Objectives: Social determinants of health (SDOH) are comprised of a patient's environmental conditions including social and economic factors, which influence access to health care and resources. Growing evidence in orthopaedic surgery has revealed that SDOH factors lead to differential access to care and ultimately health disparities after surgery. For shoulder instability, previous literature has demonstrated that the number of previous dislocations before a stabilization procedure increases the risk of recurrent instability after the procedure. The purpose of this study was to investigate the impact of SDOH on the number of dislocation events before surgical intervention. Methods: A retrospective review of patients who underwent shoulder stabilization surgery at a health system in a large metropolitan area between January 1, 2021 and April 13, 2023 were identified. Patients' demographic and social determinants variables were extracted using the electronic medical record. Social Vulnerability Index (SVI) socioeconomic subscore and Area of Deprivation Index (ADI) were collected using online mapping data based on patient zip codes. The number of dislocation events and the time they occurred were determined using clinical charts. Operative variables collected included the date of surgery to determine the time from clinical presentation to surgery and the procedure performed. Univariate linear regression analysis was used to evaluate potential predictors of increasing time to presentation and increasing time to surgery. Univariate logistic regression analysis was also performed of all potential predictors of having >1 dislocation event. A multivariate model was then created using all predictors with a P value < 0.05 in the univariate models. Results: There were 106 patients who underwent shoulder surgery for instability, including arthroscopic and open stabilization, and had complete social determinant data, with 54% (n = 57) identifying as White, 29% (n = 31) as Black/African American, and 17% (n = 18) as other. A total of 38 (35.8%) patients suffered 1 dislocation (single dislocation cohort) before undergoing surgery, and 68 (64.2%) experienced > 1 dislocation (recurrent cohort) before surgery. No significant variables were associated with increasing time to presentation or surgery. Univariate logistic regression revealed that decreasing age (odds ratio [OR] 0.94 [95% confidence interval (CI) 0.89- 0.99]; P = 0.02), decreasing body mass index (BMI) (OR 0.90 [95% CI 0.83-0.98]; P = 0.02), increasing SVI (OR 1.21 [95% CI 1.05-1.38]; P = 0.006), and increasing area deprivation index (ADI) (third tercile compared to first, OR 6.04 [95% CI 2.05-17.8]; P = 0.003) were associated with increased odds of having > 1 instability event before surgical intervention. There was no association with race found in terms of number of dislocations before surgery. Multivariate logistic regression modeling revealed that decreasing BMI (OR 0.87 [95% CI 0.78-0.98]; P = 0.02) and increasing ADI (third tercile compared to first tercile, OR 7.46 [95% CI 1.26-44.2]; P = 0.02) were associated with increased odds of having > 1 instability event before shoulder instability surgery. Conclusions: Lower socioeconomic status, as measured by ADI, is an independent predictor of a higher likelihood of recurrent instability before shoulder stabilization surgery. Recognizing these relationships may motivate surgeons to create pathways to prevent these treatment disparities among shoulder instability patients. Objectives: This study aimed to assess the influence of socioeconomic status (SES) variables on patient-reported outcomes and return to work following surgery for a multiligament knee injury. Methods: We retrospectively reviewed patients at our institution who underwent multi-ligament knee reconstruction between 2012 and 2022. SES variables of interest included median household income, occupation, race, sex, and marital status. International Knee Documentation Committee (IKDC) scores were used to assess clinical outcomes. Patients reported whether they had returned to work at final follow-up. Data was available for each SES variable for a subset of the cohort. Chi-square tests and Fisher exact tests were used to compare the proportion of patients who returned to work by race, sex, income, marital status, and job type (sedentary vs. nonsedentary) and to compare the proportion of patients with sedentary jobs within each demographic variable. Mann-Whitney U tests were used to compare the distribution of IKDC scores at final follow-up across the same demographic variables. Results: There were 144 patients with multiligament knee injuries for whom data on at least 1 SES variable was available with a median final follow-up of 3.2 years. Patients were 73.2% male with an average age of 38.8 years. Overall, 26.8% of patients were married and 56.1% of patients had sedentary jobs. Of the patients who were working before surgery (63% of the total cohort), 56.0% returned to work after surgery. There was a significant difference in the proportion of patients who had sedentary jobs by income (12.5% low income vs. 50.7% high