BACKGROUND: The technique that allows detecting various pathologies and clearly identifying the cause of death and other associated diseases without disturbing the integrity of the skin refers to postmortem radiological diagnostics and is called virtopsy. The number of autopsies decreases worldwide, which creates the need to use alternative non-invasive methods, including virtopsy. The main method of postmortem radiology is multislice computed tomography (CT), with its advantages including high sensitivity to bone pathology. Postmortem radiological diagnostics allow supplementing the classical autopsy. This multidisciplinary approach helps visualize a wide variety of pathologies and assist the pathologist in the diagnostic search. AIM: To assess the possibility of applying radiological diagnostic methods in the pathological anatomical practice and features of non-invasive autopsy (virtopsy) and its differences from the classical autopsy. METHODS: The archival material including macro preparations of two lower limbs with tumor-like masses of unclear genesis (the upper third of the femur and the calcaneal tuber) was used. The biomaterial was previously encased in a specially treated gelatin medium, which ensured its preservation and left parts of the CT scanner intact. Multislice CT was used for radiological assessment of the preparations. From the primary CT sequences obtained, scans with the best visualization in the bone and soft tissue windows were selected, and three-dimensional (3D) reconstruction was applied. Scans and 3D reconstructions of images of tumor-like masses of the heel and thigh bones were obtained. Interpretation of the borders of the mass, characteristics of morphology and possible source of growth of the neoplasm allowed assuming a malignant nature of the tumor, suggesting its bone origin. Further histologic examination in both cases confirmed the neoplasm (osteogenic sarcoma). RESULTS: CT scans and 3D reconstructions of images of tumour-like masses in the heel and femur were obtained. The interpretation of the boundaries of the mass, the morphological characteristics and the possible source of growth of the neoplasm suggested a malignant nature of the tumour, suggesting a bony origin. Further histological evaluation in both cases confirmed the nature of the neoplasm (osteogenic sarcoma). CONCLUSIONS: The use of postmortem radiological diagnostic elements provides a sufficiently accurate verification of the nature of some pathologies. However, the absence of tissue damage and disturbances of intact topographic characteristics play not the least role in the postmortem diagnosis. In contrast to autopsy, this technique allows to return to the original appearance of the structures examined and, if necessary, start the diagnostic search anew. Moreover, specialists from other regions can be remotely involved by sending the data of CT scanning.

IntroductionThe objective of this study was to demonstrate that BCD-057 is similar to innovator adalimumab (iADA) in terms of efficacy, safety, and pharmacokinetics in steady state in the target population of patients with moderate to severe plaque psoriasis (NCT02762955).MethodsPatients were randomized in 1:1 ratio to receive 80 mg of BCD-057 or iADA at week 0 and 40 mg thereafter every other week from week 1. At week 24 patients from iADA group were re-randomized (1:1) to continue iADA or to be switched to BCD-057. The primary efficacy endpoint was 75% improvement in Psoriasis Area and Severity Index from baseline (PASI 75), secondary endpoints included PASI percent improvement and relative change in affected Body Surface Area (BSA) from baseline at weeks 16, 24, 33, and 55. Safety was assessed through monitoring of adverse events (AEs) and antidrug antibodies. Pharmacokinetics was evaluated at steady state.ResultsOverall, 346 adult patients were included in the study (174/172 patients in BCD-057/iADA arms, respectively). At week 16 PASI 75 was achieved by 60.34% and 63.37% of patients in BCD-057 and iADA arms, respectively (p = 0.5622). Bounds of the calculated 95% confidence interval (CI) for the difference between PASI 75 responses in arms [-13.26%; 7.2%] fall within the equivalence margin [-15% to 15%] demonstrating equivalent efficacy of BCD-057 and iADA. At week 55 81.61%, 85.56%, and 80.49% of patients in BCD-057, iADA and iADA/BCD-057 arms achieved PASI 75. Comparison of the secondary endpoints did not show significant differences between arms. A comparable pharmacokinetics was shown at steady state. Safety profiles and proportions of patients with antidrug antibodies were similar between arms. The switch from the iADA to BCD-057 did not affect the immunogenicity profile.ConclusionObtained data demonstrate that BCD-057 and iADA are highly similar in clinical efficacy, pharmacokinetics, safety, and immunogenicity in patients with moderate to severe plaque psoriasis.

