Key Clinical Message Numerous techniques for provisionally replacing a single tooth at the aesthetic zone while planning for future dental implant placement, while soft and hard tissues heal, are critical in restorative treatment strategy. The available materials, simplicity, cost, and impact on the potential implant location should be considered when evaluating the treatment choices. Abstract Replacement of a single anterior tooth in the aesthetic zone while planning for future implant placement is crucial to the restorative treatment plan. Several methods exist for immediate provisionalisation of the extracted tooth while waiting for soft and hard tissue healing. Although there's a myriad of possible provisionalisation methods available, each option has its advantages and disadvantages. The treatment options should weigh various factors such as the available materials, ease of fabrication, costs, and the effect on the future implant site. This article describes three clinical cases demonstrating a simple yet cost‐effective technique to temporarily replace an extracted single anterior tooth, enhancing patient satisfaction and increasing compliance before receiving the definitive implant restoration. The pro and cons for each treatment option available as opposed to the technique involved in the three cases used are also described.

Abstract Background Chronic pruritus (CP), itch lasting more than 6 weeks, is common in community‐dwelling older adults. Understanding their illness perception allows the attending physician to develop a personalised care plan to mitigate CP. Aim This study explores the illness perception of CP among older Asian adults in an urban community. Design Qualitative research was conducted, framed by the Common‐Sense Model of self‐regulation (CSM). Through in‐depth interviews (IDIs), qualitative data were gathered from Asian patients with CP, and then a thematic analysis was carried out. The emergent themes were grouped according to the five domains of CSM: ‘identity’, ‘cause’, ‘time’, ‘controllability’ and ‘consequence’. Setting and Patients IDIs were conducted in a Singapore public primary care clinic before the data were saturated. Results The CSM domains illustrate the illness perception of CP. CP was identified as a ‘problem’ rather than a disease and was often described in metaphor. Patients' perception of the cause was diverse due to the lack of provision of a clear explanation by their physicians. They opined that CP continued indefinitely. Without definite time to resolution, patients adapted their help‐ and health‐seeking behaviours to control it. The consequences included therapeutic experimentation, alternative therapy, self‐isolation, avoidance behaviours, emotional disturbance and dermatological complications. Conclusion and Patient Contribution Patients provided information on their perception of CP, which aligned with the CSM. A multipronged approach is needed to deliver holistic and personalised care to patients with CP, providing clarity on its natural progression, to set their expectations on its timeline, treatment effectiveness and undertake appropriate behaviour modification to adapt to its chronicity.

Shiga toxin-producing Escherichia coli (STEC) are important enteric pathogens responsible for sporadic cases and outbreaks of gastroenteritis. E.coli O157:H7/NM (STEC O157) are the most commonly known STEC serotypes but it is now increasingly apparent that non-O157 STEC serotypes have been underreported in the past because they were not part of routine screening in many front-line laboratories. The Canadian Public Health Laboratory Network (CPHLN) has identified the need for improved detection and surveillance of non-O157 STEC and has developed the following recommendations to assist in the decision-making process for clinical and reference microbiology laboratories. These recommendations should be followed to the best of a laboratory's abilities based on the availability of technology and resources. The CPHLN recommends that when screening for the agents of bacterial gastroenteritis from a stool sample, front-line laboratories use either a chromogenic agar culture or a culture-independent diagnostic test (CIDT). CIDT options include nucleic acid amplification tests (NAATs) to detect Shiga toxin genes or enzyme immunoassays (EIAs) to detect Shiga toxins. If either CIDT method is positive for possible STEC, laboratories must have a mechanism to culture and isolate STEC in order to support both provincial and national surveillance as well as outbreak investigations and response. These CPHLN recommendations should result in improved detection of STEC in patients presenting with diarrhea, especially when due to the non-O157 serotypes. These measures should enhance the overall quality of healthcare and food safety, and provide better protection of the public via improved surveillance and outbreak detection and response. Background: Despite being a common problem, long-term disability following stroke often improves after survivors receive regular rehabilitative therapy. This study aimed to assess the oral health-related quality of life (OHRQoL) of stroke survivors who had access to a community-based rehabilitation centre after hospital discharge. Methods: Dentate post-stroke patients receiving regular rehabilitative care with slight to moderate dependency (Barthel Index >70) and without severe cognitive impairment were involved in this study. The OHRQoL parameters were measured using the Oral Health Impact Profile-14 (OHIP-14) and EuroQol five dimensions (EQ-5D-5L) questionnaires. Results: Thirty one patients were recruited in this study. Majority of the patients hardly ever or never had problems in all functional, physical, psychological and social domains, including eating discomfort and having to avoid eating, difficulty in chewing food and presumably having bad breath (58.1%, 61.3% and 71.0% respectively). Other than gender, age, ethnicity, co-morbidities and oral hygiene practices, education level was the only variable found to significantly affect patients’ OHRQoL (p = 0.028). Most of the patients had no or slight problem(s) in self-care (87.1%) and carrying out usual activities (57.1%), pain/discomfort (3.2%) and anxiety/depression (6.5%). Conclusion: Stroke survivors under regular rehabilitative care perceived have good oral health-related quality of life.

