This paper presents a numerical investigation of the air wake around a generic surface combatant ship called NATO-Generic Destroyer (NATO-GD). Naval surface combatants with flight decks must be designed taking aerodynamic concerns into account. Most of the previous studies have employed the Simple Frigate Shape (SFS) and its modified version (SFS2) to investigate the airwake. However, these generic geometries do not accurately represent modern warship designs. To address this, a modern geometry called NATO-GD proposed by the NATO Research Task Group, which represents the features of a modern destroyer, was utilized in the present work. The objective is to examine the air wake on the helicopter deck to ensure the safe operation of air vehicles such as helicopters and drones. The three-dimensional, transient airflow around the ship was solved using the unsteady Reynolds-Averaged Navier-Stokes (URANS) and Detached Eddy Simulation (DES) turbulence models. Besides, the effect of inflow was investigated by comparing uniform velocity inlet and atmospheric boundary layer (ABL) for various wind-over-deck (WOD) angles. The numerical approach was verified for the URANS turbulence model using the Grid Convergence Index (GCI) method. The numerical uncertainty was calculated with four different methods and the uncertainty was found between 1.6% and 2.1%. A detailed discussion of the flow field above the flight deck was conducted to compare the URANS and DES models fairly. It was concluded that employing the ABL profile as a boundary condition is more suitable for achieving accurate ship aerodynamics calculations. The ABL velocity profile makes a significant difference in the velocity components. According to the URANS results, these deviations are found as 8.23% in the x-component, 1.25% in the y-component and 4.89% in the z-component. The deviations were calculated using the root mean square error (RMSE) method. Furthermore, although the numerical results of the URANS and DES models were similar at some points, detailed flow field analysis is only possible with the DES results to determine safe approach patterns for air vehicles. Various wind speeds, directions, and the resulting wake structures were tested to analyze the wake behavior over the helicopter deck under different air conditions. When the wind comes from the port side with 15 degrees (R15) it wind changes the intense turbulence region and creates a low turbulence area on the starboard side while R30 wind causes small scale vortices breaking this region. In our present study, we have demonstrated that the presence of anisometropic amblyopia in one eye does not alter this unique feature significantly. We investigated whether the presence of unilateral anisometropic amblyopia due to moderate amounts of hyperopia and/or astigmatism would have any significant influence on measurements of retinal thickness (RT) and superficial vessel density (SVD) in adult subjects.Subjects and methods: In the study group, both eyes of 21 adult subjects with unilateral anisometropic amblyopia were enrolled. Only one eye of an age-matched control group including 33 subjects was randomly selected. RT and SVD were measured in foveal and parafoveal retinal areas and compared in amblyopic eyes, their fellow eyes and in the control group. Inter-ocular symmetry of RT and SVD in amblyopic and fellow eyes were investigated by using correlation analysis.Results: There were statistically significant inter-ocular correlations of foveal and parafoveal RT in amblyopic and fellow eyes (P < 0.0001, for both). Foveal and parafoveal RT of the control group were not statistically different from amblyopic eyes (P = 0.072 and P = 0.46, respectively). There was no difference between foveal SVD of amblyopic eyes and control group (P = 0.10) and a significant inter-ocular correlation was found between amblyopic and fellow eyes (P = 0.029).Conclusion: For our adult subjects, none of the investigated OCTA parameters of the amblyopic eyes were found to be statistically different from those of age-matched controls. The preservation of inter-ocular symmetry in the amblyopic and fellow eyes implies that the influence of anisometropic amblyopia in one eye would be relatively small and negligible on routine OCTA examination.Keywords: anisometropic amblyopia, hyperopia, optical coherence tomography angiography, inter-ocular symmetry

In the present paper, two-phase three-dimensional turbulent flow simulations are carried out by applying computational fluid dynamics (CFD) technique to the internal flow of a horizontal separator that is used in petroleum industry. Two different geometries are considered; the separator with a straight plate at the top and the separator with the straight plate on the side of the separator. Effects of the location, distance between the inlet of the separator and the diverter plate and inlet velocity on the separation efficiency are investigated by employing the standard k- ε turbulence model. For these purposes, three different distances between the straight diverter plate and the inlet to the separator (0.1 m, 0.15 m and 0.2 m) and four different velocities (0.25 m/s, 0.5 m/s, 0.75 m/s, and 1 m/s) are taken into account by means of Euler mixture model. It is revealed that the maximum