Algorithms that automatically identify nodular patterns in chest X-ray (CXR) images could benefit radiologists by reducing reading time and improving accuracy. A promising approach is to use deep learning, where a deep neural network (DNN) is trained to classify and localize nodular patterns (including mass) in CXR images. Such algorithms, however, require enough abnormal cases to learn representations of nodular patterns arising in practical clinical settings. Obtaining large amounts of high-quality data is impractical in medical imaging where (1) acquiring labeled images is extremely expensive, (2) annotations are subject to inaccuracies due to the inherent difficulty in interpreting images, and (3) normal cases occur far more frequently than abnormal cases. In this work, we devise a framework to generate realistic nodules and demonstrate how they can be used to train a DNN identify and localize nodular patterns in CXR images. While most previous research applying generative models to medical imaging are limited to generating visually plausible abnormalities and using these patterns for augmentation, we go a step further to show how the training algorithm can be adjusted accordingly to maximally benefit from synthetic abnormal patterns. A high-precision detection model was first developed and tested on internal and external datasets, and the proposed method was shown to enhance the model's recall while retaining the low level of false positives.

Training deep learning models on medical images heavily depends on experts’ expensive and laborious manual labels. In addition, these images, labels, and even models themselves are not widely publicly accessible and suffer from various kinds of bias and imbalances. In this paper, chest X-ray pre-trained model via self-supervised contrastive learning (CheSS) was proposed to learn models with various representations in chest radiographs (CXRs). Our contribution is a publicly accessible pretrained model trained with a 4.8-M CXR dataset using self-supervised learning with a contrastive learning and its validation with various kinds of downstream tasks including classification on the 6-class diseases in internal dataset, diseases classification in CheXpert, bone suppression, and nodule generation. When compared to a scratch model, on the 6-class classification test dataset, we achieved 28.5% increase in accuracy. On the CheXpert dataset, we achieved 1.3% increase in mean area under the receiver operating characteristic curve on the full dataset and 11.4% increase only using 1% data in stress test manner. On bone suppression with perceptual loss, we achieved improvement in peak signal to noise ratio from 34.99 to 37.77, structural similarity index measure from 0.976 to 0.977, and root-square-mean error from 4.410 to 3.301 when compared to ImageNet pretrained model. Finally, on nodule generation, we achieved improvement in Fréchet inception distance from 24.06 to 17.07. Our study showed the decent transferability of CheSS weights. CheSS weights can help researchers overcome data imbalance, data shortage, and inaccessibility of medical image datasets. CheSS weight is available at https://github.com/mi2rl/CheSS.

