This study introduces a distributed electrified heating approach that is able to innovate chemical engineering involving endothermic reactions. It enables rapid and uniform heating of gaseous reactants, facilitating efficient conversion and high product selectivity at specific equilibrium. Demonstrated in catalyst-free CH 4 pyrolysis, this approach achieves stable production of H 2 (530 g h -1 L reactor ) and carbon nanotube/fibers through 100% conversion of high-throughput CH -1 at 1150 °C, surpassing the results obtained from many complex metal catalysts and high-temperature technologies. Background: Home care is beset with work environment issues and high staff turnover, while research concerned with interventions to improve the work environment is sparse. Few of the existing interventions apply a participative approach, despite this being associated with more positive outcomes and sustainable change. This paper presents a framework, rooted in action research and action learning, for participatory work environment interventions in home care, and demonstrates how this framework has been implemented in four Swedish home care organizations., Methods: The framework has three phases (pre-intervention, intervention planning and intervention implementation) and consists of cycles of action and reflection in three constellations: a group of researchers, a reference group with labour market organization representatives and home care managers, and intervention work groups in the home care organizations. The work was documented and analysed with focus on the realization of the framework and challenges that were met on the way. The interventions were evaluated using a pre-/post-test questionnaire design., Results: Parts of the framework were successfully implemented. The pre-intervention phase and the intervention planning phase, with intervention work groups, worked well. All four groups identified one intervention relevant to their own context. However, only two of the proposed interventions were fully implemented and evaluated. The high staff and management turnover, and the high rate of organizational changes made it impossible to evaluate the interventions statistically. Yet, data from open-ended questions in the post questionnaire showed that the two implemented interventions were perceived as successful., Conclusions: The participatory framework, presented in this paper, seems promising for work environment interventions in home care. The framework was designed to reduce the risk of known disturbances affecting the process in unstable organizations. Despite this, it proved challenging to execute the framework, and especially the interventions, due to changes happening at high speed. In the two cases where organizational changes were not dominating, the interventions were implemented successfully. While the prerequisites for participation and successful implementation could be improved somewhat, the main issue, the instability of the organizational context, is hard for researchers or the individual home care units to tackle alone., (© 2022. This paper models the energy and emissions scenarios for a circular economy based clean energy transitions in a 140,000-population town in China, taking into account the new situation encountered by the COVID-19 pandemic. The modelled scenarios propose new clean energy transition roadmaps towards a sustainable urban system through the implementation of circular economy strategies. This is represented by the cascading use of industrial excess heat to form symbiosis between factories and to cover the growing building heat demand, as well as by the electrification of the transport sector and reusing the batteries for a second life as energy storage devices. The results show that for a circular economy scenario, during 2020-2040, an accumulated saving of 7.1 Mtoe final energy use (34%), a decline in 14.5 Mt CO 2 emissions (40%) and 592 t PM 2.5 emissions (43%) could be achieved compared with the business-as-usual scenario. The outcomes of the circular economy strategies are at least 7% better than the new policy scenario which simply has energy efficiency improvements. The outbreak of the COVID-19 tremendously impacts the socio-economic activities in the town. If taking the pandemic as an opportunity to enhance the circular economy, by 2040, compared with the scenario without introducing circular economy measures, the extra avoided final energy use, CO 2 emissions and PM 2.5 emissions could be 1.6 Mtoe (8%), 3.8 Mt (11%) and 229 t (17%) respectively., Competing Interests: The authors declare that they have no known competing financial interests or personal relationships that could have appeared to influence the work reported in this paper., (© 2021 The Author(s).)

Recent studies have found positive effects from hospital focus on both quality and cost. Some studies indicate that certain patient segments benefit from focus, while others have worse outcomes in focused hospital departments. The aim of this study was to establish the relationship between hospital focus and performance in elective surgery. We studied obesity surgery procedures performed in Sweden in 2016 (5152 patients), using data from the Scandinavian Obesity Surgery Registry (SOReg) complemented by a survey of all clinics that performed obesity surgery. We examined focus at two levels of the organization: hospital level and department level. We hypothesized that higher proportions of obesity surgery patients in the hospital, and higher proportions of obesity surgery procedures in the department, would be associated with better performance. These hypotheses were tested using multilevel regression analysis, while controlling for patient characteristics and procedural volume. We found that focus was associated with improved outcomes in terms of reduced complications and shorter procedure times. These positive relationships were present at both hospital and department level, but the effect was larger at the department level. The findings imply that focus is a viable strategy to improve quality and reduce costs for patients undergoing elective surgery. For these patients, general hospitals should consider implementing organizationally separate units for patients undergoing elective surgery.

Objective: The aim of this study was to evaluate and compare the abilities of clinicians and clinical prediction models to accurately triage emergency department (ED) trauma patients. We compared the decisions made by clinicians with the Revised Trauma Score (RTS), the Glasgow Coma Scale, Age and Systolic Blood Pressure (GAP) score, the Kampala Trauma Score (KTS) and the Gerdin et al model., Design: Prospective cohort study., Setting: Three hospitals in urban India., Participants: In total, 7697 adult patients who presented to participating hospitals with a history of trauma were approached for enrolment. The final study sample included 5155 patients. The majority (4023, 78.0%) were male., Main Outcome Measure: The patient outcome was mortality within 30 days of arrival at the participating hospital. A grid search was used to identify model cut-off values. Clinicians and categorised models were evaluated and compared using the area under the receiver operating characteristics curve (AUROCC) and net reclassification improvement in non-survivors (NRI+) and survivors (NRI-) separately., Results: The differences in AUROCC between each categorised model and the clinicians were 0.016 (95% CI -0.014 to 0.045) for RTS, 0.019 (95% CI -0.007 to 0.058) for GAP, 0.054 (95% CI 0.033 to 0.077) for KTS and -0.007 (95% CI -0.035 to 0.03) for Gerdin et al . The NRI+ for each model were -0.235 (-0.37 to -0.116), 0.17 (-0.042 to 0.405), 0.55 (0.47 to 0.65) and 0.22 (0.11 to 0.717), respectively. The NRI- were 0.385 (0.348 to 0.4), -0.059 (-0.476 to -0.005), -0.162 (-0.18 to -0.146) and 0.039 (-0.229 to 0.06), respectively., Conclusion: The findings of this study suggest that there are no substantial differences in discrimination and net reclassification improvement between clinicians and all four clinical prediction models when using 30-day mortality as the outcome of ED trauma triage in adult patients., Trial Registration Number: ClinicalTrials.gov Registry (NCT02838459)., Competing Interests: Competing interests: None declared., (© Author(s) (or their employer(s)) 2020. Bibliometric indicators are increasingly used to evaluate individual scientists-as is exemplified by the popularity of the many other publication and citation-based indicators used in evaluation. These indicators, however, cover at best some of the quality dimensions relevant for assessing a researcher: productivity and impact. At the same time, research quality has more dimensions than productivity and impact alone. As current bibliometric indicators are not covering various important quality dimensions, we here contribute to developing better indicators for those quality dimensions not yet addressed. One of the quality dimensions lacking valid indicators is an individual researcher's independence. We propose indicators to measure different aspects of independence: two assessing whether a researcher has developed an own collaboration network and two others assessing the level of thematic independence. Taken together they form an independence indicator. We illustrate how these indicators distinguish between researchers that are equally productive and have a considerable impact.