separation efficiency is 99.772% when the mixture enters the separator from the top with the inlet velocity of 0.25 m/s and the plate is located 0.2 m away from the inlet section of the separator. An inverse correlation is detected between the inlet velocity and the efficiency of the separation since increasing the inlet velocity decreases the efficiency of the separation. In this paper, formation and development of one of the most dominant vortex structures, namely, counterrotating vortex pair (CVP) which is seen in the jet in crossflow are investigated numerically. Influences of the inclination angles between the nozzle(s) and channel on the CVP are presented for three inclination angles, α=30°, 60° and 90° at velocity ratio, R=2.0. Effects of the number of the nozzles on the evolution of CVP is analyzed by considering the single and three side-by-side positioned circular nozzles. In addition to the CVP, some secondary vortices are also reported by considered relatively a narrow channel because their existence cannot be showed in wider channel. Simulations reveal that higher the inclination angle the more jet penetration into the channel in all directions and increasing the inclination angle causes larger CVPs in size. In the present study, results obtained from three-dimensional incompressible flow over backward-facing step in a rectangular duct using Realizable k-ε turbulence model are reported. Effects of step inclination angle have been investigated for β=30°, 45°, 90° while upper wall of duct kept parallel to the bottom wall and then rose to α=6°. Reynolds number based on freestream velocity and step height is examined by varying its magnitude in the range of 15,000-64,000. Simulated results are presented for revealing the general flow features and reattachment lengths after a successful validation of the present work with the experimental data of Driver and Seegmiller (1985). It is found that turbulence kinetic energy increases suddenly after the step for both straight and raised upper walls and reaches its maximum at x/h=5 that consistent with the published literature. The size of reattachment lengths increases with the step inclination angle, i.e., maximum reattachment length occurs at 90° for both straight and raised upper walls. Purpose: To compare intraocular pressure (IOP) control in eyes with or without clear corneal phacoemulsification following trabeculectomy. Methods: The study group included 30 eyes that underwent uneventful clear corneal phacoemulsification and foldable intraocular lens implantation following trabeculectomy without antimetabolites. Thirty eyes that had undergone filtering surgery without cataract extraction were selected as controls. Case and control groups were matched with respect to age, gender, IOP, number of glaucoma medications, glaucoma type (primary open-angle glaucoma/ pseudoexfoliative glaucoma), trabeculectomy time and follow-up. Comparisons between the study and control groups (intergroup) and within the same group at different time-points (intragroup) were performed for IOP, glaucoma medications and bleb morphology. Success rates were investigated by Kaplan−Meier survival analysis and the factors influencing final success by logistic regression. Results: Intraocular pressure (p = 0.04) and glaucoma medications (p = 0.001) increased during an average follow-up of 26.1 ± 9.9 months in both groups. Intragroup differences became statistically significant after the 6-month visit, but intergroup differences remained insignificant. Bleb height decreased significantly following phacoemulsification in the study group (p = 0.017). Success rates decreased with time in both groups, with no intergroup difference (p = 0.46). The final success rate was negatively correlated with IOP and number of glaucoma medications used at the study entry, while there was a positive correlation between the baseline and final success rates. Conclusion: Trabeculectomy success decreased in a time-dependent manner in eyes with and without subsequent phacoemulsification. Uncomplicated clear corneal phacoemulsification was not found to have any additional unfavorable influence on IOP control in eyes with filtering blebs. [ABSTRACT FROM AUTHOR]

Purpose: This prospective study aimed to assess the impact of topical latanoprost 0.005% treatment on the microvasculature of the macula and optic nerve head (ONH) in primary open-angle glaucoma (POAG)., Methods: Fifty-seven eyes of 31 subjects were enrolled. ONH whole-image (wiVD), peripapillary (ppVD), and macular vascular density (VD) parameters were measured using optical coherence tomography angiography before treatment, at the 1st month, and at the 3rd month of treatment., Results: There was a significant reduction in IOP post treatment, with a decrease of 17.0 ± 2.3 mmHg in the 1st month and 16.3 ± 2.5 mmHg in the 3rd month (P < 0.001 for both). ONH wiVD and ppVD significantly increased at the post-treatment 1st month (P = 0.001 for both) but decreased by the 3rd month, returning to baseline levels (P < 0.001 for both), indicating a return to pre-treatment levels (P > 0.05 for both). Similarly, macular wiVD and perifoveolar VD (pefVD) increased significantly at the 1st month (P = 0.013 and P < 0.001, respectively) but returned to baseline by