In this paper, we present an annotated portfolio of speculative ideas that emerged from a co-design process where we investigated the playful potential of day-to-day mealtime. Our portfolio illustrates the learnings from our participatory engagements: it embodies ours and our participants' ideas of how technology might support increasingly playful and socio-emotionally rich experiences around food. We contribute: (1) a list of play potentials of mealtime-i.e. people's existing playful practices with food-that will point designers towards socio-emotionally desirable play-food experiences; (2) a portfolio of speculative design ideas that illustrate how mealtime technology could help to realize that playful potential; and (3) a discussion of our participants' experiences with and responses to lo-fi prototypes of our ideas. Our work will provoke designers to carefully consider the impact of food-tech innovation on the quality of people's social eating experiences and inspire them to cultivate forms of food-play that are socio-emotionally rich.?

HCI researchers have been gradually shifting attention from individual users to communities when engaging in research, design, and system development. However, our field has yet to establish a cohesive, systematic understanding of the challenges, benefits, and commitments of community-collaborative approaches to research. We conducted a systematic review and thematic analysis of 47 computing research papers discussing participatory research with communities for the development of technological artifacts and systems, published over the last two decades. From this review, we identified seven themes associated with the evolution of a project: from establishing community partnerships to sustaining results. Our findings suggest that several tensions characterize these projects, many of which relate to the power and position of researchers, and the computing research environment, relative to community partners. We discuss the implications of our findings and offer methodological proposals to guide HCI, and computing research more broadly, towards practices that center communities.

This collection of essays seeks to theorize the politics of the COVID-19 pandemic in international relations (IR). The contributions are driven by questions such as: How can theorizing help us understand these unsettled times? What kind of crisis is this? What shapes its politics? What remains the same and what has been unsettled or unsettling? In addressing such questions, each of the participants considers what we may already know about the pandemic as well as what might be ignored or missed. Collectively, the forum pushes at the interdisciplinary boundaries of IR theorizing itself and, in so doing, the participants hope to engender meaningful understandings of a world in crisis and encourage expansive ways of thinking about the times that lie beyond.

Two decades after the “war on terror” was first waged, there is little conceptual clarity about what it means to win a war. Indeed, despite the burgeoning literature on endless war and victory, there is no substantive engagement with how these themes intersect when thinking ethically about the question of war and what passes for peace. This forum seeks to spark a conversation to address this gap. Bringing together theorists and ethicists working on the themes of war and peace, we ask: What might we render visible and redress by thinking critically of the politics of victory in an era of endless war? Further, to the extent that just-war theory has long offered a grammar for the ethics of war, how does it help or hinder this quest?

Although rapid advances in machine learning have made it increasingly applicable to expert decision-making, the delivery of accurate algorithmic predictions alone is insufficient for effective human-AI collaboration. In this work, we investigate the key types of information medical experts desire when they are first introduced to a diagnostic AI assistant. In a qualitative lab study, we interviewed 21 pathologists before, during, and after being presented deep neural network (DNN) predictions for prostate cancer diagnosis, to learn the types of information that they desired about the AI assistant. Our findings reveal that, far beyond understanding the local, case-specific reasoning behind any model decision, clinicians desired upfront information about basic, global properties of the model, such as its known strengths and limitations, its subjective point-of-view, and its overall design objective--what it's designed to be optimized for. Participants compared these information needs to the collaborative mental models they develop of their medical colleagues when seeking a second opinion: the medical perspectives and standards that those colleagues embody, and the compatibility of those perspectives with their own diagnostic patterns. These findings broaden and enrich discussions surrounding AI transparency for collaborative decision-making, providing a richer understanding of what experts find important in their introduction to AI assistants before integrating them into routine practice.

As the global population ages and the prevalence of chronic conditions and acute infections rise, it is becoming imperative to understand the many forms of caregiving labor and create sociotechnical systems that support them. In this paper, we report results of a large survey study with 2000 informal caregivers in the USA, highlighting the fundamental challenges that different types of caregivers face when coordinating care with others. Our findings support previous findings on caregivers’ coordination challenges, while also offering insights into the situational, mediating factors that influence the extent to which care coordination challenges are felt. These mediating factors include caregivers’ relationships, access to a variety of resources including physical, social, and financial support, and physical and mental barriers. We discuss these challenges and mediating factors, and conclude with a discussion on how they can be considered in the design of future sociotechnical systems.

