Abstract

Background Frailty assessment has acquired an increasing importance in recent years and it has been demonstrated that this vulnerable profile predisposes elderly patients to a worse outcome after surgery. Therefore, it becomes paramount to perform an accurate stratification of surgical risk in elderly undergoing emergency surgery. Study design 1024 patients older than 65 years who required urgent surgical procedures were prospectively recruited from 38 Italian centers participating to the multicentric FRAILESEL (Frailty and Emergency Surgery in the Elderly) study, between December 2016 and May 2017. A univariate analysis was carried out, with the purpose of developing a frailty index in emergency surgery called "EmSFI". Receiver operating characteristic curve analysis was then performed to test the accuracy of our predictive score. Results 784 elderly patients were consecutively enrolled, constituting the development set and results were validated considering further 240 consecutive patients undergoing colorectal surgical procedures. A logistic regression analysis was performed identifying different EmSFI risk classes. The model exhibited good accuracy as regard to mortality for both the development set (AUC = 0.731 [95% CI 0.654-0.772]; HL test chi(2) = 6.780; p = 0.238) and the validation set (AUC = 0.762 [95% CI 0.682-0.842]; HL test chi(2) = 7.238; p = 0.299). As concern morbidity, our model showed a moderate accuracy in the development group, whereas a poor discrimination ability was observed in the validation cohort. Conclusions The validated EmSFI represents a reliable and time-sparing tool, despite its discriminative value decreased regarding complications. Thus, further studies are needed to investigate specifically surgical settings, validating the EmSFI prognostic role in assessing the procedure-related morbidity risk.

The aim of this study was to analyze the correlation between acute psychiatric hospitalizations of adolescents and selected variables to highlight risk factors for psychiatric emergencies. Methods: This retrospective research was conducted in the acute psychiatric public ward, Service of Psychiatric Diagnosis and Treatment (SPDT), and in the residential facility for adolescents, “The Medlar”, located in Modena. The sample was constituted by all adolescent patients (n=101, age range 14–18) who had acute hospitalizations (n=140) in SPDT and had been successively transferred to “The Medlar” (n=83), from February 2, 2010 to January 31, 2015. From clinical charts, we extracted demographic and anamnestic characteristics of patients and clinical variables related to hospitalizations. Data were statistically analyzed. Results: Sixty-one percent of our patients lived with one divorced parent, with adoptive or immigrant family, or in institutions; 51% had experienced stressful events during childhood; 81% had a normal intellective level, but only 6% presented regular school performance. Abstract

Triple quadrupole mass spectrometry (QqQ) is the most popular and widely used technique used to analyze pesticide residues in food and environmental compartments. However, recent advances in quadrupole-time-of-flight mass spectrometry (Q-TOF) may lead to this approach becoming a useful tool for analysis of pesticide residues. In this work, results of QqQ and Q-TOF were compared to determine their accuracy, when determining the concentration of herbicides and their metabolites in water. Double distilled and river water were therefore spiked with 18 analytes, differing in physical–chemical properties, and then directly analyzed either by QqQ and Q-TOF mass spectrometry. Results of Q-TOF were comparable to QqQ. The latter, operating in tandem mass spectrometry, was superior in terms of compound coverage, LOQ and precision for most analytes. However, results between the two analysis methods were comparable for LOD, linearity range (4 orders of magnitude) and accuracy. Validation parameters in pure and surface water showed contrasting results. Overall, the results of this study suggest that Q-TOF may represent a valid alternative to triple quadrupole, although the specific mass spectrometer under use still plays a major role.

5. A Transmissive Theory of Brain Function: Implications for Health, Disease, and Consciousness.

Abstract

Identifying a complete, accurate model of brain function would allow neuroscientists and clinicians to make powerful neuropsychological predictions and diagnoses as well as develop more effective treatments to mitigate or reverse neuropathology. The productive model of brain function, which has been dominant in the field for centuries, cannot easily accommodate some higher-order neural processes associated with consciousness and other neuropsychological phenomena. However, in recent years, it has become increasingly evident that the brain is highly receptive to and readily emits electromagnetic (EM) fields and light. Indeed, brain tissues can generate endogenous, complex EM fields and ultraweak photon emissions (UPEs) within the visible and near-visible EM spectra. EM-based neural mechanisms, such as ephaptic coupling and non-visual optical brain signaling, expand canonical neural signaling modalities and are beginning to disrupt conventional models of brain function. Here, we present an evidence-based argument for the existence of brain processes that are caused by the transmission of extracerebral, EM signals and recommend experimental strategies with which to test the hypothesis. 6. Blood-Based Biomarkers for Alzheimer's Disease in Older Adults with Posttraumatic Stress Disorder.

Abstract

Background: Posttraumatic stress disorder (PTSD) is associated with cognitive decline and risk for dementia, but the neuropathology involved is unclear., Objective: The aim of this study was to determine whether PTSD is associated with increased levels of Alzheimer's disease (AD) blood-based biomarkers., Methods: Individuals aged 50 years and older with PTSD were compared to trauma-exposed healthy controls (TEHCs) at baseline on serum measures of amyloid- β (A β ) 42 and 40 levels, the A β 42 /A β 40 ratio, and total tau. Serum was analyzed using ultrasensitive Simoa Human Neurology 3-Plex A assay (N3PA). Linear regressions modeling each AD biomarker as a function of group were used to investigate between-group differences, controlling for age, sex, and educational attainment (years)., Results: TEHC participants ( N  = 26) were 53.8% male with mean age 66.8±10.7, whereas PTSD participants ( N  = 44) were 47.7% male and aged 62.5±9.1 years. No between-group differences were noted on demographic characteristics or cognitive performance measured with the NIH Toolbox Cognition Battery. There were no significant between-group differences in serum A β 40 (TEHC 105.8±51.6 versus PTSD 93.2±56.1, p  = 0.46), A β 42 (TEHC 8.1±4.6 versus PTSD 7.8±4.6, p  = 0.63), A β 42 /A β 40 (TEHC 0.08±0.03 versus PTSD 0.09±0.03, p  = 0.27), or total tau (TEHC 0.5±0.3 versus PTSD 0.5±0.4, p  = 0.77). Likewise, there were no significant interaction effects of amyloid or tau serum concentrations and PTSD group status on cognitive functioning., Conclusion: Findings from cognitive assessments and serum analyses do not support PTSD-induced neurodegeneration of the Alzheimer's type as a pathway linking PTSD to increased incidence of dementia in older adults., Competing Interests: The authors have no conflicts of interest to report., (© 2022 – The authors.
