Excessive joint surface loadings, either single (acute impact event) or repetitive (cumulative contact stress), can cause the clinical syndrome of osteoarthritis (OA). Despite advances in treatment of injured joints, the risk of OA following joint injuries has not decreased in the past 50 years. Cumulative excessive articular surface contact stress that leads to OA results from posttraumatic joint incongruity and instability, and joint dysplasia, but may also cause OA in patients without known joint abnormalities. In vitro investigations show that excessive articular cartilage loading triggers release of reactive oxygen species (ROS) from mitochondria, and that these ROS cause chondrocyte death and matrix degradation. Preventing release of ROS or inhibiting their effects preserves chondrocytes and their matrix. Fibronectin fragments released from articular cartilage subjected to excessive loads also stimulate matrix degradation; inhibition of molecular pathways initiated by these fragments prevents this effect. Additionally, injured chondrocytes release alarmins that activate chondroprogentior cells in vitro that propogate and migrate to regions of damaged cartilage. These cells also release chemokines and cytokines that may contribute to inflammation that causes progressive cartilage loss. Distraction and motion of osteoarthritic human ankles can promote joint remodeling, decrease pain, and improve joint function in patients with end-stage posttraumatic OA. These advances in understanding of how altering mechanical stresses can lead to remodeling of osteoarthritic joints and how excessive stress causes loss of articular cartilage, including identification of mechanically induced mediators of cartilage loss, provide the basis for new biologic and mechanical approaches to the prevention and treatment of OA.

In contemporary total hip arthroplasty, instability has been a complication in approximately 2% to 5% of primary surgeries and 5% to 10% of revisions. Due to the reduction in the incidence of wear-induced osteolysis that has been achieved over the last decade, instability now stands as the single most common reason for revision surgery. Moreover, even without frank dislocation, impingement and subluxation are implicated in a set of new concerns arising with advanced bearings, associated with the relatively unforgiving nature of many of those designs. Against that backdrop, the biomechanical factors responsible for impingement, subluxation, and dislocation remain under-investigated relative to their burden of morbidity. This manuscript outlines a 15-year program of laboratory and clinical research undertaken to improve the scientific basis for understanding total hip impingement and dislocation. The broad theme has been to systematically evaluate the role of surgical factors, implant design factors, and patient factors in predisposing total hip constructs to impinge, sublux, and/or dislocate. Because this class of adverse biomechanical events had not lent itself well to study with existing approaches, it was necessary to develop (and validate) a series of new research methodologies, relying heavily on advanced finite element formulations. Specific areas of focus have included identifying the biomechanical challenges posed by dislocation-prone patient activities, quantifying design parameter effects and component surgical positioning effects for conventional metal-on-polyethylene implant constructs, and the impingement/dislocation behavior of non-conventional constructs, quantifying the stabilizing role of the hip capsule (and of surgical repairs of capsule defects), and systematically studying impingement and edge loading of hard-on-hard bearings, fracture of ceramic liners, confounding effects of patient obesity, and subluxation-mediated worsening of third body particle challenge.

Retrieval analysis of total joint arthroplasty components has primarily focused on assessing wear or other damage to polyethylene components. As damage to the opposing bearing surface can accelerate polyethylene wear and damage, and especially with the use of hard-on-hard articulations, retrieval analysis benefits from incorporating evaluation of hard bearing surfaces as well. The purpose of this study is to report six case studies of metal bearing surfaces with distinctive damage patterns, to interpret them in the context of adverse events plausibly responsible for their creation, and to suggest their likely clinical or scientific significance. The specific damage patterns reported here are 1) extensive scraping, 2) circumferential discoloration, 3) a long chain of periodic micro-indentations, 4) pitting with deposits, 5) scratches with small-radius directional changes, and 6) indentation with scraping.

