2. Contextual Constraints on the Comprehension of 'Before' and 'After'.

Abstract

The "contextual hypothesis" of French and Brown (1977) concerning children's acquisition of temporal terms was tested. French and Brown claimed that it would be impossible for children to learn the meaning of temporal terms except by hearing them used in contexts where they referred to already known sequences, and further proposed that the terms would be understood in such contextually supported settings earlier than in settings where they established an order between inherently unordered events. Subjects were sixteen 3- and sixteen 4-year-old children. To assess subjects' understanding of the terms "before" and "after" 16 stories were composed that described activities with which young children could be assumed to be familiar. Half of the stories described activities having a more or less invariant real-world order. The remaining stories described activities that were familiar to young children, but that had no inherent real-world order constraints. The experimenter read each story and placed the picture corresponding to the sentence being read in front of the child. The order of the pictures corresponded to the order of events in the story. Following the presentation of each story the subject was asked what happened either before or after the third event. Results are discussed. (Author/RH)

4. Ictal haemodynamic changes in a patient affected by "subtle" Epilepsia Partialis Continua.

Abstract

We report on a 64 year-old woman presenting with Epilepsia Partialis Continua (EPC) affecting the left hand since the age of 24 without neurological deficit. Structural MRI showed a region of focal cortical dysplasia (FCD) over the right central gyrus and lesions in the mesial frontal and occipital cortex secondary to perinatal hypoxic injury. Ictal spike haemodynamic mapping using simultaneous EEG-fMRI revealed significant BOLD signal changes prominent in the region of FCD (larger cluster), occipital cortex (global statistical maximum), prefrontal cortex and cerebellum. The cluster over FCD was in good agreement with the result of EEG source analysis. Our findings provide an interesting illustration of the ability of EEG-fMRI to reveal epileptogenic networks confirming the intrinsic epileptogenic properties of dysplastic neurons. 6. Distribution of plant communities, ecological strategy types and diversity along a moisture gradient

Abstract

The influence of water regime on the zonation of wetland communities, distribution patterns of plant species, ecological strategies and biodiversity, was studied in this work. Vegetation of the herbaceous wetland was sampled along a transect, in accordance with the standard central European method. Water level was also measured. The changes in plant species composition, plant functional types (ecological strategies, life forms) and diversity were studied. Classification of vegetation relevés enabled the definition of five grassland communities. The same pattern remained in DCA ordination also when the species matrix was replaced with a matrix of ecological strategies. Relevés were always segregated into distinct plant communities and distributed along the moisture gradient in the same way. Biodiversity is strongly negatively correlated with moisture–it is decreasing with decreasing ground level. Plant ecological strategy types (C-S-R) change significantly along the transect as well. In communities thriving on the driest sites (Triseto-Centaureetum), there are three times more C- than C-S-strategists, while in the wettest sites (Caricetum elatae) C-S-strategists predominate over C-strategists by almost two times. At the same time R-and C-R-strategists, as well as therophytes, which are present in drier sites almost disappear in the wettest sites. We show that the moisture gradient essentially influences the distribution pattern of plant communities, species diversity, and manifestation of certain ecological strategy types.

7. Comparison of general practitioners and rheumatologists' prescription patterns for patients with knee osteoarthritis

Abstract

Abstract Background To compare the prescription modalities of general practitioners (GPs) and rheumatologists (RHs) for symptomatic knee osteoarthritis (OA) and to determine correlates with prescription of low-dose NSAIDs. Methods This observational, prospective, national survey was carried out among a national representative sample of GPs (n = 808) and RHs (n = 134). Each physician completed a medical questionnaire for the 2 most recent patients fulfilling the ACR criteria for knee OA. Results GPs and RHs included 1,570 and 251 patients, respectively. Mean pain level of the knee (on a VAS, 0-100 mm) was greater for GP patients than for RH patients (49.8 ± 16.3 vs. 46.2 ± 17.1 mm, respectively; p < 0.01). As compared with patients of RHs, those of GPs more frequently had another joint affected by OA: 71.2% vs. 63.7% (p < 0.0001) and more often had hypertension and diabetes mellitus (p < 0.05). As compared with RHs, GPs more frequently prescribed low-dose NSAIDs (p < 0.0001), oral NSAIDs (p < 0.05), and topical NSAIDs (p < 0.0001) but less frequently symptomatic slow-acting drugs for OA (p < 0.01). Moreover, GPs more frequently recommended rehabilitation (p < 0.01) and loss of weight (p < 0.0001). Logistic regression analysis revealed an association of low-dose NSAIDs prescription and prescription by GPs, prescription of topical NSAIDs, no prescription of oral NSAIDs or coxibs and no intra-articular injection of steroids. Conclusions This study identified speciality-related variability in some aspects of the management of knee OA. The clinical profile of patients with knee OA differed between GPs and RHs.