income, p<.001) and who returned to work postoperatively (16.7% low income vs. 75.0% high income, p=.016). Median IKDC scores for white patients were 78.0 versus 65.5 for Black/African American patients; the score distributions in the two groups differed significantly (p=.045). Patient reported outcomes were not significantly different by sex or marital status. Conclusions: Only about half of patients returned to work after multiligament knee injury surgery. White race and higher income were predictive of better patient-reported outcomes following surgery for multiligament knee injuries. Determining the extent to which SES variables influence these outcomes may lead to improvements in rehabilitation and follow-up for diverse patients. Since all policies address problems, they necessarily include implicit or explicit constructions of these problems. This paper explores how child maltreatment has been constructed in New Zealand's child protection policies. It questions the underlying assumptions of this problem construction and seeks to shed light on what has been omitted. Utilizing a qualitative content analysis of eight key policy documents, this study reveals the construction of child maltreatment has been dominated primarily by a child-centric, risk-focused approach. This approach assigns blame and shifts responsibilities onto parents and families. In addition, the vulnerability discourse and social investment approach underpinning this perspective have allowed important structural factors, such as poverty and inequality, to remain unaddressed. This paper also highlights the one-dimensional focus on the lower social class to control future liabilities. We suggest that the harm inflicted by corporations on children's well-being is another form of child exploitation currently omitted from the problem construction. We suggest that child abuse should be defined and understood in policy as harm to children's well-being and argue that the state should prevent and mitigate harm by addressing structural forces of the problem as well as protecting children against corporate harms. Much has been written about the measurement of socio-economic position (SEP) in high-income countries (HIC). Less has been written for an epidemiology, health systems and public health audience about the measurement of SEP in low- and middle-income countries (LMIC). The social stratification processes in many LMIC—and therefore the appropriate measurement tools—differ considerably from those in HIC. Many measures of SEP have been utilized in epidemiological studies; the aspects of SEP captured by these measures and the pathways through which they may affect health are likely to be slightly different but overlapping. No single measure of SEP will be ideal for all studies and contexts; the strengths and limitations of a given indicator are likely to vary according to the specific research question. Understanding the general properties of different indicators, however, is essential for all those involved in the design or interpretation of epidemiological studies. In this article, we describe the measures of SEP used in LMIC. We concentrate on measures of individual or household-level SEP rather than area-based or ecological measures such as gross domestic product. We describe each indicator in terms of its theoretical basis, interpretation, measurement, strengths and limitations. We also provide brief comparisons between LMIC and HIC for each measure. In any highly contested political domain, language can be a potent force for change or an obstacle to understanding and coalition building across difference. This is surely the case in the global debates over sexuality and gender, where even those terms themselves have aroused heated conflicts. In this spirit, we want to challenge the uncritical use of the term "sexual minorities", based on a number of historical and conceptual problems with which that term – like the larger thicket of identities and identity politics it signifies – is encumbered. These include: ignoring history, legitimating dubious normativity, fixing biological categories, and recreating exclusions. With this struggle, we seem caught in a modernist dilemma between two desires: to name and honour difference by signifying identities and to avoid exclusivity and hierarchy by reclaiming universals. The insistence of diverse groups on naming themselves and achieving recognition of their distinctness and variety will go on as long as aspirations for democracy exist, because that is the nature and necessity of emancipatory politics. At the same time, our language needs to reflect the fluidity and complexity of sexuality and gender expressions in everyday life and their intricate interweaving with other conditions such as class, race, ethnicity, time and place. Dans tout domaine politique très contesté, le langage peut être une force puissante de changement ou un obstacle à la compréhension et à la création de coalitions dépassant les différences. C'est sûrement le cas dans les débats mondiaux sur la sexualité et le genre, où même ces termes ont suscité des conflits houleux. Dans cet esprit, nous