Relationships between the AM to PL center distance, body height, and sex difference were also analyzed. Patients and methods: The positions of the AM and PL tunnel centers were defined based on previous studies using the quadrant method, and were superimposed anatomically onto the 3-D computer-aided design knee models from 68 intact femurs. The distance between the tunnel centers was measured using the 3-D DICOM software package. The correlation between the AM–PL distance and the subject's body height was assessed, and a cutoff height value for a higher risk of overlap of the AM and PL tunnel apertures was identified. Tocilizumab (TCZ) is a humanized monoclonal anti-interleukin-6 receptor antibody that was initially developed for use as an intravenous (IV) infusion. Previous studies have shown that TCZ-IV is an important treatment option in patients with moderate-to-severe RA. A subcutaneous (SC) formulation of 162 mg TCZ that was recently developed and approved provides an additional treatment option for RA patients. In the present review, we provide an update on the efficacy and safety of TCZ-SC, compared with TCZ-IV. The TCZ-SC doses of 162 mg every 2 weeks (q2w) or weekly (qw) were selected based on pharmacokinetic and pharmacodynamic studies. Both TCZ-SC q2w and qw regimens showed equivalent effects to TCZ-IV in most patients; however, the TCZ-SC qw regimen consistently showed a more rapid effect in terms of C-reactive protein normalization. Randomized controlled studies showed that TCZ-SC monotherapy or combined with disease-modifying antirheumatic drugs demonstrated comparable efficacy to TCZ-IV in patients who were both biologic-naïve and refractory to tumor necrosis factor inhibitors. TCZ-SC at both qw and q2w were generally well-tolerated for up to 24 weeks. There was a low rate of withdrawal due to adverse events, and their incidence was comparable with that seen with TCZ-IV. An injection site reaction was seen in approximately 10% of patients who received the subcutaneous formulation. In conclusion, although clinical results are still limited, the currently available evidence suggests that TCZ-SC is a promising treatment for moderate-to-severe RA, both as monotherapy and combination therapy. The extent of ovalization and the frequency of overlap of the two tunnel apertures were compared. Methods: The simulation of femoral tunnel drilling with the TT and the trans-AMP techniques was performed using three-dimensional computer aided design models from two volunteers. The incidence angle of drilling against the intercondylar wall, the femoral tunnel position, the ovalization, and the overlap were analyzed. The aperture and location of the tunnels were also examined in real anterior cruciate ligament reconstruction cases (n=36). Results: The surgical simulation showed that a lower drill incident angle induced by the TT technique made the apertures of two tunnels more ovalized, located anteromedial tunnels in a shallower position to prevent posterior wall blow out, and led to a higher frequency of tunnel overlap. The trans-AMP group had tunnel places within the footprint and had less ovalization and overlap. The results of analysis for tunnels in the clinical cases were consistent with results from the surgical simulation. Conclusion: In the TT technique, the shallow anteromedial tunnel location and more ovalized tunnel aperture can lead to a higher frequency of tunnel overlap. Compared with the TT technique, the trans-AMP technique was more useful in preparing femoral tunnels anatomically and avoiding tunnel ovalization and overlapping in double-bundle anterior cruciate ligament reconstruction. A 45-year-old male underwent anterior rotational osteotomy for osteonecrosis of the left femoral head. A radiograph obtained 4 months after surgery showed the presence of osteolytic lesions around the trochanteric wire, and the transtrochanteric osteotomy site appeared to be fuzzy. In spite of the continuation of partial weight bearing, osteolytic changes progressed. An open biopsy was performed 13 months after surgery. Samples were taken from multiple sites, including osteolytic lesions, one of which was confirmed to contain coagulase-negative staphylococcus. Nineteen days after performing thorough debridement and irrigation, the patient underwent a refixation of the osteotomy site using an iliac bone graft and a sliding hip screw system. A radiograph obtained at the final follow-up revealed bone union at the site of osteotomy.Keywords: osteonecrosis, infected nonunion, osteotomy

