Hintergrund: Ein kontinuierliches bundesweites Gesundheitsmonitoring ist wichtig, um das Wohlbefinden von Kindern und Jugendlichen im Blick zu behalten und Entwicklungsverläufe abzubilden. Anhand der Ergebnisse von 3 ausgewählten epidemiologischen Studien werden Entwicklungen zum kindlichen Wohlbefinden der letzten 20 Jahre vorgestellt. Methodik: Datengrundlage bilden (1.) die bevölkerungsbezogene BEfragung zum seeLischen WohLbefinden und VerhAlten (BELLA-Studie, 2003–2017, N = 1500–3000), die ein Modul der KiGGS-Studie ist, (2.) die COrona und PSYche Studie (COPSY, 2020–2022, N = 1600–1700), die auf der BELLA-Studie aufbaut, und (3.) die internationale Health-Behaviour in School-aged Children Studie (HBSC, 2002–2018, N = 4300–7300). Das Wohlbefinden wurde bei 7‑ bis 17-Jährigen mittels der Indikatoren gesundheitsbezogene Lebensqualität (KIDSCREEN-10), Lebenszufriedenheit (Cantril Ladder) und psychische Auffälligkeiten (Strenghts and Difficulties Questionnaire (SDQ), Screen for Child Anxiety Related Emotional Disorders (SCARED) und Center for Epidemiological Studies Depression Scale for Children (CES-DC)) erfasst. Ergebnisse: Insgesamt zeigen Kinder und Jugendliche präpandemisch (2002–2018) eine konstant hohe gesundheitsbezogene Lebensqualität und eine hohe allgemeine Lebenszufriedenheit, die sich mit Beginn der COVID-19-Pandemie 2020 zunächst verschlechterte. 2 Jahre später zeigen sich Verbesserungen, die jedoch noch nicht das Ausgangsniveau erreichen. Psychische Auffälligkeiten, ängstliche und depressive Symptome nahmen mit Pandemiebeginn um bis zu 12 Prozentpunkte zu und zeigen auch 2 Jahre nach Pandemiebeginn noch höhere Werte als präpandemische Studien. Diskussion: Die Epidemiologie kindlichen Wohlbefindens bietet eine notwendige Datengrundlage, um den Unterstützungsbedarf von Kindern und Jugendlichen zu erfassen und auf dieser Basis Maßnahmen der Gesundheitsförderung, Prävention und Intervention zu entwickeln.

Mental health problems in children and adolescents are associated with individual and family-related constraints as well as social costs. 20.0% of children and adolescents showed mental health problems at the KiGGS baseline study (2003-2006). This study investigates the current prevalence for KiGGS Wave 2 (2014-2017) as well as time trends in comparison with the KiGGS baseline study. Mental health problems were assessed for 3- to 17-year-old children and adolescents by using the parent-based version of the Strengths and Difficulties Questionnaire (SDQ). For KiGGS Wave 2, the prevalence of mental health problems was 16.9%. A decreasing trend is pronounced particularly among boys between 9 and 17 years of age. Mental health problems are displayed more frequently by girls and boys from families with a low socioeconomic status compared to their peers from families with a medium or high socioeconomic status. These findings are discussed in the light of various measures and actions in health promotion and health care.

Hintergrund: Die mit der COVID-19-Pandemie einhergehenden Veränderungen und Kontaktbeschränkungen können das psychische Wohlbefinden von Kindern und Jugendlichen beeinflussen. Ziel der Arbeit: COPSY ist die erste deutschlandweite repräsentative Studie, welche die psychische Gesundheit und Lebensqualität von Kindern und Jugendlichen während der Pandemie untersucht. Die Ergebnisse werden mit denen der repräsentativen longitudinalen BELLA-Studie aus der Zeit vor der Pandemie verglichen. Material und Methoden: Vom 26.05. bis zum 10.06.2020 wurden n = 1586 Eltern mit 7 bis 17-jährigen Kindern und Jugendlichen, von denen n = 1040 11- bis 17-Jährige auch Selbstangaben machten, befragt. Dabei wurden international etablierte Instrumente zur Erfassung von gesundheitsbezogener Lebensqualität, psychischen Auffälligkeiten, Ängstlichkeit und depressiven Symptomen eingesetzt. Die Daten wurden mittels deskriptiver Statistiken und bivariater Tests ausgewertet. Ergebnisse: 71 % der Kinder und Jugendlichen und 75 % der Eltern fühlten sich durch die erste Welle der Pandemie belastet. Im Vergleich zu der Zeit vor der Pandemie gaben die Kinder und Jugendlichen eine geminderte Lebensqualität an, der Anteil von Kindern und Jugendlichen mit psychischen Auffälligkeiten hat sich in etwa verdoppelt und ihr Gesundheitsverhalten hat sich verschlechtert. Sozial benachteiligte Kinder erlebten die Belastungen durch die Pandemie besonders stark. Zwei Drittel der Eltern wünschten sich Unterstützung im Umgang mit ihrem Kind. Diskussion: Die COVID-19-Pandemie führt zu einer psychischen Gesundheitsgefährdung der Kinder und Jugendlichen, auf die präventiv mit niedrigschwelligen und zielgruppenspezifischen Angeboten in der Schule, in der ärztlichen Praxis und in der Gesellschaft im Sinne des Kinderschutzes reagiert werden sollte., Background: The drastic changes during the COVID-19 pandemic may have a negative impact on the psychological wellbeing of children and adolescents. Objectives: COPSY is the first national, representative German study to examine mental health and quality of life of children and adolescents during the pandemic. Results are compared with data of the representative longitudinal BELLA study conducted before the pandemic. Materials and methods: Internationally established instruments for measuring health-related quality of life and mental health (including anxiety and depressive symptoms) were administered to n = 1586 parents with 7‑ to 17-year-old children and adolescents, of whom n = 1040 11- to 17-year-olds also provided self-reports, from 26 May to 10 June 2020. Data were analyzed using descriptive statistics and bivariate tests. Results: Seventy-one percent of the children and adolescents and 75% of the parents felt burdened by the first wave of the COVID-19 pandemic. Compared to the time before the pandemic, the children and adolescents reported a lower health-related quality of life, the percentage of children and adolescents with mental health problems almost doubled, and their health behavior worsened. Socially disadvantaged children felt particularly burdened by the COVID-19 pandemic. Two-thirds of the parents would like to receive support in coping with their child during the pandemic. Conclusions: The COVID-19 pandemic poses a mental health risk to children and adolescents. Schools, doctors, and society are called to react by providing low-threshold and target-group-specific prevention and mental health promotion programs.