The last several years have shown a strong growth of Artificial Intelligence (AI) technologies with promising results for many areas of healthcare. HCI has contributed to these discussions, mainly with studies on explainability of advanced algorithms. However, there are only few AI-systems based on machine learning algorithms that make it to the real world and everyday care. This challenging move has been named the “last mile” of AI in healthcare, emphasizing the sociotechnical uncertainties and unforeseen learnings from involving users in the design or use of AI-based systems. The aim of this workshop is to set the stage for a new wave of HCI research that accounts for and begins to develop new insights, concepts, and methods, for transitioning from development to implementation and use of AI in healthcare. Participants are invited to collaboratively define an HCI research agenda focused on healthcare AI in the wild, which will require examining end-user engagements and questioning underlying concepts of AI in healthcare.

Background Evidence for the effectiveness of treatments in early-onset psychosis is sparse. Current guidance for the treatment of early-onset psychosis is mostly extrapolated from trials in adult populations. The UK National Institute for Health and Care Excellence has recommended evaluation of the clinical effectiveness and cost-effectiveness of antipsychotic drugs versus psychological intervention (cognitive behavioural therapy [CBT] and family intervention) versus the combination of these treatments for early-onset psychosis. The aim of this study was to establish the feasibility of a randomised controlled trial of antipsychotic monotherapy, psychological intervention monotherapy, and antipsychotics plus psychological intervention in adolescents with first-episode psychosis. Methods We did a multicentre pilot and feasibility trial according to a randomised, single-blind, three-arm, controlled design. We recruited participants from seven UK National Health Service Trust sites. Participants were aged 14–18 years; help-seeking; had presented with first-episode psychosis in the past year; were under the care of a psychiatrist; were showing current psychotic symptoms; and met ICD-10 criteria for schizophrenia, schizoaffective disorder, or delusional disorder, or met the entry criteria for an early intervention for psychosis service. Participants were assigned (1:1:1) to antipsychotics, psychological intervention (CBT with optional family intervention), or antipsychotics plus psychological intervention. Randomisation was via a web-based randomisation system, with permuted blocks of random size, stratified by centre and family contact. CBT incorporated up to 26 sessions over 6 months plus up to four booster sessions, and family intervention incorporated up to six sessions over 6 months. Choice and dose of antipsychotic were at the discretion of the treating consultant psychiatrist. Participants were followed up for a maximum of 12 months. The primary outcome was feasibility (ie, data on trial referral and recruitment, session attendance or medication adherence, retention, and treatment acceptability) and the proposed primary efficacy outcome was total score on the Positive and Negative Syndrome Scale (PANSS) at 6 months. Primary outcomes were analysed by intention to treat. Safety outcomes were reported according to as-treated status, for all patients who had received at least one session of CBT or family intervention, or at least one dose of antipsychotics. The study was prospectively registered with ISRCTN, ISRCTN80567433. Findings Of 101 patients referred to the study, 61 patients (mean age 16·3 years [SD 1·3]) were recruited from April 10, 2017, to Oct 31, 2018, 18 of whom were randomly assigned to psychological intervention, 22 to antipsychotics, and 21 to antipsychotics plus psychological intervention. The trial recruitment rate was 68% of our target sample size of 90 participants. The study had a low referral to recruitment ratio (around 2:1), a high rate of retention (51 [84%] participants retained at the 6-month primary endpoint), a high rate of adherence to psychological intervention (defined as six or more sessions of CBT; in 32 [82%] of 39 participants in the monotherapy and combined groups), and a moderate rate of adherence to antipsychotic medication (defined as at least 6 consecutive weeks of exposure to antipsychotics; in 28 [65%] of 43 participants in the monotherapy and combined groups). Mean scores for PANSS total at the 6-month primary endpoint were 68·6 (SD 17·3) for antipsychotic monotherapy (6·2 points lower than at randomisation), 59·8 (13·7) for psychological intervention (13·1 points lower than at randomisation), and 62·0 (15·9) for antipsychotics plus psychological intervention (13·9 points lower than at randomisation). A good clinical response at 6 months (defined as ≥50% improvement in PANSS total score) was achieved in four (22%) of 18 patients receiving antipsychotic monotherapy, five (31%) of 16 receiving psychological intervention, and five (29%) of 17 receiving antipsychotics plus psychological intervention. In as-treated groups, serious adverse events occurred in eight [35%] of 23 patients in the combined group, two [13%] of 15 in the antipsychotics group, four [24%] of 17 in the psychological intervention group, and four [80%] of five who did not receive any treatment. No serious adverse events were considered to be related to participation in the trial. Interpretation This trial is the first to show that a head-to-head clinical trial comparing psychological intervention, antipsychotics, and their combination is safe in young people with first-episode psychosis. However, the feasibility of a larger trial is unclear because of site-specific recruitment challenges, and amendments to trial design would be needed for an adequately powered clinical and cost-effectiveness trial that provides robust evidence. Deep learning algorithms promise to improve clinician workflows and patient outcomes. However, these gains have yet to be fully demonstrated in real world clinical settings. In this paper, we describe a human-centered study of a deep learning system used in clinics for the detection of diabetic eye disease. From interviews and observation across eleven clinics in Thailand, we characterize current eye-screening workflows, user expectations for an AI-assisted screening process, and post-deployment experiences. Our findings indicate that several socio-environmental factors impact model performance, nursing workflows, and the patient experience. We draw on these findings to reflect on the value of conducting human-centered evaluative research alongside prospective evaluations of model accuracy.