Abstract Background Female sexual dysfunction (FSD) is increasingly being identified as a problem around the world. Women can have problems in various parts of the sexual cycle - desire, arousal, lubrication, orgasm or they may experience pain related to sexual activity. The only study involving Singapore with regard to sexual dysfunction in women, the Asian Global Studies of Sexual Attitudes and Behaviours in 2002, reported that Singapore had one of the lowest age-standardised sexual dysfunction rates of 32% compared with other Asian countries. This pilot study aims to evaluate the prevalence of female sexual dysfunction and to investigate the independent significant risk factors among allied health workers in a tertiary hospital in Singapore. Methods A cross-sectional study where an anonymous questionnaire which included 19 questions in the FSFI (Female Sexual Function Index) was distributed to all allied health workers in a tertiary hospital in Singapore aged between 18 to 70 years old. Results Three hundred thirty completed questionnaires were involved in analysis. 56.0% of women were found to have sexual dysfunction. A significant difference was found in the prevalence of FSD when comparing nurses to other allied health staff, where nurses had a decreased risk of developing FSD. Age was not found to be a significant risk factor in our study. Respondents below 40 years of age had significantly lower satisfaction scores than those above 40. Indians and Filipinos were found to have lower scores than the Chinese and Malay respondents in the lubrication (p = 0.02) and pain domains (p = 0.02). Conclusion A significant proportion our female allied health workers suffer from sexual dysfunction. In this study, we found that the overall prevalence was independent of age, race and marital status. Nurses had a lower risk of developing FSD. We will need further studies to assess the prevalence of female sexual dysfunction in the general population, to evaluate the independent significant risk factors for developing FSD, in addition to classical risk factors, as well as to assess the psychological impact of this condition and whether people would be willing to seek help for such problems.

As clinical laboratories transition to using culture-independent detection test (CIDT) panels for cases of acute gastroenteritis, culture of clinical specimens is becoming less common. The reduction in bacterial cultures available for public health activities is expected to hinder surveillance and outbreak response by public health laboratories at the local, provincial, national and international levels. These recommendations are intended to serve as guidelines for the implementation of CIDT panels in frontline laboratories in Canada. The United States of America has already seen a significant reduction in culture of stool specimens despite the Association of Public Health Laboratories recommendation to perform reflex culture on positive CIDT specimens. Priority public health organisms addressed in these Canadian guidelines include Shiga toxin-producing Escherichia coli, Shigella and Salmonella and, under regional circumstances, other organisms such as Campylobacter jejuni/coli and Yersinia enterocolitica. These recommendations suggest active engagement between primary diagnostic laboratories and provincial public health laboratories to determine the workflow and protocols for reflex or parallel culture. Consequently, notifiable disease definitions will also need modification, with consultation of all stakeholders. This report describes the clinical and technical aspects in the oral rehabilitation of an edentulous patient with knife-edge ridge at the mandibular anterior edentulous region, using implant-retained overdentures. The application of computer-aided design and computer-aided manufacturing (CAD/CAM) in the fabrication of the overdenture framework simplifies the laboratory process of the implant prostheses. The Nobel Procera CAD/CAM System was utilised to produce a lightweight titanium overdenture bar with locator attachments. It is proposed that the digital workflow of CAD/CAM milled implant overdenture bar allows us to avoid numerous technical steps and possibility of casting errors involved in the conventional casting of such bars.

In the crystal structure of the title compound, [Sn(C7H6Cl)2Cl2(C12H8N2)], the SnIV atom is chelated by the N-heterocycle and the metal atom exists in a trans-C2SnCl2N2 distorted octahedral coordination environment.

The Sn atom in the title compound, [Sn(C7H7)2(C5H5N2O)Cl], shows a distorted C2ClNOSn trigonal-bipyramidal coordination, with a Cl–Sn–O axial angle of 163.77 (3)°, but the C—Sn—C angle [141.43 (7)°] deviates from 120°. The chelating ligand exists in a zwitterionic form. Adjacent molecules are linked by an N—Hpyridinium...O hydrogen bond, forming a chain running along the c axis of the orthorhombic unit cell.

The asymmetric unit of the crystal structure of the title compound, [Sn(C7H6Cl)2Cl2(C20H14N4)], contains two independent molecules. Each SnIV atom is chelated by the bipyridine-like N-heterocycle and exists in a distorted trans-C2SnCl2N2 octahedral coordination environment. One chlorobenzyl substituent is disordered in each molecule in 0.5:0.5 and 0.778 (2):0.222 (2) ratios.