souhaitons remettre en question l'utilisation non critique de l'expression « minorités sexuelles », en nous basant sur les problèmes historiques et conceptuels avec lesquels ce terme, comme l'enchevêtrement d'identités et de politiques identitaires qu'il signifie, est associé. Ignorer l'histoire, légitimer une normativité douteuse, fixer des catégories biologiques et recréer des exclusions figurent au nombre de ces problèmes. Avec cette lutte, nous semblons nous trouver dans un dilemme moderniste pris entre deux désirs : désigner et honorer la différence en signifiant des identités, et éviter l'exclusivité et la hiérarchie en récupérant des normes universelles. L'insistance de divers groupes pour se nommer eux-mêmes et faire reconnaître leur différence se poursuivra aussi longtemps que les aspirations à la démocratie existeront, car telle est la nature et la nécessité des politiques émancipatoires. En ese espíritu, queremos cuestionar el uso ciego del término "minorías sexuales", basándonos en varios problemas históricos y conceptuales de los cuales está cargado ese término, como el matorral más amplio de identidades y política de identidades que significa. Algunos ejemplos son: hacer caso omiso de la historia, legitimar normatividad dudosa, arreglar categorías biológicas y recrear exclusiones. Con esta lucha, parecemos estar atrapados en un dilema modernista entre dos deseos: nombrar y honrar diferencias expresando identidades y evitar la exclusividad y jerarquía reclamando universales. La insistencia de diversos grupos en autonombrarse y lograr el reconocimiento de su particularidad y variedad perdurará siempre que existan aspiraciones de democracia, porque esa es la naturaleza y necesidad de la política emancipadora. Traditional costumes and crafts are a basic form and element of local culture and a vital pathway for perpetuating traditional art and design culture. They are an artistic form of historical and cultural significance. This paper focuses on three traditional costumes from Malta, Ireland, and Finland. The għonnella, worn by Maltese women of different social classes, consisted of a voluminous cape-like covering reinforced with whalebone and cardboard and was worn over the head and shoulders, reaching ankle length. Irish costumes were adorned with beautiful Irish lace, crochet, and embroidery. Celtic embroidery was added to clothing to develop a distinctive Irish dress style during the great cultural revival of the early 20th century. The Karelian costume from Finland was constructed using wool and linen. Ladies in Karelia wore handcrafted, highly embroidered gowns, and traditions were passed down from older ladies, including mothers and grandmothers. These costumes were collected in the late nineteenth and early twentieth centuries when Finnish Karelia was known as 'The Old Finland'. Examining the entanglement of democracy and social justice in education and the relationship to social mobility, this paper critiques the individualising nature of social mobility in policy discourse as inimical to human flourishing and education as a public good. The rhetoric of social mobility which responsibilises individuals for their success, without due regard to the systemic changes needed to enable this and the societal barriers to social mobility experienced disproportionately by some groups in society, is detrimental to social justice. A conception of education as a pathway to social mobility must be located in a wider vision of civic engagement in a more inclusive and fairer society. Yet civic engagement in education at the local level has been eroded, as illustrated in this paper by the example of an evolving model of school governance in England. As a corporatist, market driven reform, this model exemplifies how local democratic ties between schools and engagement with their communities can be undermined. Social mobility to enable opportunities, access and participation in democratic civic society becomes a fantasy when society is riven with systemic inequalities, lacks the necessary conditions to enable human flourishing and links to community engagement in education for democratic renewal are downplayed. Women are at greater risk of common mental disorders. The intersectionality concept provides a framework to examine the effects of multiple social disadvantages on women's mental health. We conducted a systematic review to collect and analyse information to identify the quantitative methodologies and study designs used in intersectional research to examine women's mental health and multiple social disadvantages. Included studies used accepted statistical methods to explore the intersectional effects of gender and one or more types of social disadvantage from the PROGRESS-Plus inequity framework: a place of residence, race/ethnicity, occupation, gender/ sex, religion, education, socioeconomic status, social capital (O'Neill et al. J Clin Epidemiol 67:56–64, 2014). The scope of this systematic review was limited to studies that analysed common mental disorders in women and men comparatively. Studies focusing on only one gender were excluded, ensuring a comprehensive