Objective Parents routinely access young children’s medical records, but medical societies strongly recommend confidential care during adolescence, and most medical centers restrict parental records access during the teen years. We sought to assess public opinion about adolescent medical privacy. Materials and Methods The Cornell National Social Survey (CNSS) is an annual nationwide public opinion survey. We added questions about a) whether parents should be able to see their 16-year-old child’s medical record, and b) whether teens would avoid discussing sensitive issues (sex, alcohol) with doctors if parents could see the record. Hypothesizing that highlighting the rationale for adolescent privacy would change opinions, we conducted an experiment by randomizing question order. Results Most respondents (83.0%) believed that an adolescent would be less likely to discuss sensitive issues with doctors with parental medical record access; responses did not differ by question order (P = .29). Most also believed that parents should have access to teens’ records, but support for parental access fell from 77% to 69% among those asked the teen withholding question first (P = .01). Conclusions Although medical societies recommend confidential care for adolescents, public opinion is largely in favor of parental access. A brief “nudge,” asking whether parental access might harm adolescent–doctor communication, increased acceptance of adolescent confidentiality, and could be part of a strategy to prepare parents for electronic patient portal policies that medical centers impose at the beginning of adolescence.

Objective Medical privacy policies, which are clear-cut for adults and young children, become ambiguous during adolescence. Yet medical organizations must establish unambiguous rules about patient and parental access to electronic patient portals. We conducted a national interview study to characterize the diversity in adolescent portal policies across a range of institutions and determine the factors influencing decisions about these policies. Methods Within a sampling framework that ensured diversity of geography and medical organization type, we used purposive and snowball sampling to identify key informants. Semi-structured interviews were conducted and analyzed with inductive thematic analysis, followed by a member check. Results We interviewed informants from 25 medical organizations. Policies established different degrees of adolescent access (from none to partial to complete), access ages (from 10 to 18 years), degrees of parental access, and types of information considered sensitive. Federal and state law did not dominate policy decisions. Other factors in the decision process were: technology capabilities; differing patient population needs; resources; community expectations; balance between information access and privacy; balance between promoting autonomy and promoting family shared decision-making; and tension between teen privacy and parental preferences. Some informants believed that clearer standards would simplify policy-making; others worried that standards could restrict high-quality polices. Conclusions In the absence of universally accepted standards, medical organizations typically undergo an arduous decision-making process to develop teen portal policies, weighing legal, economic, social, clinical, and technological factors. As a result, portal access policies are highly inconsistent across the United States and within individual states.

