In 10 municipalities of Antioquia (Colombia) the positivity rate in serum for total (IgG/IgM) and specific (IgM) antibodies to hepatitis E virus (HEV) was determined, and tests were done for the presence of HEV RNA in the feces of individuals positive for IgM antibodies. 28. Prevalence of adverse events and their manifestations in health professionals as second victims

Abstract

Introduction: Adverse events can cause harm to the patient, but they also affect health professionals making them second victims of the event. Interventions have been focused mostly on patients, but little on professionals, probably due to lack of tools.Objective: To estimate the prevalence of adverse events and describe their manifestations in healthcare personnel to evidence the phenomenon of second victims in a highly complex hospital.Materials and methods: We conducted an analytical cross-sectional study by a survey of 419 healthcare professionals from the hospitalization, emergency, and surgical areas in a highly complex hospital in Medellín in 2019. The frequency of adverse events was estimated and its association with some labor and demographic variables was determined. Results: We found that 93.1% of the participants knew of incident cases and 79% of serious adverse events while 44.4% had been involved in them and 99% of these had feelings as a second victim, mainly the difficulty to concentrate, guilt, fatigue, anxiety, and doubts about decisions; 95% indicated they wanted to receive training to face the consequences of adverse events and know how to inform the patient.Conclusions: Health professionals are frequently exposed to adverse events that can cause negative emotions in them such as guilt, fatigue, anxiety, and insecurity. Most professionals who participate in an adverse event express feelings as a second victim. Informing the patient about an adverse event requires preparation and most professionals requested training on the subject., Introducción. Los eventos adversos pueden causar daño al paciente y también afectar a los profesionales de la salud, lo que los convierte en segundas víctimas. Las intervenciones se han enfocado principalmente en los pacientes, pero poca atención se ha prestado a los profesionales de la salud, probablemente por falta de herramientas.Objetivo. Estimar la prevalencia de eventos adversos y describir sus manifestaciones en el personal asistencial, con el fin de evidenciar el fenómeno de las segundas víctimas en un hospital de alta complejidad.Materiales y métodos. Se hizo un estudio transversal analítico mediante una encuesta a 419 profesionales asistenciales de las áreas de hospitalización, urgencias y cirugía en un hospital de alta complejidad de Medellín en el 2019. Se estimó la frecuencia de eventos adversos, y se determinó su asociación con algunas variables laborales y demográficas.Resultados. El 93,1 % de los entrevistados conocía de casos de incidentes y el 79 %, de eventos adversos graves. El 44,4 % se había visto involucrado en un evento adverso, y el 99 % de estos expresaba sentirse como segunda víctima por experimentar dificultad para concentrarse, sentimientos de culpa, cansancio, ansiedad y dudas sobre sus decisiones. El 95 % quería recibir capacitación para afrontar las consecuencias de los eventos adversos y saber cómo informar al paciente.Conclusiones. Con frecuencia los profesionales de la salud se exponen a eventos adversos que pueden causarles emociones negativas como culpa, cansancio, ansiedad e inseguridad. La mayoría de los profesionales que participan en un evento adverso manifiestan sentimientos como segunda víctima. El informar al paciente sobre un evento adverso requiere preparación y la mayoría de los profesionales entrevistados pidió capacitación en el tema.

Abstract

Ecuador assumed the commitment of including Palliative Care (PC) in its health policies. In 2014, the Ministry of Public Health (Ministerio de Salud Pública, MSP) approved the Clinical Practice Guide for Palliative Care (Guía de Práctica Clínica sobre Cuidados Paliativos, GPCCP), with application at the national level, as a mandatory internal regulation in all institutions belonging to the National Health System. In 2021, there is no evidence about the degree of implementation. The objective was to evaluate the implementation (I) of the GPCCP guide and the knowledge (C) of the health personnel working in the Zone 7 Health Centers (HCs). This is a cross-sectional, descriptive, and prospective study. A total of 292 professionals were interviewed: managers (38), physicians (150), and nurses (104). Three surveys based on the GPCCP guide were elaborated: one for the implementation, which was applied to the individuals in charge, and the others to assess the health professionals’ knowledge. The SPSS program was used, version 25. In the three groups, more than half of the participants had no training in PC, 91.2% of the HCs have the GPCCP guide, there is PC medical history (MH) in 38.2%, and morphine is used in 14.7%. The implementation of the GPCCP guide was inadequate in 52.9% of the cases. Only 25% treat the agony symptoms and 30%, delirium; 4.4% acknowledge the use of morphine in dyspnea, and 13.3% identify the subcutaneous route as the first choice for hydration at the end-of-life phase. Strategies to implement the GPCCP guide and to improve the health personnel’s knowledge must be implemented in Zone 7 centers.