In this paper, an application of an artificial neural network algorithm is proposed to enhance the accuracy of temperature measurement using a fiber-optic sensor based on a Fabry–Perot interferometer (FPI). It is assumed that the interrogation of the FPI is carried out using an optical comb generator realizing a microwave photonic approach. Firstly, modelling of the reflection spectrum of a Fabry–Perot interferometer is implemented. Secondly, probing of the obtained spectrum using a comb-generator model is performed. The resulting electrical signal of the photodetector is processed and is used to create a sample for artificial neural network training aimed at temperature detection. It is demonstrated that the artificial neural network implementation can predict temperature variations with an accuracy equal to 0.018 °C in the range from −10 to +10 °C and 0.147 in the range from −15 to +15 °C.

The paper describes the design and manufacturing process of a fiber optic microphone based on a macro cavity at the end face of an optical fiber. The study explores the step-by-step fabrication of a droplet-shaped macro cavity on the optical fiber’s end surface, derived from the formation of a quasi-periodic array of micro-cavities due to the fuse effect. Immersing the end face of an optical fiber with a macro cavity in liquid leads to the formation of a closed area of gas where interfacial surfaces act as Fabry–Perot mirrors. The study demonstrates that the macro cavity can act as a standard foundational element for diverse fiber optic sensors, using the droplet-shaped end-face cavity as a primary sensor element. An evaluation of the macro cavity interferometer’s sensitivity to length alterations is presented, highlighting its substantial promise for use in precise fiber optic measurements. However, potential limitations and further research directions include investigating the influence of external factors on microphone sensitivity and long-term stability. This approach not only significantly contributes to optical measurement techniques but also underscores the necessity for the continued exploration of the parameters influencing device performance.

A new hi-accuracy method for slight-shift determination of low-resolution spectra is proposed. The method allows determining a spectrum shift with an accuracy exceeding the spectrum analyzer resolution to more than three orders of magnitude due to the mathematical post-processing. The method is based on representing the spectrum as a continuous and everywhere differentiable function; expanding it into the Taylor series; approximating all the function derivatives by finite differences of a given order. Thereafter, the spectrum shift is determined using the least-squares method. The method description, its mathematical foundation and the simulation results are given. The advantages of the application of the proposed method are shown.

The color palette, from the dramaturgical viewpoint, has a significant role in generating the emotive substance in the psychology of cinema speech perception. Here from follows the importance of the painter-scenographer’s creation who proposes the dramaturgy and temperature of the color palette as constitutive parts of the psychological condition and the rhythm of the whole film. In general, the activity of a scenographer in a fiction film includes the design and construction of the scenery, the “direction” of bcolor, light and other elements which will creatively “serve” the film dramaturgy, the characterological dynamics, time context etc.

Fiber Bragg gratings are sensitive elements in fiber optic sensor networks, and this paper discusses the practicalities of using neural network algorithms to determine their central wavelengths. The problem is to determine the central wavelength of a single sensor, the parameters of which are obtained using a low-resolution spectrum analyzer. The configuration of the neural network and the algorithm for producing the training and control datasets are specified. The training results for the selected neural network configuration demonstrated that the proposed method could determine the position of the central wavelength with a resolution two and a half orders of magnitude higher than the resolution of the input data sampling. The obtained results demonstrate that the approach makes it possible to determine the FBG central wavelength shift with an error not exceeding ~0.5 pm at a spectrum analyzer resolution of 167 pm.

