant parties. For the development of a mine closure plan, particular attention is required to address the community's concerns and the development of a solid relationship with the community through negotiations. It is expected that the findings of this study will be useful to researchers, practitioners and other interested persons, not only in Australia but also in other countries with a similar context. This article examines the Australian 'Continental Philosophy' community through the lens of the Azanian philosophical tradition. Specifically, it interrogates the series of conversations around race and methodology that arose from the 2017 Australasian Society for Continental Philosophy (ASCP) conference. At the heart of these were questions of place, race, Indigeneity, and the very meaning of 'Continental Philosophy' in Australia. The pages that follow pursue those questions, grappling with the relationship between the articulation of disciplinary bounds and the exercise of colonial power. Having struggled with the political and existential cost of participation in the epistemic community that is the ASCP, I argue for disengagement and the exploration of alternative intellectual communities. This is ultimately a call to intellectual work grounded on ethical relations rather than on the furtherance of the status quo. It is a call to take seriously the claim, 'the land is ours'. Background Clinical frailty scales (CFS) predict hospital-related outcomes. Frailty is more common in areas of higher socioeconomic disadvantage, but no studies exclusively report on the impact of CFS on hospital-related outcomes in areas of known socioeconomic disadvantage. Aims To evaluate the association of the CFS with hospital-related outcomes. Design Retrospective observational study in a community hospital within a disadvantaged area in Australia (Social Economic Index for Areas = 0.1%). Methods The CFS was used in the emergency department (ED) for people aged ≥ 75 years. Frailty was defined as a score of ≥4. Associations between the CFS and mortality, admission rates, ED presentations and length of stay (LOS) were analysed using regression analyses. Results Between 11 July 2017 and 31 March 2018, there were 5151 ED presentations involving 3258 patients aged ≥ 75 years. Frail persons were significantly more likely to be older, represent to the ED and have delirium compared with non-frail persons. CFS was independently associated with 28-day mortality, with odds of mortality increasing by 1.5 times per unit increase in CFS (95% CI: 1.3–1.7). Frail persons with CFS 4–6 were more likely to be admitted (OR: 1.2; 95% CI: 1.0–1.5), have higher geometric mean LOS (1.43; 95% CI 1.15-1.77 days) and higher rates of ED presentations (IRR: 1.12; 95% CI 1.04–1.21) compared with non-frail persons. Conclusions The CFS predicts community hospital-related outcomes in frail persons within a socioeconomic disadvantage area. Future intervention and allocation of resources could consider focusing on CFS 4–6 as a priority for frail persons within a community hospital setting. [ABSTRACT FROM AUTHOR]

Background: Increasing numbers of older people are receiving home nursing support for medication management to enable them to remain living at home. Home nursing clients frequently experience medication errors and adverse medication events. There has been little study of how medication management processes and interdisciplinary teamwork impact on medication management in the home nursing setting.Objective: To explore medication management processes and describe barriers and challenges from the perspective of community nurses, community pharmacists and GPs involved in the provision of medication management services for home nursing clients.Methods: Focus groups, in-depth interviews and stakeholder consultations were conducted with a convenience sample of community nurses, community pharmacists and GPs. Data were analysed using the framework approach (a deductive thematic analysis) to identify issues affecting the delivery of medication management services.Results: Ten focus groups, 12 in-depth interviews and 5 stakeholder consultation meetings were conducted with 86 health practitioners (55 community nurses, 17 GPs, 10 community pharmacists and 4 nurse managers). Participants highlighted a range of barriers and challenges associated with medication management for home nursing clients, including deficiencies in interdisciplinary communication; problems related to organizational or workplace policies, processes and systems; and ineffective team function. The negative impacts of these were recognized as compromised client safety, reduced workforce efficiency and productivity and compromised interprofessional relationships.Conclusion: Barriers and challenges with medication management for home nursing clients and associated negative impacts were identified. Background and aims Given ongoing community concern about high rates of alcohol-related crimes (ARCs) experienced by disadvantaged populations, a more specific and nuanced understanding of factors associated with ARCs would help inform the development of more sophisticated programs and policies aimed at reducing ARCs. This study estimates rates of ARCs across all communities in New South Wales (NSW), Australia, using routinely collected police data; investigates whether there are differences between communities; and identifies individual and community