the 3rd month (P < 0.001 for both). No significant difference was observed between before-treatment and post-treatment 3rd month regarding the macular wiVD and pefVD (P > 0.05 for both). Deep capillary plexus and foveal avascular zone parameters did not change during follow-up (P > 0.05 for both)., Conclusion: The initial increase in ONH and peripapillary VD, as well as macular VD, is attributed to the acute reduction in IOP from topical latanoprost 0.005%. The purpose of this study is to investigate the potential anticarcinogenic effects of three phytochemicals, namely Alpha-pinene (AP), Gamma-terpinene (GT), and P-cymene (PC), on melanoma cells (A2058 cell line). Additionally, the study aims to explore the synergistic activities of these phytochemicals with Dacarbazine, a chemotherapy drug. To understand the molecular mechanism involved in apoptosis induction in the A-2058 cell line, it was used AO/EB staining for apoptosis detection and cell cycle analysis, monitored through flow cytometry. It also determined the mRNA expression levels of different apoptosis-regulatory genes, including p53, Bax, NF-kB, Bcl-2, Bcl-xl , and caspase-3. The antitumor activities of these phytochemicals and their combinations were investigated in a subcutaneous mouse tumor model. The tumor diameter was 21.4 ± 1.1 mm in the Dacarbazine treatment group and 42.4 ± 3.1 mm in the control group. The antitumoral activities of AP and PC in the tumor model were similar to those of Dacarbazine. On the other hand, GT exhibited remarkable antitumoral activity, with a 1.75-fold reduction in tumor diameter compared to the Dacarbazine group. When different combinations of phytochemicals and Dacarbazine were used, the GT plus Dacarbazine treatment group was found to have a 3.5-fold reduction in tumor diameter compared to the Dacarbazine group. Our study employs pooled CRISPR screens, integrating 2D and 3D culture models, to identify miRNAs critical in Breast Cancer (BC) tumoursphere formation. These screens combine with RNA-seq experiments allowing identification of miRNA signatures and targets essential for tumoursphere growth. miR-4787-3p exhibits significant up-regulation in BC, particularly in basal-like BCs, suggesting its association with aggressive disease. Surprisingly, despite its location within the 5'UTR of a protein coding gene, which defines DROSHA-independent transcription start site (TSS)-miRNAs, we find it dependant on both DROSHA and DICER1 for maturation. Inhibition of miR-4787-3p hinders tumoursphere formation, highlighting its potential as a therapeutic target in BC. Our study proposes elevated miR-4787-3p expression as a potential prognostic biomarker for adverse outcomes in BC. We find that protein-coding genes positively selected in the CRISPR screens are enriched of miR-4787-3p targets. Of these targets, we select ARHGAP17, FOXO3A, and PDCD4 as known tumour suppressors in cancer and experimentally validate the interaction of miR-4787-3p with their 3'UTRs. Our work illuminates the molecular mechanisms underpinning miR-4787-3p's oncogenic role in BC. These findings advocate for clinical investigations targeting miR-4787-3p and underscore its prognostic significance, offering promising avenues for tailored therapeutic interventions and prognostic assessments in BC., (© 2024. Thoracic aortic disease (TAD) is often silent until a life-threatening complication occurs. However, genetic information can inform both identification and treatment at an early stage. Indeed, a diagnosis is important for personalised surveillance and intervention plans, as well as cascade screening of family members. Currently, only 20% of heritable TAD patients have a causative mutation identified and, consequently, further advances in genetic coverage are required to define the remaining molecular landscape. The rapid expansion of next generation sequencing technologies is providing a huge resource of genetic data, but a critical issue remains in functionally validating these findings. Induced pluripotent stem cells (iPSCs) are patient-derived, reprogrammed cell lines which allow mechanistic insights, complex modelling of genetic disease and a platform to study aortic genetic variants. Introduction: The increasing inclusion of children in the criminal justice system as "juvenile offenders" and "juvenile victims" has recently emerged as a severe and multifaceted problem. This study evaluates whether juvenile offenders differ from juveniles who have not participated in the criminal justice system and juvenile victims regarding executive function skills and attentional bias., Method: The participant group comprised 85 children aged 12-18, and the study setting was Turkey, utilizing one control group and two treatment groups with open criminal case files in Antalya Courthouse. The first treatment group consisted of 30 juvenile offenders; the second consisted of 30 juvenile victims. The control group consisted of 25 juveniles who were not juvenile offenders or victims. In this context, children's executive functions were measured with the short-form Barratt Impulsivity Scale, the Raven Standard Progressive Matrices Test, the TBAG-form Stroop test, the Wisconsin Card Sorting Test, and the Istanbul 5 Cube Planning Test. Attentional bias was measured