The work is dedicated to a comparative analysis of the following methods for fiber Bragg grating (FBG) spectral response modeling. The Layer Sweep (LS) method, which is similar to the common layer peeling algorithm, is based on the reflectance and transmittance determination for the plane waves propagating through layered structures, which results in the solution of a system of linear equations for the transmittance and reflectance of each layer using the sweep method. Another considered method is based on the determination of transfer matrices (TM) for the FBG as a whole. Firstly, a homogeneous FBG was modeled using both methods, and the resulting reflectance spectra were compared to the one obtained via a specialized commercial software package. Secondly, modeling results of a π-phase-shifted FBG were presented and discussed. For both FBG models, the influence of the partition interval of the LS method on the simulated spectrum was studied. Based on the analysis of the simulation data, additional required modeling conditions for phase-shifted FBGs were established, which enhanced the modeling performance of the LS method.

The design and usage of the addressed combined fiber-optic sensors (ACFOSs) and the multisensory control systems of the greenhouse gas concentration on their basis are investigated herein. The main development trend of the combined fiber-optic sensors (CFOSs), which consists of the fiber Bragg grating (FBG) and the Fabry–Perot resonator (FPR), which are successively formed at the optical fiber end, is highlighted. The use of the addressed fiber Bragg structures (AFBSs) instead of the FBG in the CFOSs not only leads to the significant cheapening of the sensor system due to microwave photonics interrogating methods, but also increasing its metrological characteristics. The structural scheme of the multisensory gas concentration monitoring system is suggested. The suggested scheme allows detecting four types of greenhouse gases (CO2, NO2, CH4 and Ox) depending on the material and thickness of the polymer film, which is the FPR sensitive element. The usage of the Karhunen–Loève transform (KLT), which allows separating each component contribution to the reflected spectrum according to its efficiency, is proposed. In the future, this allows determining the gas concentration at the AFBS address frequencies. The estimations show that the ACFOS design in the multisensory system allows measuring the environment temperature in the range of −60…+300 °C with an accuracy of 0.1–0.01 °C, and the gas concentration in the range of 10…90% with an accuracy of 0.1–0.5%.

It is known that a material’s volume and the surface structuring by the nanoparticles causes a significant change in the material’s basic properties. In this aspect, the structuration of the surface of semiconductors is of interest, because their wide potential application in optoelectronics can extend the products’ transparency, hardness, wettability, and other important parameters. This paper presents possible methods for the surface modification of zinc selenide and zinc sulfide when carbon nanotubes are deposited on the surface by the application of the laser-oriented technique. It also shows changes of the spectral, mechanical, and wetting characteristics of the considered materials. Using the molecular dynamic simulations, the possible process of the carbon nanotubes penetration into the considered surfaces is presented. The simulation results are partially supported by the obtained experimental data.

Introduction. The causes of intrahepatic cholestasis include cholestatic viral hepatitis, primary biliary cirrhosis, benign recurrent cholestasis, primary sclerosing cholangitis and sepsis. During sepsis, proinflammatory cytokines and nitric oxide cause cholestasis by impairing hepatocellular and ductal bile formation. Case Outline. We report a 48-year-old woman who was admitted to hospital due to malaise, jaundice, fever and pain in the neck. Physical examination revealed jaundice, tachycardia (pulse rate was 120/min), hypotension 90/60 mm Hg. Laboratory findings showed normocytic normochromic anaemia, inflammatory syndrome and abnormal liver function tests indicating cholestasis and hepatocellular necrosis. Abdominal ultrasonography detected hepatosplenomegaly. Chest computed tomography showed bronchopneumonic infiltrates. Percutaneous liver biopsy was performed using a Menghini needle of 1.4 mm. Pathohystological analysis of the liver tissue confirmed reactive, intrahepatic cholestasis. Blood cultures isolated Staphylococcus aureus. After the diagnosis was established the treatment with broad-spectrum antibiotics was carried out, resulting in the improvement of general condition of the patient, regression of inflammatory syndrome, disappearance of cholestasis and regression of pulmonary infiltrates. Abdominal ultrasonography after antibiotic treatment did not show hepatosplenomegaly. Conclusion. Concerning patients with cholestasis of uncertain origin, we should always think of sepsis as a possible cause in order to start antibiotic treatment in time.