- Abstract
Background: The COVID-19 pandemic has disrupted the lives of children and adolescents worldwide. The German COPSY study is among the first population-based longitudinal studies to examine the mental health impact of the pandemic. The objective of the study was to assess changes in health-related quality of life (HRQoL) and mental health in children and adolescents and to identify the associated risk and resource factors during the pandemic. Methods: A nationwide longitudinal survey was conducted with two waves during the pandemic (May/June 2020 and December 2020/January 2021). In total, n = 1923 children and adolescents aged 7 to 17 years and their parents participated (retention rate from wave 1 to wave 2: 85%). The self-report and parent-proxy surveys assessed HRQoL (KIDSCREEN-10), mental health problems (SDQ with the subscales emotional problems, conduct problems, hyperactivity, and peer problems), anxiety (SCARED), depressive symptoms (CES-DC, PHQ-2) and psychosomatic complaints (HBSC-SCL). Mixed model panel regression analyses were conducted to examine longitudinal changes in mental health and to identify risk and resource factors. Results: The HRQoL of children and adolescents decreased during the pandemic, and emotional problems, peer-related mental health problems, anxiety, depressive and psychosomatic symptoms increased over time, however the change in global mental health problems from wave 1 to wave 2 was not significant, and some changes were negligible. Socially disadvantaged children and children of mentally burdened parents were at particular risk of impaired mental health, while female gender and older age were associated with fewer mental health problems. A positive family climate and social support supported the mental health of children and adolescents during the pandemic. Background: This rapid review examines changes in the mental health of the German child and adolescent population during the COVID-19 pandemic. Methods: The basis are 39 publications, which were identified by means of systematic literature search (until 19.11.2021) and manual search. The databases of the included publications were systematized with regard to their representativeness for the general population, and the indicators used were categorized with regard to the depicted constructs and their reliability. Results: The large majority of the studies took place at the beginning of the pandemic until the summer plateau 2020. Representative studies mainly reported high levels of pandemic-related stress, increases in mental health problems, and negative impacts on the quality of life. Non-representative studies showed mixed results. Vulnerable groups could only be identified to a limited extent. Both routine and care-related data showed declines in the outpatient and inpatient service utilisation during the various waves of the pandemic followed by catch-up effects. Children and adolescents turned out to be more vulnerable during the pandemic compared to adults, but their stress levels varied with the waves of the pandemic and the related containment measures. Conclusions: A future forward-looking crisis and pandemic management requires a close-knit and continuous surveillance of the mental health of children as well as an improved identification of risk groups.

The German National Cohort (NAKO) is a multidisciplinary, population-based prospective cohort study that aims to investigate the causes of widespread diseases, identify risk factors and improve early detection and prevention of disease. Specifically, NAKO is designed to identify novel and better characterize established risk and protection factors for the development of cardiovascular diseases, cancer, diabetes, neurodegenerative and psychiatric diseases, musculoskeletal diseases, respiratory and infectious diseases in a random sample of the general population. Between 2014 and 2019, a total of 205,415 men and women aged 19–74 years were recruited and examined in 18 study centres in Germany. The baseline assessment included a face-to-face interview, self-administered questionnaires and a wide range of biomedical examinations. Biomaterials were collected from all participants including serum, EDTA plasma, buffy coats, RNA and erythrocytes, urine, saliva, nasal swabs and stool. In 56,971 participants, an intensified examination programme was implemented. Whole-body 3T magnetic resonance imaging was performed in 30,861 participants on dedicated scanners. NAKO collects follow-up information on incident diseases through a combination of active follow-up using self-report via written questionnaires at 2–3 year intervals and passive follow-up via record linkages. All study participants are invited for re-examinations at the study centres in 4–5 year intervals. Thereby, longitudinal information on changes in risk factor profiles and in vascular, cardiac, metabolic, neurocognitive, pulmonary and sensory function is collected. NAKO is a major resource for population-based epidemiology to identify new and tailored strategies for early detection, prediction, prevention and treatment of major diseases for the next 30 years.

