Vor dem Hintergrund der nach wie vor erschreckend hohen Mortalität beim akuten Atemnotssyndrom (ARDS) und auch bei der Idiopathischen Pulmonalen Fibrose (IPF), für deren Verlauf auch eine überschießende Aktivierung reaktiver Sauerstoffspezies und somit ein oxidativer Stress mitverantwortlich gemacht wurde, sind in der vorliegenden Arbeit einzelne Bestandteile der antioxidativen Abwehr und Abbauprodukte des oxidativen Stoffwechsels in bronchoalveolären Lavageflüssigkeiten (BALF) analysiert worden. Nach Etablierung und Validierung der Messverfahren wurden hierin Phospholipide, Proteine, Fettsäuren, oxidiertes und reduziertes Glutathion, Retinol, a-Tocopherol, Ascorbinsäure, Harnsäure, Malondialdehyd und Isoprostane bestimmt, und zwar zunächst von gesunden Probanden und Patienten mit ARDS oder Pneumonien (mit der Untergruppe PCP) sowie mit interstitiellen Lungenerkrankungen (IPF, Exogen Allergische Alveolitis und Sarkoidose), später auch von Kaninchen mit Bleomycin-induzierter Lungenfibrose. Es wurden hierbei sowohl Einzelpunktmessungen (z.B. bei chronisch interstitiellen Lungenerkrankungen) wie auch Verlaufsmessungen (z.B. bei akut-inflammatorischen ARDS- und Pneumonie-Patienten; Bleomycinmodell) durchgeführt. Es konnte gezeigt werden, dass unter verschiedenen pathologischen Bedingungen der antioxidative Status der Lunge zahlreichen Veränderungen unterliegt. Hervorzuheben ist hierbei vor allem, dass es unter erhöhtem oxidativem Stress erwartungsgemäß zu einem vermehrten Anfall von Abbauprodukten des oxidativen Stoffwechsels (GSSG, Isoprostane, Malondialdehyd) kommt. Außerdem konnte gezeigt werden, dass die Spiegel der einzelnen Antioxidantien in den meisten Fällen im Vergleich zu den gesunden Kontrollen unter vermehrtem Stress erstaunlicherweise erhöht sind, ohne dass hierdurch eine effektive Neutralisierung des oxidativen Stresses erzielt werden kann. So sind beispielsweise bei Pneumoniepatienten die Konzentrationen von a-Tocopherol, Retinol und Harnsäure hochsignifikant gesteigert. Ob dies eine adaptive Antwort des Organismus mit erhöhter lokaler Produktion von Antioxidantien oder vermehrtem Einstrom aus dem Blutkompartiment ist, bleibt jedoch noch unklar und bedarf weiterer Untersuchung. In view of the still alarmingly high mortality rate in acute respiratory distress syndrome (ARDS) and in idiopathic pulmonary fibrosis (IPF), for which course a overflowing activation of reactive oxygene species and thus oxidative stress has been considered jointly responsible, in the present thesis individual components of the antioxidative defense and by-products of oxidative metabolism in bronchoalveolar lavage fluid were examined. After measurement methods had been set up and validated, phospholipids, proteins, fatty acids, oxidised and reduced glutathione, retinol, a-tocopherol, ascorbic acid, uric acid, malondialdehyde and isoprostanes were measured. First, we analysed lavage fluid from healthy volunteers as well as patients with ARDS or pneumonia (with the subgroup PCP) and interstitial lung diseases (IPF, exogene allergic alveolitis and sarcoidosis), later we analysed rabbits with bleomycin-induced fibrosis. Isolated measurements (e.g. of chronic interstitial diseases) were carried out as well as measurements over the course of several days (e.g. for acute-inflammatoric ARDS-, and pneumonia patients; bleomycin-treated rabbits). It could be demonstrated that under different pathologic circumstances the antioxidative state of the lung changes in many ways. In this context it must be especially emphasized that – as expected - the amount of by-products (GSSG, malondialdehyde, isoprostanes) is higher, too. Furthermore, it could be shown that in most cases the levels of individual antioxidants under oxidative stress were surprisingly increased as compared to healthy controls without reaching an effective neutralisation of oxidative stress. In pneumonia patients for example concentrations of a-tocopherol, retinol and uric acid are highly significantly elevated. The question whether this is due to an adaptive answer of the organism with higher local production of antioxidants or an increased flood from the blood-compartiment hasn’t been solved yet and requires further research.