Background/Aim. Liver cirrhosis is a chronic, progressive disease and it is usually accompanied by portal hypertension. The development of oesophageal varices (OV) is one of the major complications of portal hypertension. Cirrhotic patients should be screened for the presence of OV when portal hypertension is diagnosed. In order to reduce the increasing burden that endoscopy units have to bear, some studies have attempted to identify parameters for noninvasive prediction of OV presence. The aim of our study was to evaluate the value of biochemical and ultrasonography parameters for prediction of OV presence. Methods. This study included 58 cirrhotic patients who underwent a complete biochemical workup, ultrasonography examination and upper digestive endoscopy. Right liver lobe diameter/albumin ratio was calculated and its correlation to the presence and degree of OV, and Child-Pugh score of liver cirrhosis explored. Results. The mean age of the patients included in the study was 53.07±13.09 years; 40 were males and 18 females. In the Child-Pugh class A were 53.4% patients, class B 39.7%, whereas 6.9% were in the class C. In 24.1% of the patients no OV were identified by upper digestive endoscopy, 19% had OV grade I, 34.5% grade II, 20.7% grade III, and 1.7% OV grade IV. The mean value of the right liver lobe diameter/ albumin ratio was 5.43±1.79 (range of 2.76−11.44). Statistically significant correlation (p < 0.01) was confirm by Spearman's test between OV grade and calculated index (ρ = 0.441). Conclusion. The right liver lobe diameter/albumin ratio is a noninvasive parameter which provides an accurate information pertinent to the determination of OV presence and their grading in patients with liver cirrhosis. .

Symmetrical drug-related intertriginous and flexural exanthema (SDRIFE) is a rare, symmetrical skin eruption triggered by various medications, predominantly beta-lactam antibiotics. We report the case of a 69-year-old male with moderate-to-severe ulcerative colitis who developed SDRIFE following the seventh intravenous administration of infliximab. The patient presented with symmetrical, pruritic erythema in the cubital and popliteal fossae, groins, gluteal and retroauricular regions without systemic involvement. Skin biopsy showed mild exocytosis of T lymphocytes in the epidermis and dense superficial perivascular CD3+ and CD4+ infiltration, consistent with a type IVa hypersensitivity reaction. The patient responded well to a regimen of systemic antihistamines, topical corticosteroids and tacrolimus ointment, with complete regression of lesions within one week. Despite mild recurrences of SDRIFE after each infliximab administration, the therapy was not discontinued due to the mild nature of the reaction and favourable prognosis. The authors are not aware of previously published cases of type IVa SDRIFE induced by infliximab. Unlike previous reports of severe type IVc SDRIFE reactions requiring discontinuation of infliximab, our case highlights the predominance of CD4+ cells, which may explain the mild clinical course. Understanding the underlying immunologic mechanisms of infliximab-induced SDRIFE could affect treatment decisions and prevent unnecessary discontinuation of effective therapies., (© 2025 British Pharmacological Society.)

Reactive oxygen species (ROS) generated during melanogenesis make melanocytes particularly vulnerable to oxidative stress, influencing their survival and melanin synthesis. Oxidative stress, significantly present in vitiligo and recently also detected in melasma, triggers inflammatory cascades and melanogenesis, making antioxidants a promising therapeutic avenue. A systematic search was conducted on Embase and Pubmed to study the efficacy of antioxidants for treating vitiligo and/or melasma. Meta-analysis was performed to assess the difference in Melasma Severity Index (MASI) scores between baseline and follow-up. Various antioxidants like polypodium leucotomos, ginkgo biloba, catalase/superoxide dismutase, and vitamin E have potential in vitiligo. For melasma, vitamin C, silymarin, and niacinamide were among those showing promise in reducing pigmentation, with vitamin C displaying significant effects in meta-analysis. Different antioxidants improve both vitiligo and melasma, with an increased minimal erythema dose (MED) following UV exposure being significant for vitiligo and tyrosinase inhibition being crucial for melasma. However, the efficacy of individual antioxidants varies, and their exact mechanisms, especially in stimulating melanocyte proliferation and anti-inflammatory pathways, require further investigation to understand better and optimize their use.