BACKGROUND: CT is the standard method used to assess bronchiectasis. A higher airway-to-artery diameter ratio (AAR) is typically used to identify enlarged bronchi and bronchiectasis; however, current imaging methods are limited in assessing the extent of this metric in CT scans. PURPOSE: To determine the extent of AARs using an artificial intelligence–based chest CT and assess the association of AARs with exacerbations over time. MATERIALS AND METHODS: In a secondary analysis of ever-smokers from the prospective, observational, multicenter COPDGene study, AARs were quantified using an artificial intelligence tool. The percentage of airways with AAR greater than 1 (a measure of airway dilatation) in each participant on chest CT scans was determined. Pulmonary exacerbations were prospectively determined through biannual follow-up (from July 2009 to September 2021). Multivariable zero-inflated regression models were used to assess the association between the percentage of airways with AAR greater than 1 and the total number of pulmonary exacerbations over follow-up. Covariates included demographics, lung function, and conventional CT parameters. RESULTS: Among 4192 participants (median age, 59 years; IQR, 52–67 years; 1878 men [45%]), 1834 had chronic obstructive pulmonary disease (COPD). During a 10-year follow-up and in adjusted models, the percentage of airways with AARs greater than 1 (quartile 4 vs 1) was associated with a higher total number of exacerbations (risk ratio [RR], 1.08; 95% CI: 1.02, 1.15; P = .01). In participants meeting clinical and imaging criteria of bronchiectasis (ie, clinical manifestations with ≥3% of AARs >1) versus those who did not, the RR was 1.37 (95% CI: 1.31, 1.43; P < .001). Among participants with COPD, the corresponding RRs were 1.10 (95% CI: 1.02, 1.18; P = .02) and 1.32 (95% CI: 1.26, 1.39; P < .001), respectively. CONCLUSION: In ever-smokers with chronic obstructive pulmonary disease, artificial intelligence-based CT measures of bronchiectasis were associated with more exacerbations over time. Background The solid portion size of lung cancer lesions manifesting as subsolid lesions is key in their management, but the automatic measurement of such lesions by means of a deep learning (DL) algorithm needs evaluation. Purpose To evaluate the performance of a commercially available DL algorithm for automatic measurement of the solid portion of surgically proven lung adenocarcinomas manifesting as subsolid lesions. Materials and Methods Surgically proven lung adenocarcinomas manifesting as subsolid lesions on CT images between January 2018 and December 2018 were retrospectively included. Five radiologists independently measured the maximal axial diameter of the solid portion of lesions. The DL algorithm automatically segmented and measured the maximal axial diameter of the solid portion. Reader measurements, software measurements, and invasive component size at pathologic examination were compared by using intraclass correlation coefficient (ICC) and Bland-Altman plots. Results A total of 448 patients (mean age, 63 years ± 10 [standard deviation]; 264 women) with 448 lesions were evaluated (invasive component size, 3-65 mm). The measurement agreements between each radiologist and the DL algorithm were very good (ICC range, 0.82-0.89). When a radiologist was replaced with the DL algorithm, the ICCs ranged from 0.87 to 0.90, with an ICC of 0.90 among five radiologists. The mean difference between the DL algorithm and each radiologist ranged from -3.7 to 1.5 mm. The widest 95% limit of agreement between the DL algorithm and each radiologist (-15.7 to 8.3 mm) was wider than pairwise comparisons of radiologists (-7.7 to 13.0 mm). The agreement between the DL algorithm and invasive component size at pathologic evaluation was good, with an ICC of 0.67. Measurements by the DL algorithm (mean difference, -6.0 mm) and radiologists (mean difference, -7.5 to -2.3 mm) both underestimated invasive component size. Conclusion Automatic measurements of solid portions of lung cancer manifesting as subsolid lesions by the deep learning algorithm were comparable with manual measurements and showed good agreement with invasive component size at pathologic evaluation. © RSNA, 2021 Online supplemental material is available for this article.

Background Studies on the optimal CT section thickness for detecting subsolid nodules (SSNs) with computer-aided detection (CAD) are lacking. Purpose To assess the effect of CT section thickness on CAD performance in the detection of SSNs and to investigate whether deep learning-based super-resolution algorithms for reducing CT section thickness can improve performance. Materials and Methods CT images obtained with 1-, 3-, and 5-mm-thick sections were obtained in patients who underwent surgery between March 2018 and December 2018. Patients with resected synchronous SSNs and those without SSNs (negative controls) were retrospectively evaluated. The SSNs, which ranged from 6 to 30 mm, were labeled ground-truth lesions. A deep learning-based CAD system was applied to SSN detection on CT images of each section thickness and those converted from 3- and 5-mm section thickness into 1-mm section thickness by using the super-resolution algorithm. The CAD performance on each section thickness was evaluated and compared by using the jackknife alternative free response receiver operating characteristic figure of merit. Results A total of 308 patients (mean age ± standard deviation, 62 years ± 10; 183 women) with 424 SSNs (310 part-solid and 114 nonsolid nodules) and 182 patients without SSNs (mean age, 65 years ± 10; 97 men) were evaluated. The figures of merit differed across the three section thicknesses (0.92, 0.90, and 0.89 for 1, 3, and 5 mm, respectively; P = .04) and between 1- and 5-mm sections (P = .04). The figures of merit varied for nonsolid nodules (0.78, 0.72, and 0.66 for 1, 3, and 5 mm, respectively; P < .001) but not for part-solid nodules (range, 0.93-0.94; P = .76). The super-resolution algorithm improved CAD sensitivity on 3- and 5-mm-thick sections (P = .02 for 3 mm, P < .001 for 5 mm). Conclusion Computer-aided detection (CAD) of subsolid nodules performed better at 1-mm section thickness CT than at 3- and 5-mm section thickness CT, particularly with nonsolid nodules. Pulmonary MRI provides structural and quantitative functional images of the lungs without ionizing radiation, but it has had limited clinical use due to low signal intensity from the lung parenchyma. The lack of radiation makes pulmonary MRI an ideal modality for pediatric examinations, pregnant women, and patients requiring serial and longitudinal follow-up. Fortunately, recent MRI techniques, including ultrashort echo time and zero echo time, are expanding clinical opportunities for pulmonary MRI. With the use of multicoil parallel acquisitions and acceleration methods, these techniques make pulmonary MRI practical for evaluating lung parenchymal and pulmonary vascular diseases. The purpose of this Fleischner Society position paper is to familiarize radiologists and other interested clinicians with these advances in pulmonary MRI and to stratify the Society recommendations for the clinical use of pulmonary MRI into three categories: (a) suggested for current clinical use, (b) promising but requiring further validation or regulatory approval, and (c) appropriate for research investigations. This position paper also provides recommendations for vendors and infrastructure, identifies methods for hypothesis-driven research, and suggests opportunities for prospective, randomized multicenter trials to investigate and validate lung MRI methods.