comparative analysis of the intersection of social disadvantages in mental health. Twelve papers were included in the narrative synthesis (Table 1). Eight of the included papers (67%) reported an intersectional effect of gender and one or more additional types of social disadvantage. The multiplicative effect of gender and socioeconomic status on the risk of common mental disorders was the most commonly reported interaction. This systematic review shows that multiplicative and simultaneous interactions of multiple social disadvantage increase the risk of common mental disorders experienced by women. Moreover, it underlines the potential for quantitative research methods to complement qualitative intersectionality research on gender and mental health. Background: This study takes on the challenge of quantifying a complex causal loop diagram describing how poverty and health affect each other, and does so using longitudinal data from The Netherlands. Furthermore, this paper elaborates on its methodological approach in order to facilitate replication and methodological advancement. Methods: After adapting a causal loop diagram that was built by stakeholders, a longitudinal structural equation modelling approach was used. A cross-lagged panel model with nine endogenous variables, of which two latent variables, and three time-invariant exogenous variables was constructed. With this model, directional effects are estimated in a Granger-causal manner, using data from 2015 to 2019. Both the direct effects (with a one-year lag) and total effects over multiple (up to eight) years were calculated. Five sensitivity analyses were conducted. Two of these focus on lower-income and lower-wealth individuals. The other three each added one exogenous variable: work status, level of education, and home ownership. Results: The effects of income and financial wealth on health are present, but are relatively weak for the overall population. Sensitivity analyses show that these effects are stronger for those with lower incomes or wealth. Physical capability does seem to have strong positive effects on both income and financial wealth. There are a number of other results as well, as the estimated models are extensive. Many of the estimated effects only become substantial after several years. Conclusions: Income and financial wealth appear to have limited effects on the health of the overall population of The Netherlands. However, there are indications that these effects may be stronger for individuals who are closer to the poverty threshold. Since the estimated effects of physical capability on income and financial wealth are more substantial, a broad recommendation would be that including physical capability in efforts that are aimed at improving income and financial wealth could be useful and effective. Costumes played a significant role at the royal courts of King Henry VIII, and courtiers maintained scrupulous cautiousness regarding their presentability and dress. Theatricality is a persistent trope in Mantel's Neo - Tudor novels, often exercised through strategically displayed extravagant fabrics. This study interprets the clothing culture at the courts of King Henry VIII, as represented in the three Wolf Hall novels. As social classes became increasingly stratified during the Renaissance, the bourgeoisie distinguished themselves from the commoners and sustained inclusivity into the monarchical elite by enacting strategic theatricality. This paper demonstrates how the theatricality of magnificence was performed through the politics of apparel, ornamentation, and distinguished fashion. Referring to works by John Matusiak, Tracy Borman, Maria Hayward, and Elizabeth Currie, this study argues that fashion was at the core of Royal Tudor governance, and Mantel utilises this trope to camouflage or amplify the magnitude of a political persona. In the context of the Tudor Sumptuary Laws, this paper also analyses how Mantel used dress as a motif of theatricality to demonstrate class segregation during sixteenth-century England. The gendering of clothes and its political ramifications shall be another issue tackled by this paper, focusing on the sartorial choices of the characters of Anne Boleyn, Katherine, and Jane Seymour. Simple Summary: Neurofibromatosis Type 1 is an inherited tumour predisposition syndrome with a varied clinical phenotype. Long-term monitoring through imaging is inconsistent and varies in high- and low-income countries. Implementation of a clinical practice guideline through a multidisciplinary clinic is instrumental to the care of adult Neurofibromatosis Type 1 patients. This systematic review aims to evaluate the association between a country's socioeconomic status and diagnostic modalities and strategies used for adult Neurofibromatosis Type 1 patients. Our results show multiple imaging modalities are used in high-income countries; however, there is limited use in low-income countries. The two most common diagnostic modalities used in developed countries are WB MRI and FDG PET/CT. Background: Neurofibromatosis Type 1 is an autosomal dominant tumour-predisposition condition commonly