Insects and ‘the swarm’ as metaphors and objects of research have inspired works in the genres of science fiction and horror; social and political theorists; and the development of war-fighting technologies such as ‘drone swarms’, which function as robot/insect hybrids. Contemporary developments suggest that the future of warfare will not be ‘robots’ as technological, individualised substitutions for idealised (masculine) warfighters, but warfighters understood as swarms: insect metaphors for non-centrally organised problem-solvers that will become technologies of racialisation. As such, contemporary feminist analysis requires an analysis of the politics of life and death in the insect and the swarm, which, following Braidotti (2002), cannot be assumed to be a mere metaphor or representation of political life, but an animating materialist logic. The swarm is not only a metaphor but also a central mode of biopolitical and necropolitical war, with the ‘terrorist’ enemy represented as swarmlike as well. In analysing the relations of assemblage and antagonism in the war ontologies of the drone swarm, I seek inspiration from what Hayles (1999, p. 47) describes as a double vision that ‘looks simultaneously at the power of simulation and at the materialities that produce it’. I discuss various representations and manifestations of swarms and insect life in science/speculative fiction, from various presentations of the ‘Borg’ in Star Trek (1987–1994, 1995–2001, 1996), Alien (1979) and The Fly (1958, 1986) to more positive representations of the ‘becoming-insect’ as possible feminist Utopia in Gilman's Herland (2015 [1915]) and Tiptree's Houston, Houston, Do You Read? (1989 [1976]). Posthuman warfare also contains the possibilities of both appropriating and rewriting antagonisms of masculine and feminine in the embodiment of the subject of war in the swarm. This piece seeks to analyse new ways of feminist theorising of the relations of power and violence in the embodiment of war as the swarm.

The zombie, as a Western pop culture icon, has taken up residence in International Relations. Used both humorously and as a serious teaching tool, many scholars and professors of IR have written of the zombie as a useful figure for teaching IR theory in an engaging manner, and have used zombie outbreaks to analyse the responses of the international community during catastrophe, invasion, and natural disasters. The authors of this article would like to unearth another aspect of the zombie that is often left unsaid or forgotten: namely, that the body of the zombie, as a historical phenomenon and cultural icon, is deeply imbricated in the racialisation of political subjects and fear of the Other. Through a critical analysis of biopower and race, and in particular Weheliye’s concept of habeas viscus, we suggest that the figure of the zombie can be read as a racialised figure that can provide the means for rethinking the relationship of the discipline of IR to the concept of race. We read The Walking Dead as a zombie narrative that could provide a critical basis for rethinking the concepts of bare life and the exception to consider ‘living on’ in apocalyptic times.

Objective Prior studies of computing applications that support patients’ medication knowledge and self-management offer valuable insights into effective application design, but do not address inpatient settings. This study is the first to explore the design and usefulness of patient-facing tools supporting inpatient medication management and tracking.Materials and Methods We designed myNYP Inpatient, a custom personal health record application, through an iterative, user-centered approach. Medication-tracking tools in myNYP Inpatient include interactive views of home and hospital medication data and features for commenting on these data. In a two-phase pilot study, patients used the tools during cardiothoracic postoperative care at Columbia University Medical Center. In Phase One, we provided 20 patients with the application for 24–48 h and conducted a closing interview after this period. In Phase Two, we conducted semi-structured interviews with 12 patients and 5 clinical pharmacists who evaluated refinements to the tools based on the feedback received during Phase One.Results Patients reported that the medication-tracking tools were useful. During Phase One, 14 of the 20 participants used the tools actively, to review medication lists and log comments and questions about their medications. Patients’ interview responses and audit logs revealed that they made frequent use of the hospital medications feature and found electronic reporting of questions and comments useful. We also uncovered important considerations for subsequent design of such tools. In Phase Two, the patients and pharmacists participating in the study confirmed the usability and usefulness of the refined tools.Conclusions Inpatient medication-tracking tools, when designed to meet patients’ needs, can play an important role in fostering patient participation in their own care and patient-provider communication during a hospital stay.

Games for health (G4H) aim to improve health outcomes and encourage behavior change. While existing theoretical frameworks describe features of both games and health interventions, there has been limited systematic investigation into how disciplinary and interdisciplinary stakeholders understand design features in G4H. We recruited 18 experts from the fields of game design, behavioral health, and games for health, and prompted them with 16 sample games. Applying methods including open card sorting and triading, we elicited themes and features (e.g., real-world interaction, game mechanics) around G4H. We found evidence of conceptual differences suggesting that a G4H perspective is not simply the sum of game and health perspectives. At the same time, we found evidence of convergence in stakeholder views, including areas where game experts provided insights about health and vice versa. We discuss how this work can be applied to provide conceptual tools, improve the G4H design process, and guide approaches to encoding G4H-related data for large-scale empirical analysis.