Im Zuge der Anerkennung psychischer Gesundheit als wesentlicher Bestandteil der Bevölkerungsgesundheit wird am Robert Koch-Institut eine Mental Health Surveillance (MHS) für Deutschland aufgebaut. Ziel der MHS ist die kontinuierliche Berichterstattung relevanter Kennwerte zum Zweck einer evidenzbasierten Planung und Evaluation von Public-Health- Maßnahmen. Zur Entwicklung eines Indikatorensets für die erwachsene Bevölkerung wurden potenzielle Indikatoren durch eine systematische Literaturrecherche identifiziert und in einem strukturierten Konsentierungsprozess durch internationale und nationale Expertinnen und Experten beziehungsweise Stakeholder ausgewählt. Das finale Set enthält 60 Indikatoren, die ein mehrdimensionales Public-Health-Rahmenkonzept psychischer Gesundheit in vier Handlungsfeldern abbilden. Im fünften Handlungsfeld „Gesundheitsförderung und Prävention“ besteht Bedarf der Indikatorenentwicklung. Die pilotierte Methodik erwies sich als praktikabel. Diskutiert werden Stärken und Limitationen der Recherche und Definition von Indikatoren, der Umfang des Indikatorensets sowie die partizipative Entscheidungsfindung. Nächste Schritte zur Etablierung der MHS bestehen in der Operationalisierung von Indikatoren und der Erweiterung unter anderem auf das Kindes- und Jugendalter. Bei gesicherter Datenverfügbarkeit wird die MHS zum Wissen über die Gesundheit der Bevölkerung beitragen und die gezielte Förderung psychischer Gesundheit sowie die Verringerung der Krankheitslast psychischer Störungen unterstützen.

Der vorliegende Rapid Review untersucht, wie sich die psychische Gesundheit von Erwachsenen in der Allgemeinbev��lkerung in Deutschland w��hrend der COVID-19-Pandemie ver��ndert hat. Mittels systematischer Literaturrecherche wurden zum Stand 30.07.2021 68 Publikationen eingeschlossen. Die zugrundeliegenden Studien wurden nach ihrer Eignung f��r repr��sentative Aussagen f��r die Allgemeinbev��lkerung und zur Absch��tzung zeitlicher Ver��nderungen klassifiziert. Zus��tzlich wurden Beobachtungszeitraum und Operationalisierung von Outcomes betrachtet. Die erste Infektionswelle und das Sommerplateau wurden von 65 % der Studien abgebildet. Im Forschungsdesign besonders geeignete Studien zeigten gemischte Ergebnisse, die in der Tendenz auf eine gr����tenteils resiliente erwachsene Bev��lkerung mit einem Anteil vulnerabler Personen hinweisen. Eine ��berwiegend negative Entwicklung von psychischer Gesundheit beschrieben Ergebnisse aus verzerrungsanf��lligeren Studiendesigns. Routinedatenanalysen zeigten Einbu��en in der ambulanten und vor allem station��ren Versorgung, vermehrte Nutzung eines Krisendienstes, gemischte Ergebnisse f��r ambulante Diagnosen, Arbeitsunf��higkeit und Sterblichkeit sowie Hinweise auf Verschiebungen im Diagnosespektrum. Da die aktuelle Evidenz nicht eindeutig ist, sollten verallgemeinernde Aussagen zugunsten einer differenzierten Betrachtung reflektiert werden. Forschungsbedarf besteht in Bezug auf den weiteren Pandemieverlauf, spezifische Risikogruppen und die Pr��valenz psychischer St��rungen.