diagnosed in childhood and fully penetrant by adulthood. Methods: We searched PubMed, Embase, Web of Science, and Cochrane. Relevant clinical information on the surveillance of adult Neurofibromatosis Type 1 patients worldwide was reviewed, extracted, and synthesised. Results: We identified 51 papers reporting on 7724 individuals. Multiple imaging modalities are actively employed in high-income and upper-middle-income countries for surveying adult Neurofibromatosis Type 1 patients. We did not find any relevant papers from low- and middle-income countries. Conclusions: This systematic review suggests that there is robust data on diagnostic modalities for adult Neurofibromatosis Type 1 patients in high-income countries, but not for low- and middle-income countries. There is a lack of data on consolidated diagnostic strategies from both high- and low-income countries. Efforts should be made to publish data on usual clinical practice in low- and middle-income countries to develop clinical practice guidelines describing best medical practice to fit a local context. It is documented that maternal diseases or treatments influence a newborn's clinical status at birth. If a prenatal medical history is not available, or if signs or symptoms of a mother's disease are revealed for the first time during pregnancy or postpartum, their effects on the newborn may be misattributed. Objective: The objective of this study is to emphasize the paramount importance of prenatal care, for both mothers and newborns, as a lack of maternal signs and symptoms during pregnancy does not exclude a potential severe neonatal condition. Materials and methods: We present a series of three cases of pregnant women who gave birth to very sick preterm newborns that required admission to the Neonatal Intensive Care Unit (NICU). The mothers were asymptomatic during pregnancy and unaware of their subclinical disease. The newborns' complications, considered initially as consequences of prematurity or infection, subsequently revealed transient autoimmune disease in two of the cases (myasthenia gravis and hyperthyroidism) and a severe form of thrombophilia in the third case. Results: The newborns' diagnosis preceded maternal diagnosis and contributed to the identification of the maternal pathology; adequate treatment was prescribed, with favorable short- and long-term outcomes. Conclusions: Prenatal exams and investigations throughout pregnancy are a good opportunity to detect subclinical diseases or predispositions. As newborns usually develop non-specific signs, one should have experience and pay attention to differentiating among etiologies. Our paper takes a reversed approach to the usual medical diagnosis pathway: from infant to mother instead of from mother to infant, proving that inter-specialty collaboration can work bi-directionally. Background: Non-communicable diseases (NCDs) are the leading cause of death globally. Eighty-two percent of premature NCD deaths occur within low- and lower middle-income countries (LLMICs). Research to date, largely drawn from high-income countries, suggests that disadvantaged and marginalized groups have a higher NCD burden, but there has been a dearth of research studying this relationship within LLMICs. The purpose of this systematic review is to map the literature on evidence from LLMICs on the socio-economic status (SES) gradient of four particular NCDs: cardiovascular disease, cancer, diabetes, and chronic respiratory diseases.Methods: We conducted a comprehensive literature search for primary research published between 1 January 1990 and 27 April 2015 using six bibliographic databases and web resources. We included studies that reported SES and morbidity or mortality from cardiovascular disease, cancer, diabetes and chronic respiratory diseases within LLMICs.Results: Fifty-seven studies from 17 LLMICs met our inclusion criteria. Fourteen of the 18 papers that reported significant associations between cancer and SES suggested that low SES groups had the highest cancer risk. Eleven of 15 papers reporting significant relationships between CVD and SES suggested that low SES groups have higher risk. In contrast, seven of 12 papers reporting significant findings related to diabetes found that higher SES groups had higher diabetes risk. We identified just three studies on the relationship between chronic respiratory diseases and SES; none of them reported significant findings.Conclusions: Only 17 of the 84 LLMICs were represented, highlighting the need for more research on NCDs within these countries. The majority of studies were medium to high quality cross-sectional studies. When we restricted our analyses to high quality studies only, for both cancer and cardiovascular disease more than half of studies found a significantly higher risk for those of lower SES. The opposite was true for diabetes, whilst there was a paucity of high quality research on chronic respiratory disease. Objectives: Withholding workers' salaries for months as a punishment for engaging in a strike may natively affect and influence their psychological distress. This study assessed the correlation between