Abstract

Aim: To assess the risk of tuberculosis (infection and disease) in children less than 15 years’ old who are household contacts of pulmonary tuberculosis patients in three Colombian cities (Medellín, Cali, and Popayán). Methods: A cohort of 1,040 children household contacts of 380 adults with smear-positive pulmonary tuberculosis was followed up for 24 months. Study period 2005-2009. Results: Tuberculin skin test was positive (≥10 mm) in 43.7% (95% CI: 39.2-48.2). Tuberculin skin test positivity was associated with age 10-14 years (Prevalence Ratio -PR= 1.43, 95% CI: 1.1-1.9), having a BCG vaccine scar (PR= 1.52, 95% CI: 1.1-2.1), underweight, closer proximity to the index case and exposure time >3 months. The annual risk of infection (tuberculin skin test induration increase of 6 mm or more per year) was 17% (95% CI: 11.8-22.2) and was associated with a bacillary load of the adult index case (Relative Risk -RR= 2.12, 95% CI: 1.0-4.3). Abstract

Abstract Background The insertion element IS6110 is one of the main sources of genomic variability in Mycobacterium tuberculosis, the etiological agent of human tuberculosis. Although IS 6110 has been used extensively as an epidemiological marker, the identification of the precise chromosomal insertion sites has been limited by technical challenges. Here, we present IS-seq, a novel method that combines high-throughput sequencing using Illumina technology with efficient combinatorial sample multiplexing to simultaneously probe 519 clinical isolates, identifying almost all the flanking regions of the element in a single experiment. Results We identified a total of 6,976 IS6110 flanking regions on the different isolates. When validated using reference strains, the method had 100% specificity and 98% positive predictive value. The insertions mapped to both coding and non-coding regions, and in some cases interrupted genes thought to be essential for virulence or in vitro growth. Strains were classified into families using insertion sites, and high agreement with previous studies was observed. Conclusions This high-throughput IS-seq method, which can also be used to map insertions in other organisms, extends previous surveys of in vivo interrupted loci and provides a baseline for probing the consequences of disruptions in M. tuberculosis strains.

Abstract

Resumen Objetivo: Describir el proceso de desarrollo y validación de un instrumento creado para la evaluación de los resultados de tratamiento en los trastornos por uso de sustancias. Materiales y métodos: Este fue llevado a cabo en tres fases. Fase 1: revisión de la literatura en las principales bases de datos de ciencias de la salud sobre escalas o instrumentos para evaluación de resultados de tratamiento en trastornos por uso de sustancias. Fase 2: desarrollo de un instrumento de novo y validación por expertos mediante técnica Delphi modificada a 2 fases. Fase 3: estudio de validación clínica en el que participaron 100 usuarios de una institución pública. Resultados: La escala de evaluación de tratamiento multimodal (EETM) obtuvo, luego de su análisis de contenido, unos resultados apropiados en términos de pertinencia y relevancia; adicionalmente mostró coeficientes de alpha de Cronbach entre 0,799 y 0,963 y similitudes entre los resultados del evaluador y el paciente. Conclusiones: Se concluye que la EETM demostró ser un instrumento válido para la evaluación de los resultados de tratamiento en instituciones con intervenciones integrales en los trastornos por uso de sustancias y durante el período de internamiento. Results: The “Escala de evaluación de tratamiento multimodal” (EETM), after its content analysis, obtained appropriate results in terms of pertinence and relevance. Additionally, it showed Cronbach’s Alpha coefficients between 0.799 and 0.963 and similarities between the evaluator and the patient results. Conclusions: It was concluded that the EETM proved to be a valid instrument for the evaluation of treatment results in institutions with comprehensive interventions in disorders caused by substance use and during the admission period. Resumo Objetivo: Descrever o processo de desenvolvimento e validação de um instrumento criado para a avaliação dos resultados de tratamento nos transtornos por uso de sustâncias. Materiais e métodos: Este foi levado a cabo em três fases. Fase 1: revisão da literatura nas principais bases de dados de ciências da saúde sobre escadas ou instrumentos para avaliação de resultados de tratamento em transtornos por uso de sustâncias. Fase 2: desenvolvimento de um instrumento de novo e validação por expertos mediante técnica Delphi modificada a 2 fases. Fase 3: estudo de validação clínica no que participaram 100 usuários de uma instituição pública. Resultados: A escada de avaliação de tratamento multimodal (EETM) obteve, logo de sua analise de conteúdo, uns resultados apropriados em termos de pertinência e relevância; adicionalmente mostrou coeficientes de alpha de Cronbach entre 0,799 e 0,963 e similitudes entre os resultados do avaliador e o paciente. Conclusões: Conclui se que a EETM demonstrou ser um instrumento válido para a avaliação dos resultados de tratamento em instituições com intervenções integrais nos transtornos por uso de sustâncias e durante o período de internamento.