Previous research suggests an important role for self-tracking in promoting mental wellness. Recent studies with college student populations have examined the feasibility of collecting everyday mood, activity, and social data. However, these studies do not account for students’ experiences and challenges adopting self-tracking technologies to support mental wellness goals. We present two studies conducted to better understand self-tracking for stress management and mental wellness in student populations. First, focus groups and card sorting activities with 14 student health professionals reveal expert perspectives on the usefulness of tracking for three scenarios. Second, an online survey of 297 students examines personal experiences with self-tracking and attitudes toward sharing self-tracked data with others. We draw on findings from these studies to characterize students’ motivations, challenges, and preferences in collecting and viewing self-tracked data related to mental wellness, and we compare findings between students with diagnosed mental illnesses and those without. We conclude with a discussion of challenges and opportunities in leveraging self-tracking for mental wellness, highlighting several design considerations.

Diagnostic radiology reports are increasingly being made available to patients and their family members. However, these reports are not typically comprehensible to lay recipients, impeding effective communication about report findings. In this paper, we present three studies informing the design of a prototype to foster patient-clinician communication about radiology report content. First, analysis of questions posted in online health forums helped us identify patients' information needs. Findings from an elicitation study with seven radiologists provided necessary domain knowledge to guide prototype design. Finally, a clinical field study with 14 pediatric patients, their parents and clinicians, revealed positive responses of each stakeholder when using the prototype to interact with and discuss the patient's current CT or MRI report and allowed us to distill three use cases: co-located communication, preparing for the consultation, and reviewing radiology data. We draw on our findings to discuss design considerations for supporting each of these use cases.

Through a discussion of drone warfare, and in particular the massacre of 23 people in the Uruzgan province in Afghanistan in 2010, I argue that drone warfare is both embodied and embodying. Drawing from posthuman feminist theorists such as Donna Haraway and N Katherine Hayles, I understand the turn toward data and machine intelligence not as an other-than-human process of decisionmaking that deprives humans of sovereignty, but as a form of embodiment that reworks and undermines essentialist notions of culture and nature, biology and technology. Through the intermediation of algorithmic, visual, and affective modes of embodiment, drone warfare reproduces gendered and racialized bodies that enable a necropolitics of massacre. Finally, the category of gender demonstrates a flaw in the supposed perfectibility of the algorithm in removing issues of identity or prejudice from security practices, as well as the perceptions of drone assemblages as comprising sublime technologies of perfect analysis and vision. Gender as both a mode of embodiment and a category of analysis is not removed by algorithmic war, but rather is put into the service of the violence it enables.

SummaryTo investigate patients’ experience using an inpatient personal health record (PHR) on a tablet computer to increase engagement in their hospital care.We performed observations and conducted semi-structured interviews with 14 post-operative cardiac surgical patients and their family members who received an inpatient PHR. Themes were identified using an inductive coding scheme.All participants responded favorably to having access to view their clinical information. A majority (85.7%) of participants used the application following an initial training session. Patients reported high satisfaction with being able to view their hospital medications and access educational materials related to their medical conditions. Patients reported a desire to view daily progress reports about their hospital stay and have access to educational information about their postacute recovery. In addition, patients expressed a common desire to view their diagnoses, laboratory test results, radiology reports, and procedure notes in language that is patient-friendly.Patients have unmet information needs in the hospital setting. Our findings suggest that for some inpatients and their family members, providing personalized health information through a tablet computer may improve satisfaction, decrease anxiety, increase understanding of their health conditions, and improve safety and quality of care.

Adolescents with complex chronic illnesses, such as cancer and blood disorders, must partner with family and clinical caregivers to navigate risky procedures with life-altering implications, burdensome symptoms and lifelong treatments. Yet, there has been little investigation into how technology can support these partnerships. We conducted 38 in-depth interviews (15 with teenage adolescents with chronic forms of cancer and blood disorders, 15 with their parents, and eight with clinical caregivers) along with nine non-participant observations of clinical consultations to better understand common challenges and needs that could be supported through design. Participants faced challenges primarily concerning: 1) teens' limited participation in their care, 2) communicating emotionally-sensitive information, and 3) managing physical and emotional responses. We draw on these findings to propose design goals for sociotechnical systems to support teens in partnering in their care, highlighting the need for design to support gradually evolving partnerships in care.