19. Health‐related quality of life of children born to childhood cancer survivors in Germany

Objective: Rising childhood cancer survival rates have increased the importance of health-related quality of life (HRQL) assessment. While survivors show comparable HRQL to peers, concerns that cancer treatment could impact the health of prospective children were reported. No previous publications address HRQL of childhood cancer survivor offspring. Methods: We assessed survivor offspring HRQL using the parental KINDL questionnaire. Matched-pair analysis was conducted with data from the general population (KiGGS study) using age, gender and education (1:1, n = 1206 cases). Multivariate analyses were conducted to detect the influence of parental diagnose and treatment on offspring HRQL. Results: Overall, within KINDL dimensions, survivors reported comparable to higher HRQL for their children than the general population. Survivor parents reported significantly (p < 0.001) higher psychological (86.7% vs. 83.0%, Cohen's d = 0.3) and self-esteem (79.1% vs. 73.3%, Cohen's d = 0.5) well-being scores for younger children (3-6-year-olds). As time since diagnosis increased, parents reported higher well-being scores. Accordingly, recently diagnosed survivors reported significantly lower psychological well-being scores (p = 0.28; OR = 0.457; 95% CI = 0.228-0.918) for their children. With increasing age, average HRQL scores decreased in both cohorts; yet, this drop was less pronounced for survivor offspring. The biggest difference between age groups (7-10- vs. 14-17-year-olds) was found for school-specific well-being (6.2-point drop in survivor offspring vs. 18.2-point drop in KiGGS offspring). Conclusion: Comparable to higher parentally assessed HRQL was reported for survivor offspring compared to peers. These findings are reassuring and consistent with self-reported HRQL in childhood cancer survivors. Type of parental cancer diagnosis and treatment showed no negative impact on offspring HRQL.

Kinder und Jugendliche sind von der COVID-19-Pandemie und den behördlichen Eindämmungsmaßnahmen besonders betroffen. Die Auswirkungen auf ihre psychische Gesundheit wurde bislang wenig untersucht. Ziel dieses narrativen Reviews ist es, bisher vorliegende Erkenntnisse zur psychischen Gesundheit von Kindern und Jugendlichen in den ersten Wochen der COVID-19-Pandemie und während der für ihre Eindämmung ergriffenen Maßnahmen in Deutschland zusammenzufassen. Erste internationale und nationale Studien zeigen ein differenziertes Bild. Bei Kindern und Jugendlichen traten Symptome von Angst und Depression sowie eine geminderte Lebensqualität auf. Die Schließungen der Betreuungs- und Bildungseinrichtungen und der damit einhergehende Verlust der gewohnten Tagesstruktur, Kontaktabbrüche und dem eigenständigen Lernen zu Hause stellten erhebliche Herausforderungen für betroffene Kinder und deren Familien dar. Räumliche Enge und fehlende Ausweichmöglichkeiten während der Eindämmungsmaßnahmen konnten außerdem zu erhöhtem familiärem Stress und gehäufter familiärer Aggression sowie zu häuslicher Gewalt führen. Die Befunde zeigen jedoch auch, dass viele Familien die Zeit während der Eindämmungsmaßnahmen überwiegend gut meisterten. Bei möglichen zukünftigen Pandemien oder weiteren Wellen der COVID-19-Pandemie sollten die Bedürfnisse von Heranwachsenden und deren Familien während der Eindämmungsmaßnahmen stärker berücksichtigt werden.

Children and adolescents are particularly affected by the COVID-19 pandemic and the official containment measures. However, the effects on their mental health have been little studied. The aim of this narrative review is to summarize existing evidence on the mental health of children and adolescents in the first weeks of the COVID-19 pandemic and during the measures taken to contain it in Germany. First international and national studies draw a differential picture. Children and adolescents showed symptoms of anxiety and depression as well as a reduced quality of life. The closure of childcare and educational facilities and the associated loss of the familiar daytime structure as well as loss of contact and independent learning at home posed considerable challenges for affected children and their families. Spatial confinement at home and the lack of alternative options of stay during the containment measures could also have lead to increased family stress, heightened family aggression, and domestic violence. However, the findings of several studies also show that many families coped with the time during the containment measures mostly well. In the event of possible future pandemics or further waves of the COVID-19 pandemic, the needs of adolescents and their families during the containment measures should be given greater consideration.

After a large church concert on 1 March 2020 and a first detected infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) on 9 March, the southern German community of Kupferzell in the federal state Baden-Württemberg faced a steep increase of SARS-CoV-2 infections. Investigations of the local health authorities showed increasing evidence of community spreading in a complex and chronologically dense pattern of travel returnees who attended a choir and trombone church concert. Wide-reaching infection prevention and local control measures were implemented starting in the week of the first case detection, followed by additional measures such as a ban on gatherings in the federal state starting mid-March. The number of SARS-CoV-2 infections peaked in March but waned in April, and there were only three cases in May (Figure). There were three deaths, aged 59, 81 and 91 years. The cumulative incidence of 1,760 per 100,000 in Kupferzell by the end of April was, at the time of the study, one of the highest in Germany. The Robert Koch Institute (RKI) set out to analyse the SARS-CoV-2 seroprevalence in a random sample of this community from 20 May to 9 June.