using a dot-probe task. Illiteracy, intellectual or developmental disability, and being a non-native Turkish speaker were the exclusion criteria for all three groups., Results: The study found that the scores of the juvenile offender group on the Barratt Impulsivity Scale were significantly higher than the children in the juvenile victim group and the children in the control group. For other tests measuring executive functions, the control group's scores were significantly higher than juvenile offenders and juvenile victims. Regarding attentional bias, the children in the control group exhibited less attentional bias to negative stimuli than the juvenile offenders and victims., Discussion: Researchers have generally addressed the reasons that push children to crime and become victims of crime through individual, familial, and environmental reasons. However, the number of studies investigating the neuropsychological characteristics of children dragged into crime is relatively limited in our country. In addition, there is no study comparing the executive functions and attentional bias of children who are dragged into crime, victimized children, and children without a history of being dragged into crime and victimization. Introduction: This study aims to address the positive changes due to traumatic experiences, such as being diagnosed with cancer and experiencing this disease for a certain period. Within this purpose, socio-demographic and disease-related variables, coping ways and illness perceptions that affect posttraumatic growth in cancer patients were examined. Secondly, the findings of this study, which is one of the first studies on posttraumatic growth in cancer patients in Turkey, were compared with the findings of current studies on the subject., Method: Datums were collected by an interview form and three scales (Posttraumatic Growth Scale, Ways of Coping Inventory and Illness Perception Scale-R) to 78 cancer outpatients in Istanbul University Oncology Institute in 2007., Results: Results showed that cancer patients have higher posttraumatic growth levels than the mean. According to analysis, posttraumatic growth total score between confrontive coping ( t  = -2.344, p  < 0.05), self-controlling ( t  = -3.704, p  < 0.001), accepting responsibility ( t  = -3.032, p  < 0.01), escape-avoidance ( t  = -2.285, p  < 0.05), planful problem solving ( t  = -2.502, p  < 0.05), positive reappraisal ( t  = -5.241, p  < 0.001), and seeking social support ( t  = -3.527, p  < 0.01) has relationship. Also, there is a relation between posttraumatic growth subscales and the Revised form of Illness Perception Questionnaire; Change in relationships with others subscale ( t  = 2.887, p  < 0.01) and Change in self-perception subscale ( t  = 2.660, p  < 0.01) between timeline (acute/chronic), Change in self-perception subscale between timeline (cyclical) ( t  = -2.788, p  < 0.01) and uncontrollable body factors ( t  = -1.916, p  < 0.05) Change in philosophy of life subscale between external attributions ( t  = -2.057, p  < 0.05) and Change in relationships with others subscale ( t  = -2.920, p  < 0.01) between chance factors. It was found that positive reappraisal ( F  = 78.290, p  < 0.001), self-controlling ( F  = 39.814, p  < 0.001), and distancing ( F  = 46.311, p  < 0.001) were significant predictors of posttraumatic growth total score. Results showed that ways of coping and illness perceptions were essential variables in posttraumatic growth., Discussion: Studies on posttraumatic growth in Turkey and the world have significantly increased in recent years. This study aimed to examine the findings obtained from cancer patients in 2007 in discussion with the findings in the current literature. Urban parks play an important role in urban ecosystems with multifunctional services. Park size is one of the important attributes to explain the potential capacity of urban parks. In recent years, the number of studies that focus on the relationship between park size and ecosystem services in the context of ecosystem services has been increasing. This study aims to focus on investigating the relationship between ecosystem services and urban park size. The Web of Science online databases were searched using the keywords "green space", "size" and "urban" as well as related terms. A total of 129 papers meeting the inclusion criteria were examined within the scope of the research aims. The results obtained from the systematic search were summarized and presented in the three categories with sub-categories: (i) cultural services with sub-categories of physical activity (n = 42) and housing prices (n = 10), (ii) supporting services with sub-categories of fauna (n = 44), and flora (n = 3) and (iii) regulating services with sub-categories of microclimate (n = 25) and air quality (n = 5). The results indicated that park size influences a wide range of services by providing more spaces for physical activities, increasing housing prices, creating habitats for birds, insects, and bats, and effects on their richness, diversity and density, increasing the cooling effect capacity and reducing air pollution of urban parks. In conclusion, a comprehensive approach is needed, recognizing the potential of both large and small green spaces to optimize