This article addresses considerations for using a posterior (popliteal) instead of anterior (para-patellar) approach for experimental insult to the rabbit knee medial femoral condyle (MFC) surface in vivo. The posterior approach is particularly advantageous when intending to address the pathomechanisms of OA associated with habitual cartilage loading, or the efficacy of a cartilage repair method, in a clinically relevant experimental setting.Studies using anterior versus posterior approaches for such purposes in survival rabbit models of the MFC articular surface insults were systematically surveyed. The anterior-posterior span of the primary weight-bearing region of that surface was demonstrated cadaverically.Of a total of 31 papers identified in 2007-2012, an anterior approach was utilized in 28 studies (90%). More than half (17/28) explicitly regarded the cranial half (inferior aspect) of the MFC surface as being a "weight-bearing" region. The insult site through anterior approach (identified in figures) was located in the cranial half region in all cases. Cadaverically, however, the center of habitual tibio-femoral contact locations on the MFC surface was located in the caudal half region (posterior aspect) of the MFC surface. The majority of the habitual contact region was accessible only by a posterior surgical approach.For the above-noted purposes, use of a posterior (popliteal) approach, rather than an anterior approach, is highly recommended.

Dislocation remains a serious concern for total hip arthroplasty (THA). Impingement, typically between the implant femoral neck and the acetabular cup, remains the most common dislocation impetus. Wear reductions from recent bearing technology advancements have encouraged introduction of substantially increased femoral head diameters. However, there is some evidence that range of motion with larger head sizes is limited by bone-on-bone, rather than hardware, impingement. While all impingement events are of course undesirable, currently little is known biomechanically if these two impingement modes differ in terms of generation of potentially deleterious stress concentrations or with regard to dislocation resistance. Finite element (FE) analysis was therefore used to parametrically investigate the role of head diameter on the local biomechanics of bone-on-bone versus component-on-component impingement events. Of several dislocation-prone patient motion challenges considered, only squatting consistently resulted in bone-on-bone (as opposed to hardware) impingement. Implant stress concentrations arising from hardware impingement during squatting were greater than those from bony impingement, for all head sizes considered. Additionally, dislocation resistance was substantially greater for instances of bony impingement versus hardware-only impingement. These findings suggest that hardware impingement may still be a/the the predominant mode of impingement even with the use of larger femoral heads, for sub-optimally positioned cups. Additionally, the data indicate that, should impingement occur, impingements between the implant neck and cup are (1) more likely to dislocate, and (2) have a greater propensity for causing damage to the implant compared to impingement events involving bony members.

While favorable tribological properties and allowance for larger femoral head sizes have made metal-on-metal bearings an increasingly popular choice for total hip arthroplasty, concerns have mounted regarding adverse reactions to metal wear debris and ions. Metal-on-metal cups differ from conventional polyethylene cups in terms of edge profile design and reductions from full hemisphericity, suggesting differences in loading at or near the cup edge, especially during subluxation events. Finite element analysis was used to investigate the effects of cup orientation and lip edge curvature on damage propensity for edge or near-edge loading during subluxation. Increased cup lip radius (resulting in reduced articular arc) had a detrimental effect upon subluxation-free hip range of motion and upon dislocation resistance. Contact stresses near the cup edge demonstrated complex relationships between edge radius and cup orientation, with peak stresses being influenced by both variables. The tendency for scraping wear at the egress site demonstrated similarly complex dependencies. These data indicate that acetabular cup design is an important determinant of edge and near-edge loading damage propensity.

Instability is a significant concern in total hip arthroplasty, particularly when there is structural compromise of the capsule due to pre-existing pathology or due to necessities of surgical approach. An experimentally grounded fiber-direction-based finite element model of the hip capsule was developed, and was integrated with an established three-dimensional model of impingement/dislocation. Model validity was established by close similarity to results from a cadaveric experiment in a servohydraulic hip simulator. Parametric computational runs explored effects of graded levels of capsule thickness, of regional detachment from the capsule’s femoral or acetabular insertions, of surgical incisions of capsule substance, and of capsule defect repairs. Depending strongly upon the specific site, localized capsule defects caused varying degrees of construct stability compromise, with several specific situations involving over 60% decrement in dislocation resistance. Construct stability was returned substantially toward intact-capsule levels following well conceived repairs, although the suture sites involved were often at substantial risk of failure. These parametric model results underscore the importance of retaining or robustly repairing capsular structures in total hip arthroplasty, in order to maximize overall construct stability.