We evaluated the reproducibility of computer-aided detections (CADs) with a convolutional neural network (CNN) on chest radiographs (CXRs) of abnormal pulmonary patterns in patients, acquired within a short-term interval. Anonymized CXRs (n = 9792) obtained from 2010 to 2016 and comprising five types of disease patterns, including the nodule (N), consolidation (C), interstitial opacity (IO), pleural effusion (PLE), and pneumothorax (PN), were included. The number of normal and abnormal CXRs was 6068 and 3724, respectively. The number of CXRs (region of interests, ROIs) of N, C, IO, PLE, and PN was 944 (1092), 550 (721), 280 (538), 1361 (1661), and 589 (622), respectively. CXRs were randomly allocated to training, tuning, and test sets in 70:10:20 ratios. Two thoracic radiologists labeled and delineated the ROIs of each disease pattern. The CAD system was developed using eDenseYOLO. For the reproducibility evaluation of developed CAD, paired CXRs of various diseases (N = 121, C = 28, IO = 12, PLE = 67, and PN = 20), acquired within a short-term interval from the test sets without any changes confirmed by thoracic radiologists, were used to evaluate CAD reproducibility. Percent positive agreement (PPAs) and Chamberlain’s percent positive agreement (CPPAs) were used to evaluate CAD reproducibility. The figure of merit (FOM) of five classes based on eDenseYOLO showed N-0.72 (0.68–0.75), C-0.41 (0.33–0.43), IO-0.97 (0.96–0.98), PLE-0.94 (0.92–95), and PN-0.87 (0.76–0.93). The PPAs of the five disease patterns including N, C, IO, PLE, and PN were 83.39%, 74.14%, 95.12%, 96.84%, and 84.58%, respectively, whereas the values of CPPAs were 71.70%, 59.13%, 91.16%, 93.91%, and 74.17%, respectively. The reproducibility of abnormal pulmonary patterns from CXRs, based on deep learning-based CAD, showed different results; this is important for assessing the reproducible performance of CAD in clinical settings.

Background The volume doubling time (VDT) is a key parameter in the differentiation of aggressive tumors from slow-growing tumors. How different histologic subtypes of primary lung adenocarcinomas vary in their VDT and the prognostic value of this measurement is unknown. Purpose To investigate differences in VDT between the predominant histologic subtypes of primary lung adenocarcinomas and to assess the correlation between VDT and prognosis. Materials and Methods This retrospective study included patients who underwent at least two serial CT examinations before undergoing operation between July 2010 and December 2018. Three-dimensional tumor segmentation was performed on two CT images and VDTs were calculated. VDTs were compared between predominant histologic subtypes and lesion types by using Kruskal-Wallis tests. Disease-free survival (DFS) was obtained in patients undergoing surgical procedures before July 2017. Univariable and multivariable Cox proportional hazards regression analyses were performed to determine predictors of DFS. Results Among 268 patients (mean age, 64 years ± 8 [standard deviation]; 143 men), there were 30 lepidic, 87 acinar, 109 papillary, and 42 solid or micropapillary predominant subtypes. The median VDT was 529 days (interquartile range, 278-872 days) for lung adenocarcinomas. VDTs differed across subtypes (P < .001) and were shortest in solid or micropapillary subtypes (229 days; interquartile range, 77-530 days). Solid lesions (VDT, 248 days) had shorter VDTs than subsolid lesions (part-solid lesions, 665 days; nonsolid lesions, 648 days) (P < .001). In the 148 patients (mean age, 64 years ± 8; 89 men) included in the survival analysis, 35 patients had disease recurrence and 17 patients died. VDT (