characteristics that are significantly associated with higher rates of ARCs. Short summary This study analysed routinely collected police data in New South Wales, Australia, to identify individual and community characteristics associated with alcohol-related crimes. Young people, Aboriginal Australians, socio-economically disadvantaged communities, remote and regional communities and communities with higher per capita rate of on-venue liquor licenses are at risk of alcohol-related crimes. Methods Age standardized rates of ARCs were calculated. A multi-level Poisson regression analysis was conducted to investigate the individual and community factors that were statistically significantly associated with higher rates of ARC, separately for Aboriginal and non-Aboriginal Australians. Results Rates of ARCs were statistically significantly higher for Aboriginal Australians, young people (aged 13–37 years) and on weekends. ARCs varied significantly across communities, and were significantly higher in remote or regional communities, in communities with a higher per capita rate of on-venue licences, and for socio-economically disadvantaged communities for non-Aboriginal Australians, but not for Aboriginal females. Conclusion This analysis shows that the impact of national-level and jurisdictional-level legislation and policies is uneven across communities and defined populations, leaving young people, socio-economically disadvantaged communities and Aboriginal Australians at increased risk of ARCs. To more equitably reduce the exposure of all Australians to ARC, mechanisms that effectively engage vulnerable communities and defined populations, need to be developed in consultation with them, implemented and evaluated. Background: Our study explored client experience of Australian Consumer Directed Care. This evolving funding model enables consumer autonomy and choice, allowing older people to remain in their community as they age and need support through the creation of a personalised support service. Consumer Directed Care focuses on providing services that the consumer self-determines to meeting their needs including identifying their types of services, from whom, when and how these services are delivered.Methods: Semi-structured in-depth interviews were conducted in two Australian states between August 2015 and April 2016 with 14 participants, preferably in receipt of CDC services for at least the previous 12 months. Questions explored how the participant first learned about this service; the types of services they received; whether services met their needs; and any additional support services they personally purchased. Interviews were transcribed, coded and thematically analysed.Results: Four main themes related to consumer experience emerged. Knowledge: Unsure what Consumer Directed Care Means. Acceptance: Happily taking any prescriptive service that is offered. Compliance: Unhappily acceding to the prescriptive service that is offered. External Influences: Previous aged care service experience, financial position, and cultural differences.Conclusion: Our results suggest that the anticipated outcomes of Consumer Directed Care providing a better service experience were limited by existing client knowledge of these services, how best to utilise their funding allocation, and their acceptance or compliance with what was offered, even if this was not personalised or sufficient. External influences, such as service experience, finances, cultural difference, impacted the way clients managed their allocation. Our study identified that ongoing engagement and discussion with the client is required to ensure that services are specific, directly relevant and effective to achieving a consumer directed care service. ABSTRACT The aim of this study was to investigate how people self-treat chronic wounds, why they self-treat and the assistance and support that they receive. The increasing emphasis on self-management of chronic conditions, the potential benefits of self-treatment to the health care consumer and competing demands on health care funding are good reasons to investigate self-treatment of chronic wounds as we have little data on this group of individuals. A survey study was conducted in Australia. A non-random sample of 100 participants was recruited. Participants were aged 18 years or older and currently or previously had a chronic wound that they self-treated. All participants completed one survey. Data analysis involved descriptive statistics. The sample was, on average, 64·6 years of age; half was female ( n = 50, 50%), and the majority had a lower leg wound ( n = 80, 80·0%). The sample scored 33·9/40 on the Generalized Self-efficacy Scale and 68/100 on the Medical Outcomes Social Support Scale. The majority of the 89 participants who used a wound dressing used a product that targeted bacteria (n = 59, 66.3%). The two most commonly selected reasons for self-treating were 'to be independent' ( n = 58, 58·0%) and 'to do the treatment at a time that suited' ( n = 55, 56·0%). Less than one quarter of participants reported being supervised regularly during the wound episode ( n = 22, 22%), and few ( n = 6, 6·0%) reported having received education and training to support their self-treatment. Self-treaters of chronic wounds may benefit from standardised education and closer professional supervision to optimise self-treatment practices. Efforts to improve patient satisfaction with professional care are required to promote a shared-care model when self-treating and to optimise patient outcomes. [ABSTRACT FROM AUTHOR]