Background: In adults, a significant impact of thyroid dysfunction and autoimmunity on health-related quality of life (HRQoL) and mental health is described. However, studies in children and adolescents are sparse, underpowered, and findings are ambiguous. Methods: Data from 759 German children and adolescents affected by thyroid disease [subclinical hypothyroidism: 331; subclinical hyperthyroidism: 276; overt hypothyroidism: 20; overt hyperthyroidism: 28; Hashimoto's thyroiditis (HT): 68; thyroid-peroxidase antibody (TPO)-AB positivity without apparent thyroid dysfunction: 61] and 7,293 healthy controls from a nationwide cross-sectional study (“The German Health Interview and Examination Survey for Children and Adolescents”) were available. Self-assessed HRQoL (KINDL-R) and mental health (SDQ) were compared for each subgroup with healthy controls by analysis of covariance considering questionnaire-specific confounding factors. Thyroid parameters (TSH, fT4, fT3, TPO-AB levels, thyroid volume as well as urinary iodine excretion) were correlated with KINDL-R and SDQ scores employing multiple regression, likewise accounting for confounding factors. Results: The subsample of participants affected by overt hypothyroidism evidenced impaired mental health in comparison to healthy controls, but SDQ scores were within the normal range of normative data. Moreover, in no other subgroup, HRQoL or mental health were affected by thyroid disorders. Also, there was neither a significant relationship between any single biochemical parameter of thyroid function and HRQoL or mental health, nor did the combined thyroid parameters account for a significant proportion of variance in either outcome measure. Importantly, the present study was sufficiently powered to identify even small effects in children and adolescents affected by HT, subclinical hypothyroidism, and hyperthyroidism. Conclusions: In contrast to findings in adults, and especially in HT, there was no significant impairment of HRQoL or mental health in children and adolescents from the general pediatric population affected by thyroid disease. Moreover, mechanisms proposed to explain impaired mental health in thyroid dysfunction in adults do not pertain to children and adolescents in the present study. OA Förderung 2020

Der Beitrag analysiert, wie sich das Ausmaß gesundheitlicher Ungleichheiten bei Kindern und Jugendlichen in Deutschland in den letzten zehn Jahren entwickelt hat. Die Analysen basieren auf Daten der Studie zur Gesundheit von Kindern und Jugendlichen in Deutschland (KiGGS), die für die 0- bis 17-jährige Bevölkerung in Deutschland repräsentativ sind und in drei Wellen erhoben wurden: KiGGS-Basiserhebung (2003 – 2006), KiGGS Welle 1 (2009 – 2012) und KiGGS Welle 2 (2014 – 2017). Betrachtet werden Prävalenzen zu fünf Gesundheitsoutcomes: allgemeiner Gesundheitszustand, psychische Auffälligkeiten, körperliche Aktivität, Konsum zuckerhaltiger Erfrischungsgetränke und Rauchen. Die gesundheitlichen Ungleichheiten werden an Unterschieden nach dem sozioökonomischen Status (SES) der Familie festgemacht, der als Index aus Bildung, Beruf und Einkommen der Eltern bestimmt wird. Betrachtet werden sowohl absolute als auch relative gesundheitliche Ungleichheiten. Hierzu werden der Slope Index of Inequality (SII) und der Relative Index of Inequality (RII) mittels linearer Wahrscheinlichkeitsmodelle beziehungsweise log-Binomial-Modellen berechnet. Für alle Gesundheitsoutcomes zeigen sich deutliche Ungleichheiten zuungunsten von Heranwachsenden aus Familien mit niedrigem SES. In KiGGS Welle 2 waren diese beim allgemeinen Gesundheitszustand und dem Konsum zuckerhaltiger Erfrischungsgetränke besonders stark ausgeprägt. Die Trendergebnisse sprechen hier zudem für eine Ausweitung der relativen Ungleichheiten. Bei letzterem haben gleichzeitig die absoluten Ungleichheiten abgenommen, was ebenso für das Rauchen gilt. Die unverändert hohen und zum Teil ausgeweiteten gesundheitlichen Ungleichheiten weisen darauf hin, dass Heranwachsende aus Familien mit niedrigem SES von den bislang umgesetzten Maßnahmen zur Prävention und Gesundheitsförderung im Kindes- und Jugendalter noch nicht gleichermaßen profitieren.

Abstract Background The German Health Interview and Examination Survey for Adults (DEGS) is part of the recently established national health monitoring conducted by the Robert Koch Institute. DEGS combines a nationally representative periodic health survey and a longitudinal study based on follow-up of survey participants. Funding is provided by the German Ministry of Health and supplemented for specific research topics from other sources. Methods/design The first DEGS wave of data collection (DEGS1) extended from November 2008 to December 2011. Overall, 8152 men and women participated. Of these, 3959 persons already participated in the German National Health Interview and Examination Survey 1998 (GNHIES98) at which time they were 18–79 years of age. Another 4193 persons 18–79 years of age were recruited for DEGS1 in 2008–2011 based on two-stage stratified random sampling from local population registries. Health data and context variables were collected using standardized computer assisted personal interviews, self-administered questionnaires, and standardized measurements and tests. In order to keep survey results representative for the population aged 18–79 years, results will be weighted by survey-specific weighting factors considering sampling and drop-out probabilities as well as deviations between the design-weighted net sample and German population statistics 2010. Discussion DEGS aims to establish a nationally representative data base on health of adults in Germany. This health data platform will be used for continuous health reporting and health care research. The results will help to support health policy planning and evaluation. Repeated cross-sectional surveys will permit analyses of time trends in morbidity, functional capacity levels, disability, and health risks and resources. Follow-up of study participants will provide the opportunity to study trajectories of health and disability. A special focus lies on chronic diseases including asthma, allergies, cardiovascular conditions, diabetes mellitus, and musculoskeletal diseases. Other core topics include vaccine-preventable diseases and immunization status, nutritional deficiencies, health in older age, and the association between health-related behavior and mental health.