OBJECTIVE Patients with chronic obstructive pulmonary disease (COPD) are known to be at risk of osteoporosis. The purpose of this study was to evaluate the association between thoracic vertebral bone density measured on chest CT (DThorax) and clinical variables, including survival, in patients with COPD. MATERIALS AND METHODS A total of 322 patients with COPD were selected from the Korean Obstructive Lung Disease (KOLD) cohort. DThorax was measured by averaging the CT values of three consecutive vertebral bodies at the level of the left main coronary artery with a round region of interest as large as possible within the anterior column of each vertebral body using an in-house software. Associations between DThorax and clinical variables, including survival, pulmonary function test (PFT) results, and CT densitometry, were evaluated. RESULTS The median follow-up time was 7.3 years (range: 0.1-12.4 years). Fifty-six patients (17.4%) died. DThorax differed significantly between the different Global Initiative for Chronic Obstructive Lung Disease stages. DThorax correlated positively with body mass index (BMI), some PFT results, and the six-minute walk distance, and correlated negatively with the emphysema index (EI) (all p < 0.05). In the univariate Cox analysis, older age (hazard ratio [HR], 3.617; 95% confidence interval [CI], 2.119-6.173, p < 0.001), lower BMI (HR, 3.589; 95% CI, 2.122-6.071, p < 0.001), lower forced expiratory volume in one second (FEV₁) (HR, 2.975; 95% CI, 1.682-5.262, p < 0.001), lower diffusing capacity of the lung for carbon monoxide corrected with hemoglobin (DLCO) (HR, 4.595; 95% CI, 2.665-7.924, p < 0.001), higher EI (HR, 3.722; 95% CI, 2.192-6.319, p < 0.001), presence of vertebral fractures (HR, 2.062; 95% CI, 1.154-3.683, p = 0.015), and lower DThorax (HR, 2.773; 95% CI, 1.620-4.746, p < 0.001) were significantly associated with all-cause mortality and lung-related mortality. In the multivariate Cox analysis, lower DThorax (HR, 1.957; 95% CI, 1.075-3.563, p = 0.028) along with older age, lower BMI, lower FEV₁, and lower DLCO were independent predictors of all-cause mortality. CONCLUSION The thoracic vertebral bone density measured on chest CT demonstrated significant associations with the patients' mortality and clinical variables of disease severity in the COPD patients included in KOLD cohort.

Background Evaluation of interstitial lung disease (ILD) at CT is a challenging task that requires experience and is subject to substantial interreader variability. Purpose To investigate whether a proposed content-based image retrieval (CBIR) of similar chest CT images by using deep learning can aid in the diagnosis of ILD by readers with different levels of experience. Materials and Methods This retrospective study included patients with confirmed ILD after multidisciplinary discussion and available CT images identified between January 2000 and December 2015. Database was composed of four disease classes: usual interstitial pneumonia (UIP), nonspecific interstitial pneumonia (NSIP), cryptogenic organizing pneumonia, and chronic hypersensitivity pneumonitis. Eighty patients were selected as queries from the database. The proposed CBIR retrieved the top three similar CT images with diagnosis from the database by comparing the extent and distribution of different regional disease patterns quantified by a deep learning algorithm. Eight readers with varying experience interpreted the query CT images and provided their most probable diagnosis in two reading sessions 2 weeks apart, before and after applying CBIR. Diagnostic accuracy was analyzed by using McNemar test and generalized estimating equation, and interreader agreement was analyzed by using Fleiss κ. Results A total of 288 patients were included (mean age, 58 years ± 11 [standard deviation]; 145 women). After applying CBIR, the overall diagnostic accuracy improved in all readers (before CBIR, 46.1% [95% CI: 37.1, 55.3]; after CBIR, 60.9% [95% CI: 51.8, 69.3]