Investigation of femoral head osteonecrosis would benefit from an animal model whose natural history includes progression to bony collapse of a segmental necrotic lesion. The bipedal emu holds attraction for systematic organ-level study of collapse mechanopathology. One established method of experimentally inducing segmental lesions is liquid nitrogen cryoinsult. Four cryoinsult parameters – hold temperature, freeze duration, freeze/thaw repetition, and thaw duration - were investigated to determine their individual and combined effects on resulting necrotic lesion morphology. 3D distributions of histologically-apparent osteocyte necrosis from 24 emus receiving varying cryoinsults were used to develop univariate and multivariate linear regression models relating resulting necrotic lesion morphology to particular cryoinsult input parameters. These models were then applied to predict lesion size in four additional emus receiving differing input cryoinsults. The best multivariate regression model predicted lesion volumes that were accurate to better than 8% of overall emu femoral head volume. The hold temperature during cryoinsult was by far the most influential cryoinsult input parameter. The utility of this information is to enhance the consistency and predictability of cryoinsult-induced segmental lesion size for the purposes of systematic laboratory studies at the whole-organ level.

Highly comminuted intra-articular fractures are complex and difficult injuries to treat. Once emergent care is rendered, the definitive treatment objective is to restore the original anatomy while minimizing surgically induced trauma. Operations that use limited or percutaneous approaches help preserve tissue vitality, but reduced visibility makes reconstruction more difficult. A pre-operative plan of how comminuted fragments would best be re-positioned to restore anatomy helps in executing a successful reduction.In this study, the methods for virtually reconstructing a tibial plafond fracture were developed and applied to clinical cases. Building upon previous benchtop work, novel image analysis techniques and puzzle solving algorithms were developed for clinical application. Specialty image analysis tools were used to segment the fracture fragment geometries from CT data. The original anatomy was then restored by matching fragment native (periosteal and subchondral) bone surfaces to an intact template, generated from the uninjured contralateral limb.Virtual reconstructions obtained for ten tibial plafond fracture cases had average alignment errors of 0.39 (0.5 standard deviation) mm. In addition to precise reduction planning, 3D puzzle solutions can help identify articular deformities and bone loss.The results from this study indicate that 3D puzzle solving provides a powerful new tool for planning the surgical reconstruction of comminuted articular fractures.

The processes of whole-joint osteoarthritis development following localized joint injuries are not well understood. To demonstrate this local-to-global linkage, it was hypothesized that a localized osteoarticular injury in the rabbit knee would not only cause biomechanical and histological abnormalities in the involved compartment, but also concurrent histological changes in the non-involved compartment. Twenty rabbits had an acute osteoarticular injury that involved localized joint incongruity (a 2 mm osteochondral defect created in the weight-bearing area of the medial femoral condyle), while another twenty received control sham surgery. At the time of sacrifice at eight or sixteen weeks post-surgery, the experimental knees were subjected to sagittal-plane laxity measurement, followed by cartilage histo-morphological evaluation using the Mankin score. The immediate effects of defect creation on joint stability and contact mechanics were explored in concomitant rabbit cadaver experimentation. Carpal tunnel syndrome is commonly viewed as resulting from chronic mechanical insult of the median nerve by adjacent anatomical structures. Both the median nerve and its surrounding soft tissue structures are well visualized on magnetic resonance (MR) images of the wrist and hand. Addressing nerve damage from impingement of flexor digitorum tendons co-occupying the tunnel is attractive, but to date has been restricted by lack of means for making individual identifications of the respective tendons. In this image analysis work, we have developed a region-growing method to positively identify each individual digital flexor tendon within the carpal tunnel by tracking it from a more distal MR section where the respective tendon identities are unambiguous. Illustratively, the new method was applied to MRI scans from four different subjects in a variety of hand poses. Conventional shape measures yielded less discriminatory information than did evaluations of individual tendon location and arrangement. This new method of rapid identification of individual tendons will facilitate analysis of tendon/nerve interactions within the tunnel, thereby providing better information about mechanical insult of the median nerve.