This study examines the extent to which health inequalities among children and adolescents in Germany have developed over the past decade. The analyses are based on data from the German Health Interview and Examination Survey for Children and Adolescents (KiGGS), which are representative of the 0- to 17-year-old population in Germany. The KiGGS data were collected in three waves: the KiGGS baseline study (2003-2006), KiGGS Wave 1 (2009-2012) and KiGGS Wave 2 (2014-2017). Prevalences of five health outcomes are considered: general health, mental health problems, physical activity, the consumption of sugary soft drinks, and smoking. Moreover, it defines health inequalities in relation to differences in the socioeconomic status of the family (SES), an index derived from the parents’ level of education, occupation and income, and considers both absolute and relative health inequalities. In order to do so, the Slope Index of Inequality (SII) and the Relative Index of Inequality (RII) were calculated using linear probability or log-binomial models. Significant inequalities were identified to the detriment of young people from families with a low SES. These inequalities were particularly pronounced in the KiGGS Wave 2 data with regard to general health and the consumption of sugary soft drinks. Additionally, evidence from trend analyses for these two outcomes suggests that relative inequalities have increased. However, absolute inequalities decreased during the same period, and this also applies to smoking. The persistently high and, in some cases, widened levels of health inequalities indicate that adolescents from families with a low SES do not benefit to the same extent from disease prevention and health promotion measures for children and adolescents as young people from families with a higher SES.

Die Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) gehört zu den häufigsten psychischen Störungen im Kindesund Jugendalter und geht mit vielen Beeinträchtigungen der psychosozialen und kognitiven Funktionsfähigkeit einher. In der zweiten Folgeerhebung der Studie zur Gesundheit von Kindern und Jugendlichen in Deutschland (KiGGS Welle 2, 2014 – 2017) wurden die Eltern von Kindern und Jugendlichen im Alter von 3 bis 17 Jahren zum Vorliegen einer jemals durch eine Ärztin beziehungsweise einen Arzt oder Psychologin beziehungsweise Psychologen gestellten ADHS-Diagnose befragt. 4,4 % der 3- bis 17-jährigen Kinder und Jugendlichen haben eine derartige ADHS-Diagnose erhalten. Im Vergleich zur KiGGS-Basiserhebung (2003 – 2006) ist ein Rückgang der elternberichteten Lebenszeitprävalenz von ADHS-Diagnosen um circa einen Prozentpunkt in einem Zeitraum von zehn Jahren zu verzeichnen. Dieser Rückgang betrifft ausschließlich Jungen und die Altersgruppe von 3 bis 8 Jahren. Diese Ergebnisse werden vor dem Hintergrund verschiedener Maßnahmen der Gesundheitsförderung und -versorgung diskutiert.

Attention deficit/hyperactivity disorder (ADHD) is one of the most common mental disorders in childhood and adolescence and is associated with functional, psychosocial and cognitive impairment. As part of the second wave of the German Health Interview and Examination Survey for Children and Adolescents (2014-2017), parents of children and adolescents aged between 3 and 17 years reported whether their child was diagnosed with ADHD by a physician or psychologist. Overall, 4.4% of children and adolescents have been diagnosed with ADHD in Germany. In comparison, the KiGGS baseline study (2003-2006) showed a reduction of lifetime ADHD diagnoses of almost one percentage point over a period of ten years. The reduction of parent-reported ADHD diagnoses primarily occurred among 3- to 8-year old children and boys. The results are discussed in terms of health promotion and the introduction of health care measures.

Abstract Background From May 2003 to May 2006, the Robert Koch Institute conducted the German Health Interview and Examination Survey for Children and Adolescents (KiGGS). Aim of this first nationwide interview and examination survey was to collect comprehensive data on the health status of children and adolescents aged 0 to 17 years. Methods/Design Participants were enrolled in two steps: first, 167 study locations (sample points) were chosen; second, subjects were randomly selected from the official registers of local residents. The survey involved questionnaires filled in by parents and parallel questionnaires for children aged 11 years and older, physical examinations and tests, and a computer assisted personal interview performed by study physicians. A wide range of blood and urine testing was carried out at central laboratories. A total of 17 641 children and adolescents were surveyed – 8985 boys and 8656 girls. The proportion of sample neutral drop-outs was 5.3%. The response rate was 66.6%. Discussion The response rate showed little variation between age groups and sexes, but marked variation between resident aliens and Germans, between inhabitants of cities with a population of 100 000 or more and sample points with fewer inhabitants, as well as between the old West German states and the former East German states. By analysing the short non-responder questionnaires it was proven that the collected data give comprehensive and nationally representative evidence on the health status of children and adolescents aged 0 to 17 years.