Lung lobe segmentation in chest CT has been used for the analysis of lung functions and surgical planning. However, accurate lobe segmentation is difficult as 80% of patients have incomplete and/or fake fissures. Furthermore, lung diseases such as chronic obstructive pulmonary disease (COPD) can increase the difficulty of differentiating the lobar fissures. Lobar fissures have similar intensities to those of the vessels and airway wall, which could lead to segmentation error in automated segmentation. In this study, a fully automated lung lobe segmentation method with 3D U-Net was developed and validated with internal and external datasets. The volumetric chest CT scans of 196 normal and mild-to-moderate COPD patients from three centers were obtained. Each scan was segmented using a conventional image processing method and manually corrected by an expert thoracic radiologist to create gold standards. The lobe regions in the CT images were then segmented using a 3D U-Net architecture with a deep convolutional neural network (CNN) using separate training, validation, and test datasets. In addition, 40 independent external CT images were used to evaluate the model. The segmentation results for both the conventional and deep learning methods were compared quantitatively to the gold standards using four accuracy metrics including the Dice similarity coefficient (DSC), Jaccard similarity coefficient (JSC), mean surface distance (MSD), and Hausdorff surface distance (HSD). In internal validation, the segmentation method achieved high accuracy for the DSC, JSC, MSD, and HSD (0.97 ± 0.02, 0.94 ± 0.03, 0.69 ± 0.36, and 17.12 ± 11.07, respectively). In external validation, high accuracy was also obtained for the DSC, JSC, MSD, and HSD (0.96 ± 0.02, 0.92 ± 0.04, 1.31 ± 0.56, and 27.89 ± 7.50, respectively). This method took 6.49 ± 1.19 s and 8.61 ± 1.08 s for lobe segmentation of the left and right lungs, respectively. Although various automatic lung lobe segmentation methods have been developed, it is difficult to develop a robust segmentation method. However, the deep learning–based 3D U-Net method showed reasonable segmentation accuracy and computational time. In addition, this method could be adapted and applied to severe lung diseases in a clinical workflow.

Three-dimensional (3D) printing technology, with additive manufacturing, can aid in the production of various kinds of patient-specific medical devices and implants in medical fields, which cannot be covered by mass production systems for producing conventional devices/implants. The simulator-based medical image demonstrates the anatomical structure of the disease, which can be used for education, diagnosis, preparation of treatment plan and preoperative surgical guide, etc. The surgical guide is used as a patient-specific medical device for guiding incision, resection, insertion, and marking. As 3D printers can output materials that can be inserted into the human body, the patient-specific implant device that reflects the patient’s anatomy and surgical plan could be of relevance. In addition, patient-specific aids, including gibs, splints, prostheses, and epitheses, could be used for a better outcome. Finally, bio-printing is also used to cultivate cells to produce functional artificial tissues.

Pulmonary functional imaging may be defined as the regional quantification of lung function by using primarily CT, MRI, and nuclear medicine techniques. The distribution of pulmonary physiologic parameters, including ventilation, perfusion, gas exchange, and biomechanics, can be noninvasively mapped and measured throughout the lungs. This information is not accessible by using conventional pulmonary function tests, which measure total lung function without viewing the regional distribution. The latter is important because of the heterogeneous distribution of virtually all lung disorders. Moreover, techniques such as hyperpolarized xenon 129 and helium 3 MRI can probe lung physiologic structure and microstructure at the level of the alveolar-air and alveolar–red blood cell interface, which is well beyond the spatial resolution of other clinical methods. The opportunities, challenges, and current stage of clinical deployment of pulmonary functional imaging are reviewed, including applications to chronic obstructive pulmonary disease, asthma, interstitial lung disease, pulmonary embolism, and pulmonary hypertension. Among the challenges to the deployment of pulmonary functional imaging in routine clinical practice are the need for further validation, establishment of normal values, standardization of imaging acquisition and analysis, and evidence of patient outcomes benefit. When these challenges are addressed, it is anticipated that pulmonary functional imaging will have an expanding role in the evaluation and management of patients with lung disease. © RSNA, 2021 Online supplemental material is available for this article.