Piezoresistive array pressure sensors are widely used in orthopaedic research to determine contact stress distributions across articular joint surfaces. Experience with such sensors has shown there can be inaccuracies in how the sensor perceives applied load, depending on the material stiffnesses between which it is compressed experimentally, versus in calibration. A study was undertaken to quantify the relationship between load perception of one such sensor design (Tekscan) and the stiffness of the materials between which it is compressed. A three-dimensional finite element model of a 3x3 sensel portion of the sensing matrix was formulated, along with a layer of compression test material on each side of the sensor. The elastic modulus of the test material was varied across the range representative of cartilage (12 MPa) to hard plastic (10 GPa). Using the computed contact pressure results between contacting surfaces of the sensor layers, the percentage of load passing through the active conductor intersections was determined. The results revealed that with increase of the elastic modulus of the material between which the sensor was compressed, the percentage of load on the active conductor intersections increased monotonically. The highest sensitivity of perceived loading to test material modulus (0.1%/MPa) was seen at the low end of the modulus range. The more compliant the test material, the more the sensor layers conformed around each other's geometric incongruities, the larger the true contact areas, and the higher the fraction of the total load that passed through the intermediate (non-sensing) regions between the conductors.

Intra-articular fractures can result in articular surface incongruity and joint instability, both of which can lead to posttraumatic osteoarthritis. The purpose of this study was to quantify changes in contact stresses and contact stress rates in incongruous human cadaveric ankles that were either stable or unstable. It was hypothesized that joint instability, superimposed on articular incongruity, would cause significant increases in contact stresses and contact stress rates.Intact human cadaveric ankles were subjected to quasi-physiologic stance-phase motion and loading, and instantaneous contact stresses were captured at 132 Hz. The anterior one-third of the distal part of the tibia was displaced proximally by 2.0 mm, and testing was repeated. Anterior/posterior forces were modulated during loading to cause incongruous ankles to either remain stable or become unstable during loading. Transient contact stresses and contact stress rates were measured for seven ankles under intact, stable-incongruous, and unstable-incongruous conditions. Peak and 95th percentile values of contact stress and contact stress rates for all three conditions were compared to determine the pathomechanical effects of incongruity and instability.The addition of instability caused 95th percentile and peak contact stresses to increase approximately between 20% and 25% in the unstable-incongruous specimens compared with the stable-incongruous specimens. In contrast, the addition of instability increased the magnitude of peak positive and peak negative contact stress rates by 115% and 170% in the unstable-incongruous specimens compared with the stable-incongruous specimens. Similarly, the 95th percentile contact stress rates increased 112% in the unstable-incongruous specimens compared with the stable-incongruous specimens.In human cadaveric ankles, instability superimposed on an existing articular surface incongruity causes disproportionate increases in contact stress rates compared with the increases in contact stresses.

The reliable assessment of fracture severity plays a critical part in treatment, providing essential information to guide clinical decision-making. However, current classification schemes such as the AO/OTA are constrained by limitations intrinsic to subjective categorical systems. A recently developed objective CT-based assessment methodology quantifies fracture severity by calculating the mechanical energy expended during bony fragmentation. Specifically, fracture energy is determined by comparing the bone free surface area in the fractured limb to that in the intact contralateral limb. Unfortunately, the contralateral limb is not routinely scanned in the course of fracture assessment. Consequently, fracture energy can not be obtained, since there is no datum against which to compare the fractured limb. To facilitate the application of this novel technique to large multi-center and retrospective studies where the intact contralateral CT scan is unavailable, this study aimed to establish a normative, anthropometrically scaled intact bone model to be used as a substitute datum. A mathematical model that estimated free bone surface area along the intact contralateral limb was regressed from a study group of 22 tibial plafond fracture patients. The regressed tibia model provided suitably accurate estimates of the directly measured intact surfaces areas (average error of 15%). The differences between regressed and actual bone surface areas did not ultimately affect the stratification of fracture severity, as fracture energy measures using the regressed model maintained a 0.90 concordance with the original analysis. The results from this study suggest that normative bone surface area can be incorporated into the novel CT-based objective fracture severity assessment technique.