ABSTRACT Chronic wounds are associated with financial and personal costs. The system level expense associated with chronic wounds has been established, however, the out-of-pocket cost incurred by individuals who self-fund has not been the focus of extensive investigation. Recently, there has been renewed interest in evaluating quality of life, in line with the shift to patient enablement and self-care in chronic disease management. The objectives of this research were to describe the out-of-pocket wound treatment costs and the quality of life of people who have chronic wounds. A questionnaire incorporating the Cardiff Wound Impact Schedule and purpose-designed instruments was completed by a non-probability, convenience sample of 113 people in Australia and Wales. Data was analysed using descriptive statistics. The sample was on average 63·6 years of age and had wounds that were on an average 109 weeks duration. Participants had spent on average AU$2475 on wound dressing products since the wound started, and AU$121·82 in the most recent 28 days which represented 10% of their disposable income. Health-related quality of life was sub-optimal, 6/10 (ave) according to the Cardiff Wound Impact Schedule. Younger participants reported significantly poorer quality of life on all CWIS sub-scales when compared to older participants. This study found that chronic wounds present a significant financial cost to individuals who must self-fund their wound dressings and other wound treatment related expenses. Participants who had access to wound product subsidisation also experienced personal financial costs. People who have chronic wounds experience sub-optimal quality of life therefore this condition is also costly to the individual's well-being. The quality of life of younger people has not received adequate attention and requires further consideration given the many years that younger people may have to live with this debilitating and often recurrent condition. Continued action is required to reduce the financial and personal costs experienced by people who have chronic wounds. It is imperative that healthcare funding is directed to people who have chronic wounds, in particular to alleviate the out-of-pocket costs experienced by self-funders. Background: Community-based walking programs represent a low-cost, accessible approach to increasing physical activity among inactive adults. However, recruiting participants from vulnerable and hard-to-reach groups remains a challenge. This study examined the reach, retention, sociodemographic and health characteristics, physical activity levels and motivators of participants in Heart Foundation Walking, a nationwide Australian community-based walking program. Methods: Descriptive cross-sectional analyses were undertaken with data from 22,416 participants aged 15+ years in the Heart Foundation Walking registration database in December 2015, and from four surveys of participants in 2010 (n = 2400), 2011 (n = 3274), 2012 (n = 4158) and 2015 (n = 1890). Results: Heart Foundation Walking reached participants in every geographic region of Australia, including remote and sparsely populated regions, and engaged sizeable proportions of the following at-risk participants: older than 60 years (>70%); with very low incomes (17-25%); who were overweight or obese (around 60%); and with one or more chronic disease or disease risk factors (57-81%). For all demographic groups, one-year retention rates were at least 75%. Seventy percent of participants met physical activity recommendations. Over 75% reported joining the program for health and fitness reasons while the most cited motivator for continuing was the social aspect (57-73%). Conclusions: Volunteer-run, group-based walking programs can have substantial reach and retention, in particular among those at risk for physical inactivity. The provision of opportunities for social interaction appears to be a key program element in promoting long-term participation, including among high-risk groups. [ABSTRACT FROM AUTHOR]

Background: Researchers are required to seek consent from Indigenous communities prior to conducting research but there is inadequate information about how Indigenous people understand and become fully engaged with this consent process. Few studies evaluate the preference or understanding of the consent process for research with Indigenous populations. Lack of informed consent can impact on research findings.Methods: The Picture Talk Project was initiated with senior Aboriginal leaders of the Fitzroy Valley community situated in the far north of Western Australia. Aboriginal people were interviewed about their understanding and experiences of research and consent processes. Transcripts were analysed using NVivo10 software with an integrated method of inductive and deductive coding and based in grounded theory. Local Aboriginal interpreters validated coding. Major themes were defined and supporting quotes sourced.Results: Interviews with Aboriginal leaders (n = 20) were facilitated by a local Aboriginal Community Navigator who could interpret if necessary and provide cultural guidance. Participants were from all four major local language groups of the Fitzroy Valley; aged 31 years and above; and half were male. Themes emerging from these