Almost 20 years since the publication of Krause and Williams's edited volume Critical Security Studies: Concepts and Cases (1997), critical security studies (CSS) has reached a moment in which critiques of more traditional or mainstream modes of studying the politics of war, security, and violence are supplemented by a great deal of explicit attention to their methods and methodologies for doing so. The last few years have witnessed a sudden proliferation of textbooks on methods for critical security studies that have attempted to chart the field and provide guidance for students or newcomers on methods for conducting research consistent with its values and goals. The four volumes discussed in this review essay follow on the heels of numerous other textbooks that attempt to map the terrain of security studies, such as Hansen and Buzan (2009), Peoples and Vaughan-Williams’s edited compilation of key and illustrative works (2010), Jarvis and Holland (2014), and the second edition of Karin Fierke’s Critical Approaches to International Security (2015 [2007]). In recent years, the establishment of several journals, including Critical Security Studies and particularly Security Dialogue , as well as Journal of Global Security Studies (an ISA journal) and European Journal of International Security (a BISA journal), that are dedicated to pluralistic studies of security have also suggested the popularity and continued innovation of diverse strands of scholarship that can be grouped as “critical security studies.” Furthermore, the selection of “Methods, Methodologies, and Innovation” to frame the 2014 Millennium conference and subsequent special issue also suggests a great deal of both interest and tension around the question of method and methodology for critical, interpretative, and pluralistic scholars, including the question of whether or not issues of “method” should be … lw487{at}cam.ac.uk

Recent research focused on online health information seeking highlights a heavy reliance on general-purpose search engines. However, current general-purpose search interfaces do not necessarily provide adequate support for non-experts in identifying suitable sources of health information. Popular search engines have recently introduced search tools in their user interfaces for a range of topics. In this work, we explore how such tools can support non-expert, patient-centered health information search. Scoping the current work to medication-related search, we report on findings from a formative study focused on the design of patient-centered, medication-information search tools. Our study included qualitative interviews with patients, family members, and domain experts, as well as observations of their use of Remedy, a technology probe embodying a set of search tools. Post-operative cardio-thoracic surgery patients and their visiting family members used the tools to find information about their hospital medications and were interviewed before and after their use. Domain experts conducted similar search tasks and provided qualitative feedback on their preferences and recommendations for designing these tools. Findings from our study suggest the importance of four valuation principles underlying our tools: credibility, readability, consumer perspective, and topical relevance.

Objective To systematically review existing literature regarding patient engagement technologies used in the inpatient setting. Methods PubMed, Association for Computing Machinery (ACM) Digital Library, Institute of Electrical and Electronics Engineers (IEEE) Xplore, and Cochrane databases were searched for studies that discussed patient engagement (‘self-efficacy’, ‘patient empowerment’, ‘patient activation’, or ‘patient engagement’), (2) involved health information technology (‘technology’, ‘games’, ‘electronic health record’, ‘electronic medical record’, or ‘personal health record’), and (3) took place in the inpatient setting (‘inpatient’ or ‘hospital’). Only English language studies were reviewed. Results 17 articles were identified describing the topic of inpatient patient engagement. A few articles identified design requirements for inpatient engagement technology. The remainder described interventions, which we grouped into five categories: entertainment, generic health information delivery, patient-specific information delivery, advanced communication tools, and personalized decision support. Conclusions Examination of the current literature shows there are considerable gaps in knowledge regarding patient engagement in the hospital setting and inconsistent use of terminology regarding patient engagement overall. Research on inpatient engagement technologies has been limited, especially concerning the impact on health outcomes and cost-effectiveness.

Effective communication between patients and their clinicians during clinical encounters has a positive impact on health outcomes. Technology has the potential to help transform this synchronous interaction, but re-searchers are still at early stages of developing interventions to assess and improve patient-clinician communication. In this workshop, we envision opening up a dialogue among researchers and clinicians who wish to discuss directions for future research in this domain. In particular, the workshop will focus on exploring how technologies available today, as well as projected for the future, can support the communication needs of clinicians and patients.

Remedy is a prototype consumer-centered medication-information search system, developed by applying text-processing techniques and probabilistic generative topic-modeling approaches. Our prototype system includes tools to increase consumer recognition of quality and relevance indicators in medication-related search results. Attendees will be able to use Remedy to search for a range of over-the-counter and prescription medications, and can compare the search experience with that of popular online web search interfaces.