The asymmetric unit of the title compound, C15H15ClN4O6S, contains two independent molecules, in which the pyrimidine and benzene rings are oriented at dihedral angles of 75.21 (3) and 86.00 (3)°. Intramolecular N—H...N and C—H...O hydrogen bonds result in the formation of two five- and two six-membered rings. The six-membered rings have flattened-boat conformations, while the five-membered rings adopt envelope conformations. In December 2018, an outbreak of Salmonella Enteritidis infections was identified in Canada by whole-genome sequencing (WGS). An investigation was initiated to identify the source of the illnesses, which proved challenging and complex. Microbiological hypothesis generation methods included comparisons of Salmonella isolate sequence data to historical domestic outbreaks and international repositories. Epidemiological hypothesis generation methods included routine case interviews, open-ended centralized re-interviewing, thematic analysis of open-ended interview data, collection of purchase records, a grocery store site visit, analytic comparison to healthy control groups, and case-case analyses. Food safety hypothesis testing methods included food sample collection and analysis, and traceback investigations. Overall, 83 cases were identified across seven provinces, with onset dates from 6 November 2018 to 7 May 2019. Case ages ranged from 1 to 88 years; 60% (50/83) were female; 39% (22/56) were hospitalized; and three deaths were reported. Brand X profiteroles and eclairs imported from Thailand were identified as the source of the outbreak, and eggs from an unregistered facility were hypothesized as the likely cause of contamination. This study aims to describe the outbreak investigation and highlight the multiple hypothesis generation methods that were employed to identify the source.

An investigation into an outbreak of Salmonella Newport infections in Canada was initiated in July 2020. Cases were identified across several provinces through whole-genome sequencing (WGS). Exposure data were gathered through case interviews. Traceback investigations were conducted using receipts, invoices, import documentation, and menus. A total of 515 cases were identified in seven provinces, related by 0-6 whole-genome multi-locus sequence typing (wgMLST) allele differences. The median age of cases was 40 (range 1-100), 54% were female, 19% were hospitalized, and three deaths were reported. Forty-eight location-specific case sub-clusters were identified in restaurants, grocery stores, and congregate living facilities. Of the 414 cases with exposure information available, 71% (295) had reported eating onions the week prior to becoming ill, and 80% of those cases who reported eating onions, reported red onion specifically. The traceback investigation identified red onions from Grower A in California, USA, as the likely source of the outbreak, and the first of many food recall warnings was issued on 30 July 2020. Salmonella was not detected in any tested food or environmental samples. This paper summarizes the collaborative efforts undertaken to investigate and control the largest Salmonella outbreak in Canada in over 20 years.

Shiga toxin-producing Escherichia coli (STEC) can cause severe clinical disease in humans, particularly in young children. Recent advances have led to greater availability of sequencing technologies. We sought to use whole genome sequencing data to identify the presence or absence of known virulence factors in all clinical isolates submitted to our laboratory from Southern Alberta dated 2020-2022 and correlate these virulence factors with clinical outcomes obtained through chart review. Overall, the majority of HUS and hospitalizations were seen in patients with O157:H7 serotypes, and HUS cases were primarily in young children. The frequency of virulence factors differed between O157:H7 and non-O157 serotypes. Within the O157:H7 cases, certain virulence factors, including espP , espX1 , and katP , were more frequent in HUS cases. The number of samples was too low to determine statistical significance.

Two patients with acute gastroenteritis tested positive for Shiga toxin-producing Escherichia coli (STEC) by polymerase chain reaction (PCR), and both strains carried the Shiga toxin 2 encoding gene. Since routine culture using CHROMagar STEC failed to recover these isolates, immunomagnetic separation (IMS) targeting the top six non-O157:H7 serotypes was used for isolate recovery. After two subsequent IMS runs, the STEC strains were isolated from trypticase soy broth with and without overnight enrichment for runs 1 and 2, respectively. Serotyping based on whole-genome sequencing revealed that both patients carried the strain O166:H15 STEC with the stx2 gene. Hence, the magnetic beads used in IMS appeared to have cross-reactivity with other E. coli serotypes. When the STEC isolates from both stools were cultured on CHROMagar STEC and sheep blood agar (BAP), two distinct colony sizes were apparent after overnight incubation. The small and large colonies were picked and separately cultured on both media, and colony growth was observed for 2 weeks at room temperature after an initial overnight incubation at 37°C. After 1 week, the colonies showed concentric ring structures with a darker center and a lighter surrounding on CHROMagar STEC and a "fried egg"-resembling structure with a raised circular center and a flat surrounding on BAP. Both colony types remained morphologically different on CHROMagar STEC throughout the 15 days. However, on BAP, their appearance was comparable by day 7., Importance: Shiga toxin-producing E. coli (STEC) infections can lead to severe complications such as bloody diarrhea and hemolytic uremic syndrome (HUS), especially in young children and the elderly. Strains that carry the shiga toxin 2 gene ( stx2 ), such as O157:H7, have been mostly linked with severe disease outcomes. In recent years, outbreaks caused by non-O157:H7 strains have increased. E. coli O166:H15 has been previously reported causing a gastroenteritis outbreak in 1996 as a non-STEC strain, however the O166:H15 serotype we recovered carried the stx2 gene. It was particularly challenging to isolate this strain from stools by culture. Consequently, we tested immunomagnetic separation for the STEC recovery, which was a novel approach on clinical stools.