ecosystem services delivery, promote resilience, and enhance urban well-being. This includes considering supply and demand aspects and improving measurement methods. Introduction: Malnutrition is defined as a pathological condition arising from deficient or imbalanced intake of nutritional elements. Factors such as increasing metabolic demands during the disease course in the hospitalized patients and inadequate calorie intake increase the risk of malnutrition. The aim of the present study is to evaluate nutritional status of patients admitted to pediatric intensive care units (PICU) in Turkey, examine the effect of nutrition on the treatment process and draw attention to the need for regulating nutritional support of patients while continuing existing therapies., Material and Method: In this prospective multicenter study, the data was collected over a period of one month from PICUs participating in the PICU Nutrition Study Group in Turkey. Anthropometric data of the patients, calorie intake, 90-day mortality, need for mechanical ventilation, length of hospital stay and length of stay in intensive care unit were recorded and the relationship between these parameters was examined., Results: Of the 614 patients included in the study, malnutrition was detected in 45.4% of the patients. Enteral feeding was initiated in 40.6% ( n  = 249) of the patients at day one upon admission to the intensive care unit. In the first 48 h, 86.82% ( n  = 533) of the patients achieved the target calorie intake, and 81.65% ( n  = 307) of the 376 patients remaining in the intensive care unit achieved the target calorie intake at the end of one week. The risk of mortality decreased with increasing upper mid-arm circumference and triceps skin fold thickness Z-score (OR = 0.871/0.894; p  = 0.027/0.024). The risk of mortality was 2.723 times higher in patients who did not achieve the target calorie intake at first 48 h ( p  = 0.006) and the risk was 3.829 times higher in patients who did not achieve the target calorie intake at the end of one week ( p  = 0.001). The risk of mortality decreased with increasing triceps skin fold thickness Z-score (OR = 0.894; p  = 0.024)., Conclusion: Timely and appropriate nutritional support in critically ill patients favorably affects the clinical course. The results of the present study suggest that mortality rate is higher in patients who fail to achieve the target calorie intake at first 48 h and day seven of admission to the intensive care unit. In this study, we explored the potential of hydrodynamic cavitation (HC) for use in dissolution of liquid and powder detergents. For this, microfluidic and polyether ether ketone (PEEK) tube HC reactors with different configurations were employed, and the results from the reactors were compared with a magnetic stirrer, as well as a tergotometer. According to our results PEEK tube HC reactors present the best performance for dissolution of liquid and powder detergents. In the case of liquid detergent, for the same level of initial concentration and comparable final dissolution, the PEEK tube consumed 16.7 and 70% of the energy and time of a tergotometer and 16.7 and 14.8% of that of a magnetic stirrer, respectively. In the case of powder detergent, the PEEK tube used 12% less power than a tergotometer and 81.2% less power than a magnetic stirrer. Additionally, the time required to dissolve the detergent was reduced significantly from 1200 s in the tergotometer and 1800 s in the magnetic stirrer to just 50 s in the PEEK tube. Background: This study evaluated of clinical characteristics, outcomes, and mortality risk factors of a severe multisystem inflammatory syndrome in children admitted to a the pediatric intensive care unit., Methods: A retrospective multicenter cohort study was conducted between March 2020 and April 2021 at 41 PICUs in Turkey. The study population comprised 322 children diagnosed with multisystem inflammatory syndrome., Results: The organ systems most commonly involved were the cardiovascular and hematological systems. Intravenous immunoglobulin was used in 294 (91.3%) patients and corticosteroids in 266 (82.6%). Seventy-five (23.3%) children received therapeutic plasma exchange treatment. Patients with a longer duration of the PICU stay had more frequent respiratory, hematological, or renal involvement, and also had higher D-dimer, CK-MB, and procalcitonin levels. A total of 16 patients died, with mortality higher in patients with renal, respiratory, or neurological involvement, with severe cardiac impairment or shock. The non-surviving group also had higher leukocyte counts, lactate and ferritin levels, and a need for mechanical ventilation., Conclusions: In cases of MIS-C, high levels of D-dimer and CK-MB are associated with a longer duration of PICU stay. Non-survival correlates with elevated leukocyte counts and lactate and ferritin levels. We were unable to show any positive effect of therapeutic plasma exchange therapy on mortality., Impact: MIS-C is a life-threatening condition. Patients need to be followed up in the intensive care unit. Early detection of factors associated with mortality can improve outcomes. Determining the factors associated with mortality and length of stay will help clinicians in patient management.