Opthopaedic management of femoral head osteonecrosis remains problematic, partly because of inability to systematically compare treatments in an animal model whose natural history parallels the human in terms of progression to femoral head collapse. recently, it was determined that collapse could be consistently achieved for cryogenically induced osteonecrosis in the emu. toward delineating the comparative hip joint biomechanics of emus versus humans, for purposes of establishing the emu as a model for human femoral head osteonecrosis, habitual hip joint activity level was quantified for a group of seven healthy adult emus housed in an outdoor research pen typical of those used in emu farming operations. the daily number of steps taken, and the time spent with the hips loaded (standing, or squatting/sitting) versus unloaded (recumbent), were quantified from 24-hour videotape recordings, analyzed by four independent observers. The average number of steps taken per day was 9,563, which extrapolates to 1.8 million hip loadings per year, a value that falls in the same general range as seen in normal adult humans. On average, the emus spent 4:05 hours per day idly standing, 2:12 hours squatting/sitting, and 10:44 hours recumbent; they underwent an average of 37 transitions per day between the respective posture/activity states.

We developed a poroelastic finite element (FE) model of cartilage in dynamic triaxial compression to parametrically analyze the effects of loading and boundary conditions on a baseline model. Conventional mechanical tests on articular cartilage such as confined and unconfined compression, indentation, etc., do not fully allow for modulation of compression and shear at physiological levels whereas triaxial compression does. A Triaxial Compression Bioreactor, or TRIAX, has been developed to study chondrocyte responses to multi-axial stress conditions under cyclic loading. In the triaxial setting, however, a cartilage explant's physical testing environment departs from the ideal homogeneous stress state that would occur from strict linear superposition of the applied axial and transverse pressure.An axisymmetric poroelastic FE model of a cartilage explant (4 mm diameter, 1.5 mm thick) in cyclic triaxial compression was created. Axial and transverse loads (2 MPa at 1 Hz.) were applied via a platen and containment sheath. Parameters of interest included the rise time and magnitude of the applied load, in addition to the containment sheath modulus and the friction coefficient at the cartilage/platen interfaces. Metrics of interest in addition to whole explant axial strain included axial (surface normal) stress, shear stress, pore pressure, and the fluid load carriage fraction within the explant.Strain results were compared to experimental data from explants tested in the TRIAX under conditions similar to the baseline model. Explant biomechanics varied considerably over numbers of load cycles and parameter values. Cyclic loading caused an increase in accumulated strain for the various loading and boundary conditions.Unlike what would be expected from linear superposition of the homogeneous stresses from the applied axial and transverse pressure, we have shown that the stress state within the TRIAX is considerably heterogeneous. Both the boundary influences (variation in the sheath modulus and friction coefficient) and the loading history (due to poroelastic material behavior) interact in a highly nonlinear manner to influence that heterogeneity.

One of the most commonly cited reasons for retaining the posterior cruciate ligament (PCL) during total knee arthroplasty is to preserve femoral rollback and theoretically improve extensor mechanism efficiency (lengthening the moment arm). This study was undertaken to assess PCL function in this regard and to delineate the effects of joint line elevation that can be manipulated intraoperatively by the surgeon. The anterior movement of tibiofemoral contact following PCL resection at flexion angles 60 degrees demonstrated the beneficial effect of the PCL on extensor function. This anterior translation and the concomitant increases in quadriceps tendon load and patellofemoral contact pressures were consistently observed. This study demonstrated that small changes of the joint line position significantly influenced PCL strain and knee kinematics. In order to preserve the desired functions that would be lost with an overly lax PCL and to avoid the potential adverse effects of an overly tight PCL (posterior edge loading and increased tibiofemoral contact), the surgeon should make every effort to restore the preoperative joint line. If this is not possible, consideration should be given to posterior cruciate recession or use of a posterior cruciate substituting design.