Sézary syndrome is a systemic variant of cutaneous T-cell lymphoma characterized by erythroderma, lymphadenopathy and circulating atypical lymphocytes (Sézary cells). It may present with nonspecific lesions on multiple digits. We describe an atypical case of brentuximab-induced splinter nail haemorrhages in a patient with Sézary syndrome, associated with a poor prognosis during follow-up. Concomitantly with the appearance of nail lesions, significant lymphocytosis was detected as well as infiltration of bone marrow and nail matrices. The lesions followed a precise sequence, which can be traced back to the monthly application of brentuximab and its direct cytotoxic effect on CD30+ T lymphocytes in the nail matrix. Pembrolizumab is an immune checkpoint inhibitor designed to block the interaction between programmed cell death-1 and programmed cell death-ligands 1 and 2. It shows efficacy in the treatment of patients with advanced nonsmall-cell lung cancer, among others. Side effects may involve immune-related adverse events, including vitiligo. We hereby present a 63-year-old Caucasian female with metastatic nonsmall-cell lung cancer. Immunohistochemical analysis showed programmed death-ligand 1 expression on 100% of tumour cells. The patient was eligible for immunotherapy and received pembrolizumab every 3 weeks as the first-line treatment. Three months after initiation of immunotherapy with pembrolizumab, depigmentation appeared on her upper right thoracic area of the skin overlying the affected lung lobe. Immunotherapy was generally well tolerated. Excellent response in our subject with complete remission during 16 months of follow-up potentially indicates that cutaneous immune-related adverse events, such as vitiligo, might be associated with increased efficacy of pembrolizumab in metastatic lung adenocarcinoma., (© 2020 British Pharmacological Society.)

The autoimmune basis of segmental vitiligo (SV) has only recently been recognized. Systemic autoimmune diseases are less frequently associated compared to non-segmental vitiligo (NSV), but localized skin disorders - in particular linear morphea - have been repeatedly observed in patients with SV. The inflammatory response is documented on a clinical level with cases displaying erythematous borders or a hypochromic stage, on a histopathological level with predominantly CD8 lymphocytes migrating toward the basal layer and by flow cytometry demonstrating the antimelanocyte specificity of these cytotoxic T cells. The increased risk for halo naevi and NSV in these patients further underline the immune-mediated mechanisms of SV. Nonetheless, the localized and unique distribution pattern points to somatic mosaicism. This places SV in a category of similar diseases such as lichen striatus, blaschkitis, linear lupus erythematosus, and linear scleroderma where an immune reaction against genetically mutated skin cells is believed to be the underlying cause. All these disorders are characterized by a young age of onset, a temporary disease activity with spontaneous resolution, limited response to treatment, and often long-term sequelae. When working with dermatology patients, the question sometimes arises which diagnostic tests and tools should be used for workup, particularly in cases of chronic urticaria (CU) and discoid nummular eczema, where the treatment of associated systemic diseases and infections may be crucial for patient outcome. The aim was to investigate retrospectively the influence of associated diseases on skin disease outcomes based on medical records of CU and nummular eczema patients in comparison to controls. We included patients admitted to our Dermatology Department over a 6-year period and analyzed their laboratory findings, related factors and outcomes recorded after two years of workup and treatment. Compared to controls, CU patients had a significantly higher prevalence of positive Helicobacter (H.) pylori findings (p=0.020), confirmed allergies (p=0.006), increased IgE (p=0.011) and pathologic thyroid findings (p=0.049), whereas nummular eczema patients only had significantly higher positive H. pylori findings (p=0.046). Meaningful regression of both dermatoses was recorded after treatment of associated diseases, with significant benefit from H. pylori treatment. This indicated that the diagnosis of associated infections (particularly H. pylori and urogenital infections), confirmed allergies, endocrine disorders (particularly of thyroid gland in CU patients) and serum malignancy markers could play a crucial role, as their treatment may improve disease outcomes.