physical activity, psychological distress, and socioeconomic status, and explored the factors influencing psychological distress among the striking workers in Nigeria. Methods: This cross-sectional face-to-face and online study conveniently sampled a total of 234 lecturers aged 27-69 years. The sociodemographic, physical, socio-economic, and psychological distress was assessed by the university strike physical and psychological distress questionnaire (USPAPDQ). Data analysis includes descriptive statistics, multiple linear regression, Spearman's correlation, and Man whiney U-test, using a P<0.05 as the level of significance. Results: The mean age, number of papers, and frequency/duration of exercise were 45.4±10.36 years, 4±5.82, 2.19±1.63 day/week, and 30.49±29.82 minutes/day, respectively. An inverse significant relationship was established between anxiety and age (r=-0.27; P<0.01), contemplating changing my job (r=-0.40; P<0.01), number of children (r=-0.19; P<0.01), academic rank (r=-0.27; P<0.01), frequency (r=-0.18; P<0.01), and duration (r=-0.16; P=0.02) of exercise. The significant predictors of anxiety were marital status (ß=-0.207, P<0.01), contemplation on changing my job if the strike continues (P<0.01, ß=-0.198), see anything positive about the strike (P<0.01, ß=0.178), and numbers of children (P<0.01, ß=-0.193). The significant predictors of depression were, an alternate source of income (P=0.04, ß=0.126), contemplation on changing my job if the strike continues (P=0.03, ß=-0.149), seeing anything positive about the strike (P=0.05, ß=0.118), and time (hours) spent watching television (P=0.03, ß=0.124). Discussion: Overall, the significant negative predictors of psychological distress include marital status, contemplating changing my job, and number of children. The positive predictors were seeing anything positive about the strike, alternate sources of income, and time spent watching television. Playability is an attribute that refers to the ability to stimulate individual responses or collective action in an immersive activity in an exploratory way. Playability is an important component of the enjoyment and well-being of urban dwellers, has the potential to stimulate urban vitality and is an important expression of the inclusiveness and equity of urban space. The pursuit of economic development and efficiency-oriented urban construction has led to the domination of urban space by overcrowded traffic, economy-oriented commerce and densely populated housing. Moreover, the existence of playable space has become a scarce resource and is seen as a site for the materialisation of social rights. As the haze of the COVID-19 pandemic fades and cities are again exposed to wider and more participatory use, determining how to adapt urban spaces to the playability needs of users of different ages, cultural backgrounds and social classes, and provide them with appropriate site use and experience, is becoming a hot issue of concern for building equitable and high-quality urban spaces. The study of the playability of urban spaces is highly complex, and the related research on social justice is cryptic. In order to better investigate the social equity aspects of urban playability, this paper integrates scientometric and manual methods to review the relevant research. This paper takes 2664 related papers from the Web of Science (WOS) core dataset from 1998 to 2022 as the research object and employs CiteSpace to organise the existing research results of playful urban spaces. Quantitative analysis is used to clarify the theoretical foundations, developments and research hotspots of urban spatial playability, while the manual collation and generalisation of the studies uncover the hidden issues of social justice research. Based on the proposed research method, we summarize the key three research stages related to urban spatial playability and social equity. We also discuss the development of urban spatial playability in the perspective of social justice from three aspects: micro, meso and macro. The results can help readers better understand the current status and development process of research on playful urban space. In particular, we clarify the issues related to social justice under the theme of the playability of urban spaces and provide directions for future research on building playful cities and promoting the development of urban socio-spatial equity. This paper discusses the use of photo ranking exercises together with qualitative interview data to study migrants' perception of social status. It draws on data from a mixed-method study, involving in-depth interviews with migrants from different socio-economic backgrounds and mobility experiences in Germany. The paper focuses on how photo ranking exercises can be combined with more traditional interviewing techniques in order to elicit peoples' subjective perceptions of status mobility in transnational spaces. It demonstrates that ranking exercises can be helpful in the effort to design data collection methods which are combining substantialist and relational approaches to the study of social class and social positions.