Bereits im Kindes- und Jugendalter ist ein enger Zusammenhang zwischen der sozialen und der gesundheitlichen Lage zu beobachten. Obwohl die überwiegende Mehrheit der Kinder und Jugendlichen in Deutschland gesund aufwächst, zeichnen sich soziale Unterschiede in der gesundheitlichen Entwicklung ab. Die Ergebnisse aus der zweiten Folgeerhebung der Studie zur Gesundheit von Kindern und Jugendlichen in Deutschland (KiGGS Welle 2) belegen, dass Kinder und Jugendliche mit niedrigem sozioökonomischen Status (SES) einen schlechteren allgemeinen Gesundheitszustand und häufiger gesundheitsbezogene Einschränkungen aufweisen. Deutlich stärker als bei den 12-Monats-Prävalenzen für Asthma bronchiale und Heuschnupfen kommen die sozialen Unterschiede bei der psychischen Gesundheit zum Tragen. Die statistische Chance, von psychischen Auffälligkeiten oder von einer Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) betroffen zu sein, ist bei Mädchen und Jungen mit niedrigem SES gegenüber Gleichaltrigen mit hohem SES um das 2,8- bis 4,4-Fache erhöht. Um allen Kindern und Jugendlichen ein gesundes Aufwachsen zu ermöglichen, sollten Maßnahmen der Gesundheitsförderung und Prävention bereits früh im Lebenslauf ansetzen und zielgruppenbasiert zugeschnitten werden.

Psychische Auffälligkeiten bei Kindern und Jugendlichen gehen mit individuellen und familiären Beeinträchtigungen sowie gesellschaftlichen Folgekosten einher. Zur KiGGS-Basiserhebung (2003 – 2006) waren 20,0 % der Kinder und Jugendlichen psychisch auffällig. In diesem Beitrag werden die aktuellen Prävalenzen aus KiGGS Welle 2 (2014 – 2017) berichtet und im Trend mit den Daten aus der KiGGS-Basiserhebung verglichen. Als Indikator dienten die Elternangaben des Stärken-und-Schwächen-Fragebogens (SDQ) für 3- bis 17-jährige Kinder und Jugendliche. Die Prävalenz psychischer Auffälligkeiten liegt zu KiGGS Welle 2 bei 16,9 %. Der rückläufige Trend betrifft insbesondere Jungen im Alter von 9 bis 17 Jahren. Mädchen und Jungen aus Familien mit niedrigem sozioökonomischen Status sind deutlich häufiger als Gleichaltrige aus Familien mit mittlerem und hohem sozioökonomischen Status psychisch auffällig. Diese Befunde werden vor dem Hintergrund verschiedener Maßnahmen der Gesundheitsförderung und -versorgung diskutiert.

Der erste bundesweite Kinder- und Jugendgesundheitssurvey (KiGGS) wurde von 2003 – 2006 vom Robert Koch-Institut in insgesamt 167 repräsentativ ausgewählten Städten und Gemeinden der Bundesrepublik Deutschland durchgeführt. Etwa 18.000 Kinder und Jugendliche von 0 bis 17 Jahren nahmen an der Studie teil. Nun folgt nach einem Zeitraum von ca. sechs Jahren die erste Folgeerhebung KiGGS Welle 1 in Form eines telefonischen Befragungssurveys. In dieses Follow-up, das von Juni 2009 bis Mai 2012 wieder vom Robert Koch-Institut durchgeführt wird, werden alle Teilnehmer der KiGGS-Basiserhebung erneut einbezogen. Zum Ausgleich der durch das zeitliche Intervall entstandenen »Lücke« bei den jüngeren Teilnehmern wird eine zusätzliche Stichprobe von Kindern im Alter von 0 bis 6 Jahren gezogen. Befragt werden in insgesamt ca. 24.000 Interviews alle ehemaligen KiGGS-Teilnehmer im Alter von 11 bis 24 Jahren selbst sowie die Eltern der 0- bis 17-jährigen Kinder und Jugendlichen zu einem breiten Spektrum gesundheitsrelevanter Themen. Hierbei werden sowohl die Kernindikatoren der KiGGS-Basiserhebung fortgeführt als auch zusätzliche Themen neu berücksichtigt. Damit stehen neben einem erneut repräsentativen Abbild des Gesundheitszustands und des Gesundheitsverhaltens der Kinder- und Jugendlichenbevölkerung vor allem Entwicklungstrends und individuelle gesundheitliche Verläufe der KiGGS-Basisteilnehmer im Fokus des gesundheitswissenschaftlichen und gesundheitspolitischen Interesses. Mit den jungen Erwachsenen im Alter von 18 bis 24 Jahren ist erstmalig in einem breit angelegten Gesundheitssurvey eine Altersgruppe prospektiv berücksichtigt, in der sich biografische Umbrüche vollziehen, von denen angenommen werden kann, dass sie gesundheitsrelevante Auswirkungen haben. Die KiGGS Welle 1 ist Bestandteil eines am Robert Koch-Institut seit 2007 im Auftrag des Bundesministeriums für Gesundheit etablierten Gesundheitsmonitorings für Deutschland. Die hier vorgelegte Projektbeschreibung schafft Transparenz im Hinblick auf die Auswahl bzw. Fortführung der Themen aus der KiGGS-Basisstudie, den Einsatz der Instrumente und die zugrunde liegenden Fragestellungen. Es ist vorgesehen, die erhobenen Daten später wieder als Scientific Use Files frei zu geben. Diese umfassende Projektbeschreibung erlaubt es externen Wissenschaftlern schon jetzt, eigene Konzepte für künftige Auswertungen zu entwickeln.