Abstract

Radiation monitoring at high energy proton accelerators poses a considerable challenge due to the complexity of the encountered stray radiation fields. These environments comprise a wide variety of different particle types and span from fractions of electron-volts up to several terra electron-volts. As a consequence the use of Monte Carlo simulation programs like FLUKA is indispensable to obtain appropriate field-specific calibration factors. At many locations of the LHC a large contribution to the particle fluence is expected to originate from high-energy neutrons and thus, benchmark experiments with mono-energetic neutron beams are of high importance to verify the aforementioned detector response calculations. This paper summarizes the results of a series of benchmark experiments with quasi mono-energetic neutrons of 100, 140, 200, 250 and 392 MeV that have been carried out at RCNP - Osaka University, during several campaigns between 2006 and 2014.

Abstract

The Super-Kamiokande detector can be used to search for neutrinos in time coincidence with gravitational waves detected by the LIGO-Virgo Collaboration (LVC). Both low-energy ($7-100$ MeV) and high-energy ($0.1-10^5$ GeV) samples were analyzed in order to cover a very wide neutrino spectrum. Follow-ups of 36 (out of 39) gravitational waves reported in the GWTC-2 catalog were examined; no significant excess above the background was observed, with 10 (24) observed neutrinos compared with 4.8 (25.0) expected events in the high-energy (low-energy) samples. A statistical approach was used to compute the significance of potential coincidences. For each observation, p-values were estimated using neutrino direction and LVC sky map ; the most significant event (GW190602_175927) is associated with a post-trial p-value of $7.8\%$ ($1.4\sigma$). Additionally, flux limits were computed independently for each sample and by combining the samples. Abstract

Stagnation of a cold plasma streaming to the center or axis of symmetry via an expanding accretion shock wave is ubiquitous in inertial confinement fusion (ICF) and high-energy-density plasma physics, the examples ranging from plasma flows in x-ray-generating Z pinches [Y. Maron et al., Phys. Rev. Lett. 111, 035001 (2013)] to the experiments in support of the recently suggested concept of impact ignition in ICF [H. Azechi et al., Phys. Rev. Lett. 102, 235002 (2009); M. Murakami et al., Nucl. Fusion 54, 054007 (2014)]. Some experimental evidence indicates that stagnation via an expanding shock wave is stable, but its stability has never been studied theoretically. We present such analysis for the stagnation that does not involve a rarefaction wave behind the expanding shock front and is described by the classic ideal-gas Noh solution in spherical and cylindrical geometry. In either case the stagnated flow has been demonstrated to be stable, initial perturbations exhibiting a power-law, oscillatory or monotonic, decay with time for all the eigenmodes. This conclusion has been supported by our simulations done both on a Cartesian grid and on a curvilinear grid in spherical coordinates. Abstract

BACKGROUND: Degenerative aortic valve (AoV) disease and resulting aortic stenosis are major clinical health problems. Murine models of valve disease are rare, resulting in a translational knowledge gap on underlying mechanisms, functional consequences, and potential therapies. Naïve New Zealand obese (NZO) mice were recently found to have a dramatic decline of left ventricular (LV) function at early age. Therefore, we aimed to identify the underlying cause of reduced LV function in NZO mice. METHODS AND RESULTS: Cardiac function and pulmonary hemodynamics of NZO and age‐matched C57BL/6J mice were monitored by serial echocardiographic examinations. AoVs in NZO mice demonstrated extensive thickening, asymmetric aortic leaflet formation, and cartilaginous transformation of the valvular stroma. Doppler echocardiography of the aorta revealed increased peak velocity profiles, holodiastolic flow reversal, and dilatation of the ascending aorta, consistent with aortic stenosis and regurgitation. Compensated LV hypertrophy deteriorated to decompensated LV failure and remodeling, as indicated by increased LV mass, interstitial fibrosis, and inflammatory cell infiltration. Elevated LV pressures in NZO mice were associated with lung congestion and cor pulmonale, evident as right ventricular dilatation, decreased right ventricular function, and increased mean right ventricular systolic pressure, indicative for the development of pulmonary hypertension and ultimately right ventricular failure. CONCLUSIONS: NZO mice demonstrate as a novel murine model to spontaneously develop degenerative AoV disease, aortic stenosis, and the associated end organ damages of both ventricles and the lung. Closely mimicking the clinical scenario of degenerative AoV disease, the model may facilitate a better mechanistic understanding and testing of novel treatment strategies in degenerative AoV disease.

