Abstract

Summary: Background: The protection of fourth dose mRNA vaccination against SARS-CoV-2 is relevant to current global policy decisions regarding ongoing booster roll-out. We aimed to estimate the effect of fourth dose vaccination, prior infection, and duration of PCR positivity in a highly-vaccinated and largely prior-COVID-19 infected cohort of UK healthcare workers. Methods: Participants underwent fortnightly PCR and regular antibody testing for SARS-CoV-2 and completed symptoms questionnaires. A multi-state model was used to estimate vaccine effectiveness (VE) against infection from a fourth dose compared to a waned third dose, with protection from prior infection and duration of PCR positivity jointly estimated. Findings: 1298 infections were detected among 9560 individuals under active follow-up between September 2022 and March 2023. Compared to a waned third dose, fourth dose VE was 13.1% (95% CI 0.9 to 23.8) overall; 24.0% (95% CI 8.5 to 36.8) in the first 2 months post-vaccination, reducing to 10.3% (95% CI −11.4 to 27.8) and 1.7% (95% CI −17.0 to 17.4) at 2–4 and 4–6 months, respectively. Relative to an infection >2 years ago and controlling for vaccination, 63.6% (95% CI 46.9 to 75.0) and 29.1% (95% CI 3.8 to 43.1) greater protection against infection was estimated for an infection within the past 0–6, and 6–12 months, respectively. A fourth dose was associated with greater protection against asymptomatic infection than symptomatic infection, whilst prior infection independently provided more protection against symptomatic infection, particularly if the infection had occurred within the previous 6 months. Duration of PCR positivity was significantly lower for asymptomatic compared to symptomatic infection. Interpretation: Despite rapid waning of protection, vaccine boosters remain an important tool in responding to the dynamic COVID-19 landscape; boosting population immunity in advance of periods of anticipated pressure, such as surging infection rates or emerging variants of concern. Abstract

La lectura de Hombres de caminos (1988), novela del escritor piurano Miguel Gutiérrez Correa, ha transitado por juicios valorativos generales; no existe, sin embargo, un estudio crítico profundo, a pesar de que han pasado más de tres décadas desde su publicación. En este artículo, se postula que esta novela establece una continuidad de la violencia colonial encarnada en la figura/imagen del bandolero/bandolerismo. Para comprobar esta hipótesis, en primer lugar, se explicará que dicha continuidad se perpetúa como núcleo temático a partir de la narración de la gesta bandoleril en Piura, de tal modo que la novela va a construir su narrativa a partir del movimiento del sujeto mestizo bandolero del siglo XIX hacia un sujeto mestizo indígena de los inicios de la Colonia (o viceversa). En dicho desplazamiento, se evidenciará que el discurso de la lucha de clases que se asoma a partir de la gesta del bandolerismo es en realidad una prolongación del discurso de las razas del siglo XVI. En segundo lugar, a nivel de estructura, se demostrará que la continuidad se establece a través de un encadenamiento narrativo que simula las “polémicas de las Indias” y las tensiones entre la oralidad que se llevaron a cabo durante la época colonial.

3. Training of community health workers to assist in surveillance of child development

Abstract

Studies indicate that training in the surveillance of child development can aid in the identification and/or prevention of deficiencies. The more people involved (family members, caregivers, health professionals, and educators), the better the quality of child development. The objective of this project was to promote health surveillance of the development of children in primary care, in association with family health teams in São Carlos, by means of the training of community health workers (CHWs) and occupational therapy graduate students at the Federal University of São Carlos. The extension activity was based on meaningful learning, divided into three phases. Firstly, the researchers taught the students how to use the booklet "Every time, it's time to care." In the second phase, students undertook the training of CHWs, and in the third phase, the CHWs will use the contents of the booklet to work with families in their areas. The first two phases of the project have so far been completed. Problem-solving workshops used the booklet as supporting material, and considered nine key themes: care for pregnant women, bonds and affection, vaccination schedule, child development, nutrition, hygiene, management of childhood illness, accident prevention, and rights of the child. These important issues encountered in everyday life enabled the participants to identify the themes with aspects of their personal lives, and to connect theory with practice. The problem of prompt detection of problems related to early childhood care was also highlighted. The final phase of the work is being completed, and is expected to help to enhance the empowerment of families.

Resumo O objetivo desta revisão integrativa foi conhecer a produção científica brasileira sobre a temática da qualidade de vida familiar (QdVF) de pessoas com deficiências, assim como a população participante, o referencial teórico e os instrumentos utilizados nos estudos. Foi realizado um levantamento nas bases de dados BVS, SciELO, PubMed, e no portal de Periódicos da Capes, cujos critérios de inclusão foram artigos publicados no período de 2007 a 2018, disponíveis na íntegra e online, publicado em português ou inglês e que retratassem a temática da QdVF de pessoas com deficiência e artigos oriundos de estudos realizados em contextos brasileiros. Os resultados apontaram que apenas 3 dos 19 artigos selecionados exploraram diretamente o tema da QdVF incluindo aspectos relacionados ao construto; também apontaram que o WHOQOL-Bref foi o instrumento mais utilizado para avaliar a qualidade de vida (n=11), seguido por entrevistas semiestruturadas (n=5) e que apenas 10,5%, ou seja, dois artigos utilizaram a definição apropriada de QdVF. Constatou-se com o estudo uma incipiência da temática na produção científica nacional, o uso expressivo de construtos individuais de qualidade de vida e escassa presença de instrumentos de medidas específicos para mensurar QdVF. Abstract

Zebrafish spontaneously regenerate the retina after injury. Although the gene expression profile has been extensively studied in this species during regeneration, this does not reflect protein function. To further understand the regenerative process in the zebrafish, we compared the proteomic profile of the retina during injury and upon regeneration. Using two-dimensional difference gel electrophoresis (2D-DIGE) and label-free quantitative proteomics (quadrupole time of flight LC-MS/MS), we analysed the retina of adult longfin wildtype zebrafish at 0, 3 and 18 days after Ouabain injection. Gene ontology analysis indicates reduced metabolic processing, and increase in fibrin clot formation, with significant upregulation of fibrinogen gamma polypeptide, apolipoproteins A-Ib and A-II, galectin-1, and vitellogenin-6 during degeneration when compared to normal retina. In addition, cytoskeleton and membrane transport proteins were considerably altered during regeneration, with the highest fold upregulation observed for tubulin beta 2 A, histone H2B and brain type fatty acid binding protein. Key proteins identified in this study may play an important role in the regeneration of the zebrafish retina and investigations on the potential regulation of these proteins may lead to the design of protocols to promote endogenous regeneration of the mammalian retina following retinal degenerative disease.
