Introducción: La salud mental en la población es variable según la coyuntura social que se viva, y el personal de salud no es la excepción; estamos viviendo una pandemia por COVID-19 y esto influye negativamente en la salud mental del personal médico y la población en general. Objetivos: Describir el estado de la salud mental de los residentes que ejercieron labor asistencial tras el levantamiento del estado de emergencia. Métodos: Estudio de tipo descriptivo transversal correlacional. Se incluyo a 216 médicos residentes que ejercieron labor asistencial tras el levantamiento del estado de emergencia de un total de 615. Se utilizó los cuestionarios GAD-7, cuestionario PHQ-9 y cuestionario EIE-R; para ansiedad, depresión y estrés respectivamente. Resultados: Se obtuvo una prevalencia de enfermedad para ansiedad del 50.9%, depresión del 50.9% y estrés del 70.4%, así mismo, el 43.5% presentan una ansiedad leve, 7.5% entre moderada y severa; por otro lado, el 33.3% presentan depresión leve y 17.6% entre moderada y severa, mientras que, el 38.0% presentan TEPT parcial, el 25.9% con probable diagnóstico de TEPT y sólo un 6.5% sintomatología de TEPT severa. Conclusiones: Se encuentra una alta prevalencia de los trastornos, lo que concuerda con la encontrada en otros estudios. A su vez, no se encontró una relación estadísticamente significativa para aseverar una posible relación entre los trastornos o los niveles de los mismas con las características demográficas descritas. Background: The mental health in the population varies according to the social context, this is not different in health personnel; we are now in the middle of COVID-19 Pandemic, and this has a negative impact in the mental health of health personnel and the general population. Objective: Describe the mental health among residents in a public hospital after the end of the public health emergency of COVID-19 pandemic. Methods: A cross-sectional descriptive correlational study. We include 216 medical residents that had care work after the end of the public health emergency of a total of 615 residents. The GAD-7, PHQ-9 questionnaires and the IES-R scale were utilized to address anxiety, depression and distress respectively. Results: Regarding prevalence of mental health symptoms, it was found that 50.9% of interviewed residents had anxiety symptoms, 50.9% had depression symptoms and 70.4% suffer from distress symptoms. Moreover, 43.5% presented mild, 5.6% moderate and 1.9% severe anxiety; on the other hand, 33.3% presented mild depression and 17.6% between moderate and severe, while 38.0% presented partial PTSD, 25.9% with a probable diagnosis of PTSD and only 6.5% with severe PTSD symptomatology. Conclusions: A high prevalence of anxiety, depression and distress symptoms were found between medical residents. This matches with other revised studies. To conclude, no statistical significant relation was found between the presence of mental health disorder symptoms or their levels and the sociodemographic characteristics of medical residents.

Introduction: The purpose of this study was to clarify whether blood-flow restriction during resting intervals [resting blood-flow restriction (rBFR)] is comparable to a continuous BFR (cBFR) training regarding its effects on maximum strength, hypertrophy, fatigue resistance, and perceived discomfort.Materials and Methods: Nineteen recreationally trained participants performed four sets (30-15-15-15 repetitions) with 20% 1RM on a 45° leg press twice a week for 6 weeks (cBFR, n = 10; rBFR, n = 9). Maximum strength, fatigue resistance, muscle thickness, and girth were assessed at three timepoints (pre, mid, and post). Subjective pain and perceived exertion were determined immediately after training at two timepoints (mid and post).Results: Maximum strength (p < 0.001), fatigue resistance (p < 0.001), muscle thickness (p < 0.001), and girth (p = 0.008) increased in both groups over time with no differences between groups (p > 0.05). During the intervention, the rBFR group exposed significantly lower perceived pain and exertion values compared to cBFR (p < 0.05).Discussion: Resting blood-flow restriction training led to similar gains in strength, fatigue resistance, and muscle hypertrophy as cBFR training while provoking less discomfort and perceived exertion in participants. In summary, rBFR training could provide a meaningful alternative to cBFR as this study showed similar functional and structural changes as well as less discomfort.

Purpose: Medically recommended training often faces the dilemma that necessary mechanical intensities for muscle adaptations exceed patients' physical capacity. In this regard, blood flow restriction (BFR) training is becoming increasingly popular because it enables gains in muscle mass and strength despite using low-mechanical loads combined with external venous occlusion. Since the underlying mechanisms are still unknown, we applied invasive measurements during exercise with and without BFR to promote physiological understanding and safety of this popular training technique. Methods: In a randomized cross-over design, ten healthy men (28.1 ± 6.5 years) underwent two trials of unilateral biceps curls either with (BFR) and without BFR (CON). For analysis of changes in intravascular pressures, blood gases, oximetry and electrolytes, an arterial and a venous catheter were placed at the exercising arm before exercise. Arterial and venous blood gases and intravascular pressures were analyzed before, during and 5 min after exercise. Results: Intravascular pressures in the arterial and venous system were more increased during exercise with BFR compared to CON (p < 0.001). Furthermore, arterial and venous blood gas analyses revealed a BFR-induced metabolic acidosis (p < 0.05) with increased lactate production (p < 0.05) and associated elevations in [K+], [Ca2+] and [Na+] (p < 0.001). Conclusion: The present study describes for the first time the local physiological changes during BFR training. While BFR causes greater hypertension in the arterial and venous system of the exercising extremity, observed electrolyte shifts corroborate a local metabolic acidosis with concurrent rises in [K+] and [Na+]. Although BFR could be a promising new training concept for medical application, its execution is associated with comprehensive physiological challenges.