discussions included Research-finding knowledge; Being respectful of Aboriginal people, Working on country, and Being flexible with time; Working together with good communication; Reciprocity-two-way learning; and Reaching consent.Conclusion: The project revealed how much more there is to be learned about how research with remote Aboriginal communities should be conducted such that it is both culturally respectful and, importantly, meaningful for participants. This study examines Intentional Innovation Communities (IICs) as a strategy for catalyzing community-wide innovation at the grassroots level. IICs are a type of learning and innovation network blending social theories of human interaction with intentional innovation strategies supported in the entrepreneurship literature. IICs are intended to stimulate the development of intentional, community-driven networks for innovation that transcend current institutional and organizational boundaries, with an emphasis on building both democracy and strategy. The IIC model is designed to be broadly replicable. This study presents IICs conceptually and discusses key findings from pilot research that support their design. Then, case studies are presented from five communities in rural Australia, where IICs were used to form nascent network relationships and start meaningful discussions about launching community-driven entrepreneurship and innovation projects. Background: Research on workforce development for high‐quality dementia care more often focuses on enhancing employee knowledge and skill and less on managing employee stress and coping at work. Objective: To review employee stress and coping in response to high job demands in community‐based dementia care organizations in Tasmania, Australia. Methods: Stress and coping in response to job roles of 25 community‐based dementia care workers were reviewed using self‐report questionnaire data. Data were analysed for descriptive results and at an individual case level. Individual participant scores were reviewed for clinically significant stress and coping factors to create worker profiles of adjustment. Results: Two adjustment profiles were found. The ‘global resilience’ profile, where workers showed positive adjustment and resilience indicating they found their jobs highly rewarding, were very confident in their abilities at work and had a strong match between their personal and organizational values. The second ‘isolated distress’ profile was only found in a minority and included poor opportunities for job advancement, a missmatch in personal and work values or clinically high levels of psychological distress. Conclusion: Aged care workplaces that advocate employee well‐being and support employees to cope with their work roles may be more likely to retain motivated and committed staff. Future research should consider employee stress and coping at the workforce level, and how this can influence high‐quality care delivery by applying the measures identified for this study. Background In Australia, maternity care is available through universal coverage and a parallel, competitive private health insurance system. Differences between sectors in antenatal and intrapartum care and associated outcomes are well documented but few studies have investigated differences in postpartum care following hospital discharge and their impact on maternal satisfaction and confidence. Methods Women who birthed in Queensland, Australia from February to May 2010 were mailed a self-report survey 4 months postpartum. Regression analysis was used to determine associations between sector of birth and postpartum care, and whether postpartum care experiences explained sector differences in postpartum well-being (satisfaction, parenting confidence and feeling depressed). Results Women who birthed in the public sector had higher odds of health professional contact in the first 10 days post-discharge and satisfaction with the amount of postpartum care. After adjusting for demographic and postpartum contact variables, sector of birth no longer had an impact on satisfaction (AOR 0.95, 99% CI 0.78-1.31), but any form of health professional contact did. Women who had a care provider's 24 hour contact details had higher odds of being satisfied (AOR 3.64, 95% CI 3.00-4.42) and confident (AOR 1.34, 95% CI 1.08- 1.65). Conclusion Women who birthed in the public sector appeared more satisfied because they had higher odds of receiving contact from a health professional within 10 days post-discharge. All women should have an opportunity to speak to and/or see a doctor, midwife or nurse in the first 10 days at home, and the details of a person they can contact 24 hours a day. We speak often of ‘representative democracy’ and we tend to regard it as the dominant form of government in the modern West. But who or what is represented by the electoral systems of the modern democratic state? Although we may be tempted to regard contemporary democracies as embodying essentially the same underlying conceptions of representation, a careful examination of constitutional texts, legislative frameworks, and judicial interpretations reveals important differences from one country to the next. In particular, the problem of electoral apportionment has produced a line of constitutional cases in the United States, Canada, and Australia that raise very similar sets of issues, but with strikingly different results. This article compares the case law in the United States, Canada, and Australia on the question of electoral apportionment. While the ideas appealed to in each country are complex and contain sometimes overlapping elements, it is argued that they can be classified into three general types, which the article calls ‘individualist,’ ‘communitarian,’ and ‘federalist.’ Each approach presupposes a distinct conception of the political identity of the human person and has distinct implications for design of a democratic electoral system. And, while each conception finds a place in each of the three countries under examination, there is an important sense in which individualist conceptions have dominated much of the American jurisprudence, communal approaches have influenced most of the Canadian case law, and federalist conceptions have shaped the Australian cases at several decisive points. The article concludes by arguing that, although factors such as modalities of interpretation and conceptions of the relationship between the courts and the political branches of government have been important, significantly different underlying conceptions of democracy and of the political identity of the human person have been ultimately determinative of the results. Many environmental science research programmes now adopt community-based philosophies and designs, although there are few applications in Australian Indigenous communities. This research describes the development and testing of a framework of engagement to guide collaboration between Indigenous and non-Indigenous Australians during an environmental sciences research project. That project aimed to assess trepang (sea cucumber) stocks in Aboriginal waters, and determine the commercial harvest potential of the resource to local people. Its objectives were to undertake trepang survey and mapping, integrate Indigenous knowledge about the resource, and model the existing catch data of commercial fishers operating in the region. The framework of engagement developed to guide the research process comprised a goal, research stages, and a number of guiding principles for collaboration, which were constructed from content analysis of available guidelines and literature and from data gathered during expert interviews. Further data were gathered using participant observation, while implementing the trepang research in accordance with this framework of engagement, and these data were analysed to test and evaluate the framework. Findings indicate discursive and reflective approaches such as action research or adaptive management may better facilitate equitable research partnerships for sustainable development. The general perceptions of prescribed burning were elicited from forest users for an area that has been subject to this form of land management for at least 20 years. The largest group consisted of local residents living in and around the Wombat State Forest with two smaller groups of students from a nearby university campus and local professional land managers. A questionnaire was given to each participant in order to explore how the forest was used, to determine the level of knowledge of burning in the targeted forest and Victoria and the perception of the appearance, effectiveness of protection, and accessibility to the forest after prescribed burning. Generally all groups had similar responses with community members having stronger views on the effectiveness and practicalities of prescribed burning, whereas students were more neutral in their opinions. All participants claimed knowledge of prescribed burning activities within Victoria, but fewer had experience of planned fires in the Wombat State Forest. All groups agreed that areas that had not been recently burned had a better appearance than those that had, but this result may have included a range of value judgments. Land managers had a greater understanding of the ecological importance of season and timing of burning; however, some students and community members were equally knowledgeable. Prescribed burning did not impede access to the forest, nor did smoke from prescribed burns pose any great problem. The majority of the participants felt that the amount of prescribed burning done in the forest was adequate for engendering a feeling of protection to life and property, yet many were still suspicious of this management practice. These initial findings indicate several areas in which further research would be useful including the efficacy of education programs for community members and improved communication of burn plans by land managers. Summary • The contribution of arbuscular mycorrhizas (AM) to plant community structure and diversity is reported here in an annual herbland in southern Australia. • Mycorrhizal colonization was reduced in field plots by applying the fungicide benomyl as a soil drench. The mycorrhiza-responsiveness of plant species was assessed in intact soil cores containing the indigenous AM fungi and in a pot experiment using an isolate of Glomus mosseae . • Glasshouse experiments showed that Medicago minima, Vittadinia gracilis and Velleia arguta were highly mycorrhiza-responsive, Salvia verbenaca became colonized but exhibited no growth response to AM, and Carrichtera annua remained uncolonized. There was no change in plant species richness in mycorrhiza-suppressed field plots but diversity increased owing to an increase in evenness. Treatment had no effect on