Developing a robust algorithm to diagnose and quantify the severity of the novel coronavirus disease 2019 (COVID-19) using Chest X-ray (CXR) requires a large number of well-curated COVID-19 datasets, which is difficult to collect under the global COVID-19 pandemic. On the other hand, CXR data with other findings are abundant. This situation is ideally suited for the Vision Transformer (ViT) architecture, where a lot of unlabeled data can be used through structural modeling by the self-attention mechanism. However, the use of existing ViT may not be optimal, as the feature embedding by direct patch flattening or ResNet backbone in the standard ViT is not intended for CXR. To address this problem, here we propose a novel Multi-task ViT that leverages low-level CXR feature corpus obtained from a backbone network that extracts common CXR findings. Specifically, the backbone network is first trained with large public datasets to detect common abnormal findings such as consolidation, opacity, edema, etc. Then, the embedded features from the backbone network are used as corpora for a versatile Transformer model for both the diagnosis and the severity quantification of COVID-19. We evaluate our model on various external test datasets from totally different institutions to evaluate the generalization capability. The experimental results confirm that our model can achieve state-of-the-art performance in both diagnosis and severity quantification tasks with outstanding generalization capability, which are sine qua non of widespread deployment., Graphical abstract Image, graphical abstract

Over the past few decades, pulmonary imaging technologies have advanced from chest radiography and nuclear medicine methods to high-spatial-resolution or low-dose chest CT and MRI. It is currently possible to identify and measure pulmonary pathologic changes before these are obvious even to patients or depicted on conventional morphologic images. Here, key technological advances are described, including multiparametric CT image processing methods, inhaled hyperpolarized and fluorinated gas MRI, and four-dimensional free-breathing CT and MRI methods to measure regional ventilation, perfusion, gas exchange, and biomechanics. The basic anatomic and physiologic underpinnings of these pulmonary functional imaging techniques are explained. In addition, advances in image analysis and computational and artificial intelligence (machine learning) methods pertinent to functional lung imaging are discussed. The clinical applications of pulmonary functional imaging, including both the opportunities and challenges for clinical translation and deployment, will be discussed in part 2 of this review. Given the technical advances in these sophisticated imaging methods and the wealth of information they can provide, it is anticipated that pulmonary functional imaging will be increasingly used in the care of patients with lung disease. © RSNA, 2021 Online supplemental material is available for this article.

Background Previous studies assessing the effects of computer-aided detection on observer performance in the reading of chest radiographs used a sequential reading design that may have biased the results because of reading order or recall bias. Purpose To compare observer performance in detecting and localizing major abnormal findings including nodules, consolidation, interstitial opacity, pleural effusion, and pneumothorax on chest radiographs without versus with deep learning-based detection (DLD) system assistance in a randomized crossover design. We used 3D printed-breast surgical guides (3DP-BSG) to designate the original tumor area from the pre-treatment magnetic resonance imaging (MRI) during breast-conserving surgery (BCS) in breast cancer patients who received neoadjuvant systemic therapy (NST). Targeting the original tumor area in such patients using conventional localization techniques is difficult. For precise BCS, a method that marks the tumor area found on MRI directly to the breast is needed. In this prospective study, patients were enrolled for BCS after receiving NST. Partial resection was performed using a prone/supine MRI-based 3DP-BSG. Frozen biopsies were analyzed to confirm clear tumor margins. The tumor characteristics, pathologic results, resection margins, and the distance between the tumor and margin were analyzed. Thirty-nine patients were enrolled with 3DP-BSG for BCS. The median nearest distance between the tumor and the resection margin was 3.9 cm (range 1.2–7.8 cm). Frozen sections showed positive margins in 4/39 (10.3%) patients. Three had invasive cancers, and one had carcinoma in situ; all underwent additional resection. Final pathology revealed clear margins. After 3-year surveillance, 3/39 patients had recurrent breast cancer. With 3DP-BSG for BCS in breast cancer patients receiving NST, the original tumor area can be identified and marked directly on the breast, which is useful for surgery. OBJECTIVE Emphysema and small-airway disease are the two major components of chronic obstructive pulmonary disease (COPD). We propose a novel method of quantitative computed tomography (CT) emphysema air-trapping composite (EAtC) mapping to assess each COPD component. We analyzed the potential use of this method for assessing lung function in patients with COPD. MATERIALS AND METHODS A total of 584 patients with COPD underwent inspiration and expiration CTs. Using pairwise analysis of inspiration and expiration CTs with non-rigid registration, EAtC mapping classified lung parenchyma into three areas: Normal, functional air trapping (fAT), and emphysema (Emph). We defined fAT as the area with a density change of less than 60 Hounsfield units (HU) between inspiration and expiration CTs among areas with a density less than -856 HU on inspiration CT. The volume fraction of each area was compared with clinical parameters and pulmonary function tests (PFTs). The results were compared with those of parametric response mapping (PRM) analysis. RESULTS The relative volumes of the EAtC classes differed according to the Global Initiative for Chronic Obstructive Lung Disease stages (p < 0.001). Each class showed moderate correlations with forced expiratory volume in 1 second (FEV1) and FEV1/forced vital capacity (FVC) (r = -0.659-0.674, p < 0.001). Both fAT and Emph were significant predictors of FEV1 and FEV1/FVC (R² = 0.352 and 0.488, respectively; p < 0.001). fAT was a significant predictor of mean forced expiratory flow between 25% and 75% and residual volume/total vital capacity (R² = 0.264 and 0.233, respectively; p < 0.001), while Emph and age were significant predictors of carbon monoxide diffusing capacity (R² = 0.303; p < 0.001). fAT showed better correlations with PFTs than with small-airway disease on PRM. CONCLUSION The proposed quantitative CT EAtC mapping provides comprehensive lung functional information on each disease component of COPD, which may serve as an imaging biomarker of lung function.

Background There are few reports on exercise-induced desaturation (EID) as a predictor of mortality in chronic obstructive pulmonary disease (COPD). However, the definitions of EID vary in published reports. The main purpose was to evaluate the association between EID and long-term mortality by applying two criteria of EID. Methods A total of 507 subjects were selected from the Korean Obstructive Lung Disease cohort. EID was assessed using the 6-min walk test (6MWT) and defined using two different criteria [1]: post-6MWT oxygen saturation (SpO2) of ≤88% (criterion A) and [2] post-6MWT SpO2 Results The prevalence of EID was 5.1% based on criterion A and 13.0% based on criterion B. Regardless of the criteria used, mortality was higher in the EID group than in the non-EID group (A: 50 vs. 11.4%, B: 33.3 vs. 10.4%) during up to 161 months of follow-up. COPD patients without EID survived significantly longer than those with EID (A: 143.5 vs. 92.9, B: 144.8 vs. 115.2 months). Multivariate Cox regression analysis revealed that COPD patients with EID had a 2.4-fold increased risk of death by criterion A (adjusted HR 2.375; 95% CI: 1.217–4.637; P = 0.011). The risk of death increased in COPD patients with EID by criterion B, but the difference was not statistically significant. Conclusions COPD patients with EID demonstrated significantly higher long-term mortality than those without EID. The EID criterion A has a better predictive value for mortality in COPD.

To facilitate precise tumor resection at the time of breast-conserving surgery (BCS), we developed and implemented a magnetic resonance imaging (MRI)-based three-dimensional-printed (3DP) breast surgical guide (BSG). This prospective cohort study was conducted at a single institution from July 2017 to February 2019 on women with breast cancer who underwent partial breast resection using patient-specific 3DP BSGs. Eighty-eight patients with invasive cancer were enrolled, of whom 1 patient had bilateral breast cancer. The mean size of the tumor long-axis on MRI before surgery was 2.8 ± 0.9 cm, and multiple tumors were observed in 34 patients. In 16 cases (18.0%), the resection margin was tumor-positive according to intraoperative frozen biopsy; all of these tumors were ductal carcinoma in situ and were re-excised intraoperatively. In 93.3% of the cases, the resection margin was tumor-free in the permanent pathology. The mean pathological tumor size was 1.7 ± 1.0 cm, and the mean distance from the tumor to the border was 1.5 ± 1.0 cm. This exploratory study showed that the tumor area on the MRI could be directly displayed on the breast when using a 3DP BSG for BCS, thereby allowing precise surgery and safe tumor removal.Trial Registration Clinical Research Information Service (CRIS) Identifier (No. KCT0002375, KCT0003043).

In recent years, artificial intelligence (AI) technologies have greatly advanced and become a reality in many areas of our daily lives. In the health care field, numerous efforts are being made to implement the AI technology for practical medical treatments. With the rapid developments in machine learning algorithms and improvements in hardware performances, the AI technology is expected to play an important role in effectively analyzing and utilizing extensive amounts of health and medical data. However, the AI technology has various unique characteristics that are different from the existing health care technologies. Subsequently, there are a number of areas that need to be supplemented within the current health care system for the AI to be utilized more effectively and frequently in health care. In addition, the number of medical practitioners and public that accept AI in the health care is still low; moreover, there are various concerns regarding the safety and reliability of AI technology implementations. Therefore, this paper aims to introduce the current research and application status of AI technology in health care and discuss the issues that need to be resolved., Graphical Abstract

BackgroundComputer-aided diagnosis on chest x-ray images using deep learning is a widely studied modality in medicine. Many studies are based on public datasets, such as the National Institutes of Health (NIH) dataset and the Stanford CheXpert dataset. However, these datasets are preprocessed by classical natural language processing, which may cause a certain extent of label errors.ObjectiveThis study aimed to investigate the robustness of deep convolutional neural networks (CNNs) for binary classification of posteroanterior chest x-ray through random incorrect labeling.MethodsWe trained and validated the CNN architecture with different noise levels of labels in 3 datasets, namely, Asan Medical Center-Seoul National University Bundang Hospital (AMC-SNUBH), NIH, and CheXpert, and tested the models with each test set. Diseases of each chest x-ray in our dataset were confirmed by a thoracic radiologist using computed tomography (CT). Receiver operating characteristic (ROC) and area under the curve (AUC) were evaluated in each test. Randomly chosen chest x-rays of public datasets were evaluated by 3 physicians and 1 thoracic radiologist.ResultsIn comparison with the public datasets of NIH and CheXpert, where AUCs did not significantly drop to 16%, the AUC of the AMC-SNUBH dataset significantly decreased from 2% label noise. Evaluation of the public datasets by 3 physicians and 1 thoracic radiologist showed an accuracy of 65%-80%.ConclusionsThe deep learning–based computer-aided diagnosis model is sensitive to label noise, and computer-aided diagnosis with inaccurate labels is not credible. Furthermore, open datasets such as NIH and CheXpert need to be distilled before being used for deep learning–based computer-aided diagnosis.

The term interstitial lung abnormalities refers to specific CT findings that are potentially compatible with interstitial lung disease in patients without clinical suspicion of the disease. Interstitial lung abnormalities are increasingly recognised as a common feature on CT of the lung in older individuals, occurring in 4-9% of smokers and 2-7% of non-smokers. Identification of interstitial lung abnormalities will increase with implementation of lung cancer screening, along with increased use of CT for other diagnostic purposes. These abnormalities are associated with radiological progression, increased mortality, and the risk of complications from medical interventions, such as chemotherapy and surgery. Management requires distinguishing interstitial lung abnormalities that represent clinically significant interstitial lung disease from those that are subclinical. In particular, it is important to identify the subpleural fibrotic subtype, which is more likely to progress and to be associated with mortality. This multidisciplinary Position Paper by the Fleischner Society addresses important issues regarding interstitial lung abnormalities, including standardisation of the definition and terminology; predisposing risk factors; clinical outcomes; options for initial evaluation, monitoring, and management; the role of quantitative evaluation; and future research needs.
