International audience; We report the first case of visceral leishmaniasis (VL) caused by Leishmania martiniquensis in the Caribbean, which until now, was known only to cause cutaneous leishmaniasis. The disease presented with fatigue, anemia, and hepatosplenomegaly in a 61-year-old man with human immunodeficiency virus (HIV) infection who was receiving antiretroviral therapy. Diagnosis was made by bone marrow biopsy. VL is life-threatening, and its emergence in the Caribbean is of concern.

Introduction: The hospital-home transition is an important step in the journey of patientcare. This statement is even more valid for patients leaving an intensive care unit, who may then be the result of a serious pathology. The objective of our work was to identify the difficulties encountered by attending physicians following the direct discharge of their patient from the intensive care unit of the University Hospital of Guadeloupe to the home. The secondary objectives were to look for areas for improvement proposed by the doctors, as well as the impact of the fire encountered by the CHU on these difficulties. Materials and methods: We carried out a retrospective study on the doctors of patients who had been in the intensive care unit of the University Hospital of Guadeloupe. The patients concerned had left the service between 27 November 2016 and 28 November 2018, i.e. 1 year before and after the fire. A questionnaire was submitted to the doctors interviewed via a Google Forms’slink. Results: After exclusion, 60 files were analyzed for a total of 56 physicians interviewed. We obtained a response rate of 51. 8%. Of the 29 responses received, 8 reported a lack of information on the transition to resuscitation. Only 34. 5% of doctors had the hospitalization report at the time of their consultation. The decision to return home should have been discussed with them for only 17. 2% of caregivers and 68. 9% considered the discharge to be appropriate. For the improvement proposals, 19 of the doctors based them on communication; including systematic contact at the entry or exit of patients. There is little difference between the period before and after the fire. However, physicians were satisfied with the discharge conditions of their patients at 25% before the fire, compared to 47. 1% after the fire. Discussion and conclusion: Although there appears to be an improvement in the level of physician satisfaction after the fire, doctors still reveal difficulties in communication between the intensive care unit and the city. Systematic contact with the attending physician is one of the main suggestions for improvement. The organization of an exit consultation process is also one of the areas for improvement., Introduction : La transition hôpital-domicile est une étape importante dans le parcours de soins des malades. Cette affirmation est d’autant plus valable pour les patients sortant d’un service de réanimation, pouvant alors être consécutive à une pathologie grave. L’objectif de notre travail a alors été d’identifier les difficultés rencontrées par les médecins traitants suite à la sortie directe de leur patient du service de réanimation du CHU de la Guadeloupe vers le domicile. Les objectifs secondaires étaient de rechercher les pistes d’amélioration proposées par les médecins, ainsi que l’impact de l’incendie rencontré par le CHU sur ces difficultés. Matériel et méthodes : Nous avons réalisé une étude rétrospective au sujet des médecins des patients passés dans le service de réanimation du CHU de Guadeloupe. Les patients concernés étaient sortis du service entre le 27 novembre 2016 et le 28 novembre 2018 soit 1 an avant et après l’incendie. Un questionnaire était soumis aux médecins interrogés par le biais d’un lien Google Forms. Résultats : Après exclusion, 60 dossiers ont été analysés pour un total de 56 médecins interrogés. Nous avons obtenu un taux de réponse de 51,8%. Sur les 29 réponses obtenues, 8 notifiaient un manque d’informations sur le passage en réanimation. Seuls 34,5% des médecins disposaient du compte rendu d’hospitalisation au moment de leur consultation. La décision du retour au domicile aurait dû être abordée avec eux pour seuls 17, 2% des soignants et 68,9 % jugeaient la sortie adaptée. Pour les propositions d’amélioration, 19 des médecins les basaient sur la communication ; avec notamment un contact systématique à l’entrée ou la sortie des patients. On retrouve peu de différence entre la période avant et après l’incendie. Les médecins étaient cependant satisfaits des conditions de sortie de leurs patients à 25% avant l’incendie, contre 47,1% après l’incendie.Discussion et conclusion : Bien qu’il semblerait y avoir une amélioration du niveau de satisfaction des médecins après l’incendie, ceux-ci révèlent encore des difficultés liées à la communication entre le service de réanimation et la ville. Un contact systématique du médecin traitant fait partie des principales propositions d’amélioration de ces derniers. L’organisation d’une filière de consultation de sortie fait également partie des pistes d’amélioration.