As patients are encouraged to become active participants in their own care, recent research has begun to explore the direct sharing of electronic health information with patients during hospital visits. The design of patient-facing views of clinical information is, however, a relatively recent line of inquiry. Research is needed to further understand guidelines for communicating specific types of information to hospital patients. In this work, we focus on cardiology patients' information needs related to their hospital medications. We assessed these needs to inform the design of interactive, electronic views of medication information for cardiology inpatients. We present results of in-situ interviews with 11 inpatients and 6 nurses in a cardiology step-down unit. Our findings suggest that cohesive trends in medication information needs exist across cardiology inpatients. We discuss interview results and their implications for the design of inpatient-facing information technology. We also discuss key ways in which electronic medication information, formatted for inpatient use, differs from that formatted for outpatient or transitional medication-management use.

Patients’ basic understanding of clinical events has been shown to dramatically improve patient care. We propose that the automatic generation of very short micro-explanations, suitable for real-time delivery in clinical settings, can transform patient care by giving patients greater awareness of key events in their electronic medical record. We present results of a survey study indicating that it may be possible to automatically generate such explanations by extracting individual sentences from consumer-facing Web pages. We further inform future work by characterizing physician and non-physician responses to a variety of Web-extracted explanations of medical lab tests.

Electronic medical records are increasingly comprehensive, and this vast repository of information has already contri-buted to medical efficiency and hospital procedure. However, this information is not typically accessible to patients, who are frequently under-informed and unclear about their own hospital courses. In this paper, we propose a design for in-room, patient-centric information displays, based on iterative design with physicians. We use this as the basis for a Wizard-of-Oz study in an emergency department, to assess patient and provider responses to in-room information displays. 18 patients were presented with real-time information displays based on their medical records. Semi-structured interviews with patients, family members, and hospital staff reveal that subjective response to in-room displays was overwhelmingly positive, and through these interviews we elicited guidelines regarding specific information types, privacy, use cases, and information presentation techniques. We describe these findings, and we discuss the feasibility of a fully-automatic implementation of our design.

We describe fieldwork in which we studied hospital ICU physicians and their strategies and documentation aids for composing patient progress notes. We then present a clinical documentation prototype, activeNotes, that supports the creation of these notes, using techniques designed based on our fieldwork. ActiveNotes integrates automated, context-sensitive patient data retrieval, and user control of automated data updates and alerts via tagging, into the documentation process. We performed a qualitative study of activeNotes with 15 physicians at the hospital to explore the utility of our information retrieval and tagging techniques. The physicians indicated their desire to use tags for a number of purposes, some of them extensions to what we intended, and others new to us and unexplored in other systems of which we are aware. We discuss the physicians’ responses to our prototype and distill several of their proposed uses of tags: to assist in note content management, communication with other clinicians, and care delivery.

We present activeNotes, a prototype application that supports the creation of Critical Care Notes by physicians in a hospital intensive care unit. activeNotes integrates automated, context-sensitive patient data retrieval and user control of automated data updates and alerts into the note-creation process. In a user study at New York Presbyterian Hospital, we gathered qualitative feedback on the prototype from 15 physicians. The physicians found activeNotes to be valuable and said they would use it to create both formal notes for medical records and informal notes. One surprising finding is that while physicians have rejected template-based clinical documentation systems in the past, they expressed a desire to use activeNotes to create personalized, physician-specific note templates to be reused with a given patient, or for a given condition.

Many collaborative applications, especially in scientific research, focus only on the sharing of tools or the sharing of data. We seek to introduce an approach to scientific collaboration that is based on the sharing of knowledge. We do this by automatically building organizational memory and enabling knowledge sharing by observing what users do with a particular tool or set of tools in the domain, through the addition of activity and usage monitoring facilities to standalone applications. Once this knowledge has been gathered, we apply social networking models to provide collaborative features to users, such as suggestions on tools to use, and automatically-generated sequences of actions based on past usage amongst the members of a social network or the entire community. In this work, we investigate social networking models as an approach to scientific knowledge sharing, and present an implementation called genSpace, which is built as an extension to the geWorkbench platform for computational biologists. Last, we discuss the approach from the viewpoint of social software engineering.