community productivity and therefore there was no relationship between mycorrhizal effects on diversity and productivity. • Mycorrhizal responsiveness was not a good predictor of species response to suppression of AM in the field. The mycorrhiza-responsive species V. gracilis and V. arguta were not affected by reduced mycorrhizal colonization in fungicide-treated plots, suggesting that competition from the mycorrhiza-responsive dominant M. minima offset the benefits of mycorrhizal association for these species. Background: Advanced care planning (ACP) is a process that involves thinking about what medical care one would like should individuals be seriously ill and cannot communicate decisions about treatment for themselves. The literature indicates that ACP leads to increased satisfaction from both patients and healthcare professionals. Despite the well-known benefits of ACP, it is still underutilised in Australia.Methods: The aim of this study is to investigate the effects of normalising ACP in acute and community settings with the use of specially trained normalisation agents. This is a quasi-experimental study, involving 16 sites (8 intervention and 8 control) in two health districts in Australia. A minimum of total 288 participants will be recruited (144 intervention, 144 control). We will train four registered nurses as normalisation agents in the intervention sites, who will promote and facilitate ACP discussions with adult patients with chronic conditions in hospital and community settings. An audit of the prevalence of ACP and Advanced Care Directives (ACDs) will be conducted before and after the 6-month intervention period at the 16 sites to assess the effects of the ACP service delivered by these agents. We will also collect interview and survey data from patients and families who participate, and healthcare professionals who are involved in this service to capture their experiences with ACP.Discussion: This study will potentially contribute to better patient outcomes with their health care services. Completion of ACDs will allow patients to express their wishes for care and receive the care that they wish for, as well as ease their family from the burden of making difficult decisions. The study will contribute to development of a new best practice model to normalise ACP that is sustainable and transferable in the processes of: 1) initiation of conversation; 2) discussion of important issues; 3) documentation of the wishes; 4) storage of the documented wishes; and 5) access and execution of the documented wishes. The study will generate new evidence on the challenges, strategies and benefits of normalising ACP into practice in acute and community settings.Trial Registration: This project has been approved by the Hunter New England Human Research Ethics Committee (Approval No. 17/12/13/4.16). Background: This is the first review of literature and synthesis of data on community onset methicillin resistant Staphylococcus aureus (CO-MRSA) infections in Australia. Incidence of CO-MRSA varies considerably in Australia, depending on geographic and demographic factors. Methods: Data for the rates of MRSA infections were collected from articles identified using PubMed, Scopus, the grey literature and data from State and Federal Government Surveillance Systems. We synthesized data and developed a framework for how data was selected, collated, linked, organized and interpreted. Results: The results of our literature search demonstrates considerable gaps in the reporting of CO-MRSA in Australia. Consequently, total incidences were under reported; however the available data suggests the incidence varied between 44 (Tasmania) and 388 (southern Northern Territory) cases per 100,000 person years. Hospitalised cases of CO-MRSA varied between 3.8 (regional Victoria) and 329 (southern Northern Territory). Taking the median percentage of infections by site for all regions available, skin and soft tissue infections (SSTIs) consisted of 56% of hospitalized CO-MRSA, compared with bacteremias, which represented 14%. No region had a complete data set of CO-MRSA infections treated in out-patient settings and so incidences were underestimates. Nevertheless, estimates of the incidence of CO-MRSA treated outside hospitals varied between 11.3 (Melbourne) and 285 (Northern Territory) per 100,000 person-years. These infections were chiefly SSTIs, although urinary tract infections were also noted. Incidences of CO-MRSA blood-stream infections and outpatient skin and soft tissue infections have been increasing with time, except in Tasmania. CO-MRSA is observed to affect people living in remote areas and areas of socioeconomic disadvantage disproportionately. Conclusions: We generated the first estimates of the incidence of CO-MRSA infections in Australia and identified stark regional differences in the nature and frequency of infections. Critically, we demonstrate that there has been a lack of consistency in reporting CO-MRSA and a general dearth of data. The only government in Australia that requires reporting of CO-MRSA is the Tasmanian, where the infection was least prevalent. Some regions of Australia have very high incidences of CO-MRSA. To improve surveillance and inform effective interventions, we recommend a standardized national reporting system in Australia that reports infections at a range of infection sites, has broad geographic coverage and consistent use of terminology.