Since the working medical personnel including dentists and dental technicians mainly use their hands, it is understandable that the most common occupational disease amongst medical personnel is contact dermatitis (CD) (80%-90% of cases). Development of occupational CD is caused by contact of the skin with various substances in occupational environment. Occupational etiologic factors for dental personnel are foremost reactions to gloves containing latex, followed by various dental materials (e.g., metals, acrylates), detergents, lubricants, solvents, chemicals, etc. Since occupational CD is relatively common in dental personnel, its timely recognition, treatment and taking preventive measures is needed. Achieving skin protection at exposed workplaces is of special importance, as well as implementing necessary measures consequently and sufficiently, which is sometimes difficult to achieve. Various studies have shown the benefit of applying preventive measures, such as numerous protocols for reducing and managing latex sensitivity and other forms of CD in dentistry. Active involvement of physicians within the health care system, primarily dermatologists, occupational medicine specialists and general medicine doctors is needed for establishing an accurate medical diagnosis and confirmation of occupational skin disease.

The precise regulation of synapse maintenance is critical to the development and function of neuronal circuits. Using an in vivo RNAi screen targeting the Drosophila kinome and phosphatome, we identify 11 kinases and phosphatases controlling synapse stability by regulating cytoskeletal, phospholipid, or metabolic signaling. We focus on casein kinase 2 (CK2) and demonstrate that the regulatory (β) and catalytic (α) subunits of CK2 are essential for synapse maintenance. CK2α kinase activity is required in the presynaptic motoneuron, and its interaction with CK2β, mediated cooperatively by two N-terminal residues of CK2α, is essential for CK2 holoenzyme complex stability and function in vivo. Using genetic and biochemical approaches we identify Ankyrin2 as a key presynaptic target of CK2 to maintain synapse stability. In addition, CK2 activity controls the subcellular organization of individual synaptic release sites within the presynaptic nerve terminal. Our study identifies phosphorylation of structural synaptic components as a compelling mechanism to actively control the development and longevity of synaptic connections.

Croatian dermatovenerological society of the Croatian Medical Association formed the working group which consists of leading experts for psoriasis in Croatia. After a critical analysis of relevant scientific papers, the working group has developed guidelines for the diagnosis and treatment of psoriasis.

Olea europea, the olive tree, is an ancient tree that originates from the Mediterranean environment of Asia Minor. The edible olive fruit is also used for its oil, gained by the process of pressing, a nutrient with proven beneficial effects. Virgin olive oil is the natural juice of the olive fruit, which plays a major role in the healthy Mediterranean diet. The source of its health effects are the biophenols and squalenes (oleocanthal, tyrosol, hydroxytyrosol, oleuropein) it contains. They provide an exceptional antioxidative activity, removing harmful compounds from the body. Oxidants are essential in the genesis of many diseases and conditions, such as cardiovascular disorders, cancer, osteoporosis, Alzheimer disease, and premenstrual syndrome. Oleic acid, an unsaturated fatty acid, has demonstrated a significant effect in the prevention of malignant diseases such as colon cancer and breast cancer. Biophenols from olive oil successfully suppress the synthesis of LDL, a protein that is crucial in the development of cardiovascular disease, by reducing blood pressure and the development of atherosclerotic plaques. In addition, there is strong evidence of the antimicrobic effect of the biphenols from olive oil that successfully destroy colonies of microorganisms which may cause respiratory tract, intestinal, and genital tract infections.

Neural development requires both synapse elaboration and elimination, yet relatively little is known about how these opposing activities are coordinated. Here, we provide evidence Hts/Adducin can serve this function. We show that Drosophila Hts/Adducin is enriched both pre- and postsynaptically at the NMJ. We then demonstrate that presynaptic Hts/Adducin is necessary and sufficient to control two opposing processes associated with synapse remodeling: (1) synapse stabilization as determined by light level and ultrastructural and electrophysiological assays and (2) the elaboration of actin-based, filopodia-like protrusions that drive synaptogenesis and growth. Synapse remodeling is sensitive to Hts/Adducin levels, and we provide evidence that the synaptic localization of Hts/Adducin is controlled via phosphorylation. Mechanistically, Drosophila Hts/Adducin protein has actin-capping activity. We propose that phosphorylation-dependent regulation of Hts/Adducin controls the level, localization, and activity of Hts/Adducin, influencing actin-based synapse elaboration and spectrin-based synapse stabilization.