Aims: Cross-sectional studies demonstrated associations of several sociodemographic and psychosocial factors with generic health-related quality of life (HRQoL) in children and adolescents. However, little is known about factors affecting the change in child and adolescent HRQoL over time. This study investigates potential psychosocial risk and protective factors of child and adolescent HRQoL based on longitudinal data of a German population-based study. Methods: Data from the BELLA study gathered at three measurement points (baseline, 1-year and 2-year follow-ups) were investigated in n = 1,554 children and adolescents aged 11 to 17 years at baseline. Self-reported HRQoL was assessed by the KIDSCREEN-10 Index. We examined effects of sociodemographic factors, mental health problems, parental mental health problems, as well as potential personal, familial, and social protective factors on child and adolescent HRQoL at baseline as well as over time using longitudinal growth modeling. Results: At baseline, girls reported lower HRQoL than boys, especially in older participants; low socioeconomic status and migration background were both associated with low HRQoL. Mental health problems as well as parental mental health problems were negatively, self-efficacy, family climate, and social support were positively associated with initial HRQoL. Longitudinal analyses revealed less increase of HRQoL in girls than boys, especially in younger participants. Changes in mental health problems were negatively, changes in self-efficacy and social support were positively associated with the change in HRQoL over time. No effects were found for changes in parental mental health problems or in family climate on changes in HRQoL. Moderating effects for self-efficacy, family climate or social support on the relationships between the investigated risk factors and HRQoL were not found. Conclusion: The risk factor mental health problems negatively and the resource factors self-efficacy and social support positively affect the development of HRQoL in young people, and should be considered in prevention programs. Background: Over the past decades the public health relevance of mental health conditions in children and adolescents has been of growing concern. However, so far no detailed epidemiological data has been available for a representative national sample in Germany. Objectives: The present paper reports prevalence rates of general and specific mental health problems among children and adolescents in Germany and describes the link between symptoms and impairment as well as the treatment situation. Methods: The mental health module (BELLA study) examines mental health problems in a representative sub-sample of 2,863 families with children aged 7–17 from the National Health Interview and Examination Survey among Children and Adolescents (KiGGS). Mental health problems were determined using the extended version of the strengths and difficulties questionnaire (SDQ). Further standardised screening measures were employed to screen for anxiety disorders (SCARED), conduct disorder (CBCL), attention deficit-/ hyperactivity disorder (FBBHKS, Conners’ Scale) and depressive disorders (CES-DC). Furthermore, substance abuse and suicidal tendencies were assessed. Health-related quality of life (HRQoL) and health care use were determined. Results Overall, 14.5% of the children and adolescents aged 7–17 fulfilled the criteria for at least one specific mental health problem associated with impairment, or had an overall mental health problem indicated by an abnormal SDQ score and present impairment. However, high comorbidity was found in the children concerned. Symptoms of overall mental health problems were present in 8.6% of the children and 6.6% of the adolescents. This number was reduced to prevalence rates of 6.3 and 4.9% when additional impairment was taken as a criterion. Irrespective of the type of disorder, fewer than half of the children affected were reported as receiving treatment. Background: The present study examined pain perception in children and adolescents with ADHD and the interaction between pain perception and the administration of methylphenidate (MPH) in order to generate hypotheses for further research that will help to clarify the association between ADHD diagnosis, MPH treatment and pain perception. Methods: We included 260 children and adolescents of the "German Health Interview and Examination Survey for Children and Adolescents" (KiGGS) and analyzed parent's assessments of children's pain distribution and pain perception, as well as the influence of MPH administration on pain perception in affected children and adolescents. Results: Pain perception was associated with ADHD and MPH administration, indicating that children and adolescents suffering from ADHD without MPH treatment were reported to have lower pain perception compared to both, healthy controls (HC) and ADHD patients medicated with MPH. Conclusion: We suggest that reduced pain perception in children and adolescents with ADHD not medicated with MPH may lead to higher risk tolerance by misjudgments of dangerous situations, expanding the importance of MPH administration in affected children and adolescents.