Die vorliegende Arbeit beschäftigt sich mit der Frage, ob die allgemeingültigen Regeln des Strafrechts geeignet sind, um fahrlässige Körperverletzungs- und Tötungsdelikte im Bereich der medizinischen Behandlungen zu verfolgen und ggf angemessen zu sanktionieren. Dazu wird ein interdisziplinärer Ansatz gewählt, der den strafrechtlichen Fahrlässigkeitsbegriff im Licht der Fehlerforschung, einem Bereich der Verhaltenspsychologie, der Human-Performance-Lehre und der Organisationsstruktur und Arbeitsrealität der Gesundheitsberufe darstellt. Im ersten Teil werden die grundlegenden Überlegungen aus dem Bereich der Human-Factors- und der Fehlerforschung behandelt. Da eine strafrechtliche Verfolgung nur nach einem Ereignis erfolgt, ergeben sich typische Fallstricke einer dadurch zwingenden Ex-post-Betrachtung der Geschehnisse, die zum zu untersuchenden Ereignis geführt haben. Alle Handlungen werden immer im Wissen des Ausganges (des strafrechtlichen „Erfolges“) beurteilt. Trotz der immer wieder geforderten Ex-ante-Betrachtung an die Staatsanwaltschaften, Strafgerichte und Sachverständigen, zeigt dieser Teil, dass es ohne Wissen um moderne Fehleranalysemethoden unmöglich ist, eine reale Ex-ante-Betrachtung durchzuführen. Im zweiten Teil werden die Arbeitsrealität und die dem medizinischen Bereich anhaftenden Besonderheiten dargestellt. Es wird gezeigt, wie sich aus den gewachsenen Organisationsstrukturen zwangsläufig Fehler- und Gefahrenneigungen ergeben und welchen Einfluß die physiologischen und psychologischen Grenzen der menschlichen Leistungsfähigkeit darauf haben. Der dritte Teil beschäftigt sich mit den Auswirkungen der Erkenntnisse der ersten zwei Teile auf die verschiedenen Bereiche (Tatbestand, Rechtswidrigkeit, Schuld, Strafausschließungsgründe und Sanktionen) der strafrechtlichen Beurteilung eines medizinischen Ereignisses. Das österreichische Strafrecht hat sich zu einem Verschuldensstrafrecht entwickelt. Strafe hat nicht mehr einfach einen bloßen Vergeltungscharakter. Strafe enthält ein sozialethisches Unwerturteil über einen Täter. Der Vorwurf richtet sich gegen den Täter, der sich anders hätte verhalten können. In § 4 ist der Grundsatz "keine Strafe ohne Schuld" (nulla poena sine culpa) verankert. Dieses sog. Schuldprinzip hat folgende Auswirkungen auf die unmittelbare Rechtsanwendung: Schuld ist Voraussetzung für Strafe Das Maß der Strafe darf das Maß der Schuld nicht überschreiten. Die Schuld ist sohin Grund und Grenze jeder Strafe. Das Strafrecht verfolgt mit der Strafe einen verhaltenssteuernden Effekt auf die Allgemeinheit, die Generalprävention. Die Strafe wird angedroht und verhängt, um andere Menschen von der Begehung entsprechender Taten abzuschrecken (= negative Generalprävention). Durch die Durchsetzung des Strafrechts kommt es zu einer Verstärkung des Vertrauens der Gesellschaft in die Rechtsordnung (= positive Generalprävention). Aus empirisch-kriminologischer Sicht zielt die Spezialprävention auf die Gefährlichkeit des Täters selbst ab. Unterschieden werden positive und negative Spezialprävention. Ziel der positiven Spezialprävention ist die Besserung des Täters und seine Resozialisierung. Der Schutz der Gesellschaft vor dem Täter und die Verhinderung einer neuerlichen Tat, ist die Aufgabe der negativen Spezialprävention. Es soll daher im dritten Teil diskutiert werden, inwieweit die Ziele des Strafrechts im Bereich medizinischer Fehler erreichbar und umsetzbar sind. Aufbauend auf die im dritten Teil aufgeworfenen Probleme des Strafrechts in der Beurteilung von medizinischen Sachverhalten, werden als Abschluss im vierten Teil mögliche alte und neue Lösungsansätze für das Medizinstrafrecht diskutiert. eingereicht von Ing.