Abstract

We report a case of late-onset hyperprogressive disease after cessation of a PD-1 inhibitor. A male was diagnosed with metastatic lung adenocarcinoma with little progression for 2 months before treatment. He received pembrolizumab as a second-line treatment and was subsequently prescribed docetaxel for 3 months until a slight increase in pleural effusion. At the time of progression to docetaxel, he commenced prednisolone because of immune-system-related diarrhea. After that, his general condition rapidly worsened with severe fatigue and hypoxia. Computed tomography revealed a massive increase of pleural effusion and replacement of almost the entire liver with cancer over a period of 5 weeks.

25. Elution and Containment of Harmful Substances in Construction-Generated Soil : A Case Study of Oshikado Tunnel, Japan

Abstract

In recent years, there has been an increasing number of cases in Japan where countermeasures against environmental pollution caused by natural heavy metals contained in soil and wastewater generated during construction work have become a problem. For example, when arsenic, which is often used, exceeds 0.01 mg/L, which is the criterion in the elution test, the generated soil is covered with low-permeability (permeability coefficient ≤1×10-6 cm/s) soil mixed with bentonite to prevent heavy metal leakage. Although the impact on the surrounding environment is predicted in the design of such a disposal site, it does not accurately reflect the behavior of leachate in the embankment and the elution characteristics of heavy metals. Therefore, in this study, for the purpose of optimal design of the disposal site, the distribution of heavy metals in the embankment, behavior of leachate, and elution amount of heavy metals were investigated, and a numerical analysis method using the advectiondispersion equation was examined. In addition, a large column test using soil at the site was conducted to reproduce the site. The parameters related to the permeation characteristics obtained from the column test agreed with the field test; however, the unit elution amount, which is a parameter related to the elution characteristics, was 7.5 times that of the field test. Abstract

Aims Treatment decisions in patients with scimitar syndrome (SS) are often challenging, especially in patients with isolated SS who are often asymptomatic and who might be diagnosed accidentally. We queried a large multi-institutional registry of SS patients to evaluate the natural history of this condition and to determine the efficacy of surgical treatment in terms of survival and clinical status. Methods and results We collected data on 485 SS patients from 51 institutions; 279 (57%) patients were treated surgically (STPs) and 206 (43%) were clinically monitored (CMPs). Median age at last follow-up was 11.6 years (interquartile range 4-22 years). Abstract

Nuclear interactions can be the source of atomic displacement and post-short-term cascade annealing defects in irradiated structural materials. Such quantities are derived from, or can be correlated to, nuclear kinematic simulations of primary atomic energy distributions spectra and the quantification of the numbers of secondary defects produced per primary as a function of the available recoils, residual and emitted, energies. Recoils kinematics of neutral, residual, charged and multi-particle emissions are now more rigorously treated based on modern, complete and enhanced nuclear data parsed in state of the art processing tools. Defect production metrics are the starting point in this complex problem of correlating and simulating the behaviour of materials under irradiation, as direct measurements are extremely improbable. The multi-scale dimensions (nuclear-atomic-molecular-material) of the simulation process is tackled from the Fermi gradation to provide the atomic- and meso-scale dimensions with better metrics relying upon a deeper understanding and modelling capabilities of the nuclear level. Detailed, segregated primary knock-on-atom metrics are now available as the starting point of further simulation processes of isolated and clustered defects in material lattices. This allows more materials, incident energy ranges and particles, and irradiations conditions to be explored, with sufficient data to adequately cover both standard applications and novel ones, such as advanced-fission, accelerators, nuclear medicine, space and fusion. This paper reviews the theory, describes the latest methodologies and metrics, and provides recommendations for standard